Supporting Buddhist identity in long-term care situations

The Triratna/FWBO Buddhist movement has been associated with younger people and a lifestyle in which single sex residential communities and work projects (TBRLs) have been prominent. There is now a trend towards a wider range of lifestyles including fewer people living communally. Demographic changes include 50+ average age for ordained members and some people developing Long-Term Care (LTC) needs, with limited family and financial support. This raises questions as to the extent to which ‘Buddhist identity’ can be supported in LTC situations, with informal care, mainstream LTC services and possible care-based TBRLs all relevant.Data-gathering was initially on the basis of a general investigation of LTC issues in Triratna/FWBO using an eclectic, primarily qualitative design which had features of both a case study and a cross-sectional survey. 17 interviews included participants with current LTC needs, others asked to anticipate future care preferences, and people with relevant expertise. A questionnaire was formulated to explore attitudes to possible care-based TBRLs, with 107 participants and numerous additional comments. There was a strong consensus that Buddhist-based LTC services would, for example, provide better vegetarian diets and have an understanding of Buddhist names taken at ordination. These features were included in a conceptualisation of Buddhist identity which contrasted ‘Buddhist’ and ‘Non-Buddhist’ life, and noted ‘Dreams’ and ‘Nightmares’ as to LTC. Effective basic care was seen as essential to the general level of well-being needed for Buddhist practice, whilst a higher level of support might facilitate access to Buddhist ‘life goods’, and assist people in self-verifying themselves as committed Buddhists through ongoing practice. Informal support from fellow Buddhists was available in many contexts, but not at levels of intensity and duration characteristic of some family-based care, and it was seen as modulated by perceptions of burden, ‘busy-ness’ and other factors. There was a ‘legacy of suspicion’ of mainstream LTC, mainly focussed on residential care, with acknowledgement of some good/respectful carers and care services. TBRLs in LTC were generally welcomed, being seen as suitably altruistic work which might feature an atmosphere of ‘mutuality’ between staff and clients who were Buddhist or of Buddhist sympathy. Comparative material was used here from Methodist, Jewish and Lesbian/Gay/Bisexual/Transgender (LGBT) communities in LTC contexts, with the last of these conceptualised as comparable to the Western Buddhist community as a currently emerging identity in terms of later life services. Practicality and feasibility were discussed with reference to existing TBRLs, and experience of paid-for care input between Buddhists. The latter appeared supportive of Buddhist identity and readily linked to the personalisation agenda in social care. Buddhist-friendly services were seen as a possible context for generativity, and the concept of ‘a natural part of life’ was explored in terms of the emergence of LTC in the Triratna/FWBO movement, and of Buddhism as a gradually more familiar identity which might be encountered in the sphere of LTC.

The first chapter deals with branch terminology since its clear definition is essential, not only for dialogue inside the system of palliative care providers and experts, but for interdisciplinary communication and cooperation which is - according to the definition of palliative care - the foundation of its holistic approach to patients. The second and third chapters are concerned with the development of nursing care since the ancient times to the diversification of modern hospice-palliative care at the end of the twentieth century. ”Sanningen ska göra er fria” : En kvalitativ studie om prästers möte med häktade och fängslade människor i Sverige / ”The truth will set you free” : A qualitative study about priests meeting people in custody and prisons in Sweden

Föreliggande studie undersökte häktes/fängelseprästers upplevelse av förändring hos de frihetsberövade, vilka de mötte i sin tjänst på häkten och fängelser. Studien bygger på kvalitativa semistrukturerade intervjuer med fem präster i Svenska kyrkan. Intervjuerna genomfördes sommaren 2019. I studien framkommer det att alla intervjuade upplevde att förändringar sker hos de frihetsberövade. Det handlar om att kunna bryta destruktiva mönster och göra förändringar för ett bättre och friare liv. Vägen till denna förändring går genom samtal, gudstjänster och meditationer tillsammans med de själavårdande samtalen. Oväntat visar studien även på de förändringar som skedde hos prästerna själva där de beskriver sin upplevelse av djupare andlighet och stor meningsfullhet. Resultatet av studien visar att prästerna upplevde att de allra flesta frihetsberövade, har en stark önskan om ett icke kriminellt liv och att vägen dit kan gå genom fler och mer bestående möten och band till ett socialt accepterat sammanhang. / This current study investigated priests, in the church of Sweden, working in custody and/or prison. Do priests experience any change in the prisoners’ lives during the time they are held in prison? The qualitative study consists of semi structured interviews with five priests. They were interviewed during summer 2019. The study shows that all of the priests experienced that many of the prisoners changed attitudes concerning their lives and their situation. They expressed the desire of another kind of life. Unexpectedly the study also shows the change the priests undertook, working with the prisoners. The priests expressed feelings about deeper spirituality and a sense of rich meaningfulness. The study shows that the priests experienced that almost all of the prisoners had a strong wish of living a non-criminal life. The road to this life is the one which consists of many new and constant contacts with a socially accepted environment.

