O Índice Dental Estético (DAI) tem sido amplamente usado em estudos envolvendo crianças e adolescentes, mas raramente usado em estudos com adultos. O propósito do presente estudo transversal foi avaliar a confiabilidade do DAI em uma amostra com 120 pacientes adultos, incluindo 46 do sexo masculino (38,3%) e 74 do sexo feminino (61,7%), entre 19 e 78 anos de idade (média de 49,2). Quase todos os pacientes apresentaram-se com muitas perdas dentárias e sem tratamento ortodôntico prévio. A prevalência e a severidade da má oclusão, a necessidade de tratamento ortodôntico, a autopercepção do paciente em relação à necessidade deste tipo de tratamento e a indicação, pelo cirurgião-dentista, foram avaliadas. Além disso, as opiniões subjetivas de 33 ortodontistas em 32 pacientes, por meio de fotografias intra e extrabucais e modelos de estudo foram também analisadas. Teste estatístico Kappa foi usado para avaliar o DAI e as opiniões dos profissionais. Testes qui-quadrado e de Fischer foram usados para verificar a correlação entre sexo, autopercepção e indicação do cirurgião-dentista e o escore do DAI. O teste t-student foi usado para investigar a correlação entre a idade e a necessidade de tratamento ortodôntico. A média do escore DAI foi de 38,5. Quase metade da amostra apresentou má oclusão severa ou muito severa, a qual sugeriu grande necessidade de tratamento ortodôntico. A confiabilidade intra e interexaminador do escore DAI e dos componentes foi muito alta (k>0,9). Inconsistências foram observadas na avaliação dos especialistas (k<0,4). Não houve diferenças estatisticamente significantes (P>0.05) nos escores do DAI entre sexo, idade, autopercepção da necessidade de tratamento, entretanto diferenças significantes foram encontradas em relação às indicações pelo cirurgião-dentista. Os resultados do presente estudo sugerem que a população de adultos com perdas dentárias tem elevada severidade da má oclusão e alta necessidade de tratamento ortodôntico. / Dental Aesthetic Index (DAI) has been broadly used in studies comprising children and adolescent patients but rarely used in adult studies. The aim of the present cross-sectional study was to evaluate the reliability of the DAI index in a sample of 120 adult patients, including 46 male (38.3%) and 74 female (61.7%), aged 19-78 years (mean 49.2). Several patients presented extensive dental missing and no orthodontic treatment. Prevalence, severity of malocclusion, orthodontic treatment need, self-perceived and dentist referral, were assessed. Further, subjective opinions of 33 orthodontists about 32 patients, based upon intra- and extra-oral photographs and study models were evaluated. Kappa statistics was used to assess the DAI and professional opinions. Chi-square test and Fischer test were used to test the correlation between DAI score and sex, self-perceived and dentist referral. Student ttest was used to test the correlation between age and orthodontic treatment need. The mean DAI score was 38.5. Almost half of the sample presented severe or very severe malocclusion, which suggests a substantial treatment need. Intra-rater and inter-rater reliability for DAI score and components was very high (k>0.9). Inconsistencies were observed in the professional evaluation (k<0.4). There were no statistically significant differences (P>0.05) in DAI scores between sex, age, selfperceived need, but significant differences were found between dentist referrals. The findings in the present study suggest that adult dental missing population have high severe malocclusion and high orthodontic treatment need for treatment.

