Infecção Experimental com Leptospira interrogans sorovariedade Canicola, relacionada à pesquisa de alterações oculares em cães /

Resumo: A uveíte em mamíferos tem sido relacionada à infecção por Leptospira spp., e, embora ocorra com maior frequência na espécie equina, já foi observada e relatada em outras espécies mamíferas, inclusive na humana. O objetivo deste trabalho foi investigar a relação entre a infecção por leptospiras e a ocorrência de alterações oculares em cães. Para a obtenção dos dados foram utilizados 32 cães, em idade reprodutiva, recolhidos pelo Centro de Controle de Zoonoses do Município de São José do Rio Preto, que apresentaram reação negativa, em duas provas de soroaglutinação microscópica (SAM) no intervalo de sete dias. Dos 32 cães, 20 foram inoculados com uma cepa patogênica de Leptospira interrogans sorovariedade Canicola e 12 formaram o grupo controle. Com a finalidade de avaliar as possíveis alterações do globo ocular post mortem, oito cães (cinco inoculados e três controles) foram sacrificados nos dias: sete, 15, 30 e 45 após o dia da inoculação. Amostras de humor aquoso, de um dos olhos de cada animal, foram retiradas para realização de PCR e SAM; o outro globo ocular foi retirado para realização de exame histopatológico e para a técnica de coloração de Levaditi. Nos dias zero, três, cinco, sete, 10 e após, a cada cinco dias, inclusive no dia do sacrifício, foram realizadas avaliações do estado físico do animal, do globo ocular e realizadas colheitas de sangue. O soro sanguíneo foi submetido à SAM para estabelecer os títulos obtidos em cada fase da infecção. Embora todos os animais tenham apresentado títulos sorológicos, indicando o sucesso da infecção, nenhum dos testes de detecção resultou positivo em até 45 dias de observação; no exame clínico apenas três animais apresentaram alterações perceptíveis: dois apresentavam irritação ocular, com hiperemia da esclera e um animal apresentava lacrimejamento... (Resumo completo, clicar acesso eletrônico abaixo) / Abstract: Uveitis in mammals has been related to Leptospira spp. And although it occurs more frequently in the horse has already been observed and reported in other mammalian species, including the human. The aim of this study was to investigate the relationship between leptospira infection and the occurrence of ocular changes in dogs. To obtain the data were used 32 dogs in the reproductive age, collected by the Center for Zoonosis Control in São José do Rio Preto, which showed negative reaction in both tests of microscopic agglutination test (MAT) within seven days. Of the 32 dogs, 20 were inoculated with a pathogenic strain of Leptospira interrogans sorovar Canicola and 12 formed the control group. With the aim of assessing possible changes of the eye post mortem, eight dogs (five inoculated and three controls) were sacrificed on days: seven, 15, 30 and 45 days after inoculation. Samples of aqueous humor of one eye of each animal were removed for PCR, and MAT, and the other eye was removed for histological examination and the staining technique Levaditi. On days zero, three, five, seven, 10 and, thereafter, every five days, including the day of sacrifice, were assessed the physical condition of the animal, the eyeball and blood samples. The serum was subjected to SAM to establish the qualifications obtained at each stage of infection. Although all animals have serologic evidence indicating the success of infection, no detection tests resulted positive in up to 45 days of observation and by clinical examination only three animals showed noticeable changes: two had eye irritation with redness of the sclera and one animal showed tearing. Efeitos do meloxicam e do carprofeno administrados por diferentes vias no controle da uveíte em cães (Canis familiaris - Linnaeus, 1758)

