Humanização e política de saúde: um estudo sobre os usos e sentido das propostas de humanização nas políticas de atenção à saúde da mulher

A humanização tem sido um termo frequentemente utilizado em relação às práticas de saúde no SUS, tornado-se uma bandeira de luta levantada sempre que se pensa em políticas de saúde. A polissemia do termo chama a atenção para a possibilidade de sua utilização em diferentes contextos, direcionados a diferentes auditórios. Esse trabalho tem pode objetivo analisar os usos e sentidos da noção de humanização nas propostas de ações voltadas à saúde da mulher. A história das práticas e saberes construídos em torno da saúde da mulher, com ênfase no movimento feminista, no planejamento familiar e na constituição do Programa de Atenção Integral à Saúde da Mulher foi resgatada no sentido contextualizar a discussão proposta. Diversas propostas de ações de saúde foram analisadas, assim como os projetos selecionados pelo Prêmio David Capistrano da Política Nacional de Humanização, visando a identificação de núcleos de sentidos no discurso oficial sobre humanização, e a forma como esse discurso está sendo percebido pelos profissionais. Foram identificados e discutidos quatro núcleos de sentidos: humanização enquanto atributo das relações interpessoais; humanização e redução da mortalidade materna; humanização e otimização de recursos; e humanização e processos de organização de trabalho. Concluímos que a manutenção do processo de formulação e implantação de ações de forma verticalizada, fragmentada, sem que as modificação propostas sejam pactuadas com os profissionais que deveriam implantá-las e os usuários que deveriam se beneficiar delas, acaba por promover a perpetuação do modelo assistencial vigente. / The term humanization is frequently employed to refer to health practices within the Brazilian Unified Health System (SUS), and this idea is defended whenever one thinks about public health policies. This polysemous word emphasizes the possibility of its being employed in different contexts, addressed to different publics. This work aims to analyze uses and senses of the notion of humanization in actions proposals concerned with womens health. The history of practices and knowledge on womens health, focusing on the feminist movement, on family planning and on the constitution of the Womens Integral Healthcare Program, has been recovered so as to contextualize this discussion. Several health actions proposals have been analyzed, as well as the projects selectes by the David Capistrano Prize for National Humanizations Politics to identify central meanings in the official discourse on humanization, and how such discourses is being apprehended by health workers. Four nucleus of senses have been identified and discussed: humanizations as attribute of inter personal relations; humanization and reduction of maternal mortality; humanizations and resources optimization; and humanization and labor organization processes. We came to the conclusion that the processes of formulation and implantation of actions in a vertical and fragmented way, without making a pact with the professionals supposed to implemented changes, nor with the users who would benefit from them, ands up in perpetuating the present healthcare model.

Coordenação de Aperfeiçoamento de Pessoal de Nível Superior / O presente estudo tem como objeto as representações mentais de mulheres produzidas pelas enfermeiras obstétricas na assistência ao parto. Os objetivos foram: Discutir as representações mentais das mulheres assistidas pelas enfermeiras sobre o parto e a prática obstétrica; Discutir o habitus da enfermeira obstétrica percebido pela mulher e Analisar as relações de poder simbólico entre os agentes envolvidos no processo de parturição. Trata-se de uma pesquisa qualitativa que utilizou a história oral temática e o teste de associação de palavras como técnicas de coleta de dados. Para a análise do material, utilizamos o método da análise de conteúdo de Bardin. A fim de dar sustentação teórica ao estudo, adotamos os conceitos de: campo, capitais, habitus, poder simbólico, trocas linguísticas, identidade e representações mentais, desenvolvidos por Pierre Bourdieu. Os resultados encontrados foram agrupados em duas categorias: As representações mentais das mulheres sobre o parto e a prática obstétrica: as percepções construídas e desconstruídas com o processo de parturição e O habitus da enfermeira obstétrica percebido pelas mulheres durante o processo de parturição: o poder simbólico destas agentes na construção de uma nova demanda social para o campo obstétrico. A primeira categoria apresentou as representações construídas pela socialização e as transformações das representações mentais das mulheres consequente à interação com a enfermeira no campo obstétrico. Neste sentido, as percepções das mulheres sobre o parto e a prática obstétrica confirmaram a forte influência do modelo tecnocrático nos depoimentos. Além disso, a prática humanizada da enfermeira contribuiu para a construção de uma nova visão de mundo nas mulheres pesquisadas que provocou um confronto entre suas representações mentais. A segunda categoria desvelou a identidade da enfermeira obstétrica percebida pelas mulheres através dos atributos profissionais e dos sinais distintivos que resultaram na associação de estereótipos a estas agentes. Ainda revelou que a manifestação do poder simbólico da enfermeira obstétrica foi percebido de