No. of bitstreams: 0 Previous issue date: 2006 / Esta pesquisa teórico-prática investigou junto aos estudantes de graduação em medicina os motivos que os levam em direção ao trabalho voluntário na área da saúde e a repercussão desta atividade em sua vida pessoal, social e acadêmica. Para o desenvolvimento da pesquisa, foi feito um levantamento histórico sobre o tema voluntariado, através de uma literatura variada e fragmentada. A partir dos dados históricos, foi realizada uma revisão bibliográfica sobre o conceito motivação, sugerindo ser este tema complexo, multidisciplinar e pouco convergente. As informações levantadas com o histórico e os conceitos arrolados através da literatura foram referências na orientação desta pesquisa, envolvendo alunos participantes de programas de voluntariado na área de saúde da Faculdade de Ciências Médicas da São Casa e da Faculdade de Medicina da Universidade de São Paulo. A pesquisa foi desenvolvida em duas etapas: a primeira, uma entrevista com os coordenadores dos programas de voluntariado das duas instituições, com o objetivo de levantar mais informações sobre o assunto. E em uma segunda etapa foi aplicado um questionário para identificar o perfil do aluno voluntário, os motivos que os levam se engajar no programa de voluntariado e as repercussões do exercício de tais atividades em sua vida. / This theoretical-practical research investigates the reasons which lead undergraduate medical students towards pursuing volunteer work in Health Care programs and the repercussions of such activity on their personal, social and academic lives. Through careful examination of fragmented and varied pieces of literature on volunteer work, a historical study has been undertaken in order to prepare for this project. Based on historical data, the concept of motivation has been bibliographically revised, leading to the hypothesis that this is a complex and multidisciplinary theme with little convergence. The information gathered through historical and collected research was utilized as reference. Such research involved students participating in Health Care volunteer programs at the Santa Casa University of Medical Science and University of São Paulo’s Medical School. The research was two-fold: First, coordinators of the two institutions’ volunteer programs were interviewed in order to gather further information on the subject. And secondly, volunteer students were given a questionnaire, whose purpose was to identify their profile, their reasons for enrolling in the program and the repercussions of such activities in their lives. / BV UNIFESP: Teses e dissertações

Os principais resultados do mapa de conflitos são: dificuldade em conciliar condição ideal de qualificação profissional com ausência de trabalho remunerado, constrangimento em auferir reconhecimento profissional em trabalho voluntário, incompatibilidade entre exercício de projeto social e necessidades de consumo; ausência de satisfação e compensações e significados no trabalho remunerado, desgaste, constrangimento, inexplicáveis sentimentos de vazio. Entendemos que nenhuma mudança paradigmática se dá sem a mudança do self, mediador entre conhecimento e prática. A experiência de voluntariado afeta o self, ao favorecer a flexibilização de posicionamentos e possibilitar a construção de uma cultura dialógica, implicando uma reflexão ética no processo de desenvolvimento humano. Desenvolvimento é entendido neste estudo, segundo a perspectiva da psicologia sociocultural, como processos de transformações que ocorrem na interação eu-outro, marcadas pelas negociações de significados que se concretizam nessas interações no contexto cultural. O contexto do voluntariado é potencialmente favorável ao desenvolvimento de esquemas intencionais - estruturas que organizam nossos atos internamente - implicando ética, compreendida como a busca da vida boa para si e para o outro, próximo e distante no espaço e no tempo. A pesquisa empírica com sete voluntários de uma comunidade soropositivo enfoca os sentidos das vivências do fenômeno do voluntariado, apontando interseções com os significados de ética e de sustentabilidade. O objetivo geral foi construir um novo marco de conhecimento sobre este potencial humano – o voluntariado - situado na interface com a ética e a sustentabilidade. Para tal, teve como objetivos específicos: (a) analisar os sentidos, posicionamentos e mudanças de posicionamentos na prática voluntária; e (b) identificar esquemas intencionais possíveis de se constituírem no contexto do voluntariado e que são congruentes com uma sociedade tendencialmente