SYGEPLEJERSKENS ERFARING I AT ARBEJDE MED BORGER DIAGNOSERET MED DIABETES OG DEMENS I EGET HJEM

Baggrund: Pleje af borger med diabetes og demens kræver betydelig planlægning og gennemgang, og der skal foretages løbende vurdering af patientens egenomsorgsevne, som blandt andet fødeindtag, og evnen til at administrere medicinen på en forsvarlig måde. Diabetes og demens kan manifesteres samtidigt, hvor den ene er potentielt livstruende, mens den anden forårsager et alvorligt progressivt tab af hukommelse og kognitiv funktion. Ved tilstedeværelsen af begge diagnoser, udsættes sygeplejersken for udfordringer med hensyn til at udføre sit arbejde når det kommer til at yde sygepleje interventioner til patienter, der har mistet evnen til at yde egenomsorg. Formål: Studiets formål er at undersøge hjemmesyglejerskens erfaring i at yde sygepleje til ældre borger i eget hjem som er diagnoseret med diabetes og demens. Metode: Et beskrivende kvalitativ design, hvor data blev indsamlet med interviews (n=10) og transskriberede data blev analyseret ved hjælp af en kvalitativ indholdsanalyse.Resultat: Hjemmesygeplejersken arbejder opgavecenteret frem for personcentreret, hvor Sygeplejersken udtrykker at de er hverken fagligt eller personligt er klædt på til at kunne håndtere disse udfordringer, da retningslinjerne er for generelle og er ikke tilpasset den komplekse multisyge borger. Sygeplejerskerne efterlyser mere undervisning, mere forskning og flere specialiseret sygeplejersker på arbejdspladsen som er letter tilgængelig, hvor sygeplejersken kan henvende sig ved behov. Der understreges at der også er behov for kontinuitet i arbejde da de demente borger har brug for tid og trygge rammer. Konklusion: Sygeplejerskernes erfaring er at de dagligt udsættes for udfordringer, hvor sygeplejerskens kernekompetence og faglighed bliver sat på prøve. Sygeplejerskerne udtrykker at de er hverken fagligt eller personligt klædt på til at kunne håndtere udfordringerne i forhold til kost, medicin og forebyggelse af komplikationer. De efterlyser mere forskning, uddannelse inden for diabetes og demens. Sygeplejerskerne de mener at efteruddannelse vil løfte kompetencerne som vil være til gavne for disse borger. / Background: Care for patients with diabetes and dementia requires significant consideration and ongoing review. Continuous assessment of the patient's self-care must be carried out, not only with regards to food intake but also the ability to administer the medicine in an appropriate manner. Diabetes and dementia can manifest simultaneously; the first being potentially life threatening; the second causing a serious progressive loss of memory and cognitive function. In the presence of both diagnoses, the nurse is exposed to challenges in performing their work in providing nursing interventions to patients who have lost their ability to provide self-care. Aim: To gain insight into the home nurses’ experience in providing partially compensating nursing care to citizens with the diagnosis of Diabetes and dementia in their own home. Method: An interview study conducted with 10 home nurses in different municipalities in the Danish capital Copenhagen. Data was analyzed based on a qualitative content analysis. Result: The Danish Home nurse experience in providing nursing is expressed in how the nurse describes the citizen's ability to self-care in their own home, the nurses’ experience in working preventatively and experience in providing partially compensating nursing, and with the nurses’ experience in relation to daily nursing challenges. Conclusion: The nurse is experiencing daily challenges that expose the nurses’ core competence and professionalism to the test. The nurse expresses that they are neither professionally nor personally prepared to handle these challenges. They are looking for more research, better education, and more time to do their work.

Emotional labour in the South African postgraduate supervisory process : a student perspective

To a student, postgraduate research is often characterised as a very emotional process, more often associated with negative emotion that may hinder successful and speedy completion of the postgraduate degree. The supervisory relationship may impact greatly on the emotion the student experiences. Emotional labour is the induction or suppression of emotion in order to sustain an outward appearance. Being bound in a professional and often subordinate relationship, the student may not be willing, or able to, outwardly display their emotions. Yet, the existence of, and experience of emotional labour on students in a postgraduate supervisory relationship have not been studied to date. The purpose of the study is to explore the meaning and existence of emotional labour within the postgraduate supervisory relationship from a student’s perspective. By using a qualitative, descriptive approach, in-depth information has been gathered by means of three focus groups. The focus groups consisted of students at different stages of the postgraduate supervisory process. The study resulted in the linking of a student’s perspective of the supervisory process to the emotional labour process. It was found that emotional labour is indeed present in the postgraduate supervisory process, but is dependent on the relationship between the supervisor and the student. Since emotional labour has limitedly been explored in the academic environment, it is believed that supervisors as well as students can benefit from this exploration in this fresh context. Military-Focused Leadership Talent Development: An Examination of JROTC Participation and Postsecondary Plans

