Lean Manufacturing es una filosofía centrada en reducir desperdicios que trae consigo impactos positivos en los resultados de una empresa. Sin embargo, este tipo de técnicas no son del todo compatible con el contexto de la industria minera. El estudio presenta un modelo que reúne las bases de aplicación de un sistema de Mantenimiento Productivo Total (TPM) como el inicio hacia la transformación Lean para una compañía minera. Este modelo toma como referencia modelos y casos de éxito que se implementaron anteriormente y es adaptado para garantizar el éxito de su aplicación. Asimismo, evalúa la importancia de utilizar un indicador adaptado a la minería como es el OEE minero y el MPI. Un caso de estudio es realizado para evaluar la potencial aplicación de la propuesta, diseñando los componentes principales del modelo propuesto para atacar cada uno de los problemas encontrados con el objetivo de incrementar la eficiencia de los equipos mineros. A través de un proceso de simulación para los volquetes se evaluó los efectos de aplicación de la propuesta, llegando así a un posible incremento de hasta +20.0pp en el OEE de esta familia de equipos. Este modelo puede ser aplicada a otros entornos de minería ya que es totalmente flexible en cada uno de sus pasos. A su vez, la implementación de estas estrategias crea una filosofía de trabajo centrada en la excelencia operativa para el área de mantenimiento. / Lean Manufacturing is a philosophy focused on the reduction and elimination of waste and brings with it a positive impact for companies. However, this kind of techniques are not compatible at all with an environment like the one the mining industry has. The following study shows a model for implementing Total Productive Maintenance in a mine, based on previous studies and successful cases studies to guarantee the success for the implementation and then enhance the efficiency of mining equipment. A case study in a Peruvian mine is made to evaluate the potential of implementation of the presented model, designing each of its components to solve all the diagnosed problems in the company with the aim of increase the efficiency and operability of the company’s mining equipment. The study shows how the proposed model can help to enhance the initial conditions that the mine had and optimize their operations. An evaluation was made, using process simulation for dump trucks, and the results demonstrates a possible increase on the Overall Equipment Efficiency of the Mining equipment (+20pp vs its initial state). Also, the implementation of this kind of models / methodologies / strategies, creates a new work philosophy centered on operational excellence for the maintenance department and the rest of the company. / Trabajo de investigación

MAINTAINING MAINTENANCE : The Importance of Evaluation and Upkeep in Maintenance Management Models

This thesis explores the consequences of neglect and lack of proper support and implementation in maintenance management models, specifically an old implementation of the Pm3 model. The area of study is not widely explored, with studies and evaluation of the specific model we have looked at being almost non-existent. We have conducted a qualitative case study at an IT department/company at a Swedish university and the data has been collected through interviews with different IT department leaders, as well as one interview with a representative with the company that owns and distributes the Pm3 model. The current software development methodology at the IT company studied in this thesis are agile methods but is locked down to a micro level, while the Pm3 model handles the macro level. The consequences of this are then compared against research within fields such as IT governance and IT/business alignment to find discrepancies from which the phenomena’s can be explained. With this thesis we want to explore the consequences of not maintaining maintenance management models in organizations, especially the Pm3 model as this is widely in Swedish institutions such as universities and government authorities. This thesis provides a word of caution and argue for the importance of maintenance in maintenance management models for such organizations.

