Meier, Peter 09 January 2017 (has links)
There are many indications in the literature about the effects of testosterone, on the symptoms of a testosterone deficit, on the effects of excessive levels of doping, and also international guidelines for the diagnosis and treatment of the testosterone deficit. There are many known causes of this, especially the congenital deficiency, less of the acquired deficiency. There is a list of etiologies, but no information about their frequency. This is the starting point of the work. In a general-internal practice, data are collected from a patient population of 108 men with testosterone deficit. In addition to the laboratory values and the symptoms, examination findings are recorded before the start of therapy. These data are systematically supplemented by an anamnesis of diseases that are included in the guidelines as etiologies of the acquired testosterone deficit. Prevalence of these diseases in the patient population can be calculated from this. Prevalence for the whole population can be found in the literature. These are compared statistically with each other. A higher prevalence can be detected in the patient population for the findings of testicular volume of less than 15 ml, meldescensus testis, infertility and reduced fertility and status after vasectomy. There is a correlation between testicular volume (as measured by Orchidometers according to Prader) and testosterone level, which can be represented by a linear regression. A corresponding correlation between the interval between vasectomy and diagnosis of the testosterone deficiency and the testosterone level is not significant. These data are discussed with reference to the literature, especially with regard to inflammation in the testes as possible common pathogenesis. Conclusions include recommendations for preventive examinations of the man in general practitioners and urologists, for examinations in fertility specialists and research.

Förebyggande tandvård för att främja oral hälsa hos äldre : - En registerstudie bland individer 65 till 70 år i Värmland / Preventive dental care to promote oral health in elderly : - A register-based study among individuals 65 to 70 year, in the Region of Värmland

Seleskog, Birgitta January 2020 (has links)
Introduction: Dental caries is one of the most common diseases worldwide with well-known risk factors. In order to maintain a good oral health, preventive dental care is an important part for elderly people and impacts on an individual´s quality of life. Dental caries can be prevented and treated with patient support, education and preventive dental care. Health promotion activities within dental care contributes to the individual’s well-being and reduces costs for the society.   Aim: The overall aim was to identify the occurrence and distribution of registered preventive dental care in relation to caries, among individuals aged 65 to 70 years resident in the county of Värmland in 2017.  Method: A cross-sectional register-based study of individual data (n= 2738) from 65 to 70-year-old individuals resident and dental examined, in the county of Värmland in 2017. Data was obtained from The Swedish Quality Registry for Caries and Periodontal Diseases (SKaPa) from years 2017 and 2018.  Results: Nearly three-quarters of the individuals had no caries conditions registered (72,1%). Secondary caries-conditions were about twice as common compared to primary caries, 684 and 291 registrations, respectively.  A total of 742 preventive-treatments and 3495 preventive disease-treatments were registered. “Non-invasive treatment” had the strongest association to individuals with caries conditions (OR 3,83; CI 3,04-4,83). The treatment “information when risk for disease” had the strongest association to individuals without caries conditions. The number of obtained preventive treatments were skewed and could vary from one up to nine treatments per individual. Conclusion; The results of this study indicate the importance of further studies of preventive dental care as a health promotion achievement to promote oral health among older individuals with dental caries disease. Behavioral medical prevention is underused and needs to be considered in preventive dental care in the future.

Hinder för användning av scrum i ett programvarustartföretag / Barriers to scrum adoption in a software startup company

Qasim, Muhammad, Basutkar, Shreyas January 2022 (has links)
In the modern world, Scrum is widely adopted by many companies for software development, no matter the size of the company. As Scrum provides rapid development and meets the customer needs in a faster and efficient way, many companies want to move towards Scrum but moving towards Scrum is difficult and challenging due to different barriers for example cost, lack of experience, and so on. This thesis is to address the barriers which are involved during the Scrum adoption for software start-up companies such as communication, team mindset, backlog refining, daily stand up, and retrospective meetings. In this research report, these issues are divided based on the factors such as organization, people, process, and tools. We have figured out these barriers using action research based on the observation done during the Scrum team meetings and through interviews and surveys.

The Influence of Profession and Therapy Type for the Cost Effective Treatment of Sexual Dysfunction

Fawcett, David 18 May 2011 (has links) (PDF)
Sexual dysfunctions are serious mental health issues that impact an estimated one in three Americans. Due to the complex, relational nature of most sexual dysfunctions, mental health professionals trained to work with couples and their relationship interactions are likely to have better outcomes when treating clients with sexual dysfunction. Data from CIGNA Health Solutions was analyzed to explore differences in therapy outcome for various types of mental health professions when treating clients with sexual dysfunctions. The current research is a retrospective analysis of administrative data that explores whether or not type of profession (i.e. psychologists, Masters of social work, marriage and family therapist, or professional counselor) influences the outcome of mental health treatment. This study also explores whether therapy modality (i.e. individual, conjoint, or mixed mode, a combination of individual and conjoint therapy) influences therapy outcome. Treatment outcome was measured by recidivism rates, client drop out from therapy, the total number of sessions, and cost of treatment. Participants included 230 males and 189 females ages 18 to 101 (M =38.9, SD = 11.4) who received treatment for sexual disorders from 2001 to 2006. Participants were from all regions of the United States. Results indicate that overall, psychotherapeutic treatment for sexual dysfunctions is relatively brief, averaging about seven sessions across all professions. Results suggest that marriage and family therapists treat sexual dysfunctions using a conjoint and mixed mode approach more frequently than therapists with other licenses. Results also suggest that mixed mode therapy has drastically lower dropout rates and longer retention than individual or conjoint therapy. These results suggest that utilizing a combination of relational and individual sessions is beneficial to the treatment of sexual dysfunctions.

Impact of State Legislation in Tennessee on Opioid Prescribing Practices of Orthopedic Surgeons

Guidry, Corey, Dema, Blerim, Allen, Corinne, Stewart, David 01 March 2022 (has links)
OBJECTIVE: Post-operative patients are at increased risk of becoming chronic users of opioids, and overprescribing can lead to abuse and diversion. Though data have shown a decrease in opioid prescriptions nationally, limited studies have specifically evaluated the influence of state legislation on this trend. This study aimed to assess the impact of legislation in the state of Tennessee on opioid prescribing amongst orthopedic surgeons. DESIGN: This retrospective cohort analysis evaluated patients who received opioids post-orthopedic surgery before and after the state legislation was passed. SETTING: A community teaching hospital. PATIENTS AND PARTICIPANTS: Two hundred and three post-orthopedic surgery patients were included, with 101 in the preleg-islation and 102 in the post-legislation groups. INTERVENTIONS: State legislation in Tennessee limiting amounts of prescribed opioids went into effect in July 2018. MAIN OUTCOME MEASURE(S): The primary outcome was total morphine milligram equivalents (MMEs) prescribed, with secondary outcomes of days' supply, dosage units, and MME per day. RESULTS: Orthopedic surgery patients in the post-legislation arm were prescribed significantly fewer MME than those in the prelegislation arm (median MME 375 vs. 562.5; p < 0.001). Prescription days' supply, number of dosage units, and MME per day were also significant lower in the post-legislation group. CONCLUSIONS: After orthopedic surgery, patients in the post-legislation arm were prescribed a median 187.5 MME less than those in the prelegislation arm. Our findings suggest that state opioid legislation is associated with a reduction in the amount of opioids prescribed in certain orthopedic surgery patients, though further studies evaluating adequacy of pain control are warranted.
