Predictors of Post-injury Mortality in Elderly Patients with Trauma: A Master's Thesis

Background: Traumatic injury remains a major cause of mortality in the US. Older Americans experience lower rates of injury and higher rates of death at lower injury severity than their younger counterparts. The objectives of this study were to explore pre-injury factors and injury patterns that are associated with post-discharge mortality among injured elderly surviving index hospitalization. Methods: We queried a 5% random sample of Medicare beneficiaries (n=2,002,420) for any hospitalization with a primary ICD-9 diagnosis code for injury. Patients admitted without urgent/emergent admission were excluded, as well as patients presenting from inpatient hospitalization or rehabilitation. The primary endpoint was all-cause mortality. Patients were categorized into three mortality groups: death within 0-30 days, 31-90 days, or 91- 365 days post-discharge from the index hospitalization. These groups were compared with those who survived greater than one year post-discharge. Univariate tests of association and multivariable logistic regression models were utilized to identify factors associated with mortality during the 3 examined periods. Results: 83,439 elderly patients (4.2%) were admitted with new injuries. 63,628 met inclusion criteria. 1,936 patients (3.0%) died during their index hospitalization, 2,410 (3.8%) died within 0-30 days, 3,084 (4.8%) died within 31-90 days, and 5,718 (9.0%) died within 91- 365 days after discharge. In multivariable adjusted models, advanced age, male sex, and higher Elixhauser score were associated with post-discharge mortality. The presence of critical injury had the greatest effect on mortality early after injury (0-30 days, OR 1.81, CI 1.64-2.00). Discharge to anywhere other than home without services was associated with an increased odds of dying. Conclusions: Socio-demographic characteristics, disposition, and co-morbid factors were the strongest predictors of post-discharge mortality. Efforts to reduce injury-related mortality should focus on injury prevention and modification of co-morbidities.

Barriers to Healthcare Access and Patient Outcomes After a Hospitalization for an Acute Coronary Syndrome and Other Acute Conditions

Background: Guideline-concordant therapies for survivors of an acute coronary syndrome (ACS) hospitalization require healthcare access, something that millions of Americans lack. Methods and Results: Using data from a prospective cohort study of over 2,000 survivors of a hospitalization for an ACS in central Massachusetts and Georgia from 2011 to 2013, the first two aims of this thesis sought to identify the post-discharge consequences for survival and health status of having: 1) financial barriers to healthcare, 2) no usual source of care, and 3) transportation barriers. We found that patients lacking a usual source of care and having a transportation barrier were more likely to have died within two years following hospital discharge compared to those without such barriers. Also, patients with financial barriers to healthcare were more likely to experience clinically meaningful declines in physical and mental health-related quality of life over the six months after hospital discharge. The third aim sought to better understand factors influencing the success of care transitions home after an unplanned hospitalization through a qualitative study of 22 patients. Participants described how adequate healthcare access, particularly having insurance and transportation to clinical appointments, facilitated the receipt of follow-up care and adherence to treatments. Conclusions: Limitations in healthcare access may contribute to poorer survival, health-related quality of life, and survival. Additional research is needed to identify interventions to improve healthcare access and test whether improved access leads to better patient outcomes.

