The nature and extent of participation by small scale farmers in the Development Aid from People to People Farmers' Club project in Mazowe District of Zimbabwe

Magister Artium - MA / The conventional top-down approach to development has been rendered unsustainable and is regarded as a poor strategy to achieving community empowerment and development. The past few decades have seen the promotion of bottom-up techniques whereby governments and developing agents collaborate with target beneficiaries and view them as equal partners in the development of their own communities. It is generally believed that the participation of farmers in agricultural projects improves the performance of the agricultural sector. However, despite the adoption of participatory models, agricultural societies have remained plagued by poverty. It is against this background that this research using the Mazowe District as a case study investigated the nature and extent of participation by small scale farmers in the Development Aid from People to People (DAPP) Farmers’ Club in order to document the extent to which farmers have been empowered. The study made use of both quantitative and qualitative methodologies to gather relevant data. Broadly the findings indicate that the participation of farmers in the DAPP Farmers' Club project was substantively high. The results also suggest that the project empowered farmers to farm more productively in the case study area. In light of the findings the study recommends that governments and NGOs should follow the values and principles of the people-centred development (PCD) theory when implementing agricultural projects as it has proven to be an empowering approach. This practice may transform societies as there is an opportunity to address societal needs at grassroots level. In view of this research it can be argued that capacitating farmers through training and improving their farming skills can improve their agricultural production.

The role of traditional health practitioners in health promotion: A case study of the Pfura Rural District, Mashonaland Central Province, Zimbabwe

Since time immemorial traditional health ensured and covered all major areas of health promotion, which included preventative, promotive, curative, rehabilitative, and surgical practices. In low- and middle-income countries where the number of practitioners of modern medicine may not be enough to meet the health care needs of the country, traditional health promotion is central. Given that in many societies the practices are done in secret, it therefore makes it difficult for people in general to access proper service from traditional health practitioners. This study explored the role of traditional health practitioners in promoting health in a rural community. The study adopted a qualitative approach which was descriptive, explorative and interpretative, targeting the registered traditional health practitioners in villages of Ward X of the Pfura Rural District Council. A non-probability purposive and snowball sampling method was used to identify twenty-two (22) male and female participants. The participants were registered diviners, herbalists and traditional birth attendants. The data which was collected through in-depth interviews, field notes, audio recordings and observation were thematically analyzed using Tesch’s method of data analysis and presented in categories, themes and subthemes. The study revealed that the traditional health practitioners in Pfura, are playing a vital role in offering primary health care for the people. Traditional health practitioners offer healing through their extensive knowledge of herbal and animal-based medicines and therapeutic actions such as rituals. The study established the use of herbs, traditional ceremonies, taboos, cultural norms as health promotive practices. Traditional healers in Pfura have demonstrated their efficacy and remained a powerful establishment in society through their easy access to the ancestral spirits, which has sustained the healing culture of Kore-kore people. Despite the central role being played by traditional health system in health promotion, impediments were found against the practice and its practitioners. Poor professional development, research and training of the Traditional Health Practitioners as well as the advanced age of practitioners is a setback in the practice as it contributed negatively to the sustainability of the traditional health practice. Guided by both study findings, key action areas of health promotion of the Ottawa Charter and the objectives of the World Health Organization, Traditional Medicine Strategy of 2014-2023 target, a framework/guide was developed and recommended an integrative and sustainable health promotion in a rural setting. / NRF

These families produce and reproduce their space which is marked by relations of relationships, of work and of getting along well with the district and the city. Some characteristics as the link among the members of the community, supported by its origin, by its Catholic faith, by the family ties and by a cultural that was build along the time around the land, are structures that imply the production and reproduction of this specific space. The idea of the rural district in this context is very valuable to understand this territorial formation. Therefore, we understand that these characteristics drawn a territory that is appropriated by the identity, which is reconstructed through generated groups and marked by the use of the identity power with the place. The established links by these families through practices and values allow a specific form of input in the regional and local economy that is modern and has an accelerated growth rate.

O município e o planejamento para além do perímetro urbano: o rural e os distritos rurais do Sul Goiano / The municipality and planning beyond the urban perimeter: the rural anda districts of South Goiano

