Posttraumatic Stress Disorder (PTSD) in the General Population

<p>This thesis explored the epidemiology of Posttraumatic Stress Disorder (PTSD) and different aspects of the disorder. Firstly, we investigated the lifetime prevalence of traumatic experiences and PTSD in the general adult population in Sweden and evaluated the impact of different trauma types, trauma frequency, and perceived distress. The results show that traumatic experiences are common and PTSD is not rare; roughly one out of ten traumatic events results in PTSD, with a 5.6% lifetime prevalence. The female/male ratio is 2:1. The risk for PTSD increases considerably with a high trauma-associated emotional impact. The distressing impact of a given trauma appears to be higher in women than in men, indicating an increased vulnerability in women. Secondly, we hypothesized that traffic road accidents (TRA’s) are one of the most prevalent types of traumatic events in Swedish society; therefore, we examined the impact of event and response characteristics associated with TRA’s on PTSD development. The data demonstrate that of those who had experienced a TRA (n=1074, 58.9%), 6.1% reported lifetime PTSD. TRA’s associated with fatal accidents and injury to oneself and related to high distress more than double the risk for PTSD. Thirdly, we compared the relative merits of the DSM-IV’s three-factor solution for PTSD symptoms to alternative models. We found that the symptomatology is equally well accounted for using all factor analytic models as yet presented in the literature; the DSM-IV, we found, provides as good a fit to data as other models. Fourthly, we examined the neurofunctional correlates of PTSD symptoms and whether a treatment-induced (serotonin reuptake inhibitor - SSRI) reduction of PTSD symptoms is associated with altered rCBF during symptom provocation. Our results indicate that PTSD symptoms correlates with areas involved in memory, emotion, attention, and motor control and that SSRI treatment normalizes provocation-induced rCBF in these areas.</p>

L'influence des traumas psychologiques sur l'intervention en toxicomanie

Il est possible qu’une personne qui consulte pour un problème d’utilisation de substances souffre également d’un problème associé à un état de stress post-traumatique ( ESPT ). Plusieurs auteurs croient qu’il existe un lien expliquant cette co-morbidité. Voilà pourquoi plusieurs hypothèses ont été formulées pour expliquer le lien entre un trouble tel que celui lié à l’utilisation de substance (TUS) pouvant exister avant l’ESPT, la vulnérabilité suite au TUS, ainsi que l’automédication. Plusieurs recherches proposent deux types d’interventions afin d’aider les personnes aux prises avec cette double problématique. La première consiste à traiter les deux problèmes séparément alors que la seconde, préconise une approche intégrée et simultanée. Puisque les écrits conseillent davantage un traitement intégré, il y aura une présentation d’une de ces formes de traitement nommé « À la recherche de la sécurité ». Le volet académique visait à comprendre le lien existant entre le TUS, les traumas psychologiques et l’ESPT ainsi que les différentes formes d’interventions pour aider les personnes ayant cette co-morbidité. Nous avons utilisé un module du programme « À la recherche de la sécurité » récemment traduit en français afin de vérifier sa mise en application et l’appréciation qu’en ont fait cinq usagers du Centre Dollar-Cormier-Institut universitaire sur les dépendances (CDC-IUD) ayant vécu un traumatisme au cours de leur vie. En conclusion, il sera démontré dans nos résultats que les hypothèses quant au lien de comorbidité sont effectivement présentes dans la réalité des personnes ayant un ESPT et un TUS, et qu’une intervention intégrée offre des points positifs autant pour l’usager que pour l’intervenant. / It is possible that the substance abuse disorder for which a person consults is associated with Posttraumatic Stress Disorder (PTSD). Several authors believe that there is a link explaining this co-morbidity. For this reason, several hypotheses have been formulated to explain this link as the substance abuse disorder (SUD) prior to PTSD, vulnerability following the SUD and selfmedication. Moreover, research shows the various interventions to help people struggling with these twin problems. The first is to separate the two issues while the second includes PTSD and the SUD. Research advice a more integrated treatment, there will be a presentation of one of these forms of treatment called "Seeking Safety". The academic component of our apprenticeship was to understand the link between the SUD, psychological trauma and PTSD and the various forms of interventions to help people with this co-morbidity. We used a module of "Seeking Safety" recently translated in French to ensure its implementation and its assessment from five users of the Centre Dollar-Cormier-Institut universitaire sur les dépendances who experienced trauma in their lives. In conclusion of the study, it is clearly shown that different assumptions are present in the reality of people with PTSD and SUD and an integrated response has positives for both the user and for the speaker.

