Nos serviços de saúde, o paciente se torna um integrante do processo produtivo ao exercer a função de coprodutor, realizando a gestão do seu tratamento de forma autônoma. Nesta perspectiva, é preciso entender como os serviços e os profissionais de saúde gerenciam o uso de medicamentos no contexto específico de tratamentos de longo prazo, como no caso da Terapia Antineoplásica Oral (TAO), utilizada no tratamento do câncer. Este trabalho propôs um conjunto de ações com o objetivo de reduzir falhas no processo de gestão da coprodução em tratamentos com TAO no âmbito hospitalar. Para atender a este objetivo, foi elaborada uma estrutura conceitual, a partir de um levantamento bibliográfico que pesquisou os conceitos de Coprodução em Serviços, Falhas em Coprodução, Serviços de Saúde e TAO, e de uma revisão sistemática de literatura sobre os conceitos de Coprodução em Serviços de Saúde. A partir desta estrutura conceitual foram definidas as categorias de análise, utilizadas como base para a elaboração do instrumento de coleta de dados. Um questionário semiestruturado com questões abertas foi o instrumento de coleta utilizado nesta pesquisa, realizada a partir de uma abordagem qualitativa, adotando como método o estudo de caso. Este estudo de caso foi aplicado junto a um Serviço de Oncologia, situado em um hospital de grande porte, localizado no estado do Rio Grande do Sul. As entrevistas foram analisadas por meio técnica de análise de conteúdo, que tem por objetivo interpretar as características e os padrões que estão por trás do conteúdo das mensagens e documentos. Para dar suporte a análise de conteúdo, foi utilizado o software para análise de dados qualitativos Atlas.ti – versão 8.0. Com a consolidação dos dados, os achados da pesquisa foram comparados por meio da triangulação, envolvendo resultados, perspectivas ou teorias, pesquisadores ou métodos, como uma resposta a fidedignidade e a validade do estudo com os achados teóricos. Os resultados indicam que os processos de trabalho ainda não estão ordenados de modo a produzir o melhor resultado na entrega do serviço para os pacientes no contexto dos tratamentos com TAO. Assim, o conjunto de ações proposto sugere a adoção de procedimentos para a redução de falhas na gestão da coprodução no que diz respeito à abordagem integral do paciente, adequação a rotina de distribuição de medicamentos, educação para coprodução, gestão de demanda, gestão da coprodução, e otimização do fluxo de solicitação e autorização de medicamentos. / Coproduction in Health Services and related processes are still under-researched topics. In health services, the patient becomes an integral part of the productive process by performing the role of coproducer, performing the management of his treatment autonomously. In this perspective, it is necessary to understand how the services and the health professionals manage the use of drugs in the specific context of long-term treatments, as in the case of Oral Antineoplastic Therapy (TAO), used in the treatment of cancer. This work proposed a set of actions with the objective of reducing failures in the process of coproduction management in TAO treatments in the hospital setting. In order to meet this objective, a conceptual framework was developed, based on a bibliographical survey that investigated the concepts of Co-production in Services, Failures in Coproduction, Health Services and TAO, and a systematic literature review on the concepts of Co-production in Services of Health. From this conceptual framework, the categories of analysis were defined, used as basis for the elaboration of the instrument of data collection. A semistructured questionnaire with open questions was the collection tool used in this research, based on a qualitative approach, adopting the case study as a method. The case study was carried out with an Oncology Service, located in a large hospital, located in the state of Rio Grande do Sul. The interviews were analyzed through a content analysis technique, whose purpose is to interpret the characteristics and the standards behind the content of messages and documents. To support the analysis of the content, the software for qualitative data analysis Atlas.ti - version 8.0 was used. With the consolidation of data, the research findings were compared through triangulation, involving results, perspectives or theories, researchers or methods, as a response to reliability and the validity of the study with the theoretical findings. The results indicate that the work processes are still not ordered in order to produce the best result in the delivery of the service to the patients in the context of the TAO treatments. Thus, the proposed set of actions suggests the adoption of procedures to reduce failures in coproduction management with respect to the integral approach of the patient, adequacy to the routine of drug distribution, education for coproduction, demand management, coproduction management, and optimization of the flow of medication request and authorization.

