Comparação dos resultados estéticos e de qualidade de vida entre pacientes submetidas à ressecção segmentar e à cirurgia oncoplástica no câncer de mama

As cirurgias conservadoras da mama (CC) podem resultar em deformidades e assimetrias que requerem correções futuras em cerca de 1/3 dos pacientes. Isto pode ter impacto negativo na qualidade de vida (QV). A cirurgia oncoplástica (COP), ao associar técnicas de mamoplastia, tem mudado as perspectivas da CC. No entanto, o número de estudos comparativos ainda é limitado. Portanto, este estudo teve como objetivo avaliar os resultados estéticos e a QV de pacientes com câncer de mama submetidas à ressecção segmentar e à COP. Dessa forma, foram incluídas 122 pacientes neste estudo transversal e multicêntrico, sendo 57 delas submetidas à COP (46,7%) na Unidade de Mama do Hospital Nossa Senhora das Graças, em Curitiba, e 65 submetidas à ressecção segmentar (53,3%) nos Hospitais Unimed, Regina e Municipal, em Novo Hamburgo. A avaliação estética foi realizada por quatro cirurgiões independentes e com experiência em reconstrução mamária, bem como pela aplicação do software BCCT.core. As pacientes também avaliaram o seu próprio resultado estético. A QV foi avaliada com a aplicação do SF-36. As pacientes da COP apresentaram uma proporção significativamente maior de resultados estéticos excelentes, classificados tanto pelo software (p=0,028) quanto pelos especialistas (p=0,002). No grupo ressecção segmentar, observou-se associação da idade, localização tumoral e tamanho das mamas com os resultados insatisfatórios. Não houve diferença significativa na QV entre os grupos. Apesar dos resultados considerados como excelentes terem sido superiores na COP, e da definição de sítios tumorais que podem ser considerados de risco para resultado estético insatisfatório na cirurgia conservadora clássica, bem como a relação com o volume mamário e a idade, nesta série os resultados estéticos insatisfatórios, não comprometeram a QV. Estudos com séries maiores de pacientes são necessários para definir melhor os grupos de pacientes onde a COP deve ser considerada como a primeira escolha na minimização da mutilação e melhoria da QV. / Breast conserving surgery (BCS) can result in deformities and asymmetries that require future corrections in around 1/3 of patients. Nevertheless, the number of comparative studies is still limited. The patients also did a selfevaluation of their own aesthetic results. The QOL was evaluated with the use of SF-36. In the lumpectomy group, an association of age, tumor location and size of breasts with unsatisfactory results was observed. There was no significant difference as to QOL among the groups. Although the results considered as excellent have been higher in the OP group, and the definition of tumor sites have been considered risky for the aesthetic results in the classical conserving surgery, as well as concerning the relation between breast volume and age, in this series the unsatisfactory aesthetic results did not commit the QOL. Studies involving larger groups of patients are needed, so we can better define the groups of patients in which the OP must be considered as the first choice to minimize mutilation and improve the QOL.