diversas formas: pelo efeito de mobilização na mulher, em associação com as tecnologias de cuidado e através do fortalecimento da mulher para o parto. Concluímos que no contato com a mulher, a enfermeira exerceu um poder simbólico por estar em melhores posições no campo obstétrico. Tal fato, para as mulheres, resultou em transformações das suas representações mentais em relação ao processo de parturição, o que contribuiu para a construção de uma nova demanda para o campo obstétrico. Por outro lado, a enfermeira obstétrica, ao ser reconhecida pelas usuárias estudadas, fortaleceu a posição de sua prática no campo. / The object of this study is the womens mental representations produced by obstetric nurses in labor care. Aims: To discuss the mental representations of women assisted by nurses on labor and the obstetric practice; to discuss the habitus of the obstetric nurse perceived by the women, and to analyze the symbolic power relationships among the agents involved in the labor process. It is a qualitative study that used the thematic oral history and the word association test as data collection technique. As for the material analysis, we used Bardin's content analysis methodology; In order to provide theoretical support to the study, we adopted the following concepts: field, capitals, habitus, symbolic power, linguistic exchange, identity and mental representations developed by Pierre Bourdieu. The results found were grouped into two categories: The womens mental representations on labor and the obstetric practice: the constructed and deconstructed perceptions with the labor process and The obstetric nurses habitus perceived by women during the labor process: the symbolic power of these agents in building a new social demand for the obstetric area. The first category presented the representations constructed from the socialization and the transformations of the womens mental representations as a result of the interaction with the nurse in the obstetric area. In this sense, the womens perceptions on labor and the obstetric practice confirmed the strong influence of the technocratic model on the testimonies. In addition, the nurses humanized practice contributed to the construction of a new vision of the world among the women studied, which resulted in a crash in their mental representations. The second category unveiled the identity of the obstetric nurse as perceived by women through their professional attributes and the distinctive features that resulted in the association of stereotypes to these agents. It also revealed that the manifestation of symbolic power of the obstetric nurse was perceived under several forms: by the mobilization effect on women, by the association of the care technologies, and by the strengthening of women for labor. We concluded that the nurse, in the contact with women, has exercised a symbolic power by occupying a privileged position in the obstetric area. Such fact resulted, for the women, in transformations of their mental representations in relation to the labor process, which contributed to the construction of a new demand in the obstetric area. On the other hand, the recognition of the obstetric nurse by the users strengthened the position of her field practice.

Significando a dor no parto: expressão feminina da vivência do parto vaginal / Expressing the pain in labor: the female manifestation about vaginal delivery experience

Estudo qualitativo. Teve como objetivos Identificar os significados atribuídos à dor por mulheres que vivenciaram o parto vaginal, Analisar a vivência da dor segundo a percepção de mulheres que pariram por via vaginal e Discutir as estratégias vivenciadas pelas mulheres para aliviar a dor durante o trabalho de parto e parto vaginal. Os dados foram coletados no Alojamento Conjunto da maternidade da Unidade Integrada de Saúde Herculano Pinheiro com puérperas que passaram algum tempo dos seus trabalhos de parto na instituição. O instrumento para coleta de dados foi a entrevista semi-estruturada. As entrevistas foram gravadas, transcritas e os dados tratados com a técnica de Análise temática de Conteúdo. Para análise dos dados foi utilizado um referencial teórico pertinente aos diversos modelos assistenciais ao parto no Brasil. Vimos que as mulheres significam a dor no parto como sofrimento e/ou prazer e esse sofrimento tem a dimensão redentora influenciada pela Igreja, a dimensão heróica influenciada pela cultura e a dimensão inútil influenciada pela medicalização que retira os significados da dor. Elas significam a dor também com o prazer quando a relaciona com o bebê e com a saúde. As depoentes viveram a dor no parto sentindo uma sensação intensa e até violenta ou fisiológica que tem características próprias e são pertinentes ao processo de parturição. Algumas mulheres não focaram sua atenção na dor. Ela valorizaram mais as respostas psicoemocionais. As depoentes sentiram mais dor quando estavam dentro do hospital sofrendo as intervenções do seu modelo assistencial. Perante a dor no parto elas estrategicamente seguem os conselhos medicalizadas e comportam-se de forma civilizada. Outras mulheres utilizam estratégias instintivas para lidar com a dor no parto. Todas as estratégias utilizadas parecem ter por finalidade a finalização do processo parturitivo. / Qualitative study. Its object was to identity the meanings attributed to the pain by women who experienced vaginal childbearing, analyze the experience of the pain