Postoperativ smärta och heshet i halsen; en jämförelse mellan Macintosh direktlaryngoskopi och McGrath videolaryngoskopi : En pilotstudie / Postoperative sore throat and hoarseness; a comparison between Macintosh direct laryngoscopy and McGrath videolaryngoscopy : A pilot study

Bakgrund Endotrakeal intubation används i många olika situationer där det finns ett behov av att skapa en säker luftväg. Den manipulation av luftvägarna som krävs i samband med en intubation kan bland annat orsaka postoperativ halssmärta och heshet, vilket kan ha negativa konsekvenser för patienten. För att försöka undvika komplikationer vid intubering har det utvecklats olika intubationshjälpmedel, där ett av hjälpmedlen är videolaryngoskop, som minskar den kraft som behövs för att skapa en fri insyn i larynx. Syfte Syftet med studien var att jämföra Macintosh direktlaryngoskopi mot McGrath videolaryngoskopi, för att undersöka om någon av metoderna ger minskad förekomst av halssmärta och heshet postoperativt. Metod För att besvara syftet antogs en kvantitativ ansats. Studien utfördes sedan som en förberedande undersökning med en induktiv metod. Datainsamlingen utfördes på en operationsavdelning i Västra Götaland, totalt deltog 24 patienter, varav 12 patienter intuberades med direktlaryngoskopi och 12 patienter intuberades med videolaryngoskopi. Deltagarna fick postoperativt svara på validerade enkätfrågor, och gradera eventuell förekomst av halssmärta och heshet enligt validerade fyrgradiga skalor. Resultat Resultatet visade att det eventuellt kan finnas en viss skillnad gällande postoperativ halssmärta beroende på om man intuberas med Macintosh direktlaryngoskopi eller McGrath videolaryngoskopi. Presenterat i absoluta och relativa mått visar det på att endast 1 deltagare (8%) som videolaryngoskoperades graderade lindrig halssmärta postoperativt, medan det var 3 deltagare (25%) som direktlaryngoskoperades som graderade lindrig halssmärta. Dock var skillnaden inte signifikant (p=0,56). Skillnaderna gällande heshet var större, där 9 deltagare (75%) som videolaryngoskoperades graderade sig helt besvärsfria postoperativt och 3 (25%) graderade en lindrig heshet. Ingen av deltagarna som direktlaryngoskoperades graderade sig som besvärsfri postoperativt, 9 (75%) graderade lindrig heshet och 3 (25%) graderade måttlig heshet. Skillnaden mellan grupperna var signifikant (p=0,001). Slutsats Skillnad verkar föreligga gällande framför allt heshet beroende på vilken intubationsmetod som används. Det kan även finnas en eventuell skillnad gällande halssmärta, dock är denna ej signifikant i pilotstudiens resultat. Studiepopulationen är begränsad i sitt antal, och det finns få studier att jämföra resultatet med. Det finns ett behov av fler och större studier för att finna evidens för den bästa metoden. / Background Endotracheal intubation is used in many different situations where there is a need to create a safe airway. The airway manipulation force that is required when an endotracheal intubation is needed may cause postoperative sore throat and hoarseness. This can adversely affect the patient. To avoid complications of an endotracheal intubation, different intubation aids have been developed, one of which is a videolaryngoscope, which reduces the force needed to create a free laryngeal insight. Aim The purpose of the study was to compare Macintosh direct laryngoscopy to McGrath videolaryngoscopy, to investigate whether either method reduces the incidence of sore throat and hoarseness postoperatively. Method To answer the purpose, a quantitative approach was adopted. The study was then conducted as a preliminary study with an inductive method. The data collection was carried out in an operation theatre in Västra Götaland, Sweden. A total of 24 patients participated, where 12 patients were intubated with direct laryngoscopy and 12 patients with videolaryngoscopy. Postoperatively the participants responded to validated questionnaires, and evaluated any occurrence of sore throat and hoarseness according to validated four-dimensional scales. Result There was a difference in postoperative sore throat depending on intubation with Macintosh direct laryngoscopy or McGrath videolaryngoscopy. Presented in absolute and relative terms, the result show that only 1 participant (8%) that was intubated with videolaryngoscopy graded mild sore throat postoperatively, while there 3 participants (25%) who were intubated with the use of direct laryngoscopy graded mild sore throat. However, the difference was not significant (p=0.56). The differences in hoarseness were greater, where 9 participants (75%) that was intubated with the use of videolaryngoscopy graduated no hoarseness post-operatively and 3 (25%) graded a mild hoarseness. None of the participants who were intubated with the use of direct laryngoscopy graded no hoarseness, 9 (75%) participants graded mild hoarseness and 3 (25%) graded moderate hoarseness. The difference between the groups was significant (p=0.001). Conclusions A difference exists in advance of hoarseness depending on the intubation method that is used. There may also be a difference regarding sore throat, but this result remained non-significant. However, in this pilot study the population was limited in its number, and there are few other studies in this field available for comparison. Therefore, more, and larger studies are needed to find evidence for the best intubation method.