[pt] Cenários de falhas em plataformas offshore constituem riscos ambientais, sociais e econômicos significativos devido à amplitude dos danos que podem ser provocados. Portanto, neste contexto, a adoção de boas práticas de gestão de manutenção é necessária, permitindo a continuidade operacional, através da integridade de ativos. No entanto, há ainda uma necessidade de estudos com uma abordagem holística para uma avaliação multinível de ativos, através de abordagens modernas que mitiguem as limitações da análise de modos e efeitos de falha (FMEA) tradicional. Desse modo, esta pesquisa propõe um framework para a análise FMEA que considera a subjetividade e as importâncias relativas dos critérios de risco, através de uma abordagem integrada que utiliza sistemas hierárquicos de inferência e modelos multicritério, como o Best-Worst Method (BWM) e o fuzzy VIsekriterijumsko KOmpromisno Rangiranje (VIKOR) para fornecer uma classificação estruturada de ativos, orientando o gestor na priorização de manutenção. Esse framework foi validado por meio de um estudo empírico em uma plataforma do tipo Floating Production Storage and Offloading (FPSO), que abordou a problemática da corrosão externa para gestão de manutenção, com base na coleta de dados triangulados. Portanto, a presente pesquisa traz contribuições teóricas ao preencher lacunas através da propositura e aplicação de metodologias integradas, e contribuições práticas ao fornecer aos gestores, responsáveis pela formulação de planos de manutenção, elementos objetivos para uma tomada de decisão mais eficaz, sob o paradigma da confiabilidade. Esse framework pode ser aplicado em outros domínios através do ajuste de critérios avaliativos. / [en] Failure scenarios in offshore platforms pose significant environmental, social, and economic risks due to the magnitude of potential damages they can cause. Therefore, in this context, the adoption of good maintenance management practices is necessary, enabling operational continuity through asset integrity. However, there is still a need for studies with a holistic approach for a multi-level assessment of assets, using modern approaches to mitigate the limitations of traditional Failure Mode and Effects Analysis (FMEA). Thus, this research proposes a framework for FMEA that considers the subjectivity and relative importance of risk criteria, through an integrated approach using hierarchical inference systems and multi-criteria models such as the Best-Worst Method (BWM) and fuzzy VIsekriterijumsko KOmpromisno Rangiranje (VIKOR) to provide a structured classification of assets, guiding managers in maintenance prioritization. This framework was validated through an empirical study on a Floating Production Storage and Offloading (FPSO) platform, addressing the issue of external corrosion for maintenance management, based on triangulated data collection. Therefore, this research brings theoretical contributions by filling gaps through the proposal and application of integrated methodologies, and practical contributions by providing managers responsible for formulating maintenance plans with objective elements for more effective decision-making under the reliability paradigm. This framework can be applied in other domains by adjusting evaluative criteria.

Deciphering maintenance challenges through computerized maintenance management system in Ethiopian manufacturing industries

Abstracts in English and Xhosa / Given the need for prompt responses to today’s dynamic market, maintenance and maintenance management functions are becoming increasingly fundamental for industrial companies. Reduction of waste and bottlenecks, particularly the reduction of overproduction and work in process, accentuates the impact of malfunctions or failures of equipment on production. As such, it has become ever more urgent for proactive world-class maintenance to turn to computer-based support for efficacious management. To successfully manage activities such as the scheduling of activities, the planning of preventive maintenance actions, the analysis of data (to reduce the occurrence of malfunction and failures), and augment the absolute performance of the maintenance function, industrial companies implement information systems enabled by computerised maintenance management systems (CMMS) to deliver timely and accurate information. However, while various CMMS are available on the market, not all meet the exactitudes of each industrial company. The overall objective of this thesis is threefold: to explore major barriers and obstacles that have a negative impact on implementing CMMS; to identify the most common critical success factors (CSFs) that have a positive impact on implementing CMMS; and to develop an optimised CMMS model suitable to the context of Ethiopian manufacturing industries. To achieve this, an exploratory descriptive research design was employed, utilising both quantitative and qualitative data-gathering techniques, including structured interviews and questionnaires. Both qualitative and quantitative findings Suggest that the most important CSFs for CMMS implementation were work planning and scheduling and work identification and responsibilities, while the Ethiopian manufacturing firms lagged behind in the case of information technology and appraisal. Overall, key isues in maintenance management range from several maintenance techniques, information systems, scheduling, and optimisation models. This thesis is projected to be a useful source of information for both maintenance managers and stakeholders in CMMS decision making. Intelligent maintenance management in a reconfigurable manufacturing environment using multi-agent systems