La consommation d’alcool et de drogue chez les étudiants suite à la fusillade de Dawson en 2006 : une analyse différenciée selon le sexe

Objectifs : Étudier l’incidence de la dépendance à l’alcool ou aux drogues chez les étudiants exposés à la fusillade du Collège Dawson dans les 18 mois suivant celle-ci. Identifier les précurseurs au développement d’une dépendance à une substance psychoactive en tenant compte de la sévérité d’exposition à l’événement. Examiner si la consommation d’alcool 18 mois après les événements est en lien avec les différents groupes de symptômes d’état de stress post-traumatique. Méthode : La population à l’étude est composée de l’ensemble des étudiants du Collège Dawson au moment de l’événement. Les analyses ont été faites auprès de 854 étudiants inscrits au Collège au moment de la fusillade. Résultats : Cinq pourcent des femmes et 7 % des hommes présentent pour la première fois de leur vie un problème de dépendance à une substance suite à la fusillade. Pour les hommes, leur jeune âge, la présence de pensées suicidaires au cours de leur vie, ainsi que le fait d’avoir vu le tireur au moment de la fusillade sont les principaux précurseurs de cas incidents de dépendance. Aucun des précurseurs étudiés n’est significatif pour les femmes. Les hommes et les femmes se distinguent également quant aux symptômes d’état de stress post-traumatique qui prédisent la consommation d’alcool 18 mois après la fusillade. Conclusion : La principale retombée de l’étude est de souligner l’importance de considérer le sexe des individus pour étudier leur consommation de substances psychoactives suite à un traumatisme. / Objectives: To determine the incidence of a drug or alcohol addiction among the students who witnessed the Dawson shooting, in the 18 months that followed the event. Identify predictors of development of an addiction to a psychoactive substance, taking into account the severity of the exposure to the event. Consider whether alcohol consumption, 18 months after the event, is connected to the different categories of symptoms of post-traumatic stress. Method: The population under study includes the students at Dawson College at the time of the event. Analyses were conducted with data from 854 students enrolled at the College when the shooting took place. Results: In the 18 months following the shooting, 5 % of women and 7 % of men developed an addiction to a substance for the first time in their lives. For men, younger age (< 20 years old), the prevalence of suicidal thoughts throughout their lives, as well as having seen the perpetrator at the time of the shooting, were key predictors. No predictors were significant for women. Men and women also differed with regards to post-traumatic stress symptoms that predict alcohol consumption, 18 months after the shooting. Conclusion: The main outcome of this study stresses the importance of considering the gender of individuals, when evaluating their consumption of psychotropic drugs or alcohol after a trauma.

Vergleich der physiologischen Stressreagibilität von Frauen mit komplexen Traumafolgestörungen und gesunden Frauen / Comparison of the physiological stress reactivity in healthy women and women with posttraumatic stress disorder

