O objetivo do presente estudo foi avaliar a eficácia da analgesia preemptiva na cirurgia de extração de terceiros molares inclusos. Nesse estudo duplo cego, randomizado e pareado, os pacientes foram submetidos à extração de terceiros molares inferiores bilaterais em dois tempos distintos. Em uma das duas cirurgias, no pré-operatório os pacientes fizeram uso de Cetoprofeno 150 mg via oral a cada 12 horas durante dois dias antes e, após a cirurgia, continuaram a medicação por mais três dias. Em outra cirurgia, fizeram uso de um comprimido placebo no pré-operatório a cada 12 horas durante dois dias antes e, após a cirurgia, fizeram uso de Cetoprofeno 150 mg a cada 12 horas por três dias. O analgésico de resgate utilizado foi o Paracetamol 750 mg via oral. A dor pós-operatória foi avaliada por meio de Escala Visual Analógica, Escala Nominal e quantidade de consumo de analgésicos de resgate. Os resultados não mostraram diferença significante entre o controle e cetoprofeno preemptivo na redução da dor pós-operatória e no consumo de analgésicos de resgate. Concluiu-se que neste modelo experimental, a analgesia preemptiva não se mostrou eficaz na redução da dor pós-operatória. / The aim of this study was to evaluate the effectiveness of the preemptive analgesia in the third molar surgery. In this double blind study, randomized and paired, the patients were submitted to the extraction of the third molar bilateral inferiors in two distinct times. In one of the two surgeries, during the two-day preoperative the patients made useof150 mg of ketoprofenoralevery12 hours and, after the surgery, they kept taking this medication for three days. In the other surgery, a placebo pill was used in the preoperative, also every 12 hours, for two days and, after the surgery, ketoprofen 150 mg was used every 12 hours for three days. The rescue analgesic used was the paracetamol 750 mg oral. Postoperative pain was evaluated by making use of the Analogical Visual Scale, the Nominal Scale and the amount of rescue analgesic consumption. The results did not show significant difference between the control and the preemptive ketoprofen in the reduction of postoperative pain and in the rescue analgesic consumption. We can conclude that in this experimental model, the preemptive analgesia did not prove to be efficient in the reduction of postoperative pain.

The Effects of the Ready for Success Classroom Guidance Program on the Social-Emotional Skills and Competence, Reading Proficiency, and Promotion Rate of Third-Grade StudentsUnknown Date (has links)
The purpose of the current outcome study was to determine the effects of the
Ready for Success (RFS) classroom guidance program (Brigman & Webb, 2012) on the
social-emotional skills and competence, reading proficiency, and promotion between
third-grade students who received the RFS intervention (treatment group; n = 104), and
third-grade students who did not receive the intervention (comparison group; n=91).
Following training in the manualized RFS curriculum, certified school counselors in the
treatment group implemented five, weekly, 30-minute lessons followed by three monthly
booster lessons. This study followed a quasi-experimental, non-equivalent group design.
The study employed a teacher report measure of social-emotional skills and competence
(i.e., Devereux Student Strengths Assessment) and a standardized formative assessment
of reading proficiency (i.e., Reading Running Record). Furthermore, retention data was
provided by the participating school district’s data source. Statistical significance between the groups was measured by a series of
MANCOVA analyses and a Pearson’s chi-square analysis. A partial eta-squared
ηp2 effect size was reported for each dependent variable. The data supported the RFS
classroom guidance program as an effective Social-Emotional Learning (SEL)
intervention for promoting student social-emotional skill development (i.e. selfawareness,
self-management, social awareness, relationship skills, and responsible
decision making) and overall social-emotional competence (SEC). This study provided
support for the use of school counselor-led SEL classroom programs to promote the
social-emotional development of students in the school setting. Furthermore, the study
further supports the value of school counseling interventions for students in the domain
of social-emotional development. Finally, the findings of this study provided empirical