Introdução: O manejo inadequado dos pacientes com dor torácica sugestiva de síndrome coronariana aguda (SCA) é fator de risco para morte ou outros eventos cardíacos adversos maiores (ECAM), o que contribui para o aumento da morbidade e mortalidade, do estresse da equipe e do paciente e maiores custos com o uso de serviços de saúde. Os protocolos de dor torácica para alta precoce (protocolos de diagnósticos acelerados ADP) surgem como instrumentos de melhoria da qualidade da assistência em departamentos de emergência (DE), possibilitando alta precoce (em até 6 horas) e segura a esses pacientes. Objetivo: Sintetizar as evidências científicas sobre a efetividade de protocolos de dor torácica na alta precoce e segura de adultos com sintomas sugestivos de SCA em DE, quando comparados ao tratamento usual ou outros protocolos de dor torácica (usual ou de alta precoce). Métodos: Revisão Sistemática (RS) da literatura que seguiu as recomendações do Joanna Briggs Institute. Para a busca dos estudos foi utilizada a estratégia PICOS (P= adultos (> 18 anos) com dor torácica sugestiva de SCA; I e C= protocolos de dor torácica utilizados para alta precoce; O= alta precoce e ECAM em 30 dias; e, S= ensaios clínicos randomizados, nas bases de dados: CINAHL, PubMed, Cochrane CENTRAL, LILACS, Web of Science, Scopus, EMBASE, além de banco de teses e dissertações de universidades dos quatro continentes. Não foi determinado limite temporal para a busca e esta foi finalizada em 31 de janeiro de 2018. A seleção inicial, avaliação inicial e qualidade metodológica dos estudos foi realizada por dois revisores independentes e as discordâncias foram resolvidas por consenso. A extração dos dados dos estudos foi feita pelo revisor primário, analisada, discutida e apresentada de forma descritiva e com metanálise. Resultados: Incluídos quatro estudos do tipo ensaio clínico randomizado controlado, sendo que os protocolos para alta precoce utilizadas e testadas nesses estudos foram: ADAPT-ADP, HEART Pathway, EDACS-ADP, 2-Hour Protocol e 4-Hour Protocol. Porém, devido a alta hetergneidade (I=91%) apenas dois estudos foram agrupados em metanálise e os resultados foram apresentados com gráficos de floresta. Para a alta precoce a estimativa de efeito sugeriu que os benefícios da alta precoce superam os malefícios, recomendando o uso de protocolos acelerados de dor torácica na prática assistencial de departamentos de emergência. Para ECAM, a metanálise foi inconclusiva. Conclusão: Os protocolos estudados são efetivos na alta precoce, porém ainda não há evidência suficiente para afirmar que essa alta precoce seja segura. Contudo, essa RS é um primeiro passo importante para provar que sua utilização é viável e facilitará a alta precoce e segura de DE. / Introduction: Inadequate management of patients with chest pain suggestive of acute coronary syndrome (ACS) is a risk factor for death or other major adverse cardiac events (MACE), which contribute to increased morbidity and mortality, team and patient stress, and a higher expenses with health services. Early-onset chest pain protocols (accelerated diagnostic protocols - ADPs) appear as tools for improving the quality of care in emergency departments (ED), enabling early and safe discharge (up to 6 hours) for these patients. Objective: To synthesize the scientific evidences about the effectiveness of chest pain protocols in the early and safe discharge of adults with symptoms suggestive of ACS in ED, when compared to the usual treatment or other protocols of chest pain (usual or early discharge). Methods: Systematic Review (SR) of the literature that followed the recommendations of the Joanna Briggs Institute. In order to search for the studies, the PICOS strategy was used (P = adults (> 18 years) with chest pain suggestive of ACS, I and C = thoracic pain protocols used for early discharge; O = early discharge and MACE in 30 days and S = randomized clinical trials found in the following databases: CINAHL, PubMed, Cochrane CENTRAL, LILACS, Web of Science, Scopus, EMBASE, as well as a thesis and dissertations bank from universities of the four continents. A time limit was not determined to end this research; however this was finalized on January 31, 2018. The initial selection, initial evaluation and methodological quality of the studies were performed by two independent reviewers and the disagreements were resolved by consensus. Data extraction from the studies was done by the primary reviewer, analyzed, discussed and presented in a descriptive form with meta-analysis. Results: Four randomized controlled trials were included, the earlydischarge protocols used and tested in these studies were: ADAPT-ADP, HEART Pathway, EDACS-ADP, 2-Hour Protocol and 4-Hour Protocol. However, there is high heterogeneity between the studies (I = 91%), only two studies were grouped in meta-analysis and the results present by the funnel plot. For early discharge, the effect estimate suggested that the benefits of early discharge exceed the harm, recommending the use of accelerated chest pain protocols in emergency department assistance practice. For ECAM, the meta-analysis was inconclusive. Is not enough to affirm that early discharge is higher in the experimental groups who were discharged by the new protocol. Conclusion: The protocols studied are effective at early, however there is still insufficient evidence to affirm that this early discharge is safe. But this SR is an important first step to prove that its use is feasible and will facilitate the early and safe discharge of ED.

