Common mental disorders : evaluating their impact on disability and the role of socieoeconomic factors

The general aim of this Doctoral Thesis was to assess the association of common mental disorders with disability and socioeconomic factors. These factors were considered both as important consequences (or burdens) and also as potential determinants of the incidence of common mental disorders. Data used in this Thesis came from two general population-based studies: the World Mental Health (WMH) Surveys, a worldwide initiative launched by the World Health Organization to gather data on prevalence and correlates of mental disorders of the adult population (+18); and the Netherlands Mental Health Incidence Study 2 (NEMESIS-2), a longitudinal study with two waves of data collection (baseline: 2007-9 and follow-up: 2010-12). Common mental disorders were found to be important contributors to disability in Europe and also in Spain. Socioeconomic adversities, such as job loss and household income reductions, increased the risk of incidence of mental disorders. Low functional status was also associated with the incidence of common mental disorders. This Doctoral Thesis shows that common mental disorders carry on important functional and socioeconomic burdens and, at the same time, these factors also contribute to their development. A better understanding of these bidirectional associations may help tackling the circle of disadvantage in which common mental disorders appears in its central axis. / L’objectiu general d’aquesta Tesis Doctoral va ser avaluar l’associació dels trastorns mentals comuns amb la discapacitat funcional i amb factors socioeconòmics. Aquests factors van ser considerats com conseqüències (o càrregues) i també com possibles determinants de la incidència d’aquests trastorns mentals. Les dades utilitzades en aquesta Tesis van provenir de dos estudis de base poblacional: un és la Iniciativa Mundial d’Enquestes de Salut Mental (WMH) de la Organització Mundial de la Salut que recull dades sobre prevalença i factors relacionats de trastorns mentals de la població adulta (+18 anys); i l’altre l’estudi the Netherlands Mental Health and Incidence Study (NEMESIS-2), un estudi longitudinal amb dos moments de recollida de dades (basal: 2007-9 i seguiment: 2010-12). Els trastorns mentals freqüents són importants contribuents a la discapacitat funcional en Europa i en Espanga. Les adversitats socioeconòmiques, van augmentar el risc de incidència dels trastorns mentals. El baix funcionament va ser factor de risc de desenvolupar un trastorn mental incident, en particular depressió i ansietat. Aquesta Tesis Doctoral mostra que els trastorns mentals freqüents comporten importants càrregues funcionals i socioeconòmiques i al mateix temps, aquest mateixos factors també contribueixen al desenvolupament d’aquests trastorns. Una millor comprensió d’aquestes associacions bidireccionals poden ajudar a fer front al cercle de desavantatge en el que els trastorns mentals comuns apareixen com eix central.

Reserva cognitiva, neuropsicología y factores neuroestructurales implicados en la funcionalidad del trastorno bipolar

Objetivo: Estudiar la relación de la reserva cognitiva (RC) y cerebral con factores neurocognitivos y neuroestructurales y sus efectos en la funcionalidad psicosocial del trastorno bipolar (TB). Método: Se realizan cuatro estudios transversales. Los tres primeros con una muestra de 101 sujetos (TB y controles sanos), analizando variables de reserva cognitiva, neurocognición y funcionalidad. Y el último, con una muestra de pacientes con TB comparado con sus familiares de primer grado (n=91), estudiando variables de reserva cerebral, neuropsicológicas y de funcionalidad global. Resultados: En el TB la RC y el volumen de sustancia blanca cerebral aparecen como variables predictoras del funcionamiento psicosocial o cognitivo de los pacientes, no ocurriendo de la misma manera en el grupo de los familiares. Conclusiones: La RC puede ser un punto clave en el pronóstico funcional del TB e intervenciones encaminadas a potenciarla pueden prevenir el futuro deterioro cognitivo y funcional en estos pacientes. / Objectiu: Estudiar la relació de la reserva cognitiva (RC) i cerebral amb factors neurocognitius i neuroestructurals i els seus efectes en la funcionalitat psicosocial del trastorn bipolar (TB). The last one, with a sample of bipolar patients compared with their first-degree relatives (n=91), studying variables of brain reserve, neuropsychology and functional outcome. Results: In BD, CR and brain white matter volume appear as predictors of psychosocial or cognitive functioning in bipolar patients, not occurring the same way in the relatives group. Conclusions: CR may be a key factor in bipolar patients’ functional outcome, and interventions enhancing CR may help prevent cognitive and functional impairments in this condition.

