De la formation préclinique à la formation clinique : explicitation du développement du raisonnement clinique chez des étudiantes et des étudiants en médecine ayant suivi un programme basé sur l'apprentissage par problèmes

Le but de l'étude est d'explorer le développement du raisonnement clinique selon une perspective multidimensionnelle et longitudinale en considérant le modèle multidimensionnel du raisonnement clinique et le concept de compétence. Les questions de recherche sont : (1) quelles dimensions du raisonnement clinique se manifestent chez l'étudiant? (2) quelles étapes de développement est-il possible de repérer? Il s'agit d'une étude de cas multiples explorant le développement de cinq dimensions : "Représentation", "Processus", "Métacognition", "Connaissances" et "Structures". Quatre étudiants furent rencontrés à quatre moments entre la fin de leur formation préclinique et le début de leur formation clinique (12 mois). Chaque étudiant réalisait (1) une entrevue médicale avec un patient standardisé; (2) un entretien d'explicitation à partir d'une vidéo; (3) une carte cognitive et (4) une entrevue semi-dirigée. Des analyses intracas puis intercas furent réalisées suivant les étapes de condensation, de présentation et d'élaboration. L'analyse permet de conclure que les cinq dimensions sont présentes chez les étudiants dès la fin de leur formation préclinique et qu'il existe une interdépendance entre ces dimensions. De plus, cinq transformations témoignent du développement du raisonnement clinique chez l'étudiant : (1) automatisation et adaptation des connaissances procédurales de la collecte d'informations; (2) déplacement de la cible de la régulation; (3) passage d'une recherche de cohérence entre les signes et les symptômes à l'évaluation des hypothèses; (4) passage du recours aux connaissances biomédicales à l'utilisation marquée des connaissances cliniques et (5) émergence de scripts. Ces transformations témoignent de deux grandes étapes de développement : (1) intégration de la démarche de la collecte d'informations et (2) passage d'une démarche de compréhension à une démarche de catégorisation. S'inscrivant dans l'interrelation recherche-formation-pratique, les conclusions et la méthodologie de l'étude invitent à poursuivre l'exploration du développement du raisonnement clinique chez l'étudiant en médecine ainsi que dans d'autres domaines de la santé.

Promoting Sociodramatic Play Between Children with Autism and Their Typically Developing Peers Using Activity Schedules

Young children with Autism Spectrum Disorder (ASD) demonstrate behavioral deficits and excesses that can adversely affect their play skills. Teaching children with ASD to use activity schedules with embedded scripts have led to increased appropriate game play with other children with autism and typically developing peers; however, there is sparse research on promoting more dynamic social play in children with ASD. The purpose of this study was to examine the effects of teaching the use of activity schedules with embedded scripts on the sociodramatic play of preschoolers with ASD with their typically developing peers. We also examined the extent to which we could remove scripts and schedule components and continue to observe appropriate sociodramatic play. Two participants with ASD quickly demonstrated high levels of sociodramatic play with their typically developing peers compared to baseline, and an additional participant with ASD demonstrated similar increases with procedural modifications. The participants also continued to show these increased levels after all scripts and nearly all components of the activity schedules were systematically removed, including during 1-and 2-week follow-up sessions. In addition, all participants engaged in additional unscheduled, yet contextually appropriate, sociodramatic play behaviors.

Dying of Encouragement: From Pitch to Production in Hollywood

Social scientists have long held that the media has a profound effect on modern societies. However, the cultural production of motion pictures and television shows has largely been neglected as a topic of inquiry. The following dissertation seeks to fill this lacuna in the current research by offering a systematic, comprehensive, and comparative analysis of the industry known colloquially as "Hollywood." Specifically, this dissertation seeks to uncover the matrix of causal processes that filter the infinite array of potential television shows and motion pictures to the chosen few that are selected for production. This process is known as "development and green lighting." Drawing from 110 interviews with writers, directors, producers, agents, managers, studio executives, network executives, financiers, and assistants who had been involved in the development and green lighting process, I explore not just decision making but the social milieu within which those decisions were made. Over the course of three chapters, three distinct social processes are examined in turn: institutional scripts ("Formulas"), status ("Stars"), and social capital ("Relationships"). Throughout the thesis, a new approach to cultural production is carried out, based on an inductive methodology where micro-level social processes are examined in the context of macro-level struggles over legitimacy, power, and resources. / Sociology

With increasing teenage pregnancy rates among Blacks in the United States and the negative impact on families, it is important that practitioners and communities acknowledge the changes in society. According to the research, the influence of the media, entertainment industry and technology weigh heavily on the behavior and interactions of teens. Building on Erikson’s Theory of Identity Development, sexual scripts which are drawn from hip-hop culture are utilized as points of entrance and tools for reeducating Black adolescents and preventing teenage pregnancy. Finally, intervention and prevention strategies that educate teenage girls around sexual scripts and utilize personal narratives are essential to reducing teen pregnancy are presented. / text

