La signification des pratiques humanistes-caring de professeurs en formation infirmière et leurs contributions : perception de professeurs

Plusieurs écrits scientifiques ont mentionné que les pratiques humanistes-caring en formation infirmière sont avantageuses pour les personnes étudiantes. En effet, les pratiques humanistes-caring professorales créent les conditions qui favorisent les apprentissages. Des études ont rapporté la perspective de la clientèle étudiante témoignant des bienfaits des pratiques humanistes-caring de professeurs. Elles constituent une piste de solution pouvant réduire la déshumanisation dans les milieux académiques d’enseignement et d’apprentissages des soins infirmiers. Mais à la connaissance de l’étudiant-chercheur, aucune étude issue de la discipline infirmière ne s’est intéressée à la compréhension de la signification des pratiques humanistes-caring de professeurs, de même qu’aux contributions de celles-ci à l’égard de la personne étudiante. Inspirée par l’humanisme et le caring comme philosophie, le Modèle humaniste des soins infirmiers-Université de Montréal (Cara et al., 2016) a été retenu comme perspective disciplinaire de la présente étude. Cette dernière avait pour but d’explorer et comprendre, auprès de professeurs, la signification de leurs pratiques humanistes-caring, de même que leurs perceptions quant aux contributions de celles-ci au développement des apprentissages de la personne étudiante en formation infirmière. Pour ce faire, huit entrevues ont été réalisées auprès de professeurs dans une université québécoise. Les données recueillies ont été analysées à l’aide de la méthode phénoménologique de Giorgi (1997). Le processus d’analyse a permis l’émergence de sept thèmes. Parmi ces thèmes, quatre se rapportent à la signification des pratiques humanistes-caring de professeurs (première question de recherche), à savoir : 1) valeurs humanistes-caring comme fondement du rôle professoral, 2) attitudes humanistes-caring de professeurs envers la personne étudiante, 3) comportements humanistes-caring de professeurs envers la personne étudiante et, 4) conditions contextuelles des pratiques humanistes-caring des professeurs. Finalement, les trois derniers thèmes documentent la contribution des pratiques humanistes-caring à l’égard de la personne étudiante (deuxième question de recherche), à savoir : 1) épanouissement de la personne étudiante, 2) succès académique de la personne étudiante, 3) sentiment d’accomplissement professoral. L’ensemble des sept thèmes a favorisé l’émergence de la structure essentielle du phénomène investigué qui s’intitule « influencées par des conditions contextuelles, les pratiques professorales empreintes de valeurs, d’attitudes et de comportements humanistes-caring favorisent l’épanouissement et le succès académique de la personne étudiante, de même que l’accomplissement professoral ». La présente étude s’est révélée innovante puisque les résultats constituent de précieuses données pour les connaissances en sciences infirmières. Ces résultats peuvent être utilisés pour créer ou maintenir un environnement favorable aux enseignements et aux apprentissages dans les milieux de formation infirmière. De même, les milieux cliniques de soins peuvent les utiliser en vue d’améliorer la qualité du suivi auprès des personnes étudiantes en stage. Enfin, ces résultats peuvent susciter l’intérêt de réaliser d’autres recherches subséquentes et ce, dans d’autres milieux académiques en formation infirmière. In fact, humanistic-caring professorial practices create conditions that promote learning. Studies have reported on the benefits of professors’ humanistic-caring practices from the perspective of students. They constitute a possible solution that can reduce dehumanization in academic nursing teaching and learning environments. However, to the best of the student-researcher's knowledge, no study within the nursing discipline has focused on understanding the professors’ meaning of their humanistic-caring practices, as well as their contributions for students. Inspired by humanistic and caring philosophies, the Humanistic Model of Nursing Care-Université de Montréal (Cara et al., 2016) was chosen as the disciplinary perspective for this study. The purpose of this research was to explore and understand professors’ meaning of their humanistic-caring practices, as well as their perceptions of the contributions