Estudo clínico e molecular em indivíduos com osteogênese imperfeita e análise do tratamento com bifosfonados

A Osteogênese Imperfeita (OI) é uma doença genética do tecido conjuntivo caracterizada por fragilidade óssea e susceptibilidade à fratura sob mínimo ou nenhum trauma. O objetivo deste trabalho foi estudar características clínicas e moleculares de crianças e adultos com Osteogênese Imperfeita e analisar o efeito do tratamento medicamentoso com bifosfonados em relação aos biomarcadores metabólicos e ósseos em pacientes adultos. Esta tese se dividiu em dois capítulos onde 1) foi realizado um estudo retrospectivo sobre as características clínicas no momento do diagnóstico de OI, com ênfase nas características clínicas, especialmente em relação às fraturas ósseas; 2) avaliação clínica e análise da mutação c.-14C>T no gene IFITM5 foi estuda em uma população com características sugestivas de OI tipo V; e 3) estudo retrospectivo em adultos com OI divididos em 2 grupos tratados com bifosfonados e não tratados. Em relação ao tratamento com bifosfonados foram avaliados os seguintes parâmetros: tipo de droga e duração do tratamento, valores de biomarcadores metabólicos e ósseos por um período de 5 anos, incidência de fraturas num de período de 5 ou 10 anos e densidade mineral óssea da coluna lombar, quadril total e colo femural no início e no final do tratamento. Nossos resultados mostraram que 1) no momento do diagnósico de OI características como escleras azuladas, dentinogênese imperfeita, ossos wormianos e fraturas de membros inferiores e superiores podem ser observadas. Pacientes com formas mais graves de OI foram diagnosticados mais precocemente quando comparados com pacientes com formas leves. Nenhuma criança com OI apresentou fraturas posteromediais das costelas, fratura de escápula ou lesões metafisárias. Essas informações associadas a história da saúde da criança são relevantes para a realização do diagnóstico diferencial. 2) OI tipo V correspondeu a 4% dos casos de OI atendidos no Centro de Referência para OI do HCPA. Indivíduos com OI V associada a mutação c.-14C> T no gene IFITM5 apresentaram características clínicas distintas como formação de calo hiperplásico, calcificação das membranas interósseas, deslocamento da cabeça radial e deformidade de coluna, porém a expressão da doença é variável. 3) Observamos que o tratamento de adultos com OI a longo prazo não foi associado com redução na incidência das fraturas e não se refletiu de forma significativa nos níveis de biomarcadores metabólicos e ósseos, porém houve uma melhora significativa na densidade mineral óssea da coluna lombar associada à terapia. Por ser uma doença rara com prevalência variável e ampla variabilidade fenotípica e genotípica, estudos clínicos e moleculares bem como estudos sobre o efeito do tratamento medicamentoso são imprescindíveis, contribuindo no melhor entendimento da doença, aconselhamento genético acurado e propiciando melhores estratégias de prevenção e tratamento para esta população. / Osteogenesis Imperfecta (OI) is a genetic connective tissue disease characterized by bone fragility and susceptibility to fracture under minimal or no trauma. The aim of this study was to evaluate clinical and molecular features of children and adults with OI and analyze the effect of the drug treatment with bisphosphonates in regarding to metabolic and bone biomarkers in adult patients. This thesis was divided by two chapters: 1) a retrospective study was performed where the clinical characteristic at the moment of diagnosis of OI, the clinical characteristics specially related to bone fractures was evaluated; 2) clinical evaluation and mutation analysis of c.-14C>T in the IFITM5gene was studied in a population with clinical charcteristics suggestive of OI type V; and 3) retrospective study in adults with OI divided in two groups treated with biphosphonates and not treated. Bisphosphonate treatment was evaluated according to the parameters: type of drug and duration of treatment, metabolic and bone biomarkers values for a period of 5 years, incidence of fractures in a period of 5 or 10 years and bone mineral density of the lumbar spine, total hip and femoral neck at baseline and at the end of treatment. Our results showed that 1) at the time of OI diagnosis features such as bluish slerae, dentinogenis imperfecta, wormian bones, and fractures of upper and lower limbs can be observed. Patients with more severe forms of OI were diagnosed earlier when compared with patients with mild forms. No OI children presented posteromedial fractures of the ribs, scapula fracture or metaphyseal lesions. This information associated with the child's health history are relevant for carrying out the differential diagnosis. This information is relevant for carrying out the differential diagnosis. 2) OI type V corresponds to 4% of OI cases at the Reference Center for OI at HCPA. Subjects with OI V associated to the mutation c.-14C> T in the IFITM5 gene presented distinctives clinical features as hyperplastic callus formation, calcification of interosseous membranes, dislocation of the radial head and spinal deformity, but the expression of the disease is variable. 3) We observed that long-term treatment with bisphosphonates (BP) for adults with OI was not associated with reduced incidence of fractures and was not reflected significantly in the levels of metabolic and bone biomarkers, but there was a significant improvement in bone mineral density of the lumbar spine associated to the therapy. Because it is a rare disease with a prevalence variable and wide phenotypic and genotypic variability, clinical and molecular studies and studies of the effect of drug treatment are essential, contributing to the better understanding of the disease, accurate genetic counseling and providing better strategies for prevention and treatment for this population.