Caracterización del trastorno psicopático de la personalidad: clínica, neuropsicología y neuroimagen

Objectiu: L’objectiu principal d’aquest estudi és descriure les troballes a nivell clínic, neuropsicològic i de neuroimatge derivats de l’estudi d’una mostra de subjectes empresonats avaluats amb la PCL-R. Metodologia: Aquesta tesi està formada per quatre estudis on s’investiga la comorbiditat dels diferents trastorns de la personalitat amb la psicopatia, així com les funcions executives, la impulsivitat, el reconeixement de les emocions i la correlació entre el reconeixement emocional i l’estructura regional cerebral en una mostra intrapenitenciaria de subjectes antisocials i psicòpates. Resultats: A la nostra mostra trobem un percentatge del 100% de trastorn antisocial de la personalitat en subjectes psicòpates. Els subjectes antisocials amb trastorn psicopàtic presenten una millor funció executiva que els subjectes antisocials sense psicopatia, i similars als controls. Els subjectes antisocials van ser més impulsius, mentre que els psicòpates van mostrar dèficits en el reconeixement d’algunes emocions i alteracions volumètriques cerebrals en àrees de reconeixement de les emocions. / Objetivo: El objetivo principal del presente trabajo es describir los hallazgos clínicos, neuropsicológicos y de neuroimagen derivados del estudio de una muestra de sujetos intrapenitenciarios evaluados con la PCL-R. Metodología: Esta tesis está formada por cuatro estudios dónde se investiga la comorbilidad de los diferentes trastornos de la personalidad con la psicopatía, así como las funciones ejecutivas, la impulsividad, el reconocimiento de las emociones y la correlación entre el reconocimiento emocional y la estructura regional cerebral en una muestra intrapenitenciaria de sujetos antisociales y psicópatas. Resultados: En nuestra muestra encontramos un porcentaje del 100% de trastorno antisocial de la personalidad en sujetos psicópatas. Los sujetos antisociales con trastorno psicopático presentan una mejor función ejecutiva que los sujetos antisociales sin psicopatía, y similares a los controles. Los sujetos antisociales fueron más impulsivos, mientras que los psicópatas mostraron déficits en el reconocimiento de algunas emociones y alteraciones volumétricas cerebrales en áreas de reconocimiento de las emociones. / Objective: The main purpose of the study is to describe clinical, neuropsychological and neuroimaging findings derived from a sample of incarcerated subjects evaluated with the PCL-R. Methodology: This thesis consists of four studies which investigate the comorbidity of different personality disorders with psychopathy, as well as executive functions, impulsivity, recognizing emotions and the correlation between emotion recognition and regional brain structure in a imprisoned sample of antisocial and psychopath subjects. Results: In our sample, 100% of the psychopaths obtained an antisocial personality disorder diagnosis. Antisocial offenders with psychopathy had better executive functions compared to antisocial offenders without psychopathy, and similar to controls. The antisocial subjects were more impulsive, whereas psychopaths showed some deficits in emotion recognition and volumetric brain abnormalities in relation to areas involved in recognizing emotions.

