Project Management Practices in Small Projects: 5 cases in a Canadian Hospital Setting

Healthcare is continuously changing through means of project work. Small projects in healthcare settings are of particular interest since they are usually not adequately supported by the health institution, and present many challenges of their own. However, collective successful completion of small projects has the potential to significantly impact and improve health service delivery. This thesis examined the common and successful project management practices in small projects in Canadian hospital settings to acquire new knowledge on this understudied kind of project and propose basic project management practice guidelines for future small projects conducted within these settings. Data collection was conducted in two hospitals in the form of 23 semi-structured interviews with five interprofessional project teams composed of 4-10 healthcare professionals. Each project was considered as the unit of analysis. Qualitative within-case and cross-case inferential processes were applied and a consolidated list of 43 project management practices deemed important by the majority of participants from all cases was revealed and could serve as basic project management practice guidelines for future small projects conducted in hospitals. Findings also shed light on the beneficial value of adapting principles of project management to small projects in hospital settings. Given the bottom-up nature of small projects, results suggest it is of significant importance to clearly define and understand the small project, as well as perform a thorough stakeholder analysis to be able to gain the right approvals. Insufficient time dedicated to small project management governed these shortcomings, thus team members need to regularly allot time to managing their small project. Lastly, the presence of a team leader was a significant factor influencing continuous project execution. Future studies should take into consideration allied disciplines’ contributions such as organizational behaviour to help explain the interplay between group dynamics and small project outcome.

Interprofessionellt teamarbete på akutmottagningar : En intervjustudie om sjuksköterskors upplevelser

Bakgrund: Interprofessionellt teamarbete blir allt vanligare i vården, i takt med att det blivit vanligare har det setts förbättringar inom bland annat patientsäkerhet och personcentrerad vård. Det finns dock lite forskning som studerat sjuksköterskorna upplever interprofessionellt på en akutmottagning. Syfte: Att beskriva sjuksköterskors upplevelser av interprofessionellt teamarbete på akutmottagningar. Metod: En kvalitativ intervjustudie med tio sjuksköterskor från två akutmottagningar. Datainsamlingen skedde genom enskilda semistrukturerade intervjuer. Tio sjuksköterskor som arbetade med interprofessionellt teamarbete på akutmottagning intervjuades på två olika sjukhus. Insamlade data analyserades sedan med kvalitativ innehållsanalys. Resultat: I resultatet framkom fyra teman, förutsättningar och hinder för interprofessionellt teamarbete, ökad möjlighet till god omvårdnad, effekter av närmare samverkan, och patienten en del av teamet. Dessa teman fick sedan olika många kategorier. Det framkommer av resultatet att många olika faktorer är inblandade vid ett väl fungerande interprofessionellt teamarbete. Både faktorer som inte går att påverka och faktorer som kan behöva uppmärksammas för att kunna ta hänsyn till eller åtgärda. Slutsats: Resultatet påvisade att sjuksköterskan hade mestadels positiva upplevelser av interprofessionellt teamarbete men också att det finns flera faktorer som kan påverka det sammanlagda resultatet av interprofessionellt teamarbete både för de olika professionerna och patienten. / Background: Interprofessional teamwork in health care is becoming more common, and improvements has been seen in areas such as, patient safety and person-cantered care. There is limited research on how registered nurses experience interprofessional teamwork in an emergency department. Aim: To describe nurses' experiences of interprofessional teamwork in emergency departments. Method: a qualitative interview study with ten registered nurses from two emergency units. The data collection took place through individual semi-structured interviews. Ten nurses who worked with interprofessional teamwork in an emergency department were interviewed at two different hospitals. Qualitative content analysis was then used to analyse the collected data. Result: The result revealed four themes, conditions and barriers to interprofessional teamwork, increased opportunities for good nursing, the effects of closer collaboration, and the patient being part of the team. These themes were divided in further categories. The result shows that many different factors were involved in a well-functioning interprofessional team work. Factors that cannot be influenced and factors that may need to be addressed and taken into account or adjusted. Conclusion: The result showed that the nurse had mostly positive experiences of interprofessional teamwork, but there are also several factors that can influence the overall result of interprofessional teamwork for the different professions and the patients.