Análise de terpenóides de espécies de Croton sect. Lamprocroton (Mull. Arg.) Pax (Euphorbiaceae) / Terpenoid analysis of species of Croton sect Lamprocroton (Mull. Arg.) Pax. (Euphorbiaceae)

Croton é um gênero gigante de Euphorbiaceae com cerca de 1.300 espécies distribuídas em regiões tropicais e subtropicais da América, África, Ásia e Austrália. O Brasil é um importante centro de diversificação da espécie, com mais de 350 espécies descritas, sendo muitas endêmicas. Muitas espécies são utilizadas como plantas medicinais pelas populações locais, para o tratamento de diversos males, como câncer, diabetes, febre, hipercolesterolemia, hipertensão, entre outros. Mesmo assim, a maioria das espécies não apresenta estudo fitoquímico para determinação de atividades biológicas. Na bibliografia consultada terpenóides se apresentaram como compostos predominantes em Croton. O presente estudo foi realizado com a análise de óleos voláteis e de extratos metanólicos de folhas e caules de 10 amostras de Croton seção Lamprocroton Todas as espécies do estudo têm descrição química inédita. Foram identificados 44 compostos de óleos voláteis por CG/EM, sendo 14 monoterpenos e 30 sesquiterpenos. Alguns compostos oxigenados, importantes do ponto de vista de atividade biológica, ocorreram em altas concentrações, como 1,8-cineol (C. ericoides: 24,1%; C.linearifolius: 26,9%; C. muellerianus: 23,9%), linalol (C. dusenii: 18,2%), bisabolol (C. ceanothifolius: 49,9%), 1-isopropil-7-metil-4-metileno-1,3,4,6,8-hexa-hidro-2H-naftalen-4-ol (C. linearifolius: 25,2%; C. pallidulus var. pallidulus: 23,6%). Já no extrato metanólico foram identificados por CG/EM 15 diterpenos, 9 triterpenos e 13 esteróides. Grupo de metabólito secundário mais caracteristico de Croton, foram detectados 3 diterpenos de cadeia aberta (fitol, hexadecatetraenol e furanona), um diterpeno alcoólico (retinol), além de diterpenos com esqueleto tipo labdano, caurano, clerodano e podocarpano. Os dois clerodanos foram identificados com estrutura similar à trans-desidrocrotonina, composto com alto potencial farmacológico. Os podocarpanos até então registrados em apenas duas espécies de Croton foram comuns à Croton seção Lamprocroton, sendo identificados podocarp-7-en-3-ona 13β-metil-13-vinil, podocarp-7,8-diien-3-ona-13-acetoxi, ácido podocarpa-7,13-dien-15-óico e metil-13(2-metoxi-2-oxoetildeno)-14-metil-7-oxopodocarpan-15-oato, distribuídos em 5 espécies. Os triterpenos e esteróides apresentaram alta diversidade e foram detectados para todas as espécies do estudo, com especial atenção para os triterpenos α-amirina, lupeol e lupenona; e os esteróides β e γ-sitosterol, pelas altas concentrações e pelo potencial farmacológico. Os dados obtidos possibilitaram listagem de compostos e atividades com sugestões de direcionamento para futuros testes e desenvolvimentos farmacológicos. / Croton is a large genus of Euphorbiaceae comprising around 1.300 species, widespread in tropical and subtropical regions of America, Africa, Asia and Australia. Brazil is one of the main hot spot in the world, with more than 350 species described, many of them endemic. Although several species are used in tradicional medicine for the treatement of diseases like cancer, diabetes, fever, high cholesterol and high blood pressure. Most species have no phytochemical studies concerning their biological activities. Literature shows terpenoids as predominant secondary metabolite constituents in Croton. The present study was carried outwith the analysis of essential oils and crude methanolic extracts obtained from leaves and bark of 10 samples of Croton sect. Lamprocroton. All species used on this study were submitted to their first chemical description. Forty four compounds were identified on essential oil by GC/MS, 14 monoterpenes and 30 sesquiterpenes. Some oxygenated compounds with important biological activity were detectaed in high concentrations: 1,8-cineol (C. ericoides: 24.1%; C.linearifolius: 26.9%; C. muellerianus: 23.9%), linalol (C. dusenii: 18.2%), bisabolol (C. ceanothifolius: 49.9%), 1-isopropil-7-metil-4-metileno-1,3,4,6,8-hexa-hidro-2H-naftalen-4-ol (C. linearifolius: 25.2%; C. pallidulus var. pallidulus: 23.6%). The metanolic extract analysis reveled by GC/MS15 diterpenes, 9 triterpenes e 13 steroids. Diterpenes are describe as the main secondary metabolite in Croton. Besides diterpenes with labdan, cauran, clerodan and podocarpan skeletal types, three opened ring diterpenes (fitol, hexadecatetraenol e furanona) and one alcoholic diterpene (retinol) were detected. Two clerodans were identified with trans-desidrocrotonin estructure-like, a compound with high pharmacological use. Podocarpans that have only been registered for two Croton species, were commonly found on Croton sect. Lamprocroton (podocarp-7-en-3-one 13β-metil-13-vinil; podocarp-7,8-diien-3-one-13-acetoxi; podocarpa-7,13-dien-15-oic acid; and metil-13(2-metoxi-2-oxoetildeno)-14-metil-7-oxopodocarpan-15-oate). Triterpenes and steroids shown high diversity and were detected at all studied species, with a highlight for compounds with high pharmacological potencial like triterpens α-amirine, lupeol and lupenone; and the steroids beta and gamma-sitosterol. The results obtained led to a list of compounds and suggestions for future pharmacological tests and developments.

