Fênomenos de enfermagem identificados por  enfermeiros em um caso clínico: considerações à luz das classificações da NANDA-I, NOC e NIC e a CIPE® / Nursing phenomena identified by nurses in a clinical cases: considerations in the light of the NANDA-I, NOC and NIC and ICNP ®classifications

O estudo teve como objetivo comparar os diagnósticos, resultados esperados e prescrições de enfermagem elaborados por enfermeiros, em face de um caso clínico, à luz das classificações da NANDA-I, NOC, NIC e a CIPE®.Trata-se de uma pesquisa de delineamento descritivo, com aplicação de estudo de caso, com 24 enfermeiros de instituições públicas de ensino e de assistência hospitalar da região nordeste do país. Os diagnósticos obtidos no Grupo Acorrespondem a 20 declarações diagnósticas, totalizando 51 rótulos; destes, 54,9% de alta acurácia, 23,5% acurácia nula, 15,7% baixa acurácia e 5,9% acurácia moderada.No Grupo B, 30 afirmativas diagnósticas foram arroladas com 43 declarações; destas, 44,2% na categoria acurácianula, 39,5% com alta acurácia, 16,3% baixa acurácia e nenhuma afirmativa diagnóstica foi classificada na categoria moderada acurácia.Houve convergências entre a NANDA-I e CIPE®para os títulos de diagnósticos de alta acurácia relacionados aos sistemas respiratório, tegumentar e circulatório; As divergência encontradas foram nos títulos dos diagnósticos relacionados aos cuidados com pele, em face donúmero na CIPE® ser maiorpelas combinações dos eixos foco, julgamento e localização. O uso da NANDA-I parece facilitar aos enfermeiros a elaboração de diagnóstico por apresentar características definidoras e fatores relacionados. Nos resultados esperados elaborados pelos enfermeiros do Grupo A, para os diagnósticos de alta acurácia, verificou-se, após mapeamento e refinamento, que os 25 resultados estabelecidos foram correspondentes a 11 resultados sugeridos pela NOC, com predomínio em: Cicatrização de feridas: segunda intenção(1103) e Integridade tissular: pele e mucosas (1101).Para o Grupo B, os 13 resultados de enfermagem estabelecidos para os diagnósticos de alta precisão nem sempre utilizaram no enunciado o mesmo termo do eixo foco e termo do eixo julgamento adotados nas declarações diagnósticas. Em relação às prescrições de enfermagem os diagnósticos de alta acurácia obtiveram-se 101 prescrições no Grupo A, as quais foram correspondentes com as atividades da NIC em 21 intervenções de enfermagem, nos domínios: fisiológico complexo, comportamental e segurança e nas classes: Controle de Pele/ Feridas; Controle de Risco; Controle Respiratório; Controle da Perfusão Tissular; Educação do Paciente; e Apoio Nutricional. Os 54 cuidados prescritos no Grupo B foram mapeados e refinados em 28 intervenções de enfermagem da CIPE® versão 2.0; são elas: Observar a lesão (10013461); Promover a higiene(10032477); Prevenir a úlcera de pressão (10032431); Ensinar sobre os cuidados à ferida(10034961); Promover a higiene(10032477); Avaliar a cicatrização da ferida(10007218); e Cuidar do local da ferida (10004025). O estudo mostrou a aplicabilidade das taxonomias estudadas no cotidiano da prática clínica do enfermeiro e os pontos de convergências e divergências em face à mesma situação clínica. Sugere-se a educação continuada e permanente nos órgãos formadores e nas instituições de saúde para êxito no emprego das taxonomias e consolidação das etapas do processo de cuidar / The objective in this study was to compare the diagnoses, expected outcomes and nursing prescriptions elaborated by nurses in view of a clinical case, in the light of the classifications NANDA-I, NOC, NIC and ICNP®.A descriptive research with a case study design was undertaken, involving 24 nurses from public teaching and hospital care institutions in the Northeast of Brazil. The diagnoses obtained in Group A correspond to 20 diagnostic declarations, totaling 51 labels; 54.9% of these are of high accuracy, 23.5% zero accuracy, 15.7% low accuracy and 5.9% moderate accuracy. In Group B, 30 diagnostic assertions were listed with 43 declarations; 44.2% of these were classified in the zero accuracy category and 39.5% in the high accuracy category, while 16.3% were classified as low accuracy and none of the assertions as moderate accuracy. Convergences were found between NANDA-I andICNP® for the high-accuracy diagnostic titles related to the respiratory, integumentary and circulatory systems; the divergences found were related to the diagnostic titles for skin care, as the number of titles in the ICNP®was higher due to the combinations among the focus, judgment and location axes. The use of NANDA-I seems to make it easier for the nurses to elaborate diagnoses because of its defining characteristics and related factors. In the expected outcomes elaborated by the nurses from Group A, after mapping and refining, it was verified that, for the high- accuracy diagnoses, the 25 outcomes established corresponded to 11 outcomes suggested by NOC, predominantly in: Wound healing: second intention(1103) andTissue integrity: skin and mucosa (1101). For Group B, the 13 nursing outcomes established for the high-accuracy diagnoses did not always use the same terms as the focus and judgment axes adopted in the diagnostic declarations. As regards the nursing prescriptions, for the high-accuracy diagnoses, 101 prescriptions were obtained in Group A, which corresponded to the NIC activities in 21 nursing interventions, in the following domains: complex physiological, behavioral and safety and in the classes: Skin/Wound Control; Risk Control; Respiratory Control; Tissue Perfusion Control; Patient Education; and Nutritional Support. The 54 prescriptions in Group B were mapped and refined in 28 nursing interventions of ICNP® version 2.0; these are: Observe the lesion (10013461); Promote hygiene (10032477); Prevent pressure ulcer (10032431); Teach wound care (10034961); Assess wound healing (10007218); and Take care of the wound site (10004025). The study showed the applicability of the taxonomies studied in daily nursing practice and the points of convergence and divergence in view of the same clinical situation. Continuing education in teaching and health institutions is suggested with a view to the successful use of these taxonomies and the consolidation of the steps of the care process