Realizou paracentese de câmara anterior em dois momentos (M0 e M1), com intervalo de cinco horas entre si. Em M0 e M1, colheram-se 0,2 ml de humor aquoso e determinou-se a concentração de proteína total e de prostaglandina E2 (PGE2). Em um primeiro período, constituíram-se quatro grupos (n = 5), que receberam meloxicam ao final de M0 pelas vias subcutânea (GIm), subconjuntival (GIIm) e tópica (GIIIm). Um quarto grupo não recebeu tratamento (Controle). Decorridos sete dias, os animais foram submetidos aos mesmos procedimentos adotados previamente e receberam carprofeno. Avaliação clínica foi também realizada, assim como histopatologia da conjuntiva dos animais dos grupos GIIm e GIIc. Os resultados foram avaliados estatisticamente (p LÜ 0,05). Em todos os grupos, encontrou-se aumento significativo dos níveis protéicos e de PGE2 em M1 (p < 0,001). Não se observou diferença significativa entre os grupos para os valores de proteína total e de PGE2 em M1 (p > 0,05). Observou-se correlação positiva entre proteína total e PGE2 (p < 0,05) apenas no GIm, GIc, GIIIm, GIIIc e GIIm. Exsudado inflamatório de caráter agudo e hemorragia discreta foram vistos à histopatologia após a aplicação de ambos os fármacos (p > 0,05). O meloxicam e o carprofeno foram ineficazes em inibir a síntese de PGE2 e o influxo de proteínas para a câmara anterior, por qualquer uma das vias testadas. A redução nos níveis de 44% proteínas, quando o carprofeno foi utilizado pela via tópica, sugere que por esta via, ele pode ser utilizado como adjuvante no controle da uveíte em cães. / Efficacy of meloxican and carprofen, administered by different routes, in experimental uveitis in dogs were studied. Anterior chamber paracenteses was accomplished at two different moments (M0 and M1), with a five hour interval among them. At M0 and M1, 0,2 ml of aqueous humor were collected and total protein and prostaglandin E2 (PGE2) concentration was determined. Four groups were formed in a first period (n = 5), which received meloxican at the end of M0, by the following routes: subcutaneous (GIm), subconjunctival (GIIm), and topical (GIIIm). A fourth group that received no treatment was instituted (Control). Seven days after, animals underwent the same procedures described previously and received carprofen. Clinical evaluation was also performed, as well as conjunctival histopathology of the conjunctiva of the animals of GIIm and GIIc. Results were evaluated statistically (p LÜ 0,05). In all groups, protein and PGE2 values enhanced significantly in M1 (p < 0,001). Protein and PGE2 values, did not change significantly between groups at M1 (p > 0,05). Positive correlation among total protein and PGE2 (p < 0,05) was only noted in GIm, GIc, GIIIm, GIIIc and GIIm. Inflammatory exudate of acute character and mild hemorrhage were seen at histopathology, after both agents were administered. Meloxican and carpofen were unable to inhibit PGE2 synthesis and the protein influx to the anterior chamber by any of the tested routes. The lowering of 44% in protein levels, when carprofen was used by the topical route, suggests that by this route, it can be used as an adjuvant to control uveitis in dogs.

Estudo eletrofisiológico dos efeitos da injeção intravítrea de ácido micofenólico (mpa) na retina de coelhos

O presente trabalho utilizou o eletrorretinograma (ERG) em coelhos (Oryctolagus cuniculus) para avaliar uma forma alternativa de aplicação do ácido micofenólico no tratamento da uveíte, uma doença ocular inflamatória. Uveíte é o termo utilizado para definir a inflamação da íris, coróide e corpo ciliar. O ERG é um registro de potenciais elétricos originados na retina em resposta à estimulação luminosa. O objetivo desse estudo é analisar alterações em funções da retina em olhos tratados com diferentes doses intravítreas de ácido micofenólico. Foram medidas amplitude e latência das ondas a e b do ERG para a função de intensidade-resposta e de resolução temporal. O micofenolato de mofetila (MMF) é um potente medicamento imunossupressor e vem sendo empregado com sucesso no tratamento de uveítes. O MMF é uma pró-droga, que é transformada no fígado em ácido micofenólico (MPA), o imunossupressor ativo. O uso de MMF causa efeitos colaterais e, para diminuir esses efeitos, este estudo analisa a injeção intravítea de MPA em coelhos. Foram utilizados um total de quarenta animais, divididos em seis grupos. Primeiramente, foram realizados exames eletrorretinográficos em olhos intactos, sem administração de nenhuma substância, para determinação dos parâmetros dos registros. Foram testados também os olhos injetados apenas com solução salina balanceada (SSB). Após essa etapa, foram