CAPES - Coordenação de Aperfeiçoamento de Pessoal de Nível Superior / A maior autonomia político administrativa adquirida pelo município após a Constituição Federal de 1988, elevou-o à ente federativo autônomo e autor das suas próprias políticas de desenvolvimento e gestão, sendo o Plano Diretor Municipal seu instrumento básico de política de desenvolvimento e expansão urbana. Essa visão apenas urbana, começou a mudar após a promulgação do Estatuto da Cidade em 2001 (Lei 10.257/2001) e dessa forma, os Planos Diretores Municipais passariam a abranger todo o território do município. De tal modo, essa tese teve como objetivo compreender como áreas rurais, em especial os distritos rurais da Mesorregião do Sul Goiano são abordados nos Planos Diretores Municipais pós Estatuto da Cidade, bem como mostrar a importância do município no planejamento das áreas rurais, com vistas a colaborar para a ampliação de debates sobre planejamento territorial rural. Para responder a esses objetivos, analisou-se teorias que abordam a temática, decretos, leis como as dos Planos Diretores Municipais do Sul Goiano, bem como realizou-se pesquisa de campo e posterior análise F.O.F.A em 9 (nove) vilas distritais de 5 (cinco) municípios da mesorregião do Sul Goiano. Por meio dessas leituras e análises concluiu-se que, mesmo com novos direcionamentos para as áreas que estão além do perímetro urbano, a abrangência desses espaços no planejamento municipal ainda é muito incipiente e em alguns casos, inexistente. A inexperiência e a dificuldade do município e de seus planejadores em reconhecer o que está além do seu perímetro urbano; o preconceito e a visão do rural como atrasado; o viés político contido nas decisões e na elaboração dos Planos Diretores Municipais, estão de forma geral, no cerne destas questões. As análises realizadas nos PDMs dos municípios em estudo apontaram que a inserção dos espaços que estão fora dos limites urbanos/cidades acontece de forma superficial ou quando estes representam algum interesse político e/ou econômico. Essa superficialidade se manifesta desde a definição de rural e urbano e de seus limites físicos, culturais e sociais, os quais consideram rural como tudo que está além do perímetro urbano, passando pelas diretrizes e instrumentos que configuram-se em um viés econômico e quando não o são, estão isentos das especificidades locais. Nesse contexto, os distritos rurais são compreendidos como uma extensão do urbano, sem considerar toda a complexidade de relações existentes nessas localidades, suas necessidades e todo o contexto local que faz com que essa população necessite de políticas públicas e ações específicas. Essas condições puderam ser comprovadas durante a pesquisa de campo, a qual verificou a pouca ou quase inexistente atenção dos governos municipais em relação aos distritos rurais, principalmente em relação às necessidades básicas que levem em consideração, também, os costumes e a cultura do lugar. No mais, foi compreendido também, que boa parte das ações e benéficos podem e devem partir dos municípios e ser contempladas nos Planos Diretores Municipais, principalmente porque já existem ações sendo executadas em outros distritos rurais de outros municípios no Brasil. Ainda há muito o que avançar em relação ao ordenamento do município de forma integral, principalmente porque as possibilidades e oportunidades devem ser as mesmas a toda população seja urbana ou rural, mas já estamos no caminho. / The greater political and administrative autonomy acquired by the municipality after the Federal Constitution of 1988, elevated it to the autonomous federative entity and author of its own policies of development and management, being the Municipal Master Plan its basic instrument of policy of development and urban expansion. This only urban vision began to change after the enactment of the City Statute in 2001 (Law 10.257/2001) and, thus, the Municipal Master plans would cover all the territory of the municipality. Thus, this thesis aimed to understand how rural areas, especially the rural districts of the South Goiano Mesoregion, are addressed in the Municipal Director Plans after the City Statute, as well as show the importance of the municipality in the planning of rural areas, in order to contribute to the expansion of debates on rural territorial planning. In order to respond to these objectives, it was analyzed theories that approach the thematic, decrees, laws such as those of the Municipal Governing Plans of Southern Goiás, as well as field research and subsequent F.O.F.A analysis in nine (9) districts of 5 (five) municipalities of the South Goiano mesoregion. Through these readings and analyzes it was concluded that, even with new directions for areas that are beyond the urban perimeter, the scope of these spaces in the municipal planning is still very incipient and, in some cases, nonexistent. The inexperience and difficulty of the municipality and its planners in recognizing what is beyond its urban perimeter; the prejudice and the vision of the rural as backward; the political bias contained in the decisions and in the elaboration of the Municipal Master Plans, are generally at the heart of these issues. Moreover, the analyzes carried out in the PDMs of the municipalities under study showed that the insertion of spaces that are outside the urban limits/cities happens superficially or when it represents political and/or economic interest. This superficiality is manifested from the definition of rural and urban and its physical and social limits, which consider rural as everything that is beyond the urban perimeter, passing through the guidelines and instruments that are configured in an economic bias and when they are not, they are exempt from local specificities. In this context, rural districts are understood as an extension of the urban, without considering all the complexity of existing relationships in these localities, their needs and the entire local context that causes this population to need public policies and specific actions. These conditions could be verified during the field research, which verified the abandonment of the municipal governments with respect to the rural districts, mainly with respect to the basic necessities. In addition, it was also understood that a good part of the actions and benefits can and should come from the municipalities and be contemplated in the Municipal Master Plans, mainly because there are already actions being carried out in other districts in Brazil. There is still a long way to go with respect to municipality planning in an integral way, mainly because the possibilities and opportunities must be the same for all the population, whether urban or rural, but we are already on the way. / Tese (Doutorado)