Neurobiological links between depression and drug dependence

Early life experiences play a key role in brain function and behaviour. Maternal separation produced negative early life experiences that induce emotional alterations. Contrary, the communal nest has been proposed as a protective model that may reduce the vulnerability of individuals to suffer psychiatric disorders. Moreover, early-life stress enhances the vulnerability to develop substance use disorders, principally during adolescence. Hence, depressive states are associated with drug use disorders and abuse vulnerability since depressed patients could consume drugs to alleviate their symptoms. Our results lead us to propose that detrimental early life events such as maternal neglect reproduces most of the behavioural and neurochemical alterations associated with emotional disorders in mice. In addition, maternal separation could be useful to study the comorbidity between depression and substance use disorder since induces alterations in emotional and cocaine addictive behaviours as well as in the dopaminergic system. However, in our conditions, we cannot confirm the protective role of communal nest due to the behavioural alterations found in these mice, probably because this breeding condition involves energetic cost and increases the competition for food in pups. / Las experiencias tempranas tienen un papel importante en el desarrollo cerebral y las respuestas emocionales. La separación maternal supone un factor de riesgo de desarrollar enfermedades psiquiátricas, mientras que el modelo de nido en comuna se ha propuesto como una condición protectora. Además, el estrés crónico durante la infancia aumenta la vulnerabilidad a desarrollar trastornos por uso de sustancias, principalmente durante la adolescencia. Por ello, los estados depresivos están asociados con una mayor vulnerabilidad para el uso de drogas puesto que los pacientes depresivos podrían consumir drogas para aliviar sus síntomas. Nuestros resultados demuestran que las experiencias adversas durante la infancia reproducen la mayoría de las alteraciones comportamentales y neuroquímicas relacionadas con depresión en el ratón. Además, la separación maternal puede considerarse como un modelo conveniente para el estudio de la comorbilidad psiquiátrica entre depresión y el trastorno por uso de drogas, ya que reproduce alteraciones emocionales y motivacionales en el animal de experimentación, así como en el sistema dopaminérgico. Sin embargo, no podemos confirmar el papel protector del nido en comuna debido a las alteraciones comportamentales que hemos encontrado en nuestros estudios, probablemente debido a que dicho modelo origina costes energéticos y aumenta la competencia por la comida entre las crías.

Functional connectivity anomalies in the neural networks mediating motivated behavior : assessong obsessive-compulsive disorder , chronic cannabis use, Prader-Willi syndrome and Down syndrome

Compulsive, impulsive, and addictive disorders display some behavioral commonalities associated with a dysfunction in the regulation of motivated, goal-directed behavior. Relevant to motivated behavior, there is a set of distributed large-scale neural networks connecting cortical areas, mainly frontal, with the basal ganglia. We have used MRI measurements of functional connectivity to assess the functional status of the cortico-basal ganglia circuits, as well as their interaction with other large-scale networks, in four medical conditions characteristically showing altered motivated behavior. The study samples included a group of 74 patients with obsessive-compulsive disorder, 28 chronic cannabis users, 24 Prader-Willi syndrome and 20 Down syndrome individuals. Structural MRI was additionally used to characterize gray matter volume correlations within these same networks in healthy subjects. Results showed both common and distinct functional connectivity across study groups, associated with the severity of their characteristic behavioral disturbances. All in all, the data suggest potential functional mechanisms by which flexible and adaptive behaviors may be compromised. In the specific context of frontal-basal ganglia physiology, the findings may provide new insights into the nature of obsessive compulsive behavior, its boundaries with impulsivity and the role of nonsatiated basic drives in the genesis of obsessions. / Els trastorns compulsius, impulsius, i addictius, mostren trets comuns associats a una disfunció en la regulació de la conducta motivada. El conjunt de xarxes neuronals que connecten àrees corticals, principalment frontals, amb els ganglis basals, té un paper rellevant a la conducta motivada. Hem utilitzat mesures de RM per avaluar l’estat funcional dels circuits corticals-ganglis basals, així com la seva interacció amb altres xarxes cerebrals, en quatre trastorns que d’una manera característica presenten alteracions de la conducta motivada. Les mostres d’estudi van incloure un grup de 74 pacients amb trastorn obsessiu-compulsiu, 28 consumidors crònics de cànnabis, 24 persones amb síndrome de Prader-Willi i 20 persones amb síndrome de Down. A més, vam utilitzar RM estructural per caracteritzar les correlacions volumètriques de substància grisa dins d’aquests mateixos circuits en subjectes sans. Els resultats mostren alteracions comunes i diferents entre els grups d’estudi, associades a la gravetat dels seus símptomes més característics. En el context específic de la fisiologia frontal-ganglis basals, les troballes poden proporcionar nous coneixements sobre la naturalesa del comportament obsessiu compulsiu, els límits amb la impulsivitat i el paper de les motivacions bàsiques no satisfetes en la gènesi de les obsessions.

