Transitioning from a Traditional Nursing Home Environment to Green House Homes: What are Stakeholders' Attitudes Toward and Satisfaction With the Small House Care Environment

This dissertation research was designed as two independent research studies. The first study, qualitative, and non-experimental, aimed to examine residents’, family members’, and staff members’ (stakeholders’) satisfaction with, and attitudes toward Green House living one month prior to moving and again at one and three months after moving. Focus groups were the primary method of data collection. Thirty residents and 40 staff members transitioned to one of three Green House homes beginning January, 2013. Data collected began in December, 2012. Following each focus group, tape recordings were transcribed, and coded. Using grounded theory and the constant comparative method of analysis, themes emerged. Pre-move focus group themes revealed that stakeholders were concerned about (a) the quality of care in a system using fewer staff members and (b) the challenges associated with adjusting to a new environment. Post-move focus group themes revealed that (a) stakeholders remained concerned about staffing levels; (b) residents’ had improvements in appetite, socializing, and ambulation; and (c) staff members struggled with autonomous work teams, but preferred the Green House model of care to that of a traditional nursing home. The final model reflects a synthesis of themes from which self-efficacy beliefs were hypothesized. Themes were also linked to existing gerontological theories: Person-Environment Fit, Place-Space, Thriving, and Personhood. The second study, designed to explore the construct validity of the Person-Centered Care Attitude Tool (Per-CCat), consisted of 42 Likert-type questions divided into four sections that align with person-centered care principles. Eighty-six employees of Virginia Mennonite Retirement Community completed the survey; only 70 were analyzed due to missing data. Principal Components Analysis was the analytic approached used for these data. Bartlett’s Test of Sphericity (X2 = 2006.56, p = 0.000) and Keiser-Myers-Olkins measure of sampling adequacy (0.746) indicated that the data were factorable. The final four-factor 34-item solution aligned with the following person-centered care principals: resident autonomy, social interaction and community, work culture, and feelings toward work. Further validations studies of the Per-CCat are necessary. Given the trend in long-term care toward person-centered care, a validated survey will be useful for hiring and educating caregivers and other nursing home personnel.

This thesis addresses the personality questionnaire Golden Profiler of Personality (GPOP). Its main objective is to verify a construct validity of GPOP. In the theoretical part, the required psychometric characteristics of psychodiagnostic tools are discussed. It also deals with the theoretical background of the GPOP - Jung's theory of psychological types and the Five Factor Model. The questionnaire GPOP is also compared with other psychodiagnostic tools with similar theoretical background and possibilities of its applications are discussed. The empirical part examines relationship between the scores of Golden Profiler of Personality and NEO inventory NEO-PI-R to determine the construct validity of GPOP. The research was conducted on the sample of university students. Results confirm the expected relationship and can be considered as the evidence of construct validity. The comparison with the norm sample showed that the sample of this research differs from the norm sample. Although the generalizability of results to the population is limited, this thesis offers the first validation study of Czech version of GPOP. El objetivo del presente estudio fue analizar las propiedades psicométricas (evidencias de validez y confiabilidad) de la ENCAO en trabajadores de una empresa privada del sector de hidrocarburos en Lima Metropolitana. Para esto, se evaluó a una muestra de 384 participantes, 64 % mujeres y 36 % hombres, con edades entre los 19 y los 56 años (M = 29.57 años, DE = 7.33). Como evidencias de validez de la estructura interna del instrumento, se realizó un análisis factorial exploratorio (AFE) y un análisis factorial confirmatorio (AFC), donde se encontró una estructura de cuatro factores relacionados (formación y cultura organizacional, aprendizaje social, aprendizaje grupal y aprendizaje estratégico individual) con los 21 ítems de la escala, los cuales explicaron el 60.33 % de la varianza total y obtuvieron adecuados índices de bondad de ajuste (χ² = 371.66; χ²/gl = 2.03; CFI = .99; RMSEA = .041). Como fuente de validez externa se obtuvieron correlaciones moderadas (entre .31 y .46) con la variable satisfacción laboral. Los coeficientes alfa de Cronbach de los cuatro factores oscilaron entre .60 y .84. Se concluye que la ENCAO y sus puntuaciones derivadas son una medida válida y fiable para medir el aprendizaje organizacional en el sector laboral evaluado. / Revisón por pares

The construct validity and reliability of the child memory scale, the search and memory task, and the Toulouse-Pieron Test for a sample of South African primary school learners.