sustentável. Quatro perguntas nortearam a pesquisa. (1) O que caracteriza as interações no contexto do voluntariado? (2) Essas características interatuam em outros contextos da vida do voluntário? (3) Que eventos colocaram os voluntários em situação de crise e em transição? (4) Que mudanças proporcionadas no contexto do voluntariado integram as possibilidades de sustentabilidade? Utilizamos a metodologia qualitativa, nos marcos da fenomenologia, adotando uma postura dialética histórico-materialista. Realizamos entrevistas semiestruturadas, individuais, com parâmetros da entrevista narrativa, as quais foram submetidas à análise dialógica dos significados e à análise pragmática do discurso. Os resultados indicam que o contexto de voluntariado favorece o estabelecimento de novas formas de sociabilidades, o desenvolvimento de um ser humano mais reflexivo no seu fazer social e profissional, com mais capacidade para propor e realizar intervenções adequadas em seu entorno sociocultural, com conhecimento e habilidade para o exercício da cidadania em contextos diversos, complexos e fluídos. Esquemas intencionais emergentes nesse contexto potencializam a perspicácia ética e a sustentabilidade. ______________________________________________________________________________ ABSTRACT / Potential paradigm changes in the context of volunteering point in two directions, along with others: (a) A solidary society, aimed at sustainability, understood as the possibility of reducing intra-generational inequality and, at the same time, of avoiding a social degradation that causes intergenerational inequality; and (b) the establishment of emancipatory relationships, understanding emancipation as empowering justice, and justice as the distribution of material goods, decision-making processes, social division of labor, culture and recognition. We understand that no paradigm shift takes place without the change of self, a mediator between knowledge and practice. The experience of volunteering affects the self, by favoring a flexibility of positioning, and enabling the construction of a dialogic culture, implying an ethical reflection upon the process of human development. Development is understood in this study, according to the perspective of socio-cultural psychology, as processes of transformation that occur in the interaction selfother, marked by negotiations of meanings which are realized in those interactions in the cultural context. The context of volunteerism is potentially favorable to the development of intentional schemas - structures that organize our acts internally - implying ethics, understood as the pursuit of good life for the self and the other near and far in space and time. The empirical research with seven volunteers from an HIV- positive community focuses on the sense of the experiences of the volunteerism phenomenon, demonstrating intersections with the meanings of ethics and sustainability. The overall goal was that of building a new milestone of knowledge on this human potential – volunteering - situated in the interface with ethics and sustainability. To do so, the study had as its specific objectives: (a) Analyze the senses, positioning and changes of standing in volunteering practice; and (b) identify intentional schemas possible of being constituted in the context of volunteerism and which are congruent with a society tending to be sustainable. Four questions guided the present research: (1) What characterizes the interactions in the context of volunteerism? (2) Do these characteristics interact in other contexts of a volunteer`s life? (3) Which events placed the volunteers in crisis and in transition? (4) Which changes provided in the context of volunteerism integrate the possibilities of sustainability? We use qualitative methodology in the framework of phenomenology, adopting a dialectic and historical-materialistic posture. We conducted semi-structured interviews with individual parameters of the narrative interview. The interviews were submitted to the dialogic analysis of meanings and to the pragmatic analysis of discourse. The results indicate that the context of volunteering promotes the establishment of new forms of sociability; the development of a human being who is more reflective of his/her social and professional doing, with more capacity to propose and implement appropriate interventions in his/her sociocultural surroundings, with knowledge and skill for the exercise of citizenship in various complex and fluid contexts. The intentional schemas which emerge in this context augment perceptivity in ethics and sustainability.

