Effectiveness of Prophylactic Fluconazole at Low Doses for Allogeneic Hematopoietic Stem Cell Transplant Patients

Class of 2016 Abstract / Objectives: The purpose of this study was to evaluate if fluconazole 200 mg is an acceptable alternative to the fluconazole 400 mg for fungal prophylaxis in allogenic hematopoietic stem cell patients. Lower fluconazole doses will decrease cost of therapy and may reduce adverse events associated with higher doses. Methods: This study was a retrospective chart review conducted at the Arizona Cancer Center. A total of 58 patients qualified for the study. Primary endpoints were number of days on fluconazole 200 mg and type and number of fungal infections that occurred within 1 year post transplant. Results: Out of the fifty-eight patients who qualified for the study, only eight patients had a breakthrough fungal infection while on 200 mg (13.7%) after one year. Three of those eight were identified as having systemic fungal infections (5.2%). Conclusions: Fluconazole 200 mg is a reasonable low-cost and low side effect alternative to fluconazole 400 mg for antifungal prophylaxis in allogenic hematopoietic stem cell patients.

Extraction and retention of teeth in the line of mandibular fracture: A study of a Western Cape sample

Magister Chirurgiae Dentium (MChD) / Anecdotal and controversial discussion has.always been prompted by the bias of the clinician in relation to whether teeth in the line of fracture should be extracted or retained. In recent years, it became evident to clinicians serving the Maxi 11ofaci aland Oral Surgery Clinic at Groote Schuur Hospital, Cape Town, that a tendency towards reta ining teeth in the 1ine of fractu re did not seem to increase the possi bility of infecti on, providing that circumspection was used. A retrospective analysis of these cases was conducted in order to determine the outcome of our treatment. An independent sample of the 151 patients were investigated in relation to age, sex predilection, the effect timing has on the outcome of treatment and the incidence of infection when teeth in the line of mandibular fracture were extracted or retained. The results of this study has shown that most of our patients were young (aged 20-29yrs) and therefore a tendency to retain teeth was found. In older patients, (aged 30-49yrs), the tendency was to extract teeth. Males presented more than females in a 3:1 ratio. Patients treated early « 24hrs) had most of their teeth retained while others treated after 7 days had their teeth extracted. The infecti on rate was 2 percent if teeth were retained and 5 percent if teeth were extracted. A1though the difference was not statistically significant, it appeared that the retenti on of teeth in the 1ine of fracture presented more advantages to both patient and clinician.

Abstract The thesis focuses on an insight into the attitude to prophylactic check-ups among nurses and midwives. The theoretical part deals with health, prevention, prophylactic check-ups, characteristics of attitudes and education. The role of nurses and midwives in prevention, definition of competences and lifestyle problems in relation to healthcare job performance is not omitted. The aim of the thesis was to find out whether nurses and midwives themselves regularly have prophylactic check-ups and how they approach the information on necessity of prophylactic check-ups in the public. Quantitative method was applied to the research. An anonymous questionnaire research among nurses and midwives working in primary care and in hospitals was used as the data collection technique. The total number of questionnaires returned from the respondents was 270, 91 were returned from primary care and 179 from hospitals. None of the questionnaires had to be rejected for incompleteness. A pilot study was performed within the research, where 10 questionnaires were handed out evenly to the primary care sector and to the hospital sector to verify whether no serious problems with their completion occured. Thanks to their correct completion they were included into the main research. The monitored hypotheses were statistically evaluated by means of the Pearson's Chi-Square Test and Fisher?s Test with low values in contingency tables. The fact whether participation of nurses and midwives in prophylactic check-ups differs with age was evaluated. The frequency of regular prophylactic check-ups was 62 per cent at the age below 40 and 98 per cent at the age over 40. Age has significant influence on medical prophylactic check-up attendance. Education level does not have statistically significant influence on prophylactic check-up attendance. The research has not completely refuted difference in prophylactic check-up attendance according to workplace type. Nurses and midwives working in hospitals have prophylactic check-ups less regularly than those working in primary care. Nurses and midwives in primary care are demonstrably older and they go to prophylactic check-ups more regularly. The difference in approach to prophylactic check-ups among nurses and midwives according to workplace type was monitored. Nurses and midwives have positive attitude to prophylactic check-ups regardless of the type of their workplace. They belong to the most frequent activities they apply to maintenance and improvement of their health condition. Nurses and midwives are first of all motivated to regular visits of prophylactic check-ups by the care of their own health. Statistical level was achieved in the tests for the total number of nurses and midwives regularly attending prophylactic check-ups and educating patients. The research has confirmed that nurses and midwives that regularly visit prophylactic check-ups have more positive approach to providing information to clients-patients than those that do not undergo the prophylactic check-ups. Nurses and midwives in hospitals pay more attention to the disease itself and to prevention of possible complications in patients than to general prevention and prophylactic check-ups in their educational activities. With regard to the set goals the final evaluation of the research has confirmed that nurses and midwives have positive approach to prophylactic check-ups and try to visit them regularly. Respondents from the primary care are older and visit the prophylactic check-ups more regularly. Prophylactic check-ups have priority in health condition support and maintenance for the respondents. The research has shown that the respondents working in primary care as well as those from hospitals educate themselves in the field of prevention and prophylactic check-ups and are knowledgeable about legislation applicable to prophylactic check-ups.

