The prescription of antimicrobial agents in the veterinary practices, without previous identification of the etiologic agent and its antimicrobial susceptibility pattern associated with the large use of antimicrobials applied in human medicine represent a potential source for the spread of resistant strains in animal reservoir. Azithromycin widely used in human medicine, presents bacteriostatic activity against several infectious agents, and recently it was released in Brazilian veterinary practices as an alternative for the treatment of urinary, respiratory, oral and skin infections. This work evaluated the activity pattern of azithromycin to different bacterial infections from pet animals, by the identification of the etiologic agent and its antimicrobial susceptibility pattern, and also determinate MIC values and evaluate cross-resistance to oxacilin and azithromycin in Staphylococcus spp., as a proposal to construct the real frame of azithromycin resistance in our region so as its use represent a reliable alternative in veterinary medicine. The samples were collected from different sites of infection in dogs and cats, like otitis externa, pyoderma, infections from the urinary, reproductive and respiratory tract, oral and conjuntival mucosas. The most prevalent bacterial agents were coagulase-positive Staphylococcus spp., followed by Entorabacteiaceae, coagulase-negative Staphylococcus spp., and Pseudomonas spp.. The isolates were most sensible to ampicilin+sulbactam, gentamicin, fluoroquinolones and some cephalosporins. The bacterial isolates showed an expressive azithromycin resistance, with exception of the isolates from urinary tract infection. Different tests were used to access the activity pattern of azithromycin, such as Disc Diffusion and Broth Microdiluition that detected 48,6% and 55% of resistants Staphylococcus spp., and 55,3% e 72,7% of resistants Gram-negative rods. Staphylococcus aureus MIC50 were 4,0 μg/mL, S. intermedius 1,0 μg/mL, coagulasenegative Staphylococcus ≥512 μg/mL and Gram-negative rods were 256 μg/mL. In nine (15%) isolates of oxacilin-resistant and mecA-positive Staphylococcus spp. were also possible detect azithromycin resistance. For this reasons, the laboratories investigations are necessary for the identification of the etiologic agents of bacterial infections and to determine its susceptibility to antimicrobials, for the best choice of treatment, and to impair the emergence of resistant bacteria that can be transmitted between animals and human, like what happens in oxacilin-resistant Staphylococcus spp.. The emergence of azithromycin resistance leads to the necessity of reliable data on the concern of the activity pattern of this antimicrobial and its contribution for the veterinary medicine practices without producing impact in development and spread of resistant strains. / A resist?ncia antimicrobiana ? um problema complexo, que envolve in?meras esp?cies bacterianas e diferentes mecanismos, podendo ter sua transmiss?o facilitada pelo estreito conv?vio entre animais de companhia e seus propriet?rios. A pr?tica de prescrever antibioticoterapia sem pr?via identifica??o do agente etiol?gico e do seu perfil de suscetibilidade associada ao amplo uso de antimicrobianos aplicados ? medicina humana representam potenciais fatores para a dissemina??o de cepas resistentes em reservat?rios animais. A azitromicina, j? amplamente utilizada no tratamento de infec??es em humanos, apresenta atividade bacteriost?tica contra uma gama de agentes infecciosos e est? dispon?vel atualmente para a cl?nica veterin?ria brasileira como alternativa de tratamento em processos infecciosos dos sistemas genito-urin?rio, respirat?rio, oral e pele. Dessa forma, o presente estudo avaliou o espectro de atividade da azitromicina, em diversos processos infecciosos bacterianos de animais de companhia, estabelecendo o pat?geno prevalente, seu perfil de suscetibilidade aos f?rmacos de elei??o, al?m de determinar a CIM da azitromicina e avaliar a poss?vel resist?ncia cruzada em Staphylococcus spp. oxacilina e azitromicina-resistentes, de forma a obter dados significativos para a constru??o do quadro real de resist?ncia ? azitromicina em nossa regi?o, para que seu uso represente uma alternativa terap?utica confi?vel em Medicina Veterin?ria. As amostras estudadas foram coletadas de diferentes processos infecciosos de c?es e gatos, tais como, otites externas, piodermas, infec??es do trato genito-urin?rio, respirat?rio, cavidade oral e da mucosa conjuntival. Os agentes bacterianos mais prevalentes foram