according to the perception of women who gave birth through vaginal delivery and discuss the strategies experienced by women to alleviate the pain during labor and delivery. The data was collected at the Unidade Integrada de Saúde Herculano Pinheiro with women in their immediate puerperium who had their childbearing labor at that institution. The data was collected through a semi-structured interview. The interviews were recorded, transcribed and the data treated through the technique of Thematic Content Analysis. For the data analysis, a theoretical standpoint was adopted pertaining to the various models of childbearing assistance in Brazil. We discovered that women see the pain in labor as suffering and/or pleasure and that suffering carries the redeeming aspect as influenced by the Church, the heroic aspect as influenced by culture and the useless aspect as influenced by the medicalization which removes all the significance of pain. They also see the pain as pleasure when they associate it to the child and health. The interviewed women experienced the pain in labor as an intense sensation, sometimes even violent or physiological, which bears its own characteristic relating to the delivery process. Some women did not focus on the pain. They valued the psycho-emtional response more. The interviewed women felt more pain while inside the hospital enduring the interventions of their assistential model. When facing the pain in labor, those women strategically follow the medicalized advice and behave in a civilized manner. Other women employ instinctive strategies to deal with the pain in labor. All the strategies used seem to aim to complete the delivery process.

Diversos estudos que abordam as repercussões das cirurgias ginecológicas na vida da mulher apontam para uma relação existente com aspectos subjetivos, em virtude da construção social da identidade feminina, indicando que a perda de órgãos ginecológicos pode interferir na maneira da mulher se perceber e se relacionar com o mundo. O presente estudo tem como objeto o processo de interação da mulher no contexto das cirurgias ginecológicas. Para isso foram formulados os objetivos: Descrever os significados da cirurgia ginecológica para a mulher submetida a este procedimento e Analisar o processo de interação da mulher com ela mesma e com o seu núcleo social a partir dos significados por ela atribuídos à cirurgia ginecológica, considerando o fenômeno da mutilação. O estudo de natureza qualitativa, do tipo descritivo-exploratório, teve como referencial teórico-metodológico o Interacionismo Simbólico e a Grounded Theory, que subsidiaram uma interpretação da ação e das relações das mulheres no contexto da cirurgia ginecológica e da mutilação, baseados na definição e interpretação de significados por elas atribuídos. Dos resultados emergiram quatro categorias: Investigando a doença e decidindo aceitar a cirurgia, A passagem pela cirurgia: vivendo o processo de resolução do problema, Enfrentando as mudanças impostas pela cirurgia e Valorizando a vida. Evidenciou-se que ao receber o diagnóstico da doença, a mulher interage com dúvidas, medos, a busca por informação e pelos benefícios da cirurgia, e por fim decide aceitá-la. Ao passar pela cirurgia, vivencia incômodos, complicações, compreende que perdeu uma parte do corpo e procura conformar-se, tentando crer que foi o único caminho. As diversas mudanças que ocorrem fazem-na construir novos significados e mudar a percepção de si e do seu meio social, a partir da interação consigo mesma e com este meio, fazendo-a refletir sobre a sexualidade, seus relacionamentos, sua saúde, a diferença no corpo e em si mesma, nas funções que desempenha e sobre a mutilação provocada pela cirurgia. O desenvolvimento de mecanismos de enfrentamento, como comparar sua cirurgia e histórias com outras, identificar aprendizados com a experiência, buscar formas de superação e resignar-se resultaram na valorização da vida e na vontade de viver melhor. Percebe-se a importância da enfermeira estabelecer um cuidado multidimensional, que identifique as necessidades que vão além do corpo biológico, respeitando as particularidades e a individualidade no momento do cuidado e contribuindo para o bem-estar físico, psíquico, social e espiritual das mulheres. / Several studies that approach the repercussions of gynecological surgery on womens lives indicate a relationship with subjective aspects, due to the social construction of the female identity, pointing out that the loss of gynecological organs may interfere in the way women perceive themselves and connects with the world. The object of this study is the process of interaction of women in the context of gynecological surgeries. To do so, the following objectives were devised: To describe the meanings of gynecological surgery for women undergoing such procedure and to analyze the process of interaction with women to themselves and with their social environment from the meanings attributed by her to gynecological surgery, considering the phenomenon of mutilation. This descriptive, exploratory study of qualitative nature had the Symbolic Interactionism and the Grounded Theory as theoretical-methodological references, thus supporting an interpretation of the action and relationships of women in the context of gynecological surgery, and mutilation, based on the definition and interpretation of meaning attributed by them. Four categories emerged from the results: Investigating