Qui sont les skinheads ? S’agit-il seulement de ces brutes stupides et violentes, xénophobes et racistes sisouvent exposées par les médias ? Qu’en est-il alors de ces premiers skinheads qui, à la fin des années 1960,affectionnaient tant la musique jamaïcaine qu’ils contribueront à sa diffusion et à son succès à travers toutel’Angleterre ? Que s’est-il passé entre ces années 1960 et les années 1980 qui verront l’apparition du phénomènenaziskin ? Il y a t-il une unité du mouvement skinhead ou seulement une juxtaposition de mouvances ?Qu’est-ce qui peut unir les skinheads entre eux, à travers les générations et au-delà de leurs divisions ?Afin d’apporter des réponses à ces questions, le présent travail présentera, dans une première partie, unhistorique du mouvement skinhead qui permettra de suivre et de comprendre son évolution depuis ses originesen faisant ressortir le rôle important que jouera la musique dans toutes ses mutations successives etdans sa dérive politique. Suivra ensuite, dans une seconde partie, une analyse des éléments constitutifs dela subculture skinhead – à commencer par sa constante référence à la classe ouvrière exhibée au travers deses formes stylistiques, son rapport à la violence symbolique ou réelle, les valeurs qu’elle prétend incarneret défendre – et ses incidences idéologiques. La dernière partie sera consacrée à un travail de terrain menéprincipalement en France qui fera apparaitre, au moyen d’entretiens et d’observations, comment se vit,s’organise et se représente ce milieu skinhead.Il s’agira donc de révéler le mouvement skinhead sous ses différents aspects mais aussi de montrer ce quepeut révéler ce mouvement de mode devenu phénomène social quant aux processus de radicalisation, demédiatisation et de diabolisation. / Who are the skinheads ? Are they just these brainless, violent, xenophobic and racist bullies, who are sooften exposed in the media ? What about the very first skinheads who, near the end of the 1960s, were sofond of Jamaican music that they contributed to its spreading and success all over England ? What happenedbetween the 1960s and the 1980s, when the nazi skinhead phenomenon appeared ? Is there only one skinheadmovement or is it made up of many trends ? What makes the skinheads a group as a unified whole, beyondits divisions ?To answer these questions, the first part of this thesis will provide a background history of the skinheadmovement so as to follow, and understand, its evolution since its origins. It will emphasise on the role playedby music on both its successive mutations and its political drift. In a second part, it will analysed the variouselements which constitute the skinhead subculture - from its constant reference to the working-class displayedthrough its stylistic forms, its link to symbolic or actual violence, and the values it claims to embody andfight for – to its ideological consequences. The last part will be dedicated to fieldwork carried out mainlyin France and which will show, thanks to interviews and observations, how the skinhead community lives,organises and sees itself.The purpose of this study is not only to reveal the skinhead movement in its various forms but also todemonstrate what this stylistic social phenomenon, can reveal in terms of radicalisation, media coverageand demonization.

