Spatial diffusion of telemedicine in Sweden

“Telemedicine” denotes medicine at a distance using telecommunications and information technologies. The aim of the thesis is to reveal determinants and outcomes of telemedicine diffusion—that is, the spread of telemedicine in time and space. Telemedicine is examined both at the national level and in a regional case study. At the national level, quantitative methodology is utilized to analyze the emergence of health care facilities using telemedicine to receive medical assistance. The regional case study examines the diffusion and outcomes of telemedicine networks in Northern Sweden based on non-quantitative data sources. The first telemedicine applications in Sweden emerged between hospitals in southern Sweden in the 1970s. It was in the 1990s, however, that the practice of telemedicine took off on a larger scale. By the year 2000, the number of remote sites was quite evenly distributed across the country and between specialist and primary care facilities. In northern Sweden, telemedicine was first implemented in the mid-1980s, and is now carried out in all counties in the region. The telemedicine networks in northern Sweden primarily concern teleradiology, general telemedicine, and telepathology. Commonly, telemedicine is carried out within counties in hierarchical hub and spoke-type networks connecting specialist and primary care facilities. The study shows that existing telemedicine facilitates its further diffusion at the regional level, but shows a lack of the obvious neighborhood effect that could be expected in light of many previous diffusion studies and geographical diffusion theory. The health care system in Sweden is characterized by a high degree of regional autonomy. Contacts between medical staff, and thus dissemination of information and opinions concerning telemedicine, are therefore likely to be more prevalent within than between the regional health care organizations. The health care organizations are also decision-making bodies with different telemedicine policies. In addition, already implemented telemedicine programs tend to expand within the health care organization in which they originated. Although no traditional neighborhood effect to speak of can be noticed in Swedish telemedicine diffusion, distance matters in the diffusion process in a quite different respect. The study shows that health care facilities located far from more specialized care are especially likely to adopt telemedicine. Another local factor, facility size, is also positively correlated with telemedicine diffusion. This correlation between facility size and telemedicine adoption, as well as the circumstance that the diffusion process started with, and has progressed the furthest in, specialist care, suggest that the diffusion of telemedicine exhibits hierarchical characteristics. The development of telemedicine technology significantly influences the rate of telemedicine diffusion. In a simulation of the diffusion process, it is shown that—given that computers and information technology continue to develop according to the present trend—there will be a significant increase in the number of primary care facilities functioning as remote telemedicine sites during the first decade of the 21st century. / digitalisering@umu

Os tipos de difusão espacial da religião foram reconhecidos na prática territorial da Missão Metodista que veio dos Estados Unidos para o Brasil. A Geografia Histórica ajuda na análise dos fatores políticos e econômicos que favoreceram a ocupação do espaço pelos missionários protestantes no século XIX. Constatou-se que barreiras direcionaram os fluxos da inovação religiosa para outros locais. Os estudos de Geografia da Religião de Sack (1986) e Rosendahl (2012) serviram de referencial teórico para reconhecer a organização do território religioso da Igreja Metodista no Brasil e mostra a dimensão política da religião. As estratégias pelas quais o território religioso é reconhecido e preservado constituem as territorialidades no metodismo brasileiro. A Igreja Metodista no Estado do Rio Janeiro tem como meta o crescimento quantitativo e a expansão da sua área territorial. Visando dar continuidade ao processo de difusão espacial na atualidade, a gestão religiosa incentiva à organização da Igreja em Célula. Esta pesquisa de Geografia da Religião interpreta a experiência de fé para saber como os cristãos metodistas reconhecem a manifestação do sagrado no espaço.

