Macroergonomics to Understand Factors Impacting Patient Care During Electronic Health Record DowntimeLarsen, Ethan 18 September 2018 (has links)
Through significant federal investment and incentives, Electronic Health Records have become ubiquitous in modern hospitals. Over the past decade, these computer support systems have provided healthcare operations with new safety nets, and efficiency increases, but also introduce new problems when they suddenly go offline. These downtime events are chaotic and dangerous for patients. With the safety systems clinicians have become accustomed to offline, patients are at risk from errors and delays.
This work applies the Macroergonomic methodology to facilitate an exploratory study into the issues related to patient care during downtime events. This work uses data from existing sources within the hospital, such as the electronic health record itself. Data collection mechanisms included interviews, downtime paper reviews, and workplace observations. The triangulation of data collection mechanisms facilitated a thorough exploration of the issues of downtime. The Macroergonomic Analysis and Design (MEAD) methodology was used to guide the analysis of the data, and identify variances and shifts in responsibility due to downtime. The analysis of the data supports and informs developing potential intervention strategies to enable hospitals to better cope with downtime events.
Within MEAD, the assembled data is used to inform the creation of a simulation model which was used to test the efficacy of the intervention strategies. The results of the simulation testing are used to determine the specific parameters of the intervention suggestions as they relate to the target hospitals.
The primary contributions of this work are an exploratory study of electronic health record downtime and impacts to patient safety, and an adaptation of the Macroergonomic Analysis and Design methodology, employing multiple data collection methods and a high-fidelity simulation model. The methodology is intended to guide future research into the downtime issue, and the direct findings can inform the creation of better downtime contingency strategies for the target hospitals, and possibly to offer some generalizability for all hospitals. / Ph. D. / Hospitals experience periodic outages of their computerized work support systems from a variety of causes. These outages can range from partial communication and or access restrictions to total shutdown of all computer systems. Hospitals operating during a computerized outage or downtime are potentially unable to access computerized records, procedures and conduct patient care activities which are facilitated by computerized systems. Hospitals are in need of a means to cope with the complications of downtime and the loss of computerized support systems without risking patient care. This dissertation assesses downtime preparedness and planning through the application of Macroergonomics which has incorporated discrete event simulation. The results provide a further understanding of downtime risks and deficiencies in current planning approaches. The study enhances the application of Macroergonomics and demonstrates the value of discrete event simulation as a tool to aid in Macroergonomic evaluations. Based on the Macroergonomic Analysis and Design method, downtime improvement strategies are developed and tested, demonstrating their potential efficacy over baseline. Through this dissertation, the deficiencies in current contingency plans are examined and exposed and further the application of Macroergonomics in healthcare.

Yes / Interoperability within different healthcare systems (clinics/hospitals/pharmacies)
remains an issue of further research due to a barrier in sharing of the patient’s Electronic Health
Record (EHR) information. To solve this problem, cross healthcare system collaboration is
required. This paper proposes an interoperability framework that enables a pharmacist to access
an electronic version of the patient’s prescription from the doctor using a RESTFul API with
ease. Semantic technology standards like Web Ontology Language (OWL), RDF (Resource
Description Framework) and SPARQL (SPARQL Protocol and RDF Query Language) were
used to implement the framework using JENA semantic framework tool to demonstrate how
interoperability is achieved between a pharmacy and a clinic JENA was used to generate the
ontology models for the pharmacy called pharmacy.rdf and clinic called clinic.rdf. The two
models contain all the information from the two isolated systems. The JENA reasoner was used
to merge the two ontology models into a single model.rdf file for easy querying with SPARQL.
The model.rdf file was uploaded into a triple store database created using FUSEKI server.
SPARQL Endpoint generated from FUSEKI was used to query the triple store database using a
RESTFul API. The system was able to query the triple store database and output the results
containing the prescription name and its details in JSON and XML formats which can be read

Yes / Purpose:
To identify a minimum list of metrics of international relevance to public health, research and service development which can be extracted from practice management systems and electronic patient records in primary optometric practice.
Methods:
A two stage modified Delphi technique was used. Stage 1 categorised metrics that may be recorded as being part of a primary eye examination by their importance to research using the results from a previous survey of 40 vision science and public health academics. Delphi stage 2 then gauged the opinion of a panel of 7 vision science academics and achieved consensus on contentious metrics and methods of grading/classification.
Results:
A consensus regarding inclusion and response categories was achieved for nearly all metrics. A recommendation was made of 53 metrics which would be appropriate in a minimum data set.
Conclusions:
This minimum data set should be easily integrated into clinical practice yet allow vital data to be collected internationally from primary care optometry. It should not be mistaken for a clinical guideline and should not add workload to the optometrist. A pilot study incorporating an additional Delphi stage prior to implementation is advisable to refine some response categories. / This work was supported by the College of Optometrists.