Strävan efter den goda kommunikationen i det interprofessionella operationsteamet - en intervjustudie / The pursuit for good communication in the interprofessional surgical team - an interview study

Introduktion: I den perioperativa vården arbetar flera yrkeskategorier som ska samverka tillsammans på ett patientsäkert sätt. Kommunikation är en viktig del till att kunna arbeta patientsäkert. Enligt patientsäkerhetslagen är vårdgivare skyldiga att förebygga vårdskador där en fungerande kommunikation kan vara en del att upprätthålla patientsäkerheten. Tidigare forskning har visat att kommunikation i operationsteamet kan påverka patientsäkerheten både positivt och negativt. Syfte: Syftet var att beskriva det interprofessionella operationsteamets upplevelser av kommunikation för att främja patientens säkerhet inom perioperativ vård. Metod: En kvalitativ intervjustudie utfördes med hjälp av semistrukturerade intervjuer. Intervjuerna utfördes på tre sjukhus i Mellansverige. Totalt femton intervjuer utfördes. Fem olika yrkeskategorier intervjuades, med tre deltagare från varje kategori: operationssjuksköterskor, anestesisjuksköterskor, undersköterskor, anestesiläkare och operatörer. Totalt femton intervjuer utfördes. Intervjuerna analyserades utifrån kvalitativ innehållsanalys. Resultat: Analysen resulterade i ett övergripande tema och tre kategorier. Temat var: ”Strävan efter den goda kommunikationen”, där det interprofessionella operationsteamets upplevelser beskrevs utifrån följande kategorier: ”De personliga relationernas betydelse”, ”Individuella strategier” och ”Användandet av kommunikationshjälpmedel”. Konklusion: Studien visade att det interprofessionella teamet strävade efter en god kommunikation som främjar patienten säkerhet. Dock fanns det även svårigheter i att uppnå detta.

Students perception of their own and each other’s profession

Interprofessional practice and teamwork are becoming more essential in health care. Increased collaboration gives the opportunity to enable patient centred health care with high quality of outcomes. If health professional students learn together they will be better prepared for interprofessional collaboration and teamwork, such initiative can be interprofessional education (IPE) at a clinical education ward (CEW). The aim of the study is to explore how students from medicine, nursing, physiotherapy and occupational therapy programs, perceive their own and each others` profession prior to a clinical placement at a CEW. This was a qualitative study using focus groups. 18 students participate in the study. Data were analyzed using thematic analysis and a thematic network was developed. A plethora of professional perceptions emerged as the highest order category. This was derived from four organizing themes; professional expectations, relationship to the patient, leadership and relationship across professions and from the respective basic themes: different areas of professional doing and explanation of professional images, being close, being distanced, being a coordinator and being a leader, collaboration and unclear boundaries. This perceptions are needed to discuss and reflect during the CEW and allowing the student, together rather than silos, to develop professional identities as well as the knowledge, skills and attitudes.