A amostra foi constituída por 60 pacientes voluntários, com idade superior a 18 anos, que iriam se submeter a tratamento ortodôntico na Pós-Graduação em Ortodontia da Faculdade de Ciências Médicas e da Saúde de Juiz de Fora. Foi feito inicialmente, o diagnóstico de DTM por meio do eixo I do Research Diagnostic Criteria (RDC) de todos os pacientes. Posteriormente, foram avaliadas variáveis da oclusão funcional (guia canina, protrusiva e discrepância entre RC e MIH), oclusão dentária (escore pelo índice Peer Assessment Rating (PAR) e avaliação do overjet, overbite, apinhamentos, mordida aberta e mordida cruzada) e discrepância esquelética (Classe I, II ou III). Todas as avaliações foram feitas por um único examinador. Os dados encontrados foram compilados e submetidos a análise estatística por meio do programa spss 13.0. a um nível de significância de 5%. Os pacientes com diagnóstico de DTM apresentaram maiores escores do índice PAR. Das variáveis individuais da oclusão dentária, o overjet e overbite acima de 4mm demonstraram ter associação com o desenvolvimento de DTM. Indivíduos Classe II e III esquelética demonstraram maior prevalência de DTM. Os resultados confirmam que alguns fatores oclusais devem ser considerados potenciais para o desenvolvimento das desordens temporomandibulares. No entanto, a amostra analisada é restrita, e algumas variáveis oclusais apresentaram baixa prevalência nos indivíduos, dificultando a possibilidade de correlação das mesmas com a presença de DTM. / The aim of this study was to investigate the association between temporomandibular disorders (TMD) and occlusal factors, which included the variables from functional, dental and skeletal occlusion. The sample consisted of 60 volunteer patients, aged upper 18 years, who were undergoing orthodontic treatment in Faculty of Medical and Health Sciences of Juiz de Fora. It was initially made, a diagnosis of DTM through the RDC axis I for all patients. Subsequently, were assessed the variables from functional occlusion (canine and protrusive guidance and discrepancy between RC and MIH), dental occlusion (score by PAR index and assessment of overjet, overbite, crowding, cross bite and open bite) and skeletal occlusion (Class I, II or III). All assessments were performed by a single examiner. Data were compiled and analyzed statistically using the program spss 13.0. at a level of significance of 5%. Patients diagnosed with TMD had higher index scores PAR. The variables of dental occlusion, overjet and overbite over 4mm, shown to have an association with the development of temporomandibular disorders. Individuals rated Class II and III demonstrated a higher prevalence of TMD. The results confirm that some occlusal factors can be considered a rich potential for the development of temporomandibular disorders. However, the analyzed sample is restricted, and some occlusal variables showed low prevalence in individuals, making difficult the possibility of a correlation with the presence of TMD.

Interceptive orthodontic treatment need among children attending dental clinics in the Tygerberg sub-district

>Magister Scientiae - MSc / Background In the Western-Cape province, orthodontic treatment provided to government patients is limited to tertiary hospitals such as the Tygerberg Oral Health Centre (TOHC). This results in growing waiting lists which place a huge strain on resources of the facility. Objectives To determine: 1. The prevalence of malocclusion among 7-to-10-year-old children examined at dental clinics within the Tygerberg sub-district. 2. The types of malocclusion they present with. 3. Which of these malocclusions can be treated with interceptive orthodontic treatment. Methods An analytical, descriptive, cross-sectional study was carried out and quantitative methods were used to achieve the aim and objectives. The study sample consisted of 100 children, 46 male and 54 female, from Bellville and Bishop Lavis dental clinics. Dental examinations were done by one examiner on children between the ages of 7 and 10 years. Dental problems were identified which, if left untreated, could result in the need for more complex orthodontic treatment at a later stage. All the necessary ethical clearance was obtained. Results The results showed that there is a very large interceptive orthodontic treatment need (79%) at these clinics. A large proportion (70%) presented with detrimental habits. Thumbsucking was more prevalent among girls than boys. Other problems like mouthbreathing, lip wedging, deep bite, asymmetrical mobility of primary teeth, rotated lateral incisors and crossbites were found. The biggest concern was that 71% of the children had early loss of primary teeth and only 5% of children received restorations, resulting in a high prevalence of unfavourable molar relationships. All of these should be addressed early, thereby preventing the development of complex orthodontic problems or skeletal discrepancies. iv Conclusion Interceptive and preventive orthodontic treatment should be regarded as a primary health centre service, as it may reduce the need for costly fixed orthodontic treatment. The study showed that urgent intervention is necessary from the Department of Health to address this issue.