Medicine treatment patterns of HIV/AIDS patients at a rural district hospital in the North West province / Jaques Rix

Globally an estimated 33.4 million people were living with HIV/AIDS by 2008 (UNAIDS, 2009a:7). One of the main challenges facing the Republic of South Africa (RSA) today is the HIV/AIDS epidemic (NSP, 2007:17). By mid-year 2011 an estimated 5.38 million people (10.6% of the total population) were living with HIV/AIDS in the RSA (Statistics South Africa, 2011:2). Currently South Africa has the largest number of people enrolled in the Highly Active Antiretroviral Treatment programme (HAART) in the world (WHO, 2008:59). The objective of this study was to determine retrospectively the medicine treatment patterns of HAART at a district hospital in the North West Province of South Africa. The study was conducted at Thusong hospital in the Ditsobotla sub-district of the North West Province of South Africa. A non-experimental, retrospective, cross-sectional, drug utilisation research methodology was used to obtain the data. The target population included patients of all ages who visited Thusong hospital pharmacy during the data collection period, which commenced on 01 February 2012 and ended on 31 March 2012. The data of three hundred and ninety nine (N=399) adult and one hundred and sixty one (N=161) paediatric patients on HAART were used. The adult female patients accounted for almost 70% (n=276, 69.17%) and the adult male patients for only 30% (n=123, 30.83%). The male paediatric patients represented just over 60% (n=97, 60.25%), whereas the female paediatric patients comprised less than 40% (n=64, 39.75%). The majority of adult patients were unmarried (n=323, 80.95%) and this group of patients were also the youngest group (μ=36.38 ± 8.98 years) on ARV treatment. Almost 86% (85.96%, n=343) of adult patients were registered as unemployed. Ninety two (n=92, 23.06%) adult patients and fifty eight (n=58, 36.03%) paediatric patients defaulted treatment during the defined period. The investigation into the adult medicine treatment patterns revealed that more than half (52.38%, n=209) of all the adult patients were receiving regimen 1atn (EFV, TDF and 3TC), followed by 20.80% (n=83) on regimen 1a (EFV, D4T and 3TC). Most paediatric patients (n=73, 45.34%) were on regimen P1c (EFV, D4T and 3TC) and the second most (n=45, 27.95%) were on regimen P1a (D4T, 3TC and LPV/r). The average weight of adult female patients was 57.18kg (± 15.78kg) and the average adult male patient weighed 55.87kg (± 10.17kg) on initiation of HAART. The average adult male patient was initiated on HAART with a CD4 count of 130cells/mm3 (± 99.45cells/mm3), while for adult female patients it was 160cells/mm3 (± 96.52cells/mm3). The average male child was initiated with a CD4 count of 509.1cells/mm3 and the average female paediatric patient with 477.3cells/mm3. The average viral load for adult female patients on initiation of HAART was 103046copies/mm3 (± 189146copies/mm3) and for adult male patients it was 416600copies/mm3 (± 439746copies/mm3). The difference between the viral load of adult female and male patients were described as statistically (p=0.0006) and practically (d=0.713) significant. The average viral load for female paediatric patients on initiation of HAART was 242207copies/mm3 (± 709133copies/mm3) and for male paediatric patients it was 329734copies/mm3 (± 674532copies/mm3). Adult patients that received HAART at more than 12 consultations revealed an average weight gain of 3.43kg (± 8.11kg) from initiation of treatment. This group also showed an average increase of 214.71cells/mm3 (± 248.24cells/mm3) in CD4 count and an average reduction in viral load of 170944copies/mm3 (± 191854.69copies/mm3) from the day they started HAART up to the last date of receiving treatment. The paediatric patients on treatment for more than 12 consultations showed an average weight gain of 6.56kg (± 3.75kg) from initiation of ARV treatmentup to the last date of receiving treatment. Dans un même temps, le patrimoine culturel, matériel comme immatériel, a été mondialement reconnu comme l'un des axes majeurs du développement durable par les Institutions internationales qui préconisent sa valorisation en tant que marqueur identitaire porteur de lien social. Many initiatives have been taken for the protection and valorization of the living memory of a traditional popular culture ; their implementation may benefit from news regulatory and financial measures from the Departments, Regions as well as from European programmes. At the same time, the (material as well as immaterial) heritage has been acknowledged as a major axis of sustainable development by International Institutions ; indeed, they recommend its valorization as an ident marker that facilitates social cohesion.--- From this point of view and through a systemic review of the international founding texts, European programmes, policies of the local authorities, charters and action plans of regional natural parks, cultural events and netwworks of local actors, the thesis, Local development and Occitan culture, analises the forces and weakness of the established parternships, and declineates the Occitan imprint on the local development.