Hintergrund: Vegetative Übererregbarkeit ist ein zentrales Symptom der posttraumatischen Belastungsstörung (PTBS), welche durch eine präfrontale Disinhibition des limbischen Systems mit hieraus folgenden maladaptiven peripheren Stressreaktionen erklärt wird. Lange Zeit hat die Forschung den starken Einfluss des Parasympathikus auf die Stressregulation vernachlässigt und durch Erfassung zu weniger Parameter der Komplexität der autonomen Stressregulation nicht ausreichend Rechnung getragen. In dieser Studie sollen die Auswirkungen der sympathovagalen Dysbalance auf alltägliche Stressreaktionen und das Entspannungsvermögen von Frauen mit PTBS im Vergleich zu gesunden Frauen untersucht und Erkenntnisse über verschiedene autonome Regulationsmechanismen gewonnen werden. Hierbei sollen auch medikamentöse Einflüsse berücksichtigt und das subjektive Stressempfinden mit den physiologischen Messwerten korreliert werden. Methoden: 52 Patientinnen (P) und 39 gematchte, gesunde Frauen (K) wurden mit Hilfe eines hämodynamischen Monitorsystems während zweier 5-­minütiger Stresstests (Rechentest: RT, Babyschreien: BS) und in Ruhe untersucht. Mittels EKG-­, Impedanz-­ und Blutdruckmessungen wurden für jeden Herzschlag die folgenden Parameter erhoben: Herzrate (HR), Herzindex (CI), Präejektionszeit (PEP), peripherer Gefäßwiderstandsindex (TPRI), systolischer Blutdruck (sBP), hoch-­ und niederfrequente Herzratenvariabilität (HF-­ und LF-­HRV), Standardabweichung der regulären RR-­‐Intervalle (SDNN) und Barorezeptorsensitivät (BRS). Neben dem globalen Gruppenvergleich wurde auch ein Subgruppenvergleich durchgeführt, bei welchem die Reaktionen der Patientinnen ohne kardial wirksame Medikamente (P0, n=21) jeweils mit denen der Patientinnen mit kardial wirksamer Medikation (P1, n=27) und denen der Kontrollgruppe verglichen wurde. Zu Beginn der Untersuchung, nach jeder Stressphase und nach der abschließenden Entspannungsmusik wurde zudem das subjektive Stressempfinden auf einer SUD-­Skala von 0-­10 erfragt. Ergebnisse: Die HF-­HRV und die BRS von K fielen während des RT ab, während es zu einer Aktivierung der β-­adrenergen Parameter kam (CI↑, HR↑, PEP↓). Das BS verursachte bei K hingegen einen Anstieg von HF-­HRV und BRS, ohne dass hier starke sympathischen Reaktionen beobachtet werden konnten. P lag mit der HF-­HRV während aller Messphasen signifikant unter den Werten von K (p=0,0003). Signifikante Wechselwirkungen konnten für HR (p<0,0001), PEP (p=0,0032), BRS (p=0,0002) und CI (p=0,0106) nachgewiesen werden: während des RT stiegen die HR (p<0,0001) und der CI (p=0,041) von P signifikant schwächer an als bei K, während die PEP entsprechend weniger abfiel (p=0,006). Der Anstieg der BRS während des BS war bei P ebenfalls signifikant geringer ausgeprägt (p=0,009), zu vermehrten sympathischen Reaktionen kam es bei P dennoch ebenfalls