Incorporation of the Caring Moment in Scripting

A large healthcare system in Texas mandated scripting as way to improve patient satisfaction. The purpose of this project was to incorporate a caring moment into scripting by the nursing staff to improve patient satisfaction. The project was guided by Watson's caring theory, the adult learning theory, and Lewin's planned change theory. All 35 nurses from the primary care and specialty care outpatient clinics of the Texas healthcare system participated in a staff development training to learn how to incorporate a caring moment into their script. Of these 35 nurses, 13 (37%) chose to complete the pre-education and postproject surveys. The pre-education survey indicated that nurses (30%) used the scripting almost all the time or all the time with each nurse-patient interaction. At the end of the project, the postproject survey resulted in 12 (92%) nurses who used the script almost all or all the time. At the end of the project the Healthcare Effectiveness Data and Information Set Patient Satisfaction Survey data indicated a 1% decrease in satisfaction with communication, a 9% increase in self-management support, and a 1% increase in care coordination. There was no statistical difference in the patient satisfaction scores over the project's 3-month time frame. Staff development training may lead to a positive social change for patient care as a result of nurses' understanding that interacting in a caring manner with patients makes a positive difference in patient satisfaction. The Texas health care system nurses realize that they can apply Watson's caring theory to guide nursing practice and to express caring for patients' physical and emotional needs during the outpatient appointments.

Begreppet tillit är något sjuksköterskan och patienten kommer i kontakt med varje dag omedvetet. Begreppet tillit definieras som människans spontana livsyttringar. Det finns brist på vetenskaplig kunskap om begreppet tillit inom vårdvetenskapen, nursing och caring, kunskap om begreppet kan bidra till möjligheten att stärka hälsan i det goda vårdandet. Syftet var att belysa hur begreppet tillit uttrycks inom vårdvetenskapen, nursing samt caring. Studien är byggd på en litteratursökning med en datainsamlingsmetod av vetenskapliga artiklar. Resultatet visar på hur tillit upplevs inom nursing och caring. Patienten upplevde högre tillit till sjuksköterskan när tydlig och relevant information gavs, när patienten kände sig värderad och bekräftad av sjuksköterskan, när sjuksköterskan visar god attityd. Brist på tillit gav en känsla av osäkerhet och var svår att återuppbygga. Begreppet tillit är relativt outforskat men har stor betydelse hos både patient och sjuksköterska, det stärker relationen vilket gör att sjuksköterskan känner förtroende för patienten och patienten känner högre trygghet för sjuksköterskan. Resultatet visar ett samband mellan sjuksköterskans attityd och den upplevda tilliten hos patienten. Begreppet tillit finns ej utvecklad som oberoende omvårdnadsåtgärd i litteratur och därför kan utökad forskning vara relevant för att utveckla nursing och caring. / The concept of trust is something nurse and patient come into contact with on a daily basis, without thinking about it. The concept of trust is defined as human’s spontaneous manifestations of life. There is a lack of scientific knowledge within care science, nursing and caring. Increasing the knowledge about the concept of trust can contribute to the possibility of enhancing health in a caring environment. The aim of the study was to illuminate how the concept of trust is expressed within care science, nursing and caring. The study is built on a literature research were data collection was based on scientific articles. The result shows how trust is experienced within nursing and caring. The patient felt valued and confirmed by the nurse and strengthened by the nurse’s good and respectful attitude. A lack of trust gave a feeling of insecurity. The concept of trust is relatively unexplored but is of huge importance to the nurse and patient. Trust strengthens the nurse-patient relation which creates safety and confidence. The result shows the connection between the nurse’s attitudes and the trust experienced by the patient. Trust is not developed as a nursing-care intervention in literature and further research is relevant.

Vulnerability is a phenomenon that with life changing events becomes more apparent. The nurse’s approach to their own and the patient’s vulnerability affects the caring relationship where all the feelings the patient is experiencing is considered. For the patient to dare express the feelings that vulnerability induces it is important not to disregard it. Caring tradition implies a deeper emotional connection between the patient and nurse, it fills an important and central part in caring for vulnerable patients. The aim of the study was to illuminate what vulnerability is within caring. The method was a literature study where systematical searches in different databases with care focus resulted in ten scientific result articles. With inspiration from content analysis the result articles were processed and analysed. The result clarifies experiences of vulnerability within caring through the categories: feelings, courage, alleviation, and burden-resource. It emerged that vulnerability occurs because of different reasons for the patient and nurse and how vulnerability is considered has impact on the emotional connection that is needed in a caring relationship. The conclusion from the literature study is that vulnerability is an important phenomenon to have knowledge of. It is relatively unexplored and demands further research with the purpose of exploring vulnerability in a caring context and thereby give the nurse better tools to be able to give best possible care based in scientific evidence. / Sårbarhetär ett fenomen som i samband med livsomställningar blir mer framträdande. Sjuksköterskans inställning till sin egen och patientens sårbarhet påverkar huruvida en omsorgsfull relation skapas där alla känslor patienten upplever beaktas. För att patienten ska våga uttrycka känslorna sårbarheten framkallar är det viktigt att sårbarhet inte förbises. Vårdande medcaringtraditionen i fokus som innebär en djupareemotionell anknytning mellan patient och sjuksköterskafyller därfören viktig och central del i omvårdnaden av sårbara patienter.Syftetvar att belysa vad sårbarhet är inomcaring.Metodför studien varallmän litteraturstudie där systematiska sökningar i databaser med omvårdnadfokus resulterade i tio vetenskapliga resultatartiklar. Med inspiration från innehållsanalys bearbetades samt analyserades de vetenskapliga resultatartiklarna.Resultatettydliggör upplevelser av sårbarhet inomcaringgenom kategorierna:känslor,mod,lindrandeochbörda-resurs. Det framkom att sårbarhet uppkommer av olika skäl för patienten och sjuksköterskan och hur sårbarheten beaktas har inverkan påden emotionella anknytningen som behövs i en vårdande relation.Slutsatsenfrån litteraturstudiens resultat är att sårbarhet är ett viktigt fenomen att ha kunskap om. Det är relativt obeforskat, därför rekommenderas fortsatt forskning med syfte att undersöka sårbarhet i en caring kontext för att ge sjuksköterskan bättre redskap att geen patientsäker och evidensbaserad omvårdnad.

Caring in nursing education

The question the researcher set out to answer in this research was: What does caring entail in nursing education from the point of view of both the tutor and the student? Essentially an ontological question, the answer to the question required theory generation rather than theory verification. This has been achieved through grounded theory research. A phenomenological dialectical philosophical foundation of anthropology this study formed the and symbolic interactionism was applied as methodological framework. Sampling was conducted in two phases: selective sampling for informants, and selective sampling of data and literature. Data were collected through formal unstructured qualitative interviews and were analysed through constant comparative analysis. Both structural themes and processes emerged. Based on the emergent theory it is concluded that in nursing education, caring is an extension of prosocial behaviours and caring as a means to an end cannot be separated from caring as an end in itself. / Health Studies / M.A. (Nursing Science)