Data Assimilation for Management of Industrial Groundwater Contamination at a Regional Scale

Groundwater is one of the main sources for drinking water and agricultural activities. Various activities of both humans and nature may lead to groundwater pollution. Very often, pollution, or contamination, of groundwater goes undetected for long periods of time until it begins to affect human health and/or the environment. Cleanup technologies used to remediate pollution can be costly and remediation processes are often protracted. A more practical and feasible way to manage groundwater contamination is to monitor and predict contamination and act as soon as there is risk to the population and the environment. Predicting groundwater contamination requires advanced numerical models of groundwater flow and solute transport. Such numerical modeling is increasingly becoming a reference criterion for water resources assessment and environmental protection. Subsurface numerical models are, however, subject to many sources of uncertainties from unknown parameters and approximate dynamics. This dissertation considers the sequential data assimilation approach and tackles the groundwater contamination problem at the port of Rotterdam in the Netherlands. Industrial concentration data are used to monitor and predict the fate of organic contaminants using a three dimensional coupled flow and reactive transport model. We propose a number of 5 novel assimilation techniques that address different challenges, including prohibitive computational burden, the nonlinearity and coupling of the subsurface dynamics, and the structural and parametric uncertainties. We also investigate the problem of optimal observational designs to optimize the location and the number of wells. The proposed new methods are based on the ensemble Kalman Filter (EnKF), which provides an efficient numerical solution to the Bayesian filtering problem. The dissertation first investigates in depth the popular joint and dual filtering formulations of the state-parameters estimation problem. New methodologies, algorithmically similar, but more efficient numerically, are then proposed based on a more consistent derivation with the Bayesian filtering approach. To reduce computational cost, I further extend the formulation of the hybrid EnKF-variational approach to the state parameter estimation problem and propose an adaptive scheme for the specification of the weights of the flow-dependent and static background covariance matrices. The new adaptive hybrid scheme is shown to provide much better results than the EnKF while using a fraction of the ensemble size. The new methods are implemented and successfully tested with a realistic coupled subsurface and transport-reaction model of the port of Rotterdam by assimilating industrial data on biodegradable chlorinated hydrocarbons. The observational design problem for placing hydrologic wells is subsequently considered and a new efficient solution is proposed that combines concepts from both information theory and data assimilation

Female Board Representation and Coupled Open Innovation: Evidence from Emerging Market Multinational Enterprises

Yes / Little research has been done on female board representation in emerging market multinational enterprises (EMNEs). Our paper considers the role of female board representation and its impact on open innovation (OI) in the unique context of emerging markets. We draw on upper echelons and institutional theories to understand how female board representation and cross-country institutional contexts influence coupled OI. Combining a 10-year (2009-2019) dataset with a rich in-depth content analysis of 183 (EMNEs) engaged in OI, our results reveal a significant positive association between female board representation and a firm’s commitment to coupled OI initiatives. We also find that country-level institutional factors affect and positively moderate the relationship between female board representation and coupled OI. In emerging market environments where managerial perception and cultural beliefs sometimes hinder the promotion of females into top positions, our work has implications for EMNEs regarding how they harness diversity. We contribute to the OI literature by showing that female board representation enhances corporate OI investment within EMNEs.