the disease and deciding to accept the surgery; Undergoing the surgery: coping with the problem resolution process; Facing the changes posed by the surgery; and Appreciating life. It was evident that, when women are faced with the diagnosis, they interact with doubt, fear, search for information and the benefits of the surgery, and finally decides to accept it. After undergoing the surgery, they cope with discomfort, complications, they understand that they have lost a part of their bodies and seek resignation, trying to believe that it was the only way left. The several changes that occur make them build new meanings and change their own perception and the perception of their social environment, from the interaction with themselves and with that environment, reflection upon sexuality, their relationships, their health, the differences in their bodies and in themselves, in the role they play and in the mutilation provoked by the surgery. The development of confrontation mechanisms, such as comparing their surgeries and histories with others, identifying lessons learned with the experience, seeking ways of overcoming and resignation resulted in the appreciation of life and the desire to live better. The importance of the nurse in establishing a multidimensional care is noticeable. Such care must identify the needs that go beyond the biological body, respect the special characteristics and individuality at the moment of care and contribute to the womens physical, psychic, social and spiritual welfare.

Vivenciando a amamentação e sensações de prazer sexual e/ou excitação sexual ao amamentar: significados de mulheres / Experiencing breastfeeding and sexual pleasure and/or excitement on it: meanings to women

Coordenação de Aperfeiçoamento de Pessoal de Nível Superior / Trata-se de uma pesquisa com abordagem qualitativa e descritiva, com a utilização da Grounded Theory, na perspectiva do Interacionismo Simbólico. Como objetivos tivemos: identificar os significados atribuídos por mulheres ao fato de experimentar, sentir ou vivenciar sensações de prazer sexual e/ou excitação sexual durante a amamentação; analisar e interpretar, na perspectiva do Interacionismo Simbólico, a experiência de sentir prazer sexual e/ou excitação sexual durante a amamentação, a partir dos significados atribuídos pelas mulheres. O estudo procedeu-se no Município do Rio de Janeiro, em lugares de grande circulação de pessoas, como praças, ruas, shoppings, além da Unidade de Saúde Milton Fontes Magarão, localizada na zona norte do referido Município. Essa variedade de cenários nos possibilitou encontrar uma diversidade de participantes, referente a condições socioeconômicas, culturais, religião, ideais e concepções sobre o assunto estudado. A coleta e análise dos dados aconteceram no período de maio a julho de 2014, observando todas as exigências da Resolução 466/2012 do Conselho Nacional de Saúde. Foram entrevistadas 17 participantes e formados dois grupos amostrais e três categorias expressas por: significando a amamentação como ato sagrado, inocente e assexuado: a socialização da amamentação; vivenciando e significando sensações de prazer ao amamentar; ressignificando a vivência de sensações de excitação sexual ao amamentar. Como resultados evidenciou-se que a socialização da amamentação influencia a maioria das mulheres, na vivência de sensações de prazer ao amamentar. Essas sensações são descritas através da simbologia estabelecida socialmente, de que o ato de amamentar é sagrado, puro, livre de erotismo, prevalecendo um prazer maternal. Além disso, as sensações de excitação sexual ao amamentar, para as mulheres que se dizem nunca terem vivenciado, estão presentes no seu inconsciente, porém essa vivência é admitida para os outros e não para si mesmas. O significado de vivenciar prazer sexual ao amamentar é percebido como contraditório, pois ao mesmo tempo em que se identifica o poder do corpo feminino, prevalece o poder de nutrir sobre as percepções físicas de excitação sexual ao amamentar. Desse modo, identificaram-se estratégias utilizadas para bloquear ou para não vivenciar as sensações de excitação sexual ao amamentar. Com isso, este estudo nos mostra como as mulheres agem frente às sensações de prazer sexual e/ou excitação sexual ao amamentar, através da interação social do passado e do presente, gerando os significados que cada uma carrega consigo. Além disso, percebemos como a socialização da amamentação priva a vivência da sexualidade feminina, nos aspectos de sensações de prazer sexual durante a amamentação. / It is a research with qualitative and descriptive approach, using Grounded Theory, on the Symbolic interactionism. We aimed: to identify the meanings attributed by women on the fact of trying, feeling or experiencing sexual pleasure and/or excitement sensations during breastfeeding; to analyze and interpret them. The study was developed in Rio de Janeiro, in places of great circulation of people, like squares, streets, malls, as well as a Milton Fontes Magarão Primary Care Unit, located in the North of the city. This variety of sceneries enabled us to find a diversity of participants, referring to socioeconomic and cultural conditions, religion, ideals and conceptions about the subject studied. Data collect and analysis happened in the period of may to July 2014, observing all demands from the Resolution 466/2012 from the National Health Council. 