Staphylococcus spp. coagulasepositivos, seguidos pelas enterobact?rias, Staphylococcus spp. coagulase-negativos e Pseudomas spp. De modo geral, os isolados foram mais sens?veis a ampicilina+sulbactam, gentamicina, fluoroquinolonas e algumas cefalosporinas. A azitromicina apresentou percentuais de resist?ncia significativos, com exce??o dos isolados de infec??es do trato urin?rio. Diferentes testes foram utilizados para avalia??o do perfil de atividade da azitromicina dentre estes, a Difus?o em Disco e a Microdilui??o em Caldo, detectaram resist?ncia em 48,6% e 55% dos isolados de Staphylococcus spp., respectivamente e 55,3% e 72,7% dos bastonetes Gram-negativos. A CIM50 para S. aureus foi 4,0 μg/mL, para S. intermedius 1,0 μg/mL, Staphylococcus spp. coagulase-negativos ≥512 μg/mL e bastonetes Gram-negativos 256 μg/mL. Em nove isolados (15%) de Staphylococcus spp. resistentes ? oxacilina e mecA positivos foi poss?vel tamb?m detectar resist?ncia ? azitromicina. Logo, a investiga??o laboratorial de doen?as bacterianas ? necess?ria para identificar o agente etiol?gico e determinar a suscetibilidade aos antimicrobianos a fim de selecionar o f?rmaco ideal e limitar o desenvolvimento de resist?ncia, uma vez que bact?rias resistentes podem ser transmitidas entre animais e desses para o homem, como j? observado em isolados de Staphylococcus spp. resistentes ? oxacilina. O desenvolvimento de resist?ncia a azitromicina aponta para a necessidade de estudos que forne?am dados confi?veis a respeito do perfil de atividade desse f?rmaco de forma a contribuir na terap?utica veterin?ria sem causar impacto no desenvolvimento e dissemina??o de cepas resistentes.

Therewith a deficit of scientific papers related to this activity. Considered an excellent substrate, many pathogens can be transmitted to humans through consumption of milk and its derivatives, including Staphylococcus spp., one of the main agents involved in outbreaks of food poisoning. Moreover, the increasing of antimicrobial resistance is becoming a global public health problem. Within this problematic, this study aimed to identify the species of Staphylococcus and its antimicrobial susceptibility profile of current use in human and veterinary medicine, isolated from milk of sheep origin from rural properties in the Meridional Agreste of Pernambuco. We identified 13 different species, three coagulase-positive and 10 coagulase-negative and two strains identified only as SCN, especially Staphylococcus aureus (29) followed by S. chromogenes (15) and S. intermedius (9) by its frequency. The determination of antimicrobial susceptibility by disk diffusion revealed high rates of resistance to penicillin (53.2%) and ampicillin (45.6%). The susceptibility to oxacillin was evaluated by the disk diffusion methods for oxacillin and cefoxitin, Screen Agar and determination of Minimum Inhibitory Concentration through tests of broth microdilution and agar, the latter two detected resistance in 24% and 6% of isolates, respectively. All isolates were negative for the presence of blaZ and mecA genes and was not observed a correlation between phenotypic and genotypic testing for resistance to beta-lactams. The results show that this species is a source of infection, through its products, Staphylococcus potentially pathogenic to humans. / A produ??o de leite ovino no Brasil ? uma atividade relativamente nova, cuja produ??o est? direcionada para a confec??o de queijos, iogurtes e outros derivados. Com isso h? uma defict de trabalhos cient?ficos ligados a esta atividade. Considerado um excelente substrato, muitos micro-organismos patog?nicos podem ser veiculados ao homem atrav?s do consumo de leite e seus derivados, entre eles Staphylococcus spp., um dos principais agentes envolvidos em surtos de intoxica??es alimentares. Al?m disso, o crescente aumento da resist?ncia aos antimicrobianos vem se constituindo um problema de sa?de p?blica global. Dentro desta problem?tica, o presente estudo objetivou identificar as esp?cies de Staphylococcus e seu perfil de suscetibilidade aos antimicrobianos de uso corrente em medicina humana e veterin?ria, isoladas de leite de origem ovina provenientes de propriedades rurais no Agreste Meridional de Pernambuco. Foram identificadas 13 esp?cies diferentes, sendo tr?s do grupo Staphylococcus coagulase-positivo e 10 de Staphylococcus coagulase-negativo e duas cepas identificadas apenas como SCN, destacando-se por sua frequ?ncia Staphylococcus aureus (29) seguida pelo S. chromogenes (15) e S. intermedius (9). Considerando a elevada frequ?ncia de