Frequency of Complications Following Spinal Fusion in Children with Cerebral Palsy

Background: Neuromuscular Scoliosis is a frequent complication of Cerebral Palsy that requires surgical management including spinal fusion. The objective of this observational study was to describe differences in the frequency of postoperative complications in children with Cerebral Palsy following spinal fusion surgery compared to children with Idiopathic Scoliosis. Methods: The 2016 Kids’ Inpatient Database was queried to identify pediatric patients (old) with concurrent diagnoses of Cerebral Palsy and Neuromuscular Scoliosis undergoing spinal fusion surgery. Cases were compared to children without Cerebral Palsy and with a diagnosis of Idiopathic Scoliosis undergoing the same procedure. Fitted Poisson regression analysis with robust variance was performed to estimate relative risks in the frequency of various clinical complications while adjusting for several potentially confounding variables of importance. Results: A total of 660 cases and 5,244 comparators were identified. Compared to children with Idiopathic Scoliosis, children with Cerebral Palsy were younger (13.6 vs. 14.3 years), more likely to be male (54% vs. 23%), and more likely to have had governmental insurance (52% vs. 32%). They also had longer hospital lengths of stay (8 days vs. 4 days). After adjusting for a number of potentially confounding sociodemographic and clinical variables, children with Cerebral Palsy were more likely to have postoperative pulmonary, gastrointestinal, and surgical complications, receive blood transfusions, and be admitted to the ICU. Conclusions: Children with Cerebral Palsy have an increased risk of complications following spinal fusion surgery leading to longer hospital stays. These results further inform surgical decision-making and anticipatory guidance for these children and their caregivers.

Trends in skeletal maturation patterns in a Western Cape sample

Magister Chirurgiae Dentium - MChD / Skeletal age assessment is not only an important aspect in orthodontic treatment planning, but is also widely used in forensic medicine and physi cal anthropology. Vari aus studi es have shown that chronological age may be at variance with an individual's biologic age. Current research would seem to indicate that the hand-wrist radiograph provides the most accurate method of assessing skeletal age for diagnostic purposes. In recent years the number of patients presenting with malocclusions of a skeletal nature at the University of the Western Cape has increased si gnifi cantly. If it i s accepted that treatment of jaw discrepancies associated with malocclusion is dependent on a large component of dentofacial orthopedics, then by implication it is necessary that a substanti al amount of faci al growth remai ns. The need has therefore arisen for the establishment of skeletal maturation trends in the Western Cape. Skeletal maturity was assessed from hand-wrist radiographs in a sample of 318 Western Cape chi 1dren aged 6 to 16 years for both sexes, utilising the bone specific Tanner-Whitehouse TW-2 scoring system. Data obtai ned from the present study showed a marked di fference in skel etal maturati on trends between femal es of the Western Cape to that of the British norm, while the males showed less divergence. Further, these findings show that in both sexes the epiphyseal bones matured in advance of the TW-standard. Carpal maturation, however, was delayed in the male when compared to the British standard, while that of the female conformed to that of the British standard.

The efficacy of using platelet rich plasma in treating chronic tendinopathies

As a common source of long-term pain and physical disability, overuse injuries, such as chronic tendinopathies, severely impact a patient’s quality of life. Caused by repetitive trauma, chronic tendinopathies affect hundreds of millions of people each year. The exact pathogenic mechanism in developing this musculoskeletal injury is still largely unknown, making clinical recommendations on the best course of treatment highly debatable. Nonetheless, advancements in biotechnology have made autologous blood products, specifically the use of platelet rich plasma injections, an increasingly popular method in the orthopedic field. Studies have shown that the concentrated platelet sample harbors a number of bioactive mediators. Once activated and injected at the site of injury, these growth factors and cytokines augment the natural healing process in tendinopathic cases. With limited reported complications, many clinicians believe that platelet rich plasma therapy is a safe and accessible treatment option for patients diagnosed with chronic tendinopathy. As such, the primary purpose of this paper is to determine the efficacy of platelet rich plasma injections in treating chronic tendinopathies. This literature review determined that current published studies and research on the effectiveness of PRP injections have produced contradictory results. Due to its autologous characteristic, platelet concentration differs significantly from patient to patient, contributing to high variability in terms of its effectiveness between patients. However, with low long-term costs and fast recovery, PRP injections are a promising, non-surgical intervention for treating chronic tendinopathies. Several patient-centered clinical studies have reported significant improvements in range of motion and pain management when compared to traditional injection treatments. These effects are maximized when administered under ultrasound guidance or when used in conjunction with a strict eccentric exercise program. Nonetheless, continued research is needed to determine optimal injection standards so that clinical recommendations can be further developed and supported.