feitos testes com olhos injetados apenas com o veículo (Polissorbato 80). Esse procedimento foi necessário para estabelecer se o veículo pode ser utilizado ou se apresenta algum grau de toxicidade para a retina. As doses usadas para avaliação da toxicidade da droga foram cinco: 5 g/ml, 50 g/ml, 200 g/ml, 1.000 g/ml e 10.000 g/ml. Os exames eletrorretinográficos foram realizados 7 e 30 dias após injeção intravítrea de MPA. Os resultados indicam que não houve alteração nos parâmetros de ERG nos olhos que receberam apenas o veículo. Também não acusaram indícios de toxicidade os olhos tratados com as doses de 5, 50 e 200 g/ml da droga. Apenas com as doses mais altas, de 1.000 e 10.000 g/ml, os olhos apresentaram diminuição de sensibilidade ao estímulo luminoso. Não é possível afirmar que as células da retina não foram danificadas pela injeção intravítrea de MPA, pois o ERG de campo total não detecta lesões pontuais. O pequeno número de olhos analisados e a necessidade de utilização de anestésicos podem levar a interpretações equivocadas dos resultados. As substâncias anestésicas podem afetar a neurotransmissão e, consequentemente, os resultados obtidos nos registros do ERG. As análises dos dados de amplitude e tempo implícito das ondas a e b foram muito consistentes e apresentam variações compatíveis com a literatura / This work uses the application of the electroretinogram (ERG) in rabbits (Oryctolagus cuniculus) in order to evaluate an alternative way of using the mycophenolic acid for treatment of uveitis, an inflammatory eye disease. Uveitis is an inflammation of the iris, choroid and ciliary body. The ERG is the record of electrical responses originated in the retina to luminous stimulation. The purpose of this study is to analyse changes in the retinal functions of eyes treated with distinct intravitreous doses of mycophnolic acid. A and b waves amplitude and implicit times for the intensity-response and temporal resolution functions. Mofetil mycophenolate (MMF) is a powerful immunossupressant drug successfully used for treating the uveitis. It is a pro-drug that is transformed in the liver into mycophenolic acid (MPA), the active immunossupressor. MMF causes collateral effects and, in order to reduce these effects, the intravitreous MPA injection in rabbits is analysed in this study. Forty rabbits were used, divided in 6 groups. First of all, electroretinografic exams in control eyes were made, without the application of any substance, for determining the recording parameters. Then were tested the eyes injected with only balanced saline solution (BSS). After that, tests were made in eyes injected only with the vehicle (Polissorbato 80). This procedure was necessary for finding out whether the vehicle may be used or is in some degree toxic for the retina. The doses for evaluation of the drug toxicity were five: 5 g/ml, 50 g/ml, 200 g/ml, 1.000 g/ml e 10.000 g/ml. By the examination of the distinct parameters of the ERG, several visual functions can be evaluated (inner and outer retina) and possible impairments in the temporal resolution, in the relationship luminous intensity and amplitude of the response, and in other functions. The results show there were no changes in the ERG parameters for the eyes where only the vehicle was applied. Also, no toxicity signs were detected in 5, 50 or 200 g/ml doses. Sensitivity to the luminous stimulus was only shown in eyes treated with higher doses, or, 1.000 and 10.000 g/ml. It is not possible to assert that the retina cells were not injured by the MPA injection, as the total field ERG does not detect spot lesions. The small quantity of tested eyes and the usage of anesthetics may lead to erroneous interpretations of the results. The anesthetic substances can affect the neurotransmission and consequently the results of ERG records. The analysis of the a and b wave amplitude and implicit time were very consistent and show variations consistent with the pertinent literature

