Biomarcadores de sepsis en sangre de cordón para el diagnóstico de sepsis neonatal precoz

La sepsis neonatal precoz actualmente es una importante causa de morbilidad y mortalidad en el período neonatal, y su rápido diagnóstico puede ayudar a instaurar un tratamiento antibiótico eficaz. El objetivo de este trabajo es estudiar la relación de diferentes marcadores de sepsis, tanto bioquímicos como hematológicos, en muestras de sangre de cordón procedentes de neonatos; que previamente fueron clasificados en grupos de estudio en función de la presencia o ausencia de factores de riesgo (infeccioso, prematuridad, otras causas, o sepsis neonatal precoz confirmada). Los marcadores bioquímicos de sepsis (PCR, PCT e IL-6) y hematológicos en sangre de cordón no han resultado de utilidad en el diagnóstico de sepsis neonatal precoz, y los datos clínicos continúan siendo los más determinantes. Las nuevas técnicas de biología molecular en sangre de cordón fueron indicativas de la presencia de sospecha de infección en aquellos neonatos con uno o varios factores de riesgo infeccioso. / Early-onset neonatal sepsis is currently a major cause of morbidity and mortality in the neonatal period, and its rapid diagnosis can help to establish an effective antibiotic treatment. The objective of this work is to study the relationship of different markers of sepsis, both biochemical and haematological, in cord blood samples taken from infants; that were previously classified in groups according to the presence or absence of risk factors (infectious, prematurity, other causes, or confirmed early neonatal sepsis). Biochemical markers sepsis (CRP, PCT and IL-6) and haematological in cord blood have not proved useful in the diagnosis of early neonatal sepsis, and clinical data continue to be the most decisive. New techniques of molecular biology in cord blood were indicative of the presence of suspected infection in those neonates with one or several factors of risk of infection.

Reducing carbon emissions by households : the effects of footprinting and personal allowances

Nearly half of Britain’s carbon dioxide emissions result from the activity of households, both within the home and from personal transport. This research examines how the carbon dioxide emissions of households can be reduced, particularly through the calculation of carbon footprints and testing the public’s reaction to the concept of personal carbon allowances (PCAs). Two data collection stages were used - a postal survey providing quantitative data, followed by semi-structured interviews producing mainly qualitative data. The research was carried out in a largely rural district which is run by a council noted for its work on sustainable energy, Newark and Sherwood. The survey looked at PCAs as well as a variety of contemporary issues that might influence household footprints such as energy efficiency grants and information, as well as relationships with gas and electricity suppliers. Each interview involved the calculation of a household carbon footprint, the identification of measures to reduce it, and the gathering of attitudes about personal carbon allowances, in order to identify challenges and opportunities with respect to reducing household carbon emissions. Support for PCAs was higher than anticipated, and tended to be associated with those who were prepared to use public transport or cycle more, or were supportive of renewable energy in homes. Interviewees had much to say about individual carbon reducing measures. Opposition was associated with those who envisaged that they would be unlikely to sell carbon units. Regarding personal transport, long commutes were common, and the cost of public transport was of concern. Specific findings were made about domestic heating, insulation, lighting, refrigeration, water use, commuting, public transport, and rail as an alternative to short-haul flights. There was more interest in monetary savings than carbon savings. Recommendations about policy and regarding further research are made.

The thesis start with a deep insightful study of JPEG XR encoding standard. Then, a comparative evaluation of the JPEG XR versusthe JPEG is delivered both in terms of objective and subjective way. Later, part of the JPEG XR encoder is implemented in hardware as an accelerator for further evaluation. SystemVerilog is the coding language. TSMC 40nm process library and Synopsys ASIC tool chain are used for synthesize. The throughput, area, power ofthe encoder are given and analyzed. Finally, the system integration of the JPEGXR hardware encoder to Axis ARTPEC-X SoC platform is discussed.</p>

Challenges in video game development - What does Agile management have to do with it?

The video game industry has gone through a dramatic change over the last few decades, yetseveral reports show that there are currently many challenges that developers face in their dailywork. A major challenge includes difficulties of getting projects to close within set time andresource restraints. This is something that indicates a connection to the management methodsbeing used, among which Agile management is a popular framework that many turn to. Thisthesis searches for connections between challenges in video game development and the usage ofagile methods like Scrum and Kanban. For this, a qualitative research strategy was used in orderto look into the experiences of video game developers. Five semi-structured interviews with atotal of eleven respondents were conducted. As a complement, a quantitative web-based surveywas made where 23 people participated. The results of this study show that challengespreviously defined within the video game industry, including feature creep, crunch periods anda stressful work pace can also be identified in the industry in southern Sweden to some extent.Underlying patterns indicate the industrial culture as an explanation for an incorrectimplementation of agile methods, which could eventually lead to issues surrounding riskmanagement in projects.

