Entretanto, os achados precisam ser replicados, preferentemente em estudos longitudinais.<br> / Abstract : Objective: Assess the impact of prematurity on telomere length, a cell aging marker, in children born with very low birth weight. Methods: We conducted a controlled cross-sectional study evaluating 91 children between 7 and 12 years, been 46 former very low birth weight premature and 45 controls. The relative quantification of the size of telomeres (T/S) was performed by polymerase chain reaction (PCR) in peripheral blood leukocytes. Information on perinatal variables, socioeconomic status and stressful life events were obtained. The linear regression model was used for statistical analysis, both in univariate and multivariate analysis. Results: There was no significant difference in the leukocyte T/S ratio among premature infants and controls (p = 0.841), even after adjustment for possible predictor variables. Univariate analysis showed no statistically significant association between T/S ratio and sex, age, parental age, smoking during pregnancy, education of the caregiver and the head of the family and stressful events that occurred last year. Among preterm infants, there was no relationship between the T/S ratio and birth weight below 1,000 g (ß=0.133, 95% CI= -0.141 to 0.406; p=0.341)), gestational age below 30 weeks (ß=0.030, 95% CI= -0.209 to 0.270; p=0.34) or bronchopulmonary dysplasia (ß = 0.213, 95% CI = -0.166 to 0.591; p=0.15). Conclusions: The results indicate that there is no relationship between preterm birth or very low birth weight and the leukocyte telomere length at school age. However, the findings need to be replicated, preferably in longitudinal studies.

Giraldi, Susana January 1999 (has links)
Abstract: Epidemiological studies on Pediatric Dermatology are exceptional in the literature and little is known about the frequency of pediatric dermatoses in hospitals. The present study, epidemiological and descriptive, perfomed at the Hospital de Clinicas outpatient Unit of UFPR, was aimed to analyze the morphologic category of the lesions, the characteristics of the population of patients, the pattern of geographical distribution, to determine the 15 most frequent dermatoses and to evaluate the modifications which took place in this Unit during 19 years (from 1978 to 1996). During this period, 7561 new patients were examined and they were classified in 9989 dermatoses. The number of registered patients in this specialty increased 16 times, corresponding to 1.47% of all pediatric outpatients of Hospital de Clinicas until 1996, while the number of dermatologic diseases increased 18 times in comparison to 1978. It was documented a predomination of white skin patients (93.3%) among all types of examined dermatoses, including the 15 most frequent ones. Patients were more frequently found at school age (39.8%) and the female gender was slightly more common (52.2%). Most of the patients came from the state of Parana (96.5%) and from the Capital (54.4%), however, in the last 3 years it has been noticed a predomination of outpatients coming from the metropolitan area of Curitiba. The most frequent dermatoses, observed in this period, were: atopic dermatitis (10.4%), pruritus strophulus (5.8%), warts (4.8%), ptyriasis alba (4.5%), seborrheic dermatitis (3.9%), scabies (3.9%), contact dermatitis (3.7%), keratosis pilaris (3.7%), hemangioma (3.5%), psoriasis (2.7%), impetigo (2.7%), tinha capitis (2.5%), vitiligo (2.2%), Ichthyosis vulgaris (1.8%) and molluscum contagiosum (1.6%). There was a decrease in frequency of the most common dermatosis and an increase of the less frequent ones indicating an apropriate orientation of the pediatrician in this especiality.

Data were obtained from the database DATASUS through the Health Information section. Code "tuberculosis" was used to select key data on the results of the study, from 2001 to 2012. There were two selected age groups: children and adolescents aged 0-19 years and adults over 20. It was estimated the annual incidence per 100 000, which was calculated dividing the number of cases per population estimate of the age groups, obtained from the Brazilian PNAD and Census 2010 for all years studied, respectively, and multiplying the result by 100,000.Results: incidence rates showed a reduction when analyzed in the group of adults. However, no significant reduction was observed in the group of children and adolescents. Also, when we conduct regional comparisons, the north and the state of Rio Grande do Sul have shown high incidence rates. Conclusions: it would be important to improve policies for diagnosis and treatment of pediatric patients. The northern, Rio de Janeiro and the RS should be considered priority areas in fighting against TB in Brazil.

Methods: In this study were included children and adolescents, between 6 and 18 years, divided into control (eutrophic) and obese groups according to body mass index (BMI). Data were collected regarding demographic, anthropometric, waist circumference and functional capacity through the Shuttle Walk Test (SWT). In the obese group were evaluated biochemical parameters in the blood (total cholesterol, HDL, LDL, triglycerides and glucose), and the application of a physical activity questionnaire. For statistical analysis were used the chi-square test, Student's t test for independent samples and the Pearson correlation test. Results: 77 participants were included, 27 in the control group and 50 obese. There was no significant difference between the two groups regarding sample characteristics, except for body weight, BMI and waist circumference. Most obese presented results of biochemical tests within the desirable limit, though none were considered active. There was a significant reduction (p <0. 001) functional capacity in obese group compared to control subjects. Positive correlations were identified SWT with age and height, and negative correlation with BMI. However, there were no correlations with the biochemical parameters analyzed. Conclusions: Obese children and adolescents have reduced physical capacity when compared to normal individuals. The SWT performance seems to have a negative association with BMI, but is not correlated with the blood biochemical parameters.

