Pre-Licensure Nursing Student Attitudes Toward Physician-Assisted Suicide

Physician assisted suicide (PAS) has been a legalized presence in the United States since Oregon first passed the Death with Dignity Act in 1994. Now PAS is legalized in six states and it is realistic that nurses may encounter PAS during their career. This project explores pre-licensure nursing student attitudes toward PAS. A mixed method design incorporating descriptive correlation and thematic analysis of an open-ended question was used. Surveys were sent to 550 nursing students enrolled in the UCF nursing program asking participants to complete the 34-question survey. This survey included a 12 item “Domino scale” on student nursing opinions toward physician-assisted suicide, and a 23-item demographic scale. Complete, usable results were obtained from 231 participants. Demographic data revealed that the typical participant was between 18 and 25 years of age (80%), female (82%), single (87%), white (69%), in their first two semesters of the nursing program (60%), and unemployed (56%). The total scores for the Domino scale indicated a mean of 40. Regression analyses found that participant experience of someone having asked for help with PAS, and participant religiosity were significant predictors (F = 9.82, p = .0019; and F= 160.36, p < .0001) respectively of nursing student opinions on PAS as measured by the Domino scale. Qualitative analysis produced the following themes related to participant opinion on the nurse’s role in PAS: ways nurses can help with PAS, nurses should not be involved with PAS, clarification and delineation of the PAS process, the preservation of autonomy, the need for more education and inaccurate assumptions of PAS. This study showed that nursing students are moderately in support of PAS and willing to provide care to patients who are terminally ill regardless of a his or her personal decisions regarding PAS. Participants also comment that they desire additional education. Suggestions for further education, practice enhancements, research and policy development are discussed.

Specifically, it addresses links between domesticity and terminal illness amongst adults within the building typology of hospice. The objective of the practicum is the creation of a model for hospice design in rural Manitoba. The hospice will be supported in pre-fabricated housing. The practicum will address contemporary issues arising from publicly-funded healthcare, the aging Canadian population, and cultural issues surrounding dying, death, and grieving. The main goals to be achieved with this project were the creation of care facilities that encourage dialogue, education, and wellbeing for all users. Additionally, the facilities will be designed as multi-sensory environments where all human senses can be stimulated, expanding beyond typical visual stimuli.

Bakgrund: Livslängden ökar i samhället med den ständiga utvecklingen av teknik och medicin, samtidigt är det ett ökande antal personer som lever med svåra sjukdomar och lidande. Den palliativa vården i Sverige syftar till att lindra mångfacetterat lidande i livets slutskede. Allt fler länder i världen legaliserar dödshjälp. I Sverige är dödshjälp inte lagligt men ämnet är ständigt aktuellt genom debatter där dödshjälp diskuteras ur olika synvinklar och etiska aspekter. Trots att dödshjälp inte är lagligt finns det patienter som efterfrågar dödshjälp. Syfte: Syftet var att belysa faktorer och upplevelser som påverkar att patienter med en terminal sjukdom i livets slutskede önskar dödshjälp. Metod: En litteraturöversikt inkluderande tio kvalitativa vetenskapliga originalartiklar hämtade från PubMed och Cinahl COMPLETE. Resultat: Två huvudteman identifierades, autonomi och lidande. Undertema till autonomi var rätt att bestämma över sin egen död och dö med värdighet. Undertema till lidande var smärta, förluster och börda. Diskussion: Resultatet diskuterades med hjälp av Beauchamp och Childress etiska principer. Patienter önskar dödshjälp främst på grund av smärta och upplevelsen av att vara en börda. Om dessa lindras kan dock önskan om dödshjälp upphöra. Självbestämmande och livskvalitet är viktiga faktorer för patienten att få upprätthålla även i livets slutskede. / Background: Lifespan in society is constantly increasing with the constant development of technology and medicine, at the same time there is an increasing number of people living with severe diseases and suffering. The palliative care in Sweden aim to alleviate multifaceted suffering at the end of life. More and more countries in the world are legalizing euthanasia. Euthanasia is not legal in Sweden but the subject is constantly present through debates in which euthanasia is discussed from different point of views and through ethical aspects. Although euthanasia is not legal, there are patients who are requesting euthanasia. Aim: The aim was to identify factors and experiences that lead to terminally ill patients at the end of life request euthanasia. Method: A literature review including ten qualitative scientific original articles from PubMed and Cinahl COMPLETE. Results: Two main themes were identified, autonomy and suffering. Sub theme to autonomy was right to decide over one's own death and die with dignity. The sub themes to suffering were pain, losses and burden. Discussion: The results were discussed with help of Beauchamp and Childress ethical principles. Patients request euthanasia primarily because of pain and from the experience of being a burden. However, if these are alleviated, the desire for euthanasia can cease. Self-determination and quality of life are important factors for patients to maintain at the end of life.

