Systematiskt säkerhetsarbete vid svenska universitet : En kvalitativ studie om framgångsfaktorer och hinder vi säkerhetsarbetepå svenska universitet / Systematic safety work at Swedish universities : A qualitative study on successful factors and challenges in conducting safety work at Swedish Universities

Ämnesområdet säkerhet har kommit att få mer utrymme inom verksamhetsområden som ett resultat av det omvärldsläge som förefaller idag, där kriser och olyckor tycks bli en del av det vardagliga sammanhanget snarare än extraordinära händelser som inträffar då och då. Samhället är en dynamisk plats som ständigt utvecklas, vilket innebär att såväl förebyggande arbete som åtgärder inte kommer att vara detsamma för alltid. Begränsad forskning på hur systematiskt säkerhetsarbete bedrivs och förstås utifrån teoretiska utgångspunkter utgör ett incitament att undersöka detta vidare. Universitetet kan likställas med mindre samhällen, sett till exponering och omfång och kräver således ett gediget systematiskt säkerhetsarbete för att säkerställa en trygg och säker tillvaro där risker såväl bedöms som värderas och åtgärdas. Studien syftar till att undersöka hur systematiskt säkerhetsarbete bedrivs och ter sig vid universitet i Sverige och genom detta skapa en uppfattning om såväl framgångsfaktorer som hinder. För att ge svar på studiens syfte har styrdokument samlats in från tre mellanstora universitet i Sverige, för att skapa en kunskapsöversikt och förståelse för hur säkerhetsarbetet bedrivs. I tillägg till detta har yrkesverksamma inom säkerhetsområdet vid samma tre universitet intervjuats. Studiens resultat visar på såväl hinder som framgångsfaktorer vid ett framgångsrikt säkerhetsarbete. Komplexitet föreligger i att nå ut till den stora skalan, och studenter tycks vara den målgrupp som är svårast att nå. Ett intressant fynd i kontrast till detta är att riskmedvetenheten förefaller sig vara lika hög hos studenter som anställda, om än högre. Vidare visar studien att lagar och förordningar sätter ramen för hur det systematiska säkerhetsarbetet bedrivs vid universitet, utöver de universitetens egna ambitioner, och bidrar till en ökad riskmedvetenhet. / Safety has come to gain more attention in operational areas as a result of the current state of the world, whereas crises and accidents seem to have become part of the everyday context rather than extraordinary events that occur every now and then. Society is a dynamic place that is constantly evolving, which means that both prevention work and measures will have to evolve along with society. Limited research on how systematic safety work is conducted and understood based on theoretical starting points constitutes an incentive to investigate this subject area further. The university can be compared to smaller communities, due to risk exposure and scope, that requires systematic safety work to ensure a safe and secure environment where risks are assessed as well as valued with relevant measures being taken. The study aims to investigate how systematic safety work is conducted at universities in Sweden, and by doing so create an understanding of successful factors as well as obstacles and challenges of conducting a successful safety work. To provide answers to the purpose of the study, documents from three middle sized universities in Sweden have been collected to gain knowledge of how the systematic safety work is being conducted. In addition to this, professionals in the security field at the same three universities have been interviewed. The results of the study show both obstacles and success factors in conducting a successful safety work. Complexity appears in reaching out to the large scale, and students seem to be the most difficult target group to reach. An interesting finding in contrast to this is that risk awareness appears to be just as high among students as employees, if not higher. Furthermore, the study shows that laws and regulations set the framework for how the systematic safety work is conducted at universities, in addition to the universities' own ambitions, and contribute to an increased risk awareness.

An Artificial Intelligence-Driven Model-Based Analysis of System Requirements for Exposing Off-Nominal Behaviors

With the advent of autonomous systems and deep learning systems, safety pertaining to these systems has become a major concern. The existing failure analysis techniques are not enough to thoroughly analyze the safety in these systems. Moreover, because these systems are created to operate in various conditions, they are susceptible to unknown safety issues. Hence, we need mechanisms which can take into account the complexity of operational design domains, identify safety issues other than failures, and expose unknown safety issues. Moreover, existing safety analysis approaches require a lot of effort and time for analysis and do not consider machine learning (ML) safety. To address these limitations, in this dissertation, we discuss an artificial-intelligence driven model-based methodology that aids in identifying unknown safety issues and analyzing ML safety. Our methodology consists of 4 major tasks: 1) automated model generation, 2) automated analysis of component state transition model specification, 3) undesired states analysis, and 4) causal factor analysis. In our methodology we identify unknown safety issues by finding undesired combinations of components' states and environmental entities' states as well as causes resulting in these undesired combinations. In our methodology, we refer to the behaviors that occur because of undesired combinations as off-nominal behaviors (ONBs). To identify undesired combinations and ONBs that aid in exposing unknown safety issues with less effort and time we proposed various approaches for each of the task and performed corresponding empirical studies. We also discussed machine learning safety analysis from the perspective of machine learning engineers as well as system and software safety engineers. The results of studies conducted as part of our research shows that our proposed methodology helps in identifying unknown safety issues effectively. Our results also show that combinatorial methods are effective in reducing effort and time for analysis of off-nominal behaviors without overlooking any dependencies among components and environmental entities of a system. We also found that safety analysis of machine learning components is different from analysis of conventional software components and detail the aspects we need to consider for ML safety.