In an effort to contribute to growing knowledge about measuring instruments’ applicability in different South African groups, the study attempted to ascertain the construct validity and reliability of three measuring instruments of memory. Furthermore, statistical properties and potential gender differences in a population of primary school learners in a school in KwaZulu-Natal between these instruments, namely the Child Memory Scale (CMS), the Search and Memory Task (SMT), and the Toulouse-Pieron Test (TPT), were explored. The internal reliability of the CMS was acceptable, but the Recognition section of the test was, on the whole, inconsistent and as such problematic. The SMT’s reliability estimate was lower, but acceptable. Some of the items on the test proved to be problematic in terms of the test’s overall reliability. Significant but weak relationships were found between the CMS and TPT, as well as between the SMT and TPT. However, no significant relationship was found between the SMT and the CMS. Furthermore, no significant gender differences were found between the three tests and no significant difference in correlation between the gender matrices was evident. These findings call the construct validity of the CMS, SMT and TPT in question, as literature (Gathercole & Martin, 1996; Logie, 1999) indicate probable significant inter-correlations between different aspects of memory.

Validade e reprodutibilidade de um questionário de frequência alimentar baseado nos grupos alimentares da pirâmide alimentar para adolecentes / Validity and reproducibility of a food frequency questionnaire based on the food groups of food pyramid for adolescents

Introdução: A avaliação da dieta habitual do adolescente é complexa devido às variações intra e interindividuais. Os estudos de validação e reprodutibilidade de questionários de freqüência alimentar são necessários para garantir a utilização de instrumentos capazes de mensurar a dieta habitual real. Objetivo: Desenvolver, verificar a validade e reprodutibilidade de um questionário de freqüência alimentar baseado nos grupos alimentares da pirâmide alimentar para adolescentes (QFA-PA). Materiais e métodos: Após a identificação dos alimentos a serem incluídos no QFA-PA, o método foi testado em estudo piloto. A versão final consistiu em 50 itens alimentares. O estudo foi realizado com 109 adolescentes de ambos os gêneros durante um período de quatro meses. Para verificar a validade e reprodutibilidade do QFA-PA foram aplicados quatro Recordatórios de 24h (Rec24h) e dois QFA-PA. A validade foi obtida pela comparação entre o segundo QFA-PA e à média dos Rec24h. A reprodutibilidade foi mensurada por meio da comparação dos dois QFA-PA. Resultados: O questionário desenvolvido representou 97,7% das calorias consumidas e pelo menos 90% da ingestão dos nutrientes selecionados. Foram incluídos os alimentos freqüentemente relatados ausentes no QFA e freqüentemente relatados sem maior contribuição energética. As variáveis melhor avaliadas foram os grupos do arroz, pão, massa, batata e mandioca; das carnes e de açúcares e doces. Os piores resultados foram observados para a vitamina C. Cabe ressaltar que este é o primeiro QFA desenvolvido, validado e acurado no Brasil para adolescentes que avalia o consumo alimentar baseado na pirâmide dos alimentos dos adolescentes. Conclusão: Considerando-se as análises realizadas, o QFA-PA apresentou validade satisfatória e reprodutibilidade aceitável para a maioria das variáveis. / Introduction: The assessment of habitual diet of adolescent is complex due to within and between-person variations. The studies of validity and reproducibility of food frequency questionnaire (FFQ) are needed to ensure the use of reliable and accurate instruments capable of measure usual diet. Objective: To develop, verify the validity and reproducibility of a food frequency questionnaire based on the food groups of food pyramid for adolescents (FFQ-PA). Methods: After identify the items to be included in the FFQ-PA, the method was tested in a pilot study. The final version consisted of 50 food items. The study was conducted with 109 adolescents of both genders over a period of four months. To check the validity and reproducibility of the FFQ-PA were applied four 24-hour records (Rec24h) and two FFQ-PA. The validity was obtained by comparing the second FFQ-PA and average Rec24h. The reproducibility was measured by comparing the two FFQ-PA. Results: The questionnaire represented 97,7% of the calories consumed by the population and at least 90% of the intake of selected nutrients. We included food often reported missing in the FFQ and the frequently reported without further energy contribuition. In the study of validity and reproducibility, the variables were evaluated further groups of rice, bread, pasta, potatoes and cassava, meat and sugar and sweets. The worst results were observed for vitamin C. It is noteworthy that this is the first FFQ developed, validated and accurate in Brazil for adolescents that assesses food consumption based on the adolescent food pyramid. Conclusion: Considering the analysis, the FFQ-PA showed satisfactory validity and reproducibility acceptable for most variables.