Diagnósticos de Enfermagem em pacientes oncológicos com feridas / Diagnoses in cancer patients with wounds

Studies show that 5% to 10% of patients with metastatic cancer in the last six months of life develop wounds. The wounds in cancer patients, reflected in psychosocial, biological and spiritual implications. The systematization of nursing care through the nursing process makes nurses use clinical reasoning and critical judgment for identification and inventory of problems. The nursing diagnosis is the second stage of this process, and is focused on individual responses to the disease, health processes and/or factors affecting the maintenance of the individual balance. This research aimed to develop nursing diagnosis for the International Classification for Nursing Practice(ICNP®)in cancer patients with wounds.It was developed a descriptive, cross-sectional, exploratory research methodology and a quantitative approach, conducted from August to October 2015,in an oncology reference hospital,with the participation of 30 hospitalized patients,using the theoretical framework of the Basic Human Needs Theory from Wanda Horta and following the guidelines for the preparation of Nursing Diagnoses,proposed by ISO18104:2014 and the terms contained in the axles focus and judgment of ICNP® version 2015.The implementation of the research was divided into five phases: 1) survey of cancer patients with and without wounds; 2) application of nursing history and physical examination; 3) identification and evaluation of wounds; 4)identification of nursing phenomena; and 5)the preparation of statements of nursing diagnoses.The research was discussed and approved by the Ethics Committee of the Federal University of Maranhão,according to resolution no. 466/2012 of the National Health Council, under opinion Nº.1.161.269 and Nº.CAEE 44080115.3.0000.5087. After tab in Microsoft Excel ®2007 program, the data allowed representation in proportion and frequency distribution and the development of nursing diagnoses,according to the combination of terms of axles Focus and Judgment contained in ICNP®. We obtained the following results: the prevalence of wounds in male patients,aged 49-59 years old, brownskinned color, from the interior of the state, married, incomplete primary education, income up to 2 minimum wages, farmers, evangelical and on admission to hospital for pain, through the hospital emergency department; identification of 50 wounds being prevalent neoplastic, surgical and pressure ulcers; prevalence of breast and and penis cancer diagnoses and the preparation of 242 nursing diagnoses,188 belonging to the category of physiological needs, 49 of Psychosocial and 5 psychospiritual. It is considered that this study may contribute to the practice of oncology nursing, in order to sustain the scientific knowledge and the development of nursing care abilities to this clientele and to direct the nursing staff for a development of an individualized,systematic, comprehensive care and of universal language. / O câncer é uma doença genética cuja iniciação e progressão envolve uma série de lesões no DNA ou expressão anômala de genes normais, e ganha relevância como problema de saúde pública no Brasil, devido a incidência alta e crescente de mortalidade. Estudos apontam que de 5% a 10% dos pacientes com câncer metastático nos últimos seis meses de vida, evoluem com feridas. As feridas nos pacientes oncológicos, refletem em implicações psicossociais, biológicas e espirituais. A Sistematização da Assistência de Enfermagem, por meio do processo de enfermagem, faz com que o enfermeiro utilize o raciocínio clínico e o julgamento crítico para identificação e levantamento de problemas. O diagnóstico de enfermagem representa a segunda etapa desse processo, e tem como foco, as respostas do indivíduo à doença, aos processos de saúde e/ou aos fatores que afetam a manutenção do seu equilíbrio. Esta pesquisa teve por objetivo elaborar Diagnósticos de Enfermagem da Classificação Internacional para a Prática de Enfermagem (CIPE®) em pacientes oncológicos com feridas. A metodologia da pesquisa desenvolveu-se por um estudo descritivo, transversal, exploratório e de abordagem quantitativa, realizada de agosto a outubro de 2015, num hospital de referência em oncologia, com a participação de 30 pacientes hospitalizados, utilizando o referencial teórico das Necessidades Humanas Básicas de Wanda Horta e seguindo as orientações para elaboração de Diagnósticos de Enfermagem, propostas pela ISO 18104:2014, e os termos contidos nos eixos foco e julgamento, da CIPE® versão 2015. A execução da pesquisa deu-se por cinco etapas: 1) levantamento dos pacientes oncológicos com feridas; 2) aplicação do histórico de enfermagem; 3) identificação e avaliação das feridas; 4) identificação dos fenômenos de enfermagem; e 5) elaboração de enunciados diagnósticos de enfermagem. A pesquisa foi apreciada e aprovada pelo Comitê de Ética da Universidade Federal do Maranhão, segundo a Resolução nº 466/2012 do Conselho Nacional de Saúde, sob parecer nº 1.161.269 e CAEE nº 44080115.3.0000.5087. Os dados permitiram, após a tabulação no programa Microsoft Excel ®2007, a representação em proporção e distribuição de frequências, e a elaboração dos diagnósticos de enfermagem, segundo a combinação de termos dos eixos foco e julgamento contidos na CIPE®. Os resultados apontam: a prevalência de feridas em pacientes do sexo masculino, com idade de 49 a 59 anos, cor parda, procedentes do interior do estado, casados, ensino fundamental incompleto, renda de até 2 salários mínimos, lavradores, evangélicos e admitidos com queixa de dor pelo serviço de pronto atendimento do hospital; identificação de 50 feridas entre os 30 participantes, com prevalência das feridas neoplásicas, cirúrgicas e lesão por pressão e predomínio dos diagnósticos de câncer de mama e pênis, possibilitando a elaboração de 242 enunciados de diagnósticos de enfermagem, dos quais 188 na categoria das necessidades psicobiológicas, 49 psicossociais e 5 psicoespirituais. Considera-se, que o estudo aponta contribuições para o desenvolvimento científico da prática de enfermagem oncológica, orientando a equipe de enfermagem no cuidado individualizado, sistemático e integral desta clientela.

