Improving cultural diversity awareness of physical therapy educators

In this climate of increasing diversity in the population of patients requiring Physical Therapy (PT) services, PT educators should prepare students and future clinicians to work competently in culturally diverse environments. To be able to achieve this goal, PT educators should be culturally competent as well. The purpose of this study was to develop and implement a workshop aimed at improving cultural diversity awareness of the University of the Pacific (UOP) PT educators. The seminar content and class materials were developed by the author in close consultation with the workshop facilitator, who is an expert on cultural diversity. These materials were validated by Physical Therapists and cultural diversity experts. To determine the effectiveness of the workshop in improving cultural diversity awareness, the Cultural Diversity Awareness Questionnaire (CDAQ) was developed, validated for content, analyzed for reliability, field tested and pilot tested. Results indicated that the instrument was valid and reliable. The one-day workshop was presented to twelve academic and clinical faculty of the UOP PT Department. The one group pretest-posttest experimental design was used, with the participants completing the CDAQ before and after the workshop. The attendees also completed a course evaluation at the conclusion of the seminar. Results showed a statistically significant positive change in the pretest/posttest results. This indicated that the workshop was effective in improving cultural diversity awareness of the participants. Results of the workshop evaluation affirmed the achievement of the educational objectives and effectiveness of the facilitator. This study provided-a-solid-initial-foundation-on-which a-comprehensive cultural competence program can be developed.

“Ergonomics for Occupational Therapy Practitioners” (EOTP) is an on-demand, on-line course for occupational therapy practitioners (OTPs), as well as other therapy professionals. As many OTPs had limited exposure to ergonomics in their academic education (Fisher, 2019), EOTP is intended to equip them for ergonomic practice. This 10-module on-line course provides ergonomic principles and application for a variety of environments, leveraging adult learning theory and evidence-based curriculum design. The program’s ultimate goal is to increase the number of OTPs working in the area of ergonomics by increasing their knowledge of and confidence in this distinct practice area.

Bakgrund: Kommunikationssvårigheter inom vården är vanligt förekommande och bidrar till många utmaningar vilket påverkar vårdkvalitén. Detta ställer höga krav på vårdpersonal, patient och deras omgivning. Det finns en kunskapslucka gällande forskning inom barnhabilitering utifrån fysioterapeuters perspektiv. Syfte: Syftet med denna studie var att undersöka fysioterapeuters upplevelser av att bemöta barn i skolålder (6–18 år) inom habilitering som har kommunikationssvårigheter. Metod: För denna studie användes en kvalitativ design i beskrivande intervjuform. Data analyserades genom kvalitativ innehållsanalys. Resultat: Fyra huvudkategorier kunde identifieras utifrån fysioterapeuternas svar: “Känslor kopplade till lyckad och misslyckad kommunikation”, “Fysioterapeuternas tillvägagångssätt för att hantera kommunikation med barn”, “Kommunikationens betydelse i mötet med barn” och “Omständigheter som påverkar kommunikationen”. Detta innefattade bland annat att en fungerande kommunikation är särskilt beroende av fysioterapeutens tillvägagångsätt, kunskap och erfarenheter inom området. Det framkom att kommunikationen hade betydelse för barnets delaktighet och effekten av habiliteringen. Slutsats: Studien har visat att kommunikationssvårigheter hos barn skapar utmaningar för fysioterapeuter och har betydelse för barnets delaktighet och effekten av habiliteringen. Fungerande kommunikation behövs för ett gynnsamt samarbete och effektiv och anpassad behandling för barnet. Fler studier behövs för att öka förståelsen och kunskapen inom området. / Background: Communication deficit is a commonly occouring condition whitin healthcare and often creates challenges and puts higher demands on both healthcare workers, patients and their surroundings. This affects the quality of care and the received treatment. There is a lack of studies from a physiotherapeutic perspective working with habilitation for children.  Aim: The aim of this study was to explore experiences of physiotherapists of working with children (6-18 years) with communication deficits within habilitation. Method: Qualitative study design was employed and data were collected by semi-structured interviews with six physiotherapists. Data were analysed based on qualitative content analysis with inductive approach. Result: Based on the physiotherapists´ narrative four categories emerged: “Feelings connected to successful and unsuccessful communication”, “Physiotherapists´ approach of handling communication with children”, “The importance of communication when working with children” and “Emerged circumstances that affects communication”. The physiotherapists reported that non-functioning communication had a direct effect on the child’s participation and the treatments’ outcome. Conclusion: Physiotherapists experienced that communictaion deficit affects the outcome of habilitation and the participation of the child. Their knowledge of communication strategies and their ability to include and collaborate with the child and their surroundings were aspects for successful communication. Further research is needed to gain a better understanding of the results as current evidence within this field is limited.