Avec la globalisation des échanges et la nécessité de répondre à de nouveaux enjeux liés au changement climatique et à la crise énergétique, les agricultures africaines sont sollicitées pour produire de nouvelles cultures. L‘introduction de ces dernières dans les espaces productifs a connu des succès, comme le cas de l‘anacarde (noix de cajou) en Afrique de l‘Ouest ou des déconvenues, comme c‘est actuellement le cas du jatropha dans toute cette région. Ces résultats ont des intensités, des rationalités, des formes, des trajectoires diverses en fonction des espaces dans lesquels ils ont lieu. Ainsi au sein d‘un même pays, certaines régions s‘empareront de ces cultures alors que d‘autres les rejetteront plus ou moins rapidement. Comprendre les raisons de ces succès et échecs en relation avec leur localisation est un enjeu majeur pour accompagner au mieux ces changements. Ainsi, cette thèse questionne la forme et la dynamique d‘une innovation agricole au regard de son ancrage territorial. Actuellement, le concept de Système d‘Innovation (SI) offre un cadre d‘analyse systémique des processus d‘innovations. Mais ce dernier est généralement appliqué à des échelles nationales ou sectorielles ; la dimension territoriale n‘y est que rarement prise en compte. L‘objectif de cette thèse est de construire un cadre d‘analyse territorial de l‘innovation selon une approche fonctionnaliste, permettant d‘identifier les fonctions que le SI et le territoire doivent activer conjointement pour que l‘innovation puisse se diffuser. Ce cadre a été construit à partir de la littérature et d‘observations de terrain avant d‘être appliqué aux cas de l‘anacarde et du jatropha dans le sud-ouest du Burkina Faso. La démarche mise en place, allie des enquêtes, une analyse spatiale de l‘innovation et une grille d‘évaluation des relations entre fonctionnement du SI et fonctionnement des territoires. Les résultats mettent en évidence des facteurs de blocage et les synergies expliquant le degré d‘adoption de l‘innovation, tels que l‘accessibilité du territoire, sa capacité d‘échange, son système productif, la légitimité de ses institutions, etc. Ils permettent de comprendre la dynamique de l‘innovation agricole comme un processus d‘interactions successives avec le territoire et mettent également en lumière le rôle de l‘innovation dans les changements territoriaux en cours. La grille d‘analyse offre aussi des perspectives d‘utilisation prédictive pour évaluer le développement d‘une innovation selon les caractéristiques des territoires. Enfin, des recommandations sont formulées en faveur d‘actions localisées et priorisées afin d‘améliorer l‘efficacité des systèmes d‘innovation agricoles. / Due to globalization of trade and the need to fulfill the challenges raised by climate change and the energy crisis, African agricultures are required to grow new crops. Their insertion in productive spaces has been successful like the cashew nut in West Africa, or has encountered some setbacks as it is currently the case of jatropha across the region. These results have intensities, rationalities, forms and various trajectories, depending on the spaces in which they take place. Thus, within a country, some regions will be able to take hold of these crops whereas others won‘t and will reject it more or less rapidly. Understand the inner workings of success or failures in connection with their localization is a major issue to provide the best support to these changes. So, this PhD thesis questions the shapes and dynamics of agricultural innovation in terms of its territorial integration. Currently, the Innovation System (IS) concept provides a systematic analytical frame for innovation processes. But it is usually applied to national or sectorial levels; territorial dimension is rarely taken into account. The objective of this thesis is to build a territorial analysis of the innovation in a functionalist framework, identifying the functions that the IS and the territory must activate jointly for innovation to diffuse. This analytical framework has been built on the literature and on field observations, then it has been implemented on the cases of cashew nut and jatropha in the south-west region of Burkina Faso. The developed approach combines surveys, a spatial analysis of innovation and evaluation of the relations between IS and territories functioning. Results highlight synergies and blocking factors explaining the degree of adoption of innovation, such as the accessibility of the territory, its ability to exchange, its productive system, the legitimacy of its institutions, etc. They provide a means for understanding the dynamics of agricultural innovation as a process of successive interactions with the territory, and also highlighting the role of innovation in current territorial changes. The analytical framework provides also perspectives for a predictive approach to assess the future development of an innovation according to the characteristics of the territories. Finally, recommendations are made for localized and prioritized actions in order to improve the efficiency of agricultural innovation systems.