Com a expansão da Educação a Distância (EaD) no Brasil, nos últimos anos, avaliações são fundamentais para mensurar o seu desempenho. Contudo, a qualidade é muitas vezes questionada e possivelmente uma das causas é a falta de confiabilidade e validade dos instrumentos utilizados. Desta forma, o objetivo deste estudo foi criar uma arquitetura de orientação para a construção de instrumentos de avaliação de Programas de Educação a Distância com evidências de confiabilidade e validade. A investigação é uma pesquisa descritiva e explicativa, que congrega os enfoques qualitativo e quantitativo e é sustentada por um arcabouço teórico e metodológico. Além disso, parte do pressuposto de um sistema integrado, cuja natureza cíclica do método confere dinamicidade e flexibilidade, sobretudo em ações que lastreiam qualidade ao produto final. Para isso, propõe-se um conjunto de elementos que podem ser utilizados como referência para a construção de diferentes instrumentos de avaliação. A partir da arquitetura de orientação desenvolvida criou-se um plano de assessment, o qual foi aplicado em um caso específico. Os resultados demonstram que a arquitetura é viável e tem capacidade de atender diferentes modelos e contextos de EaD, de forma que qualquer tipo de Instituição de Ensino pode utilizá-la para a construção de seus instrumentos avaliativos (assessment) de Educação a Distância. Conclui-se, portanto, que a arquitetura de orientação integra componentes de confiabilidade e validade, o que assegura uma tomada de decisão segura e precisa. / With the expansion of Distance Education (DE) in Brazil in recent years, assessments are crucial to measure its performance. However, its quality has been frequently questioned and one of the reasons may be the lack of reliability and validity in the tools used for it. Therefore, the objective of this study was to create an architecture to guide the construction of assessment tools for Distance Education Programs, with evidences of reliability and validity. The investigation is a descriptive and explanatory research, which brings together the qualitative and quantitative approaches and is underpinned by a theoretical and methodological framework. Moreover, it is based on an integrated system whose methods cyclical nature allows dynamicity and flexibility, mainly in actions that assure quality to the final product. Hence, it proposes a a set of elements that can be used as reference for the construction of different assessment instruments. From the architectural guidance designed to create an assessment plan, which was applied in a particular case. The results show that the architecture is feasible and has the capacity to suit different contexts and models of distance education, so that any type of Education Institution can use it for building their evaluative instruments (assessment) of Distance Education. Therefore, it is concluded that the Architecture Guidance integrates components of reliability and validity, which assure a precise and trustworthy decision taking.

Personalidade tipo D e doenças cardiovasculares: adaptação de uma escala / Type D personality and cardiovascular diseases: a scale adaptation