Phases et nouveaux composés à base de magnésium pour le stockage de l'hydrogène / Laves phases and new compounds based on magnesium for hydrogen storage application

Ce mémoire de thèse concerne l’étude des composés ternaires Terre rare–magnésium–nickel utilisable pour le stockage de l’hydrogène. Ces composés ont été obtenus par fusion ou par mécanosynthèse. Les intermétalliques YNi4-xAlxMg, dérivant des phases de Laves de structure cubique ont été étudiés. Ils réagissent de manière réversible avec l’hydrogène à P et T ambiantes. Le comportement structural lors d’une hydruration a été étudié par DRX in situ. Le composé conserve sa symétrie cubique mais avec diminution de la cristallinité. Cette étude est complétée par l’étude de composés : (i) riche en terre rare (e.g. Gd4NiMg) qui absorbe l’hydrogène à température ambiante de manière irréversible. La structure de l’intermétallique et de l’hydrure sont déterminées. La décomposition de l’hydrure à température supérieure à 90°C est expliquée. (ii) riche en magnésium. Nous avons pu identifié un nouveau composé de formulation proche de Mg77Gd9Ni14.5 de structure CFC. / This work deals with the study of ternary compounds Rare Earth – magnesium - nickel used ofr hydrogen storage. All the compounds are prepared by fusion and by mechanical alloying method.. The compounds YNi4-xAlxMg, derived from the cubic Laves phases have been studied in the first part. It reacts reversibly towards hydrogen at atmospheric pressure and room temperature. The structural behaviour during the hydrogen sorption has been studied by in situ XRD. The compound remains cubic with a decrease of the crystallinity. This study is completed by the study of compounds : (i) rich in rare earth (e.g. Gd4NiMg) that absorb hydrogen at room temperature but irreversibly. Structures of both the intermetallic and the hydride have been determined. The decomposition of the hydride at temperature higher than 90°C is also explained. (ii) rich in magnesium. A new compounds with a formulation closed to Mg77Gd9Ni14.5 has been identified and it crystallized with a cubic faces centred structure.

Profiling Populations Using Neutral Markers, Major Histocompatibility Complex Genes and Volatile Organic Compounds as Modeled in Equus caballus Linnaeus

Assessing the genetics of wild animal populations aims to understand selective pressures, and factors whether it be inbreeding or adaptation, that affect the genome. Although numerous techniques are available for assessing population structure, a major obstacle in studying wild populations is obtaining samples from the animals without having to capture them, which can lead to undue distress and injury. Therefore, biologists often use non-invasive sampling methods (i.e., collection of feces, hair) to extract host DNA. In this study, new DNA extraction protocols were developed that improved the quality and quantity of DNA obtained from fecal matter. Fecal samples aged up to Day 6 as well as field samples with unknown days since defecation were successful in individualization of the contributors using microsatellites and were further used to demonstrate kinship. Neutral markers such as short tandem repeat, and mitochondrial D-loop sequences are used for assessing relatedness and evolutionary relationships and can mutate without detrimental effects on the organism. Loci, such as the major histocompatibility complex (MHC), adapt more rapidly under selective pressure such as parasite load, or resistance to diseases and support natural selection processes. Analysis of the neutral microsatellites in Big Summit feral horse population demonstrated a population lacking diversity and trending towards being an inbred population. However, examination of the MHC genes showed maintenance of greater variation that may be the result of selection pressures. The MHC similarity and lower genetic demarcation between geographically separated horse populations further indicated effect of selection pressures in preserving diversity at the MHC genes. Although such molecular markers are used in profiling populations, the current study was also successful in demonstrating the use of individual odor profiles as an additional profiling tool. Volatile organic compounds (VOC) obtained from hair of domestic horses were able to individualize horses as well as differentiate between horse breeds and display kinship. The relation of genetics to odor phenotype is of interest as the inherent polymorphic nature of MHC genes has the potential to generate unique combinations of genotypes that presumably produce distinct odor phenotypes. Subsequently, this study was able to show a significant correlation between MHC genotypes and VOC odor profiles in horses. Understanding the relationship between MHC and odor using domestic horses with known relatedness provides evidence that these same correlations may be applicable to wild equids and dictates their harem hierarchal social structure.