Duração da hospitalização e faturamento das despesas hospitalares em portadores de cardiopatia congênita e de cardiopatia isquêmica submetidos à intervenção cirúrgica cardiovascular assistidos no protocolo da via rápida / Duration of the hospitalization and hospital expenditures in teh congenital heart diseases and ischemic heart disease patients submited to cardiac surgical operations in fast track recovery

Com o objetivo de avaliar o atendimento dos pacientes submetidos à intervenção cirúrgica cardiovascular no protocolo de atendimento na via rápida (fast track recovery) em relação ao protocolo convencional, foi comparada a movimentação dos pacientes atendidos em ambos os protocolos nas diferentes unidades hospitalares. O estudo foi realizado em hospital público universitário especializado em cardiologia de 400 leitos, de referência terciária para o Sistema Único de Saúde. Foram estudados 175 pacientes, 107 (61%) homens e 68 (39%) mulheres, de idades entre 2 meses a 81 anos, dos quais 107 operados no protocolo da via rápida e 68 no protocolo convencional. Foram avaliadas variáveis demográficas, clínicas e, para avaliar a movimentação dos pacientes nas diferentes unidades hospitalares, as taxas de alta por unidade de tempo em cada unidade. A análise estatística foi feita por meio de análise exploratória, método de Kaplan Meier e modelo de riscos proporcionais de Cox. A análise de variância foi empregada para comparar o faturamento das despesas. A taxa de alta das diferentes unidades hospitalares por unidade de tempo dos portadores de cardiopatia congênita atendidos no protocolo da via rápida em relação ao protocolo da via convencional foi: a) 11,3 vezes a taxa de alta quando assistidos no protocolo na via convencional quanto ao tempo de permanência no centro cirúrgico; b) 6,3 vezes quanto à duração da intervenção cirúrgica; c) 6,8 vezes quanto à duração da anestesia; d) 1,5 vezes quanto à duração da perfusão; e) 2,8 vezes quanto à permanência na unidade de recuperação pós-operatória I; f) 6,7 vezes quanto à duração da hospitalização; g) 2,8 vezes quanto à permanência na unidade de internação pré-operatória; h) 2,1 vezes quanto à permanência na unidade de internação após a alta da unidade de terapia intensiva de recuperação pós-operatória. Para os portadores de cardiopatia isquêmica, as taxas de alta das unidades hospitalares para os protocolos de atendimento no protocolo da via rápida e no protocolo convencional não demonstraram diferença estatisticamente significante. Os valores de faturamento das despesas de internação dos portadores de cardiopatia congênita decorrentes de exames e procedimentos realizados nas fases pré- e pós-operatória e dos exames da fase trans-operatória foram menores quando os pacientes foram assistidos no protocolo da via rápida. Portanto, os portadores de cardiopatias congênita apresentaram menor permanência hospitalar nos recursos médicos hospitalares instalados, quando assistidos no protocolo de atendimento na via rápida, bem como menores despesas nas fases pré- e pós- operatória da internação. / Objective - To evaluate patient assistance in pre, per and postoperative phases of cardiac surgical intervention under fast track recovering protocol compared to the conventional way. Patients - 175 patients were studied, 107 (61%) men and 68 (39%) women. Ages 2 months to 81 years old. Patients included: first surgical intervention, congenital and ischemic cardiopathy without complexity, normal ventricular function and with at least 2 preoperative ambulatory consultations. Patients submitted to emergency surgeries were excluded. Interventions - assistance submitted by fast track and conventional protocol. Statistical analysis (measures) - exploratory, uni-varied (Kaplan Meier) and multi-varied (Cox) of the time in each admission unit. Hospital installations were classified in ambulatory, preoperative admission unit, surgical center, postoperative recovery unit and postoperative admission unit; the expression of this use was the discharge rate by unit of time from the significant interaction observed between assistance protocol and the kind of cardiopathy for the stay in the surgical center, surgical intervention time, stay in postoperative recovery unit, anesthesia time and time between admission and surgery dates. Results - the patients of congenital cardiopathy who underwent the protocol of conventional way recovery in relation to the fast track protocol, in the reliability range of 95% allows one to state that discharge rate by unit of time of the congenital cardiopathy patients assisted by the fast track protocol was: 11.3 times the discharge rate when assisted by the conventional way protocol as to the time of staying in the surgical center; 6.3 times as to the duration of the surgical intervention; 6.8 times as to duration of the anesthesia; 1.5 times as to the duration of the perfusion; 2.8 times as to the stay in the postoperative recovery unit; 6.7 times as to the stay in the hospital (period of time between the admission and the discharge date); 2.8 times as to the stay in the preoperative admission unit ( period of time between the admission date and the surgery date); 2.1 times as to the stay in the postoperative unit (period of time between the date of leaving the postoperative recovery unit and the date of discharge from the hospital). For the ischemia cardiopathy patients the risks concerning the protocols of recovery by the traditional way and the fast track were the same. CONCLUSIONS - The data concerning this study allows one to suggest that the assistance can be more efficient if one takes into consideration some variables studied in the protocol of fast track recovery. The congenital and ischemic cardiopathy patients presented shorter interval of time (concerning hospital stay in doctor-hospital installed facilities) when assisted in the fast track recovery protocol as well as fewer expenses with medical and hospital assistance.

