Chronic headache : an ecosystemic exploration

Chronic headache may be the most frequently reported somatic symptom, yet it puzzles health experts and poses a considerable treatment challenge. It was suggested that this is because conventional views of headache, adhering to a Newtonian-Cartesian epistemology, focus almost exclusively on intrapsychic factors ignoring the wider social context in which the problem is embedded. An overview of the existing body of knowledge on the most widely researched headache conditions was presented, and it was argued that a conceptual shift is required to achieve a more comprehensive understanding of the problem. This study was conducted within an holistic, ecosystemic epistemology. A qualitative approach employing a case study method was adopted to provide rich descriptions of the contexts in which two chronic headache sufferers' symptoms were embedded. Exploring the experiences of mothers after participating in a mother-child interaction intervention, within an HIV context

This research was conducted as part of the formative evaluation of a mother-child interaction intervention, which was incorporated into the Kgolo Mmogo pilot study at the Kalafong Hospital in Tshwane (South Africa). The purpose of the intervention was to encourage the development of parenting skills and to improve mother-child relationships within an HIV context over a six-month period. By promoting more effective interaction between mother and child, child resilience could be enhanced and children could learn the necessary coping skills that would help them deal with the challenges posed by HIV and other life events. The aim of the study was to generate a systemic understanding of families affected by HIV/AIDS. The study explored (1) the effect of HIV on family interaction and (2) the effect of the mother-child interaction intervention on family interaction from the perspective of HIV-infected mothers. The theoretical framework chosen for the study was of a systemic nature and the standpoints, as set out by the Milan family therapy team, were implemented. Ten months after the intervention, four of the HIV-positive mothers who had participated in the intervention were interviewed and encouraged to share their experiences. Circular questions were employed in the interviews as a means of gathering data. A qualitative design was therefore the best option for this study. The research findings of this study coincide with previous literature and research findings; thus, the findings of this study have been consolidated. The current study findings support the importance of bridging the distance that is created by HIV in family relations, particularly between mother and child. Newly-diagnosed mothers often become stuck in their own processes and distance themselves from others in order to make sense of their situation. The broader social stigmas associated with HIV/AIDS contribute further to the sense of isolation that HIV-positive women experience. Often, women perceive HIV to be a disruptive force in their relationships with their partners and children, which creates tension, secrets and uncertainty within the family. HIV-infected mothers generally feel that keeping secrets from their children protects them from being traumatized by the social stigmas surrounding HIV/AIDS. Some HIV-positive mothers also feel that an emotional distance will shield their children from the pain of losing their mother. Because they are grappling with many negative feelings, such as anger and frustration, many HIV-positive mothers resort to strict disciplinary measures and avoid spending time with their children. The children misinterpret their mother’s behaviour and react in a manner that the mother perceives to be both disrespectful and disobedient, thus creating a recurring cycle in which both mothers and children become stuck. The participating mothers perceived the mother-child interaction intervention to focus on their emotional, physical, cognitive and behavioural needs as well as the needs of their children. The mothers experienced the intervention as having been particularly helpful to them and their children. They perceived themselves to be warmer, more supportive, more accepting and to have found meaning in their lives. In addition they felt that their children had begun to behave themselves and were also less avoidant of them. It enhanced their understanding of one another, and strengthened their bond so that they can depend on each other in times of difficulty. Between a baby and a boardroom : social constructions of mothers' employment decisions