Using privacy calculus theory to explore entrepreneurial directions in mobile location-based advertising: Identifying intrusiveness as the critical risk factor

Yes / Location-based advertising is an entrepreneurial and innovative means for advertisers to reach out through personalised messages sent directly to mobile phones using their geographic location. The mobile phone users' willingness to disclose their location and other personal information is essential for the successful implementation of mobile location-based advertising (MLBA). Despite the potential enhancement of the user experience through such personalisation and the improved interaction with the marketer, there is an increasing tension between that personalisation and mobile users' concerns about privacy. While the privacy calculus theory (PCT) suggests that consumers make privacy-based decisions by evaluating the benefits any information may bring against the risk of its disclosure, this study examines the specific risks and benefits that influence consumers' acceptance of MLBA. A conceptual model is proposed based on the existing literature and a standardised survey was developed and targeted at individuals with known interests in the subject matter. From these requests, 252 valid responses were received and used to evaluate the key benefits and risks of MLBA from the users' perspectives. While the results confirmed the importance of internet privacy concerns (IPC) as an important determinant, they also indicate that monetary rewards and intrusiveness have a notably stronger impact on acceptance intentions towards MLBA. Intrusiveness is the most important risk factor in determining mobile users' intentions to accept MLBA and therefore establishing effective means of minimising the perceived intrusiveness of MLBA can be expected to have the greatest impact on achieving effective communications with mobile phone users.

Gestörte Homöostase von Inflammation und Antiinflammation bei Risikopatienten nach Herzchirurgie

Kardiochirurgische Eingriffe unter Einsatz der Herzlungenmaschine führen über die Sekretion proinflammatorischer Mediatoren im allgemeinen zu einer systemischen Entzündung (SIRS). Um das Ausmaß zu begrenzen, wird diese von einer systemischen Gegenregulation (CARS) begleitet, die mit zunehmender Ausprägung den Organismus anfällig für sekundäre Infektionen macht. Septische Krankheitsbilder zählen zu den häufigsten Todesursachen auf operativen Intensivstationen mit jährlichen Kosten in Milliardenhöhe. Gerade die Früherkennung ist klinisch von größter Wichtigkeit. Bei der Suche nach neuen Infektionsmarkern ist das Verständnis der immunologischen Grundlagen eine Grundvoraussetzung. In dieser Studie sollte untersucht werden, ob das Modell "systemische Immunaktivierung - Gegenregulation mit Immundepression - hohe Infektanfälligkeit" auf Risikopatienten nach Herzchirurgie übertragen werden kann. Außerdem sollte untersucht werden, ob ein standardisiertes Immunmonitoring in der Lage ist, bei diesen Patienten Infektionen frühzeitig vor klinischer Manifestation vorherzusagen, und ob diese neuen Immunparameter konventionellen Routine-Infektionsmarkern (SIRS-Kriterien, CRP) in ihrer diskriminativen Aussagekraft überlegen sind. Die Ergebnisse zeigen, dass das Modell an diesem Patientenkollektiv bestätigt werden kann. Die Immunaktivierungsmarker total-IL-8 (nach Erythrozytenlyse), PCT und ex vivo Elastase, sowie das antiinflammatorische IL-10 im Plasma und der Immunkompetenzmarker HLA-DR auf Monozyten zeigten am 1. postoperativen Tag ein hohes diskriminatives Potential, Infektionen im 6-tägigen postoperativen Verlauf vorherzusagen. Analysen der ROC-Kurven ergaben für HLA-DR eine AUC von 0,75, die AUC von total-IL-8 betrug 0,73, ex vivo Elastase erreichte 0,72, und PCT und IL-10 kamen jeweils auf 0,68. Dagegen konnten konventionelle Infektionsmarker nicht signifikant zwischen Patienten mit versus ohne postoperativer Infektion unterscheiden (CRP), beziehungsweise errechnete sich für 2 positive SIRS-Kriterien eine AUC von nur 0,66. Durch die bei einem solchen Patientenkollektiv erstmalige Verwendung hochstandardisierter Messverfahren (exakte Quantifizierung von Oberflächenmolekülen, semi-automatisches ELISA-System) wurde neben einer besseren Quantifizierung der gestörten Homöostase zwischen Inflammation und Antiinflammation eine wichtige Voraussetzung für die klinische Etablierung dieser neuen Marker geschaffen. Auf dieser Basis lassen sich früh identifizierte Risikopatienten adjuvanten Therapieversuchen zuführen. / Basically, cardiac surgery involving cardiopulmonary bypass leads to systemic inflammation (SIRS) by the secretion of proinflammatory mediators. In order to limit its extend, systemic inflammation is associated with systemic counter-regulation (CARS), which, under some circumstances, may lead to high susceptibility of the organism to secondary infections. Septic disease is among the most common causes of death in surgical ICUs, the costs are estimated at several billion Euros per year. The early diagnosis in particular is of great importance clinically. Understanding of the immunologic principles is a basic assumption with regard to finding new markers of infection. This study was performed to determine whether the model "systemic immune activation - counter-regulation and immune depression - high susceptibility to infections" could be transferred to risk patients after cardiac surgery. In addition, a standardized immune monitoring program should be examined regarding its ability to predict infection in this patient population before clinical manifestation. It should also be determined if these new parameters have more discriminative power than conventional routine markers of infection (SIRS, CRP). The results show that this model can be confirmed in this patient collective. On the 1st postoperative day markers of immune activation, total-IL-8 (after lysis of erythrocytes), PCT and ex vivo elastase, as well as anti-inflammatory IL-10 in plasma and the marker of immune competence, HLA-DR on monocytes, have high discriminative potential to predict infections during the 6-day postoperative course. AUCs of the ROC were 0.75 for HLA-DR, 0.73 for total-IL-8, 0.72 for ex vivo elastase, 0.68 for both PCT and IL-10. On the other hand, conventional markers of infection were not able to discriminate significantly between patients with infection versus those without (CRP), or they only had an AUC of 0.66 (for 2 positive SIRS criteria). By using well-standardised laboratory methods (exact quantification of surface molecules, semi-automatic ELISA-system), which were used for the first time in such a patient collective, an important basis for clinical establishing these new markers was created, in addition to a better quantification of the immunologic unbalance (inflammation versus anti-inflammation). Thus, it is possible to supply early identificated risk patients for adjuvant therapy trials.

