Utilizamos estudo cego para não influenciar o examinador. Método: Foi realizado um estudo prospectivo cego com avaliação sistemática da via aérea superior com graduação de palato Web de 146 pacientes que realizaram polissonografia basal. Como proposta de graduação para o tamanho do palato Web classificamos conforme o local de inserção da prega palatofaríngea, ou pilar posterior amigdaliano, na úvula usando os seguintes critérios: - Web Grau I: Inserção da prega palatofaríngea no terço proximal,ou seja, superior da úvula. - Web Grau II: Inserção da prega palatofaríngea no terço médio da úvula. - Web Grau III: Inserção da prega palatofaríngea no terço distal, ou seja, inferior da úvula. Resultados: Nos pacientes com maior gravidade de SAOS os achados mais freqüentes foram: IMC maior que 31, circunferência cervical maior que 41cm, palato duro ogival, palato posterior, palato espesso, úvula espessa, hipertrofia amigdaliana grau III ou IV, língua demarcada pelos dentes, diminuição da fase de sono 3/4. Outros achados não apresentaram relação com a SAOS de maior gravidade, como: retroposicionamento mandibular, Mallampati Modificado III e IV, pregas palatofaríngeas medianizadas, oclusão dentária classe II, desvio septal nasal, hipertrofia de cornetos. Não houve relação do tamanho do palato Web. Conclusões: Alterações anatômicas podem ser identificadas, pelo exame físico sistemático, com maior freqüência conforme a gravidade da SAOS. A 11 graduação do tamanho do palato Web não auxiliou na identificação de indivíduos com maior gravidade da SAOS. / Objective: The objective of this study is to apply the systematic evaluation of physical examination in patients with Obstructive Sleep Apnea Syndrome (OSAS), including the measure of the cervical circumference and to submit a classification for graduation of the web palate. We have applied a blind study to not influence the examiner. Method: A prospective study with systematic evaluation of the upper airway and proposal of the web palate graduation was performed in 146 patients that were submitted to polysomnography. At the moment of the physical examination and interview, the researcher did not know reason of the polysomnography, thus issuing a blind study for the examiner. As proposal of graduation for the size of the web palate we classify as the place of the palatopharyngeal arch, thus posterior tonsil pillar insertion, in the uvula using the following criteria: Web Degree I: Insertion of the palatopharyngeal arch in the proximal third of the uvula, thus uvula upper position. Web Degree II: Insertion of the palatopharyngeal arch in median third of uvula. Web Degree III: Insertion of the palatopharyngeal arch in distal third, thus, inferior of the uvula. Results: The following findings had been more frequent in the groups of major severity: higharched hard palate, posterior and palate, long and thick uvula, tonsils hypertrophy degree III or IV, tongue edge crenation, IMC greater than 31, reduction of the phase of sleep 3/4, cervical circumference greater than 41cm. Other findings didn´t have relation with the severity of OSAS, as: mandible retropositioning, Mallampati III and IV, class II occlusion, septal deviation and turbinate hipertrophy. There was no relation the web palate size to OSAS severity. Conclusions: Anatomical alterations can be identified by systematic physical examination with greater 49 frequency in accordance with OSAS severity. The web palate size graduation did not support to identify patients with severe OSAS.