Sistematização da assistência de enfermagem: um estudo com auxiliares e técnicos de enfermagem / Nursing Assistance of Systhematization: a study with auxiliaries and nursing thecnicians

Trata-se de um estudo de natureza descritiva, com o objetivo de caracterizar o processo de trabalho dos auxiliares e técnicos de enfermagem quanto aos seguintes aspectos da Sistematização da Assistência de Enfermagem (SAE): informação sobre essa metodologia durante sua formação profissional; informação sobre a SAE no setor de trabalho; informação quanto à aprovação legal para participar da SAE; a percepção desses profissionais sobre a sua participação nas fases da SAE; a contribuição da SAE na organização do seu serviço, conhecer as atividades realizadas por estes profissionais vinculadas às fases da SAE e a freqüência das mesmas. A população foi composta de 70 auxiliares e 07 técnicos de enfermagem. Um questionário com perguntas estruturadas e uma escala do tipo Likert foram utilizados como intrumento de coleta de dados. Os principais resultados revelaram: a) 93,5% dos sujeitos reconhecem autilização dessa metodologia no setor onde trabalha. b) 74,0% receberam treinamento para seu uso, destes, 67,7% afirmaram ter sido após a formação profissional. c) 46,8% referiram saber quais são as fases da SAE, destes, 33,3% citaram a coleta de dados, 33,3% a avaliação, 25,0% o diagnóstico de enfermagem, 25,0% a implementação, 27,7% o planejamento de enfermagem, 52,7% responderam de forma incorreta. d) 67,5% consideram que participam da SAE. e) 75,0% não identificam em quais fases participam da SAE. f) 33,8% acreditam ter respaldo legal para participar, 3,9% que não tem autorização, 13,0% referiram não saber e 44,2% não responderam. g) 94,8% consideram que a SAE melhora a qualidade de sua assistência. Quanto às atividades desenvolvidas: a) 81,8% referem que fazem a admissão do paciente. b) 77,9% dos sujeitos solicitam informações sobre o problema de saúde atual; 70,0% sobre antecedentes pessoais; 66,2% sobre antecendentes alérgicos; 87,7% sobre o uso diário de medicamentos. b) quanto a coleta de dados, 93,5% observam a freqüência respiratória; 93,5% a presença de dispnéia; 92,2% a presença de tosse; 77,9% alterações na oxigenoterapia; 67,5% nunca realizam a ausculta pulmonar; 89,6% verificam a pressão arterial; 81,8% alterações na infusão venosa, 81,8% verificam freqüência de pulsos periféricos; 76,6% avaliam a presença de edema; 76,6% a perfusão periférica; 87,0% referem avaliar aterações no nivel de consciência e 81,8% a orientação no tempo e espaço; 98,7% verificam a temperatura; 90,9% observam a presença de lesões; 85,5% a freqüência e características da urina, 83,1% o tipo e aceitação da dieta; 80,5% o débito urinário; 76,6% a freqüência e características das fezes; 85,7% não auscultam o abdome. c) quanto ao diagnóstico de enfermagem, 85,7% referem nunca realizar esta atividade. d) quanto ao planejamento 87,0% referem nunca participar da prescrição; e) quanto a implementação: 97,4% executam a prescrição e 98,7% checam a prescrição; 98,8% informam alterações observadas durante o cuidado. e) quanto à alta hospitalar, 75,3% anotam condições de deambulação do paciente; 97,4% as condições de dreno. Concluimos que estes profissionais, embora sem saberem, têm uma importante participação nas fases do processo, executando atividades assistenciais previstas em lei, como admissão, observação, descrição e anotação de sinais e sintomas, execução do plano de cuidados e relatório de enfermagem. / It is a descriptive study, that has had as goal to characterize the nursing auxiliaries and nursing technicians working process related to the following aspects of the Nursing Assistance of Systhematization (NAS): information acquired during their professional formation; the use of the method at the working field; the knowledge of legal aspects and their perception on the participation at the phases of NAS; the contribution of it in the organization of their assistance service and the activities done by these professionals. The sample of population was composed by 70 nursing auxiliaries and 07 nursing technicians. A questionnaire with structured questions and a Likert scale were used as collecting data tool. The main results have revealed: a) 93,5% of the subjects recognize the use of this methodology at the working field; b) 74,0% received training to use it, and 67,7% of these have said that it happened after their professional formation; c) 46,8% have said that they knew what the phases of the NAS are, and 33,3% of them have mentioned the collecting data, 33,3% the test, 25,0% the nursing diagnostic, 25,0% the implementation, 27,7% the nursing planning, 52,7% have answered incorrectly and 5,5% haven?t answered; d) 67,5% considered that participate of NAS; e) 75,0% of the subjects can?t identify in which phases they participate of NAS; f) 33,8% of them believe to have a legal protection (approval from Coren ? Regional Nursing Council) in order to participate, 3,9% believe that they don?t have authorization, 13% have said that they don?t know and 44,2% of them haven?t answered the question; G) 94,8% have considered that NAS improves the quality of their nursing assistance. By this way we can conclude that these professionals, although without knowing that, have had an important participation at the phases of the process. Mainly at those phases that mirrors their attributions foreseen by law, as admission, observation, description and taking notes of signals and symptoms, execution of care plans and nursing report.