Propomos denominar essas áreas de barreiras geográficas de difusão, que dificultam ou impedem a transmissão do dengue nesses referidos clusters. As barreiras seriam componentes do Complexo Patogênico do Dengue, juntamente com as áreas nucleares e franjas, conformariam essa área de transmissão estável. Os complexos patogênicos foram definidos por Max Sorre (1933) e são constituídos pela extensão estável dos entes da cadeia epidemiológica de uma doença. Definimos como recorte empírico o estado de São Paulo. Como hipótese da pesquisa, aventamos que na análise da ampliação e consolidação do complexo patogênico do dengue no estado de São Paulo as barreiras geográficas de difusão do são formadas em relação aos determinantes socioespaciais em múltiplas escalas espaciais e temporais, modulando o processo de difusão da doença. O objetivo geral consiste em compreender o processo de ampliação e consolidação do complexo patogênico do dengue no estado de São Paulo e sua relação com a difusão espacial e as barreiras geográficas. Para tanto foi mapeada a difusão de vetores, da doença e mapeado o complexo no estado de São Paulo. / Thirty years after re-emerging dengue fever is already present in most parts of Brazil. Due to its environmental and social characteristics, almost the entire extension of national territory is propitious to the development of the disease. However, there are some municipalities’ clusters that have never notified autochthonous cases or that have rates and cases far below average. These municipalities’ clusters are in areas with similar environment and social characteristics, highlighting some geographic factor that prevents or limits the expansion/consolidation of the disease in that area. We propose nominate such areas as diffusion barriers, that difficult or blocks dengue transmission in these clusters. The barriers are components of the Dengue’s Pathogenic Complex, along with fringes and core areas, which conforms the stable transmission area. Defined by Max Sorre (1933), the Pathogenic Complex constitutes the stable spatial extension of a diseases transmission chain. We defined Sao Paulo state as the study area of the thesis. As hypothesis, we defined that in the analysis of the expansion and consolidation of the dengue fevers pathogenic complex in the Sao Paulo State the diffusion barriers are formed in relationship to the socio-spatial determinants in multiple temporal and spatial scales, modulating the diseases diffusion process. The aim of the thesis consists in comprehend the expansion and consolidation of dengue fevers pathogenic complex in Sao Paulo state and its relationship with spatial diffusion and geographical barriers. A desagregação desse fenômeno em escalas de maior detalhe, entretanto, permite-nos perceber que esse crescimento linear é na verdade o resultado de diversas dinâmicas locais, várias delas com características epidêmicas. Isto é, crescimentos abruptos fora do padrão histórico de dada localidade, portanto, crescimentos não-lineares. De acordo com a análise proposta no presente trabalho, identificamos Belém/PA enquanto um desses contextos epidêmicos de agressões letais. Tais expressões de violência naquela cidade vêm crescendo fora de sua dinâmica costumeira desde o início dos anos 2000 e a expressão geográfica desse fenômeno tem sido a difusão das mortes pelo espaço intraurbano. Dessa forma, propomos enquanto tese que tal propagação espacial da problemática, sendo uma típica expressão de difusão sobre o espaço geográfico, não se dá de forma homogênea em todas as direções a partir de um foco. Antes, segue por caminhos que oferecem condições mais favoráveis à epidemia (corredores de difusão) e evita aqueles mais desfavoráveis (barreiras à difusão). Enfim, no presente trabalho argumentamos no sentido de demonstrar essa dinâmica espaço-temporal peculiar do crescimento dos homicídios em Belém nos últimos anos, a partir de uma abordagem dos conceitos de epidemia e difusão espacial calcada em análise de dados espaciais. Ficou demonstrado, com base nessa análise, que os homicídios nessa metrópole brasileira têm alta correlação espacial, o que aponta para a necessidade dos fundamentos teóricos e metodológicos da Geografia na análise dessa problemática no contexto brasileiro. / Since at least 1980 the homicide mortality on Brazil has increased linearly at the same time of the urban slums has being shaped. However, in a larger zoom into the fenomeno, we can to see this linear growth is a result of several local dynamics, some of which are epidemic. That is, abrupt growth outside the historical pattern of a given locality, therefore, nonlinear growth. According to the analysis proposed in the present study, we identified Belém/PA as one of those epidemic contexts of lethal violence. Such expressions of violence in that city have been growing out of their usual dynamics since the early 2000s and the geographic expression of this phenomenon has been the spread of deaths through intraurban space. In this way, we propose as thesis that such spread of the problematic, as a typical expression of the diffusion on the geographical space, does not take place homogeneously in all directions from a focus. Indeed, it follows paths that offer more favorable conditions to the epidemic (diffusion channels) and avoid the more unfavorable ones (barriers to diffusion). Therefore, using a spatial data analysis approach, in the present work we argue to demonstrating this peculiar space-time dynamics of the present-day increase in the Belém’s homicides events. We do this from an approach of the concepts of epidemic and spatial diffusion based on spatial data analysis. It was demonstrated that the homicides in this Brazilian metropolis have a high spatial correlation, which points to the need of the theoretical and methodological foundations of Geography in the analysis of this problem in the Brazilian context.

