Proces léčby alkoholismu v rámci společenství Anonymních alkoholiků / The process of addiction treatment in the fellowship of Alcoholics Anonymous

This thesis concerns an alternative approach to the alcohol-addiction treatment within the fellowship of Alcoholics Anonymous and highlights the importance of self-help groups in the contemporary society. This worldwide biggest self-help group uses sociologically important processes to achieve a new way of life connected with sobriety. I have studied these processes through the participant observation. Alcoholics Anonymous influences its members through its specific culture which reflects in group rituals, high level of group affiliation, spirituality and co-constructed community story, which bears the member's knowledge. This thesis looks on the transition to identity of the sober alcoholic and the new way of living according to Twelve-Step program of Alcoholics Anonymous through the perspective of drift model of conversion, transition rituals or principles used in the narrative therapy. Within these processes there occurs radical reconstruction of perceptual, cognitive and behavioral schemes which influence the meanings in the interpretation of everyday life and consecutively change the identity of the individual and his social action. These changes of the individual are strengthened by the group members and then gradually spread out of the group boundaries too.

Familial risk and environmental stress promote the development of alcohol dependence. We investigated whether a positive family history of alcoholism affects the neuroendocrine response to a standardized laboratory stress test in healthy subjects without alcohol use disorders. Twenty-four high-risk subjects with a paternal history of alcoholism (PHA) and 16 family history negative (FHN) controls were evaluated. Psychosocial stress was induced by having subjects deliver a 5-min speech and mental arithmetics in front of an audience on separate days, after drinking either placebo or ethanol (0.6 g/kg) in a randomized sequence. Adrenocorticotropin (ACTH) was measured in 10 plasma samples covering up to 75 min after the stress test. Plasma arginine vasopressin (AVP) was determined before the stressor, at the time of maximum ACTH secretion, and at 75 min after stress onset. The stress test induced a phasic increase in ACTH secretion. At the time of maximum ACTH, AVP was significantly increased in relation to baseline. Compared to placebo, alcohol administration significantly attenuated maximum ACTH concentration in PHA but not FHN subjects, and decreased AVP measured in the same samples in PHA but not FHN subjects. We conclude that activation of the hypothalamic–pituitary–adrenal system by psychosocial stress is accompanied by an increase in peripheral plasma AVP levels. Secretion of both ACTH and AVP suggest that alcohol attenuates the stress response selectively in PHA but not FHN subjects. This might imply some short-term positive alcohol effect in sons of alcoholics, but also constitute a mechanism by which their risk to develop alcohol use disorders is increased.