The objective of this study was to analyze the nursing diagnoses of patients in the immediate prostatectomy postoperative period. This is a cross-sectional and descriptive study, developed at the surgical-clinic of Onofre Lopes University Hospital, in the Natal City RN - Brazil. The sample was composed of 50 patients included by the criteria: have presented a diagnosis of a benign prostatic hyperplasia or a prostate cancer, have been subjected to a prostate surgery at the mentioned hospital, and have been in the immediate postoperative period at the moment of the data collection. The exclusion criteria were: haven t been in an appropriate physical and mental condition, have presented a brain vascular disease, a lung disease, an advanced liver disease, a heart disease or a extensive coronary artery disease. The data collection instruments were: the script of an interview and physical examination. The data collection period was between November 2010 and April 2011. The data were organized in two phases: the diagnostic process and the construction of the database. The project was approved by the Ethics Committee of the Federal University of Rio Grande do Norte The results showed that most patients came from the countryside, was living with partners, had an average of 67.78 years, was pensionerthose with low schooling, Catholic and often did not perform preventive examinations of prostatic disease. The patients showed an average of 9.48 nursing diagnoses, defining characteristics 21.70 and 20.72 related or risk factors per patient. We identified 30 nursing diagnoses, of which 7 were above the 75 percentile: Risk of falls, Impaired ambulation, Risk of infection, Self-care deficit bath / hygiene and dress up and Risk for deficient fluid volume. The top six nursing diagnoses were in all patients, and therefore could not apply any statistical test. The others ND were associated with their defining characteristics and related or risk factors. We conclude that the nursing diagnoses identified in this study contribute to the progress of the nursing care to the prostatectomized patients in post-surgery period, allowing the deployment of nursing actions for the effective resolution of identified problems / O estabelecimento dos diagn?sticos de enfermagem em pacientes prostatectomizados ? de extrema import?ncia, pois proporciona uma linguagem pr?pria da profiss?o, facilitando a comunica??o entre os profissionais e o paciente. Objetivou-se nesse estudo analisar os diagn?sticos de enfermagem presentes em pacientes no p?s-operat?rio imediato de prostatectomia. Estudo do tipo transversal, de car?ter descritivo, realizado na cl?nica cir?rgica do Hospital Universit?rio Onofre Lopes, localizado no munic?pio de Natal-RN. Os instrumentos de coleta de dados foram: o roteiro de entrevista e o de exame f?sico. O per?odo de coleta deu-se entre os per?odos de novembro de 2010 a abril de 2011. Os dados foram organizados em duas etapas: o processo diagn?stico e a constru??o do banco. O projeto foi aprovado pelo Comit? de ?tica da Universidade Federal do Rio Grande do Norte. Os resultados mostraram que a maioria dos homens era proveniente do interior do estado, vivia com companheiras, tinha uma m?dia de 67,78 anos, era aposentada, com baixa escolaridade, cat?lica e n?o realizava exames de preven??o da pr?stata. Os pacientes apresentaram uma m?dia de 9,48 diagn?sticos de enfermagem, 21,70 caracter?sticas definidoras e 20,72 fatores relacionados por paciente. Foram identificados 30 diagn?sticos de enfermagem, dos quais 7 encontravam-se acima do percentil 75, s?o eles: Risco de quedas, Deambula??o prejudicada, Risco de infec??o, D?ficit no autocuidado para banho, higiene ?ntima e vestir-se e Risco de volume de l?quidos deficiente. Sistematização da assistência de enfermagem: um estudo com auxiliares e técnicos de enfermagem / Nursing Assistance of Systhematization: a study with auxiliaries and nursing thecnicians

Trata-se de um estudo de natureza descritiva, com o objetivo de caracterizar o processo de trabalho dos auxiliares e técnicos de enfermagem quanto aos seguintes aspectos da Sistematização da Assistência de Enfermagem (SAE): informação sobre essa metodologia durante sua formação profissional; informação sobre a SAE no setor de trabalho; informação quanto à aprovação legal para participar da SAE; a percepção desses profissionais sobre a sua participação nas fases da SAE; a contribuição da SAE na organização do seu serviço, conhecer as atividades realizadas por estes profissionais vinculadas às fases da SAE e a freqüência das mesmas. A população foi composta de 70 auxiliares e 07 técnicos de enfermagem. Um questionário com perguntas estruturadas e uma escala do tipo Likert foram utilizados como intrumento de coleta de dados. Os principais resultados revelaram: a) 93,5% dos sujeitos reconhecem autilização dessa metodologia no setor onde trabalha. b) 74,0% receberam treinamento para seu uso, destes, 67,7% afirmaram ter sido após a formação profissional. c) 46,8% referiram saber quais são as fases da SAE, destes, 33,3% citaram a coleta de dados, 33,3% a avaliação, 25,0% o diagnóstico de enfermagem, 25,0% a implementação, 27,7% o planejamento de enfermagem, 52,7% responderam de forma incorreta. d) 67,5% consideram que participam da SAE. e) 75,0% não identificam em quais fases participam da SAE. f) 33,8% acreditam ter respaldo legal para participar, 