On Monte Carlo Operators for Studying Collisional Relaxation in Toroidal Plasmas

This thesis concerns modelling of Coulomb collisions in toroidal plasma with Monte Carlo operators, which is important for many applications such as heating, current drive and collisional transport in fusion plasmas. Collisions relax the distribution functions towards local isotropic ones and transfer power to the background species when they are perturbed e.g. by wave-particle interactions or injected beams. The evolution of the distribution function in phase space, due to the Coulomb scattering on background ions and electrons and the interaction with RF waves, can be obtained by solving a Fokker-Planck equation.The coupling between spatial and velocity coordinates in toroidal plasmas correlates the spatial diffusion with the pitch angle scattering by Coulomb collisions. In many applications the diffusion coefficients go to zero at the boundaries or in a part of the domain, which makes the SDE singular. To solve such SDEs or equivalent diffusion equations with Monte Carlo methods, we have proposed a new method, the hybrid method, as well as an adaptive method, which selects locally the faster method from the drift and diffusion coefficients. The proposed methods significantly reduce the computational efforts and improves the convergence. The radial diffusion changes rapidly when crossing the trapped-passing boundary creating a boundary layer. To solve this problem two methods are proposed. The first one is to use a non-standard drift term in the Monte Carlo equation. The second is to symmetrize the flux across the trapped passing boundary. Because of the coupling between the spatial and velocity coordinates drift terms associated with radial gradients in density, temperature and fraction of the trapped particles appear. In addition an extra drift term has been included to relax the density profile to a prescribed one. A simplified RF-operator in combination with the collision operator has been used to study the relaxation of a heated distribution function. Due to RF-heating the density of thermal ions is reduced by the formation of a high-energy tail in the distribution function. The Coulomb collisions tries to restore the density profile and thus generates an inward diffusion of thermal ions that results in a peaking of the total density profile of resonant ions. / <p>QC 20130415</p>

L’évolution des mises en liens en réponse aux changements globaux et à la globalisation contemporaine précipite le risque d’émergence infectieuse. Dans cette recherche, nous dressons une géographie des vulnérabilités territoriales face à l’émergence des risques zoonotiques. Notre cas d’étude est celui des dynamiques d’invasion d’un hôte commensal, le rat noir (Rattus rattus), dans les marges orientales du Sénégal, périphérie rurale ouest-africaine en mutation. Nous défendons l’idée que les sociétés façonnent des systèmes territoriaux inégalement vulnérables à la diffusion spatiale de l’hôte.Nous montrons que les espaces du Sénégal Oriental et leurs mises en liens se transforment sous l’action des multiples acteurs territoriaux. Les dynamiques d’invasion du rat noir reflètent ces évolutions. Pourtant, les facteurs spatiaux de diffusion des lieux, des foyers, et des chemins d’invasion sont inopérants pour expliquer les dynamiques spatio-temporelles de l’hôte. En outre, la distribution spatiale du rongeur apparaît en contradiction avec les modèles classiques de diffusion fondés sur la hiérarchie ou la distance spatiale des lieux. La diffusion de l’hôte s’inscrit dans des systèmes complexes et multiscalaires tissés dans la rencontre des espaces, des lieux, et des liens construits par les sociétés. Nous proposons une grille de lecture où les formes de la diffusion dans l’espace et le temps répondent des degrés de stabilité et d’instabilité de ces systèmes socio-spatiaux, façonnant ainsi des territoires inégalement vulnérables au risque d’émergence infectieuse. / The evolution of spatial relationships in the contemporary context of global changes and globalization promote disease emergence. In this study, we draw a geography of territorial vulnerabilities to the emergence of zoonotic risks. In southeastern Senegal, a changing rural periphery of West Africa, we address the issue of the spread of an invasive species and host of pathogens, the black rat (Rattus rattus). We argue that societies produce territorial systems that are unequally vulnerable to the spatial diffusion of the host.We evidence that the spaces of Southeastern Senegal and the spatial relationships among them are transformed under the action of multiple social actors. The spread of the black rat reflects these changes. Yet, the spatial characteristics of the invaded places (such as connectivity or centrality) as well as the spatial relationships among them are insufficient in understanding the spatial and temporal dynamics of the rodent invasion. Moreover, the spatial distribution of the black rat contradicts conventional models of hierarchical or contagious diffusion. The spatial diffusion of the host responds to spaces, places, and spatial links intimately woven by societies into complex and multiscale systems. We propose to understand the many forms of spatial diffusion as the response to the stability or instability of these socio-spatial systems, which then form territories unevenly vulnerable to the risk of disease emergence.