A abdominoplastia em ancora auxilia na melhora das deformidades resultantes da perda de peso. OBJETIVO: Avaliar a qualidade de vida e a capacidade funcional em mulheres submetidas a abdominoplastia em ancora apos cirurgia bariatrica. METODOS: Medical Outcomes Study 36-Item Short-Form Health Survey e Health Assessment Questionnaire foram utilizados para mensuracao prospectiva de qualidade de vida e capacidade funcional em 30 mulheres submetidas a abdominoplastia em ancora apos cirurgia bariatrica. As medidas de qualidade de vida foram realizadas no pre-operatorio e nos pos-operatorios de tres e seis meses, e comparadas com Grupo Controle. O indice de significancia estatistica adotado foi 0,05. RESULTADOS: O dominio Capacidade Funcional do Medical Outcomes Study 36-Item Short-Form Health Survey evoluiu com melhora entre o pre-operatorio e o pos-operatorio de seis meses (p=0,047) e entre o pos-operatorio de tres meses e o pos-operatorio de seis meses (p=0,009), sem diferenca entre os grupos Abdominoplastia e Controle (p=0,442). O dominio Dor Corporal do Medical Outcomes Study 36-Item Short-Form Health Survey evoluiu com melhora entre o pre-operatorio e o pos-operatorio de seis meses (p=0,022), sem diferenca entre os grupos Abdominoplastia e Controle (p=0,723). O Health Assessment Questionnaire nao apresentou diferenca estatistica nos periodos avaliados, nem entre os grupos Abdominoplastia e Controle. CONCLUSAO: A abdominoplastia em ancora nao alterou a qualidade de vida e a capacidade funcional em mulheres apos a cirurgia bariatrica / INTRODUCTION: Bariatric surgery has allowed for weight loss with consequent excess skin and subcutaneous tissue. The anchor-line Abdominoplasty helps in the improvement of deformities resulting from weight loss. OBJECTIVE: Evaluate the quality of life and functional ability in women submitted to anchor-line Abdominoplasty after bariatric surgery. METHODS: Medical Outcomes Study 36-Item Short-Form Health Survey and the Health Assessment Questionnaire were used to measure prospective quality of life and functional ability in 30 women submitted to anchor-line Abdominoplasty after bariatric surgery. The quality of life measurement was held in the preoperative and in the postoperative of three and six months, which was compared with the Control Group. The statistical significance level adopted was 0.05. RESULTS: The Physical Functioning domain of the Medical Outcomes Study 36-Item Short-Form Health Survey evolved with improvement between the preoperative and postoperative period of six months (p=0.047) and between the postoperative of three months and the postoperative of six months (p=0.009), with no difference between the Abdominoplasty and Control Group (p=0.442). The Bodily Pain domain of the Medical Outcomes Study 36-Item Short-Form Health Survey evolved with improves between the preoperative and postoperative period of six months (p=0.022), with no difference between the Abdominoplasty and Control Group (p=0.723). The Health Assessment Questionnaire presented no statistical difference in the evaluated periods, or between the Abdominoplasty and Control Group. Prevalência de sintomas do transtorno dismórfico corporal em abdominoplastia, rinoplastia e ritidoplastia / Prevalence of symptoms of body dysmorphic disorder in abdominoplasty, rhinoplasty, and rhytidoplasty

No. of bitstreams: 0 Previous issue date: 2011-07-27 / INTRODUÇÃO: O transtorno dismórfico corporal (TDC), e a comorbidade com outros transtornos mentais podem comprometer a percepção da forma de partes do corpo levando à insatisfação do paciente que, por vezes, procura o cirurgião plástico. OBJETIVO: Identificar o grau de insatisfação com a aparência física e a prevalência de sintomas do TDC em população candidata a três procedimentos de cirurgia plástica. MÉTODOS: Distribuiuse 300 pacientes de ambos os gêneros, em três grupos: abdominoplastia (n=90), rinoplastia (n=151) e ritidoplastia (n=59). Cada grupo foi dividido em dois subgrupos: pacientes sem sintomas do TDC (n=39; n=72; n=34) e com sintomas (n=51; n=79; n=25), segundo o Body Dysmorphic Disorder Examination (BDDE), administrado no pré-operatório. RESULTADOS: A prevalência de sintomas do TDC nos pacientes do grupo abdominoplastia foi de 57%, no grupo rinoplastia 52% e no grupo ritidoplastia foi de 42%. Houve associação entre sintomas do TDC, grau de insatisfação corporal e inibição da sexualidade, nos três grupos cirúrgicos. O grupo abdominoplastia revelou mais insatisfação com a aparência e maior proporção de sintomas do TDC, do que os outros grupos. No grupo ritidoplastia identificou-se maior proporção de sintomas graves (64%), quando comparado ao de abdominoplastia (59%) e rinoplastia (38%). CONCLUSÃO: Os pacientes candidatos aos três procedimentos de cirurgia plástica apresentaram prevalência alta de sintomas