However, to change the traditional care ways demands the facing of a series of epistemological, political and cultural obstacles. One of the current challenges deals with patients aggravation processes, with management ways, with devices and with professionals, as well as with the assistance network. The purpose of this thesis was to investigate how these aggravation processes has been constituted in Natal mental healthcare network, understanding its effects in the work teams and patients. Theoretical and methodological perspective used was Institutional analysis was, subsidizing the usage of concept-tools as the implication analysis, selfmanagement and self-analysis, and restitution. The research was carried out at the Natal East Sanitary District Mental Healthcare Clinic, with the participation of technicians and patients. The research procedures were: literature and document research on the attendance and the analyzed theme; attendance registers analysis; participating observation of the institutional routine for three months and field log entries; talking groups, one with the team and one with the patients. Two main discussion points are shown: 1. The mental healthcare clinic organization logic and the intersector politics; 2. The work and management processes developed at the mental healthcare clinic. The analysis show diversity in the attended demands in the service, which has favored the patients aggravation, this device and the substitute network processes. The work processes are apart from the political sphere and from the managements processes. In this sense, we show the need to reevaluate the clinic device as well as the management models adopted in the Brazilian Psychiatry Reform context / A Reforma Psiqui?trica Brasileira, atrav?s da Estrat?gia de Aten??o Psicossocial, tem objetivado construir pr?ticas de cuidado ao louco a partir de servi?os de atendimento comunit?rio que contemplem a vida concreta dos sujeitos. Entretanto, transformar os modos tradicionais de cuidados exige o enfrentamento de uma s?rie de obst?culos epistemol?gicos, pol?ticos e culturais. Um dos desafios atuais trata de processos de cronifica??o dos usu?rios, dos modos de gest?o, dos dispositivos e dos profissionais, bem como da rede assistencial. Investigar como esses processos de cronifica??o v?m sendo constitu?dos na rede de sa?de mental de Natal, compreendendo seus efeitos nas equipes de trabalho e nos usu?rios foi a proposta dessa disserta??o. A an?lise institucional foi utilizada como perspectiva te?rica e metodol?gica, subsidiando o uso de ferramentas-conceitos como a an?lise da implica??o, auto-gest?o e auto-an?lise, e restitui??o. A pesquisa foi desenvolvida no ambulat?rio de sa?de mental no distrito sanit?rio leste da cidade de Natal/RN, com a participa??o dos t?cnicos e usu?rios. Os procedimentos da pesquisa foram: pesquisa bibliogr?fica e documental sobre o servi?o e sobre a tem?tica analisada; an?lise dos registros do servi?o; observa??o participante da rotina institucional durante tr?s meses e registros no di?rio de campo; rodas de conversa, uma com a equipe e com os usu?rios. Elencamos dois pontos principais de discuss?o: 1. A l?gica de organiza??o do ambulat?rio de sa?de mental e as pol?ticas intersetoriais; 2. Os processos de trabalho e de gest?o desenvolvidos no ambulat?rio de sa?de mental. As an?lises indicam uma diversidade de demandas atendidas no servi?o que tem favorecido aos processos de cronifica??o dos usu?rios, desse dispositivo e da rede substitutiva. Os processos de trabalho est?o dissociados da esfera pol?tica e dos processos de gest?o. Nesse sentido, apontamos a necessidade de rever o dispositivo ambulatorial bem como os modelos de gest?o adotados no contexto da Reforma Psiqui?trica brasileira

Este trabalho surge dentro do projeto de pesquisa Oficinando em Rede, que estuda a utilização de ferramentas tecnológicas como dispositivo de intervenção em saúde mental. O projeto mantém uma parceria com o Centro Integrado de Atendimento Psicossocial (CIAPS), serviço da rede pública estadual que é parte da estrutura do Hospital Psiquiátrico São Pedro (HPSP) em Porto Alegre e atende a crianças e adolescentes nas modalidades de internação e ambulatório. Nesta dissertação, buscamos analisar o que acontece no encontro entre Oficinando em Rede e CIAPS, tendo como recorte privilegiado a oficina de construção de um website do CIAPS pelos trabalhadores do serviço. Apostamos que esse exercício constitui-se como um dispositivo de virtualização do CIAPS, a partir da concepção de virtualização apresentada por Pierre Lévy, que a entende como o movimento de instauração de um campo problemático que leva à produção de uma resolução, ao que o autor denomina de atualização. Procuramos mapear as formas de coletivo que são atualizadas no encontro de Oficinando em Rede e CIAPS, buscando referenciais de análise na Inteligência Coletiva de Pierre Lévy e na Individuação Coletiva de Gilbert Simondon. A metodologia utilizada é a da pesquisa-intervenção baseada nas proposições de René Lourau, Humberto Maturana e Francisco Varela e Gilbert Simondon. Lançamos mão de três linhas de análise para essa experiência: a operatividade das tecnologias, as problematizações que se virtualizaram nesse exercício e o coletivo que se atualiza