17 participants were interviewed and two group samples were formed and three categories expressed by: breastfeeding meaning as a sacred, innocent and asexual act: breastfeeding socialization; experiencing and meaning pleasure sensations on breastfeeding; resignifying experience of sexual excitement sensations on breastfeeding. The results showed that the socializing influence of breastfeeding, most women, the experience of pleasurable sensations while breastfeeding. These sensations are described by symbols socially established that breastfeeding is sacred, pure eroticism free, prevailing a maternal pleasure. In addition, feelings of sexual arousal while breastfeeding, for women who say they have never experienced, are present in the unconscious of them, but this experience is admitted for others and not for themselves. The meaning of experiencing sexual pleasure while breastfeeding is perceived as contradictory, because at the same time that identifies the power of the female body, the prevailing power of nurturing on the physical perceptions of sexual arousal while breastfeeding. Thus, we identified strategies used to block or not to experience the sensations of sexual arousal while breastfeeding. Therefore, this study shows us how women act in the face of feelings of sexual pleasure and / or sexual arousal while breastfeeding, through social interaction of past and present, generating the meanings that each carries. In addition, we see how the socialization of breastfeeding deprives the experience of female sexuality in the aspects of sexual pleasure sensations during breastfeeding.

Neste sentido, as percepções das mulheres sobre o parto e a prática obstétrica confirmaram a forte influência do modelo tecnocrático nos depoimentos. Além disso, a prática humanizada da enfermeira contribuiu para a construção de uma nova visão de mundo nas mulheres pesquisadas que provocou um confronto entre suas representações mentais. A segunda categoria desvelou a identidade da enfermeira obstétrica percebida pelas mulheres através dos atributos profissionais e dos sinais distintivos que resultaram na associação de estereótipos a estas agentes. Ainda revelou que a manifestação do poder simbólico da enfermeira obstétrica foi percebido de diversas formas: pelo efeito de mobilização na mulher, em associação com as tecnologias de cuidado e através do fortalecimento da mulher para o parto. Concluímos que no contato com a mulher, a enfermeira exerceu um poder simbólico por estar em melhores posições no campo obstétrico. Tal fato, para as mulheres, resultou em transformações das suas representações mentais em relação ao processo de parturição, o que contribuiu para a construção de uma nova demanda para o campo obstétrico. Por outro lado, a enfermeira obstétrica, ao ser reconhecida pelas usuárias estudadas, fortaleceu a posição de sua prática no campo. / The object of this study is the womens mental representations produced by obstetric nurses in labor care. Aims: To discuss the mental representations of women assisted by nurses on labor and the obstetric practice; to discuss the habitus of the obstetric nurse perceived by the women, and to analyze the symbolic power relationships among the agents involved in the labor process. It is a qualitative study that used the thematic oral history and the word association test as data collection technique. As for the material analysis, we used Bardin's content analysis methodology; In order to provide theoretical support to the study, we adopted the following concepts: field, capitals, habitus, symbolic power, linguistic exchange, identity and mental representations developed by Pierre Bourdieu. The results found were grouped into two categories: The womens mental representations on labor and the obstetric practice: the constructed and deconstructed perceptions with the labor process and The obstetric nurses habitus perceived by women during the labor process: the symbolic power of these agents in building a new social demand for the obstetric area. The first category presented the representations constructed from the socialization and the transformations of the womens mental representations as a result of the interaction with the nurse in the obstetric area. In this sense, the womens perceptions on labor and the obstetric practice confirmed the strong influence of the technocratic model on the testimonies. In addition, the nurses humanized practice contributed to the construction of a new vision of the world among the women studied, which resulted in a crash in their mental representations. The second category unveiled the identity of the obstetric nurse as perceived by women through their professional attributes and the distinctive features that resulted in the association of stereotypes to these agents. It also revealed that the manifestation of symbolic power of the obstetric nurse was perceived under several forms: by the mobilization effect on women, by the association of the care technologies, and by the strengthening of women for labor. We concluded that the nurse, in the contact with women, has exercised a symbolic power by occupying a privileged position in the obstetric area. Such fact resulted, for the women, in transformations of their mental representations in relation to the labor process, which contributed to the construction of a new demand in the obstetric area. On the other hand, the recognition of the obstetric nurse by the users strengthened the position of her field practice.