No. of bitstreams: 0 Previous issue date: 2015-11-27 / This research aimed to understand the lived of adolescents diagnosed with Attention Deficit Disorder with Hyperactivity (ADHD) from theoretical frameworks of the phenomenology of Merleau-Ponty and phenomenological psychopathology Arthur Tatossian. Qualitative research was conducted using the phenomenological method inspired by the mundane phenomenology of Merleau-Ponty. Eight Adolescents were individually interviewed of both sexes, aged 11 and 18, starting with the triggering question: How you feel about live with the disorder attention deficit hyperactivity disorder? They were found from those indicated by the snowball method and the adolescents themselves chose the locations of the interviews. From the emerging themes in the interviews was part listed the following categories: 1. Understanding what is ADHD; 2. Experience of having ADHD; 3. Difficulties in the learning process; 4. Medication; and 5. Interpersonal relationships. Most respondents described difficulties in sustaining attention, especially in school activities. Hyperactivity was described as an experience in which the world was experienced as slower, which could cause them to times when rushed in actions and thoughts produced with little connection to each other. They described problems with self-esteem, which increased the personal insecurity, particularly in studies and social relationships. The predominant form of treatment was the use of Ritalin and was also cited psychological accompaniment and the presence of tutoring. Framing an understanding of ADHD and understanding of the lived himself was important for adolescents because it favored a change of perspective, a focus before only in passing the disease to consider the individual capabilities. In this research, the perspective of Phenomenological psychopathology allowed to look at the intersection between theory and practice, considering the knowledge already built about ADHD with the uniqueness of living of adolescents. Consider the field of inter-subjectivity is to recognize that there is no an individual who exists in itself, and thus there is no absolute and isolated ADHD in the world. Foi realizado pesquisa qualitativa, utilizando o método fenomenológico inspirado na Fenomenologia mundana de Merleau-Ponty, em que foram entrevistados individualmente oito adolescentes de ambos os sexos, com idade entre 11 e 18 anos, iniciando-se com a seguinte pergunta disparadora: Como é, para você, viver com o transtorno do déficit de atenção com hiperatividade? Os sujeitos colaboradores foram encontrados a partir de indicações pelo método snowball (bola de neve) e os locais das entrevistas foram escolhidos pelos próprios adolescentes. A partir dos temas emergentes nas entrevistas foi elencado as seguintes categorias: 1. Compreensão do que é o TDAH; 2. Experiência de ter TDAH; 3. Dificuldades no processo de aprendizado; 4. Medicação; e 5. Relacionamentos interpessoais. A maioria dos entrevistados descreveu dificuldades em manter a atenção, principalmente nas atividades escolares. A hiperatividade foi descrita como uma vivência em que o mundo era vivido como mais lento, o que podia levá-los a momentos em que se precipitavam nas ações e produziam pensamentos com pouca conexão entre si. Descreveram problemas com a autoestima, o que aumentava a insegurança pessoal, principalmente nos estudos e relacionamentos sociais. A forma de tratamento predominante foi com o uso da Ritalina, tendo sido citados também acompanhamentos psicológico e a presença de reforço escolar. A construção de um entendimento sobre o TDAH e a compreensão do próprio vivido foi importante para os adolescentes, pois favorecia uma mudança de perspectiva, de um foco antes somente na doença para passar a considerar as potencialidades individuais. Nesta pesquisa, a perspectiva da Psicopatologia fenomenológica possibilitou olhar para a interseção entre teoria e prática, considerando os conhecimentos já construídos acerca do TDAH com a singularidade do vivido dos adolescentes entrevistados. Considerar o campo da intersubjetividade é reconhecer que não há um indivíduo que exista em si mesmo, não existindo, consequentemente, o TDAH puro e isolado do mundo. He is interested in understanding the phenomenological experience with his schizophrenic patients due to his experience in the clinical follow-up of these patients, as well as to realize that in schizophrenia the loss of the natural connection between the patient and other persons is identified, being this connection defined as natural evidence . This doctoral thesis in cotutela aims to present the clinical phenomenology of the loss of natural evidence in the life world (Lebenswelt) schizophrenic having the philosophical phenomenology of the