O aumento da demanda de pacientes que necessitam deste tratamento fez com que fossem criados alguns modelos de transplante ambulatorial. A alta precoce é uma modalidade de transplante em que o paciente recebe alta hospitalar após o regime de condicionamento e infusão das células-tronco hematopoiéticas (CTH) e a continuidade do seu tratamento ocorre em regime ambulatorial. Na área da Enfermagem, o número limitado de estudos científicos relacionados à Assistência de Enfermagem nos pacientes submetidos ao TCTH com alta hospitalar precoce são ainda deficientes. Diante disso, o objetivo deste estudo foi analisar os resultados da alta hospitalar precoce como alternativa viável ao tratamento dos pacientes submetidos ao TCTHa e sua relação com a assistência de enfermagem. MÉTODO: Estudo retrospectivo, quantitativo, descritivo e transversal. Foram analisados prontuários de 112 pacientes consecutivos submetidos ao TCTHa, no período de janeiro a dezembro de 2009. Destes 12 pacientes não receberam alta hospitalar da unidade de internação até o décimo dia após o TCTH (D+10) e, por isso, foram excluídos, restando 100 pacientes. RESULTADOS: A mediana de idade foi de 48,5 anos (19-69 anos). Houve um pareamento não intencional do sexo. Todos os pacientes mobilizaram e coletaram CTH por fonte periférica. Os regimes de condicionamento mais utilizados foram BU12+Mel100 e BEAM 400. As toxicidades atribuídas ao regime de condicionamento foram bem conduzidas no ambulatório, expressa por 10 pacientes que necessitaram de internação, embora um grande número de pacientes da casuística apresentou algum grau de toxicidade. A neutropenia febril esteve presente em 58% dos pacientes até a enxertia medular. Não houve aumento na mortalidade na fase de aplasia medular; dois pacientes foram a óbito por causas infecciosas durante os 60 primeiros dias após o TCTH, sendo que apenas um não apresentava enxertia medular. A mediana de enxertia de granulócitos após o TCTHa com alta hospitalar precoce foi de 12 dias e de plaquetas 15 dias, com mediana de transfusões até a alta do serviço de três concentrados de hemácias e quatro concentrados de plaquetas. Vinte e três pacientes necessitaram de internação hospitalar em algum momento desde a alta hospitalar após o transplante até o momento de sua alta. CONCLUSÃO: A equipe de enfermagem apresenta papel fundamental no contexto da alta hospitalar precoce na conduta e manejo dos pacientes. O Enfermeiro participou na orientação e condutas durante a fase de mobilização, transplante e acompanhamento ambulatorial. A mediana de tempo para enxertia medular foi de 12 dias e durante a fase de aplasia os pacientes evoluíram com baixa internação e infecção. Houve baixa incidência de complicações e internações, sendo a toxicidade ao regime de condicionamento a maior causa de internação. The increased number of patients who need this treatment leads to new models of outpatient transplant. The early discharge is a type of transplant in which the patient is discharged after the conditioning regimen and infusion of hematopoietic stem cells (HSC) and the continuity of your treatment will occur in outpatient settings. Although the models of outpatient HSCT are well defined, there is few studies and publications that demonstrate the actual results of this modality. In the field of nursing, the limited number of scientific studies related to nursing care in HSCT patients with early hospital discharge are even more deficient. Thus, the aim of this study was to analyze the results of early discharge as a viable alternative to the treatment of patients undergoing HSCTa and its relationship to nursing care. Methods: A retrospective, quantitative, descriptive and cross study was performed. A total of 112 patients initially enrolled, 12 were excluded due to the discharged occurred after than tenth day after HSCTa (D +10) and, therefore, 100 patients were enrolled in the study. Results: The median age was 48.5 years (range: 19-69 years). There was an unintentional pairing of sex. All patients were mobilized and collected by HSC peripheral source. The conditioning regimens were used more BU12 + Mel100 and BEAM 400. The conditioning regimen-related toxicities was well at the clinic, expressed by 10 patients who required hospitalization, although a large number of patients in the sample had some degree of toxicity. Febrile neutropenia was observed in 58% of patients until the marrow engraftment. There was no increase in mortality in bone marrow aplasia phase, two patients (2%) died of infectious causes during the first 60 days after HSCTa, and only one patient showed no engraftment. The median granulocyte engraftment after HSCTa with early hospital discharge was 12 days and platelets 15 days, with a median transfusion until discharge from the service three and four units of blood transfused platelet concentrates. Twenty-three patients required hospitalization at some time from hospital discharge after transplantation until the time of his discharge. Conclusion: The nursing team has key role in the context of early hospital discharge in the conduct and management of patients. The nurse participated in the orientation and conduct during the mobilization phase, and outpatient transplant. The median time to engraftment was 12 days and during the aplasia phase of the patients improved, with low infection and hospitalization. There was a low incidence of complications and hospitalizations, and the toxicity conditioning regimen the leading cause of hospitalization. The toxicities presented to the conditioning regimen were well managed on an outpatient basis also for the Nursing Team