Os objetivos foram identificar e determinar os alelos HLA-DR e HLA-DQ em uma amostra da população infanto-juvenil piauiense com AIJ nas formas oligoarticular, sistêmica, poliarticular FR+, poliarticular FR-, artrite psoriásica, artrite relacionada a entesite e artrite indeterminada e compará-las com as frequências observadas no grupo de controle saudáveis; conhecer os alelos HLA-DR e HLA-DQ que estão associados a maior susceptibilidade e os que conferem maior proteção na população de crianças com AIJ em suas diversas formas. As tipagens dos alelos HLA-DR e HLA-DQ foram realizadas por meio da técnica de amplificação pela Reação de Cadeia da Polimerase (PCR), utilizando cadeias específicas de primers DR e DQ. O resultado da análise demostrou que, entre todas as formas de apresentação da AIJ, houve associação estatística significativa para a susceptibilidade da doença com o HLA-DRB1*10 OR 8,8 (IC 1,1 a 74,9) e HLA-DRB1*16 OR 2,8 (IC 1,1 a 7.5). Na forma oligoarticular a associação estatística significativa para a susceptibilidade ocorreu com o alelo HLA-DRB1*08 OR 4,6 (IC 1,4 a 14,6). Na forma sistêmica, essa associação aconteceu com o alelo HLA-DRB1*10 OR 22,2 (IC 1,8 a 269,5).Na forma Poliarticular FR+ a associação estatística significativa para a susceptibilidade ocorreu com os alelos DRB1*09 OR 12,1 (IC 2,2 a 66,9) e DRB1*10 OR 28,5 (IC 2,3 a 355,0). Na forma Poliarticular FR-, a associação estatística significativa para a susceptibilidade foi com o alelo DRB1*16 OR 13,4 (IC 1,6 a 111,2). A Artrite Psoriásica não apresentou nenhuma associação estatística significativa com os HLA pesquisados. O resultado da análise demostrou que, entre todas as formas de apresentação da AIJ, houve associação estatística significativa para a proteção da doença com o HLA-DRB1*03 OR 0,7 (IC 0,5 a 0,9) e, na forma sistêmica, essa associação aconteceu igualmente com DRB1*03 OR 0,7 (IC 0,6 a 0,8). As outras formas de apresentação da doença não demonstraram associação estatística significativa para a proteção com nenhum tipo da HLA pesquisado. A população estudada não apresentou nenhum caso de ERA e de Artrite indeterminada. O estudo encontrou ainda, associação para o risco entre o HLA-DQB1*03 OR = 6,06 (IC 1,3 a 27,2) e uveíte em pacientes com a forma oligoarticular da AIJ. Desta forma, concluímos que os nossos resultados apresentam tanto semelhanças em relação a susceptibilidade, quanto diferenças, principalmente quanto a proteção, nas associações tipicamente encontradas na literatura / Abstract: This case-controle valuated 74 children from Piauí with Juvenile Idiopathic Arthritis (AIJ) in its various forms: Oligoarticular, Systemic, Polyarticular Rheumatoid Factor Positive (FR +), Polyarticular Rheumatoid Factor Negative (FR-), Enthesitis-related Arthritis (ERA), Arthritis Psoriatic and Arthritis indeterminate, classified according to the Criteria of the International League of Associations for Rheumatology(ILAR), and one hundred and one healthy control smatched according to age ,sex, origin and ethnicity. The objectives were to identify and determine the HLA-DR and HLA-DQ in a sample of Piauí juvenile population subtypes JIA in oligoarticular, systemic, polyarticular RF + polyarticular RF -, psoriatic arthritis, enthesitis-related arthritis and arthritis indeterminate and compares them with the observed frequencies in the group of healthy control; know the HLA-DR and HLA-DQ alleles are associated with increased susceptibility and conferring greater protection in the population with JIA in its various forms and identify a possible relationship between the alleles HLA-DR and HLA-DQ and the most frequent and most aggressive form of the disease in the population studied. Polymerase Chain Reaction (PCR) using primers specific chains of DR and DQ performed the typing of HLA-DR and HLA-DQ using the technique of amplification. The result of the analysis demonstrate damong all cases of JIA was statistically significant association for disease susceptibility with HLA-DRB1*10 OR 8.8 (CI 1.1 to 74.9) and HLA-DRB1*16 OR 2.8 (CI 1.1 to 7.5). In oligoarticular JIA significant statistical association to susceptibility occurred with HLA-DRB1*08 allele OR 4.6 (CI 1.4 to 14.6), association systemic form happened to HLA-DRB1*10 OR 22.2 (CI 1.8 to 269.5) allele in polyarticular RF + the statistically significant association to susceptibility occurred with the DRB1*09 alleles OR 12.1(CI 2.2 to 66.9) and DRB1*10 OR 28.5(CI 2.3 to 355.0) in polyarticular RF- significant