de TDC. O grupo abdominoplastia revelou maior grau de insatisfação corporal e mais sintomas de TDC, o grupo rinoplastia apresentou menor proporção de casos graves e o grupo ritidoplastia maior proporção de casos graves. / INTRODUCTION: Body dysmorphic disorder (BDD) and comorbid mental disorders may compromise the visual perception of body parts, causing dissatisfaction in patients, who may consult a plastic surgeon. OBJECTIVE: To identify the level of dissatisfaction with appearance and prevalence of BDD symptoms in a population of candidates to three plastic surgical procedures. METHODS: Three-hundred patients of both sexes were allocated into three groups: abdominoplasty (GA) (n=90), rhinoplasty (GR) ( n=151), and rhytidoplasty (Grit) ( n=59). Each group was subdivided into two sub-groups: patients without BDD symptoms (n=39; n=72; n=34) and patients with BDD symptoms (n=51; n=79; n=25), according to the Body Dysmorphic Disorder Examination (BDDE), which was administered preoperatively. RESULTS: The prevalence of BDD symptoms in GA, GR and GRit was 57%, 52%, and 42%, respectively. Significant associations were found between BDD symptoms, degree of body dissatisfaction, and sexuality inhibition in the three groups. GA patients were the most dissatisfied with appearance and had the greatest number of BDD symptoms compared with the other groups (GR, GRit). GRit had the greatest percentage of patients with severe symptoms (64%), followed by GA (59%) and GR (38%). CONCLUSION: The candidates to the three plastic surgical procedures had high prevalence of BDD symptoms. GA showed the highest degree of body dissatisfaction and greatest number of BDD symptoms, GR had the lowest percentage severe cases, and Grit had the highest percentage of severe cases. / TEDE / BV UNIFESP: Teses e dissertações

Reservatório ileal continente : uma opção viável para ampliação vesical e derivação urinária

OBJETIVO: Apresentar os resultados da técnica de derivação urinária continente descrita por Macedo, em relação à continência, achados operatórios e complicações cirúrgicas. MÉTODOS: De janeiro de 2006 a novembro de 2016, 29 pacientes foram submetidos à técnica de Macedo. Dados demográficos, tempo de hospitalização, tempo cirúrgico, tempo de seguimento, taxa de continência, capacidade do reservatório e complicações pós-operatórias foram avaliados. RESULTADOS: Sessenta e nove por cento eram masculinos e a mediana de idade foi de 16,9 anos. A etiologia principal foi meningomielocele (69,1%). A média do tempo cirúrgico foi 4,2 h (DP 0,9 2,9-6,3). A mediana do tempo de internação foi 10 dias (IIQ: 11,3; 5-51). A média de seguimento foi 3,3 anos (DP 2,2 0,3 – 9,8). Procedimento no colo vesical foi realizado em 12 pacientes (41,3%). A taxa de continência do conduto cateterizável foi de 82,8%. A capacidade do reservatório aumentou de 134,4 para 364,4 ml (p <0.0001). A taxa de continência melhorou significativamente (20 vs. 74%, p <0.0001). Não houve mudança na taxa de filtração glomerular a longo prazo (143.1 vs. 147 ml/min, p = 0.45). Taxa de morbidade foi 58% (25 complicações em 17 pacientes), 72% ocorreram nos primeiros 60 dias e 60% foram classificadas Clavien-Dindo I ou II. CONCLUSÃO: Nossos resultados em relação a taxas de continência, tempo cirúrgico e complicações demonstram que a enterocistoplastia de Macedo é viável, reprodutível e com bons resultados. / OBJECTIVE: To present the results of technique of continent urinary diversion, described by Macedo, in relation to continence, operative findings and postoperative complications. METHODS: From January 2006 to November 2016, 29 patients were underwent to urinary diversion by Macedo’s technique. Patients demographics, hospitalization time, surgical time, follow up, continence rate, reservoir capacity and postoperative complications were evaluated. RESULTS: Sixty nine percent were male and the median age was 16.9 years. The main etiology was meningomyelocele (69.1%). The mean surgical time was 4.2 hours (SD 0.9 range 2.9-6.3). The median length of hospital stay was 10 days (IQR: 11.3 range 5-51). The mean follow up was 3.3 years (SD 2.2 range 0.3 - 9.8). Procedure in the bladder neck was performed in 12 patients (41.3%). The continence rate of the catheterizable conduit was 82.8%. The reservoir capacity increased from 134.4 to 364.4 ml (p <0.0001). The continence rate improved significantly (20 vs. 74%, p <0.0001). There was no change in glomerular filtration rates in the long term (143.1 vs. 147 ml/min, p = 0.45). Morbidity rate was 58% (25 complications in 17 patients), 72% occurred within the first 60 days and 60% were classified as Clavien-Dindo I or II. CONCLUSION: Our results regarding continence rates, surgical time and complications demonstrated that Macedo’s enterocystoplasty is feasible, reproducible and with good result.

