Objetivo: O objetivo desse estudo foi identificar a associação entre fatores clínicos e laboratoriais com o uso de insulina em gestantes com DMG no momento do diagnóstico e analisar os possíveis fatores preditores do uso de insulina. Método: Foram estudadas, de forma retrospectiva, 294 pacientes com diabetes melito gestacional (DMG) diagnosticado por meio do teste de tolerância à glicose oral de 100 gramas (TTGO-100g) entre 24 e 33 semanas completas de gestação, cujo seguimento pré-natal foi realizado ambulatorialmente pelo setor de Endocrinopatias e Gestação da Clínica Obstétrica do Hospital das Clínicas da Faculdade de Medicina da Universidade de São Paulo, no período de 1 de julho de 2002 a 30 de junho de 2008. Os seguintes fatores clínicos e laboratoriais, que pudessem estar associados ao uso de insulina para controle glicêmico, foram analisados: idade materna, obesidade pré-gestacional - índice de massa corpórea (IMC) > 30 Kg/m2, antecedente familiar de diabetes melito (DM), tabagismo, hipertensão arterial, uso de corticosteróides sistêmicos, antecedente obstétrico de DMG e de macrossomia fetal, nuliparidade, multiparidade, antecedente obstétricos de natimortos e neomortos, idade gestacional no momento do diagnóstico, gemelidade, índice de líquido amniótico (ILA) aumentado ILA > 18 cm, polidrâmnio (ILA > 25 cm), número de valores anormais do TTGO-100g, glicemia de jejum anormal no TTGO- 100g glicemia de jejum > 95 mg/dL; média das quatro glicemias aferidas no TTGO-100g; valor da glicemia de jejum, de 1ª, 2ª e 3ª horas do TTGO-100g e hemoglobina glicada (HbA1c). A associação entre cada fator e a necessidade de insulinoterapia foi analisada individualmente (2 de Pearson / teste exato de Fisher e teste t de Student). O modelo de regressão logística para a análise multivariada foi usado para predizer a probabilidade desses fatores em relação ao uso de insulina. Resultados: Das 294 pacientes avaliadas, 39,8% (117/294) necessitaram de insulinoterapia para controle glicêmico. Observou-se correlação positiva entre o uso de insulina e obesidade pré-gestacional, antecedente familiar de DM, hipertensão arterial, antecedente obstétrico de DMG e de macrossomia fetal, número de valores anormais no TTGO-100g, glicemia de jejum > 95 mg/dL no TTGO-100g; média das quatro glicemias aferidas no TTGO-100g; valor da glicemia de jejum, de 1ª, 2ª e 3ª horas do TTGO-100g e HbA1c pela análise univariada (P<0,05). Na análise do modelo de regressão logística foram desenvolvidos dois modelos que incluíam os seguintes fatores preditores do uso de insulina: obesidade pré-gestacional, antecedente familiar de DM, número de valores anormais no TTGO-100g (só modelo 1) e valor da glicemia de jejum do TTGO-100g (só modelo 2). Os dois primeiros modelos foram novamente analisados, incluindo-se a variável HbA1c para verificação de sua contribuição na predição do uso de insulina. Curvas de probabilidade e escores foram construídos com base nas quatro combinações de fatores preditores. Conclusões: É possível estimar a probabilidade do uso de insulinoterapia para controle glicêmico em gestantes com DMG por meio de IMC pré-gestacional, antecedente familiar de DM, número de valores anormais do TTGO-100g, valor da glicemia de jejum no TTGO-100g e da HbA1c. / Objective: To determine the association between clinical and laboratory parameters and insulin requirement in pregnancies complicated by gestational diabetes mellitus (GDM), and to evaluate possible factors predicting the need for insulin therapy. Methods: A total of 294 patients with GDM diagnosed by the 100- g/3-h oral glucose tolerance test (OGTT) between 24 and 33 complete weeks of gestation were retrospectively studied. These patients were under prenatal follow-up at the Obstetric Clinic of the University of Sao Paulo School of Medicine (HCFMUSP) between July 1, 2002 and June 30, 2008. The clinical and laboratory factors which could be associated to the need for insulin therapy were analyzed: maternal age, prepregnancy obesity body mass index (BMI) > 30 Kg/m2, family history of diabetes mellitus (DM), smoking, hypertension, use of systemic corticosteroids, prior GDM, prior fetal macrosomia, nulliparity, multiparity, prior stillbirth, prior neonatal death, gestational age at diagnosis of GDM, multiple pregnancy, elevated amniotic fluid index (AFI) AFI > 18 cm, polyhydramnios (AFI > 25 cm), number of abnormal 100-g/3-h OGTT values, 100-g/3-h OGTT fasting plasma glucose > 95 mg/dL, mean of the