Modeling capillarity and two-phase flow in granular media : from pore-scale to network scale / Modélisation de la capillarité et des écoulements biphasiques en milieux granulaires : de l'échelle des pores à l'échelle du réseau

Les simulations numériques à l'échelle du pore sont fréquemment utilisées pour étudier le comportement des écoulements multiphasiques largement rencontrées dans phénomènes naturels et applications industrielles. Dans ce travail, la morphologie de structures liquides et l'action capillaire sont examinées à l'échelle des pores par la méthode de Boltzmann sur réseau (LBM) à plusieurs composants selon le modèle de Shan-Chen. Les résultats numériques obtenus sont en bon accord avec les solutions théoriques. Les simulations numériques sont étendues à microstructures complexes au-delà du régime pendulaire.La LBM a été utilisée pour modéliser l'écoulement multiphasique à travers un milieu poreux idéalisé dans des conditions de drainage primaire quasi-statique. Les simulations LBM ont fourni une excellente description du déplacement de l'interface fluide-fluide à travers les grains. Pendant le drainage, les simulations LBM sont capables de reproduire la déconnexion d'une phase dans le milieu granulaire sous la forme de ponts pendulaires ou structures liquides complexes. Malheureusement, le temps de calcul nécessaire pour ce type de simulations est assez élevé. Afin d’optimiser les ressources de calcul, nous présentons un modèle 2D (modèle Throat-Network) basé sur des solutions analytiques pour décrire l'écoulement biphasique à travers un ensemble de disques dans un temps de calcul très réduit, donc le modèle 2D est susceptible de remplacer les simulations LBM lorsque les ressources de calcul sont limitées. L'approche souligne l'importance de simuler le problème a l'échelle de la gorge du pore pour obtenir les relations volume - pression capillaire locales. Le modèle Throat-Network est un point de départ pour le modèle hybride proposé pour résoudre les problèmes en 3D. Le modèle hybride combine l’efficacité de l’approche réseau de pores et la précision du LBM à l’échelle des pores. Le modèle hybride est basé sur la décomposition de l’échantillon en petits sous-domaines, dans lesquels des simulations LBM sont effectuées pour déterminer les propriétés hydrostatiques principales (pression capillaire d'entrée, courbe de drainage primaire et morphologie du liquide pour chaque gorge du pore). Malgré la réduction significative des temps de calcul obtenus avec le modèle hybride, le temps n’est pas négligeable et les modélisations numériques d'échantillons de grandes tailles ne sont pas réalistes. Les approximations données par les méthodes Incircle et MS-P, qui prédisent les propriétés hydrostatiques, sont comparées à celles de LBM et du modèle hybride. / Numerical simulations at the pore scale are a way to study the behavior of multiphase flows encountered in many natural processes and industrial applications. In this work, liquid morphology and capillary action are examined at the pore-scale by means of the multicomponent Shan-Chen lattice Boltzmann method (LBM). The accuracy of the numerical model is first contrasted with theoretical solutions. The numerical results are extended to complex microstructures beyond the pendular regime.The LBM has been employed to simulate multiphase flow through idealized granular porous media under quasi-static primary drainage conditions. LBM simulations provide an excellent description of the fluid-fluid interface displacement through the grains. Additionally, the receding phase trapped in the granular media in form of pendular bridges or liquid clusters is well captured. Unfortunately, such simulations require a significant computation time. A 2D model (Throat-Network model) based on analytical solutions is proposed to mimic the multiphase flow with very reduced computation cost, therefore, suitable to replace LBM simulations when the computation resources are limited. The approach emphasizes the importance of simulating at the throat scale rather than the pore body scale in order to obtain the local capillary pressure - liquid content relationships. The Throat-Network model is a starting point for the a hybrid model proposed to solve 3D problems. The hybrid model combines the efficiency of the pore-network approach and the accuracy of the LBM at the pore scale to optimize the computational resources. The hybrid model is based on the decomposition of the granular assembly into small subsets, in which LBM simulations are performed to determine the main hydrostatic properties (entry capillary pressure, capillary pressure - liquid content relationship and liquid morphology for each pore throat). Despite the reduction of computation time, it is still not negligible and not affordable for large granular packings. Approximations by the Incircle and the MS-P method, which predict hydrostatic properties, are contrasted with the results provided by LBM and the hybrid model. Relatively accurate predictions are given by the approximations.

