Relationship between stunting in children 6 to 36 months of age and maternal employment status in Peru: A sub-analysis of the Peruvian Demographic and Health Survey

Objectives This study aimed to determine the relationship between stunting in children 6 to 36 months old and maternal employment status in Peru. Methods A secondary data analysis was conducted using information from the Demographic and Health Survey (DHS) in Peru. We used a representative sample of 4637 mother-child binomials to determine the association between stunting in children 6 to 36 months of age and the employment status of their mothers. Results The prevalence of stunting among children was 15.9% (95% CI: 13.9–16.7). The prevalence of working mothers was 63.7%. No association was found between maternal employment status and the presence of stunting in children [prevalence ratio (PR) = 1.04; 95% confidence interval (95% CI): 0.9 to 1.2; p = 0.627). However, on multivariate analysis we found that the prevalence of stunting was significantly higher among children of mothers performing unpaid work (12.4%) (PR = 1.38; 95% CI: 1.2–1.6; p < 0.001) compared with those of paid working mothers. Conclusion No significant association was found between maternal employment status and the presence of stunting in children 6 to 36 months of age. However, children of mothers doing unpaid work are at higher risk of stunting. These findings support the implementation of educational programs and labour policies to reduce the prevalence of stunting among children. © 2019 Chávez-Zárate et al. Métodos: Oitenta pacientes femininas com fibromialgia (FM), de 18 a 65 anos de idade foram randomizadas para o grupo dança (n=40) ou grupo controle (n=40). As pacientes do grupo de dança realizaram por 16 semanas aulas de dança do ventre duas vezes por semana e as pacientes do grupo controle ficaram na lista de espera. Foram utilizados escala analógica de dor (EVA), teste de caminhada de 6 minutos, Fibromyalgia Impact Questionnaire (FIQ), Medical Outcome Survey Short Form 36 (SF-36), Inventário de Beck, Inventário de Ansiedade traço-estado (IDATE), Questionário Body Dysorphic Disorder Examination (BDDE), as avaliações foi realizada no baseline, após16 semanas e 32 semanas. Na análise estatística foram inclusos todos os pacientes. Resultados: O grupo de dança apresentou resultados estatisticamente melhores para EVA de dor (p<0,001), teste de caminhada (p<0,001), FIQ (p=0,003), no SF-36 nos domínios dor (p<0,001), aspectos emocionais (p<0,003), saúde mental (p<0,021) e BDDE (p<0,009). Conclusão: A dança do ventre pode ser utilizada no tratamento para fibromialgia, por reduzir a dor, melhorar a capacidade funcional, qualidade de vida e imagem corporal destas pacientes. / Objective: The aim of the present study was to assess the effectiveness of belly dance as a treatment for pain and improving the quality of life of patients with fibromyalgia. Methods: Eighty female patients with fibromyalgia between 18 to 65 years were randomly allocated to a dance group (n=40) and control group (n=40). Patients in the dance group underwent 16 weeks of belly dance twice a week, while the patients in the control group remained on a waiting list. Patients were evaluated with regard to pain (VAS), function (6MWT), quality of life (FIQ and SF-36), depression (Beck Inventory), anxiety (STAI) and self-image (BDDE) questionnaire. Evaluations were carried out at baseline, 16 weeks and 32 weeks. Results: The dance group achieved significant improvements when compared to the control group in VAS for pain (p<0.001), six-minute walk test (p<0.001), FIQ (p=0.003), BDDE (p<0.009) as well as the pain (p<0.001), emotional aspects (p<0.003) and mental health (p<0.021) domains of the SF-36. Conclusion: Belly dance can be used in the treatment of fibromyalgia to reduce pain and improve function capacity, quality of life and self-image. / TEDE / BV UNIFESP: Teses e dissertações

Older adults with disability in extreme poverty in Peru: How is their access to health care?

