Examining Nurse Satisfaction with a Bedside Handover Report Process

Nurses' job satisfaction affects work performance at the point of care in hospitals. The incoming nurses who are able to receive a comprehensive patient report at shift change are more prepared in comparison to incoming nurses who are not able to receive a comprehensive patient report to provide care that is safe. The purpose of this project, guided by the theory of organization change, was to explore whether the use of a bedside handover process impacts nurses' satisfaction in an adult postoperative orthopedic and spine unit. A post-implementation survey of the bedside handover process was conducted after one month and two months to examine registered nurses' (RN) (n = 50) satisfaction using a 7-question self-designed instrument with a reliability coefficient of 0.80. The Bedside Handover Report Staff Nurses' Satisfaction Survey consisted of 5-item Likert scale with scores ranging from 1 (strongly disagree) to 5 (strongly agree). The survey results found that RNs were satisfied with the bedside handover report process. Matched-pair t tests revealed significant differences between the first and second months after the handover report process was implemented. Specifically, 'Bedside handover report provides time for the incoming RN to verify patient's health issues' (p = .05),' I am satisfied with the handover report process conducted at the patient's bedside' (p = .01), and total score (p = .03) improved from the first to second month. A longitudinal study spanning 6 months to a year is recommended when the project will be implemented in the entire facility. A bedside handover report increases nurse satisfaction because the process allows the nurses to verify and address patient health issues that are essential for positive social change.

Work Environment And The Effect On Occupational Commitment And Intent To Leave: A Study Of Bedside Registered Nurses

The purpose of this research was to determine the effect work environment has on occupational commitment and intent to leave the profession for bedside registered nurses. Subscales of autonomy, control over the practice setting, nurse-physician relationship, and organizational support were incorporated into the analysis to determine which aspect of work environment most directly effects occupational commitment and intent to leave the profession. The research was undertaken in order to help administrators determine the ways in which work environment can be improved upon in order to retain bedside registered nurses in the profession. An explanatory cross sectional survey was distributed to 259 direct care bedside registered nurses employed at a rural, system affiliated hospital in Central Florida. Human subject protection was assured through the University of Central Florida Institutional Review Board. A 77 item questionnaire containing 9 demographic questions, 57 questions from the Nursing Work Index- Revised (NWI-R), 8 questions from Blau's occupational commitment scale, and 3 questions from Blau's intent to leave scale was distributed to all direct care nurses. Subjects were also given the opportunity to complete 3 short answer questions. A 32.8 percent response rate was achieved for a total of 85 complete and usable surveys. Data analysis showed that the work environment is positively related to occupational commitment and negatively related to intent to leave. In addition each of the four subscales (autonomy, control over the practice setting, relationship with physicians, and organizational support) were also positively related to occupational commitment and negatively related to intent to leave the profession. Implications for organizations, public policy and future research are discussed.

Patienters uppleverser av överrapportering vid sängkanten

Bakgrund: Överrapportering mellan skiftbyten inom vården har en viktig roll när det gäller överföring av information till nästkommande sjuksköterska. Inom hälso- och sjukvården finns olika rapporteringssätt, samtal mellan sjuksköterskor avskilt på en expedition och rapportering vid sängkanten hos patienten. I ett flertal studier har sjuksköterskors uppleverser av överrapportering vid sängkanten undersökts, vilket har visat att det ger en ökad personcentrering i omvårdnaden, ökad patientsäkerhet och minskade missförstånd vid kommunikationen.  Syfte: Studiens syfte var att beskriva hur vuxna patienter inom slutenvården upplever sjuksköterskors överrapportering vid sängkanten i samband med skiftbyten.  Metod: Litteraturöversikten har utgått från 12 kvalitativa originalartiklar som kvalitetsgranskats och analyserats där två av dessa både är kvalitativa och kvantitativa.  Resultat: Totalt fyra kategorier identifierades i resultatet: mer personcentrering, ökad trygghet, ett observandum och upplevda hinder. Majoriteten av patienterna upplevde att överrapportering vid sängkanten ökade deras delaktighet och involvering i vården. Genom att bidra med information och klargöra missförstånd i kommunikationen upplevde patienterna minskad osäkerhet och ökad trygghet. De ansåg att medicinska uttryck som användes av sjuksköterkorna kunde förhindra deras förståelse. Språkbarriärer och patienters hälsotillstånd identifierades även som hinder i överrapporteringarna. Känslig information ansågs inte som något hinder, förutsatt att informationen hanterades med varsamhet, respekt och personlig hänsyn till patienten utifrån vårdsituation.  Slutsats: I litteraturöversikten har resultat av patienters upplevelser gällande överrapportering vid sängkanten identifierats, vilka övervägande var positiva upplevelser. Det identifierades också negativa aspekter av arbetssättet. / Background: Handovers between shift changes in healthcare play an important role in the transfer of information to the next nurse. In healthcare, there are different ways of reporting, conversations between nurses in a separate office and reporting at the patient´s bedside. A number of studies have investigated nurses´experiences of bedside reporting, which have shown that it provides increased person-centeredness in nursing, increased patient safety and reduced misunderstandings in communication.  Purpose: The purpose of the study was to describe how adult patients in inpatient care experience nurse´handover at the bedside in connection with shift changes.  Method: The literature review is based on 12 original qualitative aticles who were quality reviewed and analyzed, two of which are both qualitative and quantitative.  Results: A total of four main categories was identified: more person-centered care, increased security, an observandum and perceived obstacles. The majority of patients felt that bedside handover increased their participation and involvement in their care. By providing information and clarifying misunderstandings in communication, patients experienced reduced uncertainty and increased safety. They felt that medical terms used by nurses could hinder their understanding. Language barriers and patients´health conditions were also identified as obstacles in the transfer of information. Sensitive information was not considered a barrier, provided it was handled with care, respect and personal consideration for the patient based on the care situation.  Conclusion: In the literature review, results of patients´experiences regarding bedside over-reporting have been identified, which werw predominantly positive experiences. Negative aspects regarding the topic were also identified.

