In-Vitro-Simulated Occlusal Tooth Wear Monitoring by Polarization-Sensitive Optical Coherence Tomography

Indiana University-Purdue University Indianapolis (IUPUI) / Background: Erosive tooth wear (ETW) is the loss of tooth substance due to chemo-mechanical action unrelated to bacteria. ETW affects approximately 46 percent of children/adolescents and 80 percent of adults in the U.S. Visual examination indices are available for the clinical assessment of ETW. Although useful, they are subjective and heavily based on the clinical experience of the examiner. Some quantitative techniques have been proposed and used for clinically assessing erosive tooth wear, including quantitative light-induced fluorescence, ultrasonic measurement, and more recently, polarization-sensitive optical coherence tomography (PS-OCT). Objective: The objective of this study was to explore the ability of PS-OCT to objectively measure erosive tooth wear on occlusal surfaces. Method: This study was conducted in two phases. In the first phase, 10 sound extracted human lower first premolars were selected and then exposed to tooth wear simulation gradually. PS-OCT and micro computed tomography (μ-CT) were used to evaluate enamel thickness of those premolars at the buccal cusp tip during the simulation. In phase 2, 40 extracted human lower first premolars with different severity levels of ETW on occlusal surfaces were selected based on the Basic Erosive Wear Examination (BEWE) index. A total of 10 teeth (n =10) were selected for each BEWE score (0/1/2/3). PS-OCT and μ-CT were used to evaluate the enamel thickness at the highest point on the occlusal surface. Results: There was good agreement between PS-OCT and μ-CT in both phases (phase 1: 0.89 and phase 2: 0.97) with no significant difference between PS-OCT and μ-CT. Conclusion: This result shows the potential of PS-OCT as reliable method for measuring enamel thickness and monitoring tooth wear progression on the occlusal surface

Effectiveness and mechanisms of action of whitening dentifrices on enamel extrinsic stains

These approaches are abrasives, oxidizing agents, and chemical cleaning agents. The objectives of this in-vitro study were: 1) To compare the whitening effect of commercial whitening and non-whitening dentifrices; 2) To verify the mechanism of action of whitening dentifrices by contrasting two experimental models: chemical model (toothpaste exposure only) and chemo-mechanical (toothpaste exposure with tooth-brushing abrasion). The 256 bovine enamel specimens (10 mm x10 mm) were prepared and partially stained. They were assigned to 8 groups: 6 whitening dentifrices, 1 non-whitening reference dentifrice and deionized water (control); and they were further divided in 2 subgroups (n = 16), chemical and chemo-mechanical. Specimens were exposed to dentifrice slurries 2X/day for 1 min and brushed or not, according to each model. In between dentifrice treatments, specimens were exposed to the staining solution for 5 h. This protocol was repeated for 5 consecutive days and enamel color changes (∆E, ∆L) were measured by spectrophotometry after each day. The abrasivity of the toothpastes was determined using a standard test (ISO 11609). Significantly higher ∆E values (whitening effect) were observed for all groups (p < 0.05), except for the control, when tested in the chemo-mechanical model. In this model, the whitening ability of the toothpastes was mainly determined by their abrasive levels. For the chemical model, no significant differences were observed among groups (p > 0.05). Whitening dentifrices can be effective in preventing and removing enamel surface staining, when associated with tooth-brushing abrasion. A Cephalometric Study of the Mesio-Distal Axial Inclinations of the Teeth

Indiana University-Purdue University Indianapolis (IUPUI) / Eighteen adult human skulls with "normal occlusion" were examined with oriented lateral, 15, 25, 45, 75, and 90 degree cephalograms from which the mesio-distal axial inclinations of the teeth were measured. This study indicates that a large variation of the mesio-distal axial inclinations of the teeth may occur and still have a "normal occlusion". The standard deviations varied from a maximum of 11.0 degrees for the maxillary third molar to a minimum of 3.4 degrees for the maxillary cuspid. Rotation of the midsagittal plane of the head with the film effected the measurements of the maxillary first molars and cuspids, and the mandibular bicuspids by producing divergence of their roots on the film image. No relationship was found between measurements of this sample and measurements of mandibular plane, A-B plane, Y-axis, facial angle, angle of convexity, Down's occlusal plane, lower incisor - A-Pg, and interincisal angle. A positive relationship was found between the measurements of this sample and the posterior one-half of nasal floor, posterior occlusal plane, and the labiolingual axial inclination of the lower incisor.

