L’un des accidents de dimensionnement d’un réacteur à eau pressurisée est l’Accident de Perte de Réfrigérant Primaire (APRP). L’évènement initiateur d’un tel accident est une brèche sur le circuit primaire du réacteur entrainant une perte d’inventaire en eau, et de ce fait conduit à un assèchement des assemblages combustibles. En conséquence, une augmentation considérable de la température surviendrait à l’intérieur du cœur du réacteur. Ainsi, les gaines de combustible peuvent éventuellement se déformer et des zones dites ballonnées apparaitre. Ces zones vont avoir un fort impact sur l’efficacité du refroidissement du cœur du réacteur. Pour contribuer à l’étude thermohydraulique d’un APRP, la présente thèse a pour but la caractérisation expérimentale des interactions entre un écoulement diphasique de vapeur/gouttes et une zone partiellement bouchée. Afin de reproduire un tel scénario, le banc expérimental thermohydraulique COLIBRI a été conçu. Plusieurs configurations géométriques de la zone ballonnée, caractéristiques d’un APRP, sont analysées (longueur et taux de bouchage associés au ballonnement). Afin de caractériser les échanges thermiques paroi/fluide ainsi que la dynamique des gouttes, des diagnostics optiques et thermiques sont utilisés : l’Anémométrie Phase Doppler (PDA) pour mesurer le diamètre et la vitesse des gouttes, la Fluorescence Induite par Laser (LIF) pour mesurer la température des gouttes et la Thermographie Infrarouge (IR) afin d’estimer le flux de chaleur extrait du tube par l’écoulement. En parallèle, une modélisation du problème a été développée afin d’obtenir une approche théorique de la capacité de refroidissement de l’écoulement diphasique. Le système d’équations décrivant la conservation de la masse, de la quantité de mouvement et de l’énergie permettra d’estimer l’impact respectif des différents mécanismes de transferts thermiques mis en jeu ainsi que l’évolution spatio-temporelle des paramètres thermohydrauliques / During a Loss of Coolant Accident (LOCA) in a Pressurized Water Reactor (PWR), caused by a break or a leakage on the primary circuit, partial or even complete drying of the fuel assemblies may occur. In these conditions, the fuel temperature increases, leading to a significant deformation and rupture of the fuel rod cladding. The cooling flow might be impaired, according to the size and distribution of the deformed zones within the fuel assemblies during the emergency cooling phase (Reflooding phase). To contribute to the thermalhydraulic study of the reflooding phase, this study aims to characterize experimentally the coolability of a representative deformed sub-channel by a steam-droplets flow under LOCA conditions. In order to reproduce such a scenario, the experimental thermal-hydraulic set-up COLIBRI was designed. Several geometrical blockage configurations are analyzed (Blockage ratios and axial lengths). Three measurement techniques are set up to follow the cooling transient phase of each experience: Phase Doppler Anemometry (PDA) in order to obtain both velocity and diameter of droplets, Laser Induced Fluorescence (LIF) to measure the mean droplet temperature and Infrared thermography to estimate the heat flux removed by the two-phase flow. Additionally, a one-dimensional mechanistic model, taking into account of the heat transfers mechanisms in the post-dry out region, is developed in order to analyze the experimental data and identify each one of the wall-to-fluid heat transfers (radiation with vapor and droplets, convection with vapor, evaporation, and droplet impact)

Un évènement Natech est un accident TECHnologique provoqué par l’impact d’un aléa NAturel sur une installation industrielle. Cet accident peut porter atteinte à l’extérieur de l’emprise du site, aux personnes, aux biens ou à l’environnement. L’objectif de ce travail est de mettre au point une méthodologie pour l’analyse et la prévention du risque d’accidents technologiques induits par l’inondation (Natech inondation) d’un site industriel. La méthodologie repose, dans un premier temps, sur une étude des Retours d’Expériences. Il a été identifié que les accidents Natech sont imputables à un rejet de matières dangereuses sur des équipements dits critiques endommagés directement ou indirectement par les eaux d’inondation. Les rejets de matières dangereuses vont amener à trois types d’accidents (incendie, pollution, explosion). Dans un deuxième temps, la méthode MADS-MOSAR a été adaptée à l’application aux accidents Natech inondation. L’analyse a abouti à cinq scénarios génériques d’accident Natech inondation sous forme de cinq nœuds papillons. Une soixantaine de Mesures de Maîtrise de Risque de prévention et de protection ont été identifiées. La validation de la complétude et de la plausibilité des nœuds papillons génériques est effectuée, d’une part, à l’échelle du secteur d’activité de