Background Disability rates increase with age. In 2012, Peruvian older adults ( 65 years) represented 9% of the population. Additionally, older population reported disabilities at about 5 times the rate of Peruvians between 36 and 64 years old, and 30% of older population lived in poverty. Peruvian seniors living in extreme poverty experience disabilities and the extent of their access to healthcare is unknown. Objective This study assesses associations between disability and access to healthcare among Peruvians older individuals living in extreme poverty. Methods Secondary analysis of a national representative population based survey that utilizes information from Peru’s 2012 survey Health and Wellbeing in Older Adults (ESBAM), which includes older adults living in extreme poverty. We define disability in terms of the Activities of Daily Living (ADL disability) framework. Healthcare access was assessed as having any of Peru’s available health insurance schemes combined with preventive health services (vision assessment, influenza vaccination, blood pressure assessment, diabetes screening, and cholesterol assessment). Poisson robust regression models were used to evaluate the associations among relevant variables. Prevalence Ratios and 95% confidence intervals (95%CI) were reported. Results Data from 3869 individuals (65 to 80 years old), of whom 1760 (45.5%) were females, were analyzed. The prevalence of ADL disability was 17.3% (95%CI: 16.0%-18.4%). In addition, more than 60% had never received any of the preventive measures evaluated, except for the blood pressure assessment. In the adjusted model, people with ADL disability had 63% less probability of having extensive insurance, compared to those without disability (p<0.05). Conclusions This study shows that this Peruvian older population living in extreme poverty has limited access to healthcare services. Although there was no consistent association between ADL disability and the healthcare access, there is an urgent need to reduce the inequitable access to healthcare of this poor Peruvian older population. / Wellcome Trust / Revisión por pares

Quality of life refers to the way in which the person perceives their daily experience, considering in this process the social and cultural component. This implies that to know the perception of the quality of life it is necessary to consider in its measurement the scope of physical, psychological, social, environmental and personal health. Along these lines, one of the instruments that is limited to this approach is the Barnes and Olson Quality of Life Scale (ECVOB). Objective. The purpose of the study was to determine the validity based on the internal structure and reliability of the ECVOB in schoolchildren and university students in Lima. Method. The study design is instrumental. The Spanish version of the ECVOB was used on a sample of 1239 students from Lima between males (57%) and females (43%) aged between 14 and 26 years (M age = 22.45; SD age = 3.75). The evidence of validity of the internal structure was evaluated through a confirmatory factor analysis. Results. The findings suggest a partial restructuring of the original seven-factor model proposed by Olson and Barnes. Conclusion. The new structure of the ECVOB is a valid and reliable measure in schoolchildren and university students in Lima.

Illness integration, self-management and patient-centred support in type 2 diabetes / Integration av sjukdom, sjukdomshantering och patientcentrerad support vid typ 2 diabetes