Federal and state descriptions of gifted and talented services include identifying and developing leadership talent, but in many states, services are not mandated or funded. Consequently, leadership development is often left to extracurricular programs (e.g., student organizations, athletics). The Junior Reserve Officers' Training Corps (JROTC) provides school-based military-focused leadership education and opportunities to apply emergent leadership skills. This qualitative descriptive study examined leadership talent development in JROTC and the postsecondary paths participants chose. A self-report survey was distributed to graduating seniors enrolled in JROTC in Texas public high schools and semi-structured interviews were conducted with JROTC instructors across the state. The findings highlighted characteristics of students in the sample who chose to pursue military-focused education or careers after high school and themes about the experiences and key considerations related to choosing postsecondary paths. JROTC instructors supported students with differentiated development plans and information about flexible pathways to reach postsecondary goals. Students benefitted from broad definitions of success, exposure to career options, realistic self-assessment, and alignment between intentions and preparation.

The experience and communication of symptoms in advanced pancreatic cancer patients and their families

Indiana University-Purdue University Indianapolis (IUPUI) / Symptom management is the main focus of care for patients living with advanced pancreatic cancer (APC). However, evidence shows that symptom management is far from satisfactory for this population. Poorly managed symptoms have had a profound negative impact on APC patients’ and caregivers’ life. While communicating symptoms with healthcare providers is the first step to achieve effective symptom management, some studies have revealed the poor quality of symptom discussions among cancer patients, their caregivers, and healthcare providers. The purpose of this dissertation was to advance the sciences of nursing, symptom management, and patient/caregiver and provider communication in patients with APC. Chapter two, three, and four represented three sub-studies which addressed three specific aims: (1) synthesizing the current evidence regarding the symptom experience of patients with APC, (2) examining recorded healthcare encounters between patients with APC, their caregivers, and healthcare providers to better understand the symptom experiences of patients with APC as told to their healthcare providers, and (3) developing a typology describing patterns and essential elements of real discussions between APC patients/ caregivers and healthcare providers in regards to symptoms. Specifically, chapter two was an integrative review which synthesized sixteen quantitative studies (n=1630 pancreatic cancer patients) and found that pain, fatigue, and appetite loss were primary and intense symptoms experienced by patients with APC. Chapter three was a qualitative descriptive study which used content analysis to examine 37 transcripts of APC patient/caregiver-provider health encounters originally collected for a larger communication study. This study identified ten major symptom groups often described as intense, distressing, and negatively impacting their quality of life. For chapter four, thematic analysis was used to examine 37 transcripts of APC/giver and provider interactions to develop a typology to describe patterns of interactions in regards to symptoms and symptom management. Eight common patient/caregiver-provider interaction patterns regarding symptoms and symptom management were identified. These typologies can be used to enhance patient/caregiver and provider communication programs to promote patient-centered care and improve symptom management in patients with APC. Findings overall will contribute to effective symptom management as it will deepen our understanding of symptom experience and communication processes. / 2 years

Investigating Health Related Quality of Life in People with Schizophrenia

Indiana University-Purdue University Indianapolis (IUPUI) / Schizophrenia is a serious mental illness that significantly lessens health-related quality of life (HRQoL). A better understanding of HRQoL is needed to inform holistic and patient-centered treatments for schizophrenia. The purpose of this dissertation is to conduct an in-depth exploration of HRQoL in people with schizophrenia. The dissertation includes three components. The first component is an integrative literature review aimed at identifying factors associated with physical quality of life (QoL) in people with schizophrenia. The review reveals that symptoms of schizophrenia, depression, obesity or body mass index, and physical activity are associated with physical QoL in this population. The second and third components are based on illness narratives of 20 people with schizophrenia who had participated in a larger study of individuals diagnosed with serious mental illness. The second component of the dissertation is a qualitative descriptive study conducted to describe common HRQoL concerns in people with schizophrenia. The results indicate that these concerns were related to social relationships, psychiatric symptoms, psychiatric care, and employment. The third component of the dissertation is a parallel convergent mixed-methods study conducted to explore whether computerized lexical analysis (CLA) of illness narratives of people with schizophrenia can reveal their HRQoL concerns. CLA is an automatized process that counts words in text data and sorts them into pre-defined word categories. The results provide preliminary support for the potential use of CLA to efficiently assess HRQoL in people with schizophrenia as the CLA identified some of the HRQoL concerns that had been identified in the prior qualitative descriptive study. The results of the three studies reveal the complexity of the phenomenon of HRQoL in people with schizophrenia and can inform the development of strategies to identify and promote HRQoL in this population.