nicht. Insgesamt zeigten sich auch keine signifikanten Unterschiede für den sBD. Dafür war das subjektive Stressempfinden von P über alle Messphasen und insbesondere während des BS signifikant höher (p=0,01). Eine geringe Korrelation war hier vor allem mit der HF-­HRV und der HR zu finden, für die meisten Parameter zeigte sich allerdings kein korrelativer Zusammenhang. Im Vergleich zwischen P0 und P1 fanden sich nur für CI, TPRI und SDNN signifikante Gruppenunterschiede (P1: CI↓, TPRI↑, SDNN↓) und für keinen der Parameter konnten im Subgruppenvergleich eine signifikante Wechselwirkung nachgewiesen werden. Insbesondere die HF-­HRV war auch bei P0 signifikant niedriger als bei K (p=0,0432). Das subjektive Stressempfinden beider Subgruppen unterschied sich nicht signifikant. Interpretation: Die gesunden Frauen reagierten während des Rechentests wie erwartet mit einer vagalen Disinhibition und der simultanen Aktivierung β-­adrenerger Aktivitätsparameter. Dies kann als eine aktive Stressbewältigung interpretiert werden. Während des Babyschreiens wurde jedoch offensichtlich eine andere autonome Reaktion ausgelöst, welche sich hauptsächlich in einer gesteigerten Vagusaktivität ausdrückte und kaum Veränderungen der sympathischen Parameter verursachte. Diese Studie zeigt somit als erste unterschiedliche vagale Reagibilitätsmuster auf externe Stimuli bei gesunden Frauen und verdeutlicht so den starken Einfluss der vagalen Modulation auf die verschiedenen Stressreaktionen. Bei den Patientinnen war hingegen bereits in Ruhe ein erniedrigter Vagotonus zu beobachten, welcher unter Stress eine reduzierte Reaktionsfähigkeit zeigte. Insbesondere während des Babyschreiens stiegen die vagalen Parameter bei ihnen nicht vergleichbar stark an. Darüber hinaus war bei den Patientinnen während des Rechnens eine geringere Aktivierung der β-­adrenergen Parameter und zu finden und auch während des Babyschreiens kam es trotz eines stärkeren subjektiven Stressempfindens nicht zu einer verstärkten sympathischen Reaktion. Diese Ergebnisse sprechen somit für eine vorwiegend vagale Dysfunktion bei PTBS im Sinne einer vagalen Hyporeagibilität bei insgesamt reduziertem Vagotonus. Auch wenn die Korrelation niedriger vagaler Werte mit einem erhöhten subjektiven Stressempfinden bei den Patientinnen nur schwach ist, ist sie mit den anderen Ergebnissen dieser Studie gut vereinbar. Stress scheint also möglicherweise auch durch den ausbleibenden Anstieg des Vagotonus zu entstehen und nicht nur durch eine Aktivierung des Sympathikus. Insgesamt muss jedoch eine Diskrepanz zwischen dem starken subjektiven Stresserleben und den nur gering reagierenden physiologischen Parametern hervorgehoben werden. Die kardialen Nebenwirkungen der Medikamente führten bei den Patientinnen lediglich zu einem Shift einiger Werte, nicht zu einem grundsätzlich veränderten Reaktionsmuster.

