Tecnologia assistiva : atributos de design de produto para adequação postural personalizada na posição sentada

Tecnologia Assistiva é o termo atualmente utilizado para definir uma enorme variedade de produtos e serviços destinados a pessoas com deficiências. O design de produto de equipamentos de tecnologias assistivas segue diferentes rotas tecnológicas, com produtos fabricados em série ou personalizados. Tais equipamentos objetivam otimizar e potencializar as funções corporais alteradas e, com isso, reconduzir essas pessoas às mais variadas atividades sociais. Neste sentido, esta pesquisa teve como proposta contribuir na construção de novas rotas tecnológicas para a fabricação de assentos e encostos personalizados para pessoas deficientes e usuários cadeira de rodas. A metodologia conceitual incluiu nesta pesquisa as ferramentas de digitalização tridimensional, usinagem CNC e termografia para realizar medidas e analisar os assentos e encostos personalizados fabricados a partir de dados antropométricos individualizados. Os protótipos projetados, fabricados e sistematicamente testados geraram resultados indicativos de que os modelos personalizados propostos são válidos, visto que agregam diferenciais como; uma maior distribuição das cargas, e também pela distribuição das temperaturas corporais obtidas pelas superfícies e contornos reproduzidos pelos equipamentos segundo a anatomia de cada pessoa estudada. Assim, as principais contribuições deste estudo estão na otimização e simplificação do processo de fabricação, e no baixo custo das matérias primas utilizadas na pesquisa. Finalmente, os resultados indicam que os produtos obtidos, assentos personalizados, podem beneficiar inúmeras pessoas deficientes, mesmo que apresentem uma anatomia diferenciada, sobretudo os cadeirantes por passarem vários períodos sentados, e esta tecnologia assistiva possibilita a redução dos riscos de lesões teciduais por aumentarem a área de contato, e assim distribui com eficácia a temperatura e peso corporal. / Assistive technology is the currently used term to define a wide range of services and products aimed at disabled persons. Design for assistive technology devices is seen in two different technology pathways, with serial and personalized products. These devices seek to optimize impaired body functions and, thus, reinsert these persons in various social activities. This study aims to contribute to the construction of new technology ruts for the production of personalized seats and backrests for wheelchairs. The methodology includes the application of the concepts of three-dimensional digitalization, CNC Machining, and thermography in order to create and analyze personalized seats and backrests produced from individualized anthropometric data. The results obtained show that the proposed models are valid, because they permit a wider distribution of bodily weight and temperature through the outlines inserted in the equipments according to each person’s anatomy. The greatest contribution of this study is its simplicity of production and the low cost of materials used in the research, which may benefit numerous wheelchair users who present unique anatomies, reducing the risks of tissue lesions by enlarging the area of contact, which results in a more effective distribution of bodily temperature and weight.

Prévention du cancer colo-rectal chez le sujet à risque élevé : de la recherche observationnelle à la recherche interventionnelle / Prevention of colorectal cancer in high-risk patients : from observational to interventional research

Le but de ce travail est d'élaborer et d'évaluer une intervention de santé publique, visant à augmenter la participation au dépistage ciblé par coloscopie de la population à risque plus élevé de cancer colorectal (CCR) en raison d'antécédents familiaux en faisant appel à une méthodologie qui repose sur 3 étapes successives :1- une étude observationnelle : l'identification des déterminants associés à la réalisation de la coloscopie chez la fratrie d'un patient atteint de CCR avant 60 ans. (3 publications) 2- une expérimentation contrôlée, visant à tester l'efficacité d'une intervention personnalisée, par une infirmière de prévention, sur la participation au dépistage des fratries d'un patient atteint de CCR ou d'adénome avant 60 ans. L'intervention personnalisée qui a intégré non seulement les fratries mais également les cas index et les médecins a fait la preuve d'une efficacité jusque là jamais atteinte dans les études menées antérieurement aux USA, qui ciblaient uniquement les fratries. Cette analyse épidémiologique a été enrichie par des analyses linguistiques et sociologiques afin de mieux comprendre l'impact de cette intervention téléphonique. (1 publication)3- Cependant, les contraintes propres à la conduite d’un essai interventionnel randomisé, si elles garantissent un niveau de preuve élevé du bénéfice de l’intervention, ont aussi pour conséquence un caractère expérimental ; cet essai ne pourra être généralisé à l'identique. Une étude de recherche interventionnelle en santé des populations a été soumise à l'appel à projets 2016 de l'INCa. / The aim of this work was to develop and evaluate a public health intervention to increase participation in targeted screening colonoscopy for high-risk population of colorectal cancer (CRC) because of family history using a methodology based on three successive stages:1- an observational study to identify the determinants associated with the achievement of colonoscopy among siblings of a patient with CRC before age 60. (3 publications)2- a controlled experiment to test the effectiveness of a personalized intervention by a preventive nurse on the participation in screening of siblings of a patient with CRC or adenoma before age 60. The personalized intervention that included not only siblings but also the index case, specialists and general practitioners has demonstrated efficiency never achieved previously in earlier studies in the USA, which targeted only siblings. This epidemiological analysis was enriched by crossed linguistic and sociological analyses to better understand the impact of telephone interventions. (1 publication)3- However, the constraints on the conduct of a randomized interventional trial, if they guarantee a high level of evidence of the benefit of the intervention, also have the effect of an experimental nature that cannot be generalized. An original research study on population health intervention has been developed and submitted to the 2016 INCa call for projects.