traitement de surface et, d’autre part, à l’échelle spécifique d’établissements industriels. Ce travail a abouti à un outil de diagnostic de la vulnérabilité d’une installation industrielle aux accidents Natech inondation, accompagné d’un catalogue de MMR. Enfin, ce diagnostic a été appliqué sur un site industriel afin d’évaluer son mode opératoire et de l’enrichir par l’expérimentation. / A Natech accident is the impact of a NAtural event on an industrial plant (such as an ICPE- Classified Installation for the Environmental Protection). The impact can trigger a TECHnological accident, damaging the vicinity of the industrial facility (inhabitants, their properties and the environment). The final objective of this work is to develop a risk analysis methodology for the prevention of technological accidents triggered by flooding of industrial facilities. Therefore, the work begins with an analysis of past technological accidents triggered by floods. Thus, it was identified that Natech accidents are the consequences of hazardous materials releases from critical equipment, which in turn are damaged directly and indirectly by flood waters. Depending on the type of hazardous substances released, three categories of technological accidents are observed (fire, pollution and explosion). Afterwards, a risk analysis methodology was developed. It is mainly based on the MADS-MOSAR method, which was modified in order to be adapted to the Natech issue. The analysis resulted in the identification of five generic scenarios of technological accidents triggered by floods that can occur within an industrial installation. These scenarios were represented using the bow-tie tool. Then, around sixty preventive and protective measures were identified. The validation of the elaborated scenarios was done, on one hand, in the surface treatment industrial sector and, on the other hand, in two specific industrial facilities. The validation phase helped to test the completeness and plausibility of the generic scenarios, as well as the relevance of the measures previously identified. Furthermore, the validated scenarios were used to develop a checklist helping operators to decrease the vulnerability of their industrial facilities to technological accidents triggered by floods, along with a catalog of preventive and protective measures. Finally, the tool was applied to an industrial installation in order to enhance it and test its relevance by operators.

This research assessed the criminological significance of motor vehicle crash data within the criminal justice context. The study was conducted at the southern region Accident Bureau of the Ekurhuleni Metropolitan Police Department (EMPD).
The objectives of this research are to:
a. Explore and describe the extent and criminological impact of road traffic crashes on a macro level.
b. Explore and describe the impact of road traffic crashes on a micro level.
c. Explain the criminological significance of road traffic crash data in relation to effective road safety management.
d. Evaluate and describe the attendance and recording procedure of road crashes.
e. Evaluate and describe the supervisor’s responsibility in relation to the quality of crash data recorded.
f. Evaluate and describe the effectiveness of crash statistics accessed by management and/or officials.
g. Evaluate and explain the current prosecution procedure as part of the CJS.
h. Evaluate existing policy about the crash-recording process applicable to operational law enforcement practitioners of the EMPD.
i. Explore and describe the current state of motor vehicle insurance and its necessity from a criminological perspective.
The researcher followed a mixed-methods approach, collecting quantitative data from recorded Accident Report Forms and qualitative data from interviews with participants active in their fields of expertise. Road crashes are characterised by injury and damage; victims suffer trauma and economic consequences, and the society and the economy as a whole are affected. The research suggests that accurate road crash data form a crucial component in the investigation, prosecution and development of road safety strategies in the quest towards improved road safety efforts. It also indicates that existing crash data are of such a sub-standard that traffic professionals

construction, for the year 2011, for the States of the Northeast Region. After a quick exposure
about the historical aspects and Brazilian legislation on the work accident, was estimated a model
from the RAIS data, published by the Ministry of Labor and Employment. Studying the work
accident, in numbers, it was found that the variables sex, age, salary, experience, educational
level, race and sector of activity have direct relation with the work accident, being the subject of
so much importance, as a result of mishaps and, also, the short spent by State.

