Esta tese consiste no estudo da representação de personagens escritores da obra narrativa de Max Aub (Paris, 1903 Cidade do México, 1972). O corpus está constituído por Campo cerrado (1943), Campo abierto (1951), Campo de sangre (1945), Campo del moro (1963), Campo francés (1965), Campo de los almendros (1968) e Vida y obra de Luis Álvarez Petreña (1971). Propõe-se que os romances citados colocam em movimento um processo de corrosão da imagem de autoridade dos personagens escritores. Tal processo contamina também a figura de Aub, que encena, nos Campos e em Vida y obra..., um debate sobre a diluição de seu poder de narrar. O lócus de enunciação do autor, que escreve a partir do exílio, consciente da derrota dos republicanos na Guerra Civil Espanhola (1936-1939), incrusta-se na forma dos romances, fraturando a imagem de potência dos personagens escritores. / This thesis examines the representation of characters who are writers in Max Aubs narrative work (Paris, 1903 Mexico City, 1972). The corpus is composed of Campo cerrado (1943), Campo abierto (1951), Campo de sangre (1945), Campo del moro (1963), Campo francés (1965), Campo de los almendros (1968) and Vida y obra de Luis Álvarez Petreña (1971). The study proposes that the aforementioned novels put in motion a process of corrosion of these characters image of authority. This process also contaminates the figure of Aub, who performs in Campos and Vida y obra..., a debate about the dilution of his power to narrate. The lócus of the authors enunciation, who writes from exile, and is aware of the Republicans defeat in the Spanish Civil War (1936-1939), embeds in the form of those novels, and fractures the image of potency of the characters who are writers.

O presente trabalho aprofunda as determinações do Estatuto da Criança e do Adolescente no que diz respeito às medidas sócio-educativas, com foco naquela de privação de liberdade, chegando à aplicação destes elementos em seu campo de pesquisa - o referido município. / The purpose of this text is to elaborate systematization on the history of social politics to teenagers who committed a transgression, broaching the political and legal changes as well as the analysis developed on these social actors along the History. The purpose is also to understand the implementation of social politics turned to teenagers who committed a serious transgression in Juiz de Fora city, which aim, even late, is to promote and assure these teenagers’ rights. The starting point of this work is the theoretical analysis of the so-called “social question”, its origin, interpretations and accordance to the contemporary world. Being the practice of transgression by children and teenagers one of the facets of that phenomena, we come close to the discussion so as to broach the protection system to childhood and youth, its historic incidences and prevailing tendencies, the social lead and politics established by the State and by society, the alternatives, contradictions and deployments concerning management and implementation of this assistance networking. This piece of work deepens the resolutions found on Estatuto da Criança e do Adolescente concerning the social and educative measures, focused on that freedom privation, coming to the application of these elements to its survey field – the referred city.

Cette recherche exploratoire et qualitative porte un regard sur la réalité des personnes qui vivent dans la rue avec un chien comme animal de compagnie. Sachant que le milieu de la rue est un environnement stressant et rempli d’épreuves à surmonter, il est possible de croire que d’y vivre avec un animal de compagnie peut influencer le quotidien d’une personne. Bien que seulement 10 % des personnes en situation d’itinérance vivent avec un animal de compagnie (Irvine & al. 2012 ; Irvine.2013), il est important de cerner le sens qu’elles attribuent à leur relation et comprendre leur contexte situationnel afin de saisir leur réalité. Pour ce faire, cette recherche met en lumière les bénéfices et les contraintes qu’engendrent cette présence et l’influence de ces aspects relationnels sur le processus de désaffiliation sociale dans lequel la personne se situe lorsqu’elle vit dans la rue. / Abstract : This qualitative research explores the reality of street people who share their lives with a dog as a companion. Knowing that the street is a stressful environment filled with hardships, it is possible it is contemplated that living on the street with a pet can considerably influence someone’s life. Considering that it is about 10% of people that live on the street who are having a pet (Irvine 2013), it is important to identify the meaning they attribute to their relationship. To understand their situational context, this research highlights the benefits and constraints of this presence and the influence of these relational aspects on the social disaffiliation process of a person who live on the street with animal.