3,9% que não tem autorização, 13,0% referiram não saber e 44,2% não responderam. g) 94,8% consideram que a SAE melhora a qualidade de sua assistência. Quanto às atividades desenvolvidas: a) 81,8% referem que fazem a admissão do paciente. b) 77,9% dos sujeitos solicitam informações sobre o problema de saúde atual; 70,0% sobre antecedentes pessoais; 66,2% sobre antecendentes alérgicos; 87,7% sobre o uso diário de medicamentos. b) quanto a coleta de dados, 93,5% observam a freqüência respiratória; 93,5% a presença de dispnéia; 92,2% a presença de tosse; 77,9% alterações na oxigenoterapia; 67,5% nunca realizam a ausculta pulmonar; 89,6% verificam a pressão arterial; 81,8% alterações na infusão venosa, 81,8% verificam freqüência de pulsos periféricos; 76,6% avaliam a presença de edema; 76,6% a perfusão periférica; 87,0% referem avaliar aterações no nivel de consciência e 81,8% a orientação no tempo e espaço; 98,7% verificam a temperatura; 90,9% observam a presença de lesões; 85,5% a freqüência e características da urina, 83,1% o tipo e aceitação da dieta; 80,5% o débito urinário; 76,6% a freqüência e características das fezes; 85,7% não auscultam o abdome. c) quanto ao diagnóstico de enfermagem, 85,7% referem nunca realizar esta atividade. d) quanto ao planejamento 87,0% referem nunca participar da prescrição; e) quanto a implementação: 97,4% executam a prescrição e 98,7% checam a prescrição; 98,8% informam alterações observadas durante o cuidado. e) quanto à alta hospitalar, 75,3% anotam condições de deambulação do paciente; 97,4% as condições de dreno. Concluimos que estes profissionais, embora sem saberem, têm uma importante participação nas fases do processo, executando atividades assistenciais previstas em lei, como admissão, observação, descrição e anotação de sinais e sintomas, execução do plano de cuidados e relatório de enfermagem. / It is a descriptive study, that has had as goal to characterize the nursing auxiliaries and nursing technicians working process related to the following aspects of the Nursing Assistance of Systhematization (NAS): information acquired during their professional formation; the use of the method at the working field; the knowledge of legal aspects and their perception on the participation at the phases of NAS; the contribution of it in the organization of their assistance service and the activities done by these professionals. The sample of population was composed by 70 nursing auxiliaries and 07 nursing technicians. A questionnaire with structured questions and a Likert scale were used as collecting data tool. Processo de enfermagem: AplicaÃÃo em Hospitais de Fortaleza. / NURSING PROCESS: APPLICATION IN HOSPITALS IN FORTALEZA-CE

O processo de enfermagem representa um modelo tecnolÃgico que possibilita ao enfermeiro identificar, compreender, descrever e explicar as necessidades humanas para determinar o plano de cuidados. Assim, teve-se como objetivo geral: analisar a utilizaÃÃo do processo de enfermagem no contexto hospitalar na cidade de Fortaleza-CE, Brasil. Estudo descritivo, transversal, quantitativo, realizado em 47 instituiÃÃes hospitalares de Fortaleza-CE, que atenderam aos critÃrios de inclusÃo. Participaram da pesquisa 47 diretores de enfermagem de instituiÃÃes hospitalares e 89 coordenadores de enfermagem de 114 unidades de internaÃÃo dos hospitais que possuÃam o processo de enfermagem implantado. A coleta de dados ocorreu de abril a agosto de 2012, a partir de entrevista semiestruturada realizada com os diretores de enfermagem e um questionÃrio entregue aos coordenadores das unidades. Os dados foram analisados de forma descritiva, fundamentados na literatura pertinente. O projeto foi aprovado pelo Comità de Ãtica em Pesquisa, conforme protocolo 190/11. Como resultados, constatou-se que a maioria (53%) das instituiÃÃes tinha o processo de enfermagem implantado, sendo que em 60% das instituiÃÃes o mesmo havia sido implantado hà menos de cinco anos. Quanto aos significados atribuÃdos ao processo de enfermagem para os diretores dos hospitais e os coordenadores das unidades, teve-se: instrumento de sistematizaÃÃo do cuidado, promove assistÃncia de qualidade e sistematiza o trabalho. As taxonomias padrÃo mais utilizadas foram: NANDA (82,4%), NIC (17,5%) e NOC (3,5%). As seguintes etapas do processo de enfermagem foram realizadas nas instituiÃÃes que possuÃam o mÃtodo: levantamento de dados (92%), diagnÃsticos de enfermagem (64%), planejamento (12%), implementaÃÃo do plano de cuidados (92%) e avaliaÃÃo de enfermagem (96%). Como fatores positivos para implantaÃÃo do processo de enfermagem teve-se: interesse por parte dos enfermeiros, disponibilidade do gestor de enfermagem, presenÃa de incentivos pelos gestores de enfermagem e apoio da instituiÃÃo. Os fatores que limitaram a implantaÃÃo do processo de enfermagem foram: dÃficit de funcionÃrios da enfermagem (40,9%), desinteresse de enfermeiros, tÃcnicos e/ou auxiliares de enfermagem (40,9%) e pouca disponibilidade de tempo da equipe (31,8%). Dentre as instituiÃÃes que nÃo possuÃam o processo, 68,2% tentou implantar o mÃtodo e isso ocorreu pelo menos uma vez. Os hospitais de Fortaleza estÃo evoluindo quanto à implantaÃÃo e implementaÃÃo do processo de enfermagem, tendo em vista que a maioria das instituiÃÃes possui o mÃtodo implantado, mesmo que de forma fragmentada. Assim, à necessÃria sensibilizaÃÃo de gestores de unidades quanto à necessidade