Hintergrund. In klinischen und epidemiologischen Untersuchungen wurden retrospektiv Assoziationen zwischen Angst- und Alkoholstörungen bestätigt. Bei Personen, die von beiden Störungen betroffen waren, fand man restrospektiv meist einen früheren Beginn der Angststörungen im Vergleich zu den Alkoholstörungen, was kausal im Sinne der Selbstmedikationshypothese interpretiert wurde. Diese konnte bisher nicht eindeutig bestätigt werden. Zur Aufklärung notwendige prospektive Untersuchungen, die Angststörungen auf diagnostischer Ebene erfassen, zwischen verschiedenen Angststörungen unterscheiden und sich auf Jugendliche beziehen, fehlen bisher. Fragestellung. Die Bedeutung von Angststörungen für die Entwicklung von erhöhtem Alkoholkonsum und Alkoholstörungen bei Jugendlichen und jungen Erwachsenen. Methodik. Die Analysen basieren auf den Daten der prospektiv-longitudinalen EDSP-Studie (Follow-Up: insg. 4 Jahre) mit einer epidemiologischen, repräsentativen Stichprobe von 3021 Personen im Alter zwischen 14 und 24 Jahre zu T0 aus München und Umland. Zur Erhebung von DSM-IV-Diagnosen wurde das M-CIDI verwendet. Ergebnisse. Primäre Panikstörungen, Panikattacken und Soziale Phobie erwiesen sich prospektiv als spezifische Prädiktoren für Beginn und Aufrechterhaltung von Alkoholproblemen. Umgekehrt waren Alkoholprobleme mit einem erhöhten Risiko für den Beginn von Sozialer Phobie und GAS assoziiert. Bezüglich DSM-IV Diagnosekriterien fanden sich phobische Ängste sowie Episoden intensiver Angst als Risikofaktoren für den Beginn von Alkoholproblemen. Vermeidungsverhalten war mit einem verminderten Risiko für nachfolgende Alkoholprobleme verbunden. Schlussfolgerungen. Maßnahmen zur Prävention von Alkoholproblemen können sich zielgruppenorientiert an Jugendliche mit Sozialer Phobie und Paniksymptomatik richten. Bei Patienten, die sich wegen Alkoholproblemen in Behandlung begeben, ist eine umfassende Diagnostik zur Identifikation zusätzlicher Angststörungen ratsam. / Background. Many clinical and epidemiological studies have documented significant cross-sectional comorbidities between anxiety disorders and alcohol use disorders. Analysis of retrospective age-of-onset reports suggests that the anxiety disorders often start earlier than the alcohol disorders. These data have often been interpreted in terms of a self-medication-model implying a causal mechanism. Overall, in former studies this model couldn't definitely be proofed. Prospective studies that follow a sample of adolescents into adulthood assessing different anxiety disorders on an diagnostic level are needed to confirm these associations. Methods. Baseline and four-year-follow-up data from the EDSP-Study, a prospective community survey of 3021 (2548 at follow-up) adolescents and young adults aged 14 to 24 years at baseline carried out in Munich, were used. DSM-IV anxiety disorders, alcohol use and alcohol use disorders were assessed with the M-CIDI. Results. While in retrospective analyses strong associations between nearly all anxiety disorders and alcohol problems, especially harmful use and dependence, were found, prospective analyses showed that only primary panic disorder, panic attack and social phobia are specific predictors of subsequent onset and persistence of alcohol problems. Contrary, primary alcohol problems were related to subsequent onset of social phobia and GAD. Considering the different DSM-IV-criteria, phobic anxiety as well as episodes of intensive anxiety were found to be risk factors for the following onset of alcohol problems. Avoidance behavior was associated with a lower risk for subsequent alcohol problems. Conclusions. Alcohol prevention programs could be directed to target groups with social phobia and panic. Patients with alcohol problems should carefully be screened for comorbid anxiety disorders. In order to prevent relapses the treating of social phobia and panic should be part of the therapy with comorbid alcohol patients.

The theoretical part will be explained in detail the issue of behavioral disorders in children at the secondary school. There are also included chapters dealing with under-age-limit alcohol drinking and smoking since the thesis is focused on such topics. Alcohol and smoking addiction is among others reflected in questionaire investi within the practical part. The aim of the thesis is to find out what type of alcohol is mostly consumed by lower secondary school learners, in which way the information about drugs are transmited by teachers and how much learners are interested in such information. This diploma thesis deals with an issue of treatment of alcohol dependence and the process of social inclusion of people who have undergone this process. The thesis provides an overview of services and other forms of assistance that are targeted at this group. Attention is paid mainly to the current set-up of the services and assistance system to identify strong and weak aspects of the system. The work is based on a qualitative research. The research was conducted throught expert interviews and interviews with users of these services. Theoretical basis of the work is the ethiology of addiction, which offers various explanations of the origin and persistence of the problem of alcohol dependence. The work also uses the findings of the life course perspective, which allows to map the life trajectories of the interviewees, including the recognition of key events in their lives affecting the emergence and development of addicton. The last theoretical starting point of the work is the concept of social inclusion.

The thesis "Analysis of the Causes of Over-Indebtedness" consists of a detailed analysis of issues leading up to over-indebtedness. It also aims to point out the most serious consequences of over-indebtedness. The thesis brings attention to the relation between causes and effects which, in many cases, take the form of the opposite and vice versa. The boundary between cause and effect can often be indistinct, and it is essential to distinguish between the two in order to find a solution for an over-indebted client. The theoretical part of the thesis deals with the issue of a loss of income which may lead to over-indebtedness, draws attention to the important role of education in the area of financial literacy, and notes on the most serious consequences of over-indebtedness, which it sees in the loss of housing, homelessness, and other negative effects. The research part concentrates on a social group which belongs to one of the most threatened with social exclusion, welfare applicants. The research attempts to analyse their debt situation in relation to their age, education, housing, unemployment period, willingness to change their situation, and willingness to repay their debt.
