L'actigraphie comme mesure de sommeil à l’âge préscolaire : liens avec la sécurité d’attachement et les comportements extériorisés

L’objectif général de la thèse était d’élargir les connaissances scientifiques sur le sommeil des enfants. La thèse est composée de quatre articles empiriques. Le premier visait à estimer la validité de l’actigraphie comme mesure de sommeil chez les enfants d’âge préscolaire en la comparant à la polysomnographie, et à examiner si son emplacement influence sa validité. 12 enfants âgés de 2 à 5 ans ont porté simultanément un actigraphe à la cheville et un au poignet pendant une nuit d’enregistrement polysomnographique. Les résultats démontrent que l’actigraphie permet une bonne détection du sommeil, mais qu’elle détecte moins bien l’éveil. Cet article suggère également que les jeunes enfants nécessitent un algorithme adapté à leur niveau d’activité nocturne. Enfin, la validité de l'actigraphie semble similaire pour le poignet et la cheville. Le deuxième article visait à comparer trois mesures de sommeil souvent utilisées avec de jeunes enfants, soit les agendas de sommeil, l’échelle des problèmes de sommeil du Child Behavior Checklist (CBCL) et l’actigraphie, afin de déterminer leurs similarités et leurs divergences quant aux variables de sommeil qui en sont dérivées. 80 familles ont participé à cette étude lorsque les enfants étaient âgés de 2 ans. Les enfants ont porté un actigraphe durant 72 heures consécutives et les mères ont complété un agenda de sommeil durant cette même période. Les deux parents ont aussi rempli le CBCL. Les résultats démontrent que ces mesures de sommeil évaluent des aspects différents du sommeil de l’enfant, et suggèrent une concordance particulièrement faible entre les mesures subjectives et objectives. Le troisième article visait à évaluer l’apport unique de la sécurité d’attachement dans la prédiction du sommeil de l’enfant. 62 dyades mère-enfant ont été rencontrées à deux reprises. La sécurité d’attachement et la dépendance ont été évaluées par observation à l’aide du Q-Sort d’attachement lorsque l’enfant avait 15 mois. À l’âge de 2 ans, les enfants ont porté un actigraphe durant 3 jours consécutifs. Les résultats indiquent que la sécurité d'attachement a une contribution unique à la prédiction de la durée du sommeil nocturne et de l'efficacité du sommeil nocturne. Ainsi, cette étude suggère que plus les enfants ont un attachement sécurisant envers leur mère, plus grandes sont la durée et la qualité de leur sommeil quelques mois plus tard. Le quatrième article visait à examiner la relation entre le sommeil et les comportements extériorisés. 64 familles ont participé à cette étude. À l’âge de 2 ans, les enfants ont porté un actigraphe durant 72 heures consécutives et les parents ont complété le CBCL. Lorsque les enfants étaient âgés de 4 ans, les parents ainsi que l’éducateur(trice) de garderie ont rempli le CBCL. Les résultats démontrent que le sommeil de l’enfant est associé aux comportements extériorisés concomitants et à l’augmentation de ceux-ci à travers le temps. Par ailleurs, les relations entre la qualité de sommeil et les comportements extériorisés étaient modérées par le sexe de l’enfant, c’est-à-dire significatives seulement chez les garçons. Les résultats des quatre articles sont finalement intégrés dans la discussion générale. / The overall objective of the thesis was to contribute to the expansion of the current state of knowledge on young children’s sleep. The thesis consists of four empirical articles. The first article compared the validity of three algorithms for detecting sleep with actigraphy by comparing them to polysomnography in preschoolers. The putative influence of device location (wrist or ankle) was also examined. 12 children aged 2 to 5 years simultaneously wore an actigraph on an ankle and a wrist during a night of polysomnography recording at home. The results showed that actigraphy generally provides good detection of sleep, but weaker detection of wakefulness. The article also suggests that young children require an algorithm adapted to their level of nocturnal motor activity. Finally, sleep variables derived from the ankle and wrist were not statistically different, suggesting that the validity of the actigraphy is not influenced by the location of the monitor. The second article examined associations between actigraphy, maternal sleep diaries, and the parent-completed Child Behavior Checklist (CBCL) sleep items, which are instruments widely used by clinicians and researchers to assess sleep among young children. 80 middle-class families participated in this study when children were aged 2 years. Children wore an actigraph monitor for a 72-hour period and mothers completed a sleep diary during the same period. Both parents also completed the CBCL. The results show that these sleep measures assess different aspects of children’s sleep, and suggest a particularly low concordance between the subjective and objective sleep measures. The third article used objective measures of sleep and attachment to assess the longitudinal links between mother-child attachment security and subsequent sleep, while partialling out child dependency. 62 middle-class families were met twice. Security and dependency of attachment were assessed with the observer version of the Attachment Q-Sort when the child was 15 months. At the age of 2 years, children wore an actigraph monitor for a 72-hour period. Results indicated that children more securely attached to their mothers had higher nighttime sleep duration and efficiency, and these predictions were not confounded by child dependency. The aim of the fourth article was to investigate the concurrent and longitudinal relations between sleep and externalizing symptoms among young children. Sixty-four families were met twice. At the age of 2 years, children wore an actigraph monitor for a 72- hour period and both mothers and fathers completed the CBCL. When children were aged 4 years, both parents as well as the daycare educator filled the CBCL. The results show that child sleep is associated with concurrent externalizing symptoms and with an increase in externalizing symptoms over time. Associations between sleep efficiency and externalizing symptoms were moderated by child sex, such that links were only significant among boys. Finally, the results of the four articles are integrated into the general discussion.