Desenvolvimento de programa educacional multimídia para ensino-aprendizagem de retalho cutâneo

The goal of this study is to develop and validate a multimedia software that assists in the teaching-learning rhomboid flap process. Fifty students from the fifth and sixth year of medical school completed a pretest and were randomly divided into two groups of 25 students each. For five minutes the control group studied a printed chapter of a standard book while the test group used a computer multimedia software, describing how to perform a rhomboid flap. Each group was evaluated in a blind manner by board certified surgeons (Brazilian Society of Plastic Surgery) using OSATS protocol (Objective Structured Assessment of Technical Skill) and answered a post-test. The control group was assessed a second time after studying for 5 minutes with the software. The group that used the multimedia software achieved higher scores on the practical performance and the post-test as compared to the control group. All students subjected to both methods rated the software as their choice method of learning. / Introdução: Durante o processo de ensino e aprendizagem em cirurgia, deve-se levar em conta a necessidade de oferecer metodologia de ensino e treinamento eficientes, de fácil acesso e com boa relação custo-benefício, evitando conflitos éticos em relação aos pacientes e modelos animais. O objetivo desse estudo é desenvolver e validar um software multimídia que auxilie no processo de ensino-aprendizagem do retalho rombóide.Método: 50 alunos voluntários do quinto e sexto anos do curso de medicina responderam a um pré-teste e foram randomicamente divididos em dois grupos de 25 alunos cada, o grupo controle foi exposto um capítulo de livro impresso, enquanto o grupo teste utilizou um software multimídia, descrevendo como realizar um retalho rombóide. Cada grupo confeccionou um retalho em modelo artificial de treinamento(bench model) sendo avaliado de forma cega por cirurgiões membros da SBCP (Sociedade Brasileira de Cirurgia Plástica) através do protocolo OSATS (Objective Structured Assessment of Technical Skill) e respondeu a um pós-teste. O grupo controle foi retestado num segundo momento utilizando o software. Resultados: Houve uma superioridade quanto ao desempenho do grupo software multimídia confirmada pelos resultados da lista de checagem (p < 0,002), avaliação geral global (p = 0,017) e pós-teste (p < 0,001).Conclusões: Alunos que utilizam CAL apresentam melhor desempenho subjetivo e objetivo na confecção de retalhos rombóides, bem como a classificaram como a melhor ferramenta de estudo.