As doenças cardiovasculares (DCVs) são as doenças não transmissíveis com maiores taxas de morbidade e mortalidade no Brasil e no mundo, tendo grande impacto socioeconômico. Dentre os fatores de risco (FRs) biopsicossociais estudados, a personalidade tipo D (PTD) destaca-se como um FR psicossocial emergente e crônico que se refere à personalidade não patológica que torna o indivíduo mais suscetível a outros FRs, aumentando as chances de estabelecimento e prognóstico desfavorável de DCVs. Caracteriza-se pela apresentação simultânea dos traços de personalidade Afetividade Negativa (AN) e Inibição Social (IS), podendo ser detectada pela DS-14, escala composta por 14 itens (subescalas de AN e de IS, cada uma com sete itens) cujas opções de resposta variam em escala Likert de cinco pontos, de zero (False) a quatro (True). Este trabalho teve por objetivo realizar a adaptação da DS-14 para o Brasil, além de comparar sua manifestação em pessoas com e sem DCV. O processo de tradução e adequação cultural baseou-se nas recomendações da American Academy of Orthopaedic Surgeons e do Institute of Work and Health, e teve a participação de dois tradutores, dois retrotradutores e uma linguista. As versões elaboradas foram testadas para confirmação de adequação. Para a verificação de evidências de validade, os escores das subescalas AN e IS foram correlacionados aos escores das subescalas da Bateria Fatorial de Personalidade e foi efetuada análise fatorial exploratória. Para analisar a fidedignidade foi realizado teste-reteste e análise de consistência interna. Os dados de prevalência da PTD e comparação dos escores das subescalas foram obtidos da mesma amostra do estudo psicométrico. A DS-14, versão em português, foi testada com 40 sujeitos, sendo 60,0% do sexo feminino, com média de 45,2 anos de idade (DP=14,95), 72,5% sem problemas de saúde, até que se chegasse à DS-14 versão final, utilizada no estudo psicométrico. Este teve a participação de 184 sujeitos, dos quais 54,3% eram do sexo feminino, tendo em média 47,45 anos de idade (DP=13,48), sendo que 56,5% não tinham DCV. A partir da amostra total, 140 indivíduos compuseram estratificação proporcional de acordo com os subgrupos da população cardiopata (70 sujeitos) e da população geral (70 sujeitos) de Ribeirão Preto para correlação com a BFP. Destes, 44 ainda compareceram após média de 96 dias para reaplicação da escala. Os coeficientes de correlação da AN com o traço Neuroticismo (0,63, p<0,01), assim como o da IS com o traço Extroversão (-0,32, p<0,01) correspondem aos encontrados na literatura internacional e são coerentes com suas definições teóricas. A análise fatorial exploratória encontrou dois fatores (AN e IS) que, juntos, explicam 46,58% da variância dos dados. Os itens da DS-14 se agruparam exatamente conforme escala a original, corroborando o modelo teórico e somando-se aos dados de outros trabalhos para fortalecer as evidências de validade da DS-14. Foram encontrados os índices de 0,76 (AN) e 0,65 (IS) na correlação teste-reteste, e 0,79 (AN) e 0,76 (IS) na análise de consistência interna, o que, junto aos resultados acima, demonstra que a DS-14 tem características psicométricas adequadas para uso junto à população brasileira. A prevalência de PTD na amostra total foi de 45,7%, com escore médio de 24,99 (11,60), com presença de PTD em 50% da amostra cardiopata e 42,3% da amostra sem cardiopatia. Com relação às subescalas, os escores do Grupo DCV foram maiores do que os escores do Grupo sem DCV, com diferença estatisticamente significante no traço AN. A DS-14 é de aplicação rápida e adequada à língua portuguesa usada no Brasil. Os resultados do estudo psicométrico apresentaram evidências consistentes de validade e de fidedignidade, o que a torna um instrumento seguro para uso em pesquisas brasileiras. A tendência da PTD ser mais intensa em indivíduos cardiopatas deve ser melhor estudada. / Cardiovascular diseases (CVDs) are the non-communicable diseases which