Title: Parents and Teachers’ Perceptions and Clinical Diagnosis of Autism Among White and Non-White Groups

The pervasiveness of autism has significantly increased over the past 2 decades with the 2014 Center for Disease Control and Prevention report indicating 1 in 68 children are diagnosed with Autism Spectrum Disorder (ASD). Early intervention is recommended as the most effective treatment approach. Nevertheless, previous research has indicated that White children are diagnosed with ASD about 1.5 years earlier than are Non-White children. A current gap remains in literature regarding ASD and different racial groups, and evidence has been inconclusive regarding disparities in identifying and diagnosing ASD. To fill this gap, this study investigated the relationship between child race, parents and teachers’ perceptions, and diagnosis of ASD among White and Non-White groups. The theoretical framework was the critical race theory. Archival data from the Psychological and School Services of Eastern Carolina included 48 preschool children from White (18) and Non-White (30) groups. The data’s variables of race, perceptions, and diagnosis were analyzed using multivariate analysis of variance. Results indicated a higher rate of diagnosis of ASD among the White group compared to the Non-White group. Yet, teachers’ perceptions of ASD were higher for the Non-White group, while parents’ perceptions of ASD were lower for the Non-White group. This finding confirms the nuances of ASD among racial groups which could promote efforts to better educate parents and teachers on developmental milestones, explore families’ unique beliefs, and emphasize the importance of accurate early detection. Also, considerations of culturally sensitive screening, diagnostic measures, protocols, and practices may be embraced to safeguard that children, regardless of race, receive timely and competent care.

Social-emotional and behavioral development problems in 1 to 2-year-old children in Northern Finland:reports of mothers, fathers and healthcare professionals