Thesis (M. Tech.) -- Central University of Technology, Free State, 2010 / Traditional corrective maintenance is both costly and ineffective. In some situations it is more cost effective to replace a device than to maintain it; however it is far more likely that the cost of the device far outweighs the cost of performing routine maintenance. These device related costs coupled with the profit loss due to reduced production levels, makes this reactive maintenance approach unacceptably inefficient in many situations. Blind predictive maintenance without considering the actual physical state of the hardware is an improvement, but is still far from ideal. Simply maintaining devices on a schedule without taking into account the operational hours and workload can be a costly mistake. The inefficiencies associated with these approaches have contributed to the development of proactive maintenance strategies. These approaches take the device health state into account. For this reason, proactive maintenance strategies are inherently more efficient compared to the aforementioned traditional approaches. Predicting the health degradation of devices allows for easier anticipation of the required maintenance resources and costs. Maintenance can also be scheduled to accommodate production needs. This work represents the design and simulation of an intelligent maintenance management system that incorporates device health prognosis with maintenance schedule generation. The simulation scenario provided prognostic data to be used to schedule devices for maintenance. A production rule engine was provided with a feasible starting schedule. This schedule was then improved and the process was determined by adhering to a set of criteria. Benchmarks were conducted to show the benefit of optimising the starting schedule and the results were presented as proof. Improving on existing maintenance approaches will result in several benefits for an organisation. Eliminating the need to address unexpected failures or perform maintenance prematurely will ensure that the relevant resources are available when they are required. This will in turn reduce the expenditure related to wasted maintenance resources without compromising the health of devices or systems in the organisation.

As companhias estão inseridas em um contexto global de preocupação sustentável, onde questões ambientais e sociais acompanham as preocupações econômicas. Faz-se necessário englobar, de alguma forma, nas análises de viabilidade econômica ou ainda em modelagens de Vida Econômica e Reposição de Ativos, variáveis que possam representar aspectos sociais e ambientais. Nesse panorama entra a norma PAS-55 - Publicly Avaiable Specification - 55, publicada pela British Standards Institution, que visa aperfeiçoar o sistema de gestão de ativos físicos de maneira a permitir maiores resultados econômicos, sociais e ambientais, provenientes da utilização dos equipamentos, nas empresas. Os procedimentos previstos na norma PAS-55 sugerem o alinhamento de conhecimentos oriundos da gestão de manutenção, que compõem o arcabouço da Engenharia de Confiabilidade, a conhecimentos de Engenharia Econômica, de maneira a compor ferramentas e métodos para tomar decisões em relação aos ativos físicos de empresas. O objetivo do presente trabalho é propor um modelo de substituição de ativos que considere variáveis de Confiabilidade e Sustentabilidade, juntamente com variáveis financeiras, por meio da Análise Envoltória de Dados. Os resultados apresentam um modelo conceitual de substituição de ativos, com inclusão de variáveis socioambientais e aplicação DEA e, também, um estudo de caso com dados reais que avalia a substituição de um equipamento pertencente a uma empresa produtora de ácido fosfórico. / Companies are placed in a global context of sustainable concern where environmental and social issues accompanying economic concerns. It is necessary to include, in some way, the economic feasibility analysis or in modeling of Economic Life and Asset Replacement, variables that may pose social and environmental aspects. In this scenario enters the PAS-55 standard - Publicly Avaiable Specification - 55, published by the British Standards Institution, which aims to improve the management system of physical assets in order to allow greater economic results, social and environmental issues arising from the use of equipment, in companies. The procedures laid down in PAS-55 standard suggests the alignment of knowledge from maintenance management, which make up the framework of Reliability Engineering, the knowledge of economic engineering in order to make tools and methods to make decisions in relation to physical assets companies. The objective of this study is to propose a model of asset replacement that considers variables Reliability and Sustainability, along with financial variables, using data envelopment analysis. The results present a conceptual model of asset replacement, with the inclusion of social and environmental variables and DEA application and also a case study with real data that assesses the replacement of equipment belonging to a producer of phosphoric acid.