Performance diária e condições psicossociais de adultos jovens submetidos a tratamento ortodôntico / Daily performance and psychosocial conditions of young adults subject to orthodontic treatment

Este trabalho aplicou uma metodologia adequada para correlacionar os benefícios do tratamento ortodôntico com as possíveis melhorias nas condições psicológicas, na performance diária do indivíduo e fortalecer ainda mais a associação com a melhora na qualidade de vida. O estudo avaliou o impacto do tratamento ortodôntico na performance diária e nas condições psicossociais do adulto jovem, com idade entre 18 e 30 anos. Foram aplicados os questionários específicos OIDP, PIDAQ, AC (IOTN) e o DAI, validados e testados quanto à confiabilidade e consistência, previamente ao trabalho, pelas análises estatísticas: o Alpha de Cronbach, a Análise Fatorial e a Regressão Logística, além das técnicas básicas de análise exploratória de dados como média, mediana, desvio padrão, frequência absoluta e relativa. A amostragem foi composta por dois grupos: grupo I (n=101): indivíduos que tiveram seu tratamento ortodôntico concluído nos últimos 24 meses; grupo II (n=55): indivíduos que procuraram por tratamento ortodôntico. No grupo tratado (grupo I) foi feita uma análise das fichas clínicas e de toda a documentação ortodôntica, focalizando a queixa principal que o levou a procurar o tratamento (queixa estética ou funcional) e aplicados os índices OIDP e PIDAQ. No grupo II foram realizados exames clínicos por um avaliador calibrado e aplicado os índices OIDP, PIDAQ, DAI e AC (IOTN). Resultados: O modelo foi estatisticamente significativo, com estatística de Wald igual a 30,4 e p-valor menor que 0,0001. O impacto no Grupo I foi POSITIVO, pois os valores dos impactos bucais na atividade diária foram menores, em comparação com o grupo II; a avaliação do PIDAQ apresentou valores significativos indicando que o grupo I apresentou melhores condições psicossociais em comparação com o grupo II. Conclusão: Concluímos que os indivíduos do grupo I apresentaram valores estatísticos significativos para o OIDP e para o PIDAQ, em relação do Grupo II, confirmando o impacto positivo do tratamento ortodôntico na melhoria das condições psicossociais, na performance diária e fortalecendo a associação com a qualidade de vida. / This study applied an appropriate methodology to correlate the benefits of orthodontic treatment with possible improvements in psychological conditions, in daily performance of the individual and validate the association with improved quality of life. The study evaluated the impact of orthodontic treatment on daily performance and psychosocial conditions in young adults, aged between 18 and 30 years. Specific questionnaires OIDP PIDAQ, AC (IOTN) and the DAI, validated and tested for reliability and consistency, previously to work, statistical analyzes were applied: Cronbachs alpha, the factor analysis and logistic regression, in addition to exploratory basic technical data analysis as mean, median, standard deviation, absolute and relative frequency. The sample comprised two groups: group I (n=101): individuals who had their orthodontic treatment completed within the last 24 months; group II (n=55): individuals who look for orthodontic treatment. The treated group (group I) was made an analysis of the clinical records and the whole orthodontic documentation, focusing on the main complaint that led him to seek treatment (aesthetic or functional complaints) and applied the OIDP and PIDAQ indices. In Group II Clinical examinations were performed by a calibrated evaluator and applied the OIDP, PIDAQ, DAI and AC (IOTN) index. Results: The model was statistically significant with Wald statistic equal to 30.4 and p-value less than 0.0001. The impact on the Group I was POSITIVE, because the values of oral impacts on daily activity were lower compared with group II; assessing PIDAQ known significant values indicating that group I had better psychosocial conditions compared with group II. Conclusion: We conclude that group I showed significant statistical values for the OIDP and PIDAQ, regarding Group II, confirming positive impact of orthodontic treatment in improving psychosocial conditions in daily performance and strengthening the association with quality of life.