of these practices to the development of nursing students’ learning. To this end, eight interviews were conducted with professors working in a Quebec university. The data collected were analyzed using Giorgi’s (1997) phenomenological method. The analysis process revealed seven themes. Four of these themes relate to the meaning of professors’ humanistic-caring practices (first research question), namely: 1) humanistic-caring values as the foundation of the professorial role, 2) professors’ humanistic-caring attitudes towards the student, 3) professors’ humanistic-caring behaviours towards the student and, 4) contextual conditions of professors’ humanistic-caring practices. The other three themes correspond to the contribution of humanistic-caring practices towards the student (second research question), explicitly: 1) student self-fulfillment, 2) student academic success and, finally, 3) sense of professorial accomplishment. Together, the seven themes led to the emergence of the essential structure of the investigated phenomenon: "Influenced by contextual conditions, professorial practices imbued with humanistic-caring values, attitudes and behaviours foster students’ self-fulfillment and academic success, as well as professorial accomplishment". The present study proved to be innovative since its results constitute valuable data for nursing knowledge. These results can be used to create or maintain an environment conducive to teaching and learning in nursing training environments. Clinical care settings can use them to improve the quality of the follow-up offered to students during their internship. Finally, these results may spark the interest in carrying out subsequent research, elsewhere in other academic settings in nursing education. Navigating Layers of Care and Attention in Education : A comparative Analysis of Noddings and Weil in "The kindergarten Teacher" Sara Colangelo's Film(2018)

This thesis focuses on the issue of attention based on Noddings and Weil's perspectives. Inaddition, it describes a fictional example for attention. The fictional example critically examinesthe benefits and potential risks of care and attention in modern education. It uses the 2018 film"The Kindergarten Teacher" as an example to explore the ethical boundaries of providing care andattention. The film revolves around a teacher named Lisa Spinelli, who becomes deeply involvedin nurturing a student's exceptional poetic talent. I examine Lisa's behavior using Nel Noddings'theory of care and Simone Weil's notion of attention. Applying these frameworks aims to assessLisa's motivations and actions and gain insight into her conduct.Noddings emphasizes that attention and caring develop in a relationship is a continuous process,not just a single moment. For the relationship to be strong, it must involve mutual responses fromboth parties. When a strong relationship with attention is established, it aids in the learning processespecially for students and children. However, for this relationship to be effective, it needs to bedeep and trustworthy, with responsive interactions. Noddings also suggests that "modeling","dialogue", "practice", and "confirmation" are essential components that teachers and parents canuse to enhance their students' or children's attention within their relationships.Simone Weil's conception of attention is selfless and others-focused. It involves putting asidepersonal goals to meet the needs of others. Weil introduces "impersonality" through solitude,where individuals suspend their sense of self for the collective. True attention requires thedisappearance of self and focuses beyond comprehension, like an act of prayer. Love and desireare crucial, fostering a willingness to be attentive and forget oneself. Weil explores attention's rolein pursuing truth, beauty, and goodness. It is crucial for making supreme efforts and should be theprimary focus of education. She emphasizes observing visual representations withoutinterpretation to gain understanding. Maintaining attention amidst conflicting desires andobligations is challenging. Weil suggests enduring and resisting negative desires while focusing onthe good and virtuous. Individuals can elevate the quality of their attention by detaching fromtemporal concerns. Ultimately, Weil presents attention as a transformative practice that leads tolove, genuine values, and a deeper understanding of the world.