ambiguity of Maurice Merleau-Ponty (1908-1961) as a comprehensive and critical lens. The phenomenological experience is understood by Merleau-Ponty in the interweaving between man and world, that is, from its facticity. Merleau-Ponty's phenomenology of ambiguity presents mundane man as an absolute source, putting his experience above scientific representations and proposing that to return to things themselves is to return to the pre-reflective world, prior to knowledge as it is experienced. This thesis is divided into six chapters and methodology. The first three chapters consist of theoretical discussions on the clinical phenomenology of schizophrenia and the presentation of Merleau-Ponty's phenomenology of ambiguity. Our methodological contribution is the critical phenomenological method, especially the phenomenological case study that highlights a two-step instrument: clinical encounters and descriptive reporting. Field research was carried out in a Day Hospital in the city of Fortaleza / Ce and allowed the construction of three phenomenological clinical cases: Roberto, Francisco and Miguel. Roberto, am I an angel or a vampire? consists of a phenomenological case study that highlights the significance of the experience of the loss of natural evidence as pure agitation and euphoria. Francisco, I write film scripts portrays a constant search of the researcher to try to walk hand in hand with him in his life world (Lebenswelt) and makes us question about the empathic understanding in the accompaniment of psychotic experiences. Miguel, is it spiritual or psychological? presents his lack of implication in the relations with the others by being detuned of the world and experiencing doubts about invasions of evil spirits who command their actions and their feelings. The understanding of the phenomenological case studies of Roberto, Francisco, and Miguel inspired by the phenomenology of Merleau-Ponty's ambiguity underscores a dialectical movement without synthesis, since it always implies constantly with its incompleteness and its various possibilities of meanings. For this, we assume the relevance of the phenomenological humanist attitude, be it of the clinician or the researcher, in order to facilitate the unveiling of these experiences (clinical-client-world) in a dialectical movement always open, without synthesis in a clinical space of reception, trust, empathy, freedom of expression and growth, built in this clinical-other-world relationship. Lebenswelt. / A perda da evidência natural se destaca na fenomenologia clínica com a esquizofrenia desenvolvida pelo psiquiatra alemão Wolfgang Blankenburg. Este se interessa em compreender a experiência fenomenológica com seus pacientes esquizofrênicos devido a sua experiência no acompanhamento clínico destes, bem como por perceber que na esquizofrenia se identifica a perda da conexão natural entre o paciente e as outras pessoas, sendo esta conexão definida como evidência natural. Esta tese de doutorado em cotutela tem como objetivo de pesquisa apresentar a fenomenologia clínica da perda da evidência natural no mundo vivido (Lebenswelt) esquizofrênico tendo a fenomenologia filosófica da ambiguidade de Maurice Merleau-Ponty (1908-1961) como lente compreensiva e crítica. A experiência fenomenológica é compreendida por Merleau-Ponty no entrelaçamento entre homem e mundo, ou seja, a partir de sua facticidade. A fenomenologia da ambiguidade de Merleau-Ponty apresenta o homem mundano como fonte absoluta, colocando sua experiência acima das representações científicas e propondo que retornar às coisas mesmas é voltar ao mundo pré-reflexivo, anterior ao conhecimento tal como ele é experienciado. Esta tese se divide em seis capítulos e metodologia. Os três primeiros capítulos consistem em discussões teóricas sobre a fenomenologia clínica da esquizofrenia e a apresentação da fenomenologia da ambiguidade de Merleau-Ponty. Nosso aporte metodológico é o método fenomenológico crítico, em especial o estudo de caso fenomenológico que destaca um instrumento de duas etapas: encontros clínicos e relato descritivo. A pesquisa de campo foi realizada em um Hospital Dia na cidade de Fortaleza/Ce e possibilitou a construção de três casos clínicos fenomenológicos: Roberto, Francisco e Miguel. Roberto, sou um anjo ou um vampiro? consiste em um estudo de caso fenomenológico que destaca o significado da experiência de perda da evidência natural como pura agitação e euforia. Francisco, escrevo roteiros de filme retrata uma constante busca da pesquisadora de tentar passear de mãos dadas com ele em seu mundo vivido (Lebenswelt) e nos faz questionar sobre a compreensão empática no acompanhamento de vividos psicóticos. Miguel, é espiritual ou psicológico? apresenta sua falta de implicação nas relações com os