statistical association to susceptibility was with DRB1*16 allele OR 13.4 (CI 1.6 to 111.2). The Psoriatic Arthritis showed no statistically significant association with HLA surveyed. The result of the analysis demonstrate damong all cases of JIA was statistically significant association for disease protection with HLA-DRB1*03 OR 0.7(CI 0.5 to 0.9) and the systemic form this association also happened DRB1*03 OR with 0.7 (CI 0.6 to 0.8). The other forms of the disease showed no statistically significant association for protection on any type of HLA searched. The study population did not show any cases of ERA and indeterminate Arthritis. The study also found, statistically significant difference between the HLA-DQB1*03 OR 6.0 (CI 1.3 to 27.2), and uveitis in patients with oligoarticular form of JIA. Etude des mécanismes immunitaires des uvéites idiopathiques par une approche biologique et l'optique adaptative / To determine the immune mechanisms in idiopathic uveitis by a biological approach and adaptive optics

La majorité des modèles d’uvéites expérimentales sont médiées par les sous-types lymphocytaires CD4+ T-Helper 1 (Th1) et Th17. Le but de cette thèse est d’étudier les mécanismes immunitaires mis en jeu lors d’une uvéite idiopathique (sans étiologie) chez l’humain. La première partie de cette étude est donc consacrée à l’étude du profil biologique de 30 cytokines/chimiokines/ facteurs de croissance intraoculaires (AqH) et sériques obtenues au laboratoire (technologie Luminex®). Ces médiateurs sont dosés dans le sérum et l’AqH de patients atteints d’uvéites intermédiaires et postérieures activesidiopathiques. Ces résultats sont comparés à une cohorte de patients non inflammatoires et à des groupes de patients atteints d’uvéites de diverses étiologies. Nous avons trouvé que le profil des médiateurs diffère nettement en fonction de la cause de l’inflammation intraoculaire. Une réponse pro-inflammatoire large est observée avec des concentrations accrues des cytokines de type Th1 (IFNγ, IL-12), Th17 (IL-17), TNF-α, IL-6, MIP-1α, MIP-1β, MCP-1, G-CSF et IP-10 dans les uvéites idiopathiques. Les concentrations sériques différent de celles des contrôles par les taux d’IL-17 et IP-10 augmentés. La deuxième partie concerne l’étude des mécanismes immunitaires des uvéites par l’analyse des vascularites en imagerie adaptative (FIOA) et particulièrement l’observation anatomique des modifications liées aux inflammations oculaires. Nous ne trouvons pas de corrélation entre l’aspect de cellularité périvasculaire en FIOA et les cytokines retrouvées dans l’AqH des yeux avec uvéite. Pouvoir déterminer quels médiateurs de l’inflammation interviennent dans les pathologies inflammatoires permettrait de les cibler avec des thérapies adaptées. / Most of experimental models of uveitis are mediated by lymphocytic subtypes CD4+ T-Helper 1 (Th1) et Th17. The aim of this thesis is to study the immune mechanisms in idiopathic uveitis (i.e. unknown etiology) in humans. First, we will determine the biological spectrum of 30 cytokines/ chemokines/ growth factors in aqueous humour and sera by Luminex® technology. Those mediators will be measured in the serum and aqueous humour of patients with active intermediate and posterior uveitis classified as idiopathic. Those results will be compared to a cohort of patients with no inflammation and to patients with uveitis of various causes. We found that the profile of mediators changes regarding to the cause of the intraocular inflammation. The proinflammatory process is related to increased levels of cytokines Th1 (IFNγ, IL-12), Th17 (IL-17), TNF-α, IL-6, MIP-1α, MIP-1β, MCP-1, G-CSF and IP-10 idiopathic uveitis. The sera of the patients with idiopathic uveitis have increased levels IL-17 and IP-10. Secondly, the immune mechanisms of uveitis were studied by adaptive optics imaging (FIOA) and particularly the observation of anatomic changes related to ocular inflammations. We ruled out any correlation betweeen the perivascular cellular opacification in FIOA and the cytokines/ chemokines found in the eyes with uveitis. An understanding of the profile of mediators in inflammation and their determination could help to target idiopathic uveitis with correct therapies.