Sexual aggression and victimization among college students in Brazil : prevalence and vulnerability factors

Despite the increased attention devoted to sexual aggression among young people in the international scientific literature, Brazil has little research on the subject exclusively among this group. There is evidence that sexual aggression and victimization may start early. Identifying the magnitude and factors that increase the chance for the onset and persistence of sexual victimization are the first steps for prevention efforts among this group. Using both cross-sectional and prospective analyses, this study examined the prevalence of, and vulnerability factors for sexual aggression and victimization in female and male college students (N = 742; M = 20.1 years) in Brazil, of whom a subgroup (n = 354) took part in two measurements six months apart. At Time 1, a Portuguese version of the Short Form of the Sexual Experiences Survey (Koss et al., 2007) was administered to collect information from men and women as both victims and perpetrators of sexual aggression since the age of 14. The students were also asked to provide information on their cognitive representations (sexual scripts) of a consensual sexual encounter, their actual sexual behavior, use of pornography, and experiences of child abuse. At Time 2, the same items from the SES were presented again to assess the incidence of sexual aggression in the 6-month period since T1. The overall prevalence rate of victimization was 27% among men and 29% among women. In contrast, perpetration rates were significantly higher among men (33.7%) than among women (3%). Confirming the hypotheses, cognitive (i.e., risky sexual scripts, normative beliefs), behavioral (i.e., pornography use, sexual behavior patterns) and biographical (i.e., childhood abuse) risk factors were linked to male sexual aggression and to male and female victimization both cross-sectionally and longitudinally with the path models analyses demonstrating good fit with the data. The results supported: a) the role of the sexual script for a first consensual sexual encounter as an underlying factor of real sexual behavior and sexual victimization or perpetration; b) the role of pornography as “inputs” for sexual scripts, increasing indirectly the risk for victimization, and directly and indirectly the risk for perpetration; c) the direct and indirect link between childhood experiences of (sexual) abuse and male sexual aggression and victimization mediated by sexual behavior; and d) the direct link between child sexual abuse and sexual victimization among women. Few gender differences were found in the victimization model. The findings challenge societal beliefs that sexual aggression is restricted to groups with low socio-economic status and that men are unlikely to be sexually coerced. The disparity between male victimization and female perpetration rates is discussed based on traditional gender roles in Brazil. This study is also the first prospective investigation of risk factors for sexual aggression and victimization in Brazil, demonstrating the role of behavioral, cognitive and biographical factors that increase the vulnerability among college students. / Apesar do aumento da atenção dedicada ao fenômeno da agressão sexual entre jovens na literatura científica internacional, o Brasil tem pouca pesquisa no assunto exclusivamente neste grupo. Há evidências de que a agressão e vitimização sexual podem começar precocemente. A identificação da magnitude e dos fatores que aumentam a chance do surgimento e persistência de agressão sexual são os primeiros passos para a prevenção do problema. Usando delineamento transversal e prospectivo, o presente estudo investigou a prevalência e fatores de risco para a agressão e vitimização sexual em estudantes universitários de ambos os sexos (N = 742; M = 20,1 anos) no Brasil, dos quais um subgrupo (n = 354) participou em dois momentos separados por um intervalo de seis meses. Na primeira coleta (T1), uma versão em Português de Short Form of the Sexual Experiences Survey (SES) (Koss et al. / 2007) foi aplicada para obter informações sobre experiências de vitimização e agressão sexual em homens e mulheres desde os 14 anos de idade. Os estudantes também foram convidados a fornecer informações sobre suas representações cognitivas de um encontro sexual consensual (scripts sexuais), seu comportamento sexual real, o uso de pornografia e experiências de abuso na infância. Na segunda coleta (T2), os mesmos itens de SES foram apresentados para investigar a incidência de agressão sexual no período de seis meses desde T1. Os resultados mostraram que a taxa de prevalência de vitimização foi de 27% entre os homens e 29% entre as mulheres. Em contraste, as taxas de perpetração foram significativamente maiores entre os homens (33,7%) do que entre as mulheres (3%). Confirmando as hipóteses, variáveis cognitivas (scripts sexuais e aceitação normativa de risco), comportamentais (uso da pornografia e padrões de comportamento sexual) e biográficas (história de abuso na infância) constituíram fatores de risco para agressão sexual masculina e vitimização sexual feminina e masculina, tanto transversal quanto prospectivamente. Os resultados demonstram: a) o papel dos scripts sexuais como um fator subjacente ao comportamento sexual real e à vitimização ou perpetração sexual; b) o papel da pornografia como "input" para os scripts sexuais, aumentando, direta e indiretamente, o risco de perpetração e, indiretamente, o risco para vitimização; c) a ligação direta e indireta entre as experiências de abuso (sexual) infantil na agressão e vitimização sexual masculina mediada pelo comportamento sexual e d) a ligação direta entre o abuso sexual infantil e vitimização sexual entre as mulheres. Poucas diferenças de gênero foram encontradas no modelo de vitimização. Os resultados desafiam crenças de que a agressão sexual é restrita a grupos com baixo nível sócio-econômico e que homens não estão susceptíveis à coerção sexual. A disparidade entre as taxas de vitimização masculina e perpetração feminina é discutida com base nos papéis tradicionais de gênero no Brasil. Este estudo é o primeiro com delineamento prospectivo a investigar o papel de fatores comportamentais, cognitivos e biográficos na etiologia da agressão sexual no Brasil.