de prestar assistÃncia de enfermagem fundamentada em mÃtodo cientÃfico, como o processo de enfermagem. / Objectives: to Analyze the use of the nursing process in hospitals in Fortaleza-CE. Methodological description: descriptive, cross-sectional, quantitative Study. Attended the 47 directors of nursing research hospitals and 89 coordinators of units. Data collection took place in April to August 2012. The data were analyzed descriptively. Project approved by CoÃtica (190/11 Protocol). Results: the majority (53%) of the institutions has the nursing process deployed, and in 60% of the institutions, it is deployed for less than five years. The most commonly used standard taxonomies were: NANDA (82.4%), NIC (17.5%) and NOC (3.5%). As positive factors for the implementation of the process have been: interest on the part of nurses, nursing Manager availability, presence of incentives by nursing managers and support of the institution. Conclusion: it is concluded that hospitals in Fortaleza are evolving as the deployment and implementation of the nursing process.

Porém, sua operacionalização sofreu muitas interferências que a tornaram lenta e difícil. O presente estudo teve como objetivo descrever a operacionalização do PE no CAISM/UNICAMP, desde sua implantação, até o momento atual, buscando identificar quais foram os eventos relevantes relacionados a mesma e resgatar junto às pessoas envolvidas, as dificuldades, as estratégias utilizadas e mudanças ocorridas a partir do processo vivenciado pelas pessoas envolvidas. Trata-se de estudo descritivo e exploratório. Foram realizadas entrevistas pessoais ou por meio da internet e também um questionário quando não foi possível o contato pessoal. Por meio de instrumento específico foi realizada análise documental. A amostra foi estabelecida por conveniência, utilizando-se o método da "bola-de-neve". Foram analisados 82 documentos, identificados 53 eventos relevantes e 27 enfermeiros foram entrevistados ou responderam ao questionário. Dentre os eventos relevantes, destacam-se a Integração Docente-Assistencial e o Programa de Educação Continuada, a evasão de enfermeiros, a extinção temporária do Programa de Educação Continuada e a alteração da jornada de trabalho dos enfermeiros contribuíram negativamente. A principal dificuldade inicial, na implantação do PE, foi a descrença, pelos próprios enfermeiros, no Processo (22,2%). Das dificuldades ocorridas desde a implantação, e que permanecem até o momento atual, a resistência, o desinteresse, a falta de envolvimento dos enfermeiros foram as principais. A falta de planejamento e de estabelecimento de prioridades relacionadas à assistência constituem as maiores dificuldades que surgiram no momento atual. A estratégia utilizada nessa operacionalização, considerada pelos entrevistados como mais adequada, foi o estudo de caso, que perde o seu valor quando ocorre falta de continuidade. A principal mudança ocorrida, percebida por eles, foi a adoção de partes ou totalidade do PE na assistência, com sua conseqüente melhoria. Concluiu-se que as dificuldades ainda existem e uma estratégia para vencê-la seria a retomada e manutenção dos estudos de casos. Para isso, sugere-se, ainda, que os processos de trabalho também sejam revistos e discutidos com a equipe de enfermagem / Abstract: The Center of Integral Attention of the Women Health (CAISM) from Campinas University (UNICAMP), is trying to incorporate to the regular assistance the process of nursing (PN) since the beginning of the activities in 1986, but this process suffered many interferences that had become it slow and difficult. This paper had the objective to describe how the introduction of the process of nursing in the CAISM/UNICAMP was made from the first days until the current moment, identifying all the important events in implantation; talking to the people involved, asking them about the difficulties the strategies and the changes through this time . This is a descriptive and exploratory study. Personal interviews and interviews by internet were made and a questionnaire was used when the personal contact was impossible. The documental analysis was made by a specific instrument. The sample was choosing by convenience, using the method of the snow ball. Eight two documents had been analyzed, fifty three important events were identified and twenty seven nurses were interviewed or answer the questionnaire. Among all the important events the integration professor- assistance and the program of Continue Education had prominence. Some events had a negative influence like the temporary extinguishing of the program of continued education, and the change of the shifts of nurses. Many nurses even abandon the institution because of this .The main difficult in the process of implantation of the PN was the incredulity of the own nurses with 22.2%. Among all the difficulties since the beginning until now, some still persist like: resistance, the disinterest and the lack of involvement of the nurses are the main ones. The lack of planning and establishments of priorities related to the assistance were the biggest difficulties that had appeared at the current moment. The most adequate strategy used in this work according to the interview was the study of the case that loses his value when the work is interrupt. The main change showed in the interview was the adoption of the PN and the improvement because