Reservatório ileal continente : uma opção viável para ampliação vesical e derivação urinária

OBJETIVO: Apresentar os resultados da técnica de derivação urinária continente descrita por Macedo, em relação à continência, achados operatórios e complicações cirúrgicas. MÉTODOS: De janeiro de 2006 a novembro de 2016, 29 pacientes foram submetidos à técnica de Macedo. Dados demográficos, tempo de hospitalização, tempo cirúrgico, tempo de seguimento, taxa de continência, capacidade do reservatório e complicações pós-operatórias foram avaliados. RESULTADOS: Sessenta e nove por cento eram masculinos e a mediana de idade foi de 16,9 anos. A etiologia principal foi meningomielocele (69,1%). A média do tempo cirúrgico foi 4,2 h (DP 0,9 2,9-6,3). A mediana do tempo de internação foi 10 dias (IIQ: 11,3; 5-51). A média de seguimento foi 3,3 anos (DP 2,2 0,3 – 9,8). Procedimento no colo vesical foi realizado em 12 pacientes (41,3%). A taxa de continência do conduto cateterizável foi de 82,8%. A capacidade do reservatório aumentou de 134,4 para 364,4 ml (p <0.0001). A taxa de continência melhorou significativamente (20 vs. 74%, p <0.0001). Não houve mudança na taxa de filtração glomerular a longo prazo (143.1 vs. 147 ml/min, p = 0.45). Taxa de morbidade foi 58% (25 complicações em 17 pacientes), 72% ocorreram nos primeiros 60 dias e 60% foram classificadas Clavien-Dindo I ou II. CONCLUSÃO: Nossos resultados em relação a taxas de continência, tempo cirúrgico e complicações demonstram que a enterocistoplastia de Macedo é viável, reprodutível e com bons resultados. / OBJECTIVE: To present the results of technique of continent urinary diversion, described by Macedo, in relation to continence, operative findings and postoperative complications. Retalho miocutâneo de peitoral maior na reconstrução dos defeitos da cabeça e pescoço: estudo anatômico / Pectoralis major myocutaneous pedicled flap for head and neck defects reconstruction: anatomic study

Objetivos: Avaliar se o comprimento do pedículo do retalho miocutâneo de peitoral maior assim como seu alcance para diversos sítios da região cervicofacial são influenciados por dados antropométricos e pelo lado de dissecção do retalho. O trabalho busca também determinar se a rotação infraclavicular proporciona ganho significativo quanto ao alcance do retalho em comparação à rotação supraclavicular. Delineamento: Estudo prospectivo anatômico em cadáveres e em pacientes tipo transversal. Materiais: Foram estudados 50 retalhos miocutâneos de peitoral maior em 25 cadáveres adultos não formolizados com menos de 24 horas de óbito fornecidos pelo Serviço de Verificação de Óbitos da Universidade de São Paulo e, a seguir, 15 pacientes submetidos à reconstrução de defeitos cervicofaciais com este retalho. Métodos: Padronizou-se em todos os casos estudados uma ilha de pele quadrangular, medindo 8x6 cm (altura x largura) localizada sobre a porção esternocostal do músculo peitoral maior, medialmente ao mamilo e tendo como limite caudal a borda superior da sétima costela. Todos os retalhos foram baseados apenas no ramo peitoral da artéria toracoacromial e a rotação foi realizada inicialmente sobre a clavícula. O comprimento do pedículo foi medido após a rotação, do ponto médio clavicular até o nível da borda superior da ilha de pele. Testou-se o alcance do centro da ilha de pele do retalho para as seguintes regiões: proeminência laríngea da cartilagem tireoide, mento, ângulo da mandíbula, conduto auditivo externo e órbita. Analisou-se a relação do comprimento do pedículo e do alcance do retalho com dados antropométricos e com o lado de dissecção. Posteriormente, realizou-se a rotação do retalho por baixo da clavícula apenas nos cadáveres e testou-se novamente seu alcance para as mesmas regiões, comparando com os resultados da rotação supraclavicular. Resultados: Nos cadáveres, o comprimento do pedículo apresentou um valor médio de 17,67 ± 2,24 cm, já para os pacientes, encontramos uma média de 16,03 ± 1,35 cm. Todos os retalhos obtiveram alcance para todas as regiões estudadas, com exceção da órbita, que foi alcançada nos cadáveres em 20 casos por rotação supraclavicular (40%) e 21 casos por rotação infraclavicular (42%). Nos pacientes, a óbita foi alcançada em 13,3% dos casos. Nos cadáveres, a rotação infraclavicular não apresentou