atendimentos de c?es com otite externa bacteriana, objetivou-se identificar a frequ?ncia etiol?gica da otopatia e tra?ar o perfil de resist?ncia dos Staphylococcus Meticilina Resistentes (SMR) e enterobact?rias produtoras de Beta-Lactamases de Espectro Estendido (ESBL) em pacientes ambulatoriais e de internamento. As amostras foram submetidas a isolamento prim?rio, testes de susceptibilidade ? antimicrobianos e de resist?ncia bacteriana. Para os Staphylococcus sp. aplicou-se o Teste D (Disk Test) e o teste de Resist?ncia ? Meticilina (Met-25). Nas cepas positivas para o Met-25 foi feita Rea??o em Cadeia da Polimerase (PCR) para detec??o do gene mecA. Para estirpes positivas na PCR foi determinada a Concentra??o Bactericida M?nima (CBM) do gluconato de clorexidina. Os grupos bacterianos isolados das 140 amostras coletadas foram: Staphylococcus sp. (122 ? 49,38%), Pseudomonas sp. (22 ? 8,91%), enterobact?rias (19 ? 7,69%), Enterococcus sp. (17 ? 6,88%) e Acinetobacter sp. (12 ? 4,86%). Foram isoladas 14 cepas de SMR (11,47%) e todas continham o gene mecA. Observou-se que 14 Staphylococcus sp. foram Teste D positivo. Para os Staphylococcus coagulase positiva (SCP) a CBM foi de 500.000?g/mL (0,5%) enquanto que para os Staphylococcus coagulase negativa (SCN) o valor da CBM foi de 62.500?g/mL (0,0625%). Detectou-se 3 cepas de enterobact?rias ESBL. Os antimicrobianos mais efetivos para Staphylococcus sp. foram: Linezolida (100%), Rifampicina (94,26%) e Cloranfenicol (92,62%). Para Pseudomonas sp.: Cefepime (100%), Imipenem (100%) e Meropenem (100%). Para as enterobact?rias: Ertapenem (100%), Imipenem (100%) e Cefotaxima (84,21%). Para Enterococcus sp.: Cloranfenicol (100%), Linezolida (94,12%) e Vancomicina (88,24%). Para Acinetobacter sp.: Imipenem (100%), Meropenem (100%) e Ticarcilina com Clavulanato (91,67%). Apesar do gene mecA e de enterobact?rias produtoras de ESBL terem baixa ocorr?ncia neste estudo, existe a necessidade de um monitoramento constante em cepas isoladas de animais dom?sticos. Cultura bacteriol?gica e os testes de suscetibilidade aos agentes antimicrobianos s?o de grande import?ncia na rotina cl?nica veterin?ria para garantir o sucesso do tratamento de infec??es otol?gicas caninas. / The canine external otitis is a relevant disease regarding veterinary clinic. Considering the high frequency of treatment of dogs with external bacteria otitis, this research aims to identify the etiological frequency of otopathy and trace the resistance profile of the Methicillin Resistant Staphylococcus (MRS) and enterobacteria that produce Extended Spectrum Beta-Lactamase (ESBL) in outpatients and internment patients. The samples were submitted to primary isolation, tests of antimicrobial susceptibility and bacterial resistance. For the Staphylococcus sp. the Disk Test was applied along with the resistance test to Methicillin (Met-25). In the positive strains for Met-25, a Polimerase Chain Reaction (PCR) was applied to detect the mecA gene. For the PCR positive strains, a Minimum Bactericidal Concentration (MBC) of chlorhexidine gluconate was determined. The isolated bacterial groups of the 140 samples collected were: Staphylococcus sp. (122 ? 49.38%), Pseudomonas sp. (22 ? 8.91%), enterobacteria (19 ? 7.69%), Enterococcus sp. (17 ? 6.88%) and Acinetobacter sp. (12 ? 4.86%). 14 MRS strains (11.47%) were observed and all off them had the mecA gene. 14 Staphylococcus sp. were Disk Test positive. Concerning the positive Staphylococcus coagulase, the MBC was 500.00?g/mL (0.5%) whereas for the negative Staphylococcus coagulase the MBC value was 62.500?g/mL (0.0625%). 3 ESBL enterobacteria strains were detected. The more effective antimicrobial for the Staphylococcus sp. were: Linezolid (100%), Rifampicin (94.26%) and Chloramphenicol (92.62%). Regarding the Pseudomonas sp.: Cefepime (100%), Imipenem (100%) and Meropenem (100%). Regarding the enterobacteria: Ertapenem (100%), Imipenem (100%) and Cefotaxime (84.21%). Regarding the Enterococcus sp.: Chloramphenicol (100%), Linezolid (94.12%) and Vancomycin (88.24%). Regarding the Acinetobacter sp.: Imipenem (100%), Meropenem (100%) and Ticarcillin with Clavulanate (91.67%). Although the mecA gene and the enterobacteria that produce ESBL are briefly presented in this research, there is a need for constant monitoring of isolated strains in domestic animals. The bacteriological growth and the susceptibility tests in the antimicrobial agents are of great importance in the veterinary clinic routine in order to insure the success in treating canine otological infections.