Longitudinal evaluation of sleep-related breathing disorders in an orthodontic population

Introduction: Les troubles respiratoires du sommeil (TRS), qui représentent une préoccupation croissante pour la santé, ont des effets significatifs sur la santé, le comportement et la performance académique chez l’enfant. Les malformations craniofaciales, l’hypertrophie adéno-amygdalienne et l'obésité, représentent des facteurs de risque importants dans le développement de cette condition. Les symptômes des TRS ont été étudiés dans une étude prospective chez les enfants et adolescents durant leur traitement orthodontique dans un milieu universitaire. Cette étude a cherché à décrire la prévalence et les facteurs de risque principaux des TRS, ainsi que l'impact des différentes interventions orthodontiques sur les symptômes TRS. Matériel et méthodes: dans une étude cohorte prospective, un groupe de 168 sujets âgés de 12 à 21 ans ont été soumis, quatre ans après la prise de données initiale, à un examen craniofacial en plus d'être administré des questionnaires qui ont recueilli des données sur la situation socio-démographique, le bruxisme et les troubles d’ATM, le sommeil et le comportement diurne, et les facteurs neuropsychologiques. Résultats: l'indice de masse corporelle a été augmenté mais est demeurée dans la même catégorie aux deux moments de l'enquête. Il ya eu une augmentation du serrement des dents et des symptômes de l'ATM, une diminution de la taille des amygdales, et une augmentation de la somnolence diurne. La prévalence des TRS n'a pas changé entre l’étude initiale et l’étude de suivi. Aucune intervention orthodontique s'est avérée avoir un effet cliniquement significatif sur les voies aériennes supérieures. Conclusions: la prévalence des symptômes TRS était constante par rapport aux valeurs de base pour la population étudiée, mais a augmenté si rapportée à la population générale. Les traitements orthodontiques ne montrent aucun effet sur les TRS. Mots-clés : apnée du sommeil, craniofacial, prévalence, ronflement, traitement orthodontique, voies aériennes supérieures / Introduction: Sleep-disordered breathing (SDB), a growing health concern, has significant effects on a child’s health, behaviour, and scholastic performance. Craniofacial malformations, along with adenotonsillar hypertrophy and obesity, represent important risk factors in the development of this condition. SDB symptoms in children and adolescents followed for orthodontic treatment in a university setting have been investigated in this prospective study. The aims of this study were to describe the prevalence and main risk factors of SDB and the impact of different orthodontic interventions on the SDB symptoms. Materials and methods: in a prospective cohort study, four years following an initial evaluation, a group of 168 subjects aged 12-21 years underwent a craniofacial examination in addition to being administered self-completed questionnaires that collected information on socio-demographic and psychosocial factors, bruxism and temporo-mandibular joint (TMJ) disorders, sleep and daytime behaviour, and neuropsychological factors. Results: Body mass index (BMI) was slightly increased but remained in the same category at the two time points of investigation. There was an increase in tooth clenching and TMJ symptoms, a decrease in tonsils’ size, and an increase in daytime sleepiness. Prevalence of SDB did not change between baseline and follow-up studies. No orthodontic treatment intervention proved to have any clinically significant impact on the upper airway. Conclusions: SDB symptoms prevalence was constant when compared to the baseline values for the studied population, but increased if reported to the general population. In order to create space, orthodontic treatment often requires removal of teeth. The most common teeth removed for orthodontic treatment are the premolars. It has become popular to remove second premolars in certain cases where the soft tissue profile should not be altered. The Damon self-ligating orthodontic system is renowned for not requiring dental extractions in the majority of cases. The effect of extractions on the soft tissue profile of patients, in conjunction with using the Damon system, has therefore not been researched. It is important to understand the effect that orthodontic treatment and extractions can have on the soft tissue profile of patients. This effect can accurately be determined by making use of the soft tissue cephalometric analysis, developed by Dr Reed A. Holdaway in 1983.