Avaliação clínica dos fatores etiológicos das lesões cervicais não cariosas / Clinical evaluation of etiological factors of noncarious cervical lesions

O propósito desta pesquisa foi investigar a influência de diferentes fatores etiológicos na formação das lesões cervicais não cariosas (LCNCs). Essas lesões foram identificadas e classificadas de acordo com o grau de severidade e correlacionadas com os seguintes fatores: presença/ausência de facetas de desgaste e de biofilme, idade, gênero, hábitos alimentares, história médica, hábitos de higiene oral e hábitos parafuncionais. Foram avaliados 33 estudantes de graduação e pós-graduação, com idade variando entre 18 e 30 anos. A avaliação clínica constou de aplicação de índice de biofilme dental, exame clínico para identificação de LCNCs e de modelos de gesso para identificação de facetas de desgaste e aplicação de questionário. Os resultados mostraram que 25 indivíduos (75,7%) apresentaram, pelo menos, uma LCNC, com maior prevalência nas faces vestibulares, sendo os primeiros molares com maior número de lesões, seguidos pelos primeiros pré-molares; os incisivos foram os dentes com menor número de lesões; os pré-molares foram os dentes que apresentaram lesões com maiores escores de severidade; encontrou-se correlação direta entre a severidade das lesões e a idade (p=0,04; r=0,350) e entre facetas de desgaste e lesões (p<0,05; r=0,605). Dos 250 dentes com lesões cervicais, 217 (86,8%) apresentaram facetas de desgaste e 33 (13,2%) não apresentaram facetas, o que sugere a existência de relação significativa entre a presença de facetas e de lesões (p<0,05); foi encontrada, também, correlação inversa entre a presença de biofilme e o número e a severidade das lesões (p=0,02,r=-0,403; p=0,02; r=-0,426, respectivamente). Os fatores gênero, hábitos alimentares, história médica, hábitos de higiene oral e hábitos parafuncionais não foram diferenciais com relação à presença de LCNCs. Os resultados obtidos reforçam a teoria multifatorial para a etiologia dessas lesões. / The purpose of this research was to investigate the influence of different etiological factors in the formation of noncarious cervical lesions (NCCLs). These lesions were identified and classified according to the degree of severity and correlated with the following factors: presence/absence of wear facets and biofilm, age, gender, dietary habits, medical history, oral hygiene habits and parafunctional habits. Thirty-three undergraduate and graduate students were evaluated, with ages ranging from 18 to 30 years. Clinical evaluation consisted of dental biofilm index application, clinical examination to identify NCCLs and stone casts for identification of wear facets and questionnaire. The results showed that 25 individuals (75.7%) had at least one NCCL, with higher prevalence in buccal surfaces, the first molars with the higher number of lesions, followed by the first premolar; incisors were teeth with fewer lesions; premolars were teeth that had lesions with higher severity scores; direct correlation was found between severity of lesions and age (p=0.04, r=0.350), and between wear facets and lesions (p<0.05, r= 0.605); from 250 teeth with cervical lesions, 217 (86.8%) had wear facets and 33 (13.2%) had no facets, which suggests the existence of a significant relationship between the presence of facets and NCCLs (p<0.05); inverse correlation was found between the presence of plaque and the number and severity of NCCLs (p=0.02, r=- 0.403; p=0.02, r=-0.426, respectively). Chemical Dental Plaque Control: Chlorhexidine Tooth Staining and Efficacy of Common Whitening Procedures

Chlorhexidine mouth rinses remove dental plaque from teeth, preventing dental caries, dental decay, and more serious systemic infections. Tooth discoloration due to extrinsic staining is the most prominent side effect of regular chlorhexidine use. Decreasing the concentration of chlorhexidine reduces staining area and severity. Staining can also be prevented and treated by tooth bleaching, a common cosmetic dental procedure that diminishes extrinsic staining by oxidizing chromagens adsorbed onto the tooth surface. In the proposed trial, common bleaching agents will be investigated for their efficacies in preventing chlorhexidine staining. Results of the trial could further the development of a chlorhexidine mouth rinse that is suitable for long term use.

