Shifting Paradigms: The Development of Nursing Identity in Foreign-Educated Physicians Retrained as Nurses Practicing in the United States

A unique breed of nurses for the US market is emerging-the Physician-Nurses. They are foreign-educated physicians who have retrained as nurses. The purpose of this study was to generate a theory that can explain the development of their nursing identity. Specific aims were to discover barriers that participants perceived as problematic in their transition to nursing and catalysts that influenced how they addressed the central problematic issue they articulated. Grounded theory methodology guided by the philosophical foundations of symbolic interactionism was used. Twelve Physician-Nurses were interviewed. Transcribed interviews were imported to ATLAS.ti. Text data were analyzed by constant comparative method. Concept formation, development, modification and integration were accomplished through different levels of coding. Methods were employed to ensure trustworthiness of findings. Core categories were discovered and a central social psychological problem experiencing the burdens of a new beginning and a basic social psychological process combining the best of two worlds emerged. Further theorizing generated the substantive theory combining the best of two worlds and the beginnings of a formal theory. The substantive theory explained the three-dimensional central problem and the five-stage basic social psychological process. Dimensions of the central problem were (a) crossing cultures, (b) starting from zero, and (c) crossing professions. Stages of the basic process were (a) letting go of professional identity as physician, (b) experiencing growing pains, (c) seeing nursing as a saving grace, (d) gaining authority to practice as a nurse, and (e) engaging self to nursing and asserting "I am a nurse." The substantive theory is a springboard toward the development of a formal theory which may be able to further explicate the development of nursing identity in Physician-Nurses. This theory named, Theory of Transprofessionalism, was initially conceptualized as having five phases namely: (a) disengagement, (b) discouragement, (c) enlightenment, (d) encouragement, and (d) engagement. These stages correspond to the five stages of the substantive theory. The key concept nursing identity was operationalized by utilizing three statements published by the American Nurses Association that describe the professional registered nurse, the knowledge base for nursing practice, and the code of ethics for nurses.

Samtal för samverkan : En studie av transprofessionell kommunikation och kompetensutveckling om läs- och skrivsvårigheter/dyslexi / Discussion for Development : A study of transprofessional communication and development of inter-disciplinary competence in dyslexia

The purpose of the present thesis is to study whether and how professionals from such different areas as medicine, preschool and school develop and reorganize, through inter-disciplinary focus group discussions, their competence regarding reading-and-writing difficulties/dyslexia. The theoretical framework is based on sociocultural theory of mediated action, focusing on notions such as alterity, intersubjectivity, dialogicality, mastery, appropriation, practical consciousness and discursive consciousness. The data consists of participant field observations, seven individual interviews and fifteen focus group interviews, as well as narratives and written documents concerning collaboration between pre-schools and schools. The method relates to ethnographic and action research. The data is collected over three years. The interviews are audio-recorded and transcribed as close to oral language as possible. The data is analysed with regard to alterity, intersubjectivity, dialogicality, mastery and appropriation, and with regard to professional languages, professional boundaries and domains of responsibility as well as competencies. The results from the individual interviews show that each participant’s conception of dyslexia is closely attached to her or his own field of pracitice. The focus group results show that, over time, the participants changed their conception of dyslexia, and their way of communicating about dyslexia. And, that they thereby deepened their competence on the issue. The written documents show a similar development during the focus group discussions. From hastily scrawled notes they developed into computerised, and well-structured plans of actions for supporting children of special needs. Both the spoken and written data show that inter-disciplinary co-operation leads to the development of inter-disciplinary competence, and a change in organising reading and writing for children of preschool and school.

Целью магистерской диссертации является изучение принципов процесса управления транспрофессиональными компетенциями персонала, исследование уровня транспрофессионализма сотрудников в организациях индустрии СМИ, разработка предложений по усовершенствованию процесса развития транспрофессиональных компетенций. В теоретической части рассмотрены основы управления транспрофессиональными компетенциями персонала, сформулированы особенности их управлением, а также проведена оценка степени распространение транспрофессионализма в современных СМИ. В практической части описаны база исследования, дана краткая характеристика системы управления и кадрового состава исследуемых организаций индустрии СМИ, обозначены методические основы исследования, проведен анализ уровня транспрофессионализма сотрудников в исследуемых организациях и обобщены его результаты. На основе полученных данных разработаны предложения по совершенствованию процесса управления транспрофессиональными компетенциями сотрудников. Научная новизна исследования определяется как постановкой научной проблемы, так и полученными результатами: впервые предпринят теоретический и эмпирический анализ транспрофессиональных компетенций персонала в организации индустрии средств массовой информации и осуществлен анализ проблем управления, влияющих на развитие транспрофессиональных компетенций в организации индустрии СМИ. / The purpose of the master's thesis is to study the principles of the process of managing the personnel transprofessional competencies, to investigate the level of transprofessionalism of employees in media industry organizations, to develop proposals for improving the process of developing transprofessional competencies. In the theoretical part, the foundations of the management of personnel transprofessional competencies are considered, the features of their management are formulated, and the degree of spread of transprofessionalism in modern media is assessed. The practical part describes the basis of the research, gives a brief description of the management system and staffing of the media industry organizations under study, outlines the research methodological foundations, analyzes the level of transprofessionalism of employees in the organizations under study and summarizes its results. Based on the data obtained, proposals were developed to improve the process of managing the employees transprofessional competencies. The scientific novelty of the research is determined by both the formulation of the scientific problem and the results obtained: for the first time, a theoretical and empirical analysis of the transprofessional competencies of personnel in the organization of the media industry was undertaken and an analysis of management problems affecting the development of transprofessional competencies in the organization of the media industry was carried out.

In the theoretical part, the aspects of designing the technology of personnel training in the transition to a trimodal organization are considered. The practical part describes the General characteristics of the company under study, analyzes the personnel structure, and analyzes the HR-business process "personnel Training" in Sberbank. Based on these results, a project was developed to develop the staff's TRANS-professional competencies.