Differential Diagnosis of Pan-Uveitis: Behçet’s Disease

This report describes the case of a 56-year-old man who presented with blurry vision, increased intraocular pressure, and conjunctival injection after posterior chamber intraocular lens implantation. Initially post-operative endophalmitis and foreign body inflammation were considered as differential diagnoses, but after further examination pan-uveitis was diagnosed. Uveitis is an ocular finding that may indicate several diseases, one of which is Behçet’s Disease. During the interview, the patient mentioned a history of apthous ulcers and genital ulcers which then lead to the clinical diagnosis of Behçet’s Disease. This report emphasizes that Behçet’s Disease is rare in Caucasians. Therefore, is frequently misdiagnosed in North America due to variable presentations and by not exploring the option when analyzing differential diagnoses. Early diagnosis and intervention will prevent the development of blindness and fatality due to complications of the disease.

Topical ophthalmic NSAIDs are commonly employed in the management of intraocular inflammation (uveitis), corneoconjunctival inflammatory disease and pre-operatively to prevent intraoperative miosis during cataract surgery. Despite their routine application in these clinical scenarios, little is known regarding the corneal penetration and relative anti-inflammatory efficacy of the available topical ophthalmic NSAIDs in the dog. Decisions regarding which of these agents to employ are therefore based upon factors such as cost and ease of acquisition as opposed to established efficacy. Objectives: To investigate the relative intraocular penetration and anti-inflammatory efficacy of two commonly utilized topical ophthalmic NSAIDs in dogs, diclofenac 0.1% and ketorolac 0.5%. Animals: Twenty-two client owned dogs (22 operated eyes) presenting to the VTH ophthalmology service for routine cataract surgery for mature or hypermature cataract. Methods: Subjects were randomized to be treated with either topical ketorolac 0.5% or topical diclofenac 0.1% ophthalmic solutions at specified times in the 24-hour period pre-operatively. Aqueous humor samples were obtained intra-operatively and stored for subsequent evaluation of drug concentrations and prostaglandin E2 (PGE2) concentrations via ultra performance liquid chromatography-mass spectrometry (UPLC-MS/MS) and enzyme-linked immunoassay (ELISA) analysis, respectively. Results: Median aqueous humor drug concentrations were significantly higher in dogs treated with ketorolac 0.5% (1311.6 ng/mL) compared to those treated with diclofenac 0.1% (284.9 ng/mL). There was no significant difference in aqueous humor PGE2 concentrations between the two treatment groups. No significant association was determined between aqueous humor drug concentration and PGE2 concentration. There was no significant association between diabetic status and aqueous humor drug concentration or PGE2 concentration in either group. Conclusions and clinical importance: This study suggests that topical ketorolac 0.5% and diclofenac 0.1% are efficacious in decreasing aqueous humor PGE2 concentrations and are equally suitable for use based on their comparable anti-inflammatory profiles. The purpose of this study, therefore, is to investigate the relative intraocular penetration and anti-inflammatory efficacy of two commonly utilized topical ophthalmic NSAIDs in dogs, diclofenac 0.1% and ketorolac 0.5%. Twenty-two dogs presenting to the VTH ophthalmology service for routine cataract surgery with the presence of a mature or hypermature cataract were enrolled in a prospective, randomized clinical trial. The role of microorganisms and other factors in the process of autoimmune uveitis Abstract The aim of this work was to gain new knowledge about the influence and the mechanisms of the effect of microorganisms in the process of autoimmune uveitis. A mouse model of experimental autoimmune uveitis (EAU) was used and the influence of oral broad-spectrum antibiotic, i.e. metronidazole, as well as probiotic bacteria Escherichia coli O83:K24:H31 (EcO) nad Escherichia coli Nissle 1917 (EcN) were tested in preventive and therapeutic regimens. The grade of inflammation was assessed clinically in vivo and histologically post mortem. Immunological analysis of lymph nodes and Peyer's patches were performed. Evaluation of the effect of metronidazole proved significant reduction of inflammatory activity in both regimes - intitiation 1 week or 2 weeks before the EAU induction. In case of probiotics, protective effect was proved only in case of live EcN administered 2 weeks before or from the time of EAU induction. Its protective effect was accompanied with decreased interphotoreceptor retinoid-binding protein (IRBP)-specific T-lymphocyte response in the sentinel lymph nodes of the site of immunization 7 days after the induction of EAU and cervical lymph nodes as soon as there were apparent clinical signs of intraocular...