BAKGRUND: Omvårdnad skall bedrivas holistiskt enligt International code of etics (ICN) trots detta bedrivs den till viss del naturvetenskapligt med fokus på den medicinska delen, som i sig är viktig. Tydliga riktlinjer för andlig omvårdnad saknas och forskningen är begränsad. SYFTE: Syftet med denna studie var att utifrån aktuell omvårdnadsforskning beskriva faktorer som påverkar sjuksköterskors bemötande av frågor med existentiellt innehåll. METOD: Systematiskt litteraturstudie där sju artiklar och en avhandling ingick vilka var etiskt granskade. RESULTAT: Fyra kategorier identifierades, adekvat kompetens, tillåtande vårdmiljö, holistisk människosyn och reduktionistisk människosyn. Existentiellt, andligt behov har inte en självklar plats i omvårdnad och detta kan relateras till utbildning saknas samt utbildningsansvariga och vårdorganisationens förmåga att strukturera detta. Sjuksköterskor som har längre tids erfarenhet av att bemöta patienters andliga behov utför andlig omvårdnad holistiskt och kan utföra detta utan kliniska och vetenskapliga riktlinjer. SLUTSATS: För att sjuksköterskan skall kunna utveckla sin förmåga att bemöta andligt behov krävs utbildning både före och under yrkeskarriär. En sjuksköterska behöver också stöd från och samarbete med andra yrkeskategorier som präst, psykolog och/eller socionom. / BACKGROUND: Spiritual nursing should be a natural part of nursing due to holistic guidelines in accordance with the International code of ethics (ICN) for nurses, but that isn’t the case. Nurses tend to focus on the patients biological needs, which also is important. There are no current official guidelines for spiritual nursing and there is lack of scientific studies in this subject. AIM: The objective of this study was based on current studies, to describe factors that influence nurses’ respond to questions with spiritual content from patients. METHOD: A systematic review, based on seven scientific articles and one academic dissertation. RESULTS: Four categories were identified; adequate competence, a supportive healthcare environment and holistic outlook on people and reductionistic outlook on people. Patients’ spiritual needs are not always met, because of the nurses’ lack of education in the subject and support from the healthcare organization. Nurses, who have longer time experiences in responding to patients spiritual needs, are able to support patients in a holistic way without clinical and scientific guidance. CONCLUSION: It is important that the nurses’ needs for education in spiritual care are met, both before and during their career, so they can respond to patients’ spiritual needs and be able to provide holistic care. To maintain spiritual care the nurses also need support from the healthcare organization and other professions such as priest, psychologist and social worker.

Students' lived experience of spiritual nurturing in nursing education : a phenomenological study

The purpose of this thesis is to explore the phenomenon of spiritual nurturing as experienced by students in their fourth-year of an undergraduate baccalaureate nursing education program in Canada, using van Manen’s (2002) approach to phenomenology. The goal of this study is to describe the lived experience of spiritual nurturing of fourth-year nursing students in order to reflect on how nursing students learn spiritual care. Data were collected via in-depth interviews with seven fourth-year nursing students and analyzed according to van Manen’s interpretive approach. Three themes of spiritual nurturing emerged: spiritual nurturing as exchanging energy (spirituality as relationship with others), spiritual nurturing as recharging energy (spirituality as relationship with self), and spiritual nurturing as receiving energy (spirituality as relationship with transcending). Spiritual nurturing as it applies to nursing education, nursing practice, administration of nursing programs and nursing research are discussed from the students’ perspectives. / vii, 142 leaves : ill. The family meeting as an instrument for the spiritual care of palliative patients and their families.