Febrile neutropenia was observed in 58% of patients until the marrow engraftment. There was no increase in mortality in bone marrow aplasia phase, two patients (2%) died of infectious causes during the first 60 days after HSCTa, and only one patient showed no engraftment. The median granulocyte engraftment after HSCTa with early hospital discharge was 12 days and platelets 15 days, with a median transfusion until discharge from the service three and four units of blood transfused platelet concentrates. Twenty-three patients required hospitalization at some time from hospital discharge after transplantation until the time of his discharge. Conclusion: The nursing team has key role in the context of early hospital discharge in the conduct and management of patients. The nurse participated in the orientation and conduct during the mobilization phase, and outpatient transplant. The median time to engraftment was 12 days and during the aplasia phase of the patients improved, with low infection and hospitalization. There was a low incidence of complications and hospitalizations, and the toxicity conditioning regimen the leading cause of hospitalization. The toxicities presented to the conditioning regimen were well managed on an outpatient basis also for the Nursing Team

Duração da hospitalização e faturamento das despesas hospitalares em portadores de cardiopatia congênita e de cardiopatia isquêmica submetidos à intervenção cirúrgica cardiovascular assistidos no protocolo da via rápida / Duration of the hospitalization and hospital expenditures in teh congenital heart diseases and ischemic heart disease patients submited to cardiac surgical operations in fast track recovery