Asthma in adult patients is most times associated with allergy and these patients present eosinophilic inflammation in the airways as its main feature. Most children with asthma present non-allergic disease, and it is still not well defined wich is the predominant inflamatory profile in these cases. There are few studies comparing bronchial inflammation in these different asthma phenotypes. The aim of this study was to evaluate the cytological profile of IS in children with atopic and non-atopic asthma, compared with a healthy control group. Methods: Of children who participated in the ISAAC-Phase II study in Southern Brazil, had completed the respiratory disease questionary and had done skin prick tests for common allergens, a sub-group was alleatory selected. They represented two asthma phenotypes and a healthy control group, known as: atopic asthma (AA), non-atopic asthma (NAA), and non asthmatics/non-atopic (NANA, control group). Sputum was induced with 4. 5% hypertonic saline solution for all participating children; cytology profiles were compared among these the three groups. Results: Ninety children were selected to participate in this study and 76 children were included (28 AA, 29 NAA, and 19 NANA), with mean age of 13 (0. 97) years. Sputum was successfully induced in 55 (72. 3%) children (21 AA, 21 NAA, and 13 NANA). Demographic data and VEF1 mean were similar in the three groups. The median eosinophil (interquartile range) proportion was significantly higher in the sputum of AA [9 (14)], compared with both NAA [1 (2)], and NANA [0. 5 (1)], p<0. 001. The proportion of children with sputum eosinophilia (eos ³3%) was also significantly higher among the AA (81%), compared with the NAA (23. 8%) and the NANA (without any) (p<0. 001). The median neutrophil (interquartil range) was significantly higher in the sputum of NAA [18 (5. 6)], compared with AA [11 (2)], and NANA [13 (7)], p<0. 001. Conclusions: The results suggest that airway eosinophilia, as observed in adults, can be detected since pre-adolecence in children with atopic asthma. Children with non-atopic asthma did not present this inflammatory profile. In this group a higher proportion of neutrophil was observed.

The study population was divided into three groups (A, B and C), and each group answered the test at the beginning of the ambulatory service stay and at its end three weeks later. The three groups evaluated rotated at four months interval ITom April - 1996 to April- 1997. The global analysis of the results of the tests, before and after the ambulatory service stay, was performed with the Analysis ofVariance (ANO V A) and with McNemar's Testo The ANOV A did not show any differences among the three groups of students, but has shown a significant difference (p < 0.001) between the tests of the pooled groups. McNemar's Test shows a significant difference (p < 0.001) among the tests of the three groups as well as in the tests ofthe pooled groups, thus confirming the ANOV A results. o However, the analysis, question by question, applying McNemar's Test, shows a significant difference between both tests in on1y 8 ofthe 30 questions ofthe test (questions 6, Tab.6B; 8, Tabo8B; 10, Tab.l0B; 11, Tab.llB;"16, Tab.16B; 17, Tabol7B; 23, Tab.23~). Among the 22 remaining questions, where there was no significant difference, McNemar's Test allowed the discrimination of 4 questions (3, Tab.3B; 4, Tab.4B; 15, Tab.15B and 28, Tab.28B) with a negative performance. The study allowed the conc1usion that th~ pediatric ambulatorycare service stay stage has provided a gain in cognitive knowledge on the contents of the 8 questions where the difference has been significant (p < 0.001). Additiona11y, it may be conc1uded that a test with multiple-choice questions may be more efficient1y used for the evaluation of the gain of cognitive knowledge when such test has a specialized teclmical formulation and considers on1y the taught contents / Mestrado / Mestre em Medicina

Five patients presented only single seizures. Eight developed epilepsy afier intracranial hypertension and the remain 28 presented at least two years of epilepsy and normal neurological exam. This Iast group (group NC) was compared to 32 patients with benign partial epilepsy (group EB). The following aspects of the epileptic manifestation were analyzed: epidemiological, clinical, therapeutical and prognostico The presentation of the epileptic picture (type, duration, ftequency of seizures, c1uster, status epilepticus, postictal deficit) was similar in the two groups. The statistically significant data found in group NC were: duration of the epilepsy; duration of the treatment; number of antiepileptic drugs; politherapy; utilization of maximum tolerated doses; number of seizures and recurrence . Control EEG persisted abnormal more :&equendyin the group NC. Our findings suggest that childhood neurocysticercosis epilepsy has similar clínical presentation to the benign epilepsy, however the evolution is different in relation to the therapeutic handling and with reIationship to the recurrence afier interruption of the treatment. These 28 patientes course was also compared to the eight patients with hipertensive form (group B). There were differences in relation to the indexes of status epilepticus, higher in the group B and to the EEG of the admission that was abnormal in alI the patients with hipertensive form. Advances in the care of this condition, particularly advances in respiratory management, have seen the focus of treatment Chest physiotherapy (CPT) has been an important part of CF treatment regimens for over 40 years and is recommended as part of the treatment, since it is an important therapeutic resource in the clearance of the airway mucus with positive results in the patient¿s quality of life. No definitive evidence links adherence to treatment and the disease severity, neither to social-economic status. The aim of this study was to evaluate the CPT adherence and its correlation to disease severity, social-economic status and clinical variables in CF patients. A transversal study was performed including 84 patients (43 male) from the population treated at the University Hospital. Measures included administered questionnaire to evaluate social-economic status, clinical variables and CPT adherence. Shwachman score (SS) were assigned to each patient as a measure of disease severity. The Fisher and Kruskal-Wallis tests were used to compare CPT adherence with all variables. Patient¿s age ranging from 5 months to 29 years (8.63± 6.42). SS was classified in excellent/good (44%), mild (33,3%) and moderate/severe (22,6%). CPT was performed in 83,3% of patients and satisfactory adherence was verified in 59,5%. A statistical relationship could be demonstrated between CPT adherence and number of family members, family income, mother¿s schooling, age, gender and number of annual consultations. There was no statistical relationship between CPT adherence and disease severity, father¿s schooling, number of rooms in the house, origin and diagnosis period / Mestrado / Pediatria / Mestre em Saude da Criança e do Adolescente