The contribution of tools that enable three-dimensional analysis has been gradually incorporated into dental research. Objective: This study evaluated the crown inclination and angulation of patients in orthodontic treatment using digital models and cone beam computed tomography. Methodology: With the three-dimensional cephalometric software was created cephalometric analysis to evaluate the crown angulation and inclination in digital models of 26 patients, except second and third molars. For the results of crown angulation and inclination based on Camper and Frankfurt plans were required cone beam computed tomography images. Student's t- test was used to compare the intra and inter-group averages and Pearson's correlation coefficient used to evaluate the results between the groups Camper, Frankfurt and occlusal. Results: The analysis of the reproducibility of the method did not reveal a statistically significant difference. In assessing intra-group before and after orthodontic treatment the lower lateral incisors, the lower right first molar, the canine, and the first and second premolars presented statistically significant differences in crown inclinations. The upper right first molar and the lower left molar, the lower second premolar, the lower right first premolar, and the lower lateral incisors presented statistically significant differences in angulation. In the intergroup evaluation, post treatment and prescriptions, all of the results of the crown inclination and angulation presented statistically significant differences except the crown angulation of the second superior premolars and the first inferior molars. The study shows that between the Camper-occlusal, the Camper-Frankfurt and the Frankfurt-occlusal planes, the measures of the crown inclination for incisors and canine teeth did not present a correlation and other correlations were weak or inverse. The results of the crown angulation presented a strong correlation, except for central inferior incisors and upper lateral incisors, which demonstrated a moderate correlation to the Camper-occlusal and Frankfurt-occlusal planes. Conclusion: The crown angulation and inclination can be determined by means of digital models and it is evident that there is a need for additional bend to obtain crown angulation and inclination with values close to those recommended in preset brackets. The measures of the crown angulation and inclination presented correlations when evaluated by the Camper, Frankfurt and occlusal planes with the exception of the crown inclination of the incisors and canine teeth. / Introdução: A incorporação de novas alternativas e métodos aplicáveis à ortodontia para a análise da angulação e inclinação coronária constitui um verdadeiro desafio. A contribuição de ferramentas que permitem análises tridimensionais tem sido gradativamente incorporada à pesquisa odontológica. Objetivo: O presente estudo avaliou a inclinação e a angulação coronária em pacientes tratados ortodonticamente usando modelos digitais e tomografia computadorizada de feixe cônico. Metodologia: Com o módulo de cefalometria tridimensional do Software VistaDent 3D foi criado análise cefalométrica para avaliar a inclinação e angulação coronária em modelos digitais de 26 pacientes, exceto segundos e terceiros molares. Para obter os resultados de angulação e inclinação coronária com base nos planos de Camper e Frankfurt, foram necessárias documentações de pacientes com imagens tomográficas finais. Foi utilizado o teste t-Student para comparar as médias intra e intergrupo e o coeficiente de correlação de Pearson para avaliar os resultados entre os grupos Camper, Frankfurt e oclusal. Resultados: A análise da reprodutibilidade do método não revelou diferença estatisticamente significante. Quanto à avaliação intragrupo, antes e depois do tratamento ortodôntico, os incisivos laterais inferiores, primeiro molar inferior direito, os caninos, primeiros e segundos pré-molares apresentaram diferenças estatisticamente significantes para inclinações. Primeiro molar superior direito e inferior esquerdo, segundos pré-molares inferiores, primeiro premolar inferior direito e incisivos laterais inferiores apresentaram diferenças estatisticamente significantes para angulações. Quanto à avaliação intergrupo, depois do tratamento ortodontico e prescrições, todos os resultados de inclinação e angulação apresentaram diferenças estatisticamente significantes, exceto a angulação dos segundos pré-molares superiores e primeiros molares inferiores. O estudo aponta que entre os planos Camper-oclusal, Camper-Frankfurt e Frankfurt-oclusal as medidas de inclinação coronária para incisivos e caninos não apresentaram correlação, outros correlação fraca ou inversa. Os resultados da angulação coronária apresentaram forte correlação, exceto incisivos centrais inferiores e laterais superiores que demonstraram moderada correlação para Camper-oclusal e Frankfurt-oclusal. Conclusão: A angulação e inclinação coronária podem ser mensuradas por meio de modelos digitais e os resultados sugerem a necessidade de dobras adicionais no fio ortodôntico para obtenção de angulação e inclinação coronária com valores próximos aos preconizados em braquetes pré-ajustados. As medidas de angulação e inclinação coronárias apresentaram correlacionadas quando avaliadas pelos planos de Camper, Frankfurt e oclusal, exceto a inclinação dos incisivos e caninos.