Evaluating antiviral activity of nucleic acid binding proteins across species / Évaluation de l'activité antivirale des protéines de liaison aux acides nucléiques : une analyse multi-espèces

Le projet a permis d’identifier des répertoires de protéines interagissant avec différentes espèces d’acides nucléiques caractéristiques des virus chez cinq espèces animales (Homme, souris, poulet, drosophile, nématode). Ces protéines représentent des candidats pour remplir des fonctions de récepteurs de l’immunité innée ou de molécules antivirales. Certaines d’entre elles ont été conservées au cours de l’évolution, ce qui m’a permis de tester leur fonction dans la drosophile. J’ai réalisé un crible impliquant des infections avec cinq virus différents sur 100 protéines conservées. Ce crible m’a permis d’identifier huit protéines dont l’inhibition impacte la réplication virale. Deux d’entres elles, CG5641 et Zn72D, sont nécessaires pour la réplication des virus de type picornavirus (CrPV). Le candidat le plus intéressant identifié est cependant la protéine Tao, dont l’inhibition entraîne une augmentation de la réplication de virus appartenant à plusieurs familles, chez la drosophile et dans les cellules de mammifères. / Antiviral response largely relies on the recognition of viral nucleic acids. The aim of the project was to characterize the range of nucleic acid binding proteins in the context of viral infection in flies. We identified a wide repertoire of proteins, which recognize viral nucleic acids in five species (human, mouse, chicken, fruit fly and roundworm). Among these proteins, there are ones, which are conserved in insects and humans, and therefore their function can be easily studied in the fruit fly model. Afterwards, we have performed a large screen in flies to study more precisely the function of 100 proteins in infection with 5 different viruses. We have found eight promising candidates as a result of this screen. We identified two Drosophila proteins CG5641 and Zn72D, which are also present in humans, as proviral factors. We also identified a protein Tao, which is conserved in humans, and is antiviral against several types of viruses.

蘭嶼位於台灣東南方的外海,是孤懸於西太平洋邊的島嶼,是達悟族人世居之地。達悟族是南島民族的一支,其語言、文化與菲律賓北方的巴丹島相近,應屬同語、同種而相互分離的族群,達悟族的起源傳說主要以石生、竹生為主,但野銀部落則採巴丹移入的說法,印證蘭嶼與巴丹的密切關係。 達悟族屬海洋文化,其傳統神觀為何?眾說紛紜。但其畏懼anito則是確定的。anito一般被解讀為亡魂,此種說法有窄化anito之嫌,anito更好視之為所有邪惡力量的統稱。無論如何,達悟族的神鬼信仰反應在其傳統的歲時祭儀與生命禮俗中,特別是每年舉辦的meyvanwa(召飛魚祭)、pazos(天神祖靈祭),以及傳統命名儀式、新屋落成或大船下水儀式和充滿禁忌的喪葬儀式中。 基督宗教的傳入,對達悟人的生活帶來根本的改變,特別是1954年將天主教傳入蘭嶼的紀守常神父。紀神父對蘭嶼的影響是徹底而深遠的,一般人總以為天主教的傳入是靠救濟物資的吸引,這是表象,真正的關鍵因素在於達悟族傳統文化對天主教的理解與調適,天主教本身亦處變遷之中,達悟族對變遷中的天主教的理解與調適尚在持續進行中。 天主教的傳入蘭嶼,除了興建教堂、傳教施洗外,天主教傳教士更具遠見的培植達悟人才,尊重當地文化,天主教對達悟族的民族發展做出貢獻,特別是在建立民族自信、優質文化的復振、族語的傳承、教育、婦女地位的提升以及基督徒合一方面。但在獨身聖召、教會自養、禁止離婚、青少年宗教教育等問題上,天主教與達悟文化正面臨相互的考驗。未來發展如何?值得持續關注。 關鍵字:達悟族、基督信仰、文化復振 / Précis of Thesis Orchid Island is an isolated island southeast of Taiwan in the Pacific Ocean, inhabited by the Tao people (formerly called Yami). The Tao are an Austronesian people. Their language and culture are very similar to those of the Batan Islanders in the north of the Philippines. These two groups are linguistically and anthropologically homogeneous, though they are now politically divided. There are two traditions of their origin: one claims that they originate from rock; the other that they originate from bamboo. But in the village of Ivalino there is a tradition that they came from Batan Island, which is a sign of the close relationship between the Tao of Orchid Island and the Batan Islanders. The Tao are an oceanic people. There is a great controversy over their conception of God. What is certain, however, is their fear of ‘anito’. ‘Anito’ is the collective term for all evil forces and is often interpreted as referring solely to spirits of the deceased, but this usage is overly restricted. The belief of the Tao concerning ghosts is reflected in their traditional celebrations throughout the year, especially the yearly Meyvanwa (Calling the flying fish), Pazos (Offering to God and the ancestors), as well as the traditional name-giving ceremony, the inauguration ceremony of a new house, the launching of a new boat and the burial ritual, which is full of taboos. The advent of Christianity has brought radical changes to the lives of the Tao, especially with the introduction in 1954 of the Catholic Church through Fr. Alfred Giger. The influence of Fr. Giger was thorough and far-reaching. Though generally people believe that the spread of the Catholic Church to Orchid Island was effected through the attraction of the Church’s distribution of relief goods, yet this is only a superficial understanding. The key factor was the way the Tao perceived the Catholic faith and the way they adapted to it. The Catholic Church has also adapted in this process. This ongoing understanding and adaptation of the Tao to the progressing Church is a process that is still going on. While the Catholic missionaries who arrived on Orchid Island built churches and baptized people, they were also men of vision, who formed the Tao whilst respecting their local culture. The Catholic Church contributed towards the development of the tribe, especially by strengthening the self-confidence of the people, encouraging a renaissance of their culture, handing down the tribal language, education, raising the status of women as well as promoting ecumenism between the Christian Churches. However, the Catholic Church’s position on celibate religious vocations, self-support, and opposition to divorce led it to conflict with Tao culture. There are also difficulties in ensuring religious education of the youth. Continued attention should be given to the future of the interaction of the Tao and the Catholic Church to see where it will lead. Key words: Tao people, Christianity, Cultural renaissance.