Introdução: Mulheres portadoras de mutações germinativas BRCA1 ou BRCA2 apresentam um risco aumentado de câncer de mama e de ovário em comparação com a população geral, enquanto a salpingo-ooforectomia redutora de risco (SORR) reduz significativamente a incidência desses cânceres. O objetivo deste estudo foi analisar as características clínicas e patológicas de uma população franco-canadense que realizou a SORR. A morbidade cirúrgica também foi avaliada. Materiais e Métodos: Entre dezembro de 1999 e dezembro de 2009, todas as pacientes submetidas à SORR foram identificadas. Os prontuários médicos foram revisados. Estatística descritiva, teste exato de Fischer, e teste t de Student foram utilizados para análise. Resultados: Durante o período de estudo, a SORR foi realizada em 119 mulheres. A média de idade no momento da cirurgia foi de 49 anos (35-72 anos); e 63 pacientes (53%) estavam na pré-menopausa. Sessenta e duas mulheres (52%) tinham uma história de câncer de mama in situ ou invasor. Mutações nos genes BRCA1 e BRCA2 estavam presentes em 34 pacientes (29%) e 42 pacientes (35%), respectivamente. Desse modo, 43 pacientes (36%) foram consideradas como tendo um risco aumentado de câncer de mama e de ovário, apesar de um teste negativo para ambos os genes (n = 23) ou desconhecido, porque o paciente recusou teste genético (n = 20). A maioria das pacientes com útero foi submetida a uma histerectomia complementar (65%). Seis complicações ocorreram (3 hematomas, 2 arritmias cardíacas e uma cistostomia). Em uma paciente (0,8%), um carcinoma ovariano de alto grau estadio II foi descoberto no momento da cirurgia. Atipias de tubas de falópio foram identificadas na patologia final em 8 casos (6,7%). Após um acompanhamento médio de 22 meses, 4 mulheres (3,4%) desenvolveram câncer de mama e uma paciente (0,8%) desenvolveu câncer peritoneal. O câncer cervical é a consequência mais séria da infecção por Papilomavírus humano - HPV, constituindo uma das principais causas de morte entre mulheres no mundo, remetendo a um importante desafio para a saúde pública mundial. Desenvolvemos a produção de VLPs contendo as proteínas L1 e L2 de HPV16, utilizando células epiteliais humanas 293-F em suspensão e em meio isento de soro fetal bovino. As células possuem metabolismo intenso, adequado para a expressão de proteínas heterólogas. Apresentam vantagens como a facilidade de cultivo, transfecção e ocorrência de processos como a glicosilação de proteínas, fosforilação, formação de pontes dissulfeto e outras modificações pós-traducionais essenciais para a função das proteínas, facilitando a produção similar às condições in vivo. O sistema de produção de VLPs L1L2 de HPV16 foi estabelecido de maneira eficiente, em células epiteliais humanas em suspensão e com resultados promissores, podendo contribuir para o desenvolvimento de uma vacina profilática de amplo espectro de proteção, com custo reduzido de produção. / Cervical cancer is the most serious consequence of infection by human papillomavirus - HPV, constituting one of the leading causes of death among women worldwide that points to a major challenge to global public health. We have developed the production of VLPs containing L1 and L2 proteins of HPV16, using human epithelial cells 293-F suspended in medium without fetal bovine serum. The cells have intense metabolism, suitable for expressing heterologous proteins. Present advantages such as ease of culture, transfection and occurrence of processes such as protein glycosylation, phosphorylation, formation of disulfide bonds and other essential post-translational modifications to protein function, facilitating the production similar to the conditions in vivo. The VLPs L1L2 of HPV16 production system was established efficiently in human epithelial cells in suspension and with promising results, which may contribute to the development of a prophylactic vaccine broad protection spectrum with reduced production cost.

However, the evidence for these exercises is equivocal and information regarding critical components of an exercise program is lacking. The purpose of this systematic review was to examine the evidence regarding lingual strengthening exercises as a component of a prophylactic swallowing program.

The Impact of Prophylactic Salpingo-oophorectomy on Health in Women who carry a BRCA1 or BRCA2 Mutation

Prophylactic salpingo-oophorectomy, the preventive removal of the ovaries and fallopian tubes, is recommended to women who carry a BRCA1 or BRCA2 mutation in order to reduce the risk of breast, ovarian and fallopian tube cancer. The short and long term health and quality of life effects of this procedure are not well understood. We examined the actual and perceived reduction in cancer risk associated with this surgery. The impact of prophylactic salpingo-oophorectomy on health-related quality of life, psychological distress, cancer worry, menopausal symptoms, and sexual function during the year following surgery was also evaluated. In our prospective study, prophylactic salpingo-oophorectomy was associated with an 80% reduction in ovarian and fallopian tube cancer risk. The residual risk for primary peritoneal cancer was 0.2% per year or 4.3% at 20 years after salpingo-oophorectomy. Most women accurately perceived their risk of breast cancer. However, the risk for ovarian cancer was overestimated, particularly by women who carry a BRCA2 mutation. Physical and mental health-related quality of life did not decrease in the year following surgery; and psychological distress was similar to levels experienced by the general population. Most women were significantly less worried about cancer after the surgery, however, a subset of women continued to experience significant cancer specific distress after prophylactic salpingo-oophorectomy. Women who underwent prophylactic salpingo-oophorectomy when premenopausal experienced a significant worsening of vasomotor symptoms and a decline in sexual functioning. Hormone replacement therapy mitigated these symptoms, but not to pre-surgical levels. Dyspareunia was somewhat alleviated by hormone replacement therapy, however, the decrease in sexual pleasure was not. Satisfaction with the decision to undergo prophylactic salpingo-oophorectomy was high regardless of these symptoms. These studies will provide women who are considering prophylactic salpingo-oophorectomy with information about the reduction in cancer risk associated with the surgery and the possible effects experienced during the year following surgery.
