Sjukgymnasters upplevelser av arbete på särskilt boende för äldre : En intervjustudie / Physiotherapists’ Perceptions of work in Housing for the Elderly : An Interview Study

Bakgrund: Förutsättningarna för sjukgymnasters arbete på särskilt boende ser olika ut beroende på brukarantal, möjlighet att vara tillgänglig samt resurser från kommunen. Flera studier visar på att resursbrist försvårar sjukgymnasters arbete och att det är svårt att tillgodose brukarnas behov när sjukgymnasterna har det medicinska rehabiliteringsansvaret för ett stort antal brukare. Rehabilitering sker på specifik och allmän nivå, och till stor del genom omvårdnadspersonal. Syfte: Syftet med studien var att beskriva sjukgymnasters upplevelser av sitt arbete på särskilt boende för äldre. Metod: Datainsamling skedde genom sju semistrukturerade intervjuer med sjukgymnaster som arbetade på särskilt boende. Data analyserades med kvalitativ innehållsanalys. Resultat: Vid analys framkom fem huvudkategorier: styrande faktorer, prioriteringar, samarbetet runt brukaren, arbetets bredd samt självbild och status. Konklusion: Deltagarna upplevde sitt arbete på särskilt boende för äldre som roligt och varierande, men påverkat av stress och resursbrist. Arbetet upplevdes bestå av mycket prioriteringar men vad de grundade sig på varierade, liksom upplevelsen av stödet från riktlinjer. Deltagarna uppgav att det är önskvärt med ett väl fungerande samarbete med omvårdnadspersonalen eftersom de sjukgymnastiska insatserna i stor utsträckning delegeras till dem. / Background: The conditions for the physiotherapists’ work in housing for the elderly vary depending on number of care recipients, the availability of the physiotherapists and the resources provided at the county level in Sweden. Previous studies have shown that the lack of resources makes the work difficult for the physiotherapists. Moreover, it is hard to meet the needs of individual care recipients when physiotherapists have the medical rehabilitation responsibility for a large number of care recipients. Rehabilitation is carried out on a specific level and a general level and often delegated to care assistants. Purpose: The aim of this study was to examine physiotherapists’ perceptions of their work in housing for the elderly. Method: Seven physiotherapists working in houses for the elderly were interviewed with the help of a semi-structured interview guide. The interviews were analyzed with qualitative content analysis. Results: The analysis revealed five categories: controlling factors, priorities, cooperation around the care recipient, the breadth of the work and self-image and status. Conclusion: The participants’ perceptions of work in housing for the elderly were that the job was fun and varied but influenced by stress and lack of resources. The work was perceived as consisting of a lot of prioritizing; however, the grounds on which the decisions were made varied as did the perception of support from guidelines. The participants reported that they desired a well-functioning working relationship with the care assistants because the physiotherapy inputs were often delegated to them.