ganho significativo para o alcance à órbita (p=0,839 - qui-quadrado) nem para as outras regiões estudadas, embora tenha havido um ganho de 0,61 cm na média do comprimento do pedículo, dado com significância estatística (p=0,01; pela Correlação de Person). Considerando os cadáveres, na análise univariada, foi observado que houve diferença estatisticamente significativa para o alcance do retalho à órbita em indivíduos com maior distância acrômiotrocantérica - DAT (p= 0,008 - teste \"t\" de Student), maior distância biacromial - DBA (p= 0,024 - teste \"t\" de Student) e menor valor da razão entre a distância mastoide-fúrcula esternal pela distância acrômio-trocantérica - DMF/DAT (p= 0,005 - teste \"t\" de Student). Também se observou que os cadáveres cujos retalhos alcançaram a órbita, apresentavam peso estatisticamente superior (p=0,036 - teste \"t\" de Student). Em relação ao comprimento do pedículo, na análise univariada, houve correlação positiva e estatisticamente significativo entre o comprimento do pedículo e a distância biacromial - DBA (r= 0,311; p= 0,028 - correlação de Pearson); correlação negativa e também significativo com a razão entre a distância mastoide-fúrcula esternal pela distância biacromial - DMF/DBA (r= -0,362; p= 0,010 - correlação de Pearson) e com a razão entre a distância mastoide-fúrcula esternal pela distância acrômio-trocantérica - DMF/DAT (r= -0,403; p= 0,004 - correlação de Pearson). Já nos pacientes, a análise univariada mostrou correlação positiva e estatisticamente significativo entre o comprimento do pedículo e o comprimento do esterno - CE (r= 0,722; p= 0,002 - correlação de Sperman) e correlação negativa e também significativo com a razão entre a distância mastoide-fúrcula esternal pelo comprimento do esterno - DMF/CE (r= -0,587; p= 0,021 - correlação de Sperman). Com os resultados obtidos nesta última análise, as variáveis nos pacientes com p < 0,20 foram submetidas à análise multivariada por modelo de regressão linear, visando estabelecer as variáveis que podem determinar o comprimento do pedículo vascular do retalho. Desta forma, identificou-se o comprimento do esterno como a única variável capaz de determinar o comprimento do pedículo vascular do RMPM (p=0,004). Com base nestes dados de regressão, foi então estabelecida uma equação capaz de determinar o comprimento do pedículo vascular do RMPM (COMP) baseada no comprimento do esterno, como demonstrada a seguir: COMP = 2,54 + 0,64 X CE. Conclusões: A rotação infraclavicular do retalho miocutâneo de peitoral maior não proporciona ganho no alcance do retalho em relação à região cervicofacial em comparação à rotação supraclavicular; o alcance do retalho miocutâneo peitoral maior não é influenciado pelo lado de dissecção e por dados antropométricos; o comprimento do pedículo vascular não é influenciado pelo lado de dissecção, mas sofre influência positiva do comprimento esternal. Apesar do alcance do retalho não sofrer influência de dados antropométricos, por este modelo anatômico infere-se que a equação determinante do comprimento do pedículo pode, na prática clínica, contribuir para o planejamento de reconstruções utilizando o retalho miocutâneo de peitoral maior, sobretudo para defeitos mais craniais / Objectives: Determine whether the length of the pectoralis major myocutaneous pedicled flap and its ability to reach multiple head and neck sites are influenced by anthropometric data and by the side of flap dissection. The study is also designed to determine whether infraclavicular rotation provides a significant gain in flap reach over supraclavicular rotation. Design: Prospective, cross-sectional and anatomical study on cadavers and patients. Materials: Fifty pectoral major myocutaneous flaps were studied in fresh adult cadavers less than 24 hours after death provided by the Serviço de Verificação de Óbitos of the University of São Paulo, and later in 15 patients undergoing head and neck reconstruction using this flap. Methods: For all cases a standardized quadrangular skin island measuring 8 cm x 6 cm (height x width) was employed, located over the sternocostal portion of the major pectoralis muscle, with the nipple as the medial limit and the muscle\'s inferior border as the caudal limit. All flaps were based only on the pectoralis branch of the