A Personalized Smart Cube for Faster and Reliable Access to Data

Organizations own data sources that contain millions, billions or even trillions of rows and these data are usually highly dimensional in nature. Typically, these raw repositories are comprised of numerous independent data sources that are too big to be copied or joined, with the consequence that aggregations become highly problematic. Data cubes play an essential role in facilitating fast Online Analytical Processing (OLAP) in many multi-dimensional data warehouses. Current data cube computation techniques have had some success in addressing the above-mentioned aggregation problem. However, the combined problem of reducing data cube size for very large and highly dimensional databases, while guaranteeing fast query response times, has received less attention. Another issue is that most OLAP tools often causes users to be lost in the ocean of data while performing data analysis. Often, most users are interested in only a subset of the data. For example, consider in such a scenario, a business manager who wants to answer the crucial location-related business question. "Why are my sales declining at location X"? This manager wants fast, unambiguous location-aware answers to his queries. He requires access to only the relevant ltered information, as found from the attributes that are directly correlated with his current needs. Therefore, it is important to determine and to extract, only that small data subset that is highly relevant from a particular user's location and perspective. In this thesis, we present the Personalized Smart Cube approach to address the abovementioned scenario. Our approach consists of two main parts. Firstly, we combine vertical partitioning, partial materialization and dynamic computation to drastically reduce the size of the computed data cube while guaranteeing fast query response times. Secondly, our personalization algorithm dynamically monitors user query pattern and creates a personalized data cube for each user. This ensures that users utilize only that small subset of data that is most relevant to them. Our experimental evaluation of our Personalized Smart Cube approach showed that our work compared favorably with other state-of-the-art methods. We evaluated our work focusing on three main criteria, namely the storage space used, query response time and the cost savings ratio of using a personalized cube. The results showed that our algorithm materializes a relatively smaller number of views than other techniques and it also compared favourable in terms of query response time. Further, our personalization algorithm is superior to the state-of-the art Virtual Cube algorithm, when evaluated in terms of the number of user queries that were successfully answered when using a personalized cube, instead of the base cube.

To achieve this goal the following proces was selected: The first step was to create theoretical background which describes the area of enterprise portals and how it can be seen. In the following part were set main functional areas whose coverage may be assured by the enterprise portal. These areas are : presentation, personalization and customization, security, administration, content management system, search, collaboration, accessibility and portlets. Another part is focused on the selection of specific portal solutions which are evaluated and compared at the conclusion. These are chosen products : IBM WebSphere Portal, Microsoft Sharepoint Server 2007, Oracle Portal 10g, BEA WebLogic Portal. Personal benefit is particularly comprehensive look into the issue of enterprise portals. Theoretical background makes possible to quickly obtain information which are presented in a logically organizace form. It will ensure continuity in the knowledge absorption. Practical part offers look into the enterprise portal market situation and shows what can offer specific product and at what level.