Cette étude a cherché à décrire la prévalence et les facteurs de risque principaux des TRS, ainsi que l'impact des différentes interventions orthodontiques sur les symptômes TRS. Matériel et méthodes: dans une étude cohorte prospective, un groupe de 168 sujets âgés de 12 à 21 ans ont été soumis, quatre ans après la prise de données initiale, à un examen craniofacial en plus d'être administré des questionnaires qui ont recueilli des données sur la situation socio-démographique, le bruxisme et les troubles d’ATM, le sommeil et le comportement diurne, et les facteurs neuropsychologiques. Résultats: l'indice de masse corporelle a été augmenté mais est demeurée dans la même catégorie aux deux moments de l'enquête. Il ya eu une augmentation du serrement des dents et des symptômes de l'ATM, une diminution de la taille des amygdales, et une augmentation de la somnolence diurne. La prévalence des TRS n'a pas changé entre l’étude initiale et l’étude de suivi. Aucune intervention orthodontique s'est avérée avoir un effet cliniquement significatif sur les voies aériennes supérieures. Conclusions: la prévalence des symptômes TRS était constante par rapport aux valeurs de base pour la population étudiée, mais a augmenté si rapportée à la population générale. Les traitements orthodontiques ne montrent aucun effet sur les TRS. Mots-clés : apnée du sommeil, craniofacial, prévalence, ronflement, traitement orthodontique, voies aériennes supérieures / Introduction: Sleep-disordered breathing (SDB), a growing health concern, has significant effects on a child’s health, behaviour, and scholastic performance. Craniofacial malformations, along with adenotonsillar hypertrophy and obesity, represent important risk factors in the development of this condition. SDB symptoms in children and adolescents followed for orthodontic treatment in a university setting have been investigated in this prospective study. The aims of this study were to describe the prevalence and main risk factors of SDB and the impact of different orthodontic interventions on the SDB symptoms. Materials and methods: in a prospective cohort study, four years following an initial evaluation, a group of 168 subjects aged 12-21 years underwent a craniofacial examination in addition to being administered self-completed questionnaires that collected information on socio-demographic and psychosocial factors, bruxism and temporo-mandibular joint (TMJ) disorders, sleep and daytime behaviour, and neuropsychological factors. Results: Body mass index (BMI) was slightly increased but remained in the same category at the two time points of investigation. There was an increase in tooth clenching and TMJ symptoms, a decrease in tonsils’ size, and an increase in daytime sleepiness. Prevalence of SDB did not change between baseline and follow-up studies. No orthodontic treatment intervention proved to have any clinically significant impact on the upper airway. Conclusions: SDB symptoms prevalence was constant when compared to the baseline values for the studied population, but increased if reported to the general population. Foi realizado um estudo transversal cuja população foi constituída por 311 crianças de 12 anos de idade, matriculadas no Ensino Fundamental em escolas públicas de Juiz de Fora. A amostragem do estudo foi realizada por conglomerado, com sorteio proporcional das escolas participantes dentre as escolas públicas da cidade. Para a coleta dos dados, foram usados os seguintes instrumentos: os responsáveis responderam a um questionário sobre nível socioeconômico, acesso aos serviços de saúde do menor e, uma pergunta sobre a necessidade de tratamento ortodôntico da criança. Foi realizado, também, um exame clínico ortodôntico no escolar utilizando o Índice de Estética Dental (DAI) e aplicado o questionário relacionado à aparência de seus dentes (OASIS), o questionário do Índice de Necessidade de Tratamento Ortodôntico-Componente Estético (IOTN-AC) e uma pergunta sobre a percepção da criança em relação à necessidade de tratamento ortodôntico. A análise estatística foi realizada através do teste Qui-Quadrado e do Teste Exato de Fischer. Verificou-se alta prevalência (61,4%) de maloclusão e de necessidade de tratamento ortodôntico normativa em estudantes de 12 anos de idade na cidade de Juiz de Fora. Os aspectos psicossociais como, nível socioeconômico (p=0,142), acesso aos serviços de saúde bucal (p=0,090) e percepção da necessidade de tratamento ortodôntico pela criança (p=0,165), além do fator gênero (p=0,267), não foram associados estatisticamente com a maloclusão e a necessidade de tratamento ortodôntico normativa. Já a satisfação da criança com a aparência dos dentes (p=0,002) e a percepção da necessidade de tratamento ortodôntico pelo responsável (p<0,031) se associaram significativamente com a maloclusão e a necessidade de tratamento ortodôntico normativa. A percepção da necessidade de tratamento ortodôntico, tanto pela criança (94,2%) quanto pelo responsável (90,7%) foi maior que a necessidade de tratamento ortodôntico normativa (61,4%). A indicação da necessidade de tratamento ortodôntico realizada por critérios exclusivamente normativos, não abrange os aspectos subjetivos da maloclusão, uma vez que ignora as necessidades percebidas pelos indivíduos, tal como as implicações psicossociais das disfunções dentofaciais. / The objective of this study was to evaluate the prevalence of malocclusion and the need for orthodontic regulatory treatment for students of 12 years of public