Dans le développement récent des sciences de réseau, réseaux contraints spatiales sont devenues un objet d'une enquête approfondie. Spatiales des réseaux de contraintes sont intégrées dans l'espace de configuration. Leurs structures et les dynamiques sont influencées par la distance spatiale. Ceci est prouvé par les données empiriques de plus en plus sur des systèmes réels montrant des lois exponentielles ou de distribution d'énergie distance spatiale de liens. Dans cette thèse, nous nous concentrons sur la structure de réseau spatial avec une distribution en loi de puissance spatiale. Plusieurs mécanismes de formation de la structure et de la dynamique de diffusion sur ces réseaux sont pris en considération. D'abord, nous proposons un réseau évolutif construit en l'espace de configuration d'un mécanisme de concurrence entre le degré et les préférences de distance spatiale. Ce mécanisme est décrit par un a^'fc- + (1 — a)^'lL_,1, où ki est le degré du noeud i et rni est la distance spatiale entre les noeuds n et i. En réglant le paramètre a, le réseau peut être fait pour changer en continu à partir du réseau spatiale entraînée (a = 0) pour le réseau sans échelle (a = 1). La structure topologique de notre modèle est comparé aux données empiriques de réseau de courrier électronique avec un bon accord. Spatial constrained networks are embedded in configuration space. Their structures and dynamics are influenced by spatial distance. This is proved by more and more empirical data on real Systems showing exponential or power laws spatial distance distribution of links. In this dissertation, we focus on the structure of spatial network with power law spatial distribution. Several mechanisms of structure formation and diffusion dynamics on these networks are considered. First we propose an evolutionary network constructed in the configuration space with a competing mechanism between the degree and the spatial distance preferences. This mechanism is described by a ki + (1 — a), where ki is the degree of node i and rni is the spatial distance between nodes n and i. By adjusting parameter a, the network can be made to change continuously from the spatial driven network (a = 0) to the scale-free network (a = 1). The topological structure of our model is compared to the empirical data from email network with good agreement. On this basis, we focus on the diffusion dynamics on spatial driven network (a = 0). The first model we used is frequently employed in the study of epidemie spreading : the spatial susceptible-infected-susceptible (SIS) model. Here the spreading rate between two connected nodes is inversely proportional to their spatial distance. The result shows that the effective spreading time increases with increasing a. The existence of generic epidemic threshold is observed, whose value dépends on parameter a. The maximum épidemic threshold and the minimum stationary ratio of infected nodes simultaneously locate in the interval 1.5 < a < 2. Since the spatial driven network has well defined spatial distance, this model offers an occasion to study the diffusion dynamics by using the usual techniques of statistical mechanics. First, considering the fact that the diffusion is anomalous in general due to the important long-range spreading, we introduce a composite diffusion coefficient which is the sum of the usual diffusion constant D of the Fick's laws applied over different possible transfer distances on the network. As expected, this composite coefficient decreases with increasing a and is a good measure of the efficiency of the diffusion. Our second approach to this anomalous diffusion is to calculate the mean square displacement (l²) to identify a diffusion constant D' and the degree of thé anomalousness y with the help of the power law {l²} = 4D'ty. D' behaviors in the same way as D, i.e., it decreases with increasing a. y is smaller than unity (subdiffusion) and tends to one (normal diffusion) as a increases.