Entretanto, a sobreposição entre elas (Overlap) e a associação com a obesidade, ainda necessita de investigações. Objetivos: Verificar a incidência da SAOS em portadores de DPOC obeso e as atividades inflamatória e metabólica desta tríade. Métodos: Foram incluídos portadores de DPOC moderado e grave com índice de massa corpórea (IMC) ≥ 27 Kg/cm2. Além da espirometria que classificou a DPOC, foi realizada polissonografia que diagnosticou ou não a SAOS. Foram avaliados os marcadores bioquímicos de glicemia, hemoglobina glicada (HBC), insulina, leptina, adiponectina, grelina, proteína C-reativa (PCR) e interleucina 6 (IL-6). Resultados: Este estudo transversal incluiu portadores de DPOC com (N=46) e sem (N=20) SAOS com médias de idade, IMC e VEF1, respectivamente, 61.57±11.31 x 59.75±9.68 anos, 34.00±5.67 x 33.89±6.75 kg/cm2, 57.20±16.81 x 53.85±17.65 %pred. As médias de glicemia, HBC, insulina, leptina e adiponectina com ou sem SAOS foram semelhantes. A grelina aumentou com a maior gravidade da SAOS na overlap (186.16±25.56 x 137.10±21.78 pg/ml). Não foi observado aumento da PCR para nenhum grupo. A maior gravidade da DPOC aumentou a IL-6 (5.17±5.79 x 11.18±13.10). Conclusão: A prevalência de SAOS é elevada em pacientes portadores de DPOC com obesidade. A SAOS grave elevou a atividade metabólica pelo aumento da grelina, sem influencia na expressão inflamatória, na tríade com DPOC e obesidade. A gravidade da DPOC produziu maior atividade inflamatória de IL-6. / FAPESP: 2013/02091

Comparação entre o efeito do aumento da dimensão vertical de oclusão e do avanço mandibular na qualidade do sono em pacientes idosos portadores de próteses totais bimaxilares / Comparison between the vertical dimension of occlusion increase and mandibular advance effects on sleep quality in elderly patients wearing maxilar and mandibular complete dentures

A população idosa possui alta prevalência de edentulismo e, conseqüentemente, é afetada pelos problemas a ele associados. A perda da dimensão vertical de oclusão (DVO) é um destes problemas que compromete, entre outros fatores, o desempenho do sistema estomatognático. Logo, doenças relacionadas ao colapso da musculatura da via aérea superior (VAS), como a síndrome da apnéia obstrutiva do sono (SAOS), tornam-se enfermidades relevantes para pacientes nesta faixa etária. Sendo assim, medidas terapêuticas eficazes e de baixo custo, como a utilização de aparelhos intraorais (AIOs) para liberação do fluxo aéreo, podem ser empregadas contribuindo para a qualidade do sono destes pacientes. O objetivo deste estudo foi avaliar parâmetros subjetivos e objetivos do sono, em 10 pacientes idosos portadores de Próteses totais (PTs) bimaxilares, após a utilização de novas PTs confeccionadas no Programa Envelhecer Sorrindo e após o uso de dois AIOs: um dispositivo intraoral (DIO), especialmente desenvolvido para aumentar a DVO sem provocar avanço mandibular, e um aparelho de avanço mandibular (AAM). Para isso, questionários de rastreamento da qualidade do sono e polissonografias (PSGs) foram realizados, em quatro momentos distintos: sem as PTs, com as PTs, com o DIO e com o AAM. Foram realizadas, também, telerradiografias em norma lateral (TNL) dos pacientes com as PTs, com o DIO e com o AAM a fim de avaliar alterações no diâmetro da VAS e o posicionamento mandibular nesses três momentos. Concluiu-se que, o AAM testado promove maior porcentagem de sono no estágio 1, podendo contribuir para a melhora na qualidade subjetiva do sono dos pacientes, uma vez que ajudou a diminuir o tempo necessário para iniciar o sono, além de facilitar a manutenção do estado de vigília. / There is a high prevalence of edentulism and problems associated to it in the elderly population. A decrease in vertical dimension of occlusion (VDO) is one of these problems that may compromise the stomatognathic system. Therefore, it is important to investigate diseases related to upper airway (UA) musculature collapse, such as the syndrome of obstructive sleep apnea (OSAS). Therapeutic measures of low cost and high efficacy, such as intraoral appliances (IAs) to release the air flow may be employed, contributing to patients sleep quality. The aim of this study was to evaluate subjective and objective sleep parameters in 10 elderly patients who wore maxilar and mandibular complete dentures (CD). The analyses were performed after the use of a new pair of CD and after using two IAs: an intraoral device (ID), especially developed to increase the VDO without causing mandibular advance, and a mandibular advance device (MAD). For this purpose, questionnaires and polysomnography (PSG) were performed in four distinct stages: patients not wearing CD, wearing CD, wearing ID and wearing MAD. In addition, lateral cephalograms (LC) of patients wearing FD, wearing ID and wearing MAD were performed to assess changes in UAs diameter and in the mandibular positioning. It was concluded that the MAD tested promotes higher percentage of stage 1 sleep and and may contribute to the improvement in patients subjective sleep quality, as it helped to decrease the time needed to fall asleep, and facilitate the maintenance of wakefulness.