History as Meta-Theater: Kong Shangren’s (1648-1718) The Peach Blossom Fan

This dissertation examines the uses of meta-theater in The Peach Blossom Fan, an early Qing historical drama by Kong Shangren (1648-1718), arguing that the meta-theatrical elements of the play serve as an innovative form of historiography. Kong Shangren, a member of the Confucian Kong lineage, is unusual for a Chinese playwright: he was steeped more deeply in the world of Confucian ritual music than the work of writing lyrics for dramatic arias, yet The Peach Blossom Fan is recognized as one of the last great chuanqi dramas of the Ming-Qing period. Kong wrote at a time of great social and cultural transformation, completing The Peach Blossom Fan not long after the violent conflicts of the mid-17th century Ming-Qing dynastic transition were finally coming to an end. At the same time, the literary genre of chuanqi drama was also in the midst of its own transitions, as writers of the early Qing increasingly turned to other literary genres beyond this popular late Ming form. I argue that The Peach Blossom Fan marks a key transition in the development of the chuanqi drama, owing both to the play’s formal innovations that exceed the traditional chuanqi form, such as its rejection of the conventional “grand reunion” finale and re-envisioning of the role-type system, and also to its synthesis of historiographical judgements with the world of theatrical performance. Focusing on the play’s uses of meta-theater, I show how The Peach Blossom Fan models the work of historiography by guiding its readers to cultivate the “cold, clear eyes” of a historical witness. Kong Shangren’s methods as a playwright-historian are at their best in The Peach Blossom Fan’s engagement with Ruan Dacheng (1587-1646): a blacklisted late Ming politician who was also a well-received playwright in his own time. Ruan’s life and work have been largely neglected in previous scholarship, despite his significance as a 17th century dramatist. The Peach Blossom Fan uniquely places Ruan Dacheng on stage as a dramatic character at the same time as it turns Ruan’s own chuanqi drama, The Swallow Letter, into an unsuccessful play-within-a-play. In so doing, The Peach Blossom Fan invites reflection on the writer alongside his work, synecdochically turning The Swallow Letter into the music of the collapsing Ming Dynasty and pronouncing Ruan’s fate as the villainous playwright who wrote it. Kong thereby creates a new dramatic motif of the “playwright on stage” — a method of meta-theatrical literary criticism that is picked up by later playwrights, such as the mid-Qing writer Jiang Shiquan. In The Peach Blossom Fan, Kong Shangren also creates a new vision for the worldly stage within and around his play; one in which the problems of social and theatrical performance are tied up in the formal world of the printed chuanqi drama. I analyze the textual dimensions of the play’s meta-theatrical innovations by focusing on Kong’s engagement with the late Ming Linchuan drama school, from The Peach Blossom Fan’s performative re-casting of the familiar female self-portrait motif, to the play’s meta-theatrical reflections on Kong’s own position as its early Qing playwright. The Peach Blossom Fan is framed through a series of paratexts, including an account of how the play itself came into being. The self-reflexivity of The Peach Blossom Fan as a literary text thereby extends its meta-theatrical frames to Kong Shangren’s world as its playwright, using notions of theatrical performance to examine the work of reading, writing, and ritual. Taken together, I contend, these layers of The Peach Blossom Fan theatricalize the literary genre of the chuanqi, drawing attention to the representational limits of historical narratives and capturing the ways in which writing is yet another form of performance.