Background: Type 2 diabetes is a serious disease that is increasing globally. The focus of diabetes care has been to prevent diabetes related complications and thereby reduce mortality. An older population, the disease progression and decreased ability to perform self-management activities increases the risk for complications. Group education and patient-centred care are recommended to improve self-management through increased patient empowerment. Despite these recommendations, professionals have been reluctant to adopt these methods referring to lack of knowledge, time and tools to deliver patient-centred care in diabetes. Focusing on the patient’s illness integration process has in the literature been suggested to improve self-management and metabolic balance. Aim: The overall aim of the thesis was to describe the experiences of illness integration, self-management and support in type 2 diabetes and to evaluate the metabolic effects of a nurse-led patient-centred model for self-management support. Methods: The study setting was primary health care in Västerbotten County, Sweden. In total, 21 diabetes nurses (Study I) and 257 patients (Studies II-IV) participated in the four studies (Study II, n=44; Study III, n= 18; Study IV, n= 195). Data consisted of focus group interviews (Study I), individual semi-structured interviews (Studies II-III) and laboratory measurements (Study IV). Methods for analyses were qualitative content analysis (Studies I-III) and statistics (Study IV). Results: Study I revealed that diabetes nurses found the ideal diabetes care complex to achieve. Conflicting paradigms, power relations and departmentalisation of work were described. Study II describes a process whereby illness integration and self-management in type 2 diabetes develop simultaneously. When a turning-point occurs, people view self-management as both necessary and feasible. In study III, turning points in self-management are illuminated. Turning-point transitions include existential and emotional aspects that increase inner motivation and power for lifestyle change. Study IV evaluates the effects of a nurse-led intervention in which haemoglobin A1c (HbA1c) was significantly decreased at 12 months’ follow-up. Group intervention and individual intervention were both effective compared to traditional diabetes care. Conclusions: There is a potential for improvement of type 2 diabetes care. Increased patient-centredness is important to support patients towards illness integration and self-management. Focusing on the patients’ illness experiences, including the existential and emotional aspects of having and managing type 2 diabetes, in counselling can lead to improved self-management and glycaemic control. Patients’ experiences of illness are central to their inner motives for change, and patient-centred self-management support and patient education preferably emanate from this perspective. / Bakgrund: Typ 2 diabetes är en allvarlig sjukdom som ökar globalt. Fokus i diabetesvården har varit att förebygga diabetesrelaterade komplikationer och därmed minska dödligheten. En åldrande befolkning, progression av sjukdomen samt en begränsad förmåga att utföra egenvårdsaktiviteter ökar risken för komplikationer. Grupputbildning och patientcentrerad vård rekommenderas för att förbättra egenvården genom ökad empowerment. Trots dessa rekommendationer har läkare och sjuksköterskor rapporterats vara tveksamma till dessa metoder och hänvisat till bristande kunskap, tidsbrist och avsaknad av verktyg. Att fokusera på patienters sjukdomsintegrationsprocess har i litteraturen föreslagits förbättra såväl egenvård som metabol balans. Syfte: Avhandlingens övergripande syfte var att beskriva erfarenheter av sjukdomsintegration, sjukdomshantering och support vid typ 2 diabetes samt att utvärdera effekter av en sjuksköterskeledd patientcentrerad modell för egenvårdssupport. Metod: Studierna genomfördes inom primärvården i Västerbottens läns landsting. Totalt deltog 21 diabetessjuksköterskor (Studie I) och 257 patienter (Studie II-IV), i de fyra studierna (Studie II, n=44; Studie III, n= 18; Studie IV, n= 195). Data bestod av fokusgruppsintervjuer (Studie I), individuella halvstrukturerade intervjuer (Studie II-III) och laboratoriemätningar (Studie IV). Analysmetoderna var kvalitativ innehållsanalys (Studie I-III) och statistik (Studie IV). Resultat: Studie I visade att en ideal diabetesvård är svår att uppnå enligt diabetessjuksköterskorna. Paradigmkrockar, maktrelationer och splittrade arbetsuppgifter beskrevs. Studie II beskriver en modell där sjukdomsintegration och sjukdomshantering utvecklas samtidigt. När en ”turning point” nåddes såg patienterna sjukdomshanteringen som både nödvändig och möjlig att genomföra. I studie III belystes ”turning points” och som omfattar både existentiella och emotionella aspekter som kan öka inre motivation och egenkraft att utföra livsstilsförändringar. Studie IV utvärderade 12-månaderseffekterna av en sjuksköterskeledd intervention och HbA1c sänktes signifikant. Gruppintervention och individuell intervention visades vara effektiva metoder i jämförelse med traditionell diabetesvård. Slutsatser: Det finns en förbättringspotential inom diabetesvården och ökad patientcentrering är viktig för att stödja patienter att integrera sjukdom och egenvård i livet. Att i diabetesvården fokusera på patienters sjukdomsupplevelser inklusive de existentiella och emotionella aspekterna av att leva med och hantera en sjukdom kan innebära förbättrad sjukdomshantering och metabol balans. Patienters sjukdomsupplevelser är centrala för deras inre motiv till förändring och patientcentrerad egenvårdssupport och patientutbildning bör utgå från detta perspektiv.