Embora a literatura ressalte os efeitos positivos da Pressão Positiva Contínua Aérea (CPAP) no padrões de sono, sonolência excessiva diurna e EO nos adultos são raras os estudos na população idosa. O objetivo desse estudo foi avaliar os efeitos dos parâmetros de EO em pacientes idosos, compará-los com uma população adulta e verificar o efeito do tratamento com CPAP nos pacientes idosos com SAOS e o impacto na sua qualidade de vida. Métodos: Foram selecionados indivíduos com faixa etária entre 25 a 75 anos, distribuídos em quatro grupos: idosos SAOS (n=30), idosos controle (n=27), adultos SAOS (n=13) e adultos controle (n=12). Avaliaram-se os parâmetros de polissonografia, qualidade de vida (SF-36) e EO (ácido úrico, vitamina B12, ácido fólico, vitamina E, vitamina C, peroxidação lipídica, catalase, superóxido dismutase, homocisteína e cisteína) comparando-se adultos e idosos apenas no momento basal e antes e após uso do CPAP nos idosos com SAOS. Resultados: Na comparação entre adultos e idosos em relação aos níveis de Vitamina E, os idosos SAOS (22,2 ± 1,9 umol/L) apresentaram níveis maiores do que o grupo adultos SAOS (12,7 ± 1,7 umol/L) e houve também diferença entre os idosos controle (26,5 ± 1,7 umol/L) e os adultos controle (19,2 ± 1,8 umol/L). Quanto aos níveis de homocisteína, o grupo adultos SAOS (17,2 ± 1,3 μmol/L) apresentou níveis maiores que adultos controle (10,8 ± 1,4 μmol/L), observação não encontrada nos idosos. Nos idosos com SAOS, o tratamento com CPAP mostrou reversão do estresse oxidativo pela diminuição da peroxidação lipídica expressa pela quantidade de malondialdeído (MDA) [pré: 2,7 (IC95% 1,6-3,7) vs. pós: 1,3 (0,7-1,9); p<0,01]. O tratamento com CPAP, nesse grupo mostrou melhora em dois domínios do questionário de qualidade de vida SF-36: Capacidade Funcional [pré= 77,4 (69,2-85,5) vs. pós= 83,4 (76,9-89,9); p=0,04] e Dor [pré= 65,4 (52,8-78,1) vs. pós= 77,8 (67,2-88,3); p= 0,005]. Conclusão: Nosso estudo mostrou que o tratamento com CPAP em pacientes idosos contribuiu para a reversão do EO pelo decréscimo dos níveis de MDA e na melhora da qualidade de vida. / Introduction: The effects of intermittent hypoxia and hypercapnia resulting from obstructive respiratory events Syndrome Obstructive Sleep Apnea (OSAS) in the generation of reactive oxygen species (ROS) has recently received particular attention because of its association with oxidative stress (OS) and aging process. Although the literature highlights the positive effects of Continuous Positive Air Pressure (CPAP) in sleep patterns, excessive daytime sleepiness and OS in adults are rare studies in the elderly. The aim of this study was to evaluate the effects of OS parameters in elderly patients, comparing them with an adult population and investigate the effect of CPAP treatment in elderly patients with OSAS and the impact on their quality of life. Methods: We selected individuals between 25 to 75 years old and divided into four groups: elder OSAS, elder controls, adult OSAS and adult controls. For the elderly group, 30 volunteers were selected with OSAS and 27 control volunteers matched for age. Data from 13 adult volunteers with OSA and 12 without OSA were used to compare with the elderly only at baseline. This study was prospective and comparative study between the group of elderly volunteers without OSA and the group of elderly patients with OSAS. The analysis was also made before and after use of CPAP for the OSAS group. All night polysomnography was used to determine sleep-disordered breathing AHI and the subsequent division of the groups. Questionnaires were applied to evaluate the subjective excessive daytime sleepiness (Epworth Sleepiness Scale) and quality of life (SF-36). Laboratory tests were conducted to evaluate parameters related to oxidative stress: allantoin, vitamin B12, folic acid, vitamin E, vitamin C, lipid peroxidation, catalase, superoxide dismutase, homocysteine and cysteine. A comparison was made between adults and elders and to evaluate the effect of CPAP in the elderly patients by means of appropriate statistical tests. Results: In the comparison between adults and elder, the levels of vitamin E, elder OSAS (22.2 ± 1.9 umol / L) had higher levels than the adult OSAS group (12.7 ± 1.7 umol / L) and elderly control (26.5 ± 1.7 umol / L) and