Prevalence, Risk Factors, Characteristics andComplications of Uveitis in a Swedish Cohort ofChildren with Juvenile Idiopathic Arthritis

IntroductionChildren with juvenile idiopathic arthritis (JIA) are at risk of developing sight threatening JIAassociated uveitis (JIA-U). Swedish children with JIA are included in an ophthalmologicalscreening program, based on European studies, to ensure early detection and treatment of JIAU.AimThe aim of this study was to describe the prevalence, risk factors, clinical characteristics andassociated ocular complications of JIA-U and pharmacological therapy in a cohort of Swedishchildren with JIA. MethodsThe study was conducted as a retrospective review of medical records of children with JIAtreated at Örebro University Hospital, Sweden in 2010-2019, with a minimum of one year ofophthalmological follow-up.ResultsMedical records of 200 children with JIA were reviewed, of which 96 children were includedin the study. Uveitis was detected in 21/92 (22.8%) children. Boys developed uveitisproportionally more often than girls. JIA-U onset within the first four years after JIA diagnosisoccurred in 52.4% and onset occurred later in boys than in girls (p = 0.019). Oligoarthritis wasthe only significant risk factor for uveitis development (OR 3.111, 95% CI 1.029-9.409, p =0.044). Uveitis associated complications occurred in 7/21 (33.3%) children. ConclusionIn this regional cohort study, we found a higher prevalence of JIA-U, a higher proportion ofboys developing uveitis and a lower portion of children developing uveitis within the four yearsof high frequency screening compared to recent Swedish respectively European studies. Ourfindings indicated the need of future Swedish studies to revise and optimize the currentSwedish guidelines for ophthalmological JIA-U screening.

Experimentelle Melanin-induzierte Uveitis (EMIU): Modulation der Leukozyten-Endothelzell-Interaktion durch Makrophagendepletion - intravitalmikroskopische Analysen. Einleitung: Die Experimentelle Melanininduzierte Uveitis (EMIU) dient als Modell für eine autoimmune Iridozyklitis und Choroiditis. Die frühe Entzündungsreaktion ist durch eine gesteigerte Leukozyten-Endothel-Interaktion gekennzeichnet. Um die Rolle von Makrophagen bei der Induktion der EMIU zu untersuchen, analysierten wir Veränderungen der Leukozyten-Endothel-Interaktionen in Irisvenolen anästhesierter Ratten nach Makrophagendepletion mit liposomalem Clodronat. Methoden: Die EMIU wurde durch intraperitoneale Injektion einer Emulsion aus 250 µg bovinen Melanosomen in komplettem Freund Adjuvant und Pertussistoxin bei Lewis Ratten induziert. Die Tiere wurden mit 2 ml Clodronat-Liposomen (Clodronat-lip) an den Tagen 2; 1; 4; 6 beziehungsweise 8 nach Immunisierung behandelt. Kontrolltiere erhielten anstelle von Clodronat-lip Leerliposomen (Kontrolle). Für die Intravitalfluoreszenzmikroskopie wurden Leukozyten intravasal mit Rhodamin 6G gefärbt. Anschließend wurden die postkapillären Irisvenolen am 4.; 6.; 8. und 10. Tag untersucht, um die Zahl der rollenden und fest am Endothel adhärenten Leukozyten zu quantifizieren. Weitere Parameter wie Zellzahl und Proteingehalt des Kammerwassers, TNF-alpha und IFN-gamma im Plasma und das Differentialblutbild wurden zur Charakterisierung der Entzündungsreaktion herangezogen. Ergebnisse: Bei makrophagendepletierten Tieren konnten spaltlampenmikroskopisch keine entzündlichen Veränderungen des Vorderabschnittes beobachtet werden.