present the highest rates of morbidity and mortality both in Brazil and the world, causing a major social economic impact. Among studied bio-psychosocial risk factors (RFs), Type D personality (TDP) stands out as an emergent and chronic psychosocial RF, which refers to the non-pathological personality that makes the individual more susceptible to other RFs, increasing the chances for adverse onset and prognosis of CVDs. It is characterized by the simultaneous presentation of the personality traits: Negative Affectivity (NA) and Social Inhibition (SI), and it can be detected by the DS-14, a scale consisting of 14 items (seven-item subscales of NA and SI) whose response options vary according to the five-point Likert scale, from zero (False) to four (True). This study aimed to adapt the DS-14 scale to Brazil, as well as to compare its manifestation in people with or without CVDs. The process of translation and cultural adaptation was based on the recommendations offered by the American Academy of Orthopaedic Surgeons and the Institute of Work and Health, and it was performed by two translators, two retro-translators and a linguist. The elaborated versions were tested in order to confirm the adequacy. In order to verify the evidences for validity, the scores for the subscales NA and SI were correlated to the scores for the subscales from the Factorial Personality Test Battery and an exploratory factorial analysis was conducted. A test-retest and an internal consistency analysis were accomplished in order to study the reliability. The data for the prevalence of TDP and for the comparison of the subscales scores were obtained from the same sample of the psychometric study. The DS-14 Portuguese version was tested in 40 subjects, from those, 60% were female, 45.2 years old in average (SD=14.95), 72.5% did not have any health problems. In the psychometric study 184 subjects participated, from which 54.3% were female, 47.45 years old in average (SD=13.48), and 54.3% did not have CVDs. From the total sample, 140 individuals were part of a proportional stratification, according to the subgroups of the population with heart disease (70 subjects) and the general population (70 subjects) from the city of Ribeirão Preto for the correlation with the Factorial Personality Test Battery. From these, 44 presented for retest after an average of 96 days. The correlation coefficient for NA with the trait of Neuroticism (0.63, p<0.01), as well as the coefficient for SI with the trait of Extraversion (-0.32, p<0.01) correspond to the ones found in international literature and are coherent with their theoretical definitions. The exploratory factorial analysis found two factors (NA and SI), which, together, explain the variance of 46.58% of the data. The items of the DS-14 grouped themselves exactly according to the original scale, confirming the theoretical model and adding to the data of other studies to strengthen the evidences for the DS-14 validity. Rates of 0.76 (NA) and 0.65 (SI) were found in the test-retest correlation, and 0.79 (NA) and 0.76 (SI) were found in the internal consistency analysis, which, along with the aforementioned results, demonstrate that DS-14 have adequate psychometric characteristics to be used in Brazilian population. The prevalence for TDP in the total sample was 45.7% with average score of 24.99% (11.60), as TDP was present in 50% of the population with heart disease and 42.3% in the population without any heart disease. Regarding the subscales, the scores for the CVD Group were higher than the scores for the Group without CVD, differing significantly statically in the NA trait. The DS-14 has a rapid and suitable application to be used in Brazil. The results for the psychometric study presented consistent evidences of validity and reliability, which makes it a safe instrument in Brazilian research. The tendency for TDP to be more intense in individuals with heart disease must be studied more deeply.