Problématique : La relation infirmière-personne est au cœur de la discipline et de la profession infirmière. Dans les modèles actuels de formation en approche par compétences, l’acquisition des ressources nécessaires à cette relation ne fait pas consensus et les perceptions des étudiantes n’ont pas été explorées. Objectif : Comprendre les perceptions d’étudiantes infirmières quant à l’acquisition des ressources nécessaires à la relation infirmière-personne au cours de leur formation au baccalauréat. Méthode : Cette recherche qualitative-descriptive a permis de mener quatre groupes focalisés auprès de 20 étudiantes des trois années du baccalauréat en sciences infirmières de l’Université de Montréal. Une analyse thématique inspirée de Paillé et Mucchielli (2021) a été réalisée à deux niveaux d’inférence différents. Résultats : Trois thèmes ont été identifiés : 1) la conception des étudiantes concernant la relation infirmière-personne, 2) les ressources internes et 3) les ressources externes. La conception des étudiantes se construit sur la place centrale donnée à la relation infirmière-personne, un idéal de pratique, notamment ancré sur le partenariat-patient, et des facteurs influençant cette relation. Les émotions et les expériences antérieures font partie des ressources internes. Enfin, les outils d’apprentissage et les modèles de rôles sont des ressources externes. Un diagramme permet de représenter les thèmes formant une compétence à part entière. Recommandations : Les résultats invitent les infirmières à remettre la relation infirmière-personne au cœur de leurs préoccupations. Ils encouragent particulièrement les responsables de formation à adopter des curriculums permettant de visibiliser la relation infirmière-personne et les ressources qu’elle mobilise. / Background: The nurse-patient relationship is at the heart of the nursing discipline and profession. In current models of competency-based education, the acquisition of necessary resources for this relationship is not a matter of consensus and students' perceptions were not explored. Aims: To understand the perceptions of nursing students regarding the acquisition of resources necessary for the nurse-patient relationship during their baccalaureate training. Design: This qualitative-descriptive research was conducted in four focus-groups with 20 students from the three years of the Nursing Baccalaureate program at the Université de Montréal. A thematic analysis inspired by Paillé and Mucchielli (2021) was conducted at two different levels of inference. Results: Three themes were identified: 1) students' conception of the nurse-patient relationship, 2) internal resources and 3) external resources. The students' conception is built on the centrality of the nurse-patient relationship, an ideal of practice, notably anchored on the patient-partnership, and factors influencing this relationship. Emotions and previous experiences are part of the internal resources. Finally, learning tools and role-models are external resources. A diagram represents the themes that form a competence. Recommendations: The results invite all nurses to put the nurse-patient relationship back at the heart of their concerns. They particularly encourage those responsible for education to adopt curricula that make the nurse-patient relationship and the resources it mobilizes visible.

Exporting After Trade Missions: A Qualitative Analysis of Small and Medium Enterprises

Some U.S. small and medium enterprises (SMEs) participate in trade missions but return with no results. Accordingly, some researchers question the effectiveness of these export promotion programs. The purpose of this qualitative descriptive study was to explore the experiences of SME representatives who had attended a trade mission to South Africa. The research question explored the strategies that SME leaders required to successfully export their goods and services after a trade mission using the conceptual framework of resource-based theory. Snowball sampling was used to recruit and gather interview data from 22 SMEs. Thematic analysis of interview data and document sources, inductively and deductively coded, identified themes of strategic planning processes, country briefings, reasons for being in the country, resources, barriers, positive outcomes, and export commitments. Associated with these themes, 5 stages of the trade mission process and a model of the dynamic relationships in a trade mission emerged, which include recommendations of how to effect change in the process. The results from this study are expected to inform new interventions for export promotion programs for SME exporters. This study promotes social change by preparing SMEs to export, thus building more sustainable U.S. businesses. Applying these findings can support the development of SMEs to export and become long-term exporters benefiting the businesses, employees, and their communities through improved wages and increased tax revenues.

Intergenerational Communication Strategies for Generation X Supervisors

Millennials, who by 2024 will make up approximately 34% of the U.S. workforce, will play a critical role in organizational strategies and productivity, as will the supervisors who manage them. The purpose of this multiple case study was to explore the intergenerational communication strategies that Generation X supervisors used to motivate and engage high performing millennials in the workplace. The framework for this study was Mannheim's generation theory and the 2-factor theory of motivation by Herzberg, Mausner, and Snyderman. Data were collected from parks and recreation employees in the southeastern region of the United States, including 4 Generation X supervisors who completed semistructured interviews and 2 millennial cohort focus groups. Data were transcribed, coded, and validated through member checking and methodological triangulation. The 4 themes identified were culture and socialization, relationship building and intergenerational connectedness, employee growth and development, and rewards and recognition. The findings of this research may benefit millennials, frontline supervisors, parks and recreation agencies, and leaders in other organizations by providing an understanding of generational needs. The data presented in this study may support positive social change by showing that supervisors and millennial employees can build high quality relationships within their organizations, enabling those organizations to support the communities they serve.