Framework for the implementation of euthanasia in South Africa

This dissertation aims to examine and analyse the current South African position with regard to voluntary euthanasia. An examination is made from constitutional law, common law, case law and statutory law perspectives, including the legislation proposed by the South African Law Commission (project 86). The writings of prominent authors are considered. Once the South African position is examined, a comparative study is undertaken concerning relevant aspects in the Dutch law. The most important findings are that the South African Constitution may allow, and perhaps even demand, the legalization of voluntary euthanasia in South Africa, provided that sufficient safeguards can be established to effectively and sufficiently minimize the risk of abuse. Should this be impossible, the proscription of euthanasia may be reasonable and justifiable in an open and democratic society based on human dignity, equality and freedom. El objetivo de la presente investigación fue describir los atributos conceptuales de la muerte desde la óptica de la enfermera como cuidadora del paciente oncológico en fase terminal Chiclayo, 2023. El método propuesto para el estudio fue cualitativo-descriptivo. La Población lo conformó 20 enfermeros de la Red Lambayeque. La selección de los participantes se realizó por conveniencia, con muestra no probabilística, determinada por saturación y redundancia de los discursos. La técnica de recojo de información fue la entrevista semiestructurada con preguntas abiertas a profundidad, validada por juicio de expertos y registrada en un grabador de voz. El procesamiento de los datos se realizó mediante la Teoría fundamentada. Se garantizó la calidad científica y rigor ético de la investigación. Resultados: Se obtuvo tres Categorías: Categoría I: El sentir ante la muerte, subcategorías: límite natural, frustración ante las pérdidas, afectividad perturbada, rechazo a la muerte y descanso a sus sufrimientos. Categoría II: Espiritualidad y cuidado, subcategorías: La Espiritualidad en Enfermería: Separación de la Dualidad Cuerpo-Alma, Resignación ante la Voluntad Divina, Esperanza de una Vida Mejor. Coexistir con la Muerte mediante el Cuidado: Identificación y Acompañamiento, Cuidado Humanizado como un Arte de Cuidar y Descanso a sus Sufrimientos. Categoría III: Resiliencia de la enfermera, subcategorías: Evolución del significado de la muerte, valorar la vida y preparado para morir. La muerte es un hecho trascendental y único, es así como para el personal de enfermería que cuida del paciente oncológico tiene una conceptualización singular resultado de sus experiencias previas, saber y costumbres. / The objective of this research was to describe the conceptual attributes of death from the perspective of the nurse as caregiver of the terminally ill cancer patient Chiclayo, 2023. The method proposed for the study was qualitative-descriptive. The population was made up of 20 nurses from the Lambayeque Network. The selection of participants was made for convenience, with a non-probabilistic sample, determined by saturation and redundancy of the discourses. The information-gathering technique was a semi-structured interview with in-depth open questions, validated by expert judgement and recorded on a voice recorder. Data processing was carried out using Grounded Theory. The scientific quality and ethical rigor of the research were guaranteed. Results: Three categories were obtained: Category I: Feeling in the face of death, subcategories: natural limit, frustration in the face of losses, disturbed affectivity, rejection of death and rest from their sufferings. Category II: Spirituality and Care, subcategories: Spirituality in Nursing: Separation of Body-Soul Duality, Resignation to the Divine Will, Hope for a Better Life. Coexisting with Death through Care: Identification and Accompaniment, Humanized Care as an Art of Caring and Resting from their Sufferings. Category III: Nurse resilience, subcategories: Evolution of the meaning of death, valuing life and preparing to die. Death is a transcendental and unique event, which is how for the nursing staff who take care of the cancer patient it has a unique conceptualization resulting from their previous experiences, knowledge and customs.

This study was undertaken as a qualitative needs assessment of emotional support to the adolescent with a terminally ill parent. Five participants in their adolescent life phase (14 to 18 years) with a terminally ill parent, participated in the study. Individual unstructured interviews were conducted with each participant, in order to assess their need for emotional support. With the consent of the parents and adolescents, the interviews were video-recorded and transcribed. Eight recurring themes were selected from the case studies and verified with the existing literature.