Bakgrund: Världens befolkning blir allt äldre. Idag är 1 av 11 över 65 år men år 2050 kommer 1 av 6 vara över 65 år. Antalet äldre som är över 80 år och kräver insatser i form av hemtjänst, särskilt boende, dagverksamhet eller korttidsboende ökar. Här ser vi ett av samhällsstödets, vårdens och omsorgens största framtida prövningar. Syfte: Beskriva hur äldre personer upplever att det är att bo på ett särskilt boende. Metod: En beskrivande litteraturstudie som baseras på 12 vetenskapliga artiklar med kvalitativ ansats. Sökningarna efter artiklar har skett i databasen PubMed. Huvudresultat: Flytten till ett särskilt boende påverkade den äldres känsla av frihet. Leva på boendet sågs som en lång väntan på döden och många kände sig ensamma och övergivna. De äldre kände sig begränsade och upplevde att ingen lyssnade på deras åsikter. Låsta dörrar på boendet uppfattades som begränsande och de äldre hade känslan av ett fängelse. Aktiviteter var viktigt för välmåendet, både ensam och tillsammans med andra. Kunna röra sig fritt och ha närhet till natur, friluftsliv och kunna gå ut på promenader var viktigt. Närståendes delaktighet i äldres liv var viktigt för välmåendet. Bemötandet av sjuksköterskan hade stor betydelse för trivseln. Slutsats: De äldre upplevde att aktiviteter eller träning tillsammans med andra äldre eller ensamma var en positiv upplevelse. Närståendes delaktighet var av stor betydelse för de äldre. De äldre personerna upplevde att de kände sig instängda när dörrarna var låsta och hade en önskan att kunna gå utomhus oftare. Många kände sig ensamma på det särskilda boendet och att det var en lång väntan på döden. Detta trots att många människor fanns runt dem. Det är av betydelse för de äldre att sjuksköterskan tar sin ledarroll och vägleder vårdpersonalen att ha ett gott bemötande i alla situationer. För att sjuksköterskan ska få en bättre bild av vad som påverkar de äldres upplevelse krävs mer forskning. / Background: The World population is getting older. Today is 1 of 11 persons over the age 65 but in 2050 it will be 1 of 6 persons aged over 65. The number of people over 80 years old requires efforts in the form of home service, housing for elderly, day care or short-term housing are increasing. This will be one of the biggest challenges for social support, health care and welfare in the future. Aim: To describe how older persons experience what it is like living in housing for elderly.Method: A descriptive literature study based on 12 scientific articles with a qualitative approach. The searches for articles have been done in the database PubMed. Main results: The move to housing for elderly had an impact on their feelings for freedom. Living in the housing for elderly felt like a long wait for death and many felt lonely and abandoned. The elderly felt like they were limited and experienced that no one listened to their opinions. Locked doors in the environment perceived as a limitation and the elderly felt like they were in prison. Activities seemed like important for the wellbeing, both together with others and alone. To be able to move freely, being close to nature, outdoor life and being able to take a walk outside was important. Relatives being a part of the elderly´s life was important for their wellbeing. The way the nurse treated them had a big impact on how they enjoyed living in the housing for elderly. Conclusion: The elderly experienced the activities or training sessions together with others or on their own was a positive experience. Relatives being a part of the elderly´s life was highly important. The experience of closed doors made the elderly feel like they were locked, and they had a wish of be able to be outside more often. Many had a feeling of loneliness in the housing for elderly and that it was just a long wait for death. Even If they had a lot of people around them in the housing for elderly. It is important for the elderly that the nurse takes her leadership role and guides the staff to have a good attendance in all situations. For the nurse to get a better sight of what is affecting the elderly´s experience of living in nursing homes, more research is needed.

Site is situated near the city center, in the Staré Brno district, in between of Hybešova street and Hospital of st. Anne. According to a good connection with city center by public transport and various functions for habitants, it has promising potential for creating ideals conditions for housing in the city center. Project is based on analysis and social study from last year. I've tried to get into the problematic of needs of modern people, living in the center, its problems and adavantages in context of history and sociology. My result is strategy for living, applied on Staré Brno site.

Analysis of the Housing Status of the Elderly

Quality of housing was examined in relation to living arrangement, race, socioeconomic status, age, gender, income, health, and renter-owner status. There appeared to be differences in housing quality for those of younger ages, females, and owners. White subjects occupied good quality housing while Blacks occupied fair quality housing. Proportion of income allocated to housing was examined by renter-owner status, race, gender and living arrangement. Renters allocated an average of 34 per cent compared to 17 per cent for owners. Females living alone occupied good quality housing, committed the highest proportion of income to housing, and one-third had incomes below the poverty threshold while two-thirds were near poverty. Subjects allocated more for utilities than others over sixty-five, nationally.