Coordenação de Aperfeiçoamento de Pessoal de Nível Superior / Trata-se de uma pesquisa com abordagem qualitativa e descritiva, com a utilização da Grounded Theory, na perspectiva do Interacionismo Simbólico. Como objetivos tivemos: identificar os significados atribuídos por mulheres ao fato de experimentar, sentir ou vivenciar sensações de prazer sexual e/ou excitação sexual durante a amamentação; analisar e interpretar, na perspectiva do Interacionismo Simbólico, a experiência de sentir prazer sexual e/ou excitação sexual durante a amamentação, a partir dos significados atribuídos pelas mulheres. O estudo procedeu-se no Município do Rio de Janeiro, em lugares de grande circulação de pessoas, como praças, ruas, shoppings, além da Unidade de Saúde Milton Fontes Magarão, localizada na zona norte do referido Município. Essa variedade de cenários nos possibilitou encontrar uma diversidade de participantes, referente a condições socioeconômicas, culturais, religião, ideais e concepções sobre o assunto estudado. A coleta e análise dos dados aconteceram no período de maio a julho de 2014, observando todas as exigências da Resolução 466/2012 do Conselho Nacional de Saúde. Foram entrevistadas 17 participantes e formados dois grupos amostrais e três categorias expressas por: significando a amamentação como ato sagrado, inocente e assexuado: a socialização da amamentação; vivenciando e significando sensações de prazer ao amamentar; ressignificando a vivência de sensações de excitação sexual ao amamentar. Como resultados evidenciou-se que a socialização da amamentação influencia a maioria das mulheres, na vivência de sensações de prazer ao amamentar. Essas sensações são descritas através da simbologia estabelecida socialmente, de que o ato de amamentar é sagrado, puro, livre de erotismo, prevalecendo um prazer maternal. Além disso, as sensações de excitação sexual ao amamentar, para as mulheres que se dizem nunca terem vivenciado, estão presentes no seu inconsciente, porém essa vivência é admitida para os outros e não para si mesmas. O significado de vivenciar prazer sexual ao amamentar é percebido como contraditório, pois ao mesmo tempo em que se identifica o poder do corpo feminino, prevalece o poder de nutrir sobre as percepções físicas de excitação sexual ao amamentar. Desse modo, identificaram-se estratégias utilizadas para bloquear ou para não vivenciar as sensações de excitação sexual ao amamentar. Com isso, este estudo nos mostra como as mulheres agem frente às sensações de prazer sexual e/ou excitação sexual ao amamentar, através da interação social do passado e do presente, gerando os significados que cada uma carrega consigo. Além disso, percebemos como a socialização da amamentação priva a vivência da sexualidade feminina, nos aspectos de sensações de prazer sexual durante a amamentação. / It is a research with qualitative and descriptive approach, using Grounded Theory, on the Symbolic interactionism. We aimed: to identify the meanings attributed by women on the fact of trying, feeling or experiencing sexual pleasure and/or excitement sensations during breastfeeding; to analyze and interpret them. The study was developed in Rio de Janeiro, in places of great circulation of people, like squares, streets, malls, as well as a Milton Fontes Magarão Primary Care Unit, located in the North of the city. This variety of sceneries enabled us to find a diversity of participants, referring to socioeconomic and cultural conditions, religion, ideals and conceptions about the subject studied. Data collect and analysis happened in the period of may to July 2014, observing all demands from the Resolution 466/2012 from the National Health Council. 17 participants were interviewed and two group samples were formed and three categories expressed by: breastfeeding meaning as a sacred, innocent and asexual act: breastfeeding socialization; experiencing and meaning pleasure sensations on breastfeeding; resignifying experience of sexual excitement sensations on breastfeeding. The results showed that the socializing influence of breastfeeding, most women, the experience of pleasurable sensations while breastfeeding. These sensations are described by symbols socially established that breastfeeding is sacred, pure eroticism free, prevailing a maternal pleasure. In addition, feelings of sexual arousal while breastfeeding, for women who say they have never experienced, are present in the unconscious of them, but this experience is admitted for others and not for themselves. The meaning of experiencing sexual pleasure while breastfeeding is perceived as contradictory, because at the same time that identifies the power of the female body, the prevailing power of nurturing on the physical perceptions of sexual arousal while breastfeeding. Thus, we identified strategies used to block or not to experience the sensations of sexual arousal while breastfeeding. Therefore, this study shows us how women act in the face of feelings of sexual pleasure and / or sexual arousal while breastfeeding, through social interaction of past and present, generating the meanings that each carries. In addition, we see how the socialization of breastfeeding deprives the experience of female sexuality in the aspects of sexual pleasure sensations during breastfeeding.