Assim, essa pesquisa propõe desvelar a fundamentação teórica para o entendimento da relação turismo e atividade voluntária na preservação do patrimônio natural, por meio do estudo de caso da Missão “Volunteer Vacations Onça-pintada”, realizada pela Agência Volunteer Vacations, em parceria com a ONG No Extinction – NEX, localizada no município de Corumbá de Goiás (GO). Por meio da pesquisa do tipo qualitativa, do método dialético histórico-materialista e das técnicas de triangulação de dados e análise de discurso, a pesquisa identificou a existência de uma pseudoconcreticidade embutida na prática da Missão “VV Onça-pintada”, avaliou a percepção dos turistas sobre a prática e aferiu a possibilidade de construção de uma relação Sociedade-Natureza e de uma Ética do Cuidado, a partir da conscientização do sujeito de ser/fazer parte integrante da natureza, senti-la como totalidade, consciente de que suas ações e reações interferem diretamente a si próprio e ao meio ao qual está inserido. _________________________________________________________________________________________________ ABSTRACT / The tourism, designed by the current capitalist system as a commodity to be consumed by tourists, to join volunteering is seen ideologically as a way to provide benefits through the trip, both for the communities visited and for the tourists themselves. When directed to the protection of natural heritage, is believed to extend these benefits to the social and natural environments involved. Thus, this research proposes to unveil the theoretical basis for understanding the relationship between tourism and voluntary activity in the preservation of natural heritage, through the mission case study "Volunteer Vacations Onça-pintada" held by the Agency Volunteer Vacations, in partnership with NGO No Extinction - NEX, in the Corumbá de Goiás (GO). Through the qualitative study, the historical materialist dialectical method and data triangulation techniques and discourse analysis, the study identified the existence of a built pseudoconcreticity in practice the Mission "VV Onça-pintada", assessed the perception of tourists on the practice and measured the possibility of building a society-nature relationship and an Ethics of Care, from the awareness of the subject to be / become an integral part of nature, feel it as a whole, aware that its actions and reactions interfere directly itself and the means to which it is inserted.

Compreendê-lo de modo abrangente, em seus diferentes níveis e estágios, é essencial para estimulá-lo e tornar a experiência mais significativa sob o ponto de vista pessoal, social e ambiental. Foram realizados três estudos, na perspectiva do Modelo do Processo Voluntário, que corroboraram a tese de que esta prática mescla comportamentos pró-sociais e pró-ambientais. O primeiro estudo, no nível Cultural e Social, constatou, de acordo com a avaliação de 65 homepages de instituições cadastradas no Conselho Nacional de Entidades Ambientalistas (CNEA), que promovem atividades voluntárias, sua concentração nas regiões Sudeste e Sul, o desenvolvimento de atividades nos âmbitos de Promoção e desenvolvimento social; Preservação, conservação, recuperação e manutenção do ambiente; Política, fiscalização e regulamentação; e a utilização de estratégias de captação de voluntários enfatizando predominantemente elementos de ordem ambiental e social e apelos emocionais direcionados à solução de suas necessidades, ao status do grupo e às ameaças ao ambiente. A segundo estudo, no nível Agência e Organização, identificou, em representantes de 33 instituições do CNEA e 12 do Instituto Chico Mendes de Conservação da Biodiversidade (ICMBio), que embora estas disponham de regras e normas para a gestão de voluntários, sua implementação é incipiente, sendo mais frequentes a realização de Encontros, Avaliação das realizações e o Treinamento de voluntários e menos privilegiadas as ações que envolvem a sua segurança e proteção, bem como o treinamento de profissionais que atuam junto a eles. Foram apontados, comumente, benefícios sociais e laborais decorrentes dos serviços voluntários e dificuldades de ordem financeira e de capacitação. Outros problemas foram comparativamente maiores entre as instituições do CNEA, tais como: recrutamento, disponibilidade dos voluntários e limitações legais. Para os participantes, o voluntário ideal deve ter disposição, bom desempenho, valores de justiça, altruísmo e sensibilidade ambiental e conhecimentos na área ambiental e da sua legislação. O terceiro estudo, nos níveis Individual e Interpessoal, identificou, em 34 voluntários do CNEA, 22 do ICMBio e 10 de outras instituições, graus elevados de motivação, satisfação pessoal e organizacional, integração institucional, qualidade no relacionamento entre voluntários e destes com profissionais, concordância com os princípios e objetivos institucionais, benefícios decorrentes da prática voluntária e intenção de permanência. Também reportaram escores elevados de preocupação ambiental de natureza Biosférica, seguida de Altruísticas e Egoísticas. Embora não se identifiquem diferenças significativas entre os grupos, a análise considerou a composição demográfica da amostra e elementos como crença em uma força superior, prática religiosa, tempo de experiência anterior, dedicação semanal e mensal ao voluntariado, sendo constatadas diferenças significativas (U de Mann-Whitney) para as variáveis sexo, ter filhos, não ter irmãos, crença e religião. Entre