With the objective of a complete description of the problem, there is the need to preliminarily study the oil industry, which was done transpiring succinctly, from the birth of the oil industry to the current stage of it, featuring and highlighting its most important aspects for the economy. Then it approaches the question of asymmetric information present in the fuel retail markets, and finally through the construction of econometric models that take into account assumptions formalized in works related to the problem of adulteration of fuel, then the evidence to economic determinants. / Este trabalho contribui para um melhor entendimento do fenÃmeno da adulteraÃÃo de combustÃveis, visando à identificaÃÃo de possÃveis determinantes econÃmicos, relacionados a tal prÃtica. Com relação à segurança da qualidade da água de consumo humano, um dos desafios é a remoção de microcontaminantes e patógenos emergentes e os processos de separação por membranas (PSM) têm se mostrado como uma opção promissora. As membranas de ultrafiltração (UF) são empregadas na remoção de material em suspensão e patógenos, e as de nanofiltração (NF) e osmose inversa (OI) são utilizadas na remoção de substâncias dissolvidas, entre elas o bisfenol-A (BFA). No presente trabalho foi avaliado, em escala piloto, o tratamento de água por três tipos de PSM, considerando o desempenho operacional, a rejeição de BFA e de outros contaminantes físico-químicos e microbiológicos. Foram utilizadas quatro membranas distintas, sendo: uma de UF (GS-5101U-S4); duas de NF (NF270 e NF90); e uma de OI (TW30). A UF foi empregada no tratamento da água do lago Paranoá e o seu permeado utilizado para alimentar a OI, enquanto as membranas de NF foram alimentadas com o efluente dos filtros ETA Brasília (DF). Tanto no caso da OI, como das NF, a água de alimentação foi enriquecida com bisfenol-A. A UF apresentou elevados valores de rejeição de turbidez (92%), coliformes (100%) e clorofila-a (97%) para valores iniciais de, respectivamente, 5,6UT, >24.196NNP/100mL e 3,9μg/L. Nas condições definidas como padrão o fluxo de permeado (Jp) médio da UF foi de 62L/(h.m2), a pressão transmembrana (PTM) média foi de 37,8kPa e a recuperação média foi de 96,1%. O aumento do tempo entre limpeza, do Jp e a adoção de etapa de coagulação influenciaram negativamente o desempenho operacional da UF. Entre as membranas de alta pressão estudadas, a NF90 apresentou melhor desempenho operacional, com Jp médio de 39,4L/(h.m2) e recuperação média de 22%. A membrana NF270 apresentou Jp médio de 8,7L/(h.m2) e recuperação média de cerca de 5%, e no caso da membrana de OI o Jp médio foi de 17,2L/(h.m2) e a recuperação média 8%. A rejeição de BFA pelas membranas TW30 e NF90 foi praticamente 100%, enquanto, a membrana NF270 apresentou rejeição entre 43 e 54%. Por outro lado, a rejeição de BFA pela membrana de UF foi praticamente inexistente. _______________________________________________________________________________________________ ABSTRACT / Water quality and availability in urban areas is a matter of growing concern and claim for preventive e corrective solutions. Regarding the safety of drinking water, one of the challenges is the removal of emerging micropollutants and pathogens and membrane separation processes (MSP) are promising technologies. Ultrafiltration membranes (UF) are mainly applied for the removal of suspended solids and pathogens, while nanofiltration (NF) and reverse osmosis (RO) are recommended for the retention of dissolved substances, bisphenol-A (BPA) amongst them. In this study it was evaluated, in pilot scale, the capabilities of three types of MSP in treating water, considering their operational performance, BPA rejection, as well as the rejection of other physicochemical and microbiological contaminants. Four membranes were evaluated: one UF membrane (GS-5101U-S4), two NF membranes (NF270 and NF90) and one RO membrane (TW30). The UF membrane was fed with Paranoá lake water, and the UF permeate was used to feed the pilot RO system. The filtrated water from a local water treatment plant (ETA Brasilia, DF) was used as raw water for the NF membranes pilot plant. The feed water of both RO and NF membranes was spiked with bisphenol-A. High rejection of turbidity (92%), chlorophyll-a (97%) and coliforms (100%) was obtained when UF was evaluated, for initial values of, respectively, 5.6NTU, >24,196NNP/100mL e 3.9μg/L. For the standard operation conditions, UF permeate flux was 62 L/(h.m2), transmembrane pressure (TMP) was 37.8kPa and average recovery was 96%. The adoption of chemical coagulation stepprior to UF and the increase of cleaning intervals or permeate fluxes, resulted in adverse influence on UF operational performance. The NF90 membrane showed the best overall performance amongst the high pressure membranes. NF90 average permeate flux was 39.4L/(h.m2), while NF270 and TW30 were, respectively, 8.7 and 17.2 L/(h.m2). Average recoveries were 22%, 5% and 8% for NF90, NF270 and TW30, respectively. Both NF90 and TW30 (tighter membranes) provided BPA rejections, around 100%, while the NF270 membrane (looser membrane) showed BPA rejection between 43 and 54%. There was no detectable BPA rejection by the UF membrane whatsoever.