Abstract Background and aims: Growing evidence supports the existence of clinically significant problems in social-emotional/behavioral (SEB) development among infants and toddlers and the importance of early identification of these problems. There is a lack of research on the occurrence and identification of problems in the SEB domain among the Finnish general population of 1 to 2-year-old children. The present study examined these important issues. Given the dearth of earlier research knowledge, particular emphasis was focused on analyzing possible moderating effects of the assessed child’s and informant’s gender on the results. Subjects and methods: Oulu toddler (N = 208, age 18 months), Oulu infant (N = 227, age 12 months), and Oulu Province (N = 1008, age 12 months) samples were collected during 2008–2013 in collaboration with child health centers. Both mothers and fathers completed questionnaires assessing their child´s SEB development (e.g. the Brief Infant-Toddler Social and Emotional Assessment; Briggs-Gowan & Carter, 2006) and the family’s sociodemographic characteristics. Child healthcare nurse (CHCN) worry reports concerning the children’s development and family well-being were also gathered. Main results and conclusions: In parental ratings, girls obtained higher SEB competence scores than boys, whereas boys got higher SEB problem scores than girls, particularly among the toddler sample. Thus, boys may have an elevated risk of parent-reported problem behaviors and delays in SEB competences even before the age of 2 years. Compared to fathers, mothers were prone to rate the toddlers higher in both SEB competences and problems, especially with regard to externalizing behavior problems and problems in boys. Elevated interparental rating differences were associated with relatively elevated maternal parenting stress. Therefore, it is recommended to gather data on young children’s SEB development from both parents when possible and to focus clinical attention on the issue if the parental reports differ significantly. Parents and CHCNs seldom recognized SEB problems and delays in competency as problematical in 1-year-old infants, even in the case of infants who were screened to be in the of-concern range on the age-appropriate measure of SEB development. Thus, further efforts are needed for developing the identification of early problems in SEB development by both parents and preventive child healthcare professionals in Finland. / Tiivistelmä Tausta ja tavoitteet: Kasvava näyttö tukee käsitystä, että kliinisesti merkittäviä sosioemotionaalisen ja käyttäytymisen (SEK) kehityksen ongelmia esiintyy jo vauva- ja taaperoikäisillä ja että näiden ongelmien varhainen tunnistaminen on tärkeää. SEK-kehityksen ongelmien esiintymisestä ja tunnistamisesta 1–2-vuotaiden väestössä Suomessa puuttuu tutkimustietoa. Tämä tutkimus arvioi näitä tärkeitä aiheita. Tutkimuksessa huomioitiin erityisesti sekä tutkittavan lapsen että arvioitsijan sukupuolen mahdollinen moderoiva vaikutus tuloksiin, koska aiempaa tutkimustietoa tästä näkökulmasta on vähän. Tutkittavat ja menetelmät: Oulu taapero (N = 208, ikä 18 kk), Oulu vauva (N = 227, ikä 12 kk) ja Oulun lääni (N = 1008, ikä 12 kk) -aineistot kerättiin vuosina 2008–2013 yhteistyössä lastenneuvoloiden kanssa. Sekä äidit että isät täyttivät lomakkeet, jotka arvioivat lapsen SEK-kehitystä (mm. the Brief Infant-Toddler Social and Emotional Assessment; Briggs-Gowan & Carter, 2006) ja perheen sosiodemografisia taustatekijöitä. Myös terveydenhoitajan raportit lapsen kehitykseen ja perheen hyvinvointiin liittyvistä huolista kerättiin. Päätulokset ja johtopäätökset: Vanhempien arvioimina tytöt saivat korkeampia pistemääriä SEK-taidoissa kuin pojat, kun taas pojat saivat korkeampia pistemääriä SEK-ongelmissa kuin tytöt, etenkin taaperoaineistossa. Pojilla saattaakin olla kohonnut riski vanhempien raportoimille käytösongelmille ja SEK-kehityksen viiveille jopa ennen 2 vuoden ikää. Isiin verrattuina äidit olivat taipuvaisia antamaan korkeampia pisteitä sekä SEK-taito- että -ongelmaosioissa taaperoikäisten aineistossa, etenkin koskien ulospäin suuntautuvia käytösongelmia ja poikien ongelmia. Suurentuneet eroavaisuudet vanhempien antamissa arvioissa olivat yhteydessä suhteellisesti kohonneeseen vanhemmuuden stressiin äideillä. Onkin suositeltavaa kerätä tietoa pienen lapsen SEK-kehityksestä kummaltakin vanhemmalta, jos mahdollista, ja kiinnittää kliinistä huomiota asiaan, jos vanhempien antamat tiedot eriävät merkittävästi. Vanhemmat ja terveydenhoitajat tunnistivat SEK-ongelmia ja taitojen puutteita harvoin huolenaiheiksi 1-vuotiailla, vaikka nämä vauvat olisivat seuloutuneet huolialueelle ikätasoon soveltuvalla SEK-kehityksen mittarilla. Tarvitaankin lisätoimia, jotta vanhemmat ja terveydenhuollon ammattilaiset oppivat paremmin tunnistamaan varhaisia SEK-kehityksen ongelmia Suomessa.

This study aimed to assess the existence and estimate the prevalence of mental disorders in children and adolescents living in an isolated rural community of the Kalunga quilombola ethnic group (descendants of African runaway enslaved individuals), located in the northeastern part of the state of Goiás, Brazil. In this cross-sectional, descriptive, quantitative study a sample of 204 Kalunga children and adolescents was evaluated based on the responses of their parents/guardians and teachers to the mental health problems and behavior screening instruments Child Behavior Checklist for ages 6–18 (CBCL/6–18) and Teacher’s Report Form for ages 6–18 (TRF/6–18), respectively. The prevalence of mental health problems in Kalunga children and adolescents was 31.4% using the CBCL/6–18 and 21.1% using the TRF/6–18 (p < 0.01). Differences were found between genders, i.e., girls were more affected by internalizing problems than boys. The prevalence of mental health problems found in the present research was high compared to similar studies around the world. / Os transtornos mentais são bastante comuns em crianças e adolescentes e repercutem negativamente em sua vida familiar, escolar e social, geralmente persistindo por toda a vida desses indivíduos se não forem detectados e tratados. Este estudo teve como objetivos avaliar a existência e estimar a prevalência de transtornos mentais em crianças e adolescentes que vivem em uma comunidade rural isolada do grupo quilombola Kalunga (descendentes de escravos africanos fugitivos), localizada na parte nordeste do estado de Goiás, Brasil. Neste estudo transversal, descritivo e quantitativo, uma amostra de 204 crianças e adolescentes Kalungas foi avaliada com base nas respostas dos pais/responsáveis e professores aos instrumentos de rastreamento de problemas de saúde mental e comportamento denominados Child Behavior Checklist for ages 6–18 (CBCL/6–18) e Teacher’s Report Form for ages 6–18 (TRF/6–18), respectivamente. A prevalência de problemas de saúde mental em crianças e adolescentes Kalunga foi de 31,4% usando o CBCL/6–18 e de 21,1% usando o TRF/6–18 (p < 0,01). Foram encontradas diferenças entre os gêneros, pois as meninas apresentaram mais transtornos internalizantes do que os meninos. A prevalência de problemas de saúde mental encontrada na presente pesquisa foi elevada em comparação com estudos semelhantes em todo o mundo.