Vårdande och icke-vårdande tröst / Caring and non-caring consolation

Syftet med avhandlingen är att, ur ett vårdvetenskapligt, och ett i detta inkluderat vårdteologiskt perspektiv, belysa innebörden av fenomenet tröst och hur tröst förhåller sig till lidande och vård. Det görs i två empiriska intervjustudier med vårdare och äldre vårdtagare samt en tredje delstudie som analyserar tröst såsom den framstår i Jobs bok i Gamla testamentet. Dessa delstudier behandlar vårdares upplevelse av tröst och att trösta, äldre vårdtagares upplevelse av tröst samt Jobs upplevelse av tröst. Metodologiskt har forskningen sin utgångspunkt i fenomenologi och hermeneutik. En fenomenologisk-hermeneutisk metod, inspirerad av Paul Ricoeur, har använts för textanalyserna. I avhandlingen behandlas också det som är signifikant för poetisk och religiös text. De metaforer som förekommer i de empiriska delstudierna med vårdare och äldre analyseras för att ta tillvara det överskott av mening som metaforer, enligt Ricoeur, kan uttrycka. Resultatet visar fem övergripande innebörder: Den motsägelsefulla trösten, Den sammanbindande trösten, Den stumma trösten, Den okontrollerade trösten och Den vilsamma trösten. En vårdande tröst är motsägelsefull såtillvida att den innebär dels att den lidande överlämnar sitt lidande till någon annan, dels att lidandet kan ges tillbaka för att lidas. Tröst kan således vara att lida. En sammanbindande tröst är närvarande, d.v.s. är hos den lidande och utgår från dennes lidande. Denna tröst kännetecknas av djup gemenskap, en upplevelse av att vara förstådd på ett djupare sätt. Resultatet visar också en tröst som är stum. Denna tröst svarar inte an på den lidandes upplevelse av sitt lidande, är oformlig och saknar följsamhet för lidandet. Ett exempel på en stum och icke-vårdande tröst är vännernas tröst i Jobs bok. Denna tröst förmår inte trösta därför att den inte är där Job är, d.v.s. i hans upplevelse av sitt lidande. Vidare framstår en vårdande tröst som okontrollerad därför att den dels är spontan, dels bistår den lidande att släppa kontrollen över lidandet. Att släppa kontrollen innebär bl.a. att den lidande ger upp försöken att förstå lidandet för att istället låta det oförståeliga vara oförståeligt. Genom att ge upp det som på olika sätt binder lidandet infinner sig en tröstande och hälsobringande vila i eller från lidandekampen. Resultatet som helhet tolkas ur ett vårdvetenskapligt perspektiv som vårdgemenskap, tro, hälsa och offer. Tröst som hälsa förstås utifrån en teoretisk modell, inspirerad av Katie Erikssons ontologiska hälsomodell. Filosofiskt-etiskt belyses forskningen utifrån företrädesvis Emmanuel Levinas. Resultatet diskuteras i förhållande till tidigare forskning och i förhållande till vårdvetenskap, samhälle och vård. / The aim of the thesis is, from a caring science perspective including a caring theology perspective, to illustrate the meaning of the phenomenon consolation and how consolation relates to suffering and care. Two studies were completed where staff and elderly care receivers were interviewed and a third study focused on an analysis of consolation as it is presented in the Book of Job in the Old Testament. These studies deal with carers’ experiences of consolation and consoling, elderly care receivers’ experiences of consolation, and Job’s experience of consolation. Phenomenology and hermeneutics form the basis for the methodological approach. A phenomenological-hermeneutic method, inspired by Paul Ricoeur, has been used for the text analyses. The thesis also covers significant aspects of poetical and religious texts. The metaphors that occur in the interview studies with the carers and the elderly are analysed in order to take care of the excess of meaning that,according to Ricoeur, can be expressed in metaphors. The result show five overall meanings: The contradictory consolation, The bonding consolation, The mute and rigid consolation, The uncontrolled consolation and The restful consolation. A caring consolation is contradictory in the sense that it entails that the sufferer on the one hand passes on his or her suffering to someone else and on the other hand that the suffering can be returned to be suffered. Consolation can thus entail suffering. The bonding consolation is present, i.e. is with the sufferer and is based on that person’s suffering. This consolation is characterised by a