Investigations on the subject emerged in the twentieth century and continue to be hot topic due to its complexity and importance. Many studies related to work motivation address both extrinsic as intrinsic aspects inherent to the process and are conducted in various productive sectors. However, it is proposed in this dissertation, we identify factors of influence for the motivation of physiotherapists working in clinics in João Pessoa, PB. The survey drew on the quantitative method with exploratory and descriptive approach, built from a literature review and a field study conducted by questionnaire. The research was composed of physiotherapists working in the thirty-seven clinics for physiotherapy in the city. Among the population raised from sixty-three, the sample consisted of forty-four volunteers. The collected data were recorded and analyzed using SPSS 18.0 (Statistical Packpage of Social Science) version for Windows, by using the frequency calculation, reduction tests of variables, tests of dependency such as correlation, regression analysis, and testing for consistency analysis of the items. The results indicated that the nature of the task factors, recognition and professional autonomy, the personal and professional achievement and interpersonal relationships, are influential factors in the motivation of physiotherapists working in clinics in João Pessoa PB. / A motivação humana para o trabalho é um aspecto que interfere no processo das relações humanas, na qualidade de vida e na produtividade. As investigações sobre o tema surgiram no século XX e continuam sendo assunto atual dada sua complexidade e importância. Muitos estudos relacionados à motivação no trabalho abordam tanto os aspectos intrínsecos quanto os extrínsecos inerentes ao processo e são realizados nos mais variados setores produtivos. Entretanto, é proposta desta dissertação, identificar os fatores de influência para a motivação dos fisioterapeutas que trabalham nas clínicas de João Pessoa, PB. A pesquisa valeu-se do método quantitativo com enfoque exploratório descritivo, construída a partir de um levantamento bibliográfico e um estudo de campo realizado através da aplicação de questionário. O universo da pesquisa foi composto pelos fisioterapeutas que trabalham nas trinta e sete clínicas de fisioterapia da cidade. Entre a população levantada de sessenta e três profissionais, a amostra constituiu-se de quarenta e quatro voluntários. Os dados coletados foram registrados e analisados utilizando o software SPSS 18.0 (Statistical Packpage of Social Science), versão for Windows, por meio da utilização do cálculo de freqüências, testes de redução de variáveis, testes de dependência como os de correlação, análise de regressão, além de testes para análise de consistência dos itens. Os resultados indicaram que os fatores natureza da tarefa, reconhecimento e autonomia profissional, a realização pessoal e profissional e o relacionamento interpessoal, são fatores influenciadores na motivação dos fisioterapeutas que trabalham nas clínicas de João Pessoa PB.