as conclusões, destacam-se a inclusão da dimensão política na análise social e cultural dos serviços voluntários, a importância da gestão e qualificação técnica do setor, o papel conscientizador e educativo do voluntariado e sua autodeterminação. Discutem-se as contribuições teóricas e metodológicas deste estudo, se apontam limitações e agenda para futuras pesquisas. ______________________________________________________________________________ ABSTRACT / Volunteering is reported as supplementary task force to public and private initiatives in the environmental sector and mobilizes thousands of people. Understanding it thoroughly on its different levels and stages is essential to stimulate it, and to make the experience more significant personally, socially and environmentally. Three researches were conducted, in the perspective of the Volunteer Process Model, which corroborated the thesis that this practice mixes pro-social and pro-environmental behaviors. The first research, in Cultural and Social levels, found, from the evaluation of 65 homepages of institutions registered at the National Council of Environmentalists Entities (CNEA), which promotes activities of this nature, its concentration in the Southeast and South regions and the progression of activities in the fields of Social promotion and development; Preservation, conservation , restoration and maintenance of the environment; Policy , supervision and regulation; and the use of strategies to recruit volunteers mostly by enhancing elements of environmental and social realm and by emotional appeals towards the solution of their needs, to group status and threatens against the environment. The second study, on the Agency and Organization level, showed, along with representatives of 33 institutions of the CNEA and 12 institutions of the Chico Mendes Institute for Biodiversity Conservation (ICMBio), which, although there were rules and regulations for the management of volunteers, the implementation is incipient, being more frequent the occurrence of meetings, the evaluation of the achievements and the training of volunteers and less privileged actions that involve their safety and protection, as well as the training of professionals who work with them. Social and labor benefits of volunteer services and financial and training difficulties were commonly pointed. Other problems were comparatively bigger among the institutions of the CNEA, such as: recruitment, volunteers’ availability and legal limitations. Concerning the participants, the ideal volunteer must have energy, good performance, values of fairness, altruism and environmental sensibility, and knowledge in the environmental field and its legislation. The third study, in the Individual and Interpersonal levels, showed, along with 34 volunteers from CNEA, 22 from ICMBio and 10 from other institutions, high degrees of motivation, personal and organizational satisfaction, institutional integration, quality in the relationship among volunteers and also among them and other professionals, the accordance with the institutional principles and objectives, benefits originated from voluntary practice and intention to stay. They also reported high scores on the preoccupation concerning the environment, the Biosphere, followed by Altruism and Egoism. Although significant differences among the groups aren’t shown, the analysis examined the demographic composition of the sample and elements such as belief in a higher power, religious practice, length of previous experience, weekly and monthly dedication to volunteering, showing significant differences (U of Mann - Whitney) to variables such as sex, having children, not having siblings, belief and religion. Among the conclusions, a few stand out: the inclusion of the political dimension in social and cultural analysis of volunteer services, the importance of the management and technical qualification in the field, the understanding and educational role of volunteering and their self-determination. The theoretical and methodological contributions of this research are discussed. Also, limitations and agenda for future researches are pointed out.

Buscou-se analisar tal prática na política de saúde e compreender quais as repercussões para a compreensão da saúde como um direito social, segundo o entendimento de voluntários, profissionais de saúde e a direção do hospital. A prática do voluntariado na saúde remete às Santas Casas de Misericórdia, resultado da união entre Estado e Igreja Católica, desde o século XVI. Desde então, o voluntariado foi se reconfigurando na sociedade brasileira, sendo estimulado por meio de programas governamentais em diversas áreas. No campo da saúde, o voluntariado se apresenta como um espaço de atendimento às necessidades materiais dos usuários através de diversas doações, além de ser uma forma de humanização das relações nos hospitais. A hipótese proposta pela pesquisa afirma que a prática do voluntariado não fortalece o direito à saúde pública. Isso significa que embora possa atuar no atendimento às demandas dos usuários, a referida ação não ocorre a partir de uma perspectiva de direitos, mas por meio de uma prática refilantropizada da assistência privada. A pesquisa, de caráter qualitativo, utilizou como instrumentos a análise documental, a observação participante e entrevistas semiestruturadas com dezenove participantes, sendo oito voluntários, dez profissionais de saúde e um representante da direção