Bakgrund: Informationsutbyte mellan sjuksköterskor vid skiftbyte, så kallad överrapportering är en viktig del av sjuksköterskans ansvarsområde. Sjuksköterskan ansvarar för att bedriva en god och säker vård, vilket främjas när patienten ges möjlighet till att delta i sin vård och därmed bli en i vårdteamet. Bedsiderapportering är den enda rapporteringsmodell som inkluderar patienten och har blivit allt vanligare inom hälso-och sjukvården. Syfte: Att undersöka sjuksköterskors upplevelser av bedsiderapportering. Metod: Kvalitativ intervjustudie med induktiv ansats. Åtta semistrukturerade intervjuer genomfördes med sjuksköterskor på en slutenvårdsavdelning. Resultat: Resultatet presenterar tre huvudkategorier; Patienten i fokus, Sjuksköterskans professionella ansvar och Arbetsbelastningens påverkan med tillhörande sju subkategorier. Sjuksköterskans involvering av patienten vid bedsiderapportering främjar patientdelaktigheten och är det mest framträdande ur resultatet. Bedsiderapportering upplevs som positivt men medför även vissa utmaningar som exempelvis tidskrävande och äventyrande av patientens sekretess. Konklusion: Bedsiderapportering främjar patientdelaktighet men det sker inte automatiskt. Det krävs att sjuksköterskan har kunskap och färdigheter i hur patienten involveras i överrapporteringen för att uppnå patientdelaktighet. Bedsiderapportering ger sjuksköterskan möjlighet att skapa en egen uppfattning av patienten utan påverkan av kollegors åsikter. Utmaningar med rapporteringsmodellen är att det upplevs tidskrävande samt risken att äventyra patientens sekretess. / Background: Shift handover is an important part of nurses’ responsibility. Nurses have a professional responsibility to provide safe and high-quality care which is promoted when the patient is a participant in their care. Bedside handover is the only model used during shift handover that includes the patient and has become a more common model. Aim: To investigate nurses´ experiences of bedside handover. Method: Qualitative interview study with an inductive approach. Eight semi-structured interviews were performed with nurses working in an inpatient ward. Findings: The result were presented in three main categories; The patient in focus, The nurse´s professional responsibility and The impact of workload with associated seven subcategories. The nurse´s involvement of the patient during the bedside handover reporting promotes patient participation and was the most prominent aspect. Bedside handover was perceived as something positive but also brought some challenges by being time-consuming and compromising patient confidentiality. Conclusion: Bedside handover promotes patient participation, but it does not happen automatically. It is required that the nurse has knowledge and skills in how to involve the patient in the handover to achieve patient participation. Bedside handover gives the nurse the opportunity to form their own opinion of the patient without being influenced by colleagues’ opinions. Challenges with bedside handover are that it is time-consuming and risks jeopardizing the patient´s confidentiality.