Effekten av yoga vid bröstcancer : En litteraturstudie

Bakgrund: I Sverige är bröstcancer den vanligaste cancerformen hos kvinnor. Att drabbas av bröstcancer medför ofta stora psykiska och fysiska påfrestningar och besvären kvarstår i många fall efter avslutad behandling. I dagsläget finns det endast begränsade rehabiliteringsalternativ för de som drabbats. Tidigare forskning har visat att fysisk aktivitet ger positiva effekter hos personer som drabbats av bröstcancer. Yoga är en träningsform som kombinerar fysisk aktivitet med meditativa tekniker och kan anpassas till deltagarens förutsättningar oavsett hälsotillstånd.  En ökad kunskap om yogans effekter kan ge sjuksköterskor en förståelse för hur yoga kan användas inom rehabilitering för att hjälpa den som drabbats av bröstcancer under återhämtningsprocessen. Syfte: Syftet med litteraturstudien var att beskriva effekten av yoga vid bröstcancer, där den metodologiska aspekten belyser vilka fysiska och psykiska effekter yoga har på personer som drabbats av bröstcancer. Metod: En beskrivande litteraturstudie där tolv randomiserade kontrollerade studier granskats och sammanställts för att svara på syftet.   Huvudresultat: Resultatet visade på fler förbättringar inom psykiska symtom än vid fysiska symtom. I resultatet framkom att yoga har en direkt effekt på fatigue och livskvalitet med en kvarstående effekt efter avslutad behandling jämfört med kontrollgrupper. Även resultatet för menopausala symtom visade på en tydlig minskning efter yogainterventionen. Däremot visade resultatet för sömnkvalitet endast på förbättringar om yoga utövades minst två gånger i veckan. Slutsats: Även om utfallet av litteraturstudien gav ett blandat resultat speglar huvudresultatet yogans positiva effekter på psykiska symtom hos personer som drabbats av bröstcancer. En ökad kunskap om yogans effekter kan ge sjuksköterskor en bättre förståelse för hur yoga kan användas inom rehabilitering med syfte att ge en förbättrad livskvalitet för den specifika patientgruppen. / Background: In Sweden, breast cancer is the most common form of cancer in women. Being affected by breast cancer often causes great mental and physical stress and the problems persist in many cases after treatment is completed. Currently, there are only limited rehabilitation options for those affected. Previous research has shown that physical activity produces positive effects in people suffering from breast cancer. Yoga is a form of exercise that combines physical activity with meditative techniques and can be adapted to the conditions of the participant regardless of state of health. An increased knowledge of the effects of yoga can give nurses an understanding of how yoga can be used in rehabilitation to help those affected by breast cancer during the recovery process.   Aim: The aim of this literary review was to describe the effects of yoga in breast cancer, where the methodological aspect highlights the physical and psychological effects yoga has on people affected by breast cancer. Method: A descriptive literary review where twelve randomised controlled studies have been reviewed and complied to answer the purpose. Main result: The results showed more improvements in psychological symptoms than in physical symptoms. The result showed that yoga has a direct effect on fatigue and quality of life with a lasting effect after completion of treatment compared to control groups. The results for menopausal symptoms also showed a clear decrease after the yoga intervention. In contrast, sleep quality results showed improvement only if yoga was practiced at least twice a week. Conclusion: Although the outcome of the literature study produced a mixed result, the main result reflects the yoga's positive effects on the mental symptoms of people suffering from breast cancer. Increased knowledge of the effects of yoga can give nurses a better understanding of how yoga can be used in rehabilitation with the aim of providing an improved quality of life for the specific patient group.

Application of crispr/cas9-based reverse genetics in leishmania braziliensis: Conserved roles for hsp100 and hsp23

The protozoan parasite Leishmania (Viannia) braziliensis (L. braziliensis) is the main cause of human tegumentary leishmaniasis in the New World, a disease affecting the skin and/or mucosal tissues. Despite its importance, the study of the unique biology of L. braziliensis through reverse genetics analyses has so far lagged behind in comparison with Old World Leishmania spp. In this study, we successfully applied a cloning-free, PCR-based CRISPR–Cas9 technology in L. braziliensis that was previously developed for Old World Leishmania major and New World L. mexicana species. As proof of principle, we demonstrate the targeted replacement of a transgene (eGFP) and two L. braziliensis single-copy genes (HSP23 and HSP100). We obtained homozygous Cas9-free HSP23-and HSP100-null mutants in L. braziliensis that matched the phenotypes reported previously for the respective L. donovani null mutants. The function of HSP23 is indeed conserved throughout the Trypanosomatida as L. major HSP23 null mutants could be complemented phenotypically with transgenes from a range of trypanosomatids. In summary, the feasibility of genetic manipulation of L. braziliensis by CRISPR–Cas9-mediated gene editing sets the stage for testing the role of specific genes in that parasite’s biology, including functional studies of virulence factors in relevant animal models to reveal novel therapeutic targets to combat American tegumentary leishmaniasis. / Alexander von Humboldt-Stiftung / Revisión por pares