Survey on nail discoloration and association with CD4 count among untreated HIV patients at Apin Centre, Nigeria

Magister Public Health - MPH / Eligibility for antiretroviral therapy (ART) in HIV-infected patients is defined either by a cluster of differentiation antigen 4 (CD4) count of less than 200cells/mm3 or clinical diagnosis of WHO stage III and IV. Therefore, the decision to start ART becomes difficult when CD4 cell count is not available. With limited laboratory infrastructure, the decision to start ART is usually made based on clinical symptoms leading to late commencement of ART. This calls for alternative criteria to see if nail discoloration (ND) correlates with low CD4 count among untreated HIV infected patients. This will serve as a complementary screening tool for identifying asymptomatic ARV naive HIV patients with a CD4 cell count of less than 200cells/mm3 which signifies severe immunosuppression. Study Design and Setting: This was a quantitative cross-sectional descriptive and analytical study involving adult ART naïve HIV infected patients in WHO stage I and II. Systematic sampling was used to select the participants from all adult ART naïve HIV infected patients attending APIN clinic, located at the Jos University Teaching Hospital (JUTH), Jos, Nigeria. Data Collection: Face-to-face interviews, physical examination and relevant laboratory investigations with selected participants were conducted using a questionnaire guide. Questions on socio-demographic characteristics, clinical data, general physical examinations including finger nail examination and photographing with subsequent laboratory investigations including CD4 count and western blot were employed. Data Analysis: Variables were categorized and data analyzed using descriptive statistics including the frequency, percentage frequency; mean and standard deviation of continuous variables. Association between CD4 count of ≤200cells/mm3 and ND was tested using the chisquare test with an alpha level of 0.05. Prevalence of ND, sensitivity, specificity, positive predictive and negative predictive values and accuracy of the screening test of ND was calculated. Results: 394 patients had their fingernails photographed and assessed. It was shown that distal banded and grey nails were the common types of ND seen with a prevalence of 38%. There was an association between CD4 count ≤200cells/mm3 and ND (p<0.0001). CD4 count ≤200cells/mm3 was a risk factor for developing ND (RR=2.3[1.8-3.6]). The association has a sensitivity of 78%, specificity of 55%, positive predictive value of 50%, and negative predictive value of 80% and accuracy of test 63%. Conclusion: With a significant association (p<0.0001) and a sensitivity of 78%, ND can be a useful clinical indicator of immune dysfunction mediated by HIV among patients in WHO stage I or II. ND can either be a clinical sign or a symptom in HIV patients with a CD4 of ≤200cells/mm3 as seen in the study as the specificity and sensitivity of ND compared favourably with other WHO stage III diagnosis. Recommendations: Nail discoloration should complement CD4 count as an additional staging sign to help identify patients likely to benefit from ART especially in resource-limited settings. Finally, all patients with grey or distal banded should be on co-trimoxaxole prophylaxis in line with WHO /national guideline on the use of co-trimoxaxole for all HIV positive patients with a CD4 cell count of ≤350cells/mm3. / South Africa

Contextual Factors and the Syndemic of Alcohol Use and Risky Sexual Behaviors Among Men Who Have Sex with Men