four 100-g/3-h OGTT values, 100-g/3-h OGTT fasting/one/two/three plasma glucose values, and glycated hemoglobin (HbA1c). The association between each factor and the need for insulin therapy was then analyzed individually (Pearsons chi-square/Fishers exact or Student t test). The performance of these factors to predict the probability of insulin therapy was estimated using a logistic regression model. Results: Among the 294 patients studied, 39.8% (117/294) required insulin for glycemic control. Univariate analysis showed a positive correlation between insulin therapy and prepregnancy obesity, family history of diabetes, hypertension, prior GDM, prior fetal macrosomia, number of abnormal 100-g/3-h OGTT values, 100-g/3-h OGTT fasting plasma glucose > 95 mg/dL, mean of the four 100-g/3-h OGTT values, 100-g/3-h OGTT fasting/one/two/three plasma glucose values, and HbA1c (P < 0.05). Two logistic regression models were developed and included the following parameters: prepregnancy obesity, family history of diabetes, number of abnormal 100-g/3-h OGTT values (just model 1) and 100-g/3-h OGTT fasting plasma glucose (just model 2). The two first models were analysed another time including the variable HbA1c to verify its contribution on prediction of the need for insulin therapy. Probability curves and scores were constructed based on the four combinations of predictive factors. Conclusions: The probability of insulin therapy can be estimated in pregnant women with GDM based on prepregnancy obesity, family history of diabetes, number of abnormal 100-g/3-h OGTT values, 100-g/3-h OGTT fasting plasma glucose, and HbA1c concentration.

L'hospitalisation de la femme en raison d'une grossesse à risque élevé : l'expérience de couplesLavallée, Émilie 09 1900 (has links)
Attendre un enfant est généralement un événement heureux pour un couple. Toutefois, lorsqu’une hospitalisation de la femme survient en raison d’une grossesse à risque élevé, l’expérience des futurs parents peut se transformer en véritable cauchemar. Jusqu’à maintenant, les savoirs disponibles suggèrent que cet événement imprévu ait de nombreux impacts sur les futurs parents. Néanmoins, ce sujet n’a été abordé que selon une perspective maternelle ou paternelle, sans s’intéresser au caractère systémique de la situation. Cette étude qualitative de cas multiples propose d’explorer l’expérience de couples vivant l’hospitalisation de la femme en raison d’une grossesse à risque élevé, dans une perspective systémique et constructiviste. Des entrevues semi-dirigées ont été réalisées auprès de 7 couples dont la femme était, au moment de l’entrevue, hospitalisée en raison d’une grossesse à risque élevé. L’analyse qualitative des données s’est inspirée de la thématisation et a tenu compte des cadres théoriques et épistémologique choisis, étant respectivement l’approche systémique familiale de Wright et Leahey (2013) et le constructivisme. Les résultats suggèrent que l’hospitalisation est marquée par l’intensité des émotions ressenties. Les futurs parents ressentent presque constamment des sentiments d’inquiétude, d’incertitude et de solitude. Il est toutefois intéressant de constater que le couple lui-même subit des changements au cours de l’hospitalisation. En ce sens, cet événement requiert une immense réorganisation, laquelle touche plusieurs systèmes gravitant autour du système conjugal. Enfin, afin de faire face à l’intensité et à l’imprévisibilité de la situation, les couples sont amenés à solliciter un soutien externe et à puiser dans leurs propres ressources internes. En lien avec les cadres théoriques et épistémologiques choisis, ces résultats conviennent de la nécessité d’adopter une vision systémique et contextuelle afin de développer des soins infirmiers concordants avec l’expérience des couples. Cela ramène à la nécessité de réaliser d’autres études afin de perfectionner la compréhension de l’expérience. Certaines pistes d’interventions infirmières sont néanmoins énoncées, afin d’amener les infirmières à considérer la situation du point de vue de l’expérience conjugale, dont les défis de réorganisations extérieurs et des relations conjugales. Enfin, il est proposé que l’infirmière puisse agir afin de renforcer la capacité des couples à composer avec les conséquences de l’hospitalisation anténatale. / Planning to have a child is most of the time a joyful event for a couple. However, the same event might becomes a real nightmare when the woman is hospitalized because