A terapia laser de baixa intensidade (LLLT) é capaz de modular a resposta inflamatória em vários modelos experimentais, reduzindo o infiltrado de células do sistema imune e a secreção de mediadores químicos. Estudos prévios sugerem sua utilidade também no tratamento da lesão pulmonar aguda (LPA), condição caracterizada por intensa reação inflamatória em vias aéreas distais. Assim, considerando os efeitos anti-inflamatórios do laser e a fisiopatologia da LPA, formulamos a hipótese de que a LLLT 660 nm, 10 J/cm2, poderia modular o processo inflamatório nessa patologia favorecendo a transição inflamação-reparo. Foram utilizados camundongos C57BL distribuídos aleatoriamente em 4 grupos: PBS- instilado com PBS intratraqueal; PBS+LLLT- irradiado com LLLT 6 horas após instilação intratraqueal de PBS; LPS- instilado com LPS (5 mg/kg); LPS+LLLT- irradiado com LLLT 6 horas após instilação intratraqueal de LPS. Observou-se que a instilação de LPS intratraqueal levou a um aumento do número de células inflamatórias tanto na região peribronquiolar quanto no lavado broncoalveolar (LBA), detectado na histologia pulmonar e pela expressão de F4/80. Houve também aumento da transcrição e secreção de citocinas (TNF-alfa, IL-1beta, IL-6, IL-10) e quimiocinas (MCP-1) (detectadas por PCR quantitativo e ELISA). A aplicação de LLLT 6 horas após a instilação de LPS causou uma significativa diminuição tanto no influxo de células inflamatórias quanto na secreção de mediadores. Em seguida, foram caracterizadas as populações de macrófagos presentes no tecido pulmonar e observou-se nos animais do grupo LPS+LLLT, comparados ao grupo LPS, uma diminuição na expressão de mRNA de HIF-1alfa e aumento para arginase 1, sugerindo um predomínio da população de macrófagos M2. There are some previous studies suggesting also useful in the treatment of acute lung injury (ALI), a condition characterized by an intense inflammatory reaction in the distal airways. Considering the anti-inflammatory effects of laser and the pathophysiology of ALI, we hypothesized that LLLT 660 nm, 10 J/cm2, could modulate the inflammatory process in this disease favoring the transition between inflammation and repair. C57BL mice were divided into four groups: PBS - intratracheal instillation of PBS; PBS+LLLT - irradiated with LLLT 6 hours after intratracheal instillation of PBS; LPS - intratracheal instillation of LPS (5 mg/kg); LPS+LLLT - irradiated with LLLT 6 hours after intratracheal instillation of LPS. Animals were sacrificed 24 hours after injury. Intratracheal LPS inoculation induced a marked increase in the number of inflammatory cells in perivascular and alveolar spaces (detected by lung histology and in bronchoalveolar lavage), and F4/80 expression (macrophage marker). There was also an increase in the expression and secretion of cytokines (TNF-alfa, IL-1beta, IL-6, IL-10) and chemokine (MCP-1) detected by ELISA and quantitative PCR. The LLLT application 6 hours after LPS induced a significant decrease in both of the inflammatory cells influx and mediators secretion. Then, we characterized the macrophages populations in lung tissue and we observed that in the LPS+LLLT group the expression of HIF-1alfa mRNA was reduced while arginase 1 was increased, which suggested that the M2 macrophages were predominant. The LLLT effect was also tested on peritoneal macrophage population in culture stimulated with LPS and there was decreased of cytokines and chemokine production, however any difference in polarization markers was found

Best current evidence on chest physiotherapy in non-ventilated paediatric patients (0 to 24 months) with bronchiolitis : a systematic reviewHuman, Anri 20 June 2011 (has links)
Title Best current evidence on chest physiotherapy in non-ventilated paediatric patients (0 to 24 months) with bronchiolitis: a systematic review. Purpose To determine the current scientific evidence for using three chest physiotherapy modalities namely percussion, postural drainage and suctioning in paediatric patients (0 to 24 months). Relevance The field of cardiopulmonary physiotherapy seems to be a neglected area in physiotherapy, with a subsequently limited evidence base. The author observed that in various clinical settings physiotherapists tend to administer routine chest physiotherapy to paediatric patients with bronchiolitis. Findings from this study may assist physiotherapists in their choice of effective treatment options. Sources The following databases were searched for evidence: African Health Line, CINAHL, Cochrane, Ebsco Host, Emerald Host, UP E-theses/dissertations, PEDro, Medline Ovid, Sabinet, Science Direct, Up To Date. Methodology This was a systematic review. The databases were reviewed by making use of a specified search strategy customised for each database. Keywords were: physiotherapy/physical therapy, bronchiolitis and paediatric/pediatric in combination with percussion, postural drainage and suction. The search yielded 10,016 study titles. Studies were chosen from the population of studies using pre-set inclusion and exclusion criteria. These criteria were applied to the titles, abstracts and full-text articles as appropriate. Five full text-articles were appraised and based on the scores from the appraisal three were included in the final sample. Data analysis Appraisal instruments from the National Health System Critical Appraisal Skills Programme (NHS CASP) and the PEDro scale (for randomised controlled trials) were used to evaluate and score the sample. Scoring was done independently by two researchers, and agreement reached through negotiation. The evidence was synthesised and graded according to the Sackett hierarchy of evidence. Results Owing to the heterogeneity of the sample, and the nature of results reported, a meta-analysis was not possible. Results from this study reveal that there is no evidence to support routine chest physiotherapy in uncomplicated viral bronchiolitis amongst the paediatric population. Chest physiotherapy does not decrease length of hospital stay, oxygen requirements or clinical scores indicating distress/morbidity. However, with secondary bacterial respiratory infections, chest physiotherapy may be indicated, depending on the assessment of each individual patient. Conclusion Percussion, postural drainage and suctioning are not effective in the management of bronchiolitis in children, newborn to 24 months old, except in individually assessed cases with secondary bacterial infection. In this subset, physiotherapy must be customised to the patient. Therefore routine physiotherapy is not indicated. Implications Chest physiotherapy should be based on a complete evaluation and on clinical merit, as well as on evidence and patient preference. Education of physiotherapy students at universities as well as doctors regarding the current evidence for chest physiotherapy in paediatric bronchiolitis is essential. Doctors and clinicians need to be made aware that routine chest physiotherapy for paediatric patients with bronchiolitis should not be prescribed. No / To investigate differential item functioning or item bias of the Rivermead Mobility Index (RMI) and its impact on the drawing of valid comparisons with the RMI between subgroups of patients after stroke who differ with respect to age, sex, or side of lesion. DESIGN: Cross-sectional study. SETTING: A rehabilitation center in the Netherlands and 2 stroke rehabilitation units and the wider community in the United Kingdom. PARTICIPANTS: The RMI was completed for patients undergoing rehabilitation after stroke (N=620; mean age +/- SD, 69.2+/-12.5y; 297 [48%] men; 269 [43%] right hemisphere lesion, and 304 [49%] left hemisphere lesion). INTERVENTIONS: Not applicable. MAIN OUTCOME MEASURES: Mokken scale analysis was used to investigate differential item functioning of the RMI between subgroups of patients who differed with respect to age (young vs older), sex (men vs women), and side of stroke lesion (right vs left hemisphere). RESULTS: No differential item functioning was found for any of the comparison subgroups. CONCLUSIONS: The RMI allows valid comparisons to be made between subgroups of patients undergoing rehabilitation after stroke who differ with respect to age, sex, or side of lesion.