A focal point remains the effects of maternal employment on the well-being of women and children, despite shifts in society towards gender equality. The aim of this study was to explore the experiences of mothers who either had chosen to stay at home after having children or to continue with full-time employment; and how their decision impacted their lives. This qualitative study, based on the Social Constructionist paradigm, and Second Order Cybernetics and Feminist Theories, included interviews with four full-time employed and four stay-at-home South African mothers. The data were analysed using hermeneutic analysis. The findings suggest that the participants’ tensions regarding motherhood and employment were strongly influenced by structural obstacles at work and at home, interpersonal relationships with other mothers, as well as intrapersonal experiences. Like death, the defining human reality underlying our relationships and views of life's meaning, the experiences and activities of undertakers, remains an uncommon subject for psychological research. Existential anxiety roots a society-wide denial of the fundamental nature of death. which necessitates the development of institutions to take responsibility for the dying and dead. As the image and service of the stigmatised funeral industry improves, society's experience of death should become more meaningful. The overview of the research into these topics noted the need for further studies. Within a holistic, ecosystemic epistemology, this study adopted a qualitative approach and case study method, which provided descriptions of the ecology, contexts, and relationships characteristic .of the undertaker's vocation by focussing on patterned expressions of views and attitudes. Die studie is outobiografies en is gegrond op die aanname dat daar tussen die psigoterapeut, sy geskiedenis, die wetenskap (etnografie en tweede-orde kubernetika) en die klient(e) 'n unieke patroonverband bestaan. Die psigoterapeut in die platteland het weens afstand of finansies nie altyd die voorreg om supervisie van 'n eksterne supervisor te ontvang nie. In so 'n situasie kan selfsupervisie, deurdat dit 'n proses van selfontdekking is, 'n belangrike rol in die psigoterapeut se selfontwikkeling speel. Selfsupervisie plaas die klem op die psigoterapeut as persoon en as terapeut; oor wie hy is, waar hy vandaan kom en waarheen hy binne die psigoterapeutiese proses op pad is. Vir enige psigoterapeut is dit belangrik om na 'n hoer vlak van psigoterapie te streef. Hierdie strewe impliseer 'n proses van selfondersoek, delwing, selfevaluasie en disseksie. The study is an autobiography based on the assumption that there exists an unique patterned connection between the psychotherapist, his history, science (ethnography and second-order cybernetics), and his clients. Due to distance or financial problems, a rural psychotherapist cannot experience the privilege of supervision with an external supervisor. In such a situation selfsupervision could play an important role in the self-development, because it includes a search of self that will lead to more effectiveness in psychotherapy. Self-supervision focuses on the psychotherapist as a person and therapist, who he is, where he comes from, and in what direction he, as a psychotherapist, is developing within the psychotherapeutic process. It is important to any psychotherapist to strive towards a higher level of psychotherapy. This implies a process of selfinvestigation, dissection and self-evaluation. This process was conducted by means of diary entries in which the psychotherapist interpreted his daily experiences and events in terms of his past. This places the psychotherapist, with his family history and current interpersonal situation, as intrinsic parts of himself, in the foreground. In this process the psychotherapist takes centre stage with emphasis on his responsibility concerning the process of self-supervision. The problems addressed by this personally coloured, scientific study, are problems that the psychotherapist identified through the process of self-supervision and introspection. Such introspection leads to a deeper personal knowledge which the psychotherapist can use to his own benefit but also to the benefit of his family and client system. This study is an attempt to identify a new way of research. It is comprehensive and leads to personal fulfilment and deeper self-knowledge and is also a method by which other psychotherapists could discover themselves and their worlds. It is a slow and painful process. Chapters 1 to 4 comprise of the theoretical rationale of the study and chapters 5 to 12 depict the history of the psychotherapist. The development of a therapist through participation in a reflecting team

This thesis is a study of the development of a group of students during the time they spentworking as a reflecting team in family therapy. Due to an increase in the number of students enrolled for the Masters degree in Educational Psychology in the years from 2001, there was concern about the students experience of family therapy, since they would not all be likely to counsel a family. The study, which was undertaken over a period of two years, allows a comparison of two different experiences of being part of a reflecting team. The first year the whole group formed a single team, whereas, the group in the second year split into two teams, working on a fortnightly basis. The growth of the students as therapists was assessed by means of questionnaires and an interview with the trainer in terms of self-reflection, willingness to risk. / Social work / MA(SS)(Mental Health)