outros por se desintonizar do mundo e vivenciar dúvidas sobre invasões de espíritos do mal que comandam suas ações e seus sentimentos. A compreensão dos estudos de casos fenomenológicos de Roberto, Francisco e Miguel com inspiração na fenomenologia da ambiguidade de Merleau-Ponty ressalta um movimento dialético sem síntese, pois sempre implica constantemente com seu inacabamento e suas diversas possibilidades de significados. Para isso, assumimos a relevância da atitude humanista fenomenológica, seja do clínico ou do pesquisador, no sentido de facilitar o desvelamento dessas experiências (clínico-cliente-mundo) em um movimento dialético sempre aberto, sem síntese em um espaço clínico de acolhimento, confiança, empatia, liberdade de expressão e crescimento, construído nesta relação clínico-outro-mundo. No. of bitstreams: 0 Previous issue date: 2009-10-30 / This research has as its objective to comprehend and discuss the meaning of the experience lived in the depressed body, investigating the phenomenon of depression in the lived body or the own body, under the lens of Merleau-Ponty.s Penomenology (1945/2006). The relevance of this subject consists in the importance of the phenomenon of depression has acquired in the present days, which is a target for increased preoccupation, due to its incidence and the increase in the epidemiologic indexes (Nascimento, 1999; Lafer & Amaral, 2000; WHO, 2008). When such phenomenon is comprehended under the point of view of the phenomenological pathology, the essay points towards the necessity of overcoming the standard model of psychopathology, proposing, from the contact with the lived experience of the people with depression, an approach which gives priority to the rupture with the paradigm of duality, and one that does not conceive man as a purely biological organism, but imbricated in one.s history and culture. To attain the proposed goals, a qualitative research was conducted, using the mundane phenomenological method which is based on Merleau-Ponty.s Phenomenology. The research revealed that the phenomenon of depression is difficult to be described by the patients, and that they recognize it through the signals in their body. However, the concept of body of these patients consists in the division between body and mind, which hampers the patients. correlation between corporality and existence. The testimonies from the collaborating subjects also revealed: depression is an external phenomenon to the subject; the body posture and the body rhythm of the subjects is marked by isolation, retardation and stagnation; they live a paradoxical relation with death; their self-esteem and personal value are deeply compromised; they carry a feeling of unpowerment and guilt and they establish a barrier in their relationships with other people. The conclusion is that the view of the depressed body through the lens of Merleau-Ponty.s Phenomenology can contribute to the understanding of depression as an expression of the existence of individuals not only as a group of symptoms marked by organic circumstances. Keywords: Depression; Phenomenology; Phenomenological Psychopath; Own Body; Experience Lived; Depressed Body / Esta pesquisa tem como objetivo compreender e discutir o significado da experiência vivida do corpo deprimido, investigando o fenômeno da depressão no corpo vivido ou corpo próprio, sob a lente da Fenomenologia de Merleau-Ponty (1945/2006), para além da sintomatologia. A relevância deste tema consiste na importância que o fenômeno da depressão vem atingindo na atualidade, sendo alvo de crescente preocupação, devido à sua incidência e ao aumento dos índices epidemiológicos (Nascimento, 1999; Lafer & Amaral, 2000; WHO, 2008). Ao compreender tal fenômeno do ponto de vista da psicopatologia fenomenológica, a dissertação aponta a necessidade de superação do modelo tradicional da psicopatologia, propondo, a partir do contato com a experiência vivida das pessoas deprimidas, um enfoque que priorize a ruptura com o paradigma da dualidade, e que não conceba o homem como um organismo puramente biológico, mas imbricado em sua história e sua cultura. Para atingir os objetivos propostos, foi realizada uma pesquisa qualitativa, utilizando o método fenomenológico mundano que toma por base a Fenomenologia de Merleau-Ponty. A pesquisa revelou que o fenômeno da depressão é de difícil descrição pelos pacientes, e que eles a reconhecem a partir de sinais do seu corpo. No entanto, a noção de corpo de tais pacientes consiste na divisão em mente e físico, o que prejudica a sua correlação entre corporeidade e existência. Os depoimentos dos sujeitos colaboradores revelaram ainda: que a depressão é considerada um fenômeno exterior ao sujeito; que a postura e o ritmo corporais de tais sujeitos é marcado pelo