Spiritual care is considered to be an important aspect of palliative care however; there has been much debate about the definition of spirituality. In this study a broad definition of spirituality has been utilized. Spiritual needs of palliative patients have been identified. Although evidence relating to bereavement outcomes supports the concept of working with family groups in the provision of holistic palliative care, only one family focused model of spiritual care was identified in the literature and no assessment of the efficacy of this model was found. The needs of staff in the provision of spiritual care have also been found to be important. The objectives of this study were to implement a family meeting model of care to assess it’s efficacy as an instrument for the provision of spiritual care from the perspective of palliative patients, family members and palliative care staff and to consider implications, if any for the inclusion of this model of care in the regular palliative care service. This qualitative study was informed by interpretivism and hermeneutic phenomenology. Ricoeur’s Theory of Interpretation was utilized in the interpretation of data. Participants were recruited through two metropolitan palliative care services. Twelve family meetings were facilitated by the researcher. Subsequently forty-seven patients and family members individually participated in in-depth interviews in which they described their experience of the family meeting and any outcomes they had observed. Fourteen staff members participated each in a semi-structured interview in which they described observed outcomes of the family meeting, their process of recruiting patient participants for the study and insights about the suitability of this intervention for regular palliative care services. Interviews were recorded, transcribed and analysed utilizing Ricoeur’s Theory of Interpretation and with the assistance of qualitative data management software. The most commonly identified outcomes for patient and family member participants were: experience of increased openness of communication, positive and constructive feelings and emotions, gaining new understandings and strengthening or renewal of significant relationships. Their experience of the meeting facilitation was generally positive. These outcomes suggest that spiritual care was provided that had not been made available to these people in the current system. Staff reported positive outcomes for patient and family member participants and considered that this type of intervention would meet a need within the palliative care service. Barriers to implementation such as funding priorities, staff numbers and time were identified. This original study has added to our understanding of the value of working with family units in the provision of spiritual care in palliative care services. Further research such as the application of this intervention to larger and less homogenous populations, the evaluation of long term benefits to family members and ways of identifying families most in need of this intervention are needed. This is a field research, descriptive, with a qualitative approach, having as a scope a nursing theory based on interactive process proposed by Dr. Callista Roy, who takes as a philosophical assumption that people have mutual relationships with the world and with God. The research was conducted at the Federal University of Paraíba, linked to the Health Sciences Centre and Department of Clinical Nursing, precisely in Nursing Techniques Laboratory, located in the city of João Pessoa / PB. The study gathered six nursing students of the tenth period selected by the inclusion criteria. Data were collected consecutively to the approval of this research by the Ethics Committee, under CAAE No 35391514.3.0000.5188. Therefore, with respect to data collection, it was held in October 2014, under the guidance of a form containing relevant issues to the study objectives, pointing out that, for the record data, a recording system was used audio, previously clarified as consentimento. Já term the technique used for this purpose was the focal group. Data analysis occurred through the Bardin content analysis technique. It was observed that the students correlate the care process with the exhibition held in the classroom and as enabler tool of development of their motor skills, rather than reflections on the essence of care. Thus, they did not know and need to set directly spiritual nursing care; before, correlated with spirituality and religiosity. Unanimously affirmed the importance of offering patients a spiritual nursing care, but the immaturity of the clinical practice, have not done due to numerous technical procedures to be performed, while also stating that the proposed curriculum of this institution favored this learning and, finally, that all people cared for them had a mutual relationship with the world and with God. However, it is nurses to perform and understand the importance of spiritual care, to exercise it continuously to view the inseparably patient, as it really is, taking into account their ethical and moral values grounded in pointing to a practice based on the scientific rigor, but also the love and commitment for others, moving away from the daily care mechanistic justified by arguments failed. / O presente estudo teve como objetivos averiguar o entendimento dos discentes de enfermagem a respeito do processo de cuidar em enfermagem, averiguar qual o entendimento destes sobre o cuidado de enfermagem espiritual, identificar se é necessário oferecer aos pacientes um cuidado de enfermagem espiritual, a partir da compreensão destes, descrever se a proposta curricular desta instituição é desenvolvida na perspectiva de favorecer uma prática compromissada com o ser humano em sua totalidade, envolvendo o cuidado espiritual, bem como identificar se as pessoas cuidadas pelos discentes em estágio curricular apresentaram ou demonstraram ter um relacionamento mútuo com o mundo e com Deus. Trata-se de uma pesquisa de campo, de natureza descritiva, com abordagem qualitativa, tendo como escopo referencial uma teoria de enfermagem baseada no processo interativo proposta pela Drª. Callista Roy, que assume como pressuposto filosófico que as pessoas têm relacionamentos mútuos com o mundo e com Deus. A investigação foi realizada na Universidade Federal da Paraíba, vinculada ao Centro de Ciências da Saúde e ao Departamento de Enfermagem Clínica, precisamente no Laboratório de Técnicas de Enfermagem, localizado no município de João Pessoa/PB. Participaram desta pesquisa seis discentes de enfermagem do décimo período, selecionados pelos critérios de inclusão. Os dados foram coletados consecutivamente à aprovação desta pesquisa pelo Comitê de Ética, sob CAAE nº 35391514.3.0000.5188. Para tanto, no que concerne à coleta de dados, a mesma foi realizada no mês de outubro de 2014, sob orientação de um formulário contendo questões pertinentes aos objetivos do estudo, ressalvando que, para o registro dos dados, foi utilizado um sistema de gravação de áudio, previamente esclarecido conforme termo de consentimento. Já a técnica utilizada para este fim foi o grupo focal. A análise dos dados ocorreu mediante a técnica de análise de conteúdo de Bardin. Observou-se que os discentes correlacionam o processo de cuidar com a exposição realizada em sala de aula e como ferramenta possibilitadora de desenvolvimento de suas habilidades motoras, em detrimento de reflexões sobre a essência do cuidar. Dessa forma, não souberam definir precisa e diretamente cuidado de enfermagem espiritual; antes, correlacionaram com espiritualidade e religiosidade. De forma unânime, afirmaram a importância de oferecer aos pacientes um cuidado de enfermagem espiritual, mas, pela imaturidade relacionada à prática clínica, não o fizeram devido a inúmeros procedimentos técnicos a serem realizados, apesar de afirmarem também que a proposta curricular desta instituição favoreceu esta aprendizagem e, por fim, que todas as pessoas cuidadas por eles apresentaram um relacionamento mútuo com o mundo e com Deus. Entretanto, cabe ao enfermeiro exercer e entender a importância do cuidado espiritual, de exercê-lo continuamente por visualizar o paciente de forma indissociável, como ele realmente é, levando em conta sua postura ética e moral firmada em valores que apontam para uma práxis fundamentada no rigor científico, como também no amor e compromisso pelo próximo, distanciando-se diariamente do cuidado mecanicista justificado por argumentos falidos.