THE ENVIRONMENTAL HEALTH AND SAFETY LEADERS’ PERSPECTIVES OF HIGHER EDUCATION SAFETY CULTURE

To safeguard the health and well-being of faculty, students, staff, and the community is of moral imperative for higher education institutions. Likewise, protecting the environment is a socially sound practice. Furthermore, building and maintaining a positive safety culture is believed to contribute to productive environmental health and safety (EH&S) outcomes. Higher education EH&S leaders are at the center of universities’ efforts in maintaining a positive safety culture. The purpose of this inquiry was to study higher education EH&S leaders’ perspectives on safety culture and contribute to closing the academic literature gap in the higher education setting. Interviews and a survey were the data collection techniques. EH&S leaders of U.S. higher education institutions participated in the study. I used Cooper’s (2000, 2016) reciprocal safety culture model as a theoretical framework and a mixed methods research design to find answers to the research questions. The survey results revealed how EH&S leaders viewed different aspects of their operations, and findings from the interviews revealed the leaders’ lived experiences. For example, the quantitative study showed 100% of the participants strongly agree or agree that shaping the safety culture of their campus is part of their role. In addition, the qualitative data identified distinct strategies employed by leaders to shape the safety culture of their campuses. Four major themes were identified in the qualitative data. In the first theme, The Higher Education Safety Culture, the EH&S leaders reflected on their lived experiences and the importance of positive safety culture in accomplishing their goals. They mobilize their campus communities in a collective effort to achieve a healthy and safe working environment, minimize the impact on the environment, and remain compliant with regulatory requirements. The second theme, Higher Education Environmental Health and Safety Programs, stressed the plans and procedures the leaders and their departments engage in their daily operations. The third theme, Higher Education Management’s Role in Environmental Health and Safety Operations, manifested the leaders’ equivocal voice on the necessity of the higher education leadership and upper management support to fulfill their missions. The last theme, Modus Operandi of Higher Education Environmental Health and Safety Leaders, is about a range of strategies and tactics the EH&S leaders employed to succeed in a structured, bureaucratic, and challenging environment. The findings have direct implications for both higher education EH&S professionals and higher education senior leadership. The study findings implied EH&S leaders should focus their effort where it generates the best outcome, namely: (a) orchestrate the campus community toward a positive safety; (b) build and implement effective EH&S programs; (c) bring upper management and leadership aboard; (d) apply effective communication; (e) build trust; (f) define their role as a consultant; (g) stand out; and (h) create a brand, motto, and slogan where possible. For higher education senior leadership, participants emphasized the necessity of upper management and leadership support to build and maintain a positive safety culture on the campus, agreeing with Cooper (2000, 2016). This work helps contribute to making higher education senior leadership and upper management understand their role in their campuses’ safety culture and provide due support and actively participate. This study served as an initial exploration in understanding higher education EH&S leaders’ perspectives on safety culture and contributing to closing the literature gap. It also opened a door for future research. Broadening the audience to students, faculty, and staff are reasonable candidates for further research for a more comprehensive understanding of the safety culture in higher education. In addition, expanding the survey to include more EH&S leaders of higher education will elaborate on the EH&S operations, challenges, and sentiment.

The objective of this study was to investigate the relationship between work stressors, job insecurity, union support, job satisfaction and safety motivation and -behaviour, also to determine whether the levels of job satisfaction mediate the relationship between work stressors, job insecurity, union support and safety motivation and -behaviour. The role conflict scale (Rizzo, House, & Lirtzman, 1970), role overload (qualitative and quantitative) scale (Beehr, Walsh, & Taber, 1976; Sverke, Hellgren, & Öhrming, 1999), job insecurity scale (Hellgren, Sverke, & Isaksson, 1999), union support scale (Shore, Tetrick, Sinclair, & Newton, 1994), job satisfaction scale (Hellgren, Sjöberg & Sverke, 1997), safety motivation scale (Neal, Griffin, & Hart, 2000), safety behaviour scale (Neal et al., 2000) and a biographical questionnaire were administered to employees (N = 260) from the mining industry. A cross-sectional survey design was utilised. The scales demonstrated acceptable levels of internal consistency. Increased levels of work stressors and job insecurity were found to be associated with decreased levels of job satisfaction. Also, increased levels of perceived union support were associated with increased levels of job satisfaction and safety motivation and -behaviour. Finally, it was found that job satisfaction mediates the relationship between union support and safety motivation and -behaviour. Key performance indicators for the evaluation of an air navigation service providers' safety management system