thoracoacromial artery and the rotation was initially performed over the clavicle. Pedicle length was measured after rotation, from the midpoint of the clavicle to the superior border of the skin island. The reach of the skin island center was tested to the following sites: laryngeal prominence of the thyroid cartilage, chin, angle of the mandibule, external auditive canal and orbit. Ratios of the length of the pedicle and the reach of the flap and the anthropometric data and dissection side were analyzed. Afterwards, only in the cadavers, the flap was rotated beneath the clavicle and its reach to the same regions was measured again, and then compared to the result obtained from supraclavicular rotation. Results: In the cadavers, the average pedicule flap length was 17.67 ± 2.24 cm, while for the patients, the average length was 16.03 ± 1.35 cm. All flaps reached all studied sites, except for the orbit, which was reached in 20 cases by supraclavicular rotation (40%), and in 21 cases beneath the clavicle (42%) in the cadavers, and in 13.3% of the patients. In the cadavers, infraclavicular rotation did not result in a significant gain in reach to the orbit or to any other studied site (P=0.839 - chi-square), although there was a statistically significant (p=001; Pearson Correlation) gain of 0.61 cm in the average length of the flap. In the univariate analysis, there was a statistically significant difference in the cadavers for the reach of the flap to the orbit in individuals with a greater acromion-trochanter distance (DAT; p= 0.008 - Student\'s t-test), greater biacromial distance (DBA; p= 0.024 - Student\'s t-test) and a smaller value for the ratio of the mastoidsuprasternal notch distance over the acromion-trochanter distance - DMF/DAT (p= 0.005 - Student\'s t-test). It was also observed that the cadavers whose flaps reached the orbit had statistically higher body weights (p=0.036 - Student\'s t-test). With regard to the length of the flap, in the univariate analysis, there was a positive and statistically significant correlation between the length of the flap and the biacromial distance - DBA (r= 0.311; p= 0.028 - Pearson correlation); a negative and statistically significant correlation with the ratio of the mastoid-suprasternal notch distance over the biacromial distance - DMF/DBA (r= -0.362; p= 0.010 - Pearson correlation) and with the ratio between the mastoid-suprasternal notch distance over the acromion-trochanter distance - DMF/DAT (r= -0.403; p= 0.004 - Pearson correlation). Whereas in the patients, the univariate analysis showed a positive and statistically significant correlation between the length of the flap and the length of the sternum (CE) (r= 0.722; p= 0.002 - Spearman correlation) and a negative and statistically significant correlation with the ratio between the mastoidsuprasternal notch distance over the length of the sternum - DMF/CE (r= - 0.587; p= 0.021 - Spearman correlation). With the results obtained in this last analysis, the variables in the patients with p < 0.20 were submitted to multivariate analysis using linear regression in an effort to establish an equation to predict the length of the vascular flap. Sternum length (p=0.004) was the only variable found that was capable of determining the length of the vascular pedicle of the PMMC flap. Based on these regression data, an equation was formulated to determine the length of the vascular pedicle of the PMMC flap (COMP) based on the length of the sternum (CE), as follows: COMP = 2.54 + 0.64 X CE. Conclusions: Infraclavicular rotation of the pectoral major myocutaneous flap does not add to the reach of the flap to the head and neck region as compared to supraclavicular rotation; the reach of the pectoral major myocutaneous flap is not influenced by the side of the dissection or by anthropometric measures; and the length of the vascular pedicle is not influenced by the side of dissection, but is positively influenced by sternum length. Although anthropometric measures do not influence the reach of the flap according to this anatomical model, it can be inferred that the determinant equation of the length of the pedicle can, in practice, contribute to the planning of head and neck reconstruction using the pectoral major myocutaneous flap, especially for more cranial defects