Ex vivo biomechanical comparison of four center of rotation angulation based leveling osteotomy fixation methods

Thirty-two canine tibiae with patellae and patellar tendons were harvested from 17 skeletally mature cadavers. Each tibia was randomly allocated to a construct group: plate and pin (Plate), plate with countersink compression screw (HCS), plate with tension band (TB), or plate with HCS and TB (HCSTB). Samples were loaded by distraction until failure. The stiffness, yield load, and ultimate load were compared between each fixation method. No difference in stiffness of the constructs was detected between groups. Yield load and ultimate load for the HCSTB group was greater than the TB group, the HCS group, and the Plate group. CBLO fixation augmented with a TB and HCS provided a stronger construct that withstood a greater yield load and ultimate load than either augmentation strategy alone. Augmenting a CBLO fixation with a TB and a HCS can provide increased construct strength.

Os gestores de saúde enfrentam desafios crescentes e recursos insuficientes em todo o mundo para referente às atividades realizadas, assim como observação direta dos profissionais para medir o tempo gasto em cada uma de suas atividades durante intervalos de tempo predeterminados. Posteriormente, os resultados encontrados no estudo foram aplicados ao instrumento informatizado de indicadores de carga de trabalho para estimativa do dimensionamento de pessoal (Workload Indicators of Staffing Need responder à demanda de serviços de saúde, especialmente relacionados à ortopedia e traumatologia, causando agravamento de casos e aumento do número de sequelas que reverterão em maior demanda por procedimentos de média e alta complexidade. Este estudo foi motivado pela necessidade de se analisar de forma consistente a composição e o desempenho dos profissionais na área de saúde e afins para o funcionamento pleno de uma unidade de saúde de alta complexidade em ortopedia e traumatologia. Teve como objetivo contribuir para o desenvolvimento da metodologia, disseminação do conhecimento e importância do dimensionamento da força de trabalho em instituições complexas de saúde, além de oportunidade para testar e disseminar o método de indicadores de carga de trabalho para dimensionamento de pessoal e sua importância para o planejamento e a gestão de recursos humanos em saúde. O projeto se desenvolveu em três estudos sequenciais e complementares. Inicialmente, foi feita uma revisão da bibliografia sobre o dimensionamento de recursos humanos em saúde (RHS), buscando reduzir lacuna da escassez de modelos e metodologias, bem como contribuir para o desenho e a utilização de modelos de organização e prestação de serviços que assegurem a oferta de serviços de saúde com qualidade e segurança. Em seguida, realizou-se um estudo de caso no Instituto Nacional de Traumatologia e Ortopedia (INTO)/Ministério da Saúde (MS), utilizando metodologia quantitativa e roteiros semiestruturados para estimar o tempo WISN), como testagem da metodologia promovida pela Organização Mundial da Saúde (OMS). Os resultados da análise da carga de trabalho e estimativa do dimensionamento de pessoal de saúde podem apoiar e nortear a elaboração e a implementação de políticas para melhorias na qualidade e na produtividade dos serviços de saúde. Chronic Pain and Exercise : Studies on pain intensity, biochemistry, adherence and attitudes

Chronic pain is common in western countries and entails considerable consequences for the afflicted individuals as well as for the society. Furthermore, chronic pain is complex including an advanced interplay between biological-, psychological- and social aspects. Treatment of chronic pain attempts to decrease pain intensity and increase physical-, psychological- and social functioning. However, the treatment of chronic pain is still not optimized. Different types of physical activity and exercise (PA&E) are commonly applied as non-pharmacological treatment strategies for chronic pain, but the most efficient type and dose of PA&E are unclear. In addition, adherence to prescribed PA&E is often troublesome, which further complicates the application of PA&E as treatment for chronic pain. The aim of this thesis is to increase the knowledge about PA&E as treatment for chronic pain regarding pain intensity, biochemical substances, adherence and attitudes. The findings of this thesis were that a long-term, home-based PA&E intervention comprising strength exercises as well as stretch exercises decreased pain intensity and increased function in women with chronic neck- and shoulder pain. Using microdialysis technique, differences in pain modulatory biochemical substances were found, before the intervention, in painful trapezius muscle compared to pain-free trapezius muscle. In addition, alterations in pain modulatory substances in painful trapezius muscle after the intervention were found, which possibly could imply peripheral physiological effects of PA&E. Furthermore, psychological factors could be associated to the effects of and adherence to the PA&E intervention. An intention to be physically active were expressed by patients with chronic pain, but a discordance between the intention and PA&E-behaviour were evident, even though the PA&E were experienced as valuable. In conclusion, this thesis strengthens the importance of PA&E as treatment for chronic pain. Especially, this thesis increases the knowledge about; possible peripheral pain inhibitory effects after long-term exercise; how psychological factors might affect the results of PA&E; and also about important behavioural aspects that might affect adherence to prescribed PA&E. This thesis highlights the need of more research on physiological pain inhibitory effects of long-term PA&E in chronic pain. Furthermore, improved methods for ensured adherence to prescribed PA&E are necessary in order to optimize the effect of PA&E as treatment for chronic pain.

Estudo em cadáveres sobre a associação entre os ângulos tomográficos do impacto fêmoro-acetabular e a histologia da lesão do lábio acetabular / Cadaveric study on the association between femoroacetabular impingement tomographic angles and the histology of labral tears

INTRODUÇÃO: O impacto fêmoro-acetabular (IFA) é considerado importante causa de dor do quadril no jovem, podendo evoluir para osteoartrite do quadril. O objetivo deste estudo foi investigar se há associação entre achados da tomografia computadorizada (TC) em cadáveres e lesões do lábio acetabular avaliadas histologicamente. MÉTODOS: Foram realizadas TCs de bacia de 20 cadáveres não formolizados. Os ângulos habituais relacionados ao IFA foram mensurados por um radiologista (ângulo alfa, versão femoral, versão acetabular, ângulo centro-borda e ângulo de Tönnis). O cadáver foi considerado portador de IFA tipo came quando apresentou um ângulo alfa maior que 50º. O cadáver foi considerado portador de IFA tipo pincer quando apresentou versão acetabular menor que 0º, ângulo centro-borda maior que 40º, ou ângulo de Tönnis menor que 0º. Os cadáveres foram então dissecados, o acetábulo foi ressecado, e três fragmentos foram obtidos da peça correspondendo às porções anterior, superior e posterior do rebordo acetabular. Após preparo histológico habitual, as lesões labiais foram avaliadas por um patologista. Quando presente, a lesão foi classificada segundo Seldes. RESULTADOS: A média de idade dos cadáveres foi de 50,2 anos ± 7,4. Foram estudados 13 homens e 7 mulheres. Dezesseis (80%) dos cadáveres foram considerados portadores de impacto tipo came. Oito (40%) cadáveres foram considerados portadores de impacto tipo pincer. Mulheres apresentaram maior proporção de IFA tipo pincer (21,4% dos homens apresentaram IFA tipo pincer e 83,3% das mulheres, p = 0,01). Histologicamente, 16 (80%) dos espécimes apresentaram lesão microscópica em pelos menos um fragmento. Ao dividirmos as regiões, os fragmentos anteriores apresentaram lesão labial em 65% dos casos, os fragmentos superiores 50% e os fragmentos posteriores em 25%. Segundo a classificação de Seldes, 60,7% das lesões foram do tipo 1, 28,6% foram do tipo 2, e 10,7% foram mistas. Indivíduos com lesão labial apresentaram um ângulo alfa maior (53,3º nos casos com lesão e 49,3º nos casos sem lesão, p = 0,01). Alterações tipo pincer não apresentaram associação com presença de lesões labiais (p > 0,05). Não foi encontrada associação entre pincer ou came e a classificação da lesão labial segundo Seldes (p > 0,05). CONCLUSÕES: Cadáveres com maior ângulo alfa apresentaram maior prevalência de lesão labial. Os demais ângulos analisados não apresentaram associação com a lesão labial. Nenhuma associação foi encontrada entre tipo de IFA e tipo de lesão labial segundo Seldes / INTRODUCTION: Femoroacetabular impingement (FAI) is an important cause of hip pain in the young patient, which may progress to hip arthritis. The purpose of this study was to investigate the relationship between computed tomographic findings in cadavers and histological labral findings. METHODS: Twenty fresh cadavers were submitted to a computed tomography (CT) scan. Standard FAI angles were measured, including alpha angle, femoral version, acetabular version, center-edge, and Tönnis angle. A cam lesion was defined as an alpha angle greater than 50º. A pincer lesion was defined as acetabular version less than 0º, or center-edge angle greater than 40º, or Tönnis angle less than 0º. Cadavers were dissected, the acetabulum was resected, and three fragments of each acetabuli were obtained corresponding to the anterior, superior and posterior acetabular rim. Following routine histological preparation, the histological slides were evaluated for possible labral tears. Tears were classified by Seldes type. RESULTS: The cadavers\' mean age was 50.2 years (SD: 7.4), 13 were males, 7 females. Sixteen (80%) specimens had a cam lesion. Eight (40%) specimens had a pincer lesion. Females were more likely to present a pincer lesion (21.4% males versus 83.3% females, p = 0.01). Histologically, 16 (80%) specimens had a labral tear in at least one region. When divided by region; anterior labra had lesions in 65% of cases; superior labra in 50% and posterior labra in 25%. According to Seldes classification, 60.7% of labral tears were type 1, 28.6% were type 2, and 10.7% were mixed. Patients who had a labral tear had significantly higher alpha angles (53.3º versus 49.3º, p = 0.01). Pincer lesions were not associated with labral tears (p > 0.05). We found no association between pincer or cam lesions and Seldes classification (p > 0.05). CONCLUSIONS: Cadavers presenting with higher alpha angles had higher prevalence of labral tears. Other FAI angles had no association with labral tears. No association was found between FAI type and Seldes classification