Significando a dor no parto: expressão feminina da vivência do parto vaginal / Expressing the pain in labor: the female manifestation about vaginal delivery experience

Estudo qualitativo. Teve como objetivos Identificar os significados atribuídos à dor por mulheres que vivenciaram o parto vaginal, Analisar a vivência da dor segundo a percepção de mulheres que pariram por via vaginal e Discutir as estratégias vivenciadas pelas mulheres para aliviar a dor durante o trabalho de parto e parto vaginal. Os dados foram coletados no Alojamento Conjunto da maternidade da Unidade Integrada de Saúde Herculano Pinheiro com puérperas que passaram algum tempo dos seus trabalhos de parto na instituição. O instrumento para coleta de dados foi a entrevista semi-estruturada. As entrevistas foram gravadas, transcritas e os dados tratados com a técnica de Análise temática de Conteúdo. Para análise dos dados foi utilizado um referencial teórico pertinente aos diversos modelos assistenciais ao parto no Brasil. Vimos que as mulheres significam a dor no parto como sofrimento e/ou prazer e esse sofrimento tem a dimensão redentora influenciada pela Igreja, a dimensão heróica influenciada pela cultura e a dimensão inútil influenciada pela medicalização que retira os significados da dor. Elas significam a dor também com o prazer quando a relaciona com o bebê e com a saúde. As depoentes viveram a dor no parto sentindo uma sensação intensa e até violenta ou fisiológica que tem características próprias e são pertinentes ao processo de parturição. Algumas mulheres não focaram sua atenção na dor. Ela valorizaram mais as respostas psicoemocionais. As depoentes sentiram mais dor quando estavam dentro do hospital sofrendo as intervenções do seu modelo assistencial. Perante a dor no parto elas estrategicamente seguem os conselhos medicalizadas e comportam-se de forma civilizada. Outras mulheres utilizam estratégias instintivas para lidar com a dor no parto. Todas as estratégias utilizadas parecem ter por finalidade a finalização do processo parturitivo. / Qualitative study. Its object was to identity the meanings attributed to the pain by women who experienced vaginal childbearing, analyze the experience of the pain according to the perception of women who gave birth through vaginal delivery and discuss the strategies experienced by women to alleviate the pain during labor and delivery. The data was collected at the Unidade Integrada de Saúde Herculano Pinheiro with women in their immediate puerperium who had their childbearing labor at that institution. The data was collected through a semi-structured interview. The interviews were recorded, transcribed and the data treated through the technique of Thematic Content Analysis. For the data analysis, a theoretical standpoint was adopted pertaining to the various models of childbearing assistance in Brazil. We discovered that women see the pain in labor as suffering and/or pleasure and that suffering carries the redeeming aspect as influenced by the Church, the heroic aspect as influenced by culture and the useless aspect as influenced by the medicalization which removes all the significance of pain. They also see the pain as pleasure when they associate it to the child and health. The interviewed women experienced the pain in labor as an intense sensation, sometimes even violent or physiological, which bears its own characteristic relating to the delivery process. Some women did not focus on the pain. They valued the psycho-emtional response more. The interviewed women felt more pain while inside the hospital enduring the interventions of their assistential model. When facing the pain in labor, those women strategically follow the medicalized advice and behave in a civilized manner. Other women employ instinctive strategies to deal with the pain in labor. All the strategies used seem to aim to complete the delivery process.