Atualmente essa t?cnica vem sendo bastante utilizada na Europa, Estados Unidos da Am?rica e Israel, dentre outros pa?ses, entretanto, ainda n?o foi implementada rotineiramente no Brasil e n?o h? relatos de sua aplica??o utilizando larvas da mosca Chrysomya megacephala em pacientes humanos. O presente trabalho objetivou avaliar o desbridamento de ?lceras de dif?cil cicatriza??o utilizando larvas de C. megacephala. Cinco pacientes com ?lceras cr?nicas foram inclu?dos no estudo ap?s responderem a um question?rio, serem esclarecidos sobre os poss?veis riscos e benef?cios da terapia larval e assinarem o Termo de Consentimento Livre e Esclarecido ? TCLE. Antes das aplica??es, foram colhidas amostras para identifica??o das bact?rias presentes nas ?lceras. Ap?s essa etapa, as ?lceras foram avaliadas antes e durante o tratamento atrav?s de registro fotogr?fico, mensura??o de di?metros e avalia??o dos percentuais de tecido necr?tico e de granula??o. A avalia??o foi seguida da aplica??o de aproximadamente cinco larvas est?reis de 2? est?dio de C. megacephala por cm2 de les?o. Os curativos com larvas foram trocados ap?s 48 horas e com 48 horas de intervalo entre as aplica??es. As ?lceras dos pacientes inclu?dos no trabalho apresentaram car?ter polimicrobiano e em todas elas foi isolada a esp?cie Pseudomonas aeruginosa. Todos os pacientes submetidos ? terapia larval apresentaram redu??o significativa do percentual de necrose e aumento do tecido de granula??o na superf?cie das ?lceras e uma consequente melhora no decorrer do tratamento. / Larval therapy is the use of sterile larvae in the debridement of wounds. Currently this technique has been widely used in Europe, the U.S.A., and Israel, among other countries, however, has not been implemented in Brazil yet, and there are no reports of its application using larvae of the fly Chrysomya megacephala in human patients. This study aimed to evaluate the debridement of ulcers difficult to heal by using larvae of C. megacephala. Five patients with chronic ulcers were included in the study after answering a questionnaire, to be informed about the possible risks and benefits of larval therapy and signed a Free, Prior and Informed Consent. Before the applications, samples were collected for identification of the bacteria in the ulcers. After this step, the ulcers were evaluated before and during treatment by photographic recording, measurement and evaluation of diameters, percentage of necrotic tissue and granulation. The evaluation was followed by the application of approximately 5 second instar sterile larvae of C. megacephala per cm2 of lesion. Dressings with larvae were exchanged after 48 hours with 48 hours between applications. The patients? ulcers included in this study had polymicrobial nature and in all of them was isolated Pseudomonas aeruginosa species. All patients underwent larval therapy showed a reduction in the percentage of necrosis, increase of granulation tissue on the surface of ulcers and a consequent improvement during treatment.