Com o objetivo de avaliar o atendimento dos pacientes submetidos à intervenção cirúrgica cardiovascular no protocolo de atendimento na via rápida (fast track recovery) em relação ao protocolo convencional, foi comparada a movimentação dos pacientes atendidos em ambos os protocolos nas diferentes unidades hospitalares. O estudo foi realizado em hospital público universitário especializado em cardiologia de 400 leitos, de referência terciária para o Sistema Único de Saúde. Foram estudados 175 pacientes, 107 (61%) homens e 68 (39%) mulheres, de idades entre 2 meses a 81 anos, dos quais 107 operados no protocolo da via rápida e 68 no protocolo convencional. Foram avaliadas variáveis demográficas, clínicas e, para avaliar a movimentação dos pacientes nas diferentes unidades hospitalares, as taxas de alta por unidade de tempo em cada unidade. A análise estatística foi feita por meio de análise exploratória, método de Kaplan Meier e modelo de riscos proporcionais de Cox. A análise de variância foi empregada para comparar o faturamento das despesas. A taxa de alta das diferentes unidades hospitalares por unidade de tempo dos portadores de cardiopatia congênita atendidos no protocolo da via rápida em relação ao protocolo da via convencional foi: a) 11,3 vezes a taxa de alta quando assistidos no protocolo na via convencional quanto ao tempo de permanência no centro cirúrgico; b) 6,3 vezes quanto à duração da intervenção cirúrgica; c) 6,8 vezes quanto à duração da anestesia; d) 1,5 vezes quanto à duração da perfusão; e) 2,8 vezes quanto à permanência na unidade de recuperação pós-operatória I; f) 6,7 vezes quanto à duração da hospitalização; g) 2,8 vezes quanto à permanência na unidade de internação pré-operatória; h) 2,1 vezes quanto à permanência na unidade de internação após a alta da unidade de terapia intensiva de recuperação pós-operatória. Para os portadores de cardiopatia isquêmica, as taxas de alta das unidades hospitalares para os protocolos de atendimento no protocolo da via rápida e no protocolo convencional não demonstraram diferença estatisticamente significante. Os valores de faturamento das despesas de internação dos portadores de cardiopatia congênita decorrentes de exames e procedimentos realizados nas fases pré- e pós-operatória e dos exames da fase trans-operatória foram menores quando os pacientes foram assistidos no protocolo da via rápida. Portanto, os portadores de cardiopatias congênita apresentaram menor permanência hospitalar nos recursos médicos hospitalares instalados, quando assistidos no protocolo de atendimento na via rápida, bem como menores despesas nas fases pré- e pós- operatória da internação. / Objective - To evaluate patient assistance in pre, per and postoperative phases of cardiac surgical intervention under fast track recovering protocol compared to the conventional way. Patients - 175 patients were studied, 107 (61%) men and 68 (39%) women. Ages 2 months to 81 years old. Patients included: first surgical intervention, congenital and ischemic cardiopathy without complexity, normal ventricular function and with at least 2 preoperative ambulatory consultations. Patients submitted to emergency surgeries were excluded. Interventions - assistance submitted by fast track and conventional protocol. Statistical analysis (measures) - exploratory, uni-varied (Kaplan Meier) and multi-varied (Cox) of the time in each admission unit. Hospital installations were classified in ambulatory, preoperative admission unit, surgical center, postoperative recovery unit and postoperative admission unit; the expression of this use was the discharge rate by unit of time from the significant interaction observed between assistance protocol and the kind of cardiopathy for the stay in the surgical center, surgical intervention time, stay in postoperative recovery unit, anesthesia time and time between admission and surgery dates. Results - the patients of congenital cardiopathy who underwent the protocol of conventional way recovery in relation to the fast track protocol, in the reliability range of 95% allows one to state that discharge rate by unit of time of the congenital cardiopathy patients assisted by the fast track protocol was: 11.3 times the discharge rate when assisted by the conventional way protocol as to the time of staying in the surgical center; 6.3 times as to the duration of the surgical intervention; 6.8 times as to duration of the anesthesia; 1.5 times as to the duration of the perfusion; 2.8 times as to the stay in the postoperative recovery unit; 6.7 times as to the stay in the hospital (period of time between the admission and the discharge date); 2.8 times as to the stay in the preoperative admission unit ( period of time between the admission date and the surgery date); 2.1 times as to the stay in the postoperative unit (period of time between the date of leaving the postoperative recovery unit and the date of discharge from the hospital). For the ischemia cardiopathy patients the risks concerning the protocols of recovery by the traditional way and the fast track were the same. CONCLUSIONS - The data concerning this study allows one to suggest that the assistance can be more efficient if one takes into consideration some variables studied in the protocol of fast track recovery. The congenital and ischemic cardiopathy patients presented shorter interval of time (concerning hospital stay in doctor-hospital installed facilities) when assisted in the fast track recovery protocol as well as fewer expenses with medical and hospital assistance.