Diversos estudos que abordam as repercussões das cirurgias ginecológicas na vida da mulher apontam para uma relação existente com aspectos subjetivos, em virtude da construção social da identidade feminina, indicando que a perda de órgãos ginecológicos pode interferir na maneira da mulher se perceber e se relacionar com o mundo. O presente estudo tem como objeto o processo de interação da mulher no contexto das cirurgias ginecológicas. Para isso foram formulados os objetivos: Descrever os significados da cirurgia ginecológica para a mulher submetida a este procedimento e Analisar o processo de interação da mulher com ela mesma e com o seu núcleo social a partir dos significados por ela atribuídos à cirurgia ginecológica, considerando o fenômeno da mutilação. O estudo de natureza qualitativa, do tipo descritivo-exploratório, teve como referencial teórico-metodológico o Interacionismo Simbólico e a Grounded Theory, que subsidiaram uma interpretação da ação e das relações das mulheres no contexto da cirurgia ginecológica e da mutilação, baseados na definição e interpretação de significados por elas atribuídos. Dos resultados emergiram quatro categorias: Investigando a doença e decidindo aceitar a cirurgia, A passagem pela cirurgia: vivendo o processo de resolução do problema, Enfrentando as mudanças impostas pela cirurgia e Valorizando a vida. Evidenciou-se que ao receber o diagnóstico da doença, a mulher interage com dúvidas, medos, a busca por informação e pelos benefícios da cirurgia, e por fim decide aceitá-la. Ao passar pela cirurgia, vivencia incômodos, complicações, compreende que perdeu uma parte do corpo e procura conformar-se, tentando crer que foi o único caminho. As diversas mudanças que ocorrem fazem-na construir novos significados e mudar a percepção de si e do seu meio social, a partir da interação consigo mesma e com este meio, fazendo-a refletir sobre a sexualidade, seus relacionamentos, sua saúde, a diferença no corpo e em si mesma, nas funções que desempenha e sobre a mutilação provocada pela cirurgia. O desenvolvimento de mecanismos de enfrentamento, como comparar sua cirurgia e histórias com outras, identificar aprendizados com a experiência, buscar formas de superação e resignar-se resultaram na valorização da vida e na vontade de viver melhor. Percebe-se a importância da enfermeira estabelecer um cuidado multidimensional, que identifique as necessidades que vão além do corpo biológico, respeitando as particularidades e a individualidade no momento do cuidado e contribuindo para o bem-estar físico, psíquico, social e espiritual das mulheres. / Several studies that approach the repercussions of gynecological surgery on womens lives indicate a relationship with subjective aspects, due to the social construction of the female identity, pointing out that the loss of gynecological organs may interfere in the way women perceive themselves and connects with the world. The object of this study is the process of interaction of women in the context of gynecological surgeries. To do so, the following objectives were devised: To describe the meanings of gynecological surgery for women undergoing such procedure and to analyze the process of interaction with women to themselves and with their social environment from the meanings attributed by her to gynecological surgery, considering the phenomenon of mutilation. This descriptive, exploratory study of qualitative nature had the Symbolic Interactionism and the Grounded Theory as theoretical-methodological references, thus supporting an interpretation of the action and relationships of women in the context of gynecological surgery, and mutilation, based on the definition and interpretation of meaning attributed by them. Four categories emerged from the results: Investigating the disease and deciding to accept the surgery; Undergoing the surgery: coping with the problem resolution process; Facing the changes posed by the surgery; and Appreciating life. It was evident that, when women are faced with the diagnosis, they interact with doubt, fear, search for information and the benefits of the surgery, and finally decides to accept it. After undergoing the surgery, they cope with discomfort, complications, they understand that they have lost a part of their bodies and seek resignation, trying to believe that it was the only way left. The several changes that occur make them build new meanings and change their own perception and the perception of their social environment, from the interaction with themselves and with that environment, reflection upon sexuality, their relationships, their health, the differences in their bodies and in themselves, in the role they play and in the mutilation provoked by the surgery. The development of confrontation mechanisms, such as comparing their surgeries and histories with others, identifying lessons learned with the experience, seeking ways of overcoming and resignation resulted in the appreciation of life and the desire to live better. The importance of the nurse in establishing a multidimensional care is noticeable. Such care must identify the needs that go beyond the biological body, respect the special characteristics and individuality at the moment of care and contribute to the womens physical, psychic, social and spiritual welfare.