A Gestão da Manutenção está engajada numa nova evolução, mesmo uma revolução com novas perspectivas e questões no âmbito das empresas, assim como novos desafios a serem enfrentados por cada um de seus responsáveis. Para tal foi realizada uma análise das mudanças de comportamento das equipes e fatores prioritários que motivaram os empregados a focarem nas atividades de modo a evitar acidentes no trabalho, assim a cada dia o acidente está voltado ao erro humano. A metodologia utilizada foi baseada em uma revisão bibliográfica e na pesquisa de campo. A pesquisa desencadeou em um estudo de caso em uma Indústria do Setor de Mineração, localizada na região Sudeste, Minas Gerais. Foi baseada na coleta de dados, através da aplicação de um questionário estruturado, como principal ferramenta. Os resultados apresentam a percepção das equipes referente ao principio da evolução comportamental em segurança no trabalho. Como conclusão verificou-se que a redução de acidentes no trabalho esta voltada a um processo de mudança de comportamento, onde é possível afirmar que a principal mudança foi a adoção do cuidado genuíno, “cuidar de você, cuidar do outro e deixar que cuidem de você”. / One of the great challenges of modern times is to generate organizational structures that contribute to satisfaction, growth and human development while also contributing to the profitability and productivity of an organization. In order to remain competitive in the market, companies are naturally aware of the need to achieve and deliver satisfactory performance in relation to safety and health of its employees. This study aims to analyze the behavioral perception of structural integrity maintenance teams as a tool to prevent accidents at work. The Maintenance Management is engaged in a new development, even a revolution with new perspectives and issues in the companies, as well as new challenges to be faced by each of its officers. For such an analysis was performed of the behavior change of teams and priority factors that motivate employees to focus on activities to avoid accidents at work, and every day the accident is geared to human error. The methodology used was based on a literature review and field research. The research triggered in a case study in an Industry Mining, located in the Southeast, Minas Gerais. It was based on the collection of data through the application of a structured questionnaire as the main tool. The results show the perception of the teams regarding the principle of behavioral evolution in safety. In conclusion it was found that the reduction of accidents at work is geared to a behavior change process, where it can be said that the main change was the adoption of genuine care, "take care of you, take care of each other and leave to take care of you".

Com base numa revisão de literatura e análise documental, foi estabelecido um referencial teórico que permitiu a investigação das práticas e experiências com as políticas usualmente empregadas na gestão da manutenção. Em seguida foi discutida a proposta de um modelo conceitual de gestão da atividade manutenção, objetiva, eficiente e com qualidade, que poderia ser adotado nas organizações e escolas de nível superior, como é o caso do CEFET/RJ. Foi verificado que as principais falhas na gestão da manutenção no CEFET são devido ao atual tipo de manutenção que é corretivo na sua totalidade. Based on a literature review and documentary analysis, a theoretical framework was established that allowed the investigation of practices and experiences with the policies usually employed in maintenance management. Next, the proposal of a conceptual model for the management of the maintenance activity, objective, efficient and with quality, that could be adopted in the organizations and schools of higher level, as it is the case of CEFET / RJ was discussed. It was verified that the main failures in the management of the maintenance in the CEFET are due to the current type of maintenance that is corrective in its totality. If adopted by the Institution, the proposal outlined in this study would certainly be an important advance in the maintenance of buildings, equipment and facilities.