Background According to international study results, physical therapists are afflicted with work-related musculoskeletal, psychosocial and dermal disorders as well as infections. The few existing studies in German-speaking regions focus mainly on dermal and psychosocial exposures and resulting complaints. An overview of all relevant work-related exposures and complaints of physical therapists is currently lacking. We sought to identify work-related exposures based on the subjective experiences and beliefs of physiotherapeutic representatives, in order to identify relevant work-related complaints and diseases. Likewise we aimed to compare the international evidence with the actual situation of physical therapists in Germany. Methods Two complementary qualitative approaches were used: 1) a focus group discussion with representatives of professional physiotherapy associations as well as health and safety stakeholders and 2) qualitative semi-structured telephone interviews incorporating currently employed physical therapists. The group discussion was conducted applying a moderation technique, and interviews were analyzed using the content analysis approach by Mayring. Results The focus group discussion with five participants and the 40 semi-structured interviews with physical therapists identified comparable results. The main exposures of physiotherapeutic work were considered to be musculoskeletal (e.g., awkward body postures during treatment, patient transfers, passive mobilization), psychosocial (e.g., statutory audit of prescriptions and the associated conflicts with doctors and health insurance providers) and partly dermal and infectious (e.g., wet work and risk of infection) factors. Diseases of the spine, wrist or finger joints, burnout syndrome and infections were mentioned as possible consequences. Conclusions The subjective data generated by both groups (focus group discussion and interviews) were comparable and consistent with the current state of research. The results provide new insight regarding work-related exposures and diseases of physical therapists working in Germany. These findings aided the design of a German-wide representative survey of practicing physical therapists.

Moreover, for the beginner group, sought to stimulate reflection on their actions, as well as implement measures in humanizing care through education. The participants were twenty physiotherapists post-graduate students enrolled in the specialization course of physiotherapy in intensive care at a public university in São Paulo, who deal directly with people with physical disabilities acquired in hospital environment in the intensive care unit. As an instrument of data collection were used: questionnaire prepared by the researcher according to the research objectives, and a second step, thematic meetings were given to members of the group beginner with presentation of topics related to physical disability and acquired process of humanization and interdisciplinarity. The results were tabulated and categorized, and then subjected to content analysis. As a result, research has shown that physiotherapists are not informed and updated on issues that lie outside the technical area, which relate to life and social participation of people with disabilities. In addition, the students themselves physiotherapists little quarrel among themselves on the subject, do not bother to seek or deepen their knowledge and need for more clarity on the subject so that they can act more interdisciplinarity and humanization. For students who participated in the thematic meetings, managed to rescue the true meaning of its practice, the value of their work and teamwork and the pursuit of improving their relationships with each other, with patients, so that the dimension is valued human and subjective of all involved. The results also point to some factors that can interfere with the achievement of this more interdisciplinary and humanized care. From the issues raised here, we emphasize the importance of more research and scientific production in search of better training of physical therapists and can be performed with the primary goal of putting people with physical disabilities and acquired an interdisciplinary approach / humanized foreground. / Este trabalho procurou descrever e analisar como o atendimento interdisciplinar/humanizado à pessoa com deficiência física adquirida vem sendo percebido pelo profissional fisioterapeuta pós-graduando atuante no ambiente de UTI, levando em consideração a experiência pessoal do fisioterapeuta e as informações adquiridas no decorrer de sua formação acadêmica. Participaram da pesquisa vinte fisioterapeutas pós-graduandos matriculados em curso de especialização de fisioterapia em terapia intensiva de uma universidade pública de São Paulo, sendo dez concluintes e dez iniciantes, que lidam diretamente com pessoas com deficiência física adquirida internadas em ambiente de unidade de terapia intensiva. Como instrumento de coleta foram utilizados: primeira etapa aplicação de questionário e, em uma segunda etapa, foram ministrados encontros temáticos para os integrantes do grupo iniciante com apresentação de temas relacionados à deficiência física adquirida e processo de humanização e interdisciplinaridade. Os resultados obtidos foram tabulados e categorizados, sendo posteriormente submetidos à análise de conteúdo. Como resultado, a pesquisa mostrou que os fisioterapeutas não estão informados e atualizados sobre temas que fogem da área técnica e que se referem à vida e participação social da pessoa com deficiência. Além disso, os próprios alunos fisioterapeutas pouco discutem entre si sobre o tema, não se preocupam em buscar ou aprofundar o conhecimento e necessitam de maior clareza sobre o assunto para que possam atuar com mais interdisciplinaridade e humanização. Para os alunos que participaram dos encontros temáticos, conseguiram resgatar o verdadeiro sentido de sua prática, na implementação de novas medidas humanizadoras no âmbito assistencial, no valor do trabalho individual e em equipe e a busca pelo aprimoramento das relações que estabelecem entre si, com os pacientes, de modo que seja valorizada a dimensão humana e subjetiva de todos os envolvidos. Os resultados também apontam alguns fatores que podem interferir na concretização deste atendimento mais interdisciplinar e humanizado. A partir de questões aqui levantadas, ressalta-se a importância de maior número de pesquisa e produção científica na busca de melhor formação do fisioterapeuta e que possam ser realizadas com o primordial objetivo de colocar a pessoa com deficiência física adquirida e atendimento interdisciplinar/humanizado em primeiro plano.