Integrating spirituality and psychotherapy : experiences of a sample of terminally ill patients

The general aim of this study was to determine the experiences of a sample of terminally ill patients in using spiritually focused psychotherapy. This was a qualitative study conducted to a sample of 2 terminally ill patients from hospice in Grahamstown, South Africa. The research was conducted in 3 phases: an initial in-depth interview conducted to determine the participants' level of spirituality as well as the extent to which their terminal illnesses had affected their functioning. This was followed by a minimum of 6 spiritually focused therapy (SFT) sessions as a second phase of the research. To determine the participants' experiences of SFT, 2-3 in-depth interviews were conducted during the 3m phase of the research study. The research revealed that a belief in a higher power helps terminally ill patients cope better with their illness and that social disconnectedness is related to HIV / AIDS stigma. It also revealed that terminal illness is co-morbid with other psychiatric symptoms such as depression, evokes existential concerns, results in a change in the level of spirituality and affects the whole family. Participants blamed themselves for their illness, but found that engaging in the process of forgiveness of self and others brought about psychological healing for them. They experienced SFT as a coping resource that assisted them to deal with the fear of death as well as increased insight into the development of psychopathology and spiritual blockages. It is recommended that a comprehensive and holistic assessment during intake be undertaken so that where spiritual needs are available, therapy can be spiritually augmented to ensure that such needs/ struggles are addressed. / KMBT_363 / Adobe Acrobat 9.54 Paper Capture Plug-in

The diploma thesis "Spiritual Care for the Elderly as a Part of a Holistic Approach" addresses the question of how the support of spirituality can help seniors to cope with the difficulties of their lives such as the transformation of social and work roles, the loss of a partner or other close people, illness and dealing with the topic of mortality and the end of life, which usually come at this age, if not even before,. We are thinking about whether spirituality only affects people of faith or religion, or is given in some form to everyone. Thesis is looking for possibilities and approaches to spiritual care also for people who have not been interested in spirituality during their lifetime, have not expressed devotion, or turned away from for some reason. The interes tis given how spiritual care is provided in various senior homes in Prague as part of a holistic view on elderly. This work demonstrates various forms of giving this care, its use and evaluation from the perspective of clients, but also employees, volunteers and clergy.