Como forma de se manterem competitiva no mercado, as empresas estão se conscientizando naturalmente da necessidade em alcançar e realizar desempenho satisfatório em relação à segurança e à saúde de seus empregados. O presente estudo tem como objetivo analisar a percepção comportamental das equipes de manutenção de integridade estrutural como ferramenta para evitar acidente no trabalho. A Gestão da Manutenção está engajada numa nova evolução, mesmo uma revolução com novas perspectivas e questões no âmbito das empresas, assim como novos desafios a serem enfrentados por cada um de seus responsáveis. Para tal foi realizada uma análise das mudanças de comportamento das equipes e fatores prioritários que motivaram os empregados a focarem nas atividades de modo a evitar acidentes no trabalho, assim a cada dia o acidente está voltado ao erro humano. A metodologia utilizada foi baseada em uma revisão bibliográfica e na pesquisa de campo. A pesquisa desencadeou em um estudo de caso em uma Indústria do Setor de Mineração, localizada na região Sudeste, Minas Gerais. Foi baseada na coleta de dados, através da aplicação de um questionário estruturado, como principal ferramenta. Os resultados apresentam a percepção das equipes referente ao principio da evolução comportamental em segurança no trabalho. Como conclusão verificou-se que a redução de acidentes no trabalho esta voltada a um processo de mudança de comportamento, onde é possível afirmar que a principal mudança foi a adoção do cuidado genuíno, “cuidar de você, cuidar do outro e deixar que cuidem de você”. / One of the great challenges of modern times is to generate organizational structures that contribute to satisfaction, growth and human development while also contributing to the profitability and productivity of an organization. In order to remain competitive in the market, companies are naturally aware of the need to achieve and deliver satisfactory performance in relation to safety and health of its employees. This study aims to analyze the behavioral perception of structural integrity maintenance teams as a tool to prevent accidents at work. The Maintenance Management is engaged in a new development, even a revolution with new perspectives and issues in the companies, as well as new challenges to be faced by each of its officers. For such an analysis was performed of the behavior change of teams and priority factors that motivate employees to focus on activities to avoid accidents at work, and every day the accident is geared to human error. The methodology used was based on a literature review and field research. The research triggered in a case study in an Industry Mining, located in the Southeast, Minas Gerais. It was based on the collection of data through the application of a structured questionnaire as the main tool. The results show the perception of the teams regarding the principle of behavioral evolution in safety. In conclusion it was found that the reduction of accidents at work is geared to a behavior change process, where it can be said that the main change was the adoption of genuine care, "take care of you, take care of each other and leave to take care of you".

On Risk Management of Electrical Distribution Systems and the Impact of Regulations

<p>The Swedish electricity market was de-regulated in 1996, followed by new laws and a new regulation applied to the natural monopolies of electrical distribution systems (EDS). These circumstances have motivated distribution systems operators (DSOs) to introduce more comprehensive analysis methods. The laws, the regulation and additional incentives have been investigated within this work and results from this study can be valuable when developing risk methods or other quantitative methods applied to EDS. This tendency is not unique for Sweden, the results from a comparative study of customer outage compensation laws between Sweden and UK is for example included.</p><p>As a part of investigating these incentives, studies of the Swedish regulation of customer network tariffs have been performed which provide valuable learning when developing regulation models in different countries. The Swedish regulatory model, referred to as the Network Performance Assessment Model (NPAM), was created for one of the first de-regulated electricity markets in the world and has a unique and novel approach. For the first time, an overall presentation of the NPAM has been performed including description of the underlying theory as a part of this work. However, the model has been met by difficulties and the future usage of the model is uncertain. Furthermore, the robustness of the NPAM has been evaluated in two studies with the main conclusion that the NPAM is sensitive toward small variations in input data. Results from these studies are explained theoretically investigating algorithms of the NPAM.</p><p>A pre-study of a project on developing international test systems is presented and this ongoing project aims to be a useful input when developing risk methods. An application study is included with the approach to systematically describe the overall risk management process at a DSO including an evaluation and ideas of future developments. The main objective is to support DSOs in the development of risk management, and to give academic reference material to utilize industry experience. An idea of a risk management classification has been concluded from this application study. The study provides an input to the final objective of a quantitative risk method.</p>