of an at high risk pregnancy. To date, researches exploring this abrupt event have suggested that it may conduct to numerous impacts on future parents. Even though, the subject has only been explored using a maternal or paternal perspective, without considering the systemic aspect of the situation. This qualitative multiple case study suggests to explore the couples’ experience regarding the hospitalization of the woman because of an at high risk pregnancy, in a systemic and constructivist approach. Semi-directed interviews have been conducted with seven couples with a woman hospitalized because of an at high risk pregnancy, at the moment of the interview. Qualitative data analysis was inspired of thematization and took into account the theoretical and epistemological approach, respectively the Wright and Leahey (2013) family approach and constructivism. Results suggest the hospitalization is touched by intense emotions, such as feeling almost all the time worries, uncertainties and loneliness. It is however interesting to note that even the couple himself is touched by changes resulting of the hospitalization. According to that, this event requires a huge reorganization, which implies loads of systems gravitating around the couple’s system. Lastly, in order to face the intensity and the unpredictability of the situation, couples are asking for external support, while using their own internal resources. Concerning the theoretical and epistemological approach chosen to realize this study, results confirm the necessity of adopting a systemic and contextual vision in order to develop nursing cares appropriate with couples’ experience. Besides, it becomes essential to duplicate this type of study to perfection the actual comprehension of the event. However, nursing interventions relating to the external reorganization and the couple relation reorganization are proposed, in order to reinforce nurses’ consideration of the couple’s experience. Only then, it is suggested that nurses can influence couples’ capacity to face the consequences of the antenatal hospitalization.

The perceptions of women regarding obstetric care in public health facilities in a peri-urban area of NamibiaMuntenda, Bartholomeus Mangundu January 2011 (has links)

<p>Namibia has recorded an ascending trend of maternal and neonatal mortality rate from 225 &ndash / 449 per 100 000 women from 1992 to 2006, and 38 &ndash / 46 per 1000 live births from 2000 to 2006 respectively. Kavango Region in Namibia is one among the top seven regions with high maternal and infant mortality rate. Most pregnant women in peri-urban areas of Rundu District in the Kavango region, where this study was conducted, attend ante-natal care services but do not use public health facilities for delivery. The health records from the public health facilities in Rundu, especially from Nkarapamwe clinic and Rundu Hospital maternity section, reveal that although the pregnant women comply with the required standard policy of a minimum of three visits per pregnancy or more, over 40% of women who attend public ante-natal care clinics do not deliver in the public health facility. The aim of this study was to explore the perceptions of women regarding obstetric care in public health facilities in Kehemu settlement, a peri-urban area of Rundu town. The objectives of the study were to explore the perceptions of women on accessibility and acceptability of maternity services in public health facilities. An explorative qualitative study design using focus group discussion as a data collection method was conducted with three groups of women. A purposeful sampling procedure was used to select participants. Ethical approval was obtained from the High Degree Committee of University of the Western Cape and permission to use data from local facilities was obtained from the Ministry of Health and Social Services. Participants were recruited on their own free will and they signed an agreement on confidentiality. A data reduction process was used for analysis. The study findings indicate that women wish to use public health facilities for deliveries due to perceived benefits, in particular, safety for the mother and the baby and that those services are<font size="3"> affordable. However a number of reasons hinder women to access services including the attitudes of health care providers, inability to afford transport at night and cultural influences. The study recommends that delivery services at the local clinic be expanded from eight to twenty-four hours / an information campaign on pregnancy and birth complications as well as the benefits of delivering in a public facility be implemented / refresher training for nurses to improve their caring practices during delivery should be considered and that a similar research be conducted with care providers to ascertain ways to improve maternity services in the public health facility in the area. </font></p>