Since the early 1990s with the AIDS pandemic, there has been an increasing interest on the importance of risky sexual behaviors, especially among men who have sex with men (MSM). An important antecedent for these behaviors is alcohol use. Studies consistently show an increased frequency of both alcohol use and risky sexual behaviors in MSM populations. However, to date, there has not been a precise estimate of the effect size in these diverse populations and a consistent way to measure it. More so, the importance of context is often cited as a source of variability, but is rarely measured in these studies. Contextual factors are different and specific for MSM, as they have been approached by two theories, both of which will be guiding this dissertation: Singer’s Syndemic Theory and Meyer’s Minority Stress Theory. Chapter 1, then presents a comprehensive review of both theories as they apply to alcohol use, risky sexual behaviors, and contextual factors driving them. Chapter 2 answers the first research question about effect sizes through a systematic literature review. The effect sizes or measures of association of these contextual risk and protective factors were summarized using meta-analytic techniques. Using five electronic databases, we identified 26 studies in 26 years (1990–2015), all diverse in terms of sampling techniques, assessment of sexual orientation, operationalization of alcohol use and risky sexual behaviors, contextual factors included, and measurement of effect sizes. Despite this diversity, studies reporting an effect size were pooled and summarized using both descriptive and meta-analytic techniques, as appropriate. Meta-analyses were conducted using Cochrane’s guidelines for generic inverse variance outcomes with random effects. The pooled effects of alcohol use on condomless anal intercourse (CAI; OR 1.73 [95% CI 1.43, 2.10], I2 0%), heavy episodic drinking on CAI (OR 1.88 [95% CI 1.25, 2.81], I2 32%), and heavy episodic drinking on condomless oral sex (OR: 8.00; 95% CI 2.48, 25.81), as well as the effects of substance use, mental health status, violence and victimization, and self-reported HIV status as contextual factors in the pathway between alcohol use and risky sexual behaviors were calculated, reported, and discussed along with study limitations and implications for public health. Chapter 3 answers the second research question regarding a standardized measurement model for effect sizes and the multiple mediation of contextual factors. We used the male subset of Wave IV of the AddHealth dataset to test our hypotheses through structural equation modeling approaches, including measurement analysis with invariance testing, path analysis for direct effects, and multiple mediation analysis through bootstrapping for indirect effects. The AUD scale was invariant between MSM and MSW, but the risky sexual behavior scale was not. For MSM, the standardized direct effect of AUD onto risky sexual behaviors was –1.25 and the standardized total indirect effect of the multiple mediation model was 1.58, 95% CI [1.42, 1.73]. Among the mediators, the strongest indirect effect for any measured or latent mediator was the mental health construct (2.09). We conclude that even though AUD has the same measurement structure for MSM and MSW, its effect on risky sexual behaviors does not operate the same way for these two populations, supporting both causal and contextual behavioral theories. Conclusions are individually discussed, respectively, in Chapters 2 and 3. However, Chapter 4 puts both manuscript conclusions in context and further discusses future implications for public health research, practice, and policy.

Sales assistants serving customers with traumatic brain injury

General lack of awareness regarding neurogenic communication disorders generally, and cognitive-communicative disorders following a traumatic brain injury (TBI) specifically has resulted in pervasive environmental and attitudinal barriers for these individuals. Paradigm shifts within the rehabilitation context have been highlighted which aim to remove barriers, provide social supports, and thereby enhance their participation in all aspects of life. While collaborative communication partner training programs have been advocated as a means to achieve this, a dearth of published programs is evident within the field of TBI, leading to the need for such programs to enhance the awareness and skill of the communication partner, and reduce barriers for the individual. Transformations are likewise apparent in the corporate context, where in spite of legislative changes encouraging diversity awareness programs for employees, few training programs exist worldwide, and in South Africa particularly, which remove barriers between employees and customers with a communication disability, and a TBI specifically. The current research targeted the retail supermarket environment as a context in which a significant number of everyday communicative exchanges take place. The study investigated the ability of a group of sales assistants to identify barriers to, and facilitators of interaction involving customers with a cognitive-communication disorder, using a control group design. This was achieved by the development and administration of 2 questionnaires on 2 different occasions to determine the confidence and skill with which they identified barriers and facilitators during videotaped sales interactions. A once-off training session was developed and conducted with the experimental group participants, in order to increase their confidence and skill in identifying barriers and facilitators of such interactions. The training session employed a number of customized components considered to be powerful contributors to the positive outcome of the study. These included: extensive use of customized video material professionally produced and comprising real interactions involving individuals with a TBI as “customers” in various stores of the participating national supermarket chain during operating hours. Collaboration with an individual with a TBI in the training, together with use of adult learning and diversity awareness principles were considered effective in shifting previous attitudes and fostering new learning. Inter-and-intra-group results on the confidence and skill constructs of the pre-and-post questionnaires were examined. All pointed consistently to the impact of the training session on the improvement demonstrated in the experimental group as compared to the control group on the post-questionnaires as compared with the pre-questionnaires. In addition all subjective training session evaluations by the experimental group participants were consistently highly rated, reflecting the active participation observed during training. The need for companies to expand their concept of customer service to include an acknowledgement of the customer with a disability is emphasized. Training programs empowering their employees to interact with greater awareness and confidence with customers with a TBI specifically will potentially facilitate deeper participation for both. Co-occurring Health Risks Among Middle Aged Hispanic Men Who Have Sex With Men (HMSM) in South Florida