adult control (19.2 ± 1.8 umol / L) showing an interaction of time and OSAS in the variable. As for the homocysteine levels, the adult OSAS group (17.2 ± 1.3 mmol / L) had higher levels than adult control (10.8 ± 1.4 mmol / L). In elderly patients with OSAS, CPAP treatment showed reversal of oxidative stress by decreasing lipid peroxidation expressed by the amount of malondialdehyde (MDA) [pre: 2.7 (95% 1,6-3,7) vs. post: 1.3 (0.7-1.9), p <0.01]. Treatment with CPAP in this group improved two areas of the questionnaire of quality of life SF-36: Functional Capacity [pre = 77.4 (69,2-85,5) vs. post = 83.4 (76,9-89,9), p = 0.04] and pain [pre = 65.4 (52,8-78,1) vs. post = 77.8 (67,2-88,3), p = 0.005]. Conclusion: Our study showed showed that adult OSAS group showed a reduction in the levels of vitamin E, indicating a reduction of antioxidant capacity compared to the older group (OSAS and control), suggesting that adults react heterogeneously to the effects of stress (intermittent hypoxia and reoxygenation) compared with the elderly. Treatment with CPAP in elderly patients with OSAS contributed to the reversal of oxidative stress by decreasing levels of MDA and improving the quality of life. / TEDE / BV UNIFESP: Teses e dissertações

Thus, a study titled "Severity of COPD and its association with OSAS: impact on autonomic modulation and functional capacity" was carried out with the purpose of evaluating whether this coexistence would imply greater autonomic impairment and functional capacity than when presented in the COPD conditions in isolation. In the present study, the heart rate variability indexes (HRV) of twenty four individuals with COPD (n=12) and OLDOSA (n=12) were evaluated by means of electrocardiographic recording during rest, active posture maneuver (APM), respiratory sinus arrhythmia maneuver (RSA-m) and six-minute walk test (6MWT). Our results showed that the COPD group presented higher parasympathetic modulation during APM in contrast to the OLDOSA group (p = 0.02). In contrast, the OLDOSA group had marked sympathetic modulation during RSA-m when compared to the COPD group (p = 0.00). Additionally, the exercise performance during the walking test was similarly impaired in both groups despite the greater severity of the COPD group. Therefore, our results suggest patients with OLDOSA coexistence have marked sympathetic modulation, and the presence of OSAS in COPD patients has a negative impact on functional capacity regardless of the severity of the lung disease. / Vários estudos têm investigado os efeitos deletérios da associação entrea doença pulmonar obstrutiva crônica (DPOC) e a síndrome da apnéia obstrutiva do sono (SAOS), entretanto, ainda pouco é sabido se a coexistência de DPOC e SAOS (“OLDOSA”) está associada à maior prejuízo autônomo, bem como, se as alterações desencadeadas pela sobreposição das doenças estão associadas à maior prejuízo funcional para essa população. Sendo assim, um estudo intitulado “Severidade da DPOC e sua associação com a SAOS: impacto na modulação autonômica e na capacidade funcional” foi realizado com o intuito de avaliar se essa coexistência implicaria em maiores prejuízos autônomos e de capacidade funcional do que quando apresentados nas condições da DPOC de forma isolada. No presente estudo, os índices da variabilidade da frequência cardíaca (VFC) de vinte e quatro indivíduos com DPOC (n=12) e OLDOSA (n=12) foram avaliados por meio de registro eletrocardiográfico durante repouso, manobra postura ativa (MPA), manobra de arritmia sinusal respiratória (m-ASR) e teste de caminhada de seis minutos (TC6). Nossos resultados mostraram que o grupo DPOC apresentou maior modulação parassimpática durante a MPA em contraste com o grupo OLDOSA (p=0,02). Em contrapartida, o grupo OLDOSA apresentou marcada modulação simpática durante a m-ASR quando comparado ao grupo DPOC (p=0,00). Adicionalmente, o desempenho durante o teste de caminhada apresentou-se prejudicado em ambos os grupos de forma similar, apesar da maior gravidade do grupo DPOC. Sendo assim, nossos resultados sugerem que os pacientes que apresentam a coexistência “OLDOSA” apresentam marcada modulação simpática, e a presença da SAOS em portadores de DPOC produz impacto negativo na capacidade funcional independentemente da gravidade da doença pulmonar.