Problem: Vid accepterandet av användarvillkor och cookies på Facebook får företag information om respektive användare och kan därmed skapa individanpassad reklam, något som kan uppfattas som integritetskränkande. Syfte: Analysera och beskriva användarens beteende och vidare kunskap om individanpassad reklam, via cookies, på Facebook. Teori: Den teoretiska referensramen omfattar bakgrundsinformation om Facebook, cookies och individanpassad reklam. Grundläggande teori baseras på hur företag använder sig av Facebook, cookies och individanpassad reklam för att marknadsföra sina produkter och tjänster. Metod: Studien har tillämpat en deduktiv ansats med en webbenkät som datainsamlingsverktyg. Enkäten sändes ut till 3318 programstudenter på Högskolan Dalarna, med svenska som undervisningsspråk. Resultat: Studiens resultat redovisas utifrån insamlad data från enkätundersökningen. Data sammanställdes i stapeldiagram, histogram och i tabeller med beskrivande statistik och medelvärde. Slutsats: Användaren besitter inte kunskap om vad de accepterar när de godkänner Facebooks användarvillkor och anser sig inte vara orolig över den personliga information Facebook tar del av i kommersiellt syfte. / Problem: Accepting the term of use for cookies and Facebook, gives companies information about each user of Facebook so they can create personalized advertising. Purpose: The purpose of the paper is to analyze and describe the user´s behavior and further knowledge of personalized advertising, through cookies, on Facebook. Theory: The theoretical frame includes information about Facebook, cookies and personalized advertisements. The theory is based on how companies use Facebook, cookies and personalized advertisements to promote their products and services. Method: The study has used a deductive approach with a web questionnaire as a data collection tool. The questionnaire was sent to 3318 students at Högskolan Dalarna, with Swedish as the language of instruction. Results: The results of the study is based on collected data from the survey. The data was compiled in bar charts, histograms and a table with descriptive statistics and mean. Conclusion: The user does not have knowledge of what they accept when they approve Facebook's Terms of Service and do not feel worried about the personal information Facebook delivers for commercial purposes.

Pharmacogénétique des analogues nucléosidiques : Cytidine déaminase et issue clinique / Pharmacogenetics of nucleoside analogs : cytidine deaminase and clinical outcome

La prise en charge du cancer reste dépendante de l’utilisation des agents cytotoxiques, avec les analogues nucléosidiques. Au-delà de leur similarité structurelle, certains de ces composés partagent une voie métabolique commune, où la cytidine déaminase apparaît comme enzyme majeure. L’existence d’une variabilité génétique et/ou phénotypique de la CDA nous a mené à nous intéresser aux relations entre le statut CDA et l’issue clinique des patients afin de déterminer si la CDA pouvait être considérée comme biomarqueur d’issue clinique chez les patients.Nos travaux personnels ont consisté à évaluer deux techniques permettant de mesurer l’activité de la CDA. Nous avons publié le premier cas mondial de toxicités mortelles sous azacytidine chez un patient CDA-déficient, ainsi que le premier cas de déficience en CDA et de toxicités sous cytarabine causées par la présence d’une variation génétique du gène CDA chez une patiente transplantée hépatique. L’influence du statut CDA a également été étudiée chez deux patients traités par azacytidine. Concernant la gemcitabine, nous avons démontré l’impact délétère en terme d’efficacité de l’augmentation de l’activité CDA chez les patients, ainsi que les résultats d’une étude multicentrique prospective dont le but était de déterminer si la CDA pouvait être un marqueur prédictif de l’apparition des toxicités sous gemcitabine, avec une étude pharmacocinétique en support. Les travaux préliminaires du pyroséquençage partiel de la CDA sur technologie Roche® sont présentés. L’ensemble de ces travaux de thèse confirme l’intérêt d’évaluer le statut CDA chez les patients susceptibles de recevoir une thérapie à base d’analogues nucléosidiques. / Nowadays, the management of cancer pathology remains largely dependent on the use of cytotoxic agents, including nucleoside analogs, used in a variety of settings. Beyond their structural similarity, some of these compounds also share a common metabolic pathway, wherein the cytidine deaminase (CDA) plays a pivotal role. The existence of constitutional genetic and / or phenotypic variability in CDA prompted us to study the relationships between the CDA status and clinical outcome in patients, and to determine if the constitutional CDA could be considered as a biomarker of efficacy and toxicity in patients treated with this class of drugs.Our personal work first consisted in evaluating two methods to measure the CDA enzymatic activity, in terms of robustness and cost. Then we published the first case-report of life-threatening toxicities in a CDA-deficient patient treated with azacytidine, and the first case of CDA deficiency and cytarabine-caused toxicities correlated with presence of a genetic variation in CDA gene in a liver-transplant patient. The influence of CDA status was also assessed in two patients treated with azacytidine. Regarding gemcitabine, we present the impact of an increase in CDA activity on loss of efficacy in patients, and the results of a prospective multicenter study whose purpose was to determine whether the CDA could be a predictive marker of the occurrence of gemcitabine-toxicities, with a pharmacokinetic study support. Finally, preliminary data on partial Roche®-pyrosequencing of CDA, also presented.All these thesis work confirms the interest to evaluate the CDA status in patients likely to receive a nucleosidic analogues-based therapy.