Avaliar a eficácia analgésica da associação de 30mg do fosfato de codeína com 500mg do paracetamol após exodontias de terceiros molares inferiores impactados. Foi realizado um estudo clínico bilateral com uma amostra de 47 pacientes. Em um dos lados, todos os pacientes receberam a dosagem de 30mg do fosfato de codeína em associação com 500mg do paracetamol após exodontia (grupo teste). Para a exodontia contralateral, foi disponibilizado outro frasco contendo cápsulas idênticas, porém com a dosagem de 500mg de paracetamol (grupo controle). 100% dos pacientes do grupo teste não necessitaram utilizar a medicação resgate e não consumiram doses adicionais da medicação após as cirurgias. No grupo controle, 44,7% dos participantes relataram o uso do medicamento resgate. O consumo total de comprimidos no grupo teste foi, em média, inferior quando comparados ao lado contralateral. 80,8% dos pacientes relataram maior conforto, quanto ao critério da dor, no lado em que foi utilizado a dosagem de 30mg de fosfato de codeína associado a 500mg de paracetamol. Os efeitos colaterais estiveram mais presentes no grupo teste, sendo mais comum o relato de sonolência (34%) e tontura (31,9%), não havendo relato de abandono desta medicação por nenhum dos pacientes. Concluímos que a dosagem de 30mg do fosfato de codeína associada a 500mg de paracetamol apresentou resultados favoráveis no controle da dor e uma baixa incidência de efeitos colaterais. / To assess the analgesic efficacy of regular dosage of codeine phosphate 30mg association with paracetamol 500mg after extraction of impacted lower third molars. We performed a bilateral clinical study analyzing a sample of 47 patients. All patients received a 30mg codeine phosphate dosage in combination with paracetamol 500mg after extraction (test group). For the contralateral tooth extraction, we had another bottle available containing identical capsules, with a 500mg paracetamol dosage (control group). 100% of the test group patients did not need to use rescue medication and did not consume additional doses of medication after surgeries. In the control group, 44.7% reported the use of rescue medication. Total consumption of pills in the test group was on average lower than the contralateral side. 80.8% of patients reported greater comfort, as the criterion of pain in the side that was used 30mg codeine phosphate dosage associated with paracetamol 500mg. The adverse effects were more present in the test group, with sleepiness being more common (34%) and dizziness (31.9%), without any patient medication abandonment. We conclude that the 30mg codeine phosphate dosage associated with paracetamol 500mg showed favorable results in controlling pain associated with a low incidence of side effects.

O Objetivo do presente estudo foi avaliar a relação entre ansiedade odontológica e cortisol salivar por meio de escalas e o nível de cortisol salivar em pacientes submetidos à exodontia dos terceiros molares inferiores. O estudo envolveu 15 pacientes com idade entre 18 e 29 anos, do gênero masculino e ASA I (saudável), submetidos à exodontia dos terceiros molares inferiores bilateralmente sob anestesia local. Foram coletados dados subjetivos por meio de escalas (Escala de Ansiedade Dental de Corah, Escala Visual Análoga de ansiedade e dor, Inventário de Ansiedade Traço-Estado de Spielberger, Escala Verbal e Escala Visual Numérica de ansiedade e dor) e objetivo por meio da coleta de cortisol salivar. Os dados obtidos revelaram correlação entre ansiedade odontológica e dor detectada por meio da escala visual numérica, que evidenciou diferença estatística significante na redução quando comparado os períodos de 3 minutos após a anestesia e 2 horas após o início da segunda cirurgia. O cortisol salivar no período de 2 horas após o início da primeira e segunda cirurgia expôs alteração da curva do ritmo circadiano elevando-a, contudo sem diferença estatística. As cirurgias para extração de terceiros molares inferiores foram capazes de alterar a curva do cortisol mesmo em pacientes com baixo nível de ansiedade. A experiência interfere na resposta do paciente frente à ansiedade e dor, se de forma positiva, reduzindoas de modo significante. / The objective of this study was to evaluate the relationship between dental anxiety and salivary cortisol using scales and salivary cortisol levels in patients undergoing extraction of third molars. The study involved 15 patients aged between 18 and 29 years, male and ASA I (healthy), undergoing extraction of third molars bilaterally under local anesthesia. Data were collected by means of subjective scales (Corahs Dental Anxiety Scale (DAS), Anxiety Visual Analogue Scale (VAS) of anxiety and pain, Spilberger State-trait Anxiety Inventory (STAI), Verbal Scale and Visual Numeric Scale of anxiety and pain) and objective by collecting salivary cortisol. These data revealed a correlation between dental anxiety and pain detected by visual numerical scale, showing statistically significant difference in reduction compared periods of 3 minutes after anesthesia and 2 hours after starting the second operation. The salivary cortisol within 2 hours after the onset of first and second operation curve exhibited alteration of circadian rhythm raising it, however no statistical difference. Surgeries for extraction of third molars were able to change the curve of cortisol even in patients with low anxiety levels. The experience interferes with the patient\'s response against the anxiety and pain, in a positive way, reducing them so significant.