O presente trabalho procurou analisar a inserção da psicanálise nas novas formas de cuidado terapêutico em perinatologia, mais precisamente, no domínio que envolve os acontecimentos que ocorrem entre a concepção e os 36 meses de vida da criança. Para tanto, inicialmente foi apresentada a área da saúde materno-infantil no Brasil e as políticas públicas que a sustentam. Em seguida, delineou-se o funcionamento do campo escolhido, no caso uma maternidade de alto risco. Tendo em vista, a construção de uma rede de atenção tecida a partir de diferentes olhares, se procurou enfocar os impasses da interseção entre o discurso biomédico, o da educação em saúde e o da psicanálise. Nesse ponto, foi utilizada como referência principal a contribuição de D. W. Winnicott sobre a teoria do amadurecimento pessoal. Com a finalidade de circunscrever o crescente interesse pela primeira infância, procedeu-se a um mapeamento do estudo psicanalítico dos primórdios do psiquismo, após uma breve incursão pelo texto freudiano. Promoveu-se ainda uma discussão sobre o encontro das hipóteses psicanalíticas com as novas descobertas científicas sobre as potencialidades do bebê, ressaltando as consequências possíveis de tal intercâmbio. Por fim, foram destacadas algumas concepções que fundamentam a importância do olhar psicanalítico para o cuidado integral à saúde materno-infantil, enfatizando autores como Lebovici, Cramer, Bydlowski e Golse. A presença da FUTII define o status secretor positivo, o qual é relacionado aos fenótipos eritrocitários ABO. Diante da importância epidemiológica e clínica da infecção pelo T. gondii, o objetivo desse trabalho foi testar a hipótese de que o perfil de glicoconjugados ABH expresso no trato gastrintestinal está associado à infecção por esse parasito. Foram selecionadas 367 gestantes atendidas no Ambulatório de Gestação de Alto Risco do Hospital de Base da Fundação Faculdade Regional de Medicina de São José do Rio Preto. Duas amostras de sangue, uma sem e outra com anticoagulante foram coletadas. A fenotipagem eritrocitária ABO e a detecção dos anticorpos anti-T. gondii foram realizadas pelo método hemaglutinação. A identificação do status secretor foi feita pelo método PCR-RFLP. As diferenças nas freqüências do status secretor positivo e negativo e dos fenótipos eritrocitários ABO, isoladamente ou em conjunto, não foram estatisticamente significantes na presença e na ausência desses anticorpos (p=0,26). Esses resultados sugerem que o perfil de glicoconjugados ABH expressos no trato gastrintestinal sob controle do gene FUT2 não está associado à presença de anticorpos anti-T. gondii. / Toxoplasma gondii infects humans in several manners including by the gastrointestinal tract where the a-2-L-Fucosiltransferase (FUTII) coded by FUT2 (19q13.3) controls the expression of the ABH glycoconjugates profile. Presence of FUTII defines the positive secretor status which is associated to ABO erythrocytic phenotypes. Due to the epidemiological and clinical importance of T. gondii infection, the aim of this work was to test the hypothesis that the ABH glycoconjugate profile expressed in the gastrointestinal tract is associated to infections by this parasite. A total of 367 pregnant women from the High-Risk Pregnancy Clinical of the University Hospital de Base in São José do Rio Preto were enrolled in this study. Two blood samples were drawn with only one mixed with anticoagulant. The ABO erythrocytic phenotyping and detection of anti-T. gondii antibodies were achieved by the hemagglutination method. Identification of the secretor status was by the PCR-RFLP method. Differences in the positive and negative secretor status and ABO erythrocytic phenotypes, either in isolation or in association, were not statistically significant in respect to the presence or absence of these antibodies (p-value =0.26). These results suggest that the ABH glycoconjugate profile expressed in the gastrointestinal tract under control of the FUT2 gene is not associated to anti-T. gondii antibodies.