of that. The conclusion is: there is a lot of problems to bypass and a strategy to do this is retake the study of the cases. Os objetivos específicos são: delinear o perfil profissional dos enfermeiros da Unidade de Internação do Hospital de Clínicas de Niterói (HCN) e analisar seu nível de conhecimento e experiência sobre a SAE; descrever o processo de implantação da SAE em unidade de internação de uma instituição de saúde do setor privado na visão do enfermeiro assistencial; e analisar o processo de implantação da SAE no que tange às possibilidades e aos obstáculos existentes na visão dos enfermeiros. Trata-se de um estudo de caso, com abordagem qualitativa, desenvolvido na Unidade de Internação do HCN. Uma avaliação crítica do trabalho de campo realizado nos permite afirmar a existência de um hiato entre o seu ensino teórico e a vivência dos enfermeiros no dia-a-dia das instituições de saúde; a dificuldade efetiva para a realização da sistematização da assistência de enfermagem, tendo em vista a necessidade de execução das etapas que envolvem o exercício dos enfermeiros como, por exemplo, a etapa do exame físico para levantamento dos diagnósticos de enfermagem; e a decisiva interferência das funções administrativas que incluem o planejamento, o recrutamento de pessoal, a direção e o controle no desenvolvimento do processo de enfermagem. Para se fazer efetivo o processo de implantação da SAE, como metodologia da assistência, em qualquer unidade de uma instituição de saúde, esse processo precisa ser precedido de um curso de capacitação, periodicamente renovado e buscar o aprimoramento das habilidades cognitivas e psicomotoras dos enfermeiros para associarem melhor a teoria e a prática, a ser proporcionado pelas instituições hospitalares empregadoras / The systemization of the nursing attendance provides an appropriate atmosphere for the nurses to apply their theoretical-scientific knowledge, characterizing his professional practice and collaborating with the definition of his role. During the academic life, we learned that the nursing process is an instrument that facilitates the nursing attendance rendered the patient and he gives subsidies for the nurse to order, address and evaluate his work. According to the Law no. 7.498, which deals with the regulation of the nursing exercise, it is the nurse's private activity the “planning, organization, coordination, execution and evaluation of the services of nursing attendance”, and they are those activities that compose the Process of Nursing, known today, as Systemization of the Attendance of Nursing (SAE). The general objective of this study is to analyze the process of implementation of the systemization of the nursing attendance in a unit of hospitalization of an institution of health of the private sector in the nurse's view. The specific objectives are: to delineate the nurses' profiles of the Unit of hospitalization of the Hospital of Clinics of Niterói and to analyze his knowledge level and experience on SAE; to describe the process of implementation of SAE in a unit of hospitalization of a health institution of the private sector from the nurse’s point of view; and to analyze the process of implementation of SAE with respect to the possibilities and to the existent obstacles in the nurses' perspective. It is a case study, with qualitative approach, developed in the Unit of hospitalization of HCN. A critical evaluation of the field work accomplished allows us to affirm the existence of a hiatus between theoretical teaching and the nurses' everyday work in the health institutions; the real difficulty for the accomplishment of the systemization of the nursing attendance, bearing in mind the need for the execution of the stages that involve the nurses' work, as for instance, the stage of the physical exam for rising of the nursing diagnoses and decisive interference of the administrative functions that include planning, personnel's recruitment, the direction and control of the development of the nursing process. In order to fulfill the process of implementation of SAE, as a methodology of attendance, in any unit of a hospitalization of health, that process needs to be preceded by a training course, periodically renewed and it also needs to look for the enhancement of the cognitive and psychomotor abilities of the nurses for them to better associate theory and practice, which will be provided by the employing hospital institutions

That is operationalized by a team work, articulated and complementary, enabling the care continuity. The Nursing Process first stage was inserted at the Pediatric Intensive Care Unit of Maranhão Federal University Teaching Hospital in 2013. In the last two years this service has been through a plenty of work staff alterations due to retirement, resignation and new workers hiring. This fact, associated to matters of operation and work process, made us question: what conceptions are revealed by the nurses about the Nursing Process and how they deal with the assistance history operationalization? The research had as objectives: comprehend the nurses’ conceptions about the children care Nursing Process in intensive care; assess the registers and characterize the Nursing History handling in this Intensive Care Unit. It was used the research mixed method combining quantitative and qualitative research with statistical and textual analysis. For data