L'état de stress post-traumatique-complexe : un concept novateur à explorer

Les mauvais traitements représentent un facteur de risque pouvant entraver le développement normal des enfants qui en sont victimes. L’objectif général de cette thèse est de mieux comprendre le concept d’état de stress post-traumatique-complexe (ÉSPT-C) et ce, en permettant l’évaluation rigoureuse de ses symptômes et en documentant sa relation avec d’autres variables. La présente thèse est composée de deux articles empiriques. L’objectif du premier article est d’obtenir une mesure de l’ÉSPT-C pour une population francophone. Pour ce faire, les qualités psychométriques de la traduction francophone du Structured Interview of Disorders of Extreme Stress-Self-Report (SIDES-SR) ont été testées par le biais d’analyses préliminaires auprès de 438 adultes canadiens-français provenant de la population générale et de l’Université de Montréal. Les résultats démontrent que l’instrument possède une structure factorielle de 5 facteurs comparable à celle obtenue pour l’instrument en anglais dans sa version originale. Les facteurs obtenus sont : 1) les croyances pessimistes par rapport aux autres et au futur, 2) la gestion des affects, 3) l’évitement des contacts physiques et sexuels, 4) la somatisation et 5) la prise de risques. La cohérence interne de 4 facteurs sur 5 varie d’acceptable à très satisfaisante alors que leur stabilité temporelle et leur validité convergente, divergente et concurrente ressortent comme étant assez satisfaisantes. Le but du deuxième article de cette thèse est d’examiner l’apport des symptômes d’ÉSPT-C retrouvés chez les mères et de leurs pratiques parentales (supervision et utilisation abusive de la punition) pour expliquer les symptômes d’ordre psychologique chez leurs enfants victimes d’agression sexuelle (AS). Les symptômes d’ÉSPT-C des mères et leurs pratiques éducatives ont été évalués auprès de 96 mères à l’aide de mesures auto-rapportées alors que les symptômes de leur enfant ont été mesurés à partir d’un instrument qui leur était destiné. Les résultats suggèrent qu’en contrôlant les variables sociodémographiques et les caractéristiques de l’AS de l’enfant, les symptômes d’ÉSPT-C des mères sont liés à plusieurs symptômes retrouvés chez les enfants après le dévoilement de l’AS qu’ils ont subie. Le rôle modérateur de la supervision maternelle dans la relation entre l’ÉSPT-C des mères et les symptômes de dépression et d’opposition des enfants a également été démontré. Les implications théoriques et cliniques concernant les résultats obtenus dans le cadre de ce projet doctoral sont finalement abordées. / Maltreatment is a risk factor that may hinder the normal development of children. The principal objective of this thesis is to better understand complex posttraumatic stress disorder, by evaluating it and documenting its relationship with other variables. This thesis includes two empirical articles. The objective of the first article is to develop a measure of complex posttraumatic stress disorder for French-speaking adults. The psychometric properties of the translated version of the Structured Interview for Disorders of Extreme Stress-Self-Report (SIDES-SR) were tested through preliminary analyses with 438 French-speaking adults from the general population and the University of Montreal. Results demonstrate that the instrument has a factor structure of five factors similar to the English version of the instrument. The factors obtained are: 1) pessimistic beliefs about others and future, 2) managing emotions, 3) avoidance of physical and sexual contact, 4) somatization, and 5) risk-taking behaviours. The internal consistency of 4 of the 5 factors varies from acceptable to very satisfactory, while their temporal stability and convergent, divergent and concurrent validity are acceptable. The purpose of the second article is to examine the contribution of complex posttraumatic stress disorder found in mothers and parenting practices (monitoring and abusive punishment) on psychological symptoms reported by children victims of sexual abuse. Symptoms of 96 mothers and their rearing practices were assessed using self-report measures, while children’s symptoms were measured with an instrument designed for children. Results demonstrate that, after controlling for sociodemographic variables and characteristics of the child sexual abuse, complex posttraumatic stress disorder is associated with several symptoms in children. The moderating role of maternal monitoring in the relationship between complex posttraumatic stress disorder of mothers and symptoms of depression and opposition of children is also demonstrated. Theoretical and clinical implications are discussed concerning the results obtained in this doctoral project.

Forced intimacy : the experiences of sexually victimized prisoners

Total institutions' (which include prisons) unique context prescribes "confinement" of inmates. Coping mechanisms applied "outside" (especially the ability to create distance from stressful events) is thus ineffective. Another common feature is lack of privacy, which may result in "forced intimacy" - individuals being forced into a situation of physical and psychological "invasion" (beyond the norm) of their person/personal space. Victims lose control over intimate decisions, including who may and may not be intimate with them. Within prisons, gangs "force intimacy" by sexually victimizing inmates, taking advantage of the context to heighten their power, and to control inmates "under" them. Four (subjects) victims' experiences and means of adaptation/ empowerment were investigated phenomenologically. Results indicated that inadequately empowered victims suffer prolonged and repeated victimization - a continued "posttraumatic stress disorder" - which is more traumatizing and draining than one circumscribed traumatic event (due to its intensity, immobilization and resulting drastic change of "personality"). / Psychology / M.A. (Clinical Psychology)