isolamento, pela lentificação e pela estagnação; que eles vivem uma relação paradoxal com a morte; que a sua autoestima e o seu valor pessoal encontram-se profundamente comprometidos; que eles carregam um sentimento de despotencialização e de culpa e que estabelecem uma barreira na sua relação com o outro. Concluo que o olhar para o corpo deprimido sob a lente da Fenomenologia de Merleau-Ponty pode contribuir para uma compreensão da depressão como expressão da existência dos indivíduos não apenas como um conjunto de sintomas marcado por circunstâncias orgânicas. No. of bitstreams: 0 Previous issue date: 2009-11-27 / The pa per, ¿Alcoholism: a Mundane Experience¿, had the general objective to describe the experience lived by some people during their alcoholism treatment at Caps Ad (Psychosocial Center for the Treatment of Alcohol and other Drugs) in Fortaleza. It was a phenom enological research that was built according to the qualitative approach and used Focal Group as strategy. The experiences lived during the process were described by the collaborators during the focal group sessions and were analyzed based on the critical phenomenological method. From these processes it was possible to reach the following topics: How did they start drinking; Consequences related to the use of alcohol; The quest for treatment; Facing the situation or Running Away?; What about the Alcohol?; H ow do they see Caps; Experiences of people who are not drinking now; Damage Reduction; Living ambivalent feelings; Feelings related to relapses; T he importance given to medication; E xperiencing a ¿ body ¿ with no control; The necessity of family support; How difficult it is to be assisted by SUS; and, Experiences lived during the focal group sessions. The results revealed alcoholism as a mundane experience, not seen individually, but as an experience intertwined with the world, marked by the pathological expe rience of oppression and lost of individual power. The present paper makes aware of the necessity of more researches to evaluate the effectiveness of the public policies toward the alcoholism. Key - Words: Alcoholism, Treatment, Critical Psychology, Mu ndane Phenomenology . / Esta pesquisa teve como objetivo geral, descrever a experiência vivida por pessoas em tratamento para alcoolismo em um Centro de Atenção Psicossocial para Álcool e Outras Drogas (Caps Ad) em Fortaleza.Tratou-se de uma pesquisa fenomenológica - fenomenologia existencial mundana - que utilizou-se de uma metodologia qualitativa com a estratégia de Grupo Focal.Inicialmente foi feita uma retrospectiva sócio-histórica-cultural-ideológica do uso do álcool na humanidade, seus efeitos na pessoa e as possibilidades de desenvolvimento da Síndrome de Dependência do Álcool. Logo em seguida, foram discutidos os Fundamentos para uma Compreensão Mundana do Alcoolismo a partir das lentes da Psicopatologia Crítica e da Fenomenologia Mundana. As experiências vividas descritas pelas pessoas colaboradoras durante as sessões de grupo focal foram analisadas fenomenologicamente, de onde foram retirados os seguintes temas emergentes: Como começaram a beber; As conseqüências relacionadas ao uso do álcool; A busca de tratamento; Enfretamento ou fugas?; O que é o álcool?; Como percebem o Caps; Experiências de quem esta sem beber; Redução de Danos; Vivência de sentimentos ambivalentes. Sentimentos com relação às recaídas; Importância dada às medicações; Vivência do corpo sem controle; Necessidade de apoio da família; Dificuldade de Atendimento do SUS; e, Experiências vividas nas sessões de grupo focal. Alguns temas comportaram ainda subdivisões em subtemas visando melhor compreensão e reflexão acerca do estudo em questão. O alcoolismo foi compreendido como uma experiência mundana marcada pela vivencia patológica de despotencialização e opressão. Essa pesquisa alertou para a necessidade de mais pesquisas sobre avaliação e efetividade das políticas públicas referentes ao alcoolismo, sugerindo, inclusive algumas possibilidades de intervenção. French psychiatrist with Armenian parentage, Arthur Tatossian (1929-1995) may be seen as one of the main thinkers of the phenomenological psychopathology. Bearer of great knowledge either of the philosophical phenomenology as of the ones involved with the phenomenological psycopathology, Tatossian developed a model of psychopathology that uses the phenomenology as a means of inspiration, always trying to keep away from a natural poise and searching for a phenomenological poise instead. In spite of the recognition of the extent of his works in different countries, there is a wide access difficulty to his writings in Brazil. This paper proposes to add to filling this gap. In its first part, we discuss the fundaments of this phenomenological psychopathology, researching