Aplicabilidade, validação e reprodutibilidade do Spinal Cord Independence Measure version III (SCIM III) nos pacientes com paraparesia espástica / Applicability, validation and reproducibility of the Spinal Cord Independence Measure version III (SCIM III) in patients with spastic paraplegia

Objetivo: verificar a aplicabilidade, reprodutibilidade e validade do SCIM III nos pacientes com paraparesia espástica. Método: estudo transversal incluindo 30 sujeitos (66% mulheres; 41,5±14,7 anos) com paraparesia espástica de etiologia genética, infecciosa ou a esclarecer que foram avaliados pela análise computadorizada da marcha, versão brasileira da SCIM III (0-100 pontos), MIF (18-126 pontos), por 2 examinadores (A e B) no mesmo dia e 1 semana depois (A). Resultados: o coeficiente de correlação intraclasse (CCI) para o uso da SCIM III indicou boa reprodutibilidade intra e inter-examinadores (CCI=0,9). A correlação de Spearman entre a SCIM III e a parte motora da MIF foi considerada adequada e estatisticamente significante (Spearman=0,6; p=0,001). A correlação entre as subescalas da SCIM III e os domínios da MIF foi considerada forte e significante para auto-cuidado (Spearman=0,8; p0,001) e moderada para transferências (Spearman=0,6; p=0,0005) e locomoção (Spearman=0,6; p=0,0006). A subescala mobilidade da SCIM III mostrou correlação positiva e significante para cadência (Spearman=0,8; p0,001), velocidade da marcha (Spearman=0,7; p0,001) e comprimento do passo (Spearman=0,6; p0,001). Conclusões: A SCIM III é um instrumento de avaliação funcional reprodutível intra e inter examinadores e capaz de avaliar o nível de independência dos indivíduos com paraparesia espástica. O SCIM III é mais sensível que a MIF, especialmente para pacientes com maior independência. Cadência, velocidade de marcha e comprimento do passo se correlacionaram com a subescala mobilidade / Purpose: to verify the applicability, reproducibility, and validity of the SCIM III patients with non-traumatic spastic paraplegia. Method: The cross-sectional study included 30 subjects (66% females; 41.5 ± 14.7 y) older with spastic paraparesis of any etiology were assessment by computerized gait analysis and with the Brazilian versions of SCIM III (0-100 points), FIM (18-126 points) by 2raters (A and B) at the same day and 1 week later (A). Results: The intraclass correlation coefficient (ICC) for the use of SCIM III indicated good intra and inter-evaluator reproducibility (ICC = 0.9). Correlation between the SCIM III and the motor FIM was appropriate (Spearman=0.6; p0.001). SCIM III subscales and the FIM domains correlated strongly for self-care (Spearman=0.8; p0.001), moderately for transfers (Spearman=0.6; p=0.0005) and locomotion (Spearman=0.6; p=0.0006). SCIM III mobility subscale positively correlated with the cadence (Spearman=0.8; p0.01), gait speed (Spearman=0.7; p0.01), and step length (Spearman=0.6;p0.01). Conclusions: SCIM III is a reproducible functional assessment instrument and capable of evaluating the level of independence of the individual with spastic paraplegia. The SCIM III is more sensitive than the FIM for non-traumatic spastic paraplegic patients with higher levels of independence. Linear gait parameters correlated with its mobility subscale.

Com o objetivo de quantificar o evento psico-social \"Atitude em relação a Centros de Saúde\", uma escala Thurstone dos \"Intervalos equi-aparenciais\" foi construida e apresentada. Inicialmente montou-se um quadro teórico, onde foi inserido o conceito a ser mensurado e a partir do qual construiram-se duzentos e sete estímulos- afirmações. Quatro áreas de conteúdo foram abrangidas: atitude em relação aos médicos, atitude em relação a serviços oferecidos, atitude em relação a funcionários e atitude em relação ao ambiente psico-social dos Centros de Saúde. As afirmações foram submetidas a quatro grupos de juízes compostos de médicos, enfermeiros, educadores de saúde pública e sociólogos que trabalham na área da saúde. Os valores escalares e índices de ambiguidade foram calculados para cada grupo profissional e para o grupo como um todo. As afirmações definitivas foram selecionadas através dos seguintes critérios: valor escalar, índice de ambiguidade e área de conteúdo. Foram escolhidas cinquenta e sete afirmações: dezenove na primeira área de conteúdo, treze na segunda área de conteúdo, quinze na terceira área e dez na quarta. Esta pesquisa foi somente uma primeira abordagem ao problema de mensuração de atitudes. Um pré-teste será necessário para testar validade e fidedignidade do instrumento construido para que depois seja empregado em pesquisas ligadas à educação em saúde pública. / With the purpose of gaining quantitative knowledge about the social psychological phenomena \"Atitude toward Health Centers\", A Thurstone \"Equal Appearing Interval\" scale was developed and presented. In the begining a theoritical referal system was built ineluding the concept to be measured, and 207 statements were devised. Four content areas were included: attitude toward physicians, attitude toward health services offered, attitude toward social-psychological environment of the Health Centers. The statements were presented to four groups of judges composed of psysicians, public health educators, nurses and sociologists working in public health. Scale values and a Q value were calculated separately for each statement for each group of Judges and for de whole group. A final scale was selected by three criterion: scale values, Q value and content areas. From the 207 scored statements a scale containing 57 items was developed: 19 in the first content area, 13 in the second, 15 in the third, 10 in the fourth. This research was only a first approach to measure atitudes. A pretest will be necessary to test validity and reliability of the instrument developed, to make it useful to research applied to public health education.