Previous issue date: 2017 / Mestrado Acadêmico em Ciências do Cuidado em Saúde / A abordagem aos aspectos emocionais que envolvem a mulher durante a assistência pré-natal tem-se mostrado essencial, principalmente, no que tange as gestantes de alto risco. Diante da situação de risco, emoções como medo, angústia, aflição e tristeza podem ser desencadeadas. Nesse contexto, a compreensão de cada momento vivenciado somado a um cuidado humanizado possibilita a prática do cuidado emocional a esta gestante para enfrentar os possíveis obstáculos da gestação. Teremos assim como objetivo geral: compreender as demandas de cuidado emocional das gestantes que convivem com doenças crônicas por ocasião do atendimento de pré-natal e o papel do enfermeiro nesse contexto. E como objetivos específicos: descrever o perfil sócio-demográfico e gestacional das mulheres que convivem com doenças crônicas e se encontram em acompanhamento no ambulatório de pré-natal do HUAP; discutir as demandas de cuidado emocional apresentadas por gestantes que convivem com doenças crônicas; tecer o cuidado emocional de enfermagem em grupo com as gestantes que convivem com doenças crônicas. Trata-se de um estudo descritivo, transversal e qualitativo, que utiliza como método a sociopoética. O campo de investigação foi o Ambulatório de Pré-Natal do Hospital Universitário Antônio Pedro (HUAP) localizado em Niterói/RJ. Os participantes foram 12 gestantes que convivem com problema de saúde crônico (hipertensão arterial, diabetes mellitus, hipotireoidismo e HIV). A produção de dados se deu por meio do dispositivo grupo-pesquisador. Os dados do estudo foram analisados a partir dos estudos transversal e filosófico, próprios da sociopoética. Foram definidas as seguintes categorias: os atravessamentos de uma gestação de alto risco sob o ponto de vista emocional; estratégias de enfrentamento de problemas emocionais utilizadas por gestantes de alto risco; a avaliação do grupo “As mães que superam suas emoções negativas” sob o olhar das gestantes; a aplicação da teoria de Hidelgard Peplau na Sociopoética com gestantes de alto risco; e, o cuidado do emocional em enfermagem desenvolvido através da mutualidade: o cuidar do outro cuidando de si. Contudo, fica evidente a necessidade de reorganização do serviço de pré-natal na atenção secundária, onde o cuidado emocional deve ser visto como elemento a ser avaliado e desenvolvido ao longo do período gestacional e a atuação do enfermeiro deve ser efetivada através da consulta de enfermagem e da assistência à mulher de forma integral, passando a ser reconhecido como parte integrante e fundamental da equipe multiprofissional de saúde responsável pelo cuidado a essas mulheres. / The approach to the emotional aspects that involve the woman during the prenatal care has been essential, mainly, in relation to high-risk pregnant women. At risk situation, emotions such as fear, anguish, affliction and sadness can be unleashed. In this context, the understanding of each experienced moment added to a humanized care makes possible the practice of the emotional care to this pregnant woman to face the possible obstacles of the gestation. Therefore, we will have as a general objective: Understand the emotional care demands of pregnant women living with chronic diseases due to prenatal care and the participation of nurses in this context. And as specific objectives: Describe the socio-demographic and gestational profile of women living with chronic diseases and are being followed up at the prenatal clinic of Antônio Pedro University Hospital; Discuss the emotional care demands presented by pregnant women living with chronic diseases; Develop, in group, the emotional nursing care with pregnant women living with chronic diseases. It is a descriptive, cross-sectional and qualitative study that uses the sociopoetics as a method. The investigation field was the prenatal clinic of Antônio Pedro University Hospital located in Niterói / RJ. Twelve (12) pregnant women living with a chronic health condition (hypertension, diabetes mellitus, hypothyroidism and HIV) took part in the study. The data were acquired through the group-researcher device, and were analyzed from the transversal and philosophical studies, typical of sociopoetics. The following categories were defined: the crossings of a high-risk pregnancy from the emotional point of view; strategies to deal with emotional problems used by high-risk pregnant women; the evaluation of the group "Mothers who overcome their negative emotions" under the gaze of pregnant women; the application of Hidelgard Peplau's theory in sociopoetics with high-risk pregnant women; and, the emotional care in nursing developed through the mutuality: the caring of the other taking care of itself. However, it is evident the need to reorganize the prenatal service in secondary health care, where the emotional care should be seen as an element to be evaluated and developed throughout the gestational period and the nurse's action must be carried out through the nursing consultation and the integral assistance to women, being recognized as a fundamental part of the multiprofessional health team responsible for the care of these women.