Modelagem computacional de tomografia com feixe de prótons / Computational modeling of protons tomography

Fundação Carlos Chagas Filho de Amparo a Pesquisa do Estado do Rio de Janeiro / Nessa tese foi feito um estudo preliminar, destinado à elaboração do programa experimental inicial para a primeira instalação da tomografia com prótons (pCT) brasileira por meio de modelagem computacional. A terapia com feixe de prótons é uma forma bastante precisa de tratamento de câncer. Atualmente, o planejamento de tratamento é baseado na tomografia computadorizada com raios X, alternativamente, a tomografia com prótons pode ser usada. Algumas questões importantes, como efeito de escala e a Curva de Calibração (fonte de dados iniciais para planejamento de terapia com prótons), foram estudados neste trabalho. A passagem de prótons com energias iniciais de 19,68MeV; 23MeV; 25MeV; 49,10MeV e 230MeV pelas camadas de materiais variados (água, alumínio, polietileno, ouro) foi simulada usando códigos Monte Carlo populares como SRIM e GEANT4. Os resultados das simulações foram comparados com a previsão teórica (baseada na solução aproximada da equação de transporte de Boltzmann) e com resultados das simulações feitas com outro popular código Monte Carlo MCNPX. Análise comparativa dos resultados das simulações com dados experimentais publicados na literatura científica para alvos grossos e na faixa de energias de prótons usada em medidas em pCT foi feita. Foi observado que apesar de que todos os códigos mostram os resultados parecidos alguns deslocamentos não sistemáticos podem ser observados. Foram feitas observações importantes sobre a precisão dos códigos e uma necessidade em medidas sistemáticas de frenagem de prótons em alvos grossos foi declarada. / In the present work a preliminary research via computer simulations was made in order to elaborate a prior program for the first experimental pCT setup in Brazil. Proton therapy is a high precise form of a cancer treatment. Treatment planning nowadays is performed basing on X ray Computer Tomography data (CT), alternatively the same procedure could be performed using proton Computer Tomography (pCT). Some important questions, as a scale effect and so called Calibration Curve (as a source of primary data for pCT treatment planning) were studied in this work. The 19.68MeV; 23MeV; 25MeV; 49.10MeV e 230MeV protons passage through varied absorbers (water, aluminum, polyethylene, gold) were simulated by such popular Monte Carlo packages as SRIM and GEANT4. The simulation results were compared with a theoretic prevision based on approximate solution of the Boltzmann transport equation and with simulation results of the other popular Monte Carlo code MCNPX. The comparative analysis of the simulations results with the experimental data published in scientific literature for thick absorbers and within the energy range used in the pCT measurements was made. It was noted in spite of the fact that all codes showed similar results some nonsystematic displacements can be observed. Some important observations about the codes precision were made and a necessity of the systematic measurements of the proton stopping power in thick absorbers was declared.