In 2010, men who have sex with men (MSM) represented 4% of the population in the United States (US) and accounted for 78% of all new Human Immunodeficiency Virus (HIV) infections among Hispanic men. Hispanic men who have sex with men (HMSM) accounted for the third largest number of new HIV infections (6,700 cases). This dissertation explored the effects of age, loneliness, substance use, depression, and social support on high risk sexual behaviors that predispose middle aged HMSM to sexually transmitted infections (STIs) and HIV infection risk. A sample of 150 urban HMSM aged 40 to 65 were surveyed in this study. Singer’s Syndemics Theory (1996) provided this study’s theoretical framework. Data was analyzed using a variety of parametric and non-parametric statistics. Loneliness, social support, depressive symptoms, alcohol/drug use, and sexual risk behaviors were found to have an influence on HIV infection status in this study. Partner status, religious affiliation and age did have an influence on alcohol use in this study’s participants. Also, participants with increased age had increased depressive symptoms in this study. Lastly, depressive symptoms, substance use, social support, and loneliness did have an influence on sexual risk behaviors in this study’s participants, specifically alcohol use and illicit drug use. The findings from this study should be used to assess, diagnose, plan, implement and evaluate prevention strategies geared to reduce STI and HIV infection in this population. Future research should build on these findings and develop tailored risk reduction interventions addressing HMSM, with particular attention to the understudied age group of the middle aged HMSM.

This thesis deals with the Local Agenda 21 (MA21) and its impact to the municipality development. The Local Agenda 21 was derived from The Agenda 21, the main document of sustainable development agreed by UN in 1992. It is generally known that society development, population growth and growing peoples needs meet the limited quantities of the resources. Exactly this conflict between unlimited peoples wants and the limited amount of the resources has led to creating the Agenda 21 which was the reaction to unreasonable wastage of the resources and to environment deterioration. Local Agenda 21 is a blueprint paper for local authorities and its main aim is to achieve sustainable development in the particular area. Efficient usage of the resources may be seen as a proper solution. International project Healthy cities (WHO) operates with principles of sustainable development. The theoretical recourse may be found in the first part of the thesis. The practical implementation of MA21 principles (they are derived from principles of sustainable development) in Pelhřimov may be found in the following part. Pelhřimov joined the mentioned project Healthy cities in 2005. It means that these municipalities should respect sustainable development, environment and citizens opinions. Healthy cities also should preserve the environment for future generation.

I arbetet mot människohandel och prostitution understryks vikten av att minska efterfrågan på sexköp, det vill säga att få män sluta köpa sex. Med detta är det därför relevant att skapa en förståelse för hur man kan få sexköpare att ändra sitt beteende för att på så sätt stoppa den sexuella exploatering av kvinnor som sker genom sexhandel. Syftet med denna studie är att undersöka yrkesverksammas erfarenheter av sexköpare som söker hjälp respektive sexköpare som inte gör det, för att skapa en förståelse för vad som gör att sexköpare ändrar sitt beteende. Studien bygger på kvalitativa intervjuer med yrkesverksamma från hela Sverige, materialet har sedan analyserats med tematisk analys. I resultatet framgår det att det som upplevs vara betydande för en beteendeförändring hos män som köper sex är att skapa motivation samt medvetenhet hos männen. För att detta ska hända anses det vara betydelsefullt att komma åt sexköparnas kognitiva struktur som gör att de exempelvis rättfärdigar sitt beteende och där med fortsätter med beteendet. Vidare anses även att lära sig hantera sina känslor på ett hälsosamt sätt samt att det finns konsekvenser för sexköp vara betydande för att få män att sluta köpa sex. / In the fight against human trafficking and prostitution many emphasize the importance of working towards reducing the demand, in other words, decreasing the number of men who buy sex. Therefore, it can be considered important to create an understanding of how to get sex buyers to change their behavior and by doing so, stop the sexual exploitation of women through sex trade. The purpose of this study is to examine the experiences of professionals who meet sex buyers who get help changing their behavior and sex buyers who do not, in order to create an understanding what makes men who buy sex change their behavior according to professionals. The study is based on qualitative interviews with professionals from different parts of Sweden and has been analyzed using thematic analysis. The result confers the importance of creating motivation and awareness among sex buyers, in order to get them to change their behavior. To make this happen, it is considered important directing efforts to reconstruct the cognitive structure of the sex buyers, which for instance justifies their behavior and thereby counteract changing it. Furthermore, results from the interviews also indicated that it is important for the men to learn how to manage their emotions in a non-dysfunctional way, along with the existence of consequences for buying sex.