a partir disso. Apontamos a potência da tecnologia como dispositivo de intervenção institucional e a resistência à institucionalização como um elemento que fomenta o coletivo que se atualiza no encontro de Oficinando em Rede e CIAPS. Uma trata do desmonte dos manicômios como aparato de agenciamento social da loucura – a Reforma Psiquiátrica Brasileira – e a invenção de uma nova proposta institucional balizada pelos CAPS; outra trata das propostas que viabilizam uma maior abertura da escola para as diferenças – as Políticas de Educação Inclusiva. Se os muros do hospício desmoronam e as portas da escola se abrem, então a escola se vê diante da oportunidade de acolher a diferença da loucura e de se deixar interpelar por essa experiência. Mas como pensar uma escola que perceba tal experiência como enriquecedora para o processo educativo e não como um peso a mais ou alguma coisa que precise ser apenas tolerada? Para tratar dessa questão teremos a psicanálise como parceira. Faremos uso de seu arcabouço teórico, de seu estilo – que permite entrelaçar a teoria com recortes de experiência – e principalmente, de sua posição discursiva, para inventar o que pretende ser a nossa contribuição: uma escola em movimento. O malestar dos laços sociais, discutido por Freud, retorna na teoria dos discursos de Lacan que os concebe como formas de fazer laços, laços que, como veremos, sempre remetem a um impossível, a alguma espécie de fracasso. O discurso do mestre, o discurso da histérica, o discurso universitário, o discurso do analista e o discurso capitalista, são discursos presentes em todas as instituições inventadas pelo homem, incluindo a escola. Nossa aposta é a de que o discurso do analista, e sua intenção de promover o fracasso a agente do discurso, seja capaz de promover um dinamismo interessante no movimento discursivo da escola, fazendo dela um lugar mais aberto às diferenças, a isso que desmonta e desconstrói o instituído. Uma escola em movimento é uma escola interpelada pela loucura e atravessada pelo discurso do analista, sendo assim, não pretende ser a solução dos mal-estares da educação, muito pelo contrário, sua inovação está exatamente em não ter a pretensão se livrar de tais mal-estares, mas sim transformá-los em motor e energia para o processo educativo. / This dissertation begins with the discussion of two up-to-date politicalphilosophic propositions. One of them is concerned with the disassembly of the phrenocomiuns, while locus of social madness agency – the Brazilian Psychiatry Reform – and the invention of a new institutional proposition, asserted by the CAPS. The other one are the propositions that leads to a broad aperture in the school for the difference – the Policies of Inclusive Education. If the walls of the hospice collapses and the school doors open, then the school sees itself in front of the opportunity to receive all the madness variance and be acquainted for such a task. But how do we think about a school that receive this enriching knowledge for education and not as a burden from the bare tolerance, and nothing more. To deal with that question we should use the psichoanalysis as a partner. It’s theoretical structure, it’s style – that allow us intertwine the theory with clippings of experience – and mainly, of its sweating discursive position, about to invent what we do have in mind to show: a school in movement. The uneasiness of the social ties, discoursed by Freud, return in theory in the speeches of Lacan, as he conceives the ways to do ties, ties that always refers to the impossible, a kind of failure. The master's speech, the histeria's speech, the universitary’s speech, the analyst's speech, and still, the capitalist's speech, speeches presents at every human institution, includes the school. Our proposition is: the analyst's speech, and it’s goal to promote the failure as the instrument of the speech, may avail a very interesting move in the speech that came from the school, building a place each time more and more wideopened to the human diversity, this is what really joint and, des-construct the established. A moving school its’ one questioned by the madness and crossed by the psychoanalyst speech, and so, do not intend to be the solution of the educational uneasiness, nevertheless, its innovation is exactly to afford such a discomfort, changing it, to the drive, the energy of the educational process.

The diploma thesis deals with the cooperation of health and social services in the field of mental health care with regard to the ongoing reform of psychiatric care in the Czech Republic. The theoretical part of this work consists of defining the concept of mental illness, also deals with social and health services in the field of psychiatric care, discusses the reform of psychiatric care and cooperation of services. The empirical part examines what the current situation in the field of cooperation of services looks like, collects suggestions and tools for better functioning of these services and defines the views of direct care workers on the situation. The empirical part is performed by the method of questionnaire survey. The aim of the diploma thesis is to find out what the current situation looks like and what tools would help to better cooperation of these services.