No. of bitstreams: 0 / Após os acidentes nucleares ocorridos no mundo, critérios e requisitos extremamente rígidos para a operação das instalações nucleares foram determinados pelos órgãos internacionais que regulam essas instalações. A partir da ocorrência destes eventos, as operadoras de plantas nucleares necessitam simular alguns acidentes e transientes, por meio de programas computacionais específicos, para obter a licença de operação de uma planta nuclear. Com base neste cenário, algumas ferramentas computacionais sofisticadas têm sido utilizadas como o Reactor Excursion and Leak Analysis Program (RELAP5), que é o código mais utilizado para a análise de acidentes e transientes termo-hidráulicos em reatores nucleares no Brasil e no mundo. Uma das maiores dificuldades na simulação usando o código RELAP5 é a quantidade de informações geométricas da planta necessárias para a análise de acidentes e transientes termo-hidráulicos. Para a preparação de seus dados de entrada é necessário um grande número de operações matemáticas para calcular a geometria dos componentes. Assim, a fim de realizar estes cálculos e preparar dados de entrada para o RELAP5, um pré-processador matemático amigável foi desenvolvido, neste trabalho. O Visual Basic for Applications (VBA), combinado com o Microsoft Excel, foi utilizado e demonstrou ser um instrumento eficiente para executar uma série de tarefas no desenvolvimento desse pré-processador. A fim de atender as necessidades dos usuários do RELAP5, foi desenvolvido o Programa de Cálculo do RELAP5 PCRELAP5 onde foram codificados todos os componentes que constituem o código, neste caso, todos os cartões de entrada inclusive os opcionais de cada um deles foram programados. Adicionalmente, uma versão em inglês foi criada para PCRELAP5. Também um design amigável do PCRELAP5 foi desenvolvido com a finalidade de minimizar o tempo de preparação dos dados de entrada e diminuir os erros cometidos pelos usuários do código RELAP5. Caractérisation du comportement à rupture des alliages de zirconium de la gaine du crayon combustible des centrales nucléaires dans la phase post-trempe d'un APRP (Accident de Perte de Réfrigérant Primaire) / Characterization of fracture behavior of zirconium alloys for fuel rod cladding of nuclear power plant in the post-quench stage of a LOCA (Loss of Coolant Accident)He, Mi 19 November 2012 (has links)
Dans le cadre des études visant à garantir l'intégrité de la gaine du crayon combustible, EDF est amené à caractériser la ductilité de la gaine après un Accident de Perte de Réfrigérant Primaire (APRP). La thèse porte sur la caractérisation du comportement à rupture des gaines en Zircaloy-4 détendu pour lesquels les conditions d'APRP ont été simulées en laboratoire par une oxydation à haute température suivie d'un refroidissement. L'oxydation est effectuée à 1100°C et à 1200°C pour différentes durées ce qui conduit à des niveaux d'oxydation de 3% à 30% d'ECR (Equivalent Cladding Reacted). Deux types de refroidissement sont mis en oeuvre : la trempe à l'eau et le refroidissement à l'air. Les gaines oxydées comportent deux couches fragiles, la couche de zircone externe ZrO2 et la couche α(O), et une couche présentant une ductilité résiduelle, la couche ex-β.Les gaines oxydées ont fait l'objet de caractérisations en microscope optique, par analyse à la microsonde et par nano-indentation. Une corrélation entre la teneur en oxygène et la nano-dureté et le module d'Young a été proposée.L'essai Expansion due à la Compression (EDC) a été développé avec une instrumentation par stéréo-corrélation d'images puis a été utilisé pour caractériser le comportement mécanique des gaines oxydées. Le comportement des gaines oxydées a été étudié à partir de l'analyse des courbes macroscopiques de l'essai EDC et à partir des observations des échantillons rompus ou pré-déformés.Un scénario de rupture des gaines oxydées a été proposé. Ce scénario a été validé d'une part par la réalisation d'essais sur gaines sablées ne comportant que la couche ex-β et d'autre part par la modélisation de l'essai par la méthode des éléments finis. Un critère de rupture des gaines oxydées a par ailleurs été établi. La modélisation du comportement et le critère de rupture proposés ont été validés par la modélisation des essais de compression d'anneau. / In order to guarantee the integrity of nuclear fuel rod cladding, it