Vários métodos são utilizados nas osteossínteses deste osso, tais como: pinos intramedulares; fixadores esqueléticos externos; placas ósseas e; associações entre eles. O método usado foi uma modificação da configuração denominada tie-in, na qual um PIM é conectado externamente a pinos de Schanz ou parafusos de aço inoxidável fixados com metil-metacrilato. O PIM é introduzido através de um orifício confeccionado na face medial da tíbia, próximo à crista tibia!. A configuração foi utilizada em 21 cães com fratura de tíbia atendidos no Hospital Veterinário do Câmpus da UNESP de Jaboticabal-SP. Os animais foram avaliados clínica e radiograficamente, no período pré e pós-operatório imediato e a cada 30 dias, até completar quatro meses. Os animais recuperaram, clinicamente, a função do membro no período médio de 11 dias após a cirurgia. Nove animais tiveram a configuração dinamizada, por meio da desconexão do PIM; remoção do PIM ou dos implantes de fixação e retirada dos implantes de fixação e manutenção do PIM. Radiograficamente verificou-se consolidação óssea em média de 76 dias de pós operatório, e apenas um animal apresentou não-união. A configuração tie-in mostrou se eficaz como método de osteossíntese em fraturas tibiais de cães. / The incidence of tibial fractures in dogs assumes 21 % of ali fractures that occur in long bones. Several methods are used in the osteosynthesis, of these fractures like external skeletal fixator, bone plates and associated techniques. The aim of this study is to describe the method and perform a radiographic and clinical evaluation of a group of 21 dogs with tibial fractures from the veterinary teaching hospital of the UNESP SÃO PAULO STATE UNIVERSITY- CAMPUS OF JABOTICABAL. The used method was a modification of the termed tie-in configuration, in that an IMP was connected externally with Schanz pins or stainless steel screws fixed together with methylmethacrylat. The pin was applied through hole on the medial tibia near the tibial crest. Clinical and radiographic evaluations were performed in pre-and post-operatory period and at 30, 60, 90 and 120 days. Animais recovered the function of the affected limb in a mean period of 11 days afier the surgery. Nine dogs underwent dynamization through: a) IMP disconnection, b) IMP or, c) fixation pins takeout and d) fixation pins takeout and maintenance of the IMP. Radiographicaly, the fracture union occurred in 20 dogs, in a mean period of 76 days; non-union was observed in only one dog. T~erefore, the tie-in modificated frame configuration showed efficacy as a method of osteosynthesis in tibial fractures of dogs.

The thesis is focused on the role of nurse in patient education prior anaesthesia in surgical procedures in orthopaedic. Description of this role is based on the experience at the orthopaedic clinic and problems related to it are seen in the results of survey. The aim was to assess the level and effectiveness of patient education done by nurse before anaesthesia, measure the level of satisfaction and patient awareness before surgery, assess the quality and content of education provided by nurse and identify the current competencies of nurses in patient education before anaesthesia. Data for the research survey were obtained through two questionnaires designed for nurses working at the orthopaedic clinic and for patients who have undergone scheduled intervention in this clinic. The most important finding of the research results was crossing the competencies of nurses in the preoperative patient education because the lack of visits by anaesthesiologist. Based on what I studied in the available literature and the research results, in the discussion I am presenting proposals for solutions of this situation and I suggest active offering of the information material to patient during admission, the creation of improved education material for preoperative preparation, using of different teaching methods...

Additive Manufacturing in Orthopedics and Craniomaxillofacial Surgery for the Development of High-risk Custom-made Implants : A Qualitative Study of Implementation Factors from a Multi-stakeholder Perspective / Implementering av Additiv Tillverkning i Ortopedi och Kranio- och käkkirurgi för Utveckling av Högrisk Patientspecifika Implantat : En Kvalitative Studie av Implementeringsfaktorer ur Intressenters PerspektivNioti, Antonia Evgenia January 2020 (has links)
Additive manufacturing (AM) has enabled the possibility for the hospitals to become their own implant producers developing implants that are tailored to patient’s anatomy. Despite the enormous potential of custom-made implants there are challenges that complicate the implementation of them into clinical practice. The aim of this research is to (1) identify the main driving forces and barriers for the delivery of custom-made implants; (2) explore staff stakeholder views and practices related to the implementation of AM in surgery for the development of custom-made implants; (3) formulate recommendations on how to cope with the implementation challenges. The research method was an explorative qualitative study consisted of a literature review on the challenges of custom-made implants in clinical applications coupled with the collection and inductive analysis of empirical data. The empirical study was based on ten semi-structured interviews conducted among both domestic and international hospital managers medical doctors and research engineers. The consolidated framework for implementation research (CFIR) was utilized for data collection. Using the five domains of CFIR, the following results were obtained: (1) Characteristics of individuals: Most research participants indicated a positive attitude towards the innovation expressing self-efficacy to its use; (2) Intervention characteristics: Custom-made implants were perceived to have a relative advantage in surgical practice due to their high degree of observability and geometrical adaptability providing increased surgical quality, perfect patient fit and better understanding of pathologies. However, high implementation costs, low degree of trialability and high degree of complexity in the development process were regarded as drawbacks of the innovation; (3) Outer setting: the regulatory uncertainty and lack of reimbursement limit the accessibility of custom-made implants to low income populations; (4) Inner setting: scarcity of resources, staff resistance to change, insufficient management support, communication difficulties, limited access to educational materials and training opportunities as well as lack of time and innovative capacity were regarded by the majority of participants as implementation barriers; (5) Process: central for the success of implementation is the need for a coherent implementation plan and evaluation process as well as the engagement of key stakeholders such as hospital managers, payers, regulatory and implementation advisors. This dissertation proffers a deeper understanding of the implementation issues related to custom-made implants and offers preliminary recommendations on how to cope with implementation impediments through the use of Rogers diffusion of innovation coupled with concepts from the field of organizational change and innovation management including Clayton’s disruptive innovation.