do hospital universitário. A partir da pesquisa, percebeu-se que há diversas compreensões sobre os objetivos e as possibilidades de atuação do voluntariado, encontrando-se discursos que elogiam e reforçam tal prática no cotidiano da instituição, bem como relatos que explicitam críticas e limitações a essa atividade. De modo geral, o voluntariado foi reconhecido, a partir dos dados da pesquisa, como um espaço importante para o atendimento das necessidades dos usuários e legitimado pela estrutura do hospital. Percebeu-se também que os serviços oferecidos pelos voluntários são utilizados pelos profissionais de saúde, o que fortalece tal prática, tendo em vista a ausência do Estado no oferecimento de determinadas políticas sociais e a precariedade com que estas são ofertadas. Assim, ora os profissionais se articulam com a rede pública de proteção social, ora tem-se a presença dos voluntários, o que evidencia uma disputa entre projetos políticos. O voluntariado, então, expressa uma complementaridade entre o público e o privado na prestação da assistência à saúde. A atuação do voluntariado relaciona-se, portanto, com questões presentes no atual contexto de precarização das políticas sociais e desresponsabilização do Estado, no qual se reforça a prestação privada de serviços públicos, por meio do mercado e da sociedade. A hipótese da pesquisa foi confirmada quando da ausência do Estado e em um contexto de disputa entre projetos políticos, tem-se o reforço do projeto privatista da saúde em detrimento da compreensão da saúde como um direito. A pesquisa identificou que a atuação do voluntariado visa responder às lacunas do próprio sistema de saúde e atender a demandas imediatas dos usuários dos serviços a partir de relações de doação e favor. ______________________________________________________________________________ ABSTRACT / This research aims to analyze the participation of the civil society in the public health politics through voluntary work, departing from a case study carried with the Associação de Voluntários do Hospital Universitário de Brasília (Voluntary Workers of University of Brasilia Hospital Association). We aimed to analyze this aspect in health politics and understand its repercussion for the understanding of health care as a social right, according to voluntary workers, health professionals and the hospital administration. The practice of voluntary work in health care harks back to the Santas Casas de Misericórdia, (hospitals run by the catholic church clergy), which resulted from the joined efforts of the State and the Catholic Church since the XVI century. From this time on, the voluntary work was reconfigurated in Brazilian society, being even stimulated by means of governmental programs in some areas. In the health care field, the voluntary work is a space for meeting the material needs of the patients of public hospitals through a variety of donations, besides being a way of humanization of relationships in a hospital. The hypothesis we proposed affirmed that the voluntary work did not enhance the right to public health. This means that in spite of the possibility of taking action in the care to the needs of the users of public hospitals, this does not occur in a perspective of social rights, but in a movement of ressurgence of the practice of philanthropy in the private health care. The research, which is qualitative, utilized as instruments the documental analyses, the participant observation and semi-structured interviews with nineteen participants, being eight voluntary workers, ten health care professionals e one representative of the hospital administration. Departing from the data, we realized that there is plenty of different understandings about the objectives and the possibilities of acting for voluntary work, being found speeches that praised the practice and support it in the daily life of a hospital and also speeches that highlight the limitations to it and voice critics to it. Mainly, the voluntary work was recognized, departing from the researched data, as in important space of meeting the needs of the users e legitimated by the hospital's structure. We realized also that the services offered by voluntary workers are utilized by health care professionals, which strengthens the practice due to the absence of the State in offering determinated public heath care politics and the precariety with which they are implemented. Thus, sometimes the health care professionals get articulated with the public network of welfare, and sometimes this occurs with the voluntary workers, what highlights a dispute among political projects. In this sense, the voluntary work shows a complementarity among public and private in providing health care. The action of the voluntary work is related, thus, with questions that are present in the contemporary context of precarization of welfare politics and non responsabilization of the State, through the private providing of public services, through market and civil society. The hypothesis of our research was confirmed once we could point out that in the absence of the State and in a context of rivalry between public projects, the reinforcement of the privatization of health care against the understanding of health care as a right occurs. Through the research we pointed out that the action of the voluntary work aims to meet the immediate demands of the users of public health care services from the perspective of donation and obliging.