is necessary for EDF to characterize the ductility of cladding after a Loss of Coolant Accident (LOCA). The thesis is about the characterization of the fracture behavior of cold-worked stress-relieved Zircaloy-4 claddings which have undergone LOCA conditions simulated in laboratory by a high temperature oxidation followed by a cooling. The high temperature oxidation is carried out at 1100°C and 1200°C with different times, which leads to different oxidation levels varying from 3% to 30% ECR (Equivalent Cladding Reacted). The high temperature oxidation is followed by two types of cooling: water quench and air cooling. The oxidized claddings contain two fragile layers - the outer zirconium oxide ZrO2 layer and the middle α(O) layer, and a layer which can have residual ductility - the inner ex-β layer.Characterizations by means of optical microscopy, electron probe micro analysis and nano-indentation have been carried out on the oxidized claddings. A correlation between the oxygen concentration and the nano-hardness and the Young's modulus has been proposed.The Expansion Due to Compression (EDC) test has been developed with an instrumentation of stereo digital image correlation, and then used to characterize the mechanical behavior of the oxidized claddings. The behavior of the oxidized claddings has been studied via macroscopic EDC test curves and observations of fractured or pre-deformed test samples.A fracture scenario of the oxidized claddings has been proposed. The fracture scenario has then been validated via EDC tests on oxidized claddings whose ZrO2 and α(O) layers have been removed, and via finite element modeling of EDC tests. Moreover, a fracture criterion has been established. The mechanical behavior modeling and the proposed fracture criterion have been validated by modeling of ring compression test.

Human - Technology - Organization (HTO) is a well-established, general unifying concept in the Swedish Accident Investigation Authority (SHK) that represents an approach, knowledge and use of various tools regarding interactions between people, technology and organizational factors. The HTO-perspective is well described in literature but there are few studies on how SHKs staff experiences working according to this method in their investigations. The aim of this study was therefore to describe their HTO-perspective, examine how it is used in the investigations at SHK and describe the investigators experience of working with the HTO-perspective as well as the method's usefulness compared to old methods in accident investigations. A literature study has been conducted in the areas of HTO, Theory of planned behavior (TPB) and safety culture. TPB and safety culture are described in this paper since they highlight the different aspects of a HTO-perspective. The hypothesis was answered by using semi-structured interviews. The interviews were analyzed by a content analysis and themes were identified. Furthermore the material from the interviews was subsequently structured through a Strength – Weakness – Opportunities – Threats analysis (SWOT), i.e. the informants' view on the HTO-perspective was structured based on the strengths, weaknesses, opportunities and threats SHKs staff experience that the method has in the investigative work. The informants consisted of two investigators at SHK who 3have worked with accident investigations for a long time in various roles. The results yielded that the investigators had a positive attitude towards conducting investigations according to the HTO-perspective because they feel that this perspective provides them with cross-competence. A flaw is though that the perspective lacks a structured approach. As a result of this, the investigators own experiences and expertise play a major role in the quality of the investigation. As the study's aim is met and the informants are considered to be experts the validity requirements are also fulfilled.

The master’s thesis investigates the influence of a pilot‘s age and experience on the probability of aviation accident related to general aviation in the Czech Republic. The results were obtained using statistical analyses of data from aircraft accident final reports. The aim of the thesis is to propose the ways of decreasing the number of accidents caused by the analyzed factors while taking into account the current aviation legislation.