Quando o dente avulsionado é mantido em condições inadequadas de conservação, a remoção do ligamento periodontal desvitalizado é a conduta mais apropriada para minimizar a ocorrência da reabsorção radicular. Com o objetivo de prevenir e/ou retardar a instalação de reabsorções radiculares e preservar a integridade da camada de cemento propõe-se o estudo do processo de reparo em dentes de ratos reimplantados tardiamente, após a utilização da solução de papaína e fluoreto de sódio no tratamento de superfície radicular. Essa proposta tem como base as propriedades da papaína de remoção do tecido necrótico, ação antioxidante e antimicrobiana. Foram utilizados os incisivos superiores direitos de 40 ratos, divididos em 4 grupos, sendo que no Grupo I os dentes foram imediatamente reimplantados em seus respectivos alvéolos. Nos grupos II, III, IV, os dentes extraídos foram mantidos em meio seco pelo período de 1 hora. No grupo II, os dentes foram imersos em solução de papaína a 50% por 20 minutos, friccionados com gaze por 1 minuto e imerso em fluoreto de sódio fosfato acidulado a 2%, pH 5, pelo período de 20 minutos. No grupo III, os dentes foram mantidos em soro fisiológico por 20 minutos, tiveram a superfície radicular friccionada por 1 minuto e imerso em fluoreto de sódio fosfato acidulado a 2%, pH 5, por um período de 20 minutos. No grupo IV, os dentes foram reimplantados em seus respectivos alvéolos, sem nenhum tratamento a superfície radicular. When the avulsed tooth is improperly conserved, removing the devitalized periodontal ligament is the appropriate conduct to minimize the occurrence of radicular reabsorption. The aim of this study was to search for an effective form of the above process, while preserving the integrity of the cement layer. In this case, papain was used, due to its necrotic tissue removing proprieties and its microbacterial action. The research was conducted with 40 extracted rat incisives, divided into 4 groups. In group I the teeth were replanted immediately in their respective sockets. In groups II, III and IV the extracted teeth were kept in a dry environment for a period of 1 hour. In group II, the teeth were immersed in papain solution (50%) for 20 minutes, rubbed with gauze for 1 minute and immersed in sodium fluoride (2%, pH 5) for a period of 20 minutes. In group III, the teeth were kept in a saline solution for 20 minutes, the radicular surface was rubbed for 1 minute, and they were immersed in sodium fluoride (2%, pH5) for 20 minutes. The dragon's blood (SD) has healing, antiinflammatory, antiviral, antimicrobial and antitumor properties and helps in the collagen new formation. Considering these aspects, the purpose of this study is to analyze the repair process after replantation of rat teeth, using the dragon's blood as a storage medium, for sixty minutes at concentrations of 10% and 100%. Thus, 21 upper right incisors of rats were divided into three groups: group I received immediate replantation; in groups II and III, teeth remained into 10% and 100% SD respectively, for sixty minutes. After the proposed time for groups II and III, teeth were replanted into their respective socket. Sixty days after replantation, euthanasia was performed and the specimens were processed in laboratory and microscopically and morphometrically analyzed. O propósito desta pesquisa foi investigar a influência de diferentes fatores etiológicos na formação das lesões cervicais não cariosas (LCNCs). Essas lesões foram identificadas e classificadas de acordo com o grau de severidade e correlacionadas com os seguintes fatores: presença/ausência de facetas de desgaste e de biofilme, idade, gênero, hábitos alimentares, história médica, hábitos de higiene oral e hábitos parafuncionais. Foram avaliados 33 estudantes de graduação e pós-graduação, com idade variando entre 18 e 30 anos. A avaliação clínica constou de aplicação de índice de biofilme dental, exame clínico para identificação de LCNCs e de modelos de gesso para identificação de facetas de desgaste e aplicação de questionário. Os resultados mostraram que 25 indivíduos (75,7%) apresentaram, pelo menos, uma LCNC, com maior prevalência nas faces vestibulares, sendo os primeiros molares com maior número de lesões, seguidos pelos primeiros pré-molares; os incisivos foram os dentes com menor número de lesões; os pré-molares foram os dentes que apresentaram lesões com maiores escores de severidade; encontrou-se correlação direta entre a severidade das lesões e a idade (p=0,04; r=0,350) e entre facetas de desgaste e lesões (p<0,05; r=0,605). Dos 250 dentes com lesões cervicais, 217 (86,8%) apresentaram facetas de desgaste e 33 (13,2%) não apresentaram facetas, o que sugere a existência de relação significativa entre a presença de facetas e de lesões (p<0,05); foi encontrada, também, correlação inversa entre a presença de biofilme e o número e a severidade das lesões (p=0,02,r=-0,403; p=0,02; r=-0,426, respectivamente). Os fatores gênero, hábitos alimentares, história médica, hábitos de higiene oral e hábitos parafuncionais não foram diferenciais com relação à presença de LCNCs. Os resultados obtidos reforçam a teoria multifatorial para a etiologia dessas lesões. / The purpose of this research was to investigate the influence of different etiological factors in the formation of noncarious cervical lesions (NCCLs). These lesions were identified and classified according to the degree of severity and correlated with the following factors: presence/absence of wear facets and biofilm, age, gender, dietary habits, medical history, oral hygiene habits and parafunctional habits. Thirty-three undergraduate and graduate students were evaluated, with ages ranging from 18 to 30 years. Clinical evaluation consisted of dental biofilm index application, clinical examination to identify NCCLs and stone casts for identification of wear facets and questionnaire. The results showed that 25 individuals (75.7%) had at least one NCCL, with higher prevalence in buccal surfaces, the first molars with the higher number of lesions, followed by the first premolar; incisors were teeth with fewer lesions; premolars were teeth that had lesions with higher severity scores; direct correlation was found between severity of lesions and age (p=0.04, r=0.350), and between wear facets and lesions (p<0.05, r= 0.605); from 250 teeth with cervical lesions, 217 (86.8%) had wear facets and 33 (13.2%) had no facets, which suggests the existence of a significant relationship between the presence of facets and NCCLs (p<0.05); inverse correlation was found between the presence of plaque and the number and severity of NCCLs (p=0.02, r=- 0.403; p=0.02, r=-0.426, respectively). Gender, dietary habits, medical history, oral hygiene habits and parafunctional habits showed no significant differences regarding to the presence of NCCLs. The results support the multifactorial theory for the etiology of these lesions.