Yes / frailty is an especially problematic expression of population ageing. International guidelines recommend routine
identification of frailty to provide evidence-based treatment, but currently available tools require additional resource.
Objectives: to develop and validate an electronic frailty index (eFI) using routinely available primary care electronic health record data.
Study design and setting: retrospective cohort study. Development and internal validation cohorts were established using a randomly
split sample of the ResearchOne primary care database. External validation cohort established using THIN database.
Participants: patients aged 65–95, registered with a ResearchOne or THIN practice on 14 October 2008.
Predictors: we constructed the eFI using the cumulative deficit frailty model as our theoretical framework. The eFI score is
calculated by the presence or absence of individual deficits as a proportion of the total possible. Categories of fit, mild, moderate
and severe frailty were defined using population quartiles.
Outcomes: outcomes were 1-, 3- and 5-year mortality, hospitalisation and nursing home admission.
Statistical analysis: hazard ratios (HRs) were estimated using bivariate and multivariate Cox regression analyses. Discrimination
was assessed using receiver operating characteristic (ROC) curves. Calibration was assessed using pseudo-R2 estimates.
Results: we include data from a total of 931,541 patients. The eFI incorporates 36 deficits constructed using 2,171 CTV3
codes. One-year adjusted HR for mortality was 1.92 (95% CI 1.81–2.04) for mild frailty, 3.10 (95% CI 2.91–3.31) for moderate
frailty and 4.52 (95% CI 4.16–4.91) for severe frailty. Corresponding estimates for hospitalisation were 1.93 (95% CI 1.86–
2.01), 3.04 (95% CI 2.90–3.19) and 4.73 (95% CI 4.43–5.06) and for nursing home admission were 1.89 (95% CI 1.63–2.15),
3.19 (95% CI 2.73–3.73) and 4.76 (95% CI 3.92–5.77), with good to moderate discrimination but low calibration estimates.
Conclusions: the eFI uses routine data to identify older people with mild, moderate and severe frailty, with robust predictive
validity for outcomes of mortality, hospitalisation and nursing home admission. Routine implementation of the eFI could
enable delivery of evidence-based interventions to improve outcomes for this vulnerable group.

Exploring ways to convey medical information during digital triage : A combined user research and machine learning approachAnsved, Linn, Eklann, Karin January 2019 (has links)
The aim of this project was to investigate what information is critical to convey to nurses when performing digital triage. In addition, the project aimed to investigate how such information could be visualized. This was done through a combined user research and machine learning approach, which enabled for a more nuanced and thorough investigation compared to only making use of one of the two fields. There is sparse research investigating how digital triaging can be improved and made more efficient. Therefore, this study has contributed with new and relevant insights. Three machine learning algorithms were implemented to predict the right level of care for a patient. Out of these three, the random forest classifier proved to have the best performance with an accuracy of 69.46%, also having the shortest execution time. Evaluating the random forest classifier, the most important features were stated to be the duration and progress of the symptoms, allergies to medicine, chronic diseases and the patient's own estimation of his/her health. These factors could all be confirmed by the user research approach, indicating that the results from the approaches were aligned. The results from the user research approach also showed that the patients' own description of their symptoms was of great importance. These findings served as a basis for a number of visualization decisions, aiming to make the triage process as accurate and efficient as possible.