Ageing has become a significant research area in the last few decades. The most important reason that makes ageing issue significant is the increasing rate of the older indviduals within the population all around the world. Statistical data on ageing both in Turkey and in the world reveals that, in the near future, the number of population over 65 will increase dramatically. It can be asserted that there will be an urgent need of specifically designed accommodation for this portion of the population. Hence, this thesis intends to search for and practice a design solution for the elderly housing. One of the instruments that supported the formulation of such a solution is the subject of &lsquo / universal design&rsquo / . Within the scope of this study, the principles of universal design and its relationship with ageing are defined, examined and evaluated. It is accepted that living environment has effect on the enhancement of the individual&rsquo / s quality of life. For this reason, the relationship between person and environment is briefly discussed in congruence with principles of universal design and provision of quality of life. Accessibility, safety, privacy, autonomy and independence are the common concerns of universal design and provision of a higher quality of life. With review of the definitions of specific elderly housing types, another approach for this study is to make a case study of the existing architectural solutions for the elderly housing. This helped to understand architectural approaches to the relationship between ageing and environment in the world. In the light of the findings, an architectural project, presenting a habitable environment for the elderly, is proposed. For the project, the concept of &ldquo / assisted living&rdquo / as a new category for elderly housing, is chosen.

Accessibility must transcend the interior of buildings and permeate all its surroundings and the urban environment. Make the city accessible is a challenge, and this becomes even bigger when the focus is the elderly population. It must be changed the way of thinking cities, as the range of the population over 60’s that live alone is increasing, and ensuring them accessibility and mobility is a duty of the government. This study is meant to examine the accessibility and mobility of residents of a housing project for the elderly in the city of Araraquara, SP, using as reference the Brazilian Standard NBR 9050, 2015. This analysis includes the views of residents and the responsible administrative agent on the issue, as well as the use of technical parameters of ISO 9050, for the elaboration of a check list to ascertain whether the place has the necessary accessibility for this vulnerable group. The methodology consists in applying the tools MMSE test-Mini Mental State Examination, conversation circles, as well as relevant parameters of the standard NBR 9050. Initially the results pointed to a very positive perception by the elderly, although some accessibility parameters have not been included in the construction of houses and residential facilities. Furthermore, it was found very low mobility of older people when they need to perform their daily tasks, making possible by the use of transport modes on foot and by public transport by bus. Improve accessibility both internally and in mobility for the surrounding of the residence location, would be some of the possible solutions to increase security and ensure autonomy, fostered by increased mobility. / Com o envelhecimento acentuado da população, se faz necessário repensar as cidades de modo a oferecer acessibilidade a todos seus cidadãos, possuindo ou não alguma restrição de mobilidade. A acessibilidade deve transcender o interior das construções e permear todo seu entorno e o ambiente urbano. Tornar a cidade acessível é um desafio, e este se faz maior quando o foco é a população idosa. Deve-se mudar a maneira de pensar as cidades, pois a faixa da população acima de 60 anos que mora sozinha está aumentando e garantir a eles a acessibilidade e a mobilidade é um dever do Poder Público. Este estudo tem por objetivo principal analisar a acessibilidade e mobilidade de moradores de um núcleo habitacional para idosos do município de Araraquara, tomando como referência a NBR 9050, de 2015. Esta análise conta com a visão dos moradores acerca do assunto, do agente administrativo responsável, assim como o uso de parâmetros técnicos da NBR 9050, para a formação de um check list para averiguar se o local possui a acessibilidade necessária a este grupo vulnerável. A metodologia consta em aplicar as ferramentas teste MEEM-Mini Exame do Estado Mental, a Roda de Conversa, bem como parâmetros pertinentes da Norma 9050. Os resultados apontaram inicialmente, uma percepção positiva pelos idosos, apesar de alguns parâmetros de acessibilidade não terem sido contemplados na construção das casas e equipamentos do Residencial. Constatou-se também, mobilidade bastante reduzida dos idosos quando precisam realizar suas tarefas do cotidiano, possibilitada pelo uso dos modos de transportes a pé e por transporte coletivo. Melhorar a acessibilidade tanto interna, quanto a mobilidade para o entorno do Residencial, seriam formas de aumentar a segurança e assegurar autonomia propiciada pela maior mobilidade.