Safety is the main concern of the aviation industry. All Air Navigation Service Providers must have a Safety Management System (SMS) which states how safety is handled, promoted and prioritized. By developing Key Performance indicators (KPIs), it is possible to quantify the effectiveness of a SMS, discover potential flaws and improvement measures. This thesis identifies principles behind the SMS, the development of KPIs and suggest potential KPIs for the Swedish air navigation service provider LFVs’ SMS. A literature study was performed and organisation specific documents were analysed to develop potential KPIs within the areas timely compliance with international obligations, competency and adoption and sharing of best practices based on an EASA questionnaire. This work presents a set of 27 performance indicators and recommends 6 as potential KPIs for the three areas together. The KPIs are developed specifically for LFV but could be applicable for other organisations with similar SMS structure and processes. They should be analysed within the organisation and, potentially, have thresholds set before implementation.

Safety culture is a new safety “buzz word” and is promoted as a phenomenon that could assist organisations in improving safety performance in their endeavour to safety excellence. It is however, a relatively new concept and research in this field was completed during the past two decades only. Safety culture is a multi-dimensional, dynamic and complex concept. A constructive analysis of safety control mechanisms and production costs at a coal mine

(Cost and Management Accounting, Faculty of Management Sciences), Vaal University of Technology / Statistics in South Africa show that the fatality and injury rates within the mining industry have dropped since the end of the apartheid era. Regardless of the drop the fact remains that the mine workers, who are often the breadwinners, are still dying in the country’s mines leaving their families behind without any source of income. If they do not die they may suffer crippling injuries due to mine accidents making it impossible for them to perform their daily duties at work which results in the loss of income, loss of quality of life and this produces a humanitarian and a socio-economic problem. Mining companies incur enormous costs due to fatalities and injuries. Hence a target for zero fatalities was set in the mining industry for 2013. However, the industry has failed to reach this target that it set for itself and it is therefore of the utmost importance to encourage mining companies to invest more than they do currently in safety in order to prevent these fatal and crippling injuries due to mine accidents. The purpose of this study was to determine whether or not the requirements of safety legislation are observed and complied with by a single Colliery in South Africa that was selected for this study. The study also sought to investigate whether or not the employees of this mine observe the safety regulations to ensure safety and to maintain an accident free working environment. An analysis of organizational safety control mechanisms and production cost was conducted through the use of a structured questionnaire, completed by 151 participants. Descriptive statistics, frequency tables, exploratory factor analysis (EFA), one-way analysis of variance (ANOVA) and t-test were utilized to analyse the data. The results indicate that the Colliery was compliant with the safety legislations, a positive employee safety attitude had been cultivated and there was provision of adequate safety facilities through safety investments. Cost reduction due to the application of safety controls was also established, yet it was found necessary for the Colliery to enforce and encourage the application of safety measures to reduce the costs that are still incurred as it is evident that the application of safety controls results in reduced costs. Although the Colliery was considered compliant, with its employees showing a positive attitude towards safety controls, and with safety investments made and cost reduction achieved due to safety measures, ANOVA revealed different perceptions based on the departments in which the employees work, years of experience, English proficiency and qualifications. However, no differences were found in relation to gender and designation.

Job demands, resources and the propensity to comply with safety procedures and interventions associated with needlestick injuries

A research project submitted in partial fulfilment of the requirements for the degree of Master’s (Industrial/Organisational Psychology) in the Faculty of Humanities, University of the Witwatersrand, Johannesburg, March 2017 / A South African study, based on a sample of 208 medical personnel working in public and private institutions, was conducted in order to determine whether job demands and job resources led to differences in the propensity to comply with Needlestick Injury (NSI) intervention scores. Three self-report questionnaires were completed by the participants, namely the self-developed demographic questionnaire, the Job Demands-Resources Scale (JDRS) which measured job demands and resources, and thirdly the self-developed Propensity to Comply with Interventions for Needlestick Injuries Scale (PCINS) which measured the propensity to comply with interventions for NSIs. Accidental needle stick, as well as sharp, injuries occur frequently within the medical context and are associated with high risks for blood-borne infections (Adefolalu, 2014). Specifically within South Africa blood-borne infections such as HIV carry heavy significance. Needlestick injuries often go unreported by healthcare professionals, and these injuries are widely prevalent. The researcher aimed to explore the job demands and resources that contribute to and mitigate against these injuries. Therefore support for the analysis of job demands and resources and the propensity to comply with NSI interventions exists. The results of the study suggested that there were mostly no significant differences between Job Demands and Resources and the propensity to comply with NSI Interventions scores. However significant relationships were found between length of shift and the propensity to comply with NSI interventions and growth opportunities and the propensity to comply with NSI interventions. The findings did not follow the proposed hypotheses that job demands would lead to a decrease in the propensity to comply with NSI interventions and job resources would lead to an increase in the propensity to comply with NSI interventions as longer length of shift (job demand) led to an increase in propensity to comply with NSI interventions and an association between high levels of growth opportunity (job resource), and low levels of propensity to comply with NSI interventions, was found. / GR2018