In my thesis, I postulated the target to map the methods and procedures applied by the CR Police, Regional Police Headquarters of the South-Bohemian Region within the primary, secondary as well as tertiary prevention of posttraumatic reactions and disorder. A partial target of the thesis is finding out the rate of the satisfaction with availability of intervention care for policemen and employees of the Regional Police Headquarters of the South-Bohemian Region, finding out the rate of the awareness, usability and satisfaction with the care of the posttraumatic intervention team in the South-Bohemian Region, awareness of the service Help Line in crisis and its usability. Moreover I postulated the goal to map the interest in the possible internet helpline in the scope of IZS with elaboration of SWOD analysis. Everybody, also policemen may suffer from stress reaction. Also they can have the psychical and physical signs of an acute stress reaction. They perform the most of service acts, interventions in a routine way, however, besides this, also situations occur, differing from the normal life experience and policemen get in this way very often into the stress situations, which can arouse deep mental turbulences. The impact of feelings on the mental state in case of extreme experiences is a normal human reaction in a not normal situation. At present, each policeman has the possibility to ask for help a team of specially trained experts, the team of posttraumatic intervention care. In the team, not only psychologists are working, but also experienced policemen, experts of the branch of the Ministry of Interior and the priests prepared to help with their experience, erudition and knowledge acquired by a special training. Since 2001 the impacted person may call the anonymous Crisis Help Line, which is a supporting part of the posttraumatic intervention care for the policemen, firemen, employees of the branch of the Ministry of Interior and their family members. The crisis help line is also at disposal to the public in case of extraordinary events and crisis situations. For this reason, this thesis treats the methods of posttraumatic intervention care, awareness, usability, satisfaction and trust in these branch offices of the CR Police.

Os objetivos específicos são: constatar a prevalência de TEPT nos casos de VPI acima citados; descrever a frequência de fatores como: histórico de violência na família de origem, vitimização prévia, tipo de violência sofrida, uso de álcool e drogas pelo parceiro, bem como o tempo de exposição ao evento traumático no grupo estudado; verificar a associação entre percepção de suporte social, TEPT e depressão. Métodos: Estudo transversal, quantitativo, descritivo e associativo. A amostra compõe-se de 41 mulheres vítimas de violência por parceiro íntimo, que recorreram à DPOF no primeiro semestre de 2013. Foram aplicados os seguintes instrumentos: entrevista estruturada, Post-Traumatic Stress Disorder Checklist- Civilian Version (PCL-C), Escala de Percepção de Suporte Social (EPSS) e Inventário de Depressão de Beck (BDI-I). Resultados: A partir do rastreamento dos sintomas do TEPT, obteve-se a prevalência de possíveis casos do transtorno em 82,9% das mulheres entrevistadas. Houve uma associação positiva entre depressão e TEPT, e negativa entre TEPT e percepção de suporte social. Conclusões: O trabalho aponta para a necessidade de se criar estratégias específicas de intervenção para mulheres vítimas de VPI, diante da alta prevalência do transtorno. É preciso, especialmente, realizar projetos nos órgãos jurídicos de proteção à mulher, a fim de fortalecer o suporte social da mesma ao enfrentar situações adversas relacionadas à agressão. / Violence against women is considered a public health problem due to serious physical and mental consequences it produces, such as Posttraumatic Stress Disorder (PTSD). Objectives: To investigate the relationship between PTSD and violence against women by intimate partners, with emphasis on victims served by psychology sector in Juiz de Fora DPOF (Precinct Guidance and Family Protection). The specific objectives are: to find out the prevalence of PTSD in IPV cases cited above, to describe the frequency of factors such as: history of violence in the primitive family, previous victimization, type of violence, use of alcohol and drugs by the partner, as well as the time of exposure to the traumatic event in the studied group; to verify the association between perceived social support, depression and PTSD. Methods: The study is cross-sectional, quantitative, descriptive and associative. The sample consists of 41 female victims of intimate partner violence, who resorted to DPOF the first half of 2013. The following instruments applied were: Structured interview, Post-Traumatic Stress Disorder Checklist- Civilian Version (PCL-C), Perceived Social Support Scale (EPSS) and Beck Depression Inventory (BDI-I). Results: From the trace of the symptoms of PTSD, we obtained the prevalence of disorder possible cases in 82,9% of the women interviewed. There was a positive association between depression and PTSD, and negative association between PTSD and perceived social support. Conclusions: The study points to the need to create specific intervention strategies for women victims of IPV, given the high prevalence of this disorder. We must especially perform projects in the legal bodies to protect women in order to strengthen their social support to face adverse situations related to the aggression.