A revis?o sistem?tica foi limitada a trabalhos publicados entre 2001 e 2014, utilizando-se dos bancos de dados LILACS, PubMed e SciELO. A avalia??o da resist?ncia de C. violaceum aos metais pesados ocorreu em concentra??es decrescentes de Fe, Mn e Zn, durante intervalos de 24, 48 e 72 horas por planejamento fatorial completo 23; o isolamento e a identifica??o microbiol?gica foram realizados conforme metodologia preconizada pelo Standard Methods for Examinations of Water and Wastewater; a avalia??o da capacidade biorremediadora de C. violaceum por espectrofotometria de absor??o at?mica (EAA), enquanto que a an?lise das unidades de registro a partir da adapta??o da metodologia de an?lise de conte?do de Bardin (2004). A biolixivia??o foi a t?cnica mais discutida; o ouro o substrato biorremediado mais citado e a cianeta??o o mecanismo fenot?pico prevalente. Mecanismos gen?ticos foram citados em 29,4% das publica??es e entre as prote?nas transcritas por C. violaceum 18,3% eram ditas hipot?ticas. Ambas as cepas de C. violaceum apresentaram resist?ncia aos metais testes em diferentes escalas de concentra??es, intera??es e intervalos de exposi??o. No entanto aquelas selvagens apresentaram uma resposta de fase aguda mais eficiente comparadas ?s padr?es, sobretudo para as concentra??es de 80 a 480 g/100ml x 10-3. A an?lise f?sico-qu?mica da ?gua revelou par?metros aceit?veis, conforme a resolu??o vigente, exceto para o ponto 5, no que tange a an?lise do pH, nesse ponto ocorreu o isolamento da C. violaceum, cuja capacidade biorremediadora aos elementos Fe, Mn e Zn mostrou-se mais eficiente quanto ?s cepas ATCC. Todos os livros did?ticos analisados, embora tenham sido recomendados pelo PNLD, apresentaram car?ncias quanto ? tem?tica biorremedia??o, fazendo-se necess?rio o desenvolvimento de materiais did?ticos que visem suprir tal abordagem. Os dados avaliados demonstram a fundamental import?ncia da C. violaceum como microorganismo biorremediador de metais pesados e evidencia a import?ncia do monitoramento ambiental, desenvolvimento e aplica??o da biorremedia??o em ambientes impactados, bem como destaca a necessidade da reformula??o da interface educa??o ambiental e biotecnologia, com a finalidade de possibilitar ao docente e ao estudante de ensino m?dio e fundamental uma reflex?o mais cr?tica sobre as rela??es que envolvem a polui??o ambiental e as poss?veis ferramentas mitigadoras dessa problem?tica. / The low water quality in the Brazilian Semiarid, in the most of cases is associated to human activities, such as the release of metallic waste from the mined industry. This study aims to compare and discuss the research evolution performed worldwide, which made use of the potential of C. violaceum for bioremediation of heavy metals; understand the bioremediation and phenotypic resistance patterns in wild and Standard strains (ATCC 12472) of heavy metals, iron (Fe), zinc (Zn) and manganese (Mn), in differents responce time, concentrations and metal interactions; evaluate the physicochemical and microbiological parameters in Engenheiro Armando Ribeiro Gon?alves reservoir, located in the county of Jucurutu (semiarid region of Rio Grande do Norte - RN); analyze the approach of the subject bioremediation in textbooks from elementary and high school appointed by 2014 and 2012 PNLD, respectively, as well as developing an educational product targeted to approaching the technical and scientific knowledge and the environment interface. The systematic review was limited to research published between 2001 to 2015, using LILACS, PubMed, and SciELO databases. The evaluation of C. violaceum resistance for heavy metals occurred in decreasing concentrations of Fe, Zn and Mn, at intervals of 24, 48 and 72 hours by a full factorial design 23; the isolation and microbiological identification were performed according to the methodology recommended by the Standard Methods for Examinations of Water and Wastewater; the evaluation the use of the potential of C. violaceum for bioremediation of heavy metals was performed by atomic absorption spectrophotometry (AAS), while the analysis of the reporting units was carried out from the adaptation of Bardin content analysis methodology (2004). The systematic survey showed that bioleaching technique was further discussed; gold was the bioremediated substrate most mentioned, and cyanidation was the prevalent phenotypic mechanism. Genetic mechanisms were mentioned in 29.4% of the publications, and among proteins transcribed by C. violaceum 18.3% were hypothetical. Whereas experimental data indicated that both strains of C. violaceum showed resistance to the metals tests on different scales concentrations, interactions and exposure intervals (p<10-5). However that wild showed a more efficient acute phase response compared to the standard, especially for metal concentrations from 80 to 480 g/100 mL x 10-3. The physicochemical water analysis revealed acceptable parameters as the current resolution, except for point five, as regards the analysis of pH, on this point was the isolation of C. violaceum, whose the use in bioremediation of Fe, Zn and Mn showed more efficient as the ATCC strain (p<10-5). Finally, the educational axis directed to the analysis of the textbooks showed that all of them, although they were recommended by PNLD, showed shortcomings regarding the subject bioremediation, making it necessary to develop educational materials aiming at to supply such an approach. The evaluated data demonstrated the fundamental importance of C. violaceum in bioremediation of heavy metals and highlights the importance of environmental monitoring, development and application of bioremediation in impacted environments and emphasizes the need to reworking the environmental education and biotechnology interface, in order to enable the teacher and high school and fundamental students to a critical reflection about the relationships involving environmental pollution and possible mitigation tools of this problem.