Resection of DNA double strand breaks in the germline of Caenorhabditis elegans

Repair of double-strand DNA breaks (DSBs) by the homologous recombination (HR) pathway results in crossovers (COs) required for a successful first meiotic division. DSB resection is the nucleic degradation of DSB ends to expose 3’ single strand DNA (ssDNA), an intermediate required for HR. To investigate genes involved in meiosis, a forward genetic screen was performed to search for novel genes or informative new mutant alleles of known genes. Mre11 is one member of the MRX/N (Mre11-Rad50-Xrs2/Nbs1) complex required for meiotic DSB formation and for resection in budding yeast. In Caenorhabditis elegans, evidence for the MRX/N’s role in DSB resection is limited. We isolated the first separation of function allele in C. elegans , mre 11(iow1), isolated from our forward genetic screen. The mre-11(iow1) mutants are specifically defective in meiotic DSB resection but not in DSB formation. The mre 11(iow1) mutants display chromosomal fragmentation and aggregation in late prophase I. Recombination intermediates and crossover formation is greatly reduced in mre 11(iow1) mutants. Irradiation induced DSBs during meiosis fail to be repaired from the early to middle prophase I in mre 11(iow1) mutants. Our data suggest that some DSBs in mre 11(iow1) mutants are repaired by the non homologous end joining (NHEJ) pathway because removing NHEJ partially suppresses some meiotic defects conferred by mre 11(iow1). In the absence of NHEJ and a functional MRX/N, meiotic DSBs are channeled to an EXO 1 dependent form of recombination repair. Overall, our analysis supports a role for MRE-11 in the resection of DSBs in early to middle meiotic prophase I and in blocking NHEJ. A reverse genetic screen and a yeast two hybrid screen were performed to search for genes with genetic and/or physical interactions with mre-11. The reverse genetic screen isolated a novel meiotic gene, nhr-2, as a partial suppressor of the meiotic defects conferred by mre-11(iow1). The yeast two hybrid screen identified kin-18 interacting with mre-11. KIN-18 is the C. elegans homolog of mammalian Thousand And One kinase (TAO) kinase. KIN-18/TAO is MAPK kinase kinase whose meiotic role was unknown. We have found that KIN-18 is essential for normal meiotic progression as kin-18 mutants exhibit accelerated meiotic recombination, ectopic germ cell differentiation, and enhanced levels of germline apoptosis. In C.elegans MPK-1 activation in late pachytene is required for physiological apoptosis (nuclei removed by apoptosis serve as nursing cells for oocytes) and oocyte differentiation. The kin-18 mutants also showed absence of MPK-1 activation and aberrant MPK-1 activation that includes ectopic activation in the wrong regions in the germline or more than one time of activation. The progression defects in kin-18 mutants are suppressed by inhibiting an upstream activator, KSR-2, of the canonical MPK-1 signaling. Our data suggest KIN-18 affects meiotic progression by modulating the timing of MPK-1 activation. This regulation ensures the proper timing of recombination and normal apoptosis, which is required for the formation of functional oocytes. Meiosis is a conserved process; revealing that KIN-18 is a novel regulator of meiotic progression in C. elegans will motivate hypothesis for TAO kinase’s role in the germline development in higher eukaryotes. Meiosis is a crucial for sexually reproducing organisms to maintain ploidy level from one generation to the next. Accurate chromosome segregation in the meiosis requires meiotic recombination between homologous chromosomes. Failure in recombination can lead to abnormal segregation of chromosomes in meiosis, which leads to aneuploidy. Anueploidy is a leading cause of miscarriages and attributes to chromosomal related birth defects. Meiotic recombination starts with programmed DNA double strand breaks (DSBs), followed by repair of these DSBs by homologous recombination (HR) pathway. One key step in HR is resection, a process to covert DSB ends into single strand DNA (ssDNA). To broaden our understanding of meiotic DSB resection, we used a nematode, C. elegans, as a model to investigate genes in DSB resection. We have isolated a specific mutant allele of a meiotic gene, mre-11. Our data suggest meiotic DSB resection in C. elegans requires collaboration of mre-11 and another gene exo-1; efficient resection of DSB ends is important to safeguard repair of DSB by HR against other illegitimate repair pathway. In addition, we identified a gene kin-18 by looking for genes interacting with mre-11. Characterization of kin-18 show meiotic recombination is tightly coordinated with germ cell progression. Our analysis provides significant improvement in the understanding of meiotic recombination in C. elegans. Given the high conservation of the two genes, mre-11 and kin-18, our finding may be applied to other organisms.