Microbial Activity in Sediments: Effects on Soil Behavior

Microorganisms have played a critical role in geological processes and in the formation of soils throughout geological time. It is hypothesized that biological activity can also affect soil properties in short engineering time-scales. Bioactivity in sediments is determined by the classical limiting factors (i.e., nutrients, water, C for biomass, temperature and pH) as well as by pore-size geometrical limits and mechanical interactions between bacterial cells and soil particles. These constraints restrict the range of grain size and burial depth where biomediated geochemical processes can be expected in sediments, affect the interpretation of geological processes and the development of engineering solutions such as bioremediation. When biological, geometrical and mechanical limiting factors are satisfied, bioactivity can be designed to alter the mechanical properties of a soil mass, including lowering the bulk stiffness of the pore fluid through controlled gas bio-generation, increasing the shear stiffness of the soil skeleton by biomineralization, and reducing hydraulic conduction through biofilm formation and clogging. Each of these processes can be analyzed to capture the bio-chemo-hydro-mechanical coupling effects, in order to identify the governing equations that can be used for process design. Design must recognize the implications of spatial variability, reversibility and environmental impacts.

Experimental And Numerical Investigation Of Formation Damage Caused By Drilling Fluids

In this thesis, permeability impairment caused by drilling fluids and subsequent cleaning and permeability enhancement by back-flow were investigated by means of experimental and simulation studies. Permeability damage caused by three different drilling fluids was measured experimentally by core tests as a function of the filtration pressure and analyzed using a simulator describing the fines migration and retention in porous media. The pore throat plugging criteria for the three drilling fluids were determined. The particle concentration and the fraction of depositing particles were obtained simultaneously as a function of time and distance along the core length by numerical solution. Simulations were run both with experimental data in forward and backward directions along the core samples. Permeability damage ratio was correlated with respect to drilling filtration pressure specially for each type of the drilling fluids and type curves were constructed. Simulation results accurately match the experimental data, indicating that this simulator can be used for the estimation of permeability reduction, and the permeability and porosity variation along the core samples at various filtration pressures. X-Ray digital image subtraction was applied to different sections of the core plugs before and after the circulation to visualize the fines migration into porous media. The maximum damage ratio was obtained with the CMC added drilling fluid with 81 %. In the absence of CMC and Polymer-XT, the damage ratio was found as 72.8%. It was also determined that a polymer-added drilling fluid characterized with 63.8% permeability damage ratio is the optimum drilling fluid, causing less formation damage than the water-based bentonite mud.

Control strategies and inspection methods for welded part

Present and future demonstrator designs were used to demonstrate the quality assurance of welds. The NDT methods tested on prototype demonstrator parts are: visual inspection, radius gauges, throat size gauge, liquid-penetrant testing, magnetic particle testing and ultrasonics with pulse echo and phased array. The other methods like eddy current, time of flight diffraction, radiography, impression test, macro test and infrared thermographs are currently being analyzed along with their inspection costs.   The control plans for present and future designs with corresponding present and future NDT methods are suggested to minimize a shift in process. Magnetic particle testing revealed a lack of fusion and cracks for fillet welds, whereas ultrasonic pulse echo and phased array identified an internal lack of fusion, inner pores/slag inclusions on butt welds. Ultrasonic PAUT & TOFD could be used for accurate defect identification and thermography for online identification of lack of penetration, depth of penetration and weld parameters. / Weight reduction by improved weld quality (WIQ)