O nebivolol, um fármaco com quatro centros quirais, está disponível na clínica como mistura racêmica dos isômeros d-nebivolol (SRRR) e l-nebivolol (RSSS). A atividade -adrenérgica do nebivolol reside no isômero d-nebivolol, enquanto o l-nebivolol promove a liberação de óxido nítrico das células endoteliais. O nebivolol é eliminado por metabolismo dependente do CYP2D6. O estudo avalia a farmacocinética e a relação farmacocinética-farmacodinâmica (PK-PD) dos isômeros do nebivolol em voluntários sadios. Foram investigados 15 voluntários sadios (10 homens e 5 mulheres) fenotipados com metoprolol como metabolizadores extensivos (EM, n=13) ou metabolizadores lentos para o CYP2D6 (PM, n=2). Os voluntários sadios foram tratados com dose única oral de 10 mg de nebivolol racêmico. As amostras seriadas de sangue foram coletadas até 48 h após a administração do fármaco. Os isômeros do nebivolol foram resolvidos na coluna Chirobiotic® V e analisados nas amostras de plasma empregando LC-MS/MS. Os parâmetros farmacocinéticos foram calculados por modelo bicompartimental com lag time, empregando o programa WinNonLin. A farmacodinâmica do nebivolol foi avaliada empregando como parâmetro a variação da frequência cardíaca entre os períodos final e anterior ao teste de esforço isométrico durante 2 min utilizando o handgrip a 30% da contratilidade voluntária máxima. A análise PK-PD relacionando o efeito na variação da frequência cardíaca induzida pelo exercício isométrico com as concentrações plasmáticas do isômero d-nebivolol foi avaliada empregando o modelo Emax sigmóide inibitório. A disposição cinética do nebivolol é enantiosseletiva nos voluntários sadios EM, com razões isoméricas de AUCl/ AUCd de 1,41. Os valores de concentração plasmática máxima (1,46 vs 0,79 ng/mL), área sob a curva concentração plasmática versus tempo (6,45 vs 3,99 ng.h/mL), clearance aparente (774,51 vs 1252,70 L/h) e volume de distribuição aparente (10936 vs 19082 L) mostram diferenças com significância estatística (Teste de Wilcoxon, p<0,05) entre os isômeros l-nebivolol e d-nebivolol para os voluntários sadios EM. A disposição cinética do nebivolol não é enantiosseletiva nos voluntários sadios PM investigados, com razões isoméricas de AUCl/AUCd de 0,93 e 0,98. Os valores de clearance aparente obtidos para os voluntários PM (87-350 vs 81-344 L/h, respectivamente para o l-nebivolol e d-nebivolol) são menores do que para os EM (775 vs 1253 L/h). O modelo Emax sigmóide inibitório descreveu a análise PK-PD relacionando o efeito na variação da frequência cardíaca induzida pelo exercício isométrico com as concentrações plasmáticas do isômero d-nebivolol em voluntários sadios EM com valores de Emax de 4,47 bpm (IC 95% 1,37-7,57) e de EC50 de 222,16 pg/mL (IC 95% 96,29-540,60 pg/mL). / Nebivolol is a drug with four chiral centers. It is administered in clinical practice as a racemic mixture of the isomers d-nebivolol (SRRR) and l-nebivolol (RSSS). The - blocking activity of nebivolol is attributed to d-nebivolol, whereas l-nebivolol promotes the release of nitric oxide from endothelial cells. Nebivolol is eliminated by metabolism dependent on CYP2D6. The present study evaluates the pharmacokinetic and pharmacokinetic-pharmacodynamic (PK-PD) of nebivolol isomers in healthy volunteers (10 men and 5 women) phenotyped with metoprolol as extensive metabolisers (EM, n=13) or poor metabolisers for CYP2D6. The healthy volunteers receveid a single oral dose of 10mg of racemic nebivolol. Serial blood samples were collected from 0 to 48 h after the administration of nebivolol. The isomers of nebivolol were analyzed by LC-MS-MS on a Chirobiotic® V column and the pharmacokinetic parameters (bicompartment model, micro, lag time, first order) were calculated by the software Winnonlin. The pharmacodynamic of nebivolol was evaluated using the variation of heart rate as parameter between the end and one minute before the handgrip exercise. Thus, the patients were oriented to conduct the isometric exercise with handgrip for 2 min at 30% of their maximum voluntary contractility. The PK-PD analysis relating the effect on the variation of heart rate induced by the isometric exercise and the plasma concentrations of the isomer d-nebivolol were evaluated using the Inhibitory effect sigmoid Emax model. The kinetic disposition of nebivolol is enantioselective on healthy volunteers EM, with isomeric ratios of AUCl/ AUCd of 0,93 e 0,98. The values of maximum plasma concentration (1,46 vs 0,79 ng/mL), area under the concentration time curve (6,45 vs 3,99 ng.h/mL), apparent clearance(774,51 vs 1252,70 L/h) and volume of distribution (10936 vs 19082 L) show statistically significant differences (p<0.05, Wilcoxon test) between the isomers l-nebivolol and d-nebivolol for the healthy volunteers EM. The kinetic disposition of nebivolol is not enantioselective on the healthy volunteers PM investigated, with isomeric ratios of AUCl/ AUCd of 1,07. The values of apparent clearance obtained for the volunteers pm (87-350 vs 81-344 L/h, respectively to l-nebivolol and d-nebivolol) are smaller than that for EM (775 vs 1253 L/h). The Inhibitory effect sigmoid Emax model described the PK-PD analysis described the effect on the variation of heart rate induced by handgrip isometric exercise with the plasma concentrations of the isomer d-nebivolol in healthy volunteers EM with Emax values of 4,47 bpm (IC 95% 1,37-7,57) and EC50 of 222,16 pg/mL (IC 95% 96,29-540,60 pg/mL).