Comparison of the Virological Response According to the Antiretroviral Regimens in Peruvian HIV Patients Who Presented the M184V Mutation in Two National Hospitals during the Years 2008 to 2019

El texto completo de este trabajo no está disponible en el Repositorio Académico UPC por restricciones de la casa editorial donde ha sido publicado. / Introduction: In patients with HIV in antiretroviral treatment (ART) and virological failure to the first-line regimen, establishing a therapeutic regimen after having identified the M184V mutation, which confers ART resistance, represents a dilemma. Objective: To compare the virological response of the therapeutic regimens prescribed to patients with HIV who presented the M184V mutation in two national hospitals in Lima, Peru, during the years 2008 to 2019, and to determine the risk factors associated with poor virological response. Methods: A retrospective cohort study was developed based on the information of the HIV program participants with the M184V mutation. Results: A total of 175 participants were eligible for the study. The male sex predominated (75.4%), the current median age was 41 years [interquartile range (IQR) 35.84-47.47], and the time on ART was 89 months (IQR 57.7-124.53). The median initial viral load (VL) was 4.5 log10 copies/mL (IQR 3.97-5.09) and the time between genotyping and the change of therapy was 2 months (IQR 0-3.56). The most used antiretroviral regimen was protease inhibitor plus two nucleoside reverse transcriptase inhibitors (55.4%). With the protease inhibitor plus integrase inhibitor (PI + INI) ART, 69% less risk of poor virological response was obtained [p = .019 (confidence interval 95% 0.117-0.825)]. Conclusions: In patients with HIV and the M184V mutation, the PI + INI ART has shown a greater decrease in control VL and, thus, a good virological response. The risk factors associated with a poor virological response were the delay between genotyping and change of therapy, high levels of initial VL, and poor adherence among the participants. / Revisión por pares

Association between household socioeconomic level and consumption of fast food and soft drinks: A cross-sectional assessment of the Young Lives cohort in Peru

Introduction: The consumption of fast food and soft drinks is a risk factor for developing overweight and obesity. This study aimed at assessing if there is association between household socioeconomic level and the consumption of fast food and soft drinks among children. Material and Methods: A cross-sectional assessment of the data from the third round (2009-2010) of the youngest cohort of the Young Lives study in Peru was conducted. Sampling was conducted in three stages: In the first one, the country was divided into equal geographical regions, excluding the 5% of the richest district; in the second stage, 20 sentinel sites were chosen and an area within each sentinel site was selected. Finally, in the third stage, eligible children were selected. Outcomes were the self-reported consumption of fast food and soft drinks (never, sometimes, and always), whereas the exposure was household socioeconomic status (in quintiles). Crude and adjusted models were created between variables of interest using Poisson regression models, with robust variance, to report prevalence ratios (PR) and 95% confidence intervals (95% CI). Results: Data of 1901 children, of which 942 (49.6%) were girls, with a mean age of 7.5 (SD: 0.5) was analyzed. A total of 24.1% (95%CI: 22.2%-26.1%) reported always consuming fast food, whilst this number was 22.4% (20.5%-24.3%) for soft drinks. Compared to the lowest socioeconomic quintile, those who were in higher socioeconomic status had more probability of consuming fast food and soft drinks (Chi-squared for trends <0.001). The highest socioeconomic quintile had a greater probability to always consume fast food (PR=1.42; 95%CI: 1.08-1.88) and soft drinks (PR=1.71; 95%CI: 1.24-2.37). Conclusions: This study shows that there is a significant association between the household socioeconomic level and the consumption of soft drinks and fast food. / Revisión por pares