A disponibilidade de grandes bases de dados informatizadas em saúde tornou a técnica de relacionamento de fontes de dados, também conhecida como linkage, uma alternativa para diferentes tipos de estudos. Esta técnica proporciona a geração de uma base de dados mais completa e de baixo custo operacional. Objetivo- Investigar a possibilidade de completar/aperfeiçoar as informações da base de dados do RCBP-SP, no período de 1997 a 2005, utilizando o processo de linkage com três outras bases, a saber: Programa de Aprimoramento de Mortalidade (PRO-AIM), Autorização e Procedimentos de Alta Complexidade (APAC-SIA/SUS) e Fundação Sistema Estadual de Análise de Dados (FSeade). Métodos- Neste estudo foi utilizada a base de dados do RCBP-SP, composta por 343.306 com casos incidentes de câncer do município de São Paulo, registrados no período de 1997 a 2005, com idades que variaram de menos de um a 106 anos, de ambos os sexos. Para a completitude das informações do RCBP-SP foram utilizadas as bases de dados, a saber: PRO-AIM, APAC-SIA/SUS e FSeade. Foram utilizadas as técnicas de linkage probabilística e determinística. O linkage probabilístico foi realizado pelo programa Reclink III versão 3.1.6. Quanto ao linkage determinístico as rotinas foram realizadas em Visual Basic, com as bases hospedadas em SQL Server. Foram calculados os coeficientes brutos de incidência (CBI) e mortalidade (CBM) antes e após o linkage. A análise de sobrevida global foi realizada pela técnica de Kaplan-Meier e para na comparação entre as curvas, utilizou-se o teste de log rank. Foram calculados os valores da área sob a curva, sensibilidade e especificidade para determinar o ponto de corte do escore de maior precisão na identificação dos pares verdadeiros. Resultados- Após o linkage, verificou-se um ganho de 101,5 por cento para a variável endereço e 31,5 por cento para a data do óbito e 80,0 por cento para a data da última informação. Quanto à variável nome da mãe, na base de dados do RCBP-SP antes do linkage esta informação representava somente 0,5 por cento , tendo sido complementada, no geral, em 76.332 registros. A análise de sobrevida global mostrou que antes do processo de linkage havia uma subestimação na probabilidade de estar vivo em todos os períodos analisados. No geral, para a análise de sobrevida truncada em sete anos, a probabilidade de estar vivo no primeiro ano de seguimento antes do linkage foi menor quando comparada a probabilidade de estar vivo ao primeiro ano de seguimento após o linkage (48,8 por cento x 61,1 por cento ; p< 0,001). Conclusão- A técnica de linkage tanto probabilística quanto determinística foi efetiva para completar/aperfeiçoar as informações da base de dados do RCBP-SP. Além do mais, o CBI apresentou um ganho de 3,4 por cento . Quanto ao CBM houve um ganho de 25,8 por cento . Before and after record linkage, crude incidence (CIR) and mortality rates (CMR) were calculated. The overall survival analysis was performed using the Kaplan-Meier technique and for the comparison between curves, the log rank test was employed. In order to determine the most precise cut-off scores in identifying true matches, we calculated the area under the curve, as well as, sensitivity and specificity. Results After record linkage, it was verified a gain of 101.5 per cent for the variable address, 31.5 per cent for death date and 80,0 per cent for the date of latest information. As for the variable mother´s name, in the database of the RCBP-SP before record linkage, this information represented only 0.5 per cent , having been completed, in general, in 76,332 registries. The overall survival analysis showed that before the record linkage there was an underestimation of the probability of being alive for all periods assessed. In general, for the truncated survival at seven years, the probability of being alive at the first year of follow up before record linkage was lower when compared to the probability of being alive at the first year of follow up after record linkage (48.8 per cent x 61.1 per cent ; p< 0.001). Conclusion Both the probabilistic and deterministic record linkage were effective to complete/improve information from the database of the RCBP-SP. Moreover, the CIR had a gain of de 3.4 per cent . La thèse analyse un document intitulé Record Labeling Hearing (RLH) (1985), qui reprend dans son intégralité une séance qui s’est tenue au Congrès des États-Unis à propos du contenu jugé problématique de certains albums de musique. Au cœur de cette séance, se retrouve le Parents Music Resource Centre (PMRC), un groupe de pression qui voit le jour en 1984. D’après le PMRC, les albums avec des paroles traitant de sexe, de violence, d’occulte, de drogues et d’alcool méritent l’autocollant du « Parental Advisory : Explicit Lyrics » afin d’avertir les parents des thématiques abordées. La séance réunit des sénateurs du Congrès américain, les membres du PMRC et des alliés de ces derniers, dont la National Parent/Teacher Association (NPTA), ainsi que des représentants de l’industrie de la musique et trois artistes. Nous verrons dans la thèse qu’en fait, c’est le heavy metal que le PMRC et ses alliés visaient lors du RLH et non par exemple certains contenus transversaux à divers types d’albums de musique. A cette époque, la sous-culture du heavy metal était encore en émergence et pour tout dire bien fragile. En 1985, elle était même menacée de fragmentation en raison de la popularité de certains de ses sous-genres. Or, en présentant le heavy metal comme une contre-culture, le RLH a aidé ce genre musical à se constituer en véritable sous-culture.

Previous issue date: 2009-08-25 / No decorrer de sua vida, um paciente é atendido por várias instituições de saúde e é submetido a uma série de procedimentos. A quantidade de informações armazenadas sobre esse paciente é crescente, tanto em volume quanto em diversidade. Existem ainda diferentes identificações para um mesmo paciente, gerando alto custo com duplicação de procedimentos e colaborando com a imprecisão dos diagnósticos e tratamentos. Nesse sentido, o presente
trabalho utiliza técnicas de Record Linkage e geração de MPI (Master Patient Index), combinadas com as especificações do perfil de integração PIX (Patient Identifier Cross-Referencing), para estabelecer uma identificação única de pacientes em diferentes sistemas de informação em saúde, que contenham fontes de dados heterogêneas e distribuídas. Com a
utilização desses conceitos e tecnologias, foi especificado um projeto e desenvolvido um protótipo de um IHE (Integrating the Healthcare Enterprise)/PIX. Experimentos foram
realizados em três cenários com dados reais.