obtainment, it was used the participant observation and primary data collection with the 12 nurses, supported by a questionnaire with open questions analyzed with the Content Analysis methodological support. The research second stage was documental and retrospective using as documental support 72 hospitalized children Nursing Histories in the period between January and August, 2015. For the Nursing History analysis it was used a tool made specifically for this goal, from the items of each Human Basic Need on the same mode they are presented on the Nursing History of the research service context. For the analysis of the Nursing History filling it was used scores which vary from 1 to 189 points. It was classified as Terrible (1 to 66 points); Bad (67 to 100 points); Regular (101 to 145 points); Good or Great (145 or more points). On the qualitative approach two thematic categories were built (Nursing History Valorization and Difficulties on the Nursing History Handling) and seven subcategories (First Contact and Bond Mobilizing Opportunity and Interactive Process; Care Planning and Management Instrument; Individualized and Enlarged Care Strategy; Risks Security and Minimization Resource; Nursing Valorization Instrument; Relative to Working Process; Relative to Nursing History Structure). The qualitative results revealed the nurses assign bureaucratic characteristics to the Nursing History, despite of considering it contributes for the integral care, ample and resolutive on an ambivalent construct between what they think and do as a substantial number of the analyzed items were incomplete or blank. Regarding the Nursing History filling 47,2% were classified as regular, 23,7% bad, 22,2% great and 6,9% terrible. The results characterized the Nursing History handling by nurses of the pediatric intensive care from a subjective and objective perspective of the data analysis revealing it as ambivalent, ambiguous and disqualified in terms of its filling during the nurse care practices. / A Sistematização da Assistência de Enfermagem é a metodologia usada para planejar, executar e avaliar o cuidado, tendo como essência o cuidar e como ferramenta de organização do trabalho, o Processo de Enfermagem. Este é operacionalizado por um trabalho em equipe, articulado e complementar possibilitando a continuidade do cuidado. A primeira etapa do Processo de Enfermagem foi implantada na Unidade de Terapia Intensiva Pediátrica do Hospital Universitário da Universidade Federal do Maranhão em 2013. Nos últimos dois anos esse serviço alterou o quadro funcional por aposentadoria, demissões e admissões de novos funcionários. Esse fato, aliado a questões operacionais e do processo de trabalho, nos fez questionar: que concepções são reveladas pelos enfermeiros sobre o Processo de Enfermagem e como lidam com a operacionalização do histórico na assistência? A pesquisa teve como objetivos: compreender concepções dos enfermeiros sobre o Processo de Enfermagem no cuidado a crianças em terapia intensiva; avaliar os registros e caracterizar o manejo do histórico de enfermagem nessa Unidade de Terapia Intensiva. Utilizou-se o método misto de pesquisa combinando pesquisa quantitativa e qualitativa com análises estatísticas e textuais. Para obtenção dos dados, utilizou-se a observação participante e coleta dos dados primários com 12 enfermeiros apoiada por questionário com perguntas abertas analisados com apoio metodológico da Análise de Conteúdo. A segunda etapa da pesquisa foi documental e retrospectiva utilizando 72 Históricos de Enfermagem de crianças internadas no serviço de janeiro a agosto de 2015. Para análise, utilizou-se instrumento construído para esse fim, a partir dos itens de cada Necessidade Básica Humana da mesma maneira que estão apresentados no Histórico de Enfermagem do serviço contexto da pesquisa. Para análise do preenchimento, utilizou-se escores que variaram de 1 a 189 pontos. Foi classificado em Péssimo (1 a 66 pontos); Ruim (67 a 100 pontos); Regular (101 a 145 pontos); Bom ou Ótimo” (145 ou mais pontos). Na abordagem qualitativa foram construídas duas categorias temáticas (Valorização do Histórico de Enfermagem e Dificuldades para o Manejo do Histórico de Enfermagem) e sete subcategorias (Oportunidade de Primeiro Contato e Mobilizador de Vínculo e Processos Interativos; Instrumento para o Planejamento e Gestão de Cuidado; Estratégia para o Cuidado Individualizado e Ampliado; Recurso para Segurança e Minimização de Riscos; Instrumento de Valorização da Enfermagem; Relativas ao Processo de Trabalho; Relativas a Estrutura do Histórico de Enfermagem). Os resultados qualitativos revelaram que os enfermeiros atribuem ao Histórico de Enfermagem características burocráticas, apesar de considerarem que o mesmo contribui para o cuidado integral, amplo e resolutivo em um constructo ambivalente entre o que pensam e o que fazem, pois um número considerável dos itens desse analisados estavam incompletos ou em branco. Quanto ao preenchimento do Histórico de Enfermagem 47,2% foi classificado como regular, 23,7% ruim, 22,2% ótimo e 6,9% péssimo. Os resultados caracterizaram o manejo do Histórico de Enfermagem por enfermeiros da terapia intensiva pediátrica numa perspectiva subjetiva e objetiva da análise dos dados revelando-o como ambivalente, ambíguo e desqualificado no tocante à formalidade no preenchimento do mesmo nas práticas de cuidado do enfermeiro.