schools in the city of Juiz de Fora and verify the relationship of psychosocial factors as socioeconomic status, access to services oral health, children's satisfaction with appearance of teeth and perceived need for orthodontic treatment for the child and the responsibility, beyond gender factor, with malocclusion and the need for orthodontic treatment rules. A cross-sectional study was conducted whose population consisted of 311 children 12 years of age, enrolled in primary education in public schools in Juiz de Fora. The study sample was performed by conglomerate with proportional draw of the participating schools from public schools in the city. To collect the data, the following instruments were used: the guardians answered a questionnaire on socioeconomic status, less access to health services, and a question about the need for child's orthodontic treatment. Was also carried out an orthodontic clinical examination in the school using the Dental Aesthetic Index (DAI) and applied the questionnaire related to the appearance of your teeth (OASIS), the questionnaire Treatment Need Index Orthodontic-component Aesthetic (IOTN-AC) and a question about the child's perception of the need for orthodontic treatment. Statistical analysis was performed using the chi-square test and Fisher exact test. There was a high prevalence (61.4%) of malocclusion and need for orthodontic treatment rules for students 12 years of age in the city of Juiz de Fora. The psychosocial aspects as socioeconomic status (p=0.142), access to oral health services (p=0.090) and perceived need for orthodontic treatment for children (p=0.165), and the gender factor (p=0.267), not they were statistically associated with malocclusion and rules need for orthodontic treatment. Already the satisfaction of the child with the appearance of teeth (p=0.002) and the perceived need for orthodontic treatment by the head (p<0.031) were significantly associated with malocclusion and the need for orthodontic treatment rules. The perceived need for orthodontic treatment, both for the child (94.2%) and by the head (90.7%) was greater than the need for orthodontic treatment rules (61.4%). The indication of the need for orthodontic treatment performed by exclusively normative criteria does not cover the subjective aspects of malocclusion, since it ignores the needs perceived by individuals as the psychosocial implications of dentofacial disorders.

A dor pode aumentar o estresse psicológico, que relaciona-se com a alfa-amilase salivar. A intensidade da dor pode variar segundo a idade do paciente e sua motivação ao tratamento. O objetivo foi avaliar a intensidade de dor e sua relação com a motivação dos pacientes e com as concentrações salivares de sIgA e alfa-amilase, em adultos e crianças durante duas fases do tratamento ortodôntico. Vinte indivíduos (10 crianças e 10 adultos) responderam à um questionário de avaliação da motivação ao tratamento. Amostras de saliva foram coletadas e a intensidade de dor foi registrada diariamente, antes e após a colagem dos bráquetes e após a inserção do arco inicial. Apenas uma questão, relacionada à percepção da severidade da má oclusão, apresentou correlação com a intensidade de dor. Não houve diferença significante na intensidade de dor e na concentração sIgA entre adultos e crianças. De modo geral as crianças exibiram menor prevalência de dor, porém com maior intensidade. Houve uma tendência de correlação negativa entre a dor na mucosa bucal e a concentração de sIgA nas crianças, o que pode indicar a importância da sIgA na proteção da mucosa bucal. A concentração de alfa-amilase não teve correlação significante com a intensidade de dor, porém apresentou um aumento progressivo durante o período de avaliação, provavelmente devido ao estresse psicológico causado pela presença e ativação do aparelho fixo. / Pain is usually reported during orthodontic treatment. It comes from the strength applied to the teeth and traumatic lesions in the buccal mucosa, where secretory immunoglobulin A (sIgA) is the main protection and plays a role in integrity maintenance. Pain can increase the psychological stress, which induces changes in salivary alpha amylase. Pain intensity can vary according to patient’s age and his/her motivation towards treatment. The aim of this study was to evaluate pain intensity and its relation with patient’s motivation and salivary levels of slgA and alpha amylase, in adults and children, during two stages of orthodontic treatment. Twenty individuals (10 children and 10 adults) answered a questionnaire regarding their motivation towards treatment. Saliva samples were collected and pain intensity was evaluated on a daily basis, after and before the bonding of brackets and after the insertion of the initial arch. Only one question regarding the perception of malocclusion severity correlated with pain intensity. There was no significant difference in pain intensity and in slgA concentrations between adults and children. In general, pain prevalence in children was lower, yet reported pain was more intense. Pain in the buccal mucosa was negatively correlated with slgA concentrations in children, a finding that suggests a protective effect of slgA in the buccal mucosa. Although there was no significant correlation between the concentrations of alpha amylase and pain intensity, the levels of this enzyme increased during the evaluation period, probably due to the psychological stress caused by the presence and activation of the fixed braces.