Algumas das posições de inclusão do terceiro molar podem acarretar comprometimento periodontal nos segundos molares adjacentes. Com isso o presente trabalho buscou desenvolver através deste estudo clínico controlado, um protocolo com o laser de baixa intensidade para aplicação no pós-operatório das exodontias de terceiros molares mandibulares, de modo a minimizar os sinais e sintomas e melhorar os parâmetros periodontais dos segundos molares adjacentes. O laser empregado foi o Photon Laser III - DMC, São Carlos, SP/Brasil - como meio de condução o vermelho, com comprimento de onda 660 nm, potência útil de 30 mW, meio ativo AsGaAl. Sessenta pacientes de ambos os gêneros, com necessidade de exodontia dos terceiros molares, em classificações padrões de posição, foram randomizados em três grupos para análise - Grupo I: 10J/cm2, Grupo II: 30J/cm2 e Grupo III: Sham, e acompanhados pelo período de 6 meses. A análise do trismo e edema facial no pós-operatório e do nível clínico de inserção encontraram diferenças estatísticas significantes entre os grupos que receberam a terapia laser quando comparado ao grupo sham. Portanto, a utilização do laser de baixa intensidade como terapia adjuvante após a exodontia de terceiros molares demonstrou efetividade. Constructing, negotiating and reconstructing English Language Learner Identity : a case study of a public sector university in Postcolonial PakistanUmrani, Sumera January 2016 (has links)
This is an instrumental case study that focuses on the construction of English Language Learner Identity (ELLI) in postcolonial Pakistan. It is a study of students at a public sector university in the province of Sindh. The study broadly examines how English language learners reconstruct, redefine and negotiate their language learner identities during their English language learning journeys. In particular, it attempts to explore learners’ investment and agency in learning English and what ‘future possible selves’ they want to achieve after acquiring English language skills. Consideration is given to how learning English as a second language may be impacted by students’ gender, social class and ethnolinguistic selves and how learners’ English Language Learner Identity is formed and reformed in postcolonial Pakistan. This instrumental case study of the University of Sindh did not attempt to explore the case in its entirety but rather studied a particular aspect of it. In order to gather the data for my study I recruited three cohorts - primary (Year 1) and secondary (Year 2, 3 and 4) participants and other stakeholders from the Institute of English Language and Literature (IELL), the University of Sindh (UoS). Year 1 students were the key participants in the study but with the involvement of 2nd, 3rd and 4th year students, I was able to construct a possible sense of language learner identity and language learning evolution beyond the first year students’ experiences. Year 1 students were interviewed twice over a twelve-month period during which time they also wrote reflective diaries twice a month. Engagement with each of the other year groups involved one focus group discussion with each year once only in the middle of the data collection journey. In addition, participant shadowing and non-participant classroom observations were also utilised to enhance understanding and to triangulate the data. The views of other stakeholders such as the language teacher, the Director of the Institute and the Dean of the Faculty were also gathered to supplement and inform the data collected from students. The key findings of this study suggested that investment, learner agency, desire for possible future selves and historical and cultural consciousness are the main constructs of language learner identity in postcolonial Pakistan. Learners have invested in English language learning through a number of processes and have had unique language learning journeys exercising their learner agency. It reinforced an understanding of learners’ identities as dynamic and multidimensional and fluid in nature, being continually reconstructed and negotiated over time in different academic, social and cultural contexts leading to a hybridised English Language Learner Identity (ELLI) situated in the ‘third space’. It was found that Year 1 students were open to negotiating their multidimensional identities but conformed to an acceptance of the primacy of English while their senior counterparts resisted and sometimes challenged not only English language learning but also the significance of English as a mechanism for linguistic and cultural manipulation. It was noticed that the nature and extent of investment, agency and identity negotiations were related to learners’ individual experiences, social class, academic, family and ethnolinguistic background and their year of the degree programme.