This thesis overviews my experience during graduate school making tangible,object oriented sculpture. I have been working formally to compose space in a way that develops a narrative between parts. The work is also a bridge between the fields of painting and sculpture, in terms of drawing with form and both painted and local, material color. My palette has mostly consisted of bronze casting, steel fabrication, fiberglass and epoxy resin, paint, the found object, woodworking, and mold making. This work is also conceptually based in showing the hand worked qualities of the materials, the transfer of meaning through casting, and my emotional relationship with the various parts of the sculptures. The three major themes of the work are: divine love and the complex of the apocalypse, the complexities and psychology concerning the relationship between a man and a woman, and the intrigue, potential energy, and beauty of the systems mankind hasinvented to harness the atom. The major artistic influences for this body of work have been: Jasper Johns, Marcel Duchamp, Constantine Brancusi, Alberto Giacommetti, Reg Butler, Henry Moore, Lynn Chadwick, Kenneth Armitage, Jeff Koons, Terry Winters, William DeKooning, Richard Diebenkorn, David Smith and Charles Long. I retain a strongrelationship with the movements of Dada, Surrealism, Futurism, and Assemblage, and amalso currently involved in solidifying the Manifesto of Raubeaux with a small group ofesteemed colleagues.

The present study focusses on the theatre audience, and on its’ role in the maintenance of the theatrical situation. Using video-recorded performances of relatively naturalistic, modern dramas, the study examines the behaviour of the audience in relation to the unfolding of stage events. Such behaviour is described through close inspection of the sounds the audience produces, consisting primarily of coughing, throat-clearing, and laughter. The study contributes to the growing body of research surrounding ethnomethodological conversation analysis (CA). CA methods are used to analyse not only an audience’s overt reactions to stage events, but also the actions occurring outside these relatively short-lived phenomena in the context of a theatre performance. It is demonstrated that members of the audience refrain from making « vocal noise » during the verbal interaction of actors, and some of the resources used to achieve this end are described. These include the interpretation of the emerging dialogue, of the relative positioning of actors and of the actors’ use of gesture. Members of the audience are observed making vocal noise around possible completions in the sequence of ongoing stage actions, a placing which seems to make it maximally unobtrusive. Furthermore, the audience’s laughter is described. It is argued that members of the audience negociate collective moments of laughter with each other and with the actors. In doing so, the audience displays a sensitive awareness of the other members of the audience and the performers on stage. It is finally suggested that vocal noise on one hand and laughter on the other are differently placed in relation to an emerging action. This relative placing seems to indicate their producers’ different orientations to these actions, according to which vocal noise is to be hidden and laughter to be taken as an overt reaction.

Taluppfattbarhet med strupmikrofon / Speech intelligibility with throat microphone

Contact microphones, especially throat microphones, have been developed to be used in environments with high background noises to improve the speech intelligibility in communication. They pick up vibrations from the surface they are attached to and are therefore less sensitive to sound and noise from the air. Comparison of the speech intelligibility with other types of microphones have previously been done by letting test persons examine the communication devices in question. This study examines the possibility to make use of the STI-method instead and therefore make a comparison faster, more cost-efficient and customizable. The thought is that if the relationship between speech signals and vibrations were known, it could be used to transform the STI test signal to vibrations and then use the STImethod to estimate the speech intelligibility for the chosen contact microphone. This study, containing 22 men and women, evaluated the vibrations at the most suitable locations on the head for contact microphones and compared it with the speech signal of the same test person. Frequency responses were calculated for all locations of the head and a more detailed study showed that the frequency response of the neck may be approximated as a second order lowpass filter with a cut-off frequency of about 300 Hz that attenuates speech signals with higher frequencies. Experiments were also done to measure the STI value of a throat microphone with the known relationship. However, the results pointed out several problems that needs to be addressed before a STI method can be performed successfully. The results from this study may also be used to deeper study the relationships between different vibrations resulting from speech signals and suggestions on how the performance of contact microphones may be improved are given.