close fellowship, a feeling of being understood at a deeper level. The results also reveal a consolation that is mute and rigid. This consolation does not respond to the sufferer’s experience of his or her suffering, is shapeless and therefore unable to follow the suffering. An example of a mute, rigid and non-caring consolation is the consolation of the friends in the Book of Job. This consolation is not capable of consoling because it does not correspond to where Job is, i.e. in his experience of his suffering. A caring consolation is also uncontrolled because it is on the one hand spontaneous and on the other hand helps the sufferer to lose control over the suffering. To lose control entails, amongst other things, the sufferer giving up trying to understand suffering and instead lets that which is incomprehensible be incomprehensible. A consoling and health-bringing rest in or from the struggle with suffering presents itself by giving up what in various ways is tied to the suffering. The result as a whole is interpreted from a caring science perspective with the following important concepts: caring relationship, faith, health and sacrifice. Consolation as health is considered on the basis of a theoretical model inspired by Katie Eriksson’s ontological health model. The research is also illustrated from a philosophical-ethical perspective, mainly based on the work of Emmanuel Levinas. Background: A shortage of nurses is experienced in aged care as these nurses experience adverse working conditions. Resilience might empower these nurses to survive, thrive and even flourish. A paucity of research exists regarding resilience in nurses caring for older persons. Objectives: The purpose of this research was to investigate the level of resilience in nurses caring for older persons, and to explore and describe their strengths and coping abilities, in order to formulate recommendations to strengthen resilience in nurses caring for older persons. Method: An explorative, descriptive design with multiple phases was used. An all-inclusive sample of nurses caring for older persons in an urban setting in the North West Province was used. During phase one (sample size n=43) the level of resilience, demographic information and narratives were obtained. During phase two (sample size n=17) focus group interviews were conducted. The quantitative data was analysed using descriptive statistics and the qualitative data using content analysis. Results: The participants had a moderately high to high level of resilience. Participants did experience adverse working circumstances and they needed resilience due to a need for balance, the emotional nature of the work, work ethics and the work environment. Nurses caring for older persons use personal, professional, contextual and spiritual strengths to handle adverse working conditions. Conclusion: Recommendations to strengthen resilience in nurses caring for older persons were formulated in phase three of the research, focusing on strengthening nurses‟ personal, professional, contextual and spiritual strengths in order that they can handle adverse workplace conditions. / MCur, North-West University, Potchefstroom Campus, 2014

Att drabbas av schizofreni kan leda till konsekvenser som innebär att upplevelsen av livskvalitet drastiskt förändras. Syftet med studien var att belysa faktorer som påverkar upplevelsen av livskvalitet bland personer som drabbats av schizofreni, och som vårdas inom en öppenvård. Metoden var en kvalitativ litteraturstudie med sju vetenskapliga artiklar. Datamaterialet analyserades och granskades med hjälp av en manifest innehållsanalys. Resultatet visade på fyra faktorer som kan påverka den drabbades upplevelse av livskvalitet: Sjukdomens uttryck och behandling, Stigma, Social förlust och Stöd. Många av sjukdomens symtom är skrämmande, obehagliga och svåra för den drabbade att hantera själv. Samhällets stigmatiserande syn på psykiskt sjuka orsakar ett stort lidande för de drabbade. Förlust av sociala relationer är en av sjukdomens många konsekvenser som ofta leder till ensamhet och isolering. För att kunna acceptera och hantera sjukdomen behöver personer drabbade av schizofreni ett stort stöd från både vårdpersonal och andra personer i deras omgivning. Resultatet ger vägledning, för vårdpersonal, om faktorer som kan påverka livskvaliteten bland personer drabbade av schizofreni.