Long-Term Survival and Prognostic Factors in Patients with Acute Decompensated Heart Failure According to Ejection Fraction Findings: A Population-Based Perspective: A Master Thesis

Limited data exists describing the long-term prognosis of patients with acute decompensated heart failure (ADHF) further stratified according to currently recommended ejection fraction (EF) findings. In addition, little is known about the magnitude of, and factors associated with, long-term prognosis for these patients. Based on previously validated and clinically relevant criteria, we defined HF-REF as patients with an EF value ≤40%, HF-PEF was defined as an EF value > 50%, and HF-BREF was defined as patients with an EF value during their index hospitalization between 41 and 49%. The hospital medical records of residents of the Worcester (MA) metropolitan area who were discharged after ADHF from all 11 medical centers in central Massachusetts during the 5 study years of 1995, 2000, 2002, 2004, and 2006 were reviewed. Follow-up was completed through 2011 for all patient cohorts. The average age of this population was 75 years, the majority was white, and 44% were men. Patients with HF-PEF experienced higher post discharge survival rates than patients with either HF-REF or HF-BREF at 1, 2, and 5-years after discharge. Advanced age and lower estimated glomerular filtration rate findings at the time of hospital admission were important predictors of 1-year death rates, irrespective of EF findings. Previously diagnosed chronic obstructive pulmonary disease, chronic kidney disease, and atrial fibrillation were associated with a poor prognosis in patients with PEF and REF whereas a history of diabetes was an important prognostic factor for patients with REF and BREF. In conclusion, although improvements in 1-year post-discharge survival were observed for patients in each of the 3 EF groups examined to varying degrees, the post- 7 discharge prognosis of all patients with ADHF remains guarded. In addition, we observed differences in several prognostic factors between patients with ADHF with varying EF findings, which have implications for more refined treatment and surveillance plans for these patients.

Ontario’s Home First Approach, Care Transitions, and the Provision of Care: The Perspectives of Home First Clients and Their Family Caregivers

Home First is an Ontario transition management approach that attempts to reduce the pressure on hospital and Long Term Care (LTC) beds through early discharge planning, the provision of timely and appropriate home care, and the delay of LTC placement. The purpose of this qualitative descriptive study was to obtain descriptions from South Eastern Ontario Home First clients and their family caregivers of their experiences with and thoughts about care transitions, the provision of care, and the Home First approach. The goal was to enable insight into the Home First approach, care transitions, and the provision of care through access to the perspectives of study participants. Nine semi structured interviews (and one or more follow-up calls for each interview) with Home First clients discharged from hospitals in South East Ontario and their family caregivers were conducted and their content analyzed. All participating Home First clients were pleased to be home from hospital and did not consider LTC placement a positive option. All had family involved with their care and used a mix of formal and informal services to meet their care needs. Four general themes were identified: (a) maintaining independence while responding (or not) to risks, (b) constraints on care provision, (c) communication is key, and (d) relationship matters. Although all Home First clients participating in the study were discharged home successfully, a sense of partnership between health care providers, families, and clients was often lacking. The Home First approach may be successfully addressing hospital alternative level of care issues and getting people home where they want to be, but it is also putting increasing demands on formal and informal community caregivers. There is room for improvement in how well their needs and those of care recipients are being met.