Appliqué à des problèmes actuels de recherche pharmaceutique, ce mémoire traite de la génération de signatures de biomarqueurs par une approche d'extraction de règles d'association et une Analyse Formelle de Concepts. Elle a aboutit au développement d'une méthodologie qui a été validée par six projets de recherche de signatures de biomarqueurs.Alors qu'il n'existe pas de méthode optimale pour traiter les données biomarqueurs, cette méthodologie logique s'appuie sur un scénario global d'analyse déployant quatre méthodes, chacune dépendante de procédés différents. Cette architecture qualifie une problématique centrale de manière à optimiser la qualité d'une solution aux différents problèmes scientifiques posés. Les six applications pratiques ont démontré l'intérêt de la prise en compte précoce des critères de qualité énoncés par les experts du domaine. L'interactivité est soutenue tout au long du processus de découverte et produit des résultats imprévus pour l'expert. La méthodologie s'inscrit dans la lignée des approches dédiées à la stratification systématique des individus, qui constitue le premier palier vers une médecine personnalisée. / In the framework of current intricate questions to be solved by the pharmaceutical industry, this manuscript examines the generation of biomarker signatures through an approach that combines association rules extraction and Formal Concept Analysis. It led to the development of a methodology which was validated by six research industrial projects. While there is no single optimal method to handle biomarkers datasets, this logical methodology relies on a global datamining scenario made up of four different methods. Each method utilizes different processes. This architecture qualifies global approach that helps to optimize a response to different biomarker signatures discovery problems. The six applications presented in this manuscript demonstrate the interest of an early consideration of the quality criteria are expressed by the experts in the field. The interactivity is supported throughout the process of discovery and produces unexpected results for the expert. The methodology helps the systematic stratification of individuals, which constitutes the first step towards personalized medicine.

Defining cellular and molecular mechanisms of hereditary transthyretin amyloidosis

Hereditary transthyretin amyloidosis (ATTR amyloidosis) is a multi-system protein folding disorder that results from >100 described mutations in the transthyretin (TTR) gene. In the disease, non-natively folded TTR, originally produced by the liver, travels throughout circulation and deposits extracellularly at downstream target organs. The multi-tissue etiology of the disease makes it difficult to study in vitro, while no mouse model accurately recapitulates disease pathology. Therefore, we utilized patient-specific induced pluripotent stem cells (iPSCs) to test the hypothesis that production of and exposure to destabilized TTRs results in distinct cellular and molecular changes. The liver’s contribution to the deposition of TTR at distal tissues is understudied. As a result, in Aim 1 we sought to assess the effects of destabilized TTR production on effector hepatic cells. To this end, we utilized gene editing to generate isogenic, patient iPSCs expressing either mutant or wild-type TTR. Combining this tool with single cell RNAseq, we identified hepatic proteostasis factors, including unfolded protein response (UPR) pathways, whose expression coincided with the production of destabilized TTR. Enhancing endoplasmic reticulum (ER) proteostasis within patient hepatic cells via exogenous activation of adaptive UPR signaling, we demonstrated preferential reduction in the secretion of pathogenic TTR. In turn, we demonstrated that production of disease-associated TTR correlates with expression of proteostasis factors capable of regulating TTR secretion and in turn downstream pathogenesis. ATTR amyloidosis patients exhibit extreme phenotypic variation (e.g. TTR fibril deposits at cardiac tissue and/or peripheral nerves). In Aim 2, we sought to define responses of target cell types to pathologically-diverse TTRs. To accomplish this, we profiled transcriptomic changes resulting from exposure to a variety of destabilized TTRs to determine 1) target cell response to TTR exposure and 2) how this response changes across diverse variants and cell types. In doing so, we found that TTR exposure elicits distinct variant- and cell type-specific transcriptional responses. Herein, we addressed our central hypothesis by profiling destabilized TTR production within hepatic cells and TTR exposure at target cell types. Collectively, these data may result in the discovery of unidentified and potentially druggable pathologically-associated pathways for ATTR amyloidosis and other systemic amyloid diseases.