Predicting performance on the Physical Therapist Assistant licensure examination

The purpose of this study was to determine if there was any relationship between student success as defined by grades in earned in basic sciences of anatomy and physiology as well as overall college grade point average with the score on the National Physical Therapy Examination (NPTE) for Physical Therapist Assistant (PTA) students. This pilot study examined the first two cohorts of PTA students at the College of the Sequoias from 2012 and 2013. Correlations and multiples regressions were the primary quantitative methods used to determine which prerequisites were most likely to predict passing the NPTE. Demographic information of ethnicity and age, did not show any significant correlation with score on the NPTE-PTA. Further analysis determined that both anatomy and overall grade point average had statistical correlations. However, overall college grade point average was the only variable that contributed significantly to the NPTE score. Therefore, overall grade point average was used to develop a predictive admission formula that can be used for further research and analysis. The procedures for this pilot study can be used to duplicate the study on a larger scale and continue to build a predictive admission formula that has the potential for establishing a consistent admission criterion for PTA programs. The long range goal of this study has been to help identify students who have the greatest potential to pass the NPTE and become employed as a PTA.

Introduktion: En främre korsbandsskada är en vanlig idrottsskada där många efter rehabiliteringen inte återgår till sin tidigare aktivitetsnivå. Förutom nedsatt knäfunktion kan många faktorer spela in i valet att återgå eller inte exempelvis motivation, rädsla och identitetsförlust. Studiens syfte var att utforska fysioterapeuters tankar, erfarenheter och rutiner kring psykosocial hälsa hos patienter med en främre korsbandsskada med målet om att återgå till tidigare idrott. Metod: Studien har genomförts genom en kvalitativ intervjustudie med en induktiv ansats samt en semistrukturerad intervjuguide. Fem fysioterapeuter som aktivt arbetade med främre korsbandsskador inkluderades. Intervjuerna analyserades utifrån innehållsanalys enligt Graneheim och Lundman. Resultat: Innehållsanalysen resulterade i ett tema; Psykosocial hälsa, en gråzon för fysioterapeuter med tre kategorier; Psykosociala faktorer påverkar patientens rehabilitering, En god rehabilitering kräver engagemang och ett helhetsperspektiv, Delad syn på betydelsen av psykosocial hälsa inom rehabilitering. Resultatet visar på en medvetenhet om att en korsbandsskada kan påverka patientens psykosociala hälsa negativt, men hur fysioterapeuterna arbetar med psykosocial hälsa skiljer sig åt. Slutsats: Psykosocial hälsa är en synlig faktor i rehabiliteringen av en främre korsbandsskada och är ett område där fysioterapeuterna saknar rutiner, resurser och tillräcklig kunskap. Det lyfts en delad åsikt hos fysioterapeuterna kring vem som ansvarar för patientens psykosociala hälsa. Den här studien illustrerar en brist på tydlighet kring fysioterapeutens roll vid psykosocial hälsa i rehabiliteringen och kan ligga till grund för framtida forskning.