Traditionellt sker rapporteringen i frånvaro av patienten och utan patientens delaktighet, trots att det handlar om patientens behov, resurser och vidare planering. Bedsiderapportering är en metod som sker tillsammans med patienten och patientens delaktighet blir en del av processen.Syfte: Studiens syfte är att beskriva patienters erfarenhet av bedsiderapportering i slutenvården.Metod: Litteraturstudien är baserad på tolv kvalitativa empiriska vetenskapliga artiklar. Artiklarna har framtagits genom litteratursökning från databaserna Cinahl och PubMed och granskats enligt SBU:s kvalitetsgranskningsmall. Data analyserades i fyra steg i enlighet med Friberg (2022) för att identifiera relevanta tema vilket resulterade i en ny helhet.Resultat: Resultatet är presenterat i fyra huvudteman som är relaterat till syftet och beskriver patienternas erfarenheter. Dessa huvudtema är delaktighet, information, patientsäkerhet och brister. Utifrån dessa huvudteman har tio subtema skapas.Konklusion: Majoriteten av patienterna var positivt inställda till bedsiderapportering. Det bidrog till förbättrad patientsäkerhet och ökade patienternas delaktighet och involvering i vården. Patienterna upplevde att de blev välinformerade och hade möjlighet att ställa frågor och komplettera informationen som delgavs. Bedsiderapporteringen bidrog till att skapa relationer mellan sjuksköterska och patient. / Background: Reporting is a central part of the care and in the planning of care around the patient. This is an important part of the nurse's job. Traditionally, reporting takes place in the absence of the patient and without the patient's participation, even though it is about the patient's needs, resources, and further planning. Bedside handover is a method that takes place in the presence of the patient and the patient's participation becomes part of the process.Aim: The aim of the study is to describe patients' experience of bedside handover in inpatient care.Method: The literature study is based on twelve qualitative empirical scientific articles. The articles were produced from the Cinahl and PubMed databases and were reviewed according to SBU's quality review template. The data was analyzed in four steps in accordance with Friberg (2022) to identify relevant themes, which resulted in a new whole.Result: The result is presented in four main themes that are related to the purpose and describe the patients' experiences. These main themes are participation, information, patient safety and shortcomings. Based on these main themes, ten subthemes were created.Conclusion: Most patients were in favor of bedside handover. It contributed to improved patient safety and increased patients' participation and involvement in their care. Patients felt that they were well informed and had the opportunity to ask questions and supplement the information that was served. Bedside handover helped to create relationships between nurse and patient.

Lampan har haft krav på hållbar utveckling. Lampan har en energieffektiv ljuskälla, vald med hänsyn till sitt syfte, och ingående material har valts med stark miljöhänsyn. Form och funktion har varit viktiga begrepp och har parallellt följts åt i projektet. Produkten har konstruerats för en okomplicerad isärtagning så att delarna enkelt kan sorteras och återvinnas vid armaturens avslutade användning. / The project has served to, in cooperation with Örsjö Belysning AB, develop a wall mounted luminaire for hotel rooms that will function as a bedside reading light. The lamp has had requirements for sustainable development. The lamp has an energy-efficient light source, selected for its purpose, and the materials have been selected by strong environmental concerns. Form following function is an important concept for this project. The product is designed for a straightforward disassembly so that parts can be easily sorted and recycled at the end of use of the fixture.

Bakgrund : Delaktighet och personcentrerad vård är centrala begrepp i den vård som bedrivs idag. För att patienten ska vara delaktig krävs det att patienten är mer involverad i de dagliga händelserna som till exempel skiftrapportering. Vid varje skiftbyte sker överrapportering från en sjuksköterska till en annan, överrapporteringarna saknar ofta patientmedverkan. En rapporteringsmodell är bedsiderapportering vilket innebär att rapporteringen sker i patientens närvaro. Syfte : Syftet var att beskriva patientens delaktighet i vården vid bedsiderapportering. Metod : En litteraturöversikt. Resultat : Bedsiderapportering fick patienterna att känna sig delaktiga i vården, vilket delvis berodde på att de kände sig som värdefulla deltagare. Patienterna betraktades i första hand som en person. Både patienter och sjuksköterskor ansåg att rapporteringssättet var avgörande för patientens delaktighet i vården. De flesta patienter uppskattade att få vara med i rapporteringen och få viktig information som de ansåg sig ha rätt till. Dock ville inte alla patienter delta, vilket bland annat berodde på att rapporten ansågs vara till för sjuksköterskor. Slutsats : Bedsiderapportering ökade patientens delaktighet i vården enligt både patienten och sjuksköterskan. Dock ville inte alla patienter delta i rapporteringen och graden av delaktighet var beroende av på vilket sätt rapporten gavs. / Background : Participation and person-centered care are key concepts in the care that are being given today. In order for the patient to participate, they must be involved in the daily work such as shift reporting. At each shift, reporting occurs from one nurse to another which often lacks patient involvement. A reporting model is bedside report, which means reporting takes place in the patient's presence. Aim : The aim of this study was to describe patient’s participation in care during bedside report. Method : A literature review. Results : Bedside report made patients feel more involved in their care, partly because they felt valued as a participant in the report. The patients were considered a person in the first hand. Both patients and nurses felt that the reporting method was crucial for the participation. Most patients appreciated being included in the reporting and that they received valuable information that they considered justifiable. However, not all patients wanted to participate, which was due, among other things, to the fact that the report was considered for the nurses. Conclusion : Bedside report increased patient involvement in care, according to both the patient and the nurse. However, not all patients wanted to participate and the reporting method were important for the level of participation.