Livskvalitet är en subjektiv upplevelse och det är av stor vikt att sjuksköterskor är medvetna om patienters individuella upplevelse av livskvalitet. En litteraturgenomgång visar att befintlig vetenskaplig litteratur inom humanvetenskapen och vårdvetenskapen är otillräcklig i upplevelsen av jaget i relation till livskvalitet i vårdandet. Syftet med studien är att utforska begreppet livskvalitet i vårdandet. Studien genomfördes som en litteraturstudie med åtta vetenskapliga artiklar, där alla vetenskapliga artiklar var kvalitativa och belyste livskvalitet i vårdandet. I resultatet framkom det ett övergripande teman frid och frihet, med två huvudkategorier livskvalitet – jagets mening samt livskvalitet – jagets livsmening. Resultatet visar att livskvalitet i vårdandet är en omfattande del i att uppnå frid och frihet. Viktiga aspekter för att uppleva livskvalitet i vårdandet är mellanmänskliga relationer, roller, identitet, autonomi, tillfredsställelse och meningsfullhet. Utan någon eller flera av ovanstående aspekter har människan närmare till avsaknad av livskvalitet. Essensen i livskvalitet är att det är en subjektiv och individuell upplevelse som behöver belysas i samtal mellan sjuksköterskor och patienter för att gynna det goda vårdandet. För att sjuksköterskor skall kunna ordinera oberoende omvårdnadsåtgärder i relation till livskvalitet krävs evidensbaserad kunskap om patienters subjektiva upplevelse av livskvalitet

<p><strong>Bakgrund: </strong>Inom hälso- och sjukvården finns två viktiga budskap för en god och säker vård, det ena är att vård och behandling ska ske i samverkan mellan vårdare och patient och det andra är att sjuksköterskan ska utforma individuella vårdplaner. Studier visar att individuella vårdplaner i hälso- och sjukvård i princip är obefintliga och att sjuksköterskor istället utformar standardvårdplaner. <strong>Problem: </strong>Risken att arbeta med standardvårdplaner är att patienternas delaktighet i vården förbises eftersom standardvårdplaner utformas i förväg genom förväntade omvårdnadsproblem, till skillnad från individuella vårdplaner som utarbetas i samverkan med patienten. <strong>Syfte: </strong>Examensarbetets syfte var att beskriva på vilket vis patientens delaktighet framkom i omvårdnadsjournalen vid användning av standardvårdplaner, vid vård av patienter med instabil kranskärlssjukdom. <strong>Metod: </strong>En kvalitativ, beskrivande forskningsmetod användes för att undersöka sjuksköterskans omvårdnadsdokumentation av tretton omvårdnadsjournaler. Text som innehöll någon form av dokumenterad delaktighet i sjuksköterskans omvårdnadsdokumentation analyserades utifrån manifest innehållsanalys som till slut även analyserades genom latent innehållsanalys. <strong>Resultat: </strong>I omvårdnadsjournalerna, vid användning av standardvårdplaner framkommer patienternas delaktighet genom kategorier sjukdomsberättelsen, kroppsfunktion, självuppfattade problem, utanförskap och självbestämmande. Kategorierna visade mönster som urskilde tema passivt och aktivt deltagande där det passiva deltagandet i omvårdnadsjournalerna framträdde som starkast.</p>

The primary purpose of this study was to describe relationships between the concepts of caring behaviors of nurses, adolescent hopefulness and adolescent health care outcomes. The secondary purpose was to test and refine instruments developed to index those concepts. The conceptual framework used for the study was derived from two sources: an existential theory of nursing, Humanistic Nursing, as developed by Paterson and Zderad (1976). and a set of inductive studies on nurse-adolescent patient interactions (Hinds, 1983). A longitudinal, descriptive-correlational design having three data collection points was used. The study sample consisted of 25 adolescents (x age 15.6 years) receiving inpatient treatment for substance abuse. The data collection points occurred at 24-48 hours after admission (T₁), 96-120 hours before discharge (T₂), and 4-5 weeks after discharge (T₃). Adolescents completed visual analogue instruments and responded to a set of open-ended questions indexing the study concepts. Instrumentation data were analyzed using descriptive and correlational statistics and tests of significance for change. Qualitative data resu1ting from the interviews were content analyzed. Findings included statistically significant positive relationships between the concepts of caring behaviors of nurses and adolescent hopefulness at T₁, and T₂. Secondly, the relationship between adolescent hopefulness and adolescent health care outcomes was statistically significant at T₃. The concepts of caring behaviors of nurses and adolescent hopefulness changed significantly in a positive direction from T₁ to T₂. Content analysis findings indicated the concept of adolescent hopefulness was qualitatively different at each of the data collection points. Findings provide support for the theorized link between nurse-patient relationships and positive patient change. The purposeful use of a caring self for positive patient change represents the blend of art and science that defines the nursing profession.