O propósito desta tese foi investigar como tem ocorrido o processo de sucessão em empreendimentos sociais no Brasil, com foco no associativismo produtivo, identificando quais fatores limitam e quais facilitam esse processo a partir das dimensões Indivíduo, Organização e Ambiente. Trata-se de uma pesquisa de caráter exploratório e descritivo, desenvolvida em duas etapas complementares. Na primeira etapa, o grupo amostral compôs-se de 378 empreendimentos, que foram objeto de um survey, para identificar as associações produtivas. Destas foram selecionadas para constituírem a amostra aquelas que já passaram ou estão passando por processo sucessório e cujos dirigentes demonstraram disposição em participar da pesquisa. Na segunda etapa, composta por uma amostra com 32 empreendimentos, buscou-se analisar como interagem as dimensões Indivíduo, Organização e Ambiente na configuração do processo sucessório, identificando quais os fatores que, em cada uma dessas dimensões, podem facilitar e aqueles que podem limitar o processo sucessório. Para analisar a estrutura e as principais características do processo sucessório, tomou-se por base os seguintes eixos orientadores: dimensão Indivíduo - papéis da liderança, habilidade da liderança e estilos de liderança; dimensão Organização - estrutura, planejamento, conselhos, comunicação (transparência), controle e avaliação; dimensão Ambiente - influências dos stakeholders (comunidade, fornecedores, clientes, parceiros) no processo sucessório. Os resultados obtidos apontaram que uma das principais características das associações produtivas pesquisadas é a existência de um entrelaçamento entre dimensões política, econômica e social. No entanto, tais iniciativas têm como um dos principais desafios, o de criar contextos favoráveis a essa organização socioeconômica e política entre cidadãos que compartilham mais carências que recursos para a construção e manutenção de um projeto associativo. Sobre o processo sucessório, os dirigentes das associações mostraram-se conscientes da importância de planejar e de gerenciar seu desenvolvimento, mas dispõem ainda de ferramentas pouco estruturadas para isso. As associações também atribuem importância à gestão compartilhada do processo sucessório como forma de conciliar expectativas tanto do público interno, quanto do público externo, porém, não dispõem de um sistema estruturado de administração desse processo. O processo sucessório nas associações pesquisadas encontra-se em fase de construção: adapta-se às demandas do cenário atual, mas apresenta evidentes necessidades de aprimoramento para uma condução mais efetiva do planejamento e da gestão compartilhada do processo. / The purpose of this thesis is to investigate how the succession process in Brazilian social enterprises have been executed, focusing on productive social activism. The study identifies which factors limit and facilitate this process, based on the following aspects: Individual, Organizational and Environmental. The nature of this research is exploratory and descriptive and was developed in two complementary stages. In the first stage the chosen group consisted of 378 enterprises that were the subject of a survey to identify the productive associations. The survey was set up to identify associations which are productive and within them, the enterprises which are active and those which are going through a succession process. The disposition of the leaders in taking part in the survey was also taken into consideration. The second stage was composed of a sample of 32 enterprises in which their interaction in an Individual, Organizational and Environmental level when going through a succession process was analyzed, identifying what facilitates and what limits this succession process. To analyze the structure and main characteristics of the succession process, these were taken as the base according to the following guidelines: Individual dimension leadership roles, leadership abilities and leadership styles; Organizational dimension structure, planning, counseling, communication (transparency), control and evaluation; Environmental dimension stakeholders influence (community, suppliers, clients, partners) in the succession process. The end results suggest that the main characteristics of those productive associations analyzed here is the intertwining of political, economic and social dimensions. However, one of the major challenges for these associations is to create positive outcomes in a socio-economic and political environment where community resources are scarce, rather than sufficient to build and maintain a cooperative project. Regarding the succession process, the associations leaders showed an understanding about the importance of planning and managing their progress, but still had tools with lack of structure to do so. The associations also attribute importance to the participative management in the succession process as a way of reconciling internal and external expectations, however they do not have a structured system to manage this process. The succession processes in the surveyed associations are in a constructive phase, whilst dealing with their day to day demands, but still show clear improving necessities for a more effective planning and participative management in the process.

Protéiforme, le tiers au marché public se rencontre essentiellement sous deux statuts, selon l’intérêt lésé direct ou indirect qu’il démontrera. Lésé par un manquement aux règles de passation, le tiers est effectivement protégé par le droit, qui instaure à son profit des voies de recours lui permettant tant de contester la légalité du marché que de demander des dommages et intérêts en réparation du préjudice subi. Citoyen, le tiers est défavorisé par sa condition, qui ne lui offre pas toujours les moyens d’actions de nature à satisfaire sa volonté de s’assurer de la légalité des procédures de passation. Il dispose pourtant, au sens du droit européen, d’un intérêt légitime à s’assurer qu’il existe de bonnes procédures de passation. Ce postulat conduira à proposer des solutions juridiques de nature à mettre le droit français en conformité avec ces nouvelles exigences. / The third party in the public procurement essentially meets under two statuses, according to the direct or indirect harmed interest which it will demonstrate. Hurt by a breach on procurement procedure, the third party is actually protected by the law, which establishes in its profit legal remedies allowing him to dispute the legality of the contact as well as asking for compensation for the loss suffered. Citizen, the third party is disadvantaged by its status, which does not always offer him actions likely to satisfy his will to make sure of the legality of the procurement procedure. He has nevertheless, under European law, a legitimate concern to make sure that there are good procurement procedures. That premise will lead to propose legal solutions likely to put the french law in accordance with these new requirements.