Bakgrund: Kommunikation är en nyckelfaktor för att kunna bedriva en säker och tillfredsställande vård. Vårdteamet består av flera olika professioner som behöver kommunicera för att uppnå satta mål. Av alla vårdskador är 70% orsakade på grund av bristande kommunikation. För att öka patientsäkerheten krävs det att vårdteamet har rutiner för att säkerställa att patientinformation överförs på ett säkert sätt. Inte minst mellan läkare och sjuksköterska krävs ett säkert kommunikationssystem eftersom professionerna samarbetar kontinuerligt. Syfte: Att belysa sjuksköterskors erfarenheter av interprofessionell kommunikation med läkare och dess betydelse för patientsäkerheten. Metod: Litteraturstudie baserad på tio vetenskapliga artiklar med kvalitativ ansats som granskats, analyserats och sammanställts till ett resultat. Resultat: Sju teman har framställts i resultatet bestående av “Roller”, “Tidsbrist”, “Förminskning”, “Anpassad kommunikation”, “Brist på information”, “Följder för patienten av bristande kommunikation” och “Teamarbete”. Konklusion: Sjuksköterskor upplever att deras kunskaper inte tas på allvar av läkarna. När läkarna undanhåller information om patienten upplevs frustration. De anser att en god relation till läkaren gynnar kommunikationen. För att förmedla information till läkarna krävs det att sjuksköterskrona anpassar sin kommunikationsstil beroende på specifik läkare och situation. / Background: Communication is a key factor to be able to provide safe and satisfactory care. The healthcare team consists of multiple professions who need to communicate to achieve their goals. 70% of all instances where the patient comes to harm in healthcare is because of communication errors. To increase the patient safety the healthcare team needs routines to ensure that patient information is transferred between the professions in a safe manner. Especially between the nurse and the physician the communication needs to function since the two professions corporate continuously. Aim: To examine nurses experiences of communication with physicians and the impact on patient safety. Methods: A literature review based on ten scientific articles with qualitative data that has been reviewed, analysed and compiled to a result. Results: Seven themes were produced in the results consisting of “Roles”, “Lack of time”, “Diminishing”, “Adapted communication”, “Lack of information”, “Consequences for the patient because of lack in communication” and “Teamwork”. Conclusion: The nurses experience that their knowledge is not taken seriously by the physicians. When the physicians are withholding information, the nurses feel frustration. They believe that a good relation with the physician improves the communication. To mediate information to the physicians the nurses sometimes need to adapt their communication style depending on which physician they are communicating with.

Inledning: Kommunikation är en viktig del inom hälso- och sjukvård och innebär utbyte av information mellan människor. Speciellt vid multitrauma är det viktigt att kommunikationen fungerar eftersom personal från olika avdelningar ska samarbeta med varandra. Dock kan kommunikation påverkas av olika faktorer, som i sin tur kan påverka patienten. Syfte: Syftet med studien var att beskriva vilka faktorer som kan påverka interprofessionell kommunikation vid multitrauma. Metod: Studien genomfördes som en allmän litteraturöversikt där tio vetenskapliga artiklar inkluderades. Resultat: Resultatet i studien visade fyra olika kategorier som påverkar interprofessionell kommunikation både positivt och negativt vid multitrauma. Dessa faktorer kategoriserades till; strukturerad överrapportering, stressig och kaotisk arbetsmiljö, erfarenhet och kunskap samt ledarskap. Att inneha erfarenhet och kunskap inom multitrauma är viktigt för att ett traumateam ska fungera på optimalt sätt. Personal utan erfarenhet eller kunskap kring multitrauma eller kommunikationen mellan teamet påverkar samarbetet negativt. Slutsats: Studiens resultat visar att det finns ett flertal faktorer som kan påverka interprofessionell kommunikation negativt, men även positivt. Med hjälp av traumaledare kan arbetet struktureras upp, men det krävs en viss nivå av kunskap och erfarenhet för att kommunikationen mellan traumateamet ska fungera på optimalt sätt. / Introduction: Communication is an important part of healthcare and involves the exchange of information between people. Especially in the case of multitrauma, it is important that the communication work because staff from different departments must cooperate with each other. However, communication can be affected for various reasons, which in turn can affect the patient. Aim: The purpose of the study is to describe the factors that can affect interprofessional communication in multitrauma. Method: The study was conducted as a general literature review where 10 scientific articles were included. Results: The results of the study showed four different categories that affect interprofessional communication both positively and negatively in multiple trauma. These factors were categorized into; overreporting, stressful and chaotic work environment, experience and knowledge as well as leadership. Having experience and knowledge in multiple trauma is important for a trauma team to function optimally. Staff without experience or knowledge of multiple trauma or communication between the team has a negative effect on the collaboration. Conclusion: The results of the study show that there are a number of factors that can affect interprofessional communication negatively, but also positively. With the help of a trauma leaders, the work can be structured, but a certain level of knowledge and experience is required for the communication between the trauma team to function optimally.
