Man över bord : En analys av olyckor med beredskapsbåtar / Man overboard : An analysis of accidents involving rescue boats

Fartygets beredskapsbåt är en viktig del i sjöräddningsarbetet för att till exempel rädda en nödställd person ur vattnet, men dessvärre sker olyckor i samband med användandet av denna. Syftet med detta arbete var att undersöka varför olyckor med beredskapsbåtar inträffar, genom att analysera haverirapporter om beredskapsbåtsolyckor ombord på fartyg med anknytning till Europa. 13 rapporter analyserades med hjälp av HFACS-MA-modellen. Metoden kategoriserar orsaker till olyckor och har givit en bild av vilka faktorer som varit de mest frekvent bidragande i de undersökta olycksfallen. Resultatet visade att fel och brister i konstruktionen (av till exempel dävert, krok och säkerhetsbrytare), otillräcklig eller obefintlig dokumentation inom organisationen (checklistor, manualer och instruktioner) och den fysiska arbetsplatsen (hur arbetsplatsen var utformad) var de främsta orsakerna till att olyckor med beredskapsbåtar inträffade. Av resultatet framgick även att kategorin förutsättningar, som till exempel yttre miljö och kommunikation, var en stor bidragande faktor. / The ship's rescue boat is an important part of search and rescue, for example to rescue a person in distress from the water, but unfortunately accidents happen in conjunction with the use of the boat. The purpose of this essay was to investigate why accidents with rescue boats occur by analyzing accident reports involving rescue boat accidents on ships with a connection to Europe. 13 reports were analyzed with the HFACS-MA model. The method categorizes the causes of accidents and has provided a picture of the factors that have been most contributory in the investigated accidents. The result showed that deficiencies in the design (for example davit, hook and breaker), insufficient or nonexistent documentation within the organization (checklists, manuals and instructions) and the function of the physical workplace, were the main reasons for why accidents involving rescue boats occur. The result also showed that the category preconditions, such as the external environment and communication, was a major contributing factor.

Labor protection, that is prevention of occupational disease and reducing the frequency of accident, has always been a matter of major concern of mining industry. Management and the government should promote and maintain high safety standards through some measures and tools to reduce frequency of accidents and occupational diseases. This thesis describes the development of a national occupational health and safety management tool that is composed of educational, statistical and database interfaces for mine safety and health administration. The detailed analysis of an accident requires knowledge of many parameters such as location, time, type, cost of the accident, victim information, nature of injury, result of the accident etc. that can be obtained from a standard coded accident report form. So, database interface of the management tool is developed with this sense to realize collecting accident data in a nationally used format to produce a common safety reporting system. Prepared database maintains user-friendly environment on Internet to submit accident information. Dynamic structure and ease of use of the developed database allow administered user to expand it without detailed computer programming knowledge. This was achieved by prepared modules to change or register new data fields within the database. Created database is also secure since only gives data input access rights to registered users. Database administrator is able to create registered users. Registered users could be safety engineer or manager of a mine who is responsible from the submission of data to the ministry of labor. So, standard and secure accident data collected rapidly through Internet connection. The other feature of the database is that, it is open to all people to query accidents with many aspects. Prepared management tool also includes educational interface, content of which can also be enlarged, as the new ideas, information or solutions for accidents are improved. This information is also open to all people since educating workers and managers about accidents and prevention techniques can improve working conditions and increase awareness. Knowing the fact that submitted accident data is still collected in hard copies in folders, the need for a kind of management tool, which is completed in this study, is obvious. Application of this kind of management tool will be able to prevent the collection of accident data in dusty shelves and share the accident data information with all people who are interested in with this subject.

This research assessed the criminological significance of motor vehicle crash data within the criminal justice context. The study was conducted at the southern region Accident Bureau of the Ekurhuleni Metropolitan Police Department (EMPD). The objectives of this research are to: a. Explore and describe the extent and criminological impact of road traffic crashes on a macro level. b. Explore and describe the impact of road traffic crashes on a micro level. c. Explain the criminological significance of road traffic crash data in relation to effective road safety management. d. Evaluate and describe the attendance and recording procedure of road crashes. e. Evaluate and describe the supervisor’s responsibility in relation to the quality of crash data recorded. f. Evaluate and describe the effectiveness of crash statistics accessed by management and/or officials. g. Evaluate and explain the current prosecution procedure as part of the CJS. h. Evaluate existing policy about the crash-recording process applicable to operational law enforcement practitioners of the EMPD. i. Explore and describe the current state of motor vehicle insurance and its necessity from a criminological perspective. The researcher followed a mixed-methods approach, collecting quantitative data from recorded Accident Report Forms and qualitative data from interviews with participants active in their fields of expertise. Road crashes are characterised by injury and damage; victims suffer trauma and economic consequences, and the society and the economy as a whole are affected. The research suggests that accurate road crash data form a crucial component in the investigation, prosecution and development of road safety strategies in the quest towards improved road safety efforts. It also indicates that existing crash data are of such a sub-standard that traffic professionals will not be able to develop effective and efficient road safety strategies in pursuit of road safety / Criminology and Security Science / M.A. (Criminology)

Kommunikationens betydelse vid haverier till sjöss : En analys baserad på haverirapporter / The importance of communication in the case of accidents at sea : An analysis based on marine reports

Antalet haverier som är orsakade av den mänskliga faktorn minskar inte. En del av den mänskliga faktorn ligger i kommunikation mellan olika parter inom sjöfarten, både sjö- och landbaserade. Denna studie är en litteraturstudie vars syfte är att undersöka hur den verbala kommunikationen har praktiserats och vilken betydelse den haft vid kollisioner och grundstötningar. Studien är baserad på haverirapporter från Statens Haveri-kommission som har analyserats med hjälp av ett verktyg specifikt framtaget för denna studie. Resultatet visar på en kommunikationsbrist i en betydande majoritet av de analyserade haverierna. Ofta brister det i kommunikationen mellan VTS och fartyg samt mellan lots och befälhavare. Vanligt förekommande är även språkförbistring där inte alla parter förstått det språket som använts vid kommunikationen.  Med hjälp av ett mer utbrett utövande av redan kända kommunikationstekniker skulle antalet olyckor och tillbud kunna minskas. / The number of marine accidents caused by the human error does not decrease. A part of the human error is in the communication between different agents within the maritime cluster, at sea and also in the landbased sector. This study is a literature study whose purpose is to investigate how the verbal communication has been practiced and what significance it has had in collisions and groundings. The study is based on reports from the Swedish Accident Investigation Authority with a tool specifically developed for this particular study. The result shows a communication gap in a decisive majority of the analyzed accidents. There is often a lack of communication between VTS and vessels as well as between pilots and commanders. Common prevalence is also language problem where not all parties understand the language used in the communication. With the help of a more widespread practice of already known communication technologies, the number of accidents could be reduced.