Sexual Scripts and Structured Action: Exploring Gendered Language in Cases of Female Sexual Offending

Few research studies have examined female sexual offending. Furthermore, most of what we know about sexual offending is based on male perpetrators. Our conceptions of female criminals who act outside their designated sexual scripts are formed by prevailing stereotypes of femininity. This research expands the available literature in the field of sexual offending, while examining how women are constituted when it comes to female child sexual offending. I conduct a critical discourse analysis of court transcripts and electronic news articles of cases involving women as child sexual perpetrators. Three criminal cases are examined from Wisconsin, U.S. The goal is to examine whether, and through what processes, traditional sexual scripts are discursively reproduced in court proceedings and media reporting of female sexual offending. Two cases reveal that women who adhere to their expected sexual scripts despite having committed a sexual offense receive sympathetic responses from legal officials and news journalists. The legal and media responses also ensure these women are able to continue their expected roles as women during and following sentencing. On the other hand, one case reveals that women who overtly contravene their expected gender scripts are legally and morally condemned. The call for strict punishment centred upon removing the female offender’s ability to participate in motherhood, because of the offender’s contravention of expected gender scripts by sexually offending against her own children. Furthermore, the male co-offender in this latter case was not deemed to have contravened his sexual scripts, resulting in less social condemnation. Qur'ans from the Eastern Islamic world between the 4th/10th and 6th/12th centuries

This thesis identifies and studies Qur’ans produced in the eastern Islamic world between the 4th/10th and 6th/12th centuries. The period coincides with major transformations in the environment of Qur’an production: the replacement of “Kufic” with newer types of scripts; the use of paper instead of parchment as a writing material; and the introduction of the vertical format, which gradually replaced the old horizontal format of Qur’an manuscripts. It was during this period that the Seljuqs and Ghaznavids rose to power alongside other local dynasties in the eastern Islamic world following the breakdown of the Abbasid Empire in the 4th/10th century. The boundaries between these different empires, however, did not prevent the mobility of craftsmen to, from and within Greater Iran. The extant Qur’ans from this period point to a shared visual vocabulary due to the fluidity of borders and the mobility of motifs. Yet, within this common language, local trends emerged defying unified dynastic or regional labels. The similarities and differences in Qur’ans produced in Iraq, Iran, Syria, the Jazira, Khurasan and Transoxiana attest to this idea. At the turn of the 5th/11th century, new scripts were being stylised while the illumination was in continuity with past traditions. Qur’ans that survive from Greater Iran, Baghdad and Cairo, studied in the first and second chapters, point to local manners of script and illumination stylisation. Some of their epigraphic and decorative forms find parallels on architecture, pottery, and coins pointing to the travel of motifs not only across geographic boundaries but also across artistic fields. The third chapter identifies a group of Qur’ans copied in the first half of the 5th/11th century in Nishapur and hence represent a local style of Qur’an production. Imperial Ghaznavid and Ghurid Qur’ans copied between the second half of the 5th/11th century and the 6th/12th century, studied in the fourth and fifth chapters, exemplify trends of Qur’anic script and illumination in Khurasan. The aesthetic of the Ghaznavid Qur’ans is rooted in earlier traditions with links to Baghdad and Nishapur while that of the Ghurid Qur’ans appear in continuity with the Ghaznavid yet with new features. Their visual vocabulary resonates with the local eclectic style of architectural decoration and the ceramics, metalwork, coins and silk produced in Greater Iran. A section of the fourth chapter investigates the work of al-warrāq al-ghaznawī (the Warrāq from Ghazna), a recurrent title in the colophons of Ghaznavid Qur’ans that points to a collaborative work environment, and offers insights into the production of these Qur’ans. Based on similarities with the Ghaznavid and Ghurid corpus, additional Qur’ans are attributed to Khurasan and Transoxiana in the sixth chapter. Their visual languages also draw from Qur’anic and non-Qur’anic artistic productions of Khurasan and largely that of Greater Iran. Finally, Qur’ans copied in the 6th/12th century in the Central Islamic lands appear to be mutually related and further apart from those produced in Khurasan yet with visible links. The seventh and eighth chapters examine the aesthetic diversity in Qur’ans produced in Iran. Iraq, the Jazira and Syria. Their distinct features point to local stylisation of script and illumination that was shaped from the fluidity of motifs throughout the Mashriq.