through their main foundations and its epistemological basis as well. It is discussed the distinction between symptom and phenomena, the paradox between theory and practice, the notions of average and pathological, culture and unconscious, as well as and also the importance of the Lebenswelt concept (Lifeworld) and the conception of phenomenological subjectivity. In the second part, we search the comprehension of the depression phenomena, one of the most discussed psychopathological phenomena by Tatossian, delineating a historical path of the concept, going through the difference between sadness and depression, through the conditions to the allowance of the lived depressed Time, the body, Space and the other , until arriving at the conception of depressivity. The ambiguous relationship between psychopathology and clinic emerges as an axis, at the same time, pivotal and mutual of what was discussed in this introductory path. With Tatossian, psychopathology emerges as eminently clinical and clinic itself develops beyond the therapeutics, as the place for the psychopathological experience. The search for a widening experience and the influence of the Lebenswelt concept, in its pre-reflexive ambiguity, allow Tattosian to develop a psychopathology and a Lebenswelt clinic. It is a search for the comprehension of the phenomenological experience that is shown in the clinic and, in its own turn, is placed in the Lebenswelten interception of the patient and the clinical. He comprehends in a ambiguous way both the fundaments of its psychopathology and the comprehension of the psychopathological living that is shown in the contact with the subject and with the psychopathology too. Tatossian s clinical phenomenology always trespassed by the ambiguity, valorizes and prioritizes the daily contact with the subject the clinical experience itself without ever discarding the psychopathological theory and proposes, this way, to radically consider, via phenomenology, the nature of the mental disease, its constitution and the forms of presentation, allowing distinct dialogues and horizons. Psiquiatra francês, de origem armênia, Arthur Tatossian (1929-1995) pode ser considerado um dos principais pensadores da psicopatologia fenomenológica. Grande conhecedor tanto dos promotores da fenomenologia filosófica quanto dos da psicopatologia fenomenológica, Tatossian desenvolveu um modelo de psicopatologia que utiliza a fenomenologia como inspiração, buscando sempre se afastar de uma atitude natural para buscar uma atitude fenomenológica. Apesar do reconhecimento da importância de sua obra em diferentes países, há grande dificuldade, no Brasil, de acesso aos seus escritos. Esta dissertação se propõe a contribuir para o preenchimento desta lacuna. Na primeira parte, discutimos os fundamentos desta psicopatologia fenomenológica, analisando seus alicerces principais, bem como as suas bases epistemológicas. São discutidos a distinção entre sintoma e fenômeno, o paradoxo entre teoria e prática, as noções de normal e de patológico, de cultura e de inconsciente, bem como a importância do conceito de Lebenswelt (mundo vivido) e a concepção de subjetividade fenomenológica. Na segunda parte, buscamos compreender a depressão, um dos fenômenos psicopatológicos mais discutidos por Tatossian, traçando um caminho histórico do conceito, passando pela diferenciação entre tristeza e depressão, pelas condições de possibilidade do vivido depressivo o tempo, o corpo, o espaço e o outro , até chegar à concepção de depressividade. A relação ambígua entre psicopatologia e clínica surge como ponto central e comum daquilo que discutimos neste percurso introdutório. Com Tatossian, a psicopatologia assume uma função eminentemente clínica e a clínica se desenvolve para além de uma terapêutica, como o lugar da experiência psicopatológica. A busca do alargamento da experiência, e a influência do conceito de Lebenswelt, em sua ambiguidade pré-reflexiva, permitem que Tatossian desenvolva uma psicopatologia e uma clínica do Lebenswelt. Trata-se de buscar a compreensão da experiência fenomenológica que se apresenta na clínica, que, por sua vez, está situada na interseção dos Lebenswelten do paciente e do clínico. Compreende de forma ambígua os fundamentos de sua psicopatologia e a compreensão do vivido psicopatológico que se apresenta no contato tanto com o sujeito quanto com a psicopatologia. A fenomenologia clínica de Tatossian, sempre atravessada pela ambiguidade, valoriza e prioriza o contato cotidiano com o sujeito a experiência clínica mesma sem nunca descartar a teoria psicopatológica e se propõe, assim, a refletir radicalmente, via fenomenologia, a natureza da doença mental, sua constituição e formas de apresentação, possibilitando diferentes diálogos e horizontes.