Para isso, questionários de rastreamento da qualidade do sono e polissonografias (PSGs) foram realizados, em quatro momentos distintos: sem as PTs, com as PTs, com o DIO e com o AAM. Foram realizadas, também, telerradiografias em norma lateral (TNL) dos pacientes com as PTs, com o DIO e com o AAM a fim de avaliar alterações no diâmetro da VAS e o posicionamento mandibular nesses três momentos. Concluiu-se que, o AAM testado promove maior porcentagem de sono no estágio 1, podendo contribuir para a melhora na qualidade subjetiva do sono dos pacientes, uma vez que ajudou a diminuir o tempo necessário para iniciar o sono, além de facilitar a manutenção do estado de vigília. / There is a high prevalence of edentulism and problems associated to it in the elderly population. A decrease in vertical dimension of occlusion (VDO) is one of these problems that may compromise the stomatognathic system. Therefore, it is important to investigate diseases related to upper airway (UA) musculature collapse, such as the syndrome of obstructive sleep apnea (OSAS). Therapeutic measures of low cost and high efficacy, such as intraoral appliances (IAs) to release the air flow may be employed, contributing to patients sleep quality. The aim of this study was to evaluate subjective and objective sleep parameters in 10 elderly patients who wore maxilar and mandibular complete dentures (CD). The analyses were performed after the use of a new pair of CD and after using two IAs: an intraoral device (ID), especially developed to increase the VDO without causing mandibular advance, and a mandibular advance device (MAD). For this purpose, questionnaires and polysomnography (PSG) were performed in four distinct stages: patients not wearing CD, wearing CD, wearing ID and wearing MAD. In addition, lateral cephalograms (LC) of patients wearing FD, wearing ID and wearing MAD were performed to assess changes in UAs diameter and in the mandibular positioning. It was concluded that the MAD tested promotes higher percentage of stage 1 sleep and and may contribute to the improvement in patients subjective sleep quality, as it helped to decrease the time needed to fall asleep, and facilitate the maintenance of wakefulness.

Estudo cefalométrico de indivíduos com síndrome da apnéia hipopnéia obstrutiva do sono.

To evaluate a predicting cephalometric pattern for Sleep Apnea Hipopnea Obstructive Syndrome (SAHOS), a sample of 40 subjects was divided into two experimental groups: patients with mild to moderate and severe SAHOS, previously submitted to polyssonographic study, according to their Apnea or Hipopnea Índex (AHI), comparing them with a control group, 10 subjects, with no sleep disorder. The first experimental group had an AHI=15 <30 (moderate sleep apnea). The second group, an AHI> or = 30 (severe sleep apnea). After the results and the statistical methods used to compare the different studied groups, the conclusion was that the SAHOS patients when compared with the control group presented: some positional alterations in their skeletal anatomical structures, mainly a lowering of the hyoide bone in relation to the cervical vertebrae; mandilbular retrusion, increased dimensions of the pharyngeal soft tissues, increased length and width of the soft palate with a decrease of the posterior and superior airway space. / O propósito deste estudo é identificar associação entre as características craniofaciais esqueletais e de tecidos moles em telerradiografias cefalométricas laterais com os dados clínicos e polissonográficos, verificando as áreas anatômicas que mais contribuem para a obstrução das vias aéreas superiores nos pacientes com Síndrome da Apnéia Hipopnéia Obstrutiva do Sono (SAHOS). Para avaliar se existe um padrão cefalométrico previsível para a Síndrome da Apnéia Hipopnéia Obstrutiva do Sono, foram comparadas as diferenças das médias das dimensões esqueléticas craniofaciais e dos tecidos moles da faringe, em norma lateral, de uma amostra de 40 indivíduos, divididos em 2 grupos-teste, com da Síndrome da Apnéia Hipopnéia Obstrutiva do Sono Moderada e Síndrome da Apnéia Hipopnéia Obstrutiva do Sono Grave, previamente submetidos ao estudo polissonográfico e um grupo controle de 10 indivíduos sem a doença. Pertenciam ao primeiro grupo teste os pacientes com Índice de Apnéia e Hipopnéia (IAH) > 15 < 30 (apnéia do sono moderada) e ao segundo grupo teste os pacientes com Índice de Apnéia e Hipopnéia (IAH) &#8805; 30 (apnéia do sono grave). Diante dos resultados obtidos e após a aplicação dos métodos estatísticos usados para comparação dos diferentes grupos estudados, concluiu-se que os pacientes apnéicos quando comparados com o grupo controle apresentaram: alterações no posicionamento das estruturas anatômicas esqueletais, principalmente um posicionamento inferior do osso hióide em relação às vértebras cervicais, retrusão mandibular, dimensões aumentadas dos tecidos moles faringeanos, aumento no comprimento e largura do palato mole com diminuição do espaço aéreo póstero-superior.