A expansão do número de CTs e a diversidade de constituição da equipe e métodos utilizados levaram a SENAD, em parceria com FMB-UNESP, a organizarem um curso para trabalhadores de CTs. Objetivo: Identificar os fatores associados à adesão de profissionais, terapeutas, voluntários, gestores e líderes de Comunidades Terapêuticas a um curso de capacitação a distância. Métodos: Trata-se de um estudo longitudinal, de avaliação quantitativa de 7.572 alunos matriculados no curso que consentiram em participar desse estudo, entre dezembro de 2013 e julho de 2014. Foi utilizado um questionário específico para a obtenção dos dados. Resultados: Dos 7.572 alunos 2.963 (39,1%)abandonaram o curso.Quanto à adesão, neste estudo 4.609 (60,9%) dos alunos concluíram o mesmo.A taxa de concluintes entre os trabalhadores de CT foi de 64,9% (N= 2.384) contra 57,1% dos demais alunos (p<0,001). A posição profissional na CT influiu na adesão e certificação: 70,1% dos gestores foram certificados, 66,4% dos funcionários e 61,8% dos voluntários. No modelo final de regressão logística, o desfecho abandono se associou aos seguintes fatores: cor negra/parda (para todos os alunos); nunca ter utilizado a plataforma Moodle, não ter realizado nenhum curso de atualização em dependência química nos últimos dois anos e ser proveniente da região Centro Oeste (apenas para alunos que não eram de CT) e não ter realizado o curso de "Prevenção ao uso indevido de drogas: curso de capacitação para conselheiros municipais" (apenas para alunos que trabalhavam em CTs), ao passo que o desfecho... / Abstract: Introduction: It is estimated that in Brazil 80% of the care to people with alcohol, crack and other drugs problems take place in Therapeutic Communities (TC), hence the importance given to this type of facility by alcohol and other drugs policies drugs, especially the Program "Crack - we can win".The expansion of the number of TCs in Brazil and the diversity of its staff constitution and approaches led SENAD, in partnership with FMB-UNESP, to organize a training course addressed to managers, workers and volunteers connected with TCs. Objective: To identify factors associated with adherence to a free online course aimed at training professionals, therapists, volunteers, managers and leaders of TCs dealing with alcohol, crack and other drugs dependents. Methods: This is a longitudinal evaluation, quantitative study of 7,572 students enrolled in the training course who agreed to participate, between December 2014 and July 2015 Results: Out of the 7,572 enrolled subjects, 2,963 (39.1%) withdrew before the final exam and 4,609 (60.9%) did the final exam. The rate of completion among those working in a TC was 64.9%, as compared to 57.1% among other enrolled students (p<0.001). Professional status was significantly associated with completion rate: 70.1% of managers, 66.4% of other workers and 61.8% of volunteers completed the course. In the final logistic regression model, Outcome withdrawal was associated with the following factor: Black/Brown color (all subjects), never had used Moodle, not having had any continuing education course on substance use in the last two years and being from the Center West Region (only for subjects not connected with a TC) and not having completed a training course on "Substance use prevention - a course for county counsellors" (only for those working in TCs) whereas Outcome